Abstract:Abstract Introduction We determined whether slow wave sleep is associated with plasma levels of Aβ40, Aβ42, Aβ42/Aβ40, Tau, tau/Aβ42 and NfL and whether this relationship differed between Blacks/African-Americans and non-Hispanic Whites. Methods This was a cross-sectional analysis of baseline data from 171 community-dwelling cognitively normal older-adults, participating in ongoing NYU studies on memory, sleep and aging. Non-rapid eye movement sleep (NREM) slow wave sleep (SWS) duration was calculated from 2 nights of in-lab NPSGs. Plasma Aβ40, Aβ42, Tau and NfL were determined using single molecule array (SIMOA). Associations of NREM SWS duration and plasma AD biomarker levels were assessed using adjusted generalized linear models and Pearson correlation analysis after data normalization. Analyses were adjusted for age, sex, BMI, race, and education. Results Of the 171 subjects (128 non-Hispanic Whites and 43 Blacks/African-Americans), 112 (65.5%) were females, and mean (SD) age was 68.6 (6.6) years, BMI was 27.6 (6.1) kg/m**2, and education was 16.9 (2.1). There were no racial differences in age, sex, BMI, NREM SWS and AHI4%. Compared to non-Hispanic Whites, Blacks/African-Americans had significantly lower years of education (14.2 vs. 17.2, p <.01), plasma Aβ40 (248.3 vs. 262.5 pg/ml) and NfL levels (11.4 vs. 15.2 pg/ml) p<.05 for both. There were no significant racial differences in levels of plasma Aβ42, Aβ42/Aβ40, Tau, Tau/Aβ40 and Tau/Aβ42 (p>.05 for all). NREM SWS duration was not associated with plasma Aβ42, Aβ40 or tau in the overall sample (p>.05 for all). However, in non-Hispanic Whites, NREM SWS negatively correlated with plasma Aβ42 (r=-0.28, p<=0.05), plasma Aβ40 (r=-0.087, p=0.72), though not significant, and plasma Tau (r=-0.153, p=0.27), though not significant. In Black/African-Americans, NREM SWS positively correlated with plasma Aβ42 (r=0.48, p=0.05), plasma Aβ40 levels (r=0.32, p=0.04), and plasma Tau levels (r=0.52, p=0.04). NREM SWS was not associated with plasma tau/Aβ42, plasma tau/Aβ40 or plasma NfL in the overall sample and across racial subgroups. Conclusion Race-specific divergent associations between NREM SWS and plasma Aβ42, Aβ40 & Tau may suggest differences in SDOH factors and mechanisms that could influence sleep and AD-risk in older-adults. Abstract:Abstract Introduction Sleep is regulated by the microbiota-gut-brain axis, which plays a critical role in amyloid beta (Aβ) production and clearance; two key processes in the progression of sleep-related Alzheimer’s disease (SRAD). C-reactive protein (CRP) is an inflammatory biomarker correlated with Aβ levels in SRAD. Although Blacks are disproportionately affected by SRAD, a paucity of knowledge exists regarding effective early detection and prevention of this disease in this population. This study investigated the levels of CRP in Black participants with subjective Sleep apnea (SA) to determine what indicators or thresholds may serve as a risk predictive marker for Aβ blood biomarker screening for SRAD. Methods Blood from a total of 104 Blacks enrolled in two NIH-funded community-based sleep studies, ESSENTIAL and MOSAIC, was collected from January 2020 until October 2023. CRP levels were accessed according to clinical standards in ranges of normal or minor elevation, moderate elevation, marked elevation, and severe elevation. Frequency distributions and analyses were performed using SPSS29. Results Twenty-one participants self-reported an existing SA diagnosis; of which 19% had CRP levels at 0.3 - 1.0 mg/dL, 62% at 1.0 - 10.0 mg/dL, 19% at >10.0 mg/dL, and none at >50.0 mg/dL. Of the 83 participants without SA, 31% had CRP levels at 0.3 - 1.0 mg/dL, 61% at 1.0 - 10.0 mg/dL, 7% at >10.0 mg/dL, and 1% at >50.0 mg/dL. In total, 81% of participants with subjective SA had CRP levels above minor elevation, while 69% of participants without subjective SA had levels above minor elevation, showing a 10% difference in elevated CRP levels between both groups. These results suggest that SA can lead to greater levels of inflammation among Blacks. Conclusion A 10% difference in elevated CRP levels of participants with and without SA diagnosis may suggest that undiagnosed participants could be at risk for SA. Identifying factors that predict blood Aβ levels such as CRP may lead to a cost-effective, SRAD specific prescreening panel to identify patients at higher risk of SRAD, and aid risk stratification in Blacks. Abstract:Abstract Blacks/African‐Americans (blacks) take longer to fall asleep, have lower sleep efficiency and spend a smaller proportion of time in deep stages of non‐rapid eye movement (NREM) slow wave sleep/activity [SWS]), relative to non‐Hispanic whites (whites). Notably, blacks have a higher burden of severe obstructive sleep apnea [OSA] with excessive daytime sleepiness (EDS). These sleep metrics have been linked to increased vascular risks, inflammation, brain injury, and cognitive impairment, all known risk factors for Alzheimer’s disease (AD). This presentation will provide an update on putative mechanisms that underlie the association between sleep and risk of developing AD, both from a mechanistic (primarily preclinical) as well as a clinical/epidemiological perspective, with a focus on how disturbed sleep might be a race‐dependent physiologic risk conferring increased AD‐risk in blacks. It is vital to examine how sociocultural factors, often through behavioral processes, become physiologically expressed as increased AD‐risk in blacks. Data presented will include cross‐sectional and longitudinal studies. Utilizing