Abstract:Background Untreated sleep problems in both persons living with dementia (PLWD) and their family care partners (CP) impact their health and quality of life. This pilot study tested a sleep intervention program for both dyad members. Methods Thirty dyads were randomized to a 5‐session Care2Sleep intervention (n = 15 dyads) or an information‐only control group (n = 15 dyads) delivered in‐person or by video‐telehealth by trained sleep educators. Care2Sleep is a manual‐based program, incorporating key components of cognitive behavioral therapy for insomnia, daily light exposure and walking, and problem‐solving for dementia‐related behaviors. Adherence with Care2Sleep recommendations was assessed. Sleep outcomes included actigraphy‐measured sleep efficiency (SE) and total wake time (TWT) for dyads, and the Pittsburgh Sleep Quality Index (PSQI) for CP. Other outcomes for CP included the Zarit Burden Interview (ZBI) and positive aspects of caregiving (PAC). Outcomes were measured at baseline, posttreatment, and 3‐month follow‐up. A 2 (group) by 3 (time) mixed model analysis of variance tested treatment effects. Results Study feasibility was demonstrated, with 13 dyads completing all five sessions of Care2Sleep program and 14 completing the control condition. In the Care2Sleep group, the dyads adhered to recommended sleep schedules of 76% for bedtime and 72% for get‐up time for PLWD, and 69% for bedtime and 67% for get‐up time for CP. There were several nonsignificant trends in outcomes from baseline to 3‐month follow‐up between the two groups. For example, SE increased by 3.2% more for PLWD and 3.2% more for CP with Care2Sleep versus control. TWT decreased by 14 min more for PLWD and 12 min more for CP with Care2Sleep versus control at the 3‐month follow‐up. CP in Care2Sleep also showed improvement in the PSQI, ZBI, and PAC scores. Conclusions A dyadic approach to sleep improvement is feasible. Larger trials are needed to test effects of this intervention for PLWD and their family CP. ClinicalTrials.gov: NCT03455569.

Efficacy of Mindfulness-Based Intervention ('Mindfulness-Based Joyful Sleep') in Young and Middle-Aged Individuals with Insomnia Using a Biomarker of Inflammatory Responses: a Prospective Protocol of a Randomised Controlled Trial in China.

Abstract:INTRODUCTION:Insomnia is a prevalent and significant public health concern. Insomnia can lead to increased inflammatory markers associated with chronic diseases such as cardiovascular disease, diabetes and cancer. Studies suggest that mindfulness-based interventions (MBIs) are more easily delivered within the community than cognitive behavioural therapy for insomnia (CBT-I) which was recommended as the preferred non-pharmacological treatment by the American Academy of Sleep Medicine, are effective in insomnia treatment and can reduce inflammatory markers level in older individuals with insomnia. This study aims to compare the effectiveness of an MBI to CBT-I in young and middle-aged individuals with insomnia disorder and explore its effect on nuclear factor kappa B (NF-κB), a transcription factor that controls the expression of genes involved in inflammation.METHODS AND ANALYSIS:This report describes a protocol for a randomised controlled trial. Seventy eligible participants will be assigned to mindfulness-based joyful sleep or CBT-I for 2-hour sessions weekly for 8 weeks. The primary outcome is sleep quality assessed by the Pittsburgh Sleep Quality Index, severity of insomnia symptoms assessed by the Insomnia Severity Index and sleep parameters recorded using sleep diary and polysomnography. Secondary outcomes include perceived stress, anxiety and depression. The exploratory outcome is serum level of NF-κB. Outcomes will be evaluated at baseline, the end of the ntervention period and at a 3 month follow-up. Data will be analysed using general linear models, specifically analysis of covariance and analysis of variance will be used.ETHICS AND DISSEMINATION:Full ethical approval for this study has been obtained from the Ethics Committee of the Third Xiangya Hospital, Central South University, Changsha, China (2018-S236). Abstract:Objectives: Sleep disturbances are highly prevalent and have adverse health consequences for both people living with dementia and their carepartners. Despite this, they are under-addressed caregiving settings. This study aimed to explore these sleep disturbances and co-design a multimodal sleep intervention for people living with dementia and their carepartners. Methods: We conducted two focus groups and five semi-structured interviews (n = 4 people living with dementia, n = 6 carepartners). Active involvement of community advisors was sought throughout the design, development, and facilitation phases. Reflexive thematic analysis was used to explore sleep-related experiences and receive feedback to shape intervention development. Findings: People living with dementia reported disruptions to sleep and circadian rhythms, including sleep disturbances and confusion between day and night. Multiple sleep challenges were encountered by carepartners including insomnia, hypervigilance, and daytime impairment. The proposed sleep intervention was received positively, with significant insights emphasising the need for a multimodal toolkit approach, adaptation of the intervention across different dementia stages, and a focus on tailoring the program to carepartners. Conclusion: Sleep interventions for caregivers and care-recipients should target both sleep and daytime functioning to ensure holistic support. Participants were receptive towards time-friendly, online, multimodal sleep interventions that combine cognitive behaviour therapies, light therapy, mindfulness, and exercise elements.

