Abstract:Although a diagnosis of a life‐limiting cancer is likely to evoke emotions, such as fear, panic and anxiety, for some people it can also provide an opportunity to live life differently. This article is based on research undertaken in Aotearoa New Zealand on the topic of exceptional cancer trajectories. Eighty‐one participants who had been identified as living with a cancer diagnosis longer than clinically expected were interviewed, along with 25 people identified by some of the participants as supporters in their journey. For some participants the diagnosis provided the opportunity to rethink their lives, to undertake lifestyle and consumption changes, to be culturally adventurous, to take up new skills, to quit work and to change relationships with others. The concepts of biographical disruption and posttraumatic growth are considered in relation to these accounts, and it is argued that the event of a cancer diagnosis can give license for people to breach social norms.

Abstract:Purpose: Novels and autopathographies that employ cancer as a central theme offer a wealth of opportunities for researching the way patients with cancer make sense of their illness and its treatment. Such literatures can also inform clinical care, because they can support patients in living with their illness. The use of novels and autopathographies for research and care in persons with cancer fits within the framework of 'Health Humanities', the interdisciplinary field where medicine and social science meet. This paper presents a concise overview of novels and autopathographies that explore cancer as their theme. Methods: Literature searches were conducted using PubMed, major scientific journals of medicine and clinical oncology, and databases in the Health Humanities. Searches focused on novels and autopathographies where cancer is the central theme, which are available in English, and which can be considered to represent 'high literature'. Results: Twenty-nine books were identified. The majority of the books were written originally in English, and breast cancer and lung cancer were the most frequently discussed types of cancer. The core themes identified were giving meaning to illness; coping with medical treatment; and the psychological and social consequences of illness. Conclusion: Novels and autopathographies about cancer represent an innovative base for research on living with cancer and offer rich data on how people make sense of cancer and its medical treatment. Clinical implications of this review pertain to interventions based on bibliotherapy and expressive writing. Novels and autopathographies are just part of the Health Humanities context: a wide range of art genres may prove helpful in improving the quality of life of persons with cancer.

Abstract:Background The biology of peripartum depression remains unclear, with altered stress and the Hypothalamus-Pituitary-Adrenal axis response having been implicated in its pathophysiology. Methods The current study was undertaken as a part of the BASIC project (Biology, Affect, Stress, Imaging, Cognition), a population-based longitudinal study of psychological wellbeing during pregnancy and the postpartum period in Uppsala County, Sweden, in order to assess the association between evening salivary cortisol levels and depressive symptoms in the peripartum period. Three hundred and sixty-five pregnant women from the BASIC cohort were recruited at pregnancy week 18 and instructed to complete a Swedish validated version of the Edinburgh Postnatal Depression Scale at the 36th week of pregnancy as well as the sixth week after delivery. At both times, they were also asked to provide evening salivary samples for cortisol analysis. A comprehensive review of the relevant literature is also provided. Results Women with postpartum EPDS score ≥ 10 had higher salivary evening cortisol at six weeks postpartum compared to healthy controls (median cortisol 1.19 vs 0.89 nmol/L). A logistic regression model showed a positive association between cortisol levels and depressive symptoms postpartum (OR = 4.1; 95% CI 1.7–9.7). This association remained significant even after controlling for history of depression, use of tobacco, partner support, breastfeeding, stressful life events, and sleep problems, as possible confounders (aOR = 4.5; 95% CI 1.5–14.1). Additionally, women with postpartum depressive symptoms had higher postpartum cortisol levels compared to both women with depressive symptoms antenatally and controls (p = 0.019 and p = 0.004, respectively). Conclusions Women with depressive symptoms postpartum had higher postpartum cortisol levels, indicating an altered response of the HPA-axis in postpartum depression.

Abstract:Everyone understands the tragedy of cancer; however, the severity of its effects are only known to those who have first-hand experience. Thus, not everyone understands how it feels to receive a diagnosis yourself or to support a loved one, or how colleagues and peers react at work and school. It begs the question: why are people who are dealing with a cancer diagnosis often still keeping it a secret? I have spoken with many patients who were diagnosed with cancer during their childhood or as a teenager or young adult; in this scenario, the parents are responsible for bringing their children to a check-up in the hospital or their health practitioner, which also comes with a heavy emotional burden. A patient who recently finished her chemotherapy told me that she did not want to talk with high school friends about her diagnosis and that she would select carefully when and to whom she would open up about her cancer. People would react differently, ranging from overly friendly to avoiding the topic at all, meaning that it quickly became clear who her true friends were and who she could trust. Now that she has finished treatment, she still does not necessarily bring up the topic with new friends on her college campus. However, speaking with someone who is aware of how difficult life can be after cancer can enable her to understand her circumstances and support her during her studies, as college life can be hard on anyone, even those in perfect health.