data from various cohorts including NYU cohort of older‐adults participating in studies of sleep, aging and memory, and the National Alzheimer’s Coordinating Center, we will describe novel evidence implicating sleep health disparities [SHD] (i.e., shorter sleep duration [SD], smaller proportion of time in deep stages of NREM SWS/SWA and disproportionate burden of symptomatic OSA, that are more prevalent among racial/ethnic minorities, as a risk factor for AD; as a physiologic marker associated with AD pathology/biomarkers (i.e. amyloid and tau), and as a modifier of AD‐risk. We will describe race and sex specific mechanisms underlying SHD‐AD risk, including independent and synergistic effects with Aβ and/or tau, as well as vascular risk. Race‐specific associations of micro and macro‐architectural sleep parameters with novel plasma AD biomarkers will also be presented. Findings will show that social determinants of health factors mediate heterogeneity in sleep and AD outcomes, with Black subjects having worse SHD metrics i.e., shorter SD, reduced NREM SWS/SWA and greater OSA severity, and exhibiting worse race‐specific AD pathology and CVD risk markers (i.e., worse office‐based Framingham Heart Study general CVD [FHS‐CVD] risk scores, & greater WML and abnormal DKI indices) potentially leading to AD disparities.

Abstract:Abstract Introduction Growing evidence suggests a bi-directional association between sleep and Alzheimer’s disease (AD), with several studies showing an association between self-reported sleep disturbances and AD biomarkers. However, the association between polysomnography (PSG)-assessed sleep architecture and AD biomarkers remains unknown. We aim to investigate the associations between sleep architecture features and AD biomarkers in a clinical population with and without cognitive impairment. Methods We examined 128 participants [mean age of 70.9 years (±9.7), 43.8% of men] from an outpatient memory clinic, of whom 23 were healthy controls, 41 patients with mild cognitive impairment, and 64 patients with AD. Sleep architecture features were derived from PSG data and categorized by tertiles. Four AD biomarkers were measured, including tau-181, Brain-Derived Neurotrophic Factor (BDNF), and neurofilament light (NFL) obtained from blood samples, along with Aβ levels measured from amyloid positron emission tomography scans. Multivariable linear regressions models were performed to assess the associations between sleep architecture features and AD biomarkers. Results After adjustment for age, gender, APOE4 status, diabetes mellitus, smoking habits, and body mass index, participants in the highest REM latency tertile (i.e., with longer REM latency; >192.7 minutes) were associated with increased levels of tau-181 (β= 0.25, 95% confidence interval (CI)= 0.01;0.55) and Aβ (β= 0.15, 95% CI= 0.06;0.26), and with decreased level of BDNF (β= -0.51, 95% CI= -0.70;-0.20) compared to participants in the lowest tertile (i.e., with shorter REM latency; < 98.2 minutes). Moreover, being in the middle SWS percentage tertile (i.e., SWS percentage between 2.5 and 10.2) was associated with a lower level of tau-181 (β= -0.18, 95% CI= -0.34;-0.02) compared to being in the lowest tertile (i.e., shorter SWS percentage; < 2.5)). We did not find any association between AD biomarkers and sleep duration, efficiency, latency, or other sleep architecture features. Conclusion In this cross-sectional study of a clinical population, we highlight the importance of REM latency, and to a lesser extent, SWS percentage, in relation to AD biomarkers. Future research is needed to examine the longitudinal association between sleep architecture and AD biomarkers and clarify underlying mechanisms. Support (if any)

Abstract:Abstract Study Objectives While short sleep could promote neurodegeneration, long sleep may be a marker of ongoing neurodegeneration, potentially as a result of neuroinflammation. The objective was to evaluate sleep patterns with age of expected Alzheimer's disease (AD) onset and neuroinflammation. Methods We tested 203 dementia-free participants (68.5±5.4y/o, 78M). The PREVENT-AD cohort includes older persons with a parental history of AD whose age was nearing their expected AD onset. We estimated expected years to AD onset by subtracting the participant’s age from their parent's at AD dementia onset. We extracted actigraphy sleep variables of interest (times of sleep onset and morning awakening, time in bed, sleep efficiency, sleep duration) and general profiles (sleep fragmentation, phase delay, hypersomnia). CSF inflammatory biomarkers were assessed with OLINK multiplex technology. Results Proximity to, or exceeding, expected age of onset was associated with a sleep profile suggestive of hypersomnia (longer sleep, later morning awakening time). This hypersomnia sleep profile was associated with higher CSF neuroinflammatory biomarkers (IL-6, MCP-1, global score). Interactions analyses revealed that some of these sleep-neuroinflammation associations were present mostly in those closer/exceeding the age of expected AD onset, APOE4 carriers, and those with better memory performance. Conclusions Proximity to, or exceeding, parental AD dementia onset was associated with a longer sleep pattern, which was related to elevated proinflammatory CSF biomarkers. We speculate that longer sleep may serve a compensatory purpose potentially triggered by neuroinflammation as individuals are approaching AD onset. Further studies should investigate whether neuroinflammatory-triggered long sleep duration could mitigate cognitive deficits.

