Abstract:This article presents and interprets illness narratives, told by patients treated for cancer. The aim is to discuss the meaning of cancer metaphors. The interpretation focuses on the relationships of metaphors to bodily experience and to social and cultural levels. The experience of cancer and cancer therapy obviously involve the patient's body. To comprehend what is happening is a process full of uncertainty and obscurity. Metaphors, such as cancer "eating," are widespread and persistent while the meaning of that metaphor today mirrors new scientific and lay explanations of cancer and cancer treatment. Personal newly created metaphors express threatening experiences in which fear of losing oneself and fear of death are involved. Ambivalent hope in relation to treatment and caregivers is expressed. Creating new metaphors involves imaginatively using the available concepts and metaphors. Narrative communication gives access to patients' experiences of an unwanted and painful physiologic process leading to forced embodiment of sickness. Patients want consolation and must overcome solitude by articulating experience, being listened to, and, in this way, recreating and strengthening identity.

Abstract:Background: The Aesthetics of Health (AOH) undergraduate visual art studies course at the University of Texas at Austin aimed to enhance art students' awareness of cancer's impact not only medically but also socially, emotionally, financially, and spiritually and to examine how this experience might impact students' artwork, capacity for empathy, and connection to audience. Methods: During the spring 2021 semester, the AOH course instructors employed assorted pedagogical methods, including art, illness narrative, and community engagement, in special sessions led by professors, community practice artists, and cancer experts, respectively, as well as oral storytelling by those with lived experience of cancer (ie, cancer patients, posttreatment survivors, and loved ones). For the course's final project, the 15 student-artists created self-selected media works combining health and activism and displayed them in public spaces, including online. Student-artists took the Toronto Empathy Questionnaire during the first and last weeks of the course and provided feedback. Two group interviews were also held with cancer storytellers following their participation. Results: Student-artists' average score on the Toronto Empathy Questionnaire increased from 52.46 at pretest to 55.38 at posttest. Student-artists and storytelling participants also reported having positive experiences. Conclusions: The AOH course's social practice approach encouraged student-artists to realize new ideas and relationships and modestly increased their capacity for empathy. The AOH framework demonstrates promise for increasing empathy through the arts in other educational, clinical, and artistic institutions. Further research is needed with larger sample sizes to measure the impact of the course and to demonstrate its potential for addressing burnout and moral distress.
Striving to Thrive: A Qualitative Study on Fostering a Relational Perspective Through Narrative Therapy in Couples Facing Cancer

Abstract:The present study explored how couples, in which one partner self-reported receiving a medical diagnosis of stage one or two cancer, fostered a relational perspective regarding cancer via narrative therapy interventions. This is a qualitative study with a multiple case study design and thematic analysis. Eight heterosexual couples (N =16) participated in dyadic, researcher-facilitated, audio-recorded therapeutic sessions (i.e., 208 sessions in one year). In-depth interviews were conducted with couples in a therapeutic setting. Three themes emerged: (a) increased vulnerability, (b) increased mindfulness, and (c) improved communication. Findings support the use of psychosocial therapeutic interventions based on narrative therapy.
Recounting the untold stories of breast cancer patient experiences: lessons learned from a patient-public involvement and engagement storytelling event

Abstract:Objectives: Breast cancer remains a prevalent disease in women worldwide. Though advancements in breast cancer care have improved patient survival, a breast cancer diagnosis, and subsequent interventions have a lasting impact on patients' lived experiences during the pandemic. Methods: We present the collaborative learning process from this patient engagement workshop series as a community-academic partnership. Narrative medicine tools were used to recount patients' lived experiences following diagnosis, where both patients and researchers shared their cancer research activities in each workshop, and the role of the multidisciplinary healthcare team was discussed. Key findings: We used an iterative approach to cohort building, narrative development, and the use of multiple media formats to capture stories. Over 20 patients with breast cancer shared their stories for the first time since their diagnosis with a wider audience. Here, we present the learning process and considerations from this event. Conclusions: Understanding patients' lived experiences can support researchers and healthcare professionals in developing an empathetic approach to shared healthcare decision making. Moreover, understanding the lived experiences of patients is critical to addressing disparities in healthcare.