Abstract:Abstract Introduction Prior research assessing self-reported or nocturnal sleep suggested that specific sleep traits (sleep restriction in the laboratory; poor sleep quality, sleep disorders) are associated with both pro and anti-inflammatory cytokine and C-Reactive Protein (CRP) levels. To extend prior research, we investigated associations between multiple dimensions of objectively assessed sleep health with both circulating and stimulated inflammatory markers in a diverse sample of older adults. Methods Participants in the Einstein Aging Study (EAS) were included (N=218, Mage=77.5 years, 6% female; 46% White, 38% Black, 14% Hispanic/other). A confirmatory factor analysis using wrist actigraphy sleep variables over 16 days yielded 6-component multi-dimensional sleep health facets: Regularity, Alertness/Sleepiness (daytime), Timing, Efficiency, Duration, and Rhythmicity. Inflammatory markers collected in the morning included CRP and both circulating markers reflecting current systemic inflammation, and lipopolysaccharide (LPS)-stimulated levels of (IL-1β, IL-4, IL-6, IL-8, IL-10, TNFα), which reflect responses to an inflammatory challenge, an index of immune reactivity. Associations between each inflammatory marker and each sleep health facet were examined using linear regression, controlling for BMI, gender, age, education, ethnicity/race, number of health conditions, nocturnal hypoxemia, Oxygen Desaturation Index (ODI)≥15. Results Participants with more daytime sleepiness (i.e., higher number and longer duration of naps and lower activity level) had higher circulating IL-6 and CRP levels. Lower sleep efficiency was associated with lower levels of the anti-inflammatory circulating cytokine IL-4 and higher levels of stimulated IL-8 and TNFα. Longer sleep duration was associated with higher levels of circulating IL-1β and CRP. Less rhythmicity across days was associated with higher levels of circulating IL-6. Nocturnal hypoxemia was associated with higher levels of CRP and stimulated IL-4, IL-8, and TNFα. We found no association between sleep timing and regularity and any inflammatory marker. Conclusion We observed that various sleep health facets are related to markers of inflammation and immune reactivity in older adults. This research also demonstrates the value of using 24-hour, device-based estimates of different sleep health facets relevant for immune function. Associations between 24-h sleep and inflammation are particularly important for older adults, who evidence more daytime sleepiness and less rhythmic sleep/active patterns in daily life. Abstract:A major barrier to recombination between bacterial species lies in the mismatch repair system, a complex of proteins that has evolved to proof-read freshly replicated DNA. It now appears that a second system, involving an inducible DNA recombination, repair and mutagenesis pathway, also regulates interspecies recombination, but in a positive way, being required for recombination between Escherichia coli and Salmonella typhimurium. Thus the rate at which newly emerging species of bacteria diverge can be seen as a balance between a permissive state associated with inducible repair and recombination, and the proof-reading of intermediates in the recombination pathway by the mismatch correction system.
Multi-modal sleep intervention for community-dwelling people living with dementia and primary caregiver dyads with sleep disturbance: protocol of a single-arm feasibility trial