Abstract:Objective: Poor sleep is associated with increased inflammation, thereby increasing the risk of chronic diseases and mortality. However, the effects of behavioral sleep interventions on the upstream inflammatory system are unknown among family care partners (CP). The present study explored the role of a behavioral sleep intervention program on inflammatory gene expression. Methods: This was part of a randomized controlled trial of a sleep intervention for dementia care dyads with sleep problems. Thirty dyads were randomized to sleep intervention or control groups. Sleep outcomes for CP were assessed with 1 week of actigraphy and sleep diary, and the Pittsburgh Sleep Quality Index. Other information included CP demographics, body mass index, and intensity of caregiving tasks. All outcomes were collected at baseline, post-treatment, and 3-month follow-up. Results: Neither group showed any significant differential changes in gene expression from baseline to post-treatment or 3-month follow-up. A decrease in inflammatory gene expression was significantly associated with more nights of good sleep (i.e. nights without trouble falling or staying asleep at night). This finding remained significant after controlling for group (intervention/control), timepoint (baseline, post-treatment, and 3-month follow-up), and CP characteristics (e.g. age and ethnicity). Conclusions: Although better sleep was associated with decreased inflammatory gene expression, this study did not demonstrate any benefits of a behavioral sleep intervention over control, most likely due to a small sample. Studies with larger sample sizes are needed to test the specific aspects of disturbed sleep that relate to inflammatory biology among CP of persons living with dementia.

Abstract:Abstract Introduction Short sleep increases Alzheimer’s disease (AD) risk, and sleep facilitates clearance of AD-related metabolites. Blood-based AD biomarkers (e.g., beta amyloid [Aβ] 42/40 ratio, phosphorylated tau181 [p-Tau181], neurofilament light chain [NfL], glial fibrillary acidic protein [GFAP]) are validated and increasingly used clinically. However, the impact of sleep deprivation and recovery sleep on these markers is unclear. This study determined the effects of sleep deprivation and recovery sleep on serum AD biomarkers in retired adults and explored: 1) whether changes in EEG slow-wave activity (SWA) from baseline to recovery sleep were associated with serum AD biomarker changes, and 2) whether findings differed between retired night shift and day workers. Methods Participants were 58 cognitively normal retired shift workers (n = 28) and retired day workers (n = 30). All completed a 60-hour laboratory study including a baseline night of EEG sleep recording, one night of total sleep deprivation, and one night of recovery sleep. EEG SWA (0.5-1Hz) was assessed during baseline and recovery sleep. Blood was collected the morning after each night. Serum samples were analyzed for Aβ40, Aβ42, p-Tau181, NfL, and GFAP using ultra-sensitive immunoassay. Linear mixed models determined the effects of sleep deprivation and recovery sleep on biomarker levels adjusted for age, sex, race/ethnicity, and education. Results Mean participant age was 67.8 +/- 5.5 years, 52% were female, 86% were White, and mean education was 16.0 +/- 1.9 years. Serum Aβ42/40 ratio, NfL, and GFAP decreased following one night of sleep deprivation. Following recovery sleep, the Aβ42/40 ratio remained below baseline, while NfL and GFAP increased to baseline levels. Serum pTau-181 did not change following sleep deprivation or recovery sleep. Greater SWA increase from baseline to recovery sleep was associated with smaller Aβ42/40 ratio decrease. Findings did not differ between shift and day work groups. Conclusion In retired adults, acute sleep deprivation and recovery sleep impacted serum AD biomarker levels. Sleep characteristics may therefore affect the diagnostic accuracy of these tests. Former night shift work exposure did not influence serum biomarker responses, suggesting no effect of long-term shift work exposure over time. SWA may aid Aβ clearance during sleep. Abstract:Introduction: In older adults, where sleep disturbances and cognitive impairment are common, mounting evidence suggests a potential connection between sleep and cognitive function, highlighting the significance of utilizing sleep as a biomarker for early detection of cognitive impairment to improve clinical outcomes