Abstract:This article presents a case study of one family affected by a cancer diagnosis in both the father and the daughter, who were diagnosed within the same time interval and who underwent treatment at the same time. The article examines the relationship between the caregivers and the oncology patient as well as with one another when the stress of diagnosis is compounded by multiple, simultaneous, and similar diagnoses in a highly condensed period of time. A thorough examination of the literature reveals that there are significant gaps regarding how multiple cancer diagnoses in one family affect the family dynamic, individual and collective coping styles, and caregiver burden. The diagnoses can also dramatically exacerbate economic stressors in a family. The coordination of psychosocial care from the perspectives of the adult and pediatric oncology social workers at an urban academic medical center will be discussed. The social work role, importance of collaboration, and family centered care perspective will be discussed as a method of easing the treatment experience for families in psychosocial distress.

Primary reninism, Liddle's syndrome and neurovascular contact has been presented in the article. The pathogenesis, clinical symptoms and treatment of these rare forms of hypertension is discussed in this article.

Abstract:This particular issue of our journal featuring author contributions related to cancer speaks to me loudly. Last year, on my birthday no less, I got a recall letter from my annual mammogram. We can all agree it was not a great way to mark the occasion. After more imaging a biopsy was required. Luckily the biopsy was benign but still suspicious so based on several factors I am now in a high risk category for breast cancer and require screenings every six months. Last month a second mass was found. I feel understood by Botto et al. as they noted that stress is highest in the follow up stage and ever present during a reoccurrence. While my first mass was not detectable from a self-exam, my second mass was underscoring the importance of the work of Gül and Büyükbayram in doing all possible to increase awareness and improving detection. Karisani et al. further share the important work of screening for cervical cancers. While I have not had to make any surgery decisions yet, I am glad to read the work of Koksalmis and colleagues as their decision-making instrument requires physicians to take into account patient factors. While I cannot say that I would one day opt for conservation over mastectomy, I know that a decision so personal must be made with an individual's characteristics in mind.

Abstract:How do metaphors shape and reflect patients' experience of cancer? How can repeated immersion in challenging poetry affect our understanding of cancer and other diseases? This essay addresses these two questions through an exploration of the varied and sometimes contradictory metaphors crafted by poet and seven-time cancer patient Judy Rowe Michaels. Although cancer is often regarded as an enemy intrusion to be eradicated, it is an illness in which the threat to life comes from within the self. Michaels's metaphors complicate the relationship between an embodied self and the cancerous cells the body produces, presenting multifarious responses to living with cancer. Defying expectations of a linear trajectory from symptom to diagnosis to treatment to recovery, these metaphors provide a means to understand the changing emotions; the changing relationship to illness, to wellness, and to self; and the changing status as patient (symptomatic, diagnosed, in treatment, in remission, in recurrence) that characterize the lived experience of cancer-and of other recurring or chronic diseases. Examining these poems demonstrates the value for practitioners, patients, and families of honing a poet's mode of understanding, as a vital complement to the mode of thinking that currently drives health care.

Abstract:Purpose: This study investigated how cancer diagnosis and treatment lead to career disruption and, consequently, loss of income and depletion of savings. Design: This study followed a qualitative descriptive design that allowed us to understand the characteristics and trends of the participants. Method: Patients recruited (n = 20) for this study were part of the University of Kansas Cancer Center patient advocacy research group (Patient and Investigator Voices Organizing Together). The inclusion criteria were that participants must be cancer survivors or co-survivors, be aged 18 years or older, be either employed or a student at the time of cancer diagnosis, have completed their cancer treatment, and be in remission. The responses were transcribed and coded inductively to identify themes. A thematic network was constructed based on those themes, allowing us to explore and describe the intricacies of the various themes and their impacts. Results: Most patients had to quit their jobs or take extended absences from work to handle treatment challenges. Patients employed by the same employer for longer durations had the most flexibility to balance their time between cancer treatment and work. Essential, actionable items suggested by the cancer survivors included disseminating information about coping with financial burdens and ensuring that a nurse and financial navigator were assigned to every cancer patient. Conclusions: Career disruption is common among cancer patients, and the financial burden due to their career trajectory is irreparable. The financial burden is more prominent in younger cancer patients and creates a cascading effect that financially affects close family members.