Abstract:Background: Disturbed sleep is common among people living with dementia and their informal caregivers, and is associated with negative health outcomes. Dyadic, multi-modal interventions targeting caregiver and care-recipient sleep have been recommended yet remain limited. This protocol details the development of a single-arm feasibility trial of a multi-modal, therapist-led, six-week intervention targeting sleep disturbance in dyads of people living with dementia and their primary caregiver. Methods: We aim to recruit 24 co-residing, community-dwelling dyads of people living with dementia and their primary informal caregiver (n = 48) with sleep concerns (Pittsburgh Sleep Quality Index ≥5 for caregivers, and caregiver-endorsed sleep concerns for the person living with dementia). People who live in residential care settings, are employed in night shift work, or are diagnosed with current, severe mental health conditions or narcolepsy, will be excluded. Participants will wear an actigraph and complete sleep diaries for two weeks prior, and during the last two weeks, of active intervention. The intervention is therapist-led and includes a mix of weekly small group video sessions and personalised, dyadic sessions (up to 90 min each) over six weeks. Sessions are supported by a 37-page workbook offering strategies and spaces for reflections/notes. Primary feasibility outcomes are caregiver: session attendance, attrition, and self-reported project satisfaction. Secondary outcomes include dyadic self-reported and objectively-assessed sleep, depression and anxiety symptoms, quality of life, and social support. Self-report outcomes will be assessed at pre- and post-intervention. Discussion: If feasible, this intervention could be tested in a larger randomised controlled trial to investigate its efficacy, and, upon further testing, may potentially represent a non-pharmacological approach to reduce sleep disturbance among people living with dementia and their caregivers. Intervention for sleep problems in nursing home residents with dementia: a cluster-randomized study

Abstract:Objective: To reduce sleep problems in people living with dementia using a multi-component intervention. Design: Cluster-randomized controlled study with two parallel groups and a follow-up of 16 weeks. Setting: Using external concealed randomization, 24 nursing homes (NH) were allocated either to the intervention group (IG, 12 clusters, 126 participants) or the control group (12 clusters, 116 participants). Participants: Participants were eligible if they had dementia or severe cognitive impairment, at least two sleep problems, and residence of at least two weeks in a NH. Intervention: The 16-week intervention consists of six components: (1) assessment of sleep-promoting activities and environmental factors in NHs, (2) implementation of two "sleep nurses," (3) basic education, (4) advanced education for staff, (5) workshops to develop sleep-promoting concepts, and (6) written information and education materials. The control group (CG) received standard care. Measurements: Primary outcome was ≥ two sleep problems after 16 weeks assessed with the Sleep Disorders Inventory (SDI). Results: Twenty-two clusters (IG = 10, CG = 12) with 191 participants completed the study. At baseline, 90% of people living with dementia in the IG and 93% in the CG had at least two sleep problems. After 16 weeks, rates were 59.3% (IG) vs 83.8% (CG), respectively, a difference of −24.5% (95% CI, −46.3% – −2.7%; cluster-adjusted odds ratio 0.281; 95% CI 0.087–0.909). Secondary outcomes showed a significant difference only for SDI scores after eight and 16 weeks. Conclusions: The MoNoPol-Sleep intervention reduced sleep problems of people living with dementia in NH compared to standard care.

Exploring the relationship of sleep duration on cognitive function among the elderly: a combined NHANES 2011-2014 and mendelian randomization analysis

Abstract:Background: As one of the key features of sleep, sleep duration (SD) has been confirmed to be associated with multiple health outcomes. However, the link between SD and cognitive function (CF) is still not well understood. Methods: We employed a combined approach utilizing data from the National Health and Nutrition Examination Survey (NHANES 2011-2014) and Mendelian Randomization (MR) methods to investigate the relationship between SD and CF. In the NHANES cross-sectional analysis, the association between these variables was primarily examined through multivariate linear regression to explore direct correlations and utilized smoothing curve fitting to assess potential nonlinear relationships. To ensure the robustness of our findings, subgroup analyses were also conducted. MR analysis was used to assess the causal relationship between SD and sleeplessness on CF. After excluding confounding factors, univariate and multivariate MR were performed using inverse variance weighting (IVW) as the main analysis method, and sensitivity analysis was performed. Results: The results of our cross-sectional study indicate a notable negative association between SD and CF, forming an inverted U-shaped curve with the inflection point occurring at SD = 6 h. This relationship remains consistent and robust across subgroup analyses differentiated by variables such as age, levels of physical activity, and frequency of alcohol intake. In MR analysis, IVW analysis showed no causal relationship between SD and sleeplessness on CF (Both P > 0.05). Conclusion: Cross-sectional studies suggest the existence of an inverted U-shaped correlation between SD and CF among the elderly. However, MR analysis did not reveal a causal relationship between SD and CF, which the lack of nonlinear MR analysis may limit. These findings provide evidence from a sleep perspective for optimizing cognitive strategies in older adults.