in a noninvasive, cost-effective manner. Areas covered: This review describes the relationship between sleep and cognitive function in older adults, encompassing both subjective and objective measures of sleep quality, duration, architecture, and sleep-disordered breathing. The authors consider the directionality of the associations observed in prospective and cross-sectional studies, exploring whether sleep disturbances precede cognitive decline or vice versa. Furthermore, they discuss the potential bidirectional relationships between sleep and Alzheimer's disease (AD) risks in older adults while also examining the neurodegenerative pathways of this relationship. Expert opinion: Routine sleep monitoring in primary care settings has the potential to bolster early detection and treatment of sleep disturbance, and by extension, reduce the risk of dementia. Improving sleep assessment tools, such as wearables, provide scalable alternatives to traditional methods like polysomnography, potentially enabling widespread monitoring of sleep characteristics. Standardized measurement and inclusive participant recruitment are needed to enhance generalizability, while longitudinal studies are essential to understand the interaction between sleep and AD pathology.

Abstract:Abstract Introduction Impairments in the microstructure of the limbic tracts are seen in early stages of the Alzheimer’s disease (AD) spectrum. Disrupted sleep has been linked with AD. We examined associations between measures of limbic white matter tracts and objective sleep parameters in cognitively unimpaired older adults. Methods This cross-sectional study included 170 community-dwelling cognitively unimpaired older adults (mean±SD: age=67.2±5.2y) participating in NYU studies of sleep, aging, and memory. Subjects completed polysomnography (NPSG) and brain MRI. Sleep measures of interest included total sleep time (TST), NREM stages 2, 3 and REM sleep duration, sleep efficiency and fragmentation, and AHI4% and REM AHI4%. Microstructural properties of the cingulum, uncinate fasciculus (UF), and fornix were estimated using diffusional tensor imaging (DTI) metrics including radial kurtosis (RK), radial diffusivity (RD), and fractional anisotropy (FA). Linear mixed-effects regression models examined associations between sleep variables and DTI metrics. Models were adjusted for age, race, education, sex, time between NPSG and MRI. Results Participants were 71.2% female, 44.1% Black/African-American, and had 16.8±2.5y of education. In the right cingulum, higher AHI4% in REM was associated with decreased RK(β[IV]= β[SE}; β[AHI4%] =-0.0014[0.00054], p=0.01) and shorter stage 2 sleep duration was associated with decreased RK (β [stage 2 sleep duration] =0.00062[0.00026], p=0.02). In both hemispheres of the cingulum, increased sleep fragmentation in stage 3 was associated with increased RD (β [sleep fragmentation, right] = 0.0017[0.00080], p=0.03; β [sleep fragmentation, left] = 0.0026[0.00087], p=0.003), and higher AHI4% in REM was associated with reduced FA (β [AHI4% left] =-0.00039[0.00019], p=0.048; β [AHI4% right] =-0.00055[0.00017], p=0.02). In the right UF, with increasing age, reduced TST was associated with lower RK(β[TST]=0.0000047[0.000002], p=0.02). In the right UF, reduced time in REM sleep was associated with increased RD (β [REM duration] =-0.0022[0.00078], p=0.006). In the fornix, higher AHI4% was associated with lower FA(β[AHI4%] =-0.00061[0.00029], p=0.04). Conclusion In this sample of community-dwelling cognitively unimpaired older adults, reduced sleep duration and OSA severity (NREM and REM) were associated with limbic white matter tracts alterations. Longitudinal studies are needed to investigate the role of disrupted sleep in limbic white matter microstructure alterations in early stages of the AD spectrum. Associations between self-reported sleep patterns and health, cognition and amyloid measures: results from the Wisconsin Registry for Alzheimer's Prevention