Abstract:Purpose: Novels and autopathographies that employ cancer as a central theme offer a wealth of opportunities for researching the way patients with cancer make sense of their illness and its treatment. Such literatures can also inform clinical care, because they can support patients in living with their illness. The use of novels and autopathographies for research and care in persons with cancer fits within the framework of 'Health Humanities', the interdisciplinary field where medicine and social science meet. This paper presents a concise overview of novels and autopathographies that explore cancer as their theme. Methods: Literature searches were conducted using PubMed, major scientific journals of medicine and clinical oncology, and databases in the Health Humanities. Searches focused on novels and autopathographies where cancer is the central theme, which are available in English, and which can be considered to represent 'high literature'. Results: Twenty-nine books were identified. The majority of the books were written originally in English, and breast cancer and lung cancer were the most frequently discussed types of cancer. The core themes identified were giving meaning to illness; coping with medical treatment; and the psychological and social consequences of illness. Conclusion: Novels and autopathographies about cancer represent an innovative base for research on living with cancer and offer rich data on how people make sense of cancer and its medical treatment. Clinical implications of this review pertain to interventions based on bibliotherapy and expressive writing. Novels and autopathographies are just part of the Health Humanities context: a wide range of art genres may prove helpful in improving the quality of life of persons with cancer.

Abstract:Maintaining the continuity of the 'self' is crucial for finding meaning and ensuring psychological well-being. However, the traumatic encounter with cancer disrupts the normal flow of life stories, challenging coherence. Beyond its physical toll, cancer challenges one's sense of self, identity and overall psychological resilience. As personal narratives unfold, encompassing evolving life stories from one's reconstructed past to the present and envisioned future, inherently reflecting the dynamic conception of self, it becomes imperative to investigate how these narratives shape self-concept and resilience throughout the intricate emotional and psychological landscape of the cancer journey. This narrative review explores the intricate relationship among personal narratives, self-concept and psychological resilience in the context of cancer. The study draws on various theoretical constructs, encompassing self-concept, coping, narrative identity, and psychological resilience theories, to provide a comprehensive exploration of the psychological processes involved. Incorporating insights from both Western and Indian philosophical traditions, the review not only enriches the theoretical framework but also emphasizes the practical implications for healthcare practices. Integrating narrative resilience principles into medical education and advocating for culturally sensitive approaches are deemed essential for advancing patient-centred care. The study underscores both practical and theoretical contributions to psycho-oncology, emphasizing the need for personalized narrative interventions, empathetic healthcare strategies, longitudinal studies and innovative technologies that recognize and support the intricate interplay between personal narratives, self-concept and psychological resilience in the cancer survivorship journey.
Regional adipose tissue metabolism in men and postmenopausal women.

Abstract:The metabolism of subcutaneous abdominal and femoral fat cells was studied in postmenopausal women and matched men of similar age and weight index. Lipoprotein lipase (LPL) activity was similar in both regions in the postmenopausal women. However, a group of women who underwent treatment with estrogen and progestins had considerably higher LPL activity in the femoral cells. Testosterone treatment for 7 days did not change LPL in either region in middle-aged men. Femoral adipocytes from both men and postmenopausal women were markedly unresponsive to catecholamine in comparison with abdominal cells. No clear effect of the sex hormones was seen on lipolysis in the women. Treating men with testosterone increased basal lipolysis in the abdominal cells. Abdominal fat cells were more responsive to the antilipolytic effect of insulin. Both responsiveness and sensitivity to insulin were somewhat greater in the abdominal fat cells from men (ED50 approximately 0.02 nm) than from women (ED50 approximately 0.17 nm). Insulin binding, however, was similar in fat cells from the abdominal and the femoral regions when the results were expressed per unit surface area. Thus, regional differences in fat cell metabolism are seen in both postmenopausal women and elderly men. LPL activity in the femoral region seems to be under the control of female sex hormones while lipolysis appears to be less influenced by sex hormones.

