Abstract:Objective To apply a surgical approach of minimal invasive closure to atrial septal defect(ASD)without cardiopulmonary bypass.Methods Between Sept.2004 and Apr.2007,46 cases(male 22,female 24)of atrial septal defeet(ASD)were treated by minimal invasive closure without cardiopulmonary bypass.Their age ranged from 8mon tO 16yr(mean 4.7±3.9 yr)and weight ranged from 8.5 to 35 kg.(mean 1 5.2±6.5 kg).The incision was through the right 4th intercostals space after general anesthesia,and the character and size of ASD were confirmed via the transesophageal eehocardiography(TEE).The occluder device suitable to the ASD was placed through the right atrium and released after making sure it's firmly in place of the defect.Results Among them.45 cases of ostium secundum ASD were closed successfully and 1 failure.The size of ASD was from 8 mm to 25 mm,and the shortest distance of the rim of ASD was 7mm to the mitral valve,8 mm to the top of left atrium,6 mm to superior vena cava and 6 mm to inferior vena cava.There were no residual shunts and operative complications.Conclusions The advantages of minimally invasive closure of ASD without cardiopulmonary bypass are less traumas,no radiation,easy and security of manipulation.It is valuable for clinical application.
Transcatheter Closure of Secundum Atrial Septal Defect with Amplatzer Occluder Device

Abstract:Objective To evaluate the efficiency of transcatheter closure of secundum atrial septal defect (ASD) with Amplatzer occluder device.Methods Twenty-one patients with ASD was treated using Amplatzer occluder device through the percutaneous procedure under fluoroscopy and transesophageal echocardiography (TEE) guidance.Results The mean diameter of ASD was 20.5 ± 7.1mm (ranged from 6 to 32mm)shown by TEE before the procedure,The mean balloon stretched diameter of the ASD was 23.3±7.7mm (ranging from 9 to 34 mm).The mean diameter of the Amplatzer occluder selected was 25.9±8.1mm (ranging from 10 to 38 mm).For two cases with double ASD,Two occluder device of 22 and 10 mm in diameter were used in one patient,a larger occluder device of 28 mm in diameter was used for closing two defects in the other patient with good result.All patients were treat successfully.The success rate of placement of the Amplatzer occluder was 100%.No complications occurred.All of 21 patients was no residual shunt found during 1～18 months follow-up.Conclusion Transcatheter closure of secundum ASD using Amplatzer septal occluder device was an efficient and safe nonsurgical method.

Abstract:Aims：This study aimed to investigate the safety, feasibility and availability of perimembranous ventricular septal defect (PmVSD) closure via a left parasternal ultra-minimal trans intercostal incision in children. Methods and results：From January 2015 to January 2019, 131 children with restrictive PmVSDs were enrolled in this study and successfully done in 126 patients (96.18%). PmVSDs were occluded via an ultra-minimal trans intercostal incision (≤1 cm), and the entire occlusive process was guided and monitored by TEE. A pericardium hanging technique was employed without sternal incision. PmVSDs were closed through a short delivery sheath assembled using a concentric occluder device. All patients were followed up for a perid ranging from18 months to 24 months. Thirteen patients with PmVSD had aneurysm of membranous septum (AMS). Multistream (more than or equal to 2) PmVSDs with AMS were found in eleven cases. After the operation, mild residual shunt beside the amplatzer occluder in one patient was found and had self-healing result during the 5-month follow-up period. Five patients transferred to ventricular septal defect repair operation under direct visualization with a cardiopulmonary bypass. One reason was ventricular fibrillation when guide wire passed the PmVSD, another was device dislocation, and others were the guide wire cannot pass through the PmVSD. Conclusions：PmVSDs closure using a concentric occluder via a left parasternal ultra-minimal trans intercostal incision under TEE guidance is feasible, safe, and effective in children. This approach can be considered as an alternative treatment to open-heart surgery for restrictive PmVSDs.