Abstract:Objectives To evaluate the impact of a mentoring program to encourage staff-delivered sleep-promoting strategies on sleep, function, depression, and anxiety among skilled nursing facility (SNF) residents. Design Modified stepped-wedge unit-level intervention. Setting and Participants Seventy-two residents (mean age 75 ± 15 years; 55.9% female, 41% non-Hispanic White, 35% Black, 20% Hispanic, 3% Asian) of 2 New York City urban SNFs. Methods Expert mentors provided SNF staff webinars, in-person workshops, and weekly sleep pearls via text messaging. Resident data were collected at baseline, post-intervention (V 1 ), and 3-month follow-up (V 2 ), including wrist actigraphy, resident behavioral observations, Pittsburgh Sleep Quality Index (PSQI), Patient Health Questionnaire-9 (PHQ-9) depression scale, Brief Anxiety and Depression Scale (BADS), Brief Cognitive Assessment Tool (BCAT), and select Minimum Data Set 3.0 (MDS 3.0) measures. Linear mixed models were fit for continuous outcomes and mixed-effects logistic models for binary outcomes. Outcomes were modeled as a function of time. Planned contrasts compared baseline to V 1 and V 2 . Results There was significant improvement in PSQI scores from baseline to V 1 ( P = .010), and from baseline to V 2 ( P = .007). Other significant changes between baseline and V 1 included decreased depression (PHQ-9) ( P = .027), increased daytime observed out of bed ( P ≤ .001), and increased daytime observed being awake ( P < .001). At V 2 (vs baseline) being observed out of bed decreased ( P < .001). Daytime sleeping by actigraphy increased from baseline to V 1 ( P = .005), but not V 2 . MDS 3.0 activities of daily living and pain showed improvements by the second quarter following implementation of SLUMBER ( P 's ≤ .022). There were no significant changes in BADS or BCAT between baseline and V 1 or V 2 . Conclusions and Implications SNF residents had improvements in sleep quality and depression with intervention, but improvements were not sustained at 3-month follow-up. The COVID-19 pandemic led to premature study termination, so full impacts remain unknown.

Sleep Quality and Inflammation in Married Heterosexual Couples: an Actor-Partner Analysis

Abstract:Background: Subjective sleep quality is a predictor of important health outcomes, but little work has examined the social context of sleep that might inform theoretical models and intervention approaches. The present study tested actor-partner models of sleep quality and its links to inflammatory markers. Method: Participants were 84 middle-age to older adult heterosexual married couples who completed the Pittsburgh Sleep Quality Index and had blood drawn for determination of CRP and IL-6 levels. Results: Main results indicated that only actor levels of poor global sleep quality predicted higher CRP levels. No actor × partner or gender × actor/partner interactions were significant. These results were also not moderated by relationship quality. Secondary analyses, focused on the different components of sleep quality, revealed marginally significant evidence for partner's poor sleep (i.e., sleep disturbances, sleep latency) on one's own inflammatory outcomes. Conclusion: These results suggest the promise of modeling sleep quality as a dyadic process that can impact inflammation and potentially related health outcomes.