Abstract:As the number of people living with cancer increases, it is important to understand how people can live well with and after cancer. First Nations people diagnosed with cancer in Australia experience survival disparities relating to health service accessibility and a lack of understanding of cultural needs and lived experiences. This study aimed to amplify the voices of First Nations individuals impacted by cancer and advance the development of a culturally informed care pathway. Indigenist research methodology guided the relational and transformative approach of this study. Participants included varied cancer experts, including First Nations people living well with and after cancer, health professionals, researchers, and policy makers. Data were collected through online Yarning circles and analysed according to an inductive thematic approach. The experience of First Nations people living well with and after cancer is inextricably connected with family. The overall themes encompass hope, family, and culture and the four priority areas included the following: strength-based understanding of cancer, cancer information, access to healthcare and support, and holistic cancer services. Respect for culture is interwoven throughout. Models of survivorship care need to integrate family-centred cancer care to holistically support First Nations people throughout and beyond their cancer journey.

Abstract:Qualitative Health Research, Ahead of Print. Many more cancers are treated with intent to cure now than in previous decades, but for most, this involves significant effects from which people need to recover psychologically and socially, as well as physically. This longitudinal photo-elicitation interview study uses grounded theory to explain how people discharged from specialist care made use of everyday social and material resources to manage this process at home. Recovery is presented as a curve in life's pathway requiring gradual reorientation, drawing on social worlds and domestic resources to calibrate this process. Findings are described in three stages: (1) responding to diagnosis and treatment, (2) using social resources for meaning-making, and (3) developing assets for recovery. During treatment, participants drew on past identities to reinforce their sense of self, and personalized health care communication supported this process. In the weeks after treatment, new frameworks of understanding were constructed from perspectives on cancer encountered in the family, workplace, and outpatient clinics. Recovery processes included the negotiation of personal change, the renegotiation of close relationships, and the use of everyday resources to regain three sensations: control, comfort, and continuity. Supportive care would benefit from an individualized exploration of the assets that can help people to negotiate this challenging phase as treatment comes to a close. Possibilities for self-care (the maintenance of health and well-being in the context of everyday life) can be explored and assessed through personalized discussion around the identities, social worlds, and everyday resources available to each individual.

Abstract:Introduction Introduction: Coping with a breast cancer diagnosis is individualized and patient strategies vary throughout their dealing with this cancer. Coping methods include, but are not limited to: support groups, counseling, practicing mindfulness, spending time with family and friends, etc. One such technique is expressive writing (EW); writing that is focused on intrusive thoughts and emotional aspects related to their cancer. Objective Objectives: This meta-analysis reviewed published data regarding EW and its effects on the quality of life (QOL), including the sexual life, of breast cancer patients and survivors. Writing is a form of self-expression, which not only allows patients control of their emotions and treatment but also serves to improve the quality of care given to breast cancer patients and survivors. Thus, the ability to better implement personalized medicine remains at the center of holistic care. Additionally, we aimed to emphasize the existing clinical gap between minority groups, specifically Asian Americans, and ethnically prevalent groups. Methods Methods: A literature search was performed using PubMed to identify Randomized Controlled Trials (RCT) conducted within the United States between the years 2008 and 2023. Only RCTs with quantitative results from pre- and post-writing intervention were considered. While the structure of RCTs varied, all included interventions incorporated one or more EW group(s) and/or one or more NW (neutral writing) group(s). Measures of interest included: self-reflections from participants, linguistics, sleep quality, and Post-traumatic Stress (PTSD) symptoms. Results Results: The writing interventions studied included both NW and EW groups. These interventions resulted in various reductions of physical symptoms (ex: pain, nausea, headaches, etc.) and psychological changes (in mood, sleep, depression, and anxiety), after patients in EW groups self-reflected. Writing interventions with ethnic minorities are limited; however, writing interventions with Asian American participants improved PTSD-related symptoms and quality of life. Chinese-American women face cultural stigmatism, which may cause patients to fear sharing their deepest emotions, a possible explanation for the observed positive influence of EW for these participants. Unfortunately, no RCTs that associated EW with sexual well-being were analyzed. Conclusions Conclusions: The findings of this meta-analysis note that RCTs suggest that there is a link between EW and better coping with breast cancer. However, results may differ based on population, demographics, and cancer stage/treatment. The narrow scope of RCTs with ethnic minorities highlights that EW as a coping method for such populations remains understudied. As well, there are no data on its impact on sexual well-being-an area of research that requires more attention as women of all walks of life navigate their respective breast cancer diagnoses. The role of narrative and metaphor in the cancer life story: a theoretical analysis