Abstract:Objective This study was aimed to discuss the safety, feasibility and availability of perimembranous ventricular septal defects(PmVSD) closure via super minimal intercostal incision under transesophageal echocardiography(TEE) guidance in children.Methods There were 81 cases of PmVSD via super minimally invasive transintercostal device closure operation(length of incision ≤ 1cm) from August 2014 to August 2016.TEE was used to guide and monitor the entire procedure.Assessed the effectiveness of device closure by postoperative regular follow-up.Results 80 patients were successfully operated by super minimally invasive transintercostal device closure in all 81 cases.Operation success rate reached 98.77%.The average diameter of ventricular septal defects was(3.72±0.96)mm.The average diameter of amplatzer occluder was(4.88±0.95)mm.Postoperative follow-up time was from 6 months to 24 months.One of 16 cases that with aneurysm of membranous septum had more than two shunts, remained a mild residual shunt beside of the amplatzer occluder but self cured during follow-up.Mild pericardial effusion was found in one patients after the operation and disappeared during follow-up.There were no cases of perioperative death, device deformation or displacement, residual shunt, complete atrioventricular block and valve involvement in the patients whom were successfully operated.One patient transferred to ventricular septal defect repair operation under direct visualization with a cardiopulmonary bypass, because ventricular fibrillation happened when guide wire passed the ventricular septal.Conclusion Super minimally invasive transintercostal device closure of PmVSD in children guided by TEE was safety and availability, that could avoid x-ray radiation and sternotomy, and operate simply, with small incision and low complication.

Abstract:INTRODUCTION: Anterior mini-thoracotomy approach is a good alternative to median sternotomy in Atrial Septal Defect (ASD) repair. Our purpose is to explain the details of our technique and peresent the results.METHODS: Seventy five patients with ASD (52 female and 23 male) were operated with anterior mini-thoracotomy approach in our tertiary center between March 2012 and March 2014. The mean age was 14±10 ranged from 2 to 42 years. Outcomes were defined according to cardiopulmonary and aortic cross-clump time, intensive care unit stay time, morbidity, mortality, the size of incision, the amount of post-operative bleeding, need for blood transfusion and reoperation.RESULTS: Mean Cardiopulmonary bypass time was 49.62 minutes (26 to 105 minutes) and mean aortic cross clamp time was 22.29±6.77 minutes (11 to 47 minutes). The mean amount of blood transfusion was 47.49± 62.22 mm (0 to 200 cc) and the mean chest tube drainage after surgery was 80.17 ±121.06 mm (0 to 600 cc). One patient re-operated for dehiscence of ASD surgical sutures and there was no reoperation for surgical bleeding or tamponade drainage in these patients. In 74 cases the defect was secundum type, in 2 patients it was sinus venosus type and in one with associated partial Anomalous repair.CONCLUSION: Anterior thoracotomy approach is safe and may be the surgical technique of choice for secundum ASD repair in all age groups and we can utilize this technique also for more complicated kinds of surgery for instance, sinus venosus type ASD with or without Partial Anomalous Defect.

Abstract:This case report describes an 82-year-old woman with severe mitral bioprosthesis stenosis and NYHA II-III heart failure, EuroScore II of 22.54% and STS Score of 13.7%. She received innovative treatment with transatrial mitral valve-in-valve implantation of a rapid deployment balloon-expandable prosthesis via video-assisted right lateral mini-thoracotomy, with myocardial protection through ventricular fibrillation. The patient was discharged and, under cardiology follow-up, achieved NYHA I class with no angina.

Abstract:Percutaneous closure has emerged as the standard treatment for secundum-type atrial septal defects (ASDs). However, there is a rare but serious complication of occluder device migration and embolization to the heart chambers or distal vasculature during or shortly after implantation. Although this occurrence is extremely rare, it can have disastrous consequences. Fortunately, advancements in equipment and technology have facilitated the transition from surgical procedures to percutaneous techniques for removing embolized occluder devices. In this report, we present a case in which an Amplatzer septal occluder (ASO) device embolized to the descending thoracic aortic isthmus two days after implantation. The device was successfully retrieved using a percutaneous technique, and another ASO device was subsequently redeployed to the ASD. Regrettably, the patient experienced an intraoperative cardiac arrest. Despite prompt rescue efforts and recovery of vital signs, the patient still suffered postoperative sequelae. The main reason for occluder device migration in this case may have been the undersizing of the ASO device due to the operator's lack of caution.