Abstract:The 2',5'-oligoadenylate synthetases are key enzymes that mediate antiviral actions of interferon (IFN). The mRNAs for the intermediate isoforms (p69) of human 2',5'-oligoadenylate synthetase are rapidly induced 10- to 20-fold in HT1080 glioma cells by IFN-beta and induced 3-fold at 24 h by IFN-gamma. Induction is mediated by three regulatory elements, an IFN-stimulated response element and two identical sites resembling interferon response factor binding sites that are located within 300 bp of the transcriptional start site. Maximal induction requires all three elements, yet mutation in the most distal IRF-1-like site diminishes transcription only slightly. Mutation in the ISRE substantially decreases constitutive expression but does not abrogate the response to IFNs. Simultaneous mutation in all three elements abolishes responsiveness to both IFN-beta and IFN-gamma. Both constitutive and IFN-beta-induced expression from the p69 promoter is blocked in mutant cell lines deficient in components of the transcription factor, interferon-stimulated gene factor 3, suggesting that it is the primary factor controlling IFN-beta induced expression of this gene.

Abstract:Previous studies suggest associations between self-reported sleep problems and poorer health, cognition, Alzheimer's disease pathology and dementia-related outcomes. It is important to develop a deeper understanding of the relationship between these complications and sleep disturbance, a modifiable risk factor, in late midlife, a time when Alzheimer's disease pathology may be accruing. The objectives of this study included application of unsupervised machine learning procedures to identify distinct subgroups of persons with problematic sleep and the association of these subgroups with concurrent measures of mental and physical health, cognition and PET-identified amyloid. Dementia-free participants from the Wisconsin Registry for Alzheimer's Prevention (n = 619) completed sleep questionnaires including the Insomnia Severity Index, Epworth Sleepiness Scale and Medical Outcomes Study Sleep Scale. K-means clustering analysis identified discrete sleep problem groups who were then compared across concurrent health outcomes (e.g. depression, self-rated health and insulin resistance), cognitive composite indices including episodic memory and executive function and, in a subset, Pittsburgh Compound B PET imaging to assess amyloid burden. Significant omnibus tests (P < 0.05) were followed with pairwise comparisons. Mean (SD) sample baseline sleep assessment age was 62.6 (6.7). Cluster analysis identified three groups: healthy sleepers [n = 262 (42.3%)], intermediate sleepers [n = 229 (37.0%)] and poor sleepers [n = 128 (20.7%)]. All omnibus tests comparing demographics and health measures across sleep groups were significant except for age, sex and apolipoprotein E e4 carriers; the poor sleepers group was worse than one or both of the other groups on all other measures, including measures of depression, self-reported health and memory complaints. The poor sleepers group had higher average body mass index, waist-hip ratio and homeostatic model assessment of insulin resistance. After adjusting for covariates, the poor sleepers group also performed worse on all concurrent cognitive composites except working memory. There were no differences between sleep groups on PET-based measures of amyloid. Sensitivity analyses indicated that while different clustering approaches resulted in different group assignments for some (predominantly the intermediate group), between-group patterns in outcomes were consistent. In conclusion, distinct sleep characteristics groups were identified with a sizable minority (20.7%) exhibiting poor sleep characteristics, and this group also exhibited the poorest concurrent mental and physical health and cognition, indicating substantial multi-morbidity; sleep group was not associated with amyloid PET estimates. Precision-based management of sleep and related factors may provide an opportunity for early intervention that could serve to delay or prevent clinical impairment.

Abstract:Background and objectives: Sleep disturbances occur in >60% of persons living with cognitive impairment, affecting their quality of life (QOL). Regulating the sleep-wake cycle through engaging cognitive, physical, and sensory-based activities delivered at strategic times may reduce sleep disturbances and be a feasible nonpharmacological treatment for sleep problems. The objective of this trial was to test the efficacy of a timed-activity intervention in improving QOL and sleep disturbances in persons living with cognitive impairment. Research design and method: Randomized 2-group parallel design involving 209 dyads of community-residing persons living with cognitive impairment and care partners. Dyads were randomly assigned (1:1) to 1-hr home activity sessions administered weekly in the morning, afternoon, or evening over 4 weeks (the Healthy Patterns Sleep Program), or to an attention-control condition consisting of sleep hygiene training plus education on home safety and health promotion. QOL, objective and subjective sleep quality, and neuropsychiatric symptoms were assessed at baseline and 4 weeks later. Results: QOL was significantly improved in the intervention group compared to control (p = .0491). There were no significant effects on objective or subjective sleep or neuropsychiatric symptoms. In a subgroup analysis, subjective sleep as measured by the PROMIS (Patient Reported Outcomes Measurement Information System) Sleep-Related Impairment survey was significantly improved in the intervention group compared to the control group for individuals with symptoms of depression (p = .015) or poor observed sleep at baseline (p = .005). Discussion and implications: The Healthy Patterns Intervention may benefit QOL for persons living with cognitive impairment and those with poor subjective sleep. A longer dose may be necessary to elicit improvement in actigraphically measured sleep-wake activity. Clinical trial registration number: NCT0368218 5.