Abstract:Being diagnosed with cancer can be one of those critical incidents that negatively affect the self. Identity is threatened when physical, psychological, and social consequences of chronic illness begin to erode one's sense of self and challenge an individual's ability to continue to present the self he or she prefers to present to others. Based on the notion of illness trajectory and adopting a Ricoeurian narrative perspective, this theoretical paper shall explore the impact of cancer disease on identity and establish the crucial importance of metaphor in the narratives of life with cancer. Findings indicate that in cancer narratives the illness experience supplies the narrative structure with temporal and spatial meeting points that make the narrative comprehensible and meaningful. Cancer forces identity changes not only from having to endure the long-term physical and psychosocial effects of the disease, but also from inevitable existential questions about life's meaning. Improved medical knowledge today means improved ethnomedical practices. Metaphor can bridge the gap between the cancer experience and the world of technology and treatment, helping patients to symbolically control their illness.
Regaining autonomy, competence, and relatedness: Experiences from two Shared Reading groups for people diagnosed with cancer

Abstract:This study explored 12 cancer patients' experiences from participating in an online and on-site Shared Reading group for 16 weeks in Norway. Shared Reading is a practice in which prose and poetry are read aloud in small parts and discussed along the way. The study is a qualitative evaluation study with a particular focus on how the participants experienced the reading group supported their life living with cancer. The study was mainly based on the data collected from focus group discussions with the participants, which was analysed qualitatively through open coding. In total, four themes were identified: (1) open space, (2) disconnecting through connecting, (3) community, and (4) resonances and echoes. The participants expressed that the RG helped them to "balance life and cancer", and "disconnect" from their illness. The cognitive effort needed was beneficial for the participants as a form for "cognitive training." Since many of the participants had, due to their illness, completely stopped reading books, the reading group also brought literature back into the participants' lives. Furthermore, it was essential for the participants to feel they contributed to a community, to feel useful and valuable for others. The texts were also important, as some of them resonated strongly with the participants in the way of activating memories and connecting a text to own experiences. After a session, a text could still have an impact as an echo. The results are synthesised, discussed, and supported through the framework of self-determination theory and, more specifically, the basic psychological need theory. The reading group was experienced as a support for autonomy, competence, and relatedness and promoted a feeling of intrinsic motivation that brought about new dimensions in the participants' lives. The study wishes to increase our knowledge of the benefits of integrating Shared Reading groups as a low-cost, literature-based psychosocial support in cancer organisations.

Abstract:The use of metaphors to talk about cancer experiences has attracted much research and debate, especially in the case of military metaphors. However, questions remain about what metaphors are used by different populations for different aspects of the cancer experience. This scoping review aims to answer them.We searched PubMed, PsycINFO, CINAHL, Scopus and Web of Science databases. Eligible sources include peer-reviewed scientific research published in English between 2013 and 2023, investigating patterns of metaphor use from adult populations (age ≥18) for cancer-related topics, such as cancer itself, the general experience of being ill, treatment, and people and relationships.Out of 1929 articles identified, 30 met the criteria, spanning over different populations. While most papers focused on cancer in general, some focused on specific cancer types, such as breast cancer. Both spontaneous and elicited data were collected in ten languages: mostly English (N=12), Swedish (N=3) and Arabic (N=3). The identified metaphors were subsumed under various broad categories, including particularly violence and journey. Other categories include education and non-human animate entity for the cancer itself, confinement and deprivation and cleanliness for the general experience of being ill with cancer, Poison and gardening for cancer treatment, and distance for patients' social relationships.It was found that metaphors help to identify how patients describe experiences of vulnerability and empowerment. To provide patient-centred care, clinicians and researchers should avoid blanket conclusions about helpful or unhelpful metaphors, but consider the ways in which different metaphors are used by different populations in different contexts.
Reanimating the Body: Comics Creation as an Embodiment of Life with Cancer