Real-time 3D transoesophageal echocardiography for guiding Amplatzer septal occluder device deployment in an adult patient with atrial septal defect

Abstract:A 32-year-old male with secundum atrial septal defect underwent percutaneous transcatheter occlusion. The procedure was performed under general anaesthesia and guided by real-time 3D transoesophageal echocardiography (TEE) (i.e. 33, matrix probe 2-7 MHz). The device delivery system was advanced through a femoral vein catheter and placed in the left upper pulmonary vein. A 3D left atrial view was instantaneously obtained. The size of the septal occluder was chosen according to the balloon catheter method. A 12 mm Amplatzer device was deployed under real-time 3D TEE without fluoroscopy. The 3D views depicted the progression of the left-side disc from the left atrial cavity towards the septum. Left and right atrial views demonstrated the perfect application of the device to the atrial septum. Keeping a steady traction on the delivery rod, the right-side disc of the device was then deployed on the right atrial side of the defect. A pushing and pulling of the delivery cable ensured that the device was in a secure and stable position. Absence of peri prosthetic residual shunt was assessed by 2D colour Doppler TEE before release. Transthoracic echocardiography performed the day after ensured the right position of the device. Real-time 3D TEE is a very recent technology allowing on-line guidance of atrial septal occluder device deployment; such imaging should increase delivery safety and decrease fluoroscopy time.
Follow Up of Medium-Term or Prostecdtive Efficacy with Transcatheter Closure of Secundum Type Atrial Septal Defects Using the Homemade Amplatzer Septal Occluder in Middle-Aged or Geratic Patients

Abstract:【Objective】 To evaluate the safety and medium-term or prostecdtive efficacy of the transcatheter occlusion of secundum type atrial septal defect(ASD) using the homemade Amplatzer septal occluder in middle-aged or geratic patients.【Methods】 118 cases(28 males,90 females) aged from 40 to 79 years old(mean age(52.7±9.2) years) with ASD which maximal dimensions from 6 to 38(average(23±4) mm) were confirmed by clinical manifestation,electrocardiogram(ECG),X-ray fluoroscopy,transthoracic color Doppler echocardiography(TTE) from January 2007 through May 2011.According to the edge conditions,ASD were divided into central types(42 cases) and edge-insufficiency types(76 cases).Edge-insufficiency ASD was in-cluded in four types: aortic edge-insufficiency(65 cases),posterior(aortic contralateral edge) edge-insufficiency(2 cases),arium-roof edge-insufficiency(4 cases),mitral valve edge-insufficiency(5 cases).In addition there were consists of 114 single aperture and 4 multiaperture ASD.All of them were received the transcatheter occlusion successfully.Afteroperation all cases received ECG,X-ray fluoroscopy and TTE after 1,3,6,12 months and interval of one year.The shortest follow-up time was 6 months and the longest 4 years.【Results】 118 cases were all occluded successfully,successful rate of 100%.The sizes of homemade Amplatzer septal occluder who using were 10 ～42(27.2 ±7.1) mm.In follow-up period,X-ray fluoroscopy was showed pulmonary venous pleonaemia reduced and cardio-thoracic ration decreased.After 1 month and 3 months all cases with enlargement of internal diameter of atrium dextrum and right ventricular,step-up of pulmonary arterial systolic pressure were found reduced obviously(P 0.05),and more after 6 months(118 cases),1 year(101 cases),2 years(67 cases),3 years(36 cases),4 years(14 cases)(P 0.01).All of them were alived and there was no significant complication in all follow-up period.None of them then underwent therapy again.【Conclusions】 It is safety and good medium-term or prostecdtive efficacy that transcatheter closure of ASD using the homemade Amplatzer septal occluder in middle-aged or geratic patients,and there are advantage of easy procedure,less complication,high successful rate and short length of stay.

Abstract:BACKGROUND Congenital heart diseases (CHDs) are the most common form of birth defects, affecting the structure and function of neonatal hearts. Pulmonary valve stenosis (PVS) and ventricular septal defects (VSD) are 2 of the more prevalent forms, both of which can lead to significant morbidity if left untreated. The emergence of transcatheter techniques has revolutionized the therapeutic landscape, presenting minimally invasive yet effective alternatives to open-heart surgery and significantly reducing associated patient morbidity and recovery time. CASE REPORT The presented case details the management of a 19-year-old man with complex CHDs, highlighting the nuanced decision-making process that led to a transcatheter approach. The patient's clinical presentation, marked by symptoms reflective of significant cardiac compromise, demanded a tailored approach that utilized the latest advancements in non-surgical intervention. The successful closure of the VSD with an Amplatzer device and the resolution of PVS via balloon valvuloplasty were achieved without complications, showcasing the potential of these techniques in managing similar cases. The post-intervention period was marked by a noteworthy recovery, confirming the procedural efficacy and enhancing the patient's quality of life. CONCLUSIONS The favorable outcome of this case highlights the pivotal role of transcatheter interventions in treating complex CHDs and suggests a shift towards less invasive approaches in cardiac care. This case contributes valuable insights to the existing body of evidence, reinforcing the potential of transcatheter techniques to become the preferred treatment modality. With promising immediate and short-term results, these techniques highlight the need for continued research into their long-term efficacy and application across diverse patient demographics.
Role of Echocardiography in Minimally Invasive Operation Closure of Great Atrial Septal Defect