Abstract:Previous studies suggest associations between self-reported sleep problems and poorer health, cognition, Alzheimer's disease pathology and dementia-related outcomes. It is important to develop a deeper understanding of the relationship between these complications and sleep disturbance, a modifiable risk factor, in late midlife, a time when Alzheimer's disease pathology may be accruing. The objectives of this study included application of unsupervised machine learning procedures to identify distinct subgroups of persons with problematic sleep and the association of these subgroups with concurrent measures of mental and physical health, cognition and PET-identified amyloid. Dementia-free participants from the Wisconsin Registry for Alzheimer's Prevention (n = 619) completed sleep questionnaires including the Insomnia Severity Index, Epworth Sleepiness Scale and Medical Outcomes Study Sleep Scale. K-means clustering analysis identified discrete sleep problem groups who were then compared across concurrent health outcomes (e.g. depression, self-rated health and insulin resistance), cognitive composite indices including episodic memory and executive function and, in a subset, Pittsburgh Compound B PET imaging to assess amyloid burden. Significant omnibus tests (P < 0.05) were followed with pairwise comparisons. Mean (SD) sample baseline sleep assessment age was 62.6 (6.7). Cluster analysis identified three groups: healthy sleepers [n = 262 (42.3%)], intermediate sleepers [n = 229 (37.0%)] and poor sleepers [n = 128 (20.7%)]. All omnibus tests comparing demographics and health measures across sleep groups were significant except for age, sex and apolipoprotein E e4 carriers; the poor sleepers group was worse than one or both of the other groups on all other measures, including measures of depression, self-reported health and memory complaints. The poor sleepers group had higher average body mass index, waist-hip ratio and homeostatic model assessment of insulin resistance. After adjusting for covariates, the poor sleepers group also performed worse on all concurrent cognitive composites except working memory. There were no differences between sleep groups on PET-based measures of amyloid. Sensitivity analyses indicated that while different clustering approaches resulted in different group assignments for some (predominantly the intermediate group), between-group patterns in outcomes were consistent. In conclusion, distinct sleep characteristics groups were identified with a sizable minority (20.7%) exhibiting poor sleep characteristics, and this group also exhibited the poorest concurrent mental and physical health and cognition, indicating substantial multi-morbidity; sleep group was not associated with amyloid PET estimates. Precision-based management of sleep and related factors may provide an opportunity for early intervention that could serve to delay or prevent clinical impairment.

Abstract:Introduction: Sleep is crucial for memory consolidation and the clearance of toxic proteins associated with Alzheimer's disease (AD). We examined the association between sleep characteristics and imaging biomarkers of early amyloid beta (Aβ) and tau pathology as well as neurodegeneration in brain regions known to be affected in the incipient stages of AD. Methods: Thirty-nine cognitively unimpaired (CU) participants of the Harvard Aging Brain Study underwent at-home polysomnography as well as tau positron emission tomography (flortaucipir-PET), amyloid PET (Pittsburgh compound B [PiB]-PET), and magnetic resonance imaging-derived assessment of cortical thickness (CT). Results: Increased N1 sleep was associated with a higher tau PET signal (β = 0.009, p = 0.001) and lower CT in the temporal composite region of interest (β = -0.017, p = 0.007). Decreased slow-wave sleep (SWS) was associated with higher tau burden in the temporal composite (β = -0.008, p = 0.005) and lower CT (β = 0.008, p = 0.002), even after controlling for global PiB-PET. Discussion: In CU older adults, lower SWS and higher N1 sleep were associated with higher tau burden and lower CT in brain regions associated with early tau deposition and vulnerable to AD-related neurodegeneration through mechanisms dissociable from amyloid deposition. Highlights: We report the results of an observational study, which leveraged -a well-characterized cohort of healthy aging (Harvard Aging Brain Study) by adding in-home full polysomnograms.By adding at-home polysomnograms to this unique and deeply phenotyped cohort, we examined variations in sleep architecture that are associated with Alzheimer's disease (AD) pathologic changes.Our results confirmed the association of sleep changes with early tau and cortical neurodegenerative changes that were independent of amyloid.The results will be of importance in monitoring sleep-related variations in relation to the natural history of AD pathology and in designing sleep-focused clinical trials.