Abstract:Abstract Prior research links poor sleep and higher systemic inflammation. Gender differences in both sleep and in inflammatory responses have been observed. We investigated gender differences in the association between levels as well as variability of objective sleep health and inflammatory markers (including stimulated inflammatory markers) among a diverse sample of older adults. Participants free of dementia (N=215, Mage=77.5 years, 64% women; 46% White, 39% Black, 15% Hispanic/other) were from The Einstein Aging Study. Actigraphy (2 weeks) yielded wake after sleep onset (WASO), total night sleep time (TST), and midpoint night sleep. Inflammatory markers included circulating C-reactive protein (CRP) and circulating and lipopolysaccharide (LPS)-stimulated levels of interleukin (IL)-1β, IL-4, IL-6, IL-8, IL-10, and tumor necrosis factor (TNF)-α. Bayesian variability models evaluated associations of inflammatory markers with mean and variability of sleep measures, adjusted for BMI, gender, age, education, race/ethnicity. Circulating inflammatory markers were not associated with sleep measures, except higher TST variability with higher circulating IL-6 among women. For stimulated cytokines, among men, higher mean WASO, TST, and later midpoint were associated with higher stimulated IL-8 and TNF-α. All stimulated cytokines except IL-1β were associated with greater variability in WASO, TST, and sleep timing among men. Among women, the only significant association with stimulated cytokines was between higher mean TST and higher stimulated IL-8. We found gender differences in the associations between sleep and inflammation in older adults, primarily in men and for stimulated (not circulating) cytokines. Interestingly, associations were also more frequently observed for sleep variability than mean levels.
A 6-month educational program improves sleep behaviour in community-dwelling frail older adults: A randomised controlled trial

Abstract:Background: The prevalence of sleep-related issues among older adults is a significant concern, with half of the older population reporting these problems. Consequently, strategies to improve sleep are needed for this population. This study aims to assess the effects of a health educational program on sleep behaviour among pre-frail or frail older adults residing in the community and to explore possible associations with frailty. Methods: This randomised controlled trial (NCT05610605) included a total of 197 community-dwelling older adults with frailty/pre-frailty, divided into control (n = 88) and educational (n = 109) groups, were assessed at baseline, after the 6-month educational program (6 months), and 6 months after the intervention (12 months). The intervention comprised four group sessions and six follow-up phone calls, focusing on frailty, physical activity, dietary habits, and cognitive training. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI) and wrist-worn accelerometry. Results: At 6 months, a significant time-by-group interaction was found for self-reported [β = -0.449, 95%CI (-0.844, -0.053), p = 0.026] and accelerometer-measured [β = 0.505, 95%CI (0.085, 0.926), p = 0.019] sleep efficiency, showing improved sleep efficiency in the intervention group vs. controls. A significant time-by-group interaction at 6 months was noted for sleep awakenings [β = -0.402, 95%CI (-0.825, -0.020), p = 0.047]. The educational program led to a significant decrease in awakenings, while the control group experienced an increase. The change in the number of awakenings (Rs = 0.183, p = 0.020) at 6 months was significantly associated with changes in frailty. Moreover, a significant time-by-group interaction was reported at the 12-month assessment [β = -0.449, 95%CI (-0.844, -0.053), p = 0.026] for self-reported sleep quality, indicating better results in the intervention group compared to controls. Conclusion: The educational program improved sleep quality and sleep efficiency while reducing the number of awakenings per night among community-dwelling frail older adults, offering a practical approach to addressing sleep-related challenges in this demographic.