Abstract:Objective To assess the application value of echocardiography in evaluating minimally invasive Amplatzer closure of great secundum atrial septal defect (ASD) patients. Methods Two hundred and fifty two patients (male, 116) were involved in this study. The defect diameter was 34～38 mm in 230 cases, 22cases were atrial septal aneurysm complicating porosity defects. Each patient was positioned a Amplatzer device to close the defect through parasternal minimized surgical incisions. TEE were used to monitoring the whole procedure, to guide the device placing and evaluate the curative effect instantly after operation. Results All patients were successfully positioned closure devices by echocardiography monitoring. Only 34 cases had micro to small amounts residual shunt instantly after operation,11 cases still had small amounts residual shunt at the time of 48 hours after the operation. In the one year follow-up,6 cases had residual shunt, but the diameters of right chamber heart were significantly reduced than preoperative,and the pulmonary artery systolic pressure were also reduced. Conclusions Echocardiography possesses important role of preoperative patients screening,monitoring the devices deploying intraoperation, and evaluating the curative effect postoperative. Abstract:ObjectivesTo evaluate the safety, efficiency, and midterm outcomes of transcatheter perimembranous ventricular septal defect (pmVSD) closure using the new KONAR‐MF™ VSD occluder. BackgroundOff‐label percutaneous pmVSD closure is a well‐established procedure with promising results. Yet, interventionists are still searching for the ideal device. MethodsBetween June 2018 and March 2019, 20 patients with hemodynamically significant but restrictive‐type pmVSD underwent an attempted transcatheter closure using the new KONAR‐MF™. All implantations were performed retrogradely under general anesthesia, transoesophageal echocardiography, and fluoroscopic guidance. Prospective follow‐up using transthoracic echocardiography and electrocardiogram was done until August 2019. ResultsThe median age was 6.4 years (8 months to 43.4 years), and the median body weight was 17.3 (9–74) kg. The mean defect size on the left ventricular aspect was 11.7 ± 2.8 mm. All devices were successfully and rapidly implanted. One device embolized in the pulmonary artery, 24 hr after implantation and was percutaneously retrieved with no complication. Over a mean follow‐up period of 8.2 ± 3.0 months, complete closure was achieved in 84.2% of patients. One new onset of mild aortic regurgitation was detected before discharge and remained stable. Tricuspid valve regurgitation, complete heart block, major complication, and death were not observed. ConclusionsTranscatheter pmVSD closure using the KONAR‐MF™ can be successfully performed in adult and pediatric patients. It is a safe and promising device, designed to provide high conformability to septal defects with a lower risk of heart block. Retrograde implantation allows procedural flexibility, efficient deliverability, and control of valvular interferences.

Transcatheter Closure of Secundum Atrial Septal Defects Using Amplatzer Device

Abstract:Objective To evaluate the feasibility and short term results of transcatheter closure of secundum atrial septal defects (TCSASDs) using Amplatzer occluder device. A new self-centering device, the Amplatzer atrial septal occluder, has recently been evaluated in an animal model with excellent results. This paper reports our clinical research with this device.Methods 30 patients underwent an attempt at TCSASDs at an average age of 35.6 +/- 15.9 y (range, 5 - 62 y) and average weight of 63.6 +/- 14.5 kg (range, 17 - 78 kg) using the Amplatzer atrial septal occluder. The average ASD diameter measured by transesophageal echocardiography (TEE) was 23.6 +/- 4.0 mm (range, 16 - 31 mm), and the average atrial septal defects (ASD) balloon stretched diameter was 25.2 +/- 5.8 mm (range, 9 - 34 mm). All patients had right atrial and ventricular volume overload with a Qp/Qs of 3.4 +/- 1.2 (1.4 - 5.2). A F8-12 catheter was used for delivery of the device in all patients.Results The device was placed correctly in all patients. There was immediate and complete closure in 30 patients. The average device waist size used to close the defect was 25.6 +/- 5.9 mm (range, 9 - 34 mm). There was no episode of device embolization or any other complication. Follow-up was performed using transthoracic echocardiography (TTE) 1 day, 1 month, 3 months, 6 months and 1 year after the procedure. The first day after operation; there was complete closure of the ASD in all patients; there have been no episodes of endocarditis, thromboembolism, or wire fracture.Conclusion TCSASDs using the Amplatzer occluder device was an efficient nonsurgical method which had a very high technical success rate and satisfactoy short term and follow-up results.
Percutaneous closure of secundum atrial septal defect with a new self centering device ("angel wings")