Abstract:Cancer is regarded as a disease that redefines an individual's life and relationships. The medicalization and reclamation of the individual's sense of body, self, and social life have been long examined by psychiatry and anthropology alike. We argue that creating comics is a form of artistic narrative that affirms and proclaims the existence of a past and future possibilities for individuals diagnosed with cancer. Despite the interconnections among lived experience and meaning making, little attention has been paid to the potential therapeutic effects of comics creation. Individuals diagnosed with cancer were recruited for ten weekly comics making workshops. Data include qualitative interviews and workshop observations. Six women who were diagnosed with cancer consented to participate. Meaning making themes included (1) slowing down to process their experiences, (2) expressing frustration with medical encounters, and (3) reflecting on traumatic relationships. The process of redefining their cancer experience connects the sufferer's individual and social context. We find that the physical act of 'making' comics works to create meaning and an embodied expression of meaning. Creating comics, for our participants, offered multiple entry points and perspectives for redefining their stories that provided new insights and paths to explore their medical traumas and reanimating their bodies.
Supportive Communication Between Emerging Adults With Cancer and Nurses During Diagnosis and Cancer Treatment: An Exploratory Qualitative Study Using Interviews

Abstract:Background: Having a cancer diagnosis during early adulthood can be a significant challenge for an individual. Nurses' supportive communication plays a vital role during the diagnosis and treatment period to lessen psychological distress and promote coping. Objective: This exploratory study aimed to examine (1) the experiences of emerging adults with cancer (EAs) aged between 18 and 25 years in communicating with nurses during diagnosis and treatment and (2) nurses' experiences of providing supportive communication with this patient group. Methods: Semistructured interviews were conducted with EAs and nurses with experience caring for this patient group. Thematic analysis was conducted, guided by interpretive hermeneutic perspectives. Results: Eight EA participants and 7 nurse participants participated in interviews. Five themes emerged: (1) having casual conversations with nurses helped EAs cope during cancer treatment and (2) helped EAs fulfill the need for social connectedness, (3) nurses as a different form of peer-like support, (4) nurses used themselves as a therapeutic tool to foster trust and emotional safety of EAs, and (5) nurses needed to maintain professional boundaries while being compassionate. Conclusions: This study highlighted a rather underdocumented aspect of supportive communication: meeting psychosocial needs through casual, day-to-day conversations. Implications for practice: Having casual conversations with nurses appeared to help EAs' psychological coping during cancer treatment. These casual conversations, which on the surface seemed clinically insignificant, fulfilled their psychosocial needs. Considering the valuable interpersonal engagement that led to emotional benefits for EAs, communication training for cancer nurses needs to build their capacity to maintain both relational and emotional boundaries.

Abstract:Owing to the increasing ethnic diversity in the USA, healthcare providers need to understand the specific needs of minority cancer survivors. The primary goal of this article was to understand beliefs about cancer, its treatment, and relevant concerns among Chinese American breast cancer survivors (BCS). Compared with non-Hispanic White BCS in the USA, Chinese immigrant BCS were less likely to resolve treatmentrelated issues (i.e., physical distress), potentially due to their lower tendency to ask physicians questions, ask for referrals, and make repeated attempts to cope with physical distress [1]. A recent study also highlights that unmet psychosocial needs (e.g., lack of opportunities for open discussions of feelings, needs for medical information, and needs for culturally and linguistically appropriate support groups) disrupted quality of life among Chinese American BCS [2]. As Chinese American is the largest Asian subgroup in the USA, understanding cancer-related and treatment-related beliefs within this population will help to identify informational and medical needs that healthcare providers can intervene in future services. Culture can influence how individuals respond to cancer diagnosis and treatment. Standardized tools, however, cannot accurately assess cultural beliefs about cancer. Thus, this study used a qualitative and inductive approach. Culture can also influence how individuals disclose their private thoughts or emotions. We have previously argued that expressive writing [3] allows people to privately disclose their deepest thoughts and feelings and thus can help them overcome cultural barriers that prevent them from disclosing. These barriers include stigma related to breast cancer, cultural beliefs of bearing burden alone, and a norm of suppressing emotions. In this study, we used expressive writing as a paradigm to gather qualitative data by allowing in-depth disclosure of thoughts while guaranteeing confidentiality.
Expressive Writing Intervention Improves Quality of Life Among Chinese-American Breast Cancer Survivors: A Randomized Controlled Trial.