Abstract:In January 2023, diagnosed with numerous metastases of lung cancer in my brain, I felt that I must accomplish a mission. If everything happens for a reason, my cancer, in particular, I must find out how metastatic cancer can be treated with curative intent. This is my mission now, and the reason I was ever born. In January 2023, I understood the meaning of life, of my life. I was born to write this article. In this article, I argue that monotherapy with targeted drugs, even when used in sequence, cannot cure metastatic cancer. However, preemptive combinations of targeted drugs may, in theory, cure incurable cancer. Also, I share insights on various topics, including rapamycin, an anti-aging drug that can delay but not prevent cancer, through my personal journey.

Abstract:The world of cancer science is moving toward a paradigm shift in making connections with neuroscience. After decades of research on genetic instability and mutations or on the tumor microenvironment, emerging evidence suggests that a malignant tumor is able to hijack and use the brain and its network of peripheral and central neurons as disrupters of homeostasis in the body. Whole-body homeostasis requires brain–body circuits to maintain survival and health via the processes of interoception, immunoception, and nociception. It is now likely that cancer disturbs physiological brain–body communication in making bidirectional brain tumor connections.

Abstract:Purpose. This study aimed to study the challenges and ways of coping with living with chronic lymphocytic leukemia (CLL) before the initiation of treatment. Methods. Semistructured interviews were carried out with 8 people living with CLL (4 males and 4 females) who had never received any treatment. Interpretative phenomenological analysis was utilized for the analysis of the data. Results. The following three themes were developed: (1) “Still waters run deep” highlights the contrast between living with minimal symptoms while experiencing high anxiety for the future, (2) “Surviving uncertainty” portrays participants’ supportive networks, communication challenges, and internal coping mechanisms to face the threatening overtones of CLL, and (3) “Turning over a new leaf” delineates participants’ realization of life’s finiteness and the way this acts as a nudge for psychological growth. Conclusion. Despite the limited physical discomfort, the CLL diagnosis and the watch-and-wait phase bring about psychological distress, which drives meaning-making efforts and an array of coping mechanisms, potentially leading to posttraumatic growth processes for people living with CLL.

Being a family member is described as strenuous and, often, associated with stress, anxiety and feelings of loneliness. There is a heightened risk of distress for family of those with fast-progressing, severe oesophageal or gastric cancer. Early palliative care involving family is vital yet often overlooked. In order to include family members in early palliative care their management in everyday life needs to be explored.

Abstract:Qualitative Health Research, Ahead of Print. This study feeds into ongoing discussions on the metaphors used by cancer patients. Its aim is to explore how women living with a history of breast cancer use metaphors to express and interpret the experience of cancer remission. Data were collected in interviews designed to capture a rich and metaphorical description of participants' experiences with breast cancer and what these experiences mean to them. Ten participants were recruited. An interpretative phenomenological analysis of the participants' narratives highlighted a central metaphor: the cancer trace in one's life. The participants had to adapt to four specific traces of cancer: (1) the identity trace, (2) the existential trace, (3) the bodily trace, and (4) the narrative trace. We discuss how cancer challenges one's sense of biographical continuity and initiates a search for a new way of being. We also discuss how the metaphor of the trace differs from the metaphor of the cancer hero living without any trace of cancer.

Abstract:Maintaining the continuity of the 'self' is crucial for finding meaning and ensuring psychological well-being. However, the traumatic encounter with cancer disrupts the normal flow of life stories, challenging coherence. Beyond its physical toll, cancer challenges one's sense of self, identity and overall psychological resilience. As personal narratives unfold, encompassing evolving life stories from one's reconstructed past to the present and envisioned future, inherently reflecting the dynamic conception of self, it becomes imperative to investigate how these narratives shape self-concept and resilience throughout the intricate emotional and psychological landscape of the cancer journey. This narrative review explores the intricate relationship among personal narratives, self-concept and psychological resilience in the context of cancer. The study draws on various theoretical constructs, encompassing self-concept, coping, narrative identity, and psychological resilience theories, to provide a comprehensive exploration of the psychological processes involved. Incorporating insights from both Western and Indian philosophical traditions, the review not only enriches the theoretical framework but also emphasizes the practical implications for healthcare practices. Integrating narrative resilience principles into medical education and advocating for culturally sensitive approaches are deemed essential for advancing patient-centred care. The study underscores both practical and theoretical contributions to psycho-oncology, emphasizing the need for personalized narrative interventions, empathetic healthcare strategies, longitudinal studies and innovative technologies that recognize and support the intricate interplay between personal narratives, self-concept and psychological resilience in the cancer survivorship journey.