Abstract:Objective: To investigate the safety, efficacy, and clinical application of a new self centering device ("angel wings") for closure of secundum atrial septal defects (ASD II) and persistent foramen ovale in all age groups. Design: Multicentre, prospective, nonrandomised study. Inclusion criteria: defects with an occlusive diameter of < or = 20 mm and a surrounding rim of > 4 mm; body weight > 10 kg; and an indication for surgical closure of secundum atrial septal defect. Additionally, there were compassionate indications for closure in patients with persistent foramen ovale. Interventions: Defects were closed by a transcatheter device consisting of two square frames made of superelastic nitinol wire. The frames are covered by elastic polyester fabric, which is sewn together at a central circle. All procedures, except for three interventions that were carried out under sedation, were performed under general anaesthesia using transoesophageal echocardiography and fluoroscopy to monitor intervention. Results: Closure was attempted in 75 (71%) of 105 patients. An ASD II was present in 35 children and 15 adults. A persistent foramen ovale was present in 25 adults with suspected paradoxical embolism. Transcatheter closure was unsuccessful in three children and crossover to surgery was required. Residual shunts were found in 20 patients (27%) immediately after the procedure. A transient atrioventricular third degree block occurred in three patients (4%) and the right atrial disk was not fully deployed in three. A minor shunt (< 3 mm) was present in only three (4%) of 72 patients during follow up of 1-17 months. Blood clots on the right atrial disks in two patients (one required lysis) were seen during follow up transoesophageal echocardiography. Serious complications demanding surgical removal of the device occurred in three patients. One patient had haemopericardial tamponade because of an aortic lesion. Left atrial thrombus formation due to an unfolded right atrial disk was found in a second patient and dislodgement of the left atrial disk resulted in a large residual shunt in a third. Conclusions: Percutaneous closure of a central ASD with a diameter < or = 20 mm in paediatric and adult patients is feasible and effective with this new device. It is a promising alternative to surgical closure. Modifications of the design, however, seem to be mandatory as 4% of patients developed serious complications.

Abstract:Six months after undergoing a Fontan operation, a 7-year-old boy with right atrial isomerism and a single functional ventricle was admitted to our emergency department with cyanosis. Emergency cardiac catheterization revealed a large veno-venous fistula that began in a left hepatic vein, connected to the left accessory hepatic veins, and drained into the common atrium, resulting in desaturation. The fistula was occluded proximally with an Amplatzer septal occluder, with satisfying results; the patient's systemic arterial saturation decreased during his hospital stay. Three weeks after the first intervention, a second procedure was performed to retrieve the first device and to close the fistula distally. Multiple attempts with different types of gooseneck snares and a bioptome catheter failed to retrieve the first device, so a telescopic method was used to re-screw it. Using a Mullins long sheath and delivery sheath, the delivery cable was manipulated to fit into the slot of the end screw, and the cable was rotated gently in a clockwise direction to re-screw the device. Then, another Amplatzer septal occluder was placed at the distal end of the fistula. In conclusion, distal transcatheter occlusion of intrahepatic veno-venous fistulas might lead to better clinical outcomes in selected patients. Amplatzer septal occluder device can be retrieve without any complication within three weeks.