Abstract:Abnormal sleep duration, as defined as ≤7or ≥9 hours nightly is associated with cognitive dysfunction. During sleep, blood pressure should dip between 10% to 20%; dipping patterns outside of this norm has been associated with poor cognition. Here, we examine the potential association between sleep duration, nighttime dipping patterns, and cognitive test performance in adult children with a parental history of Alzheimer's disease (AD), cross-sectionally. Participants completed 1 hour of cognitive testing, consisting of domains of global cognition, executive function, and verbal and visuospatial ability. Sleep patterns were collected through a validated Sleep Questionnaire. Participants wore a 24 hour ambulatory blood pressure monitor. Z-scores were computed for each test. Multivariate analysis of covariance was used to assess the relationship between sleep duration and cognition controlling for age, gender, race, education, and ApoE4 status. The interaction of sleep duration and nighttime dipping pattern was included to examine whether nighttime dipping pattern modified the effect of sleep duration on cognitive performance. Participants (n=80) were middle-age (59.1 ±6.8 years), 66.3% female, and 37.5% African-American.47.5% were ApoE ɛ4 positive. Average sleep duration was 6.8 (±1.1) hours and 21.3% had a normal nighttime dipping pattern. Abnormal sleep duration (≤7 or ≥9 hours nightly) and abnormal nighttime dipping (≤10 or ≥20%) pattern were significantly related to verbal memory, and the relationship between sleep duration and verbal memory was modified by nighttime dipping patterns (p=0.0010 for the interaction). Stratified analyses based on the dipping patterns showed that for abnormal dippers, but not for normal dippers, verbal memory was impaired in those with abnormal sleep duration (p=0.0272). Sleep duration and nighttime dipping patterns were related to verbal memory; such that poor sleep is related to impaired verbal memory in individuals with abnormal nighttime dipping patterns (less than 10% or greater than 20%). Nighttime dipping patterns play an important role in the relationship between sleep problems and verbal memory. Healthcare providers should consider assessing sleep patterns and completing ambulatory screenings to ascertain nighttime sleep and dipping patterns, for adults with risk factors for AD, to mitigate potential risk factors for cognitive dysfunction.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is common in older adults and has recently been implicated in the pathogenesis of Alzheimer’s disease (AD). Sleep disruption is a possible reversible cause of memory impairment. Furthermore, quality of sleep has been determined to be one of the most important variables affecting overnight memory consolidation. As part of a larger study, data are presented evaluating correlations between markers of sleep apnea severity and sleep-dependent memory consolidation. Methods Seventy-one older adults ages 65-83 (M=70, SD=4.1) with normal cognition underwent polysomnography (PSG) and completed sleep questionnaires as part of a larger study. The sample was 54.9% women, 83.1% White, and 91.5% Non-Hispanic. Severity of sleep apnea was assessed using several metrics: Daytime sleepiness measured by the Epworth Sleepiness Scale (ESS), apnea-hypopnea index (AHI) based on American Academy of Sleep Medicine recommended, Total Sleep Time with Oxygen Saturation below 90% (T90), the lowest level of oxygen saturation reached during sleep (SPO2 Nadir), and hypoxic burden. Sleep-dependent memory consolidation was assessed using the Word-pairs Association (WPA) task, which shows semantically related word pairs for 5 seconds for the participant to learn, and the next morning, post-sleep, the delayed recognition test is performed. Results Descriptive analyses showed a median daytime sleepiness of 7 (SD=3.1), AHI of 28 (SD=24.3), and body mass index of 27 (SD=7.1). Nonparametric correlations were performed to assess markers of sleep apnea severity as correlates of sleep-dependent memory consolidation. There were no significant associations between objective measures of OSA severity, particularly AHI, hypoxic burden, T90, and SPO2 Nadir. However, there was a significant negative association between subjective daytime sleepiness (ESS) and sleep-dependent memory consolidation (r=-0.30, p=0.01). Conclusion This study provides preliminary insights into the association between daytime sleepiness and sleep-dependent memory consolidation. The observed finding that lower daytime sleepiness is related to better sleep-dependent memory consolidation warrants further investigation into the underlying mechanisms and to assess whether alternate cognitive measures may be associated with objective severity metrics. Future research should assess relations with psychological factors, such as mood and coping, as these factors can impact memory and are not captured by PSG. Abstract:IMPORTANCE Older adults commonly report disturbed sleep, and recent studies in humans and animals suggest links between sleep and Alzheimer disease biomarkers. Studies are needed that evaluate whether sleep variables are associated with neuroimaging evidence of β-amyloid (Aβ) deposition. OBJECTIVE To determine the association between self-reported sleep variables and Aβ deposition in community-dwelling older adults. DESIGN, SETTING, AND PARTICIPANTS Cross-sectional study of 70 adults (mean age, 76 [range, 53-91] years) from the neuroimaging substudy of the Baltimore Longitudinal Study of Aging, a normative aging study. EXPOSURE Self-reported sleep variables. MAIN OUTCOMES AND MEASURES β-Amyloid burden, measured by carbon 11-labeled Pittsburgh compound B positron emission tomography distribution volume ratios (DVRs). RESULTS After adjustment for potential confounders, reports of shorter sleep duration were associated with greater Aβ burden, measured by mean cortical DVR (B = 0.08 [95% CI, 0.03-0.14]; P = .005) and precuneus DVR (B = 0.11 [0.03-0.18]; P = .007). Reports of lower sleep quality were associated with greater Aβ burden measured by precuneus DVR (B = 0.08 [0.01-0.15]; P = .03). CONCLUSIONS AND RELEVANCE Among community-dwelling older adults, reports of shorter sleep duration and poorer sleep quality are associated with greater Aβ burden. Additional studies with objective sleep measures are needed to determine whether sleep disturbance causes or accelerates Alzheimer disease.