Abstract:Background Expressive writing interventions are shown to confer health benefits for Caucasian cancer survivors. However, few studies reported improved quality of life or studied ethnic minorities. Purpose The study evaluated whether a culturally sensitive expressive writing intervention improved quality of life. Methods Chinese-speaking breast cancer survivors (n = 136) in the USA were randomly assigned to one of three conditions to write three 30-min weekly essays: a cancer-fact condition to write about facts relevant to the cancer experience for three weeks; a self-regulation condition to write about deepest feelings at week 1, stress and coping at week 2, and finding benefits at week 3; or an enhanced self-regulation condition to write about stress and coping at week 1, deepest feelings at week 2, and finding benefits at week 3. Quality of life was assessed by Functional Assessment of Cancer Therapy at baseline, 1-, 3-, and 6-month follow-ups. Results Growth curve models showed that quality of life was increased in the sample from baseline to the 6-month follow-up. The enhanced self-regulation condition had a large and statistically significant effect (d = 0.90, 95% CI [0.02, 1.687]), and the self-regulation condition had a small effect (d = 0.22, 95% CI [-0.79, 1.07]) on quality of life improvement compared with the cancer-fact group. Conclusion Expressive writing is shown to be an effective intervention to improve quality of life for Chinese-American cancer survivors. Future efforts are warranted to disseminate and implement this low-dose and brief intervention in community and clinical settings.

The method used in this research is a systematic review, where we conducted journal searches in the ScienceDirect, PubMed, ProQuest, Springer, and EBSCOhost databases using the keywords' cancer patients', 'psychological well-being, 'and 'self-compassion. ' The research results show that self-compassion influences levels of depression, stress, anxiety, quality of life, body image disturbance, body shame, stigma, and other things that affect the psychology of cancer patients. Abstract:Purpose This study examined the feasibility and preliminary efficacy of Expressive Writing (EW) in improving informal cancer caregiver (IC) and patient health, and enhancing ICs' emotion regulation. Method Fifty-eight breast cancer ICs and patients participated in a randomized controlled feasibility trial of remote EW. ICs were randomly assigned to the EW or control group and completed 3 weekly writing sessions. ICs and patients completed health and emotion regulation assessments at baseline, intervention completion, and 3 months post-intervention. Screening, recruitment, assessment process, randomization, retention, treatment adherence, and treatment fidelity were computed for feasibility. Effect sizes were calculated using the PROMIS Depression Short Form, RAND Short Form 36 Health Survey, Breast Cancer Prevention Trial Hormonal Symptom checklist, healthcare utilization, and the Emotion Regulation Questionnaire for efficacy. Results Of the 232 interested individuals, 82 were screened, and 60 enrolled (6 monthly). Two individuals withdrew and 19 were lost to follow-up, leaving 39 individuals. ICs completed at least one assessment and two sessions, and patients completed at least two assessments. All sessions were administered as intended. ICs generally followed instructions (88%–100%), wrote the full time (66.7%–100%), and were engaged ( M(SD) = 3.00(1.29)-4.00(0.00)). EW had small-to-medium effects in improving IC health ( g = −0.27–0.04) and small-to-large effects in improving patient health ( g = −0.28-0.86). EW moderately decreased suppression ( g = 0.53–0.54) and slightly increased reappraisal, at least 3 months post-intervention ( g = −0.34-0.20). Conclusions Remote EW may be feasible with cancer ICs and improve cancer IC and patient health. However, it can benefit from additional retention strategies and rigorous testing.