Abstract:OBJECTIVE: To show that transcatheter closure of perimembranous ventricular septal defect (PMVSD) with the Amplatzer ductal occluder (ADO, AGA Medical Corp, Plymouth, Minnesota) is a safe and effective treatment option.BACKGROUND: Transcatheter closure of PMVSD is a challenging procedure. Recently, the Amplatzer PMVSD occluder (APMVSDO, AGA Medical Corp, Plymouth, Minnesota) has been shown to be effective in closing hemodynamically significant PMVSDs. However, the high incidence of complete atrioventricular block (CAVB) after device occlusion of a PMVSD has been a hot issue as well.METHODS: Among several devices used in closing PMVSD percutaneously, we prefer the ADO because of the anatomic resemblance between PMVSD with aneurysm and patent ductus arteriosus, and it has no right ventricular disc which may contribute to the occurrence of CAVB.RESULTS: Between August 2009 and May 2012, 21 patients (5 males and 16 females) underwent percutaneous PMVSD closure using ADO. The patients' ages ranged from 3 to 42 years (median: 7 years), and their weights ranged from 18 kg to 60 kg (median: 27 kg). All patients showed echocardiographic signs of left ventricular volume overload and trivial to small mitral regurgitation (Qp/Qs = 1.7 ± 0.4). The mean defect size of the right ventricular side was 4.3 ± 1.0 mm. Devices 2 mm larger than the measured narrowest VSD diameter were selected in most patients. The ADOs were successfully implanted in all patients without any significant complications except one transient CAVB, one case of delivery wire fracture, and one case of surgery due to significant residual leak. Small residual shunts were observed immediately after the device implantation, but they disappeared during follow-up for 18 of 20 patients. The mean follow-up period was 20 ± 9 months, and CAVB or aortic regurgitation was not observed in all patients.CONCLUSION: Transcatheter closure of PMVSD with the ADO is a safe and promising treatment option, but long-term follow-up in a large number of patients would be warranted.

Feasibility of robotic-assisted atrial septal defect repair in a 6-year-old patient

Abstract:Background: The feasibility, safety and advantages of minimally invasive or robotic repair of atrial septal defect (ASD) in adults were reported previously. However, there is limited data for the application of these systems in paediatric patients. Although current robotic systems still have large instruments for surgical repair in children, some appropriate patients may benefit from this technology. Method: A 6-year-old child with ASD underwent robotic assistant repair via Da-Vinci Robotic Systems. Venous cannulation was achieved by internal jugular and femoral veins (10F-14F) and arterial cannulation was performed via femoral artery under transesophageal echocardiography (TEE) guidance (10F). A 3 cm incision was made in the right fourth intercostal space, used for working and the camera port in the same time. The ports were placed considering not to damage the potential developing breast tissue. After the port implantation (8F) and cardiac arrest, the ASD repair was completed with primary closure technique. Result: The perioperative period was uneventful and the patient was discharged from hospital 5 days after surgery. Conclusion: ASD closure with robotic assistant was achieved in a large enough sized paediatric patient. With the development of thinner and shorter robotic arms, it will be possible to use robotic assistance more common during the repair of congenital heart diseases.

Abstract:This research article introduces an innovative Atrial Septal Defect (ASD) Occluder tailored for treating atrial septal defects. Crafted from nitinol wire, the occluder incorporates strategically placed PET fabric in its design, enhancing its thrombogenicity and occlusion efficacy. The primary objective of this occluder is to halt blood flow between the atria, thus correcting the defect's impact on cardiac function. Through rigorous in-vitro testing, the occluder's deployment via catheter and its performance and efficacy are comprehensively evaluated. The study focuses on assessing its ability to effectively obstruct blood flow across the defect site while minimizing the risk of adverse effects or complications. Abstract:Background: Surgery is typically used to correct challenging ventricular septal defects (VSDs), such as VSD with pulmonary hypertension and multiple defects. In this case report, we would like to highlight the feasibility of multiple defects VSD closure with single device percutaneously using zero-fluoroscopy technique. Case presentation: A 7-year-old child was referred with the main symptom of shortness of breath. She started experiencing repeated respiratory tract infections, feeding issues, and failure to thrive at the age of six months. Her body weight was only 18 kg. TEE revealed several muscular VSD with 2-3 mm and 12 mm diameters, 3 mm spacing between VSD, L to R shunt, AR (-), and TR mild with septal leaflet tricuspid prolapse. Following right heart catheterization (Qp:Qs 3.5, PVRi 5.23WUmsq, PVR 4.55 WU, PVR/SVR 0.16), we made the decision to correct the defect using an Amplatzer Septal Occluder (AGA) No. 16 mm using transjugular method. Full device deployment was successfully performed with several episodes of PVC storm and severe bradycardia. One and a half years after the procedure, her TVG dropped to only 18 mmHg, her visible indicators of PH subsided, and the PA dilator treatment was discontinued. Her body weight had increased to 28 kg, and she had no complaints. Conclusions: Our experience demonstrated that percutaneous closure of multiple VSD with a single device is possible, even with pulmonary hypertension.