Abstract:The neuropathological mechanisms underlying the association between sleep duration and mild cognitive impairment remain poorly understood. This population-based study included 2032 dementia-free people (age ≥ 60 years; 55.1% women) derived from participants in the Multimodal Interventions to Delay Dementia and Disability in Rural China; of these, data were available in 841 participants for Alzheimer's plasma biomarkers (e.g. amyloid-β, total tau and neurofilament light chain), 1044 for serum microvascular biomarkers (e.g. soluble adhesion molecules) and 834 for brain MRI biomarkers (e.g. whiter matter, grey matter, hippocampus, lacunes, enlarged perivascular spaces and white matter hyperintensity WMH). We used electrocardiogram-based cardiopulmonary coupling analysis to measure sleep duration, a neuropsychological test battery to assess cognitive function and the Petersen's criteria to define mild cognitive impairment. Data were analysed with multivariable logistic and general linear models. In the total sample (n = 2032), 510 participants were defined with mild cognitive impairment, including 438 with amnestic mild cognitive impairment and 72 with non-amnestic mild cognitive impairment. Long sleep duration (>8 versus 6-8 h) was significantly associated with increased likelihoods of mild cognitive impairment and non-amnestic mild cognitive impairment and lower scores in global cognition, verbal fluency, attention and executive function (Bonferroni-corrected P < 0.05). In the subsamples, long sleep duration was associated with higher plasma amyloid-β40 and total tau, a lower amyloid-β42/amyloid-β40 ratio and smaller grey matter volume (Bonferroni-corrected P < 0.05). Sleep duration was not significantly associated with serum-soluble adhesion molecules, white matter hyperintensity volume, global enlarged perivascular spaces and lacunes (P > 0.05). Alzheimer's and neurodegenerative pathologies may represent common pathways linking long sleep duration with mild cognitive impairment and low cognition in older adults.

Abstract:BACKGROUND: A growing body of literature has suggested that obstructive sleep apnea (OSA) and habitual short sleep duration are linked to poor cognitive function. Neuroimaging studies may provide insight into this relation.OBJECTIVE: We tested the hypotheses that OSA and habitual short sleep duration, measured at ages 54-73 years, would be associated with adverse brain morphology at ages 67-89 years.METHODS: Included in this analysis are 312 ARIC study participants who underwent in-home overnight polysomnography in 1996-1998 and brain MRI scans about 15 years later (2012-2013). Sleep apnea was quantified by the apnea-hypopnea index and categorized as moderate/severe (≥15.0 events/hour), mild (5.0-14.9 events/hour), or normal (<5.0 events/hour). Habitual sleep duration was categorized, in hours, as <7, 7 to <8, ≥8. MRI outcomes included number of infarcts (total, subcortical, and cortical) and white matter hyperintensity (WMH) and Alzheimer's disease signature region volumes. Multivariable adjusted logistic and linear regression models were used. All models incorporated inverse probability weighting, to adjust for potential selection bias.RESULTS: At the time of the sleep study participants were 61.7 (SD: 5.0) years old and 54% female; 19% had moderate/severe sleep apnea. MRI imaging took place 14.8 (SD: 1.0) years later, when participants were 76.5 (SD: 5.2) years old. In multivariable models which accounted for body mass index, neither OSA nor abnormal sleep duration were statistically significantly associated with odds of cerebral infarcts, WMH brain volumes or regional brain volumes.CONCLUSIONS: In this community-based sample, mid-life OSA and habitually short sleep duration were not associated with later-life cerebral markers of vascular dementia and Alzheimer's disease. However, selection bias may have influenced our results and the modest sample size led to relatively imprecise associations.