Abstract:Abstract Introduction Sleep deficiencies, such as manifested in short sleep duration or insomnia symptoms, are known to increase the risk for multiple chronic diseases. Inflammation is considered a mechanism through which deficient sleep acts as risk factor for these diseases. Thus, mitigating inflammation might be a potential way to diminish negative health consequences related to sleep deficiency. To investigate a pharmacological approach for this, we used low-dose acetylsalicylic acid (ASA, aspirin), known for its counter-inflammatory actions including the cyclooxygenase (COX)-, NF-kB-, and resolution-pathways. Our aim was to investigate whether low-dose ASA can blunt the pro-inflammatory response to experimental sleep restriction. Methods 46 healthy adults (19F/27M, 19-63 years) participated in a randomized placebo-controlled crossover trial with 3 protocols each consisting of a 14-day at-home phase followed by an 11-day (10-night) in-hospital stay (sleep restriction/ASA, sleep restriction/placebo, control sleep/placebo). In the sleep restriction/ASA condition, participants took low-dose ASA (81 mg/day) during the at-home phase and in-hospital stay. Each in-hospital stay started with 2 nights of 8h-sleep opportunity. Then, under the sleep restriction conditions, participants were exposed to 5 nights of 4h-sleep opportunity, followed by 3 nights of recovery sleep (8h/night). The control sleep condition provided 8h-sleep opportunity throughout the in-hospital stay. Sleep and immunologic/hematologic measures were assessed at baseline, after the 5th night of sleep restriction/control sleep, and after the 2nd night of recovery sleep. Generalized linear mixed models were used for analysis. Results Administration of low-dose ASA reduced interleukin (IL)-6 expression (p<.05 for condition*day) and COX-1/COX-2 double positive cells in lipopolysaccharide (LPS)-stimulated monocytes (p<.05 for condition) as well as C-reactive protein (CRP) serum levels (p<.01 for condition) in the sleep restriction condition compared to placebo. Baseline comparisons revealed no differences between conditions (p>.05 for condition). Conclusion The results show that preemptive administration of low-dose ASA can reduce pro-inflammatory responses to experimental sleep restriction in humans. These findings may open new therapeutic approaches to prevent or control inflammation and its consequences in those experiencing periods of sleep deficiency. Mindfulness-based therapy for insomnia for older adults with sleep difficulties: a randomized clinical trial

Abstract:Objective: Poor sleep is a modifiable risk factor for multiple disorders. Frontline treatments (e.g. cognitive-behavioral therapy for insomnia) have limitations, prompting a search for alternative approaches. Here, we compare manualized Mindfulness-Based Therapy for Insomnia (MBTI) with a Sleep Hygiene, Education, and Exercise Program (SHEEP) in improving subjective and objective sleep outcomes in older adults. Methods: We conducted a single-site, parallel-arm trial, with blinded assessments collected at baseline, post-intervention and 6-months follow-up. We randomized 127 participants aged 50-80, with a Pittsburgh Sleep Quality Index (PSQI) score ⩾5, to either MBTI (n = 65) or SHEEP (n = 62), both 2 hr weekly group sessions lasting 8 weeks. Primary outcomes included PSQI and Insomnia Severity Index, and actigraphy- and polysomnography-measured sleep onset latency (SOL) and wake after sleep onset (WASO). Results: Intention-to-treat analysis showed reductions in insomnia severity in both groups [MBTI: Cohen's effect size d = -1.27, 95% confidence interval (CI) -1.61 to -0.89; SHEEP: d = -0.69, 95% CI -0.96 to -0.43], with significantly greater improvement in MBTI. Sleep quality improved equivalently in both groups (MBTI: d = -1.19; SHEEP: d = -1.02). No significant interaction effects were observed in objective sleep measures. However, only MBTI had reduced WASOactigraphy (MBTI: d = -0.30; SHEEP: d = 0.02), SOLactigraphy (MBTI: d = -0.25; SHEEP: d = -0.09), and WASOPSG (MBTI: d = -0.26; SHEEP (d = -0.18). There was no change in SOLPSG. No participants withdrew because of adverse effects. Conclusions: MBTI is effective at improving subjective and objective sleep quality in older adults, and could be a valid alternative for persons who have failed or do not have access to standard frontline therapies.