Abstract:Introduction: Sleepiness has been associated with cognitive decline and dementia in the elderly. Older adults with excessive daytime sleepiness appear to be more vulnerable to longitudinal amyloid PET accumulation before the onset of the dementia. However, it remains unclear whether sleepiness is similarly associated with other biomarkers of Alzheimer's disease (AD), axonal integrity, and inflammation, which may also contribute to neurodegeneration and cognitive decline. Methods: In this cross-sectional analysis, we identified 260 cognitively unimpaired adults (>60 years) from the Mayo Clinic Study of Aging, a population-based cohort from Olmsted County (MN), who underwent CSF quantification of AD biomarkers (Aβ42, p-tau, p-tau/Aβ42) in addition to at least one of the following biomarkers [neurofilament light chain (NfL) interleukin-6 (IL-6), IL-10, and tumor necrosis factor-α (TNF-α)]. We fit linear regression models to assess associations between sleepiness, as measured by the Epworth Sleepiness Scale (ESS), and CSF biomarkers, controlling for age, sex, APOε4 status, body mass index, hypertension, dyslipidemia, and prior diagnosis of obstructive sleep apnea. Results: Higher ESS scores were associated with higher CSF IL-6 and NfL, but not with the other CSF biomarkers. For every ESS score point increase, there was a 0.009 ([95% CI 0.001-0.016], p = 0.033) increase in the log of IL-6 and 0.01 ([95% CI 0.002-0.018], p = 0.016) increase in the log of NfL. A sensitivity analysis showed an association between ESS scores and log of p-tau/Aβ42 only in participants with an abnormal ratio (>0.023), highly predictive of amyloid positivity. For every ESS score point increase, there was a 0.006 ([95% CI 0.001-0.012], p = 0.021) increase in the log of CSF p-tau/Aβ42. Conclusion: Sleepiness was associated with greater CSF IL-6 and NfL levels, which could contribute to neurodegeneration or alternatively cause sleepiness. Higher NfL levels may result from sleep disruption and/or contribute to sleepiness via disturbed connectivity or damage to wake-promoting centers. Associations between sleepiness and p-tau/Aβ42 in participants with abnormal ratio suggest that amyloid positivity contributes to vulnerability to sleep disturbance, which may further amyloid accumulation in a feed-forward loop process. Prospective studies of these markers are needed to determine cause-effect relationships between these associations.
High-Quality Sleep Mitigates ABCA7-Related Generalization Deficits in Healthy Older African Americans

Abstract:Background: Both sleep deficiencies and Alzheimer's disease (AD) disproportionately affect older African Americans. Genetic susceptibility to AD further compounds risk for cognitive decline in this population. Aside from APOE ɛ4, ABCA7 rs115550680 is the strongest genetic locus associated with late-onset AD in African Americans. While sleep and ABCA7 rs115550680 independently influence late-life cognitive outcomes, we know too little about the interplay between these two factors on cognitive function. Objective: We investigated the interaction between sleep and ABCA7 rs115550680 on hippocampal-dependent cognitive function in older African Americans. Methods: One-hundred fourteen cognitively healthy older African Americans were genotyped for ABCA7 risk (n = 57 carriers of risk "G" allele; n = 57 non-carriers), responded to lifestyle questionnaires, and completed a cognitive battery. Sleep was assessed via a self-reported rating of sleep quality (poor, average, good). Covariates included age and years of education. Results: Using ANCOVA, we found that carriers of the risk genotype who reported poor or average sleep quality demonstrated significantly poorer generalization of prior learning-a cognitive marker of AD-compared to their non-risk counterparts. Conversely, there was no genotype-related difference in generalization performance in individuals who reported good sleep quality. Conclusion: These results indicate that sleep quality may be neuroprotective against genetic risk for AD. Future studies employing more rigorous methodology should investigate the mechanistic role of sleep neurophysiology in the pathogenesis and progression of AD associated with ABCA7. There is also need for the continued development of non-invasive sleep interventions tailored to racial groups with specific AD genetic risk profiles.
Resting-state Functional Connectivity Changes in Older Adults with Sleep Disturbance and the Role of Amyloid Burden

Abstract:Sleep and related disorders could lead to changes in various brain networks, but little is known about the role of amyloid β (Aβ) burden-a key Alzheimer's disease (AD) biomarker-in the relationship between sleep disturbance and altered resting state functional connectivity (rsFC) in older adults. This cross-sectional study examined the association between sleep disturbance, Aβ burden, and rsFC using a large-scale dataset from the Alzheimer's Disease Neuroimaging Initiative (ADNI). Sample included 489 individuals (53.6% cognitively normal, 32.5% mild cognitive impairment, and 13.9% AD) who had completed sleep measures (Neuropsychiatric Inventory), PET Aβ data, and resting-state fMRI scans at baseline. Within and between rsFC of the Salience (SN), the Default Mode (DMN) and the Frontal Parietal network (FPN) were compared between participants with sleep disturbance versus without sleep disturbance. The interaction between Aβ positivity and sleep disturbance was evaluated using linear regressions, controlling for age, diagnosis status, gender, sedatives and hypnotics use, and hypertension. Although no significant main effect of sleep disturbance was found on rsFC, a significant interaction term emerged between sleep disturbance and Aβ burden on rsFC of SN (β=0.11, P=0.006). Specifically, sleep disturbance was associated with SN hyperconnectivity, only with the presence of Aβ burden. Sleep disturbance may lead to altered connectivity in the SN when Aβ is accumulated in the brain. Individuals with AD pathology may be at increased risk for sleep-related aberrant rsFC; therefore, identifying and treating sleep problems in these individuals may help prevent further disease progression.
