Abstract:Objectives: To assess the diagnostic accuracy of radiographs in identifying humeral intracondylar fissures in dogs. Materials and methods: Clinical data from dogs undergoing CT and radiographic elbow investigations at a single institution were reviewed. Based on CT diagnosis, radiographs were classified as having fissures and not having fissures. The radiographic images were evaluated by three blinded observers for the presence/absence of a fissure, type (partial or complete), and for secondary signs that could suggest the presence of a fissure in case this was not visible. Results: A total of 81 radiographs (76 elbows) were reviewed. Based on CT findings, 37 elbows had fissures and 44 did not. Radiographic interpretation demonstrated a sensitivity of 47.7% (confidence interval 95%: 0.40 to 0.59) and specificity of 85.6% (confidence interval 95%: 0.72 to 0.93) in correctly identifying humeral intracondylar fissures. The positive predictive value was 75.5% (confidence interval 95%: 0.64 to 0.84) and the negative predictive value was 66.1% (confidence interval 95%: 0.63 to 0.68). Agreement with CT images was <0.4, and interobserver and intraobserver agreements were <0.8. The radiographic orientation and type of fissure did not significantly affect interpretation outcomes. Clinical significance: Radiographic imaging alone is insufficient for reliably diagnosing humeral intracondylar fissures in dogs.

Abstract:Postoperative pain is a clinically relevant issue in orthopedic patients, affecting more than 40% 1 year after foot and ankle surgery. Because of the very complex anatomy with many different joints and several motion axes, clinical examination and conventional imaging are sometimes not sufficient to identify a local pain generator. Local uptake of bone-seeking radiopharmaceuticals is known to correlate accurately with sites of pain generating foci and, thus, bone scintigraphy has been an established method to evaluate these respective patients for many years. However, the specificity is rather low if only planar images are acquired. The development of SPECT and especially of hybrid SPECT and CT imaging has significantly enhanced the specificity of this technique. The combination of both functional and morphological imaging, ideally performed with a dedicated SPECT/CT system to minimize misregistration owing to motion artifacts and to enhance image quality by attenuation correction, allows an early and reliable detection of pathologic bone processes, even in patients where radiological imaging with MRI or CT is hampered by metal implants. In diabetic patients with a neuropathic Charcot osteoarthropathy, infection can be differentiated from inflammatory bone alterations (causing bone marrow edema) almost certainly using SPECT/CT with radiolabeled white blood cells and antigranulocyte antibodies, allowing an individual and precise treatment planning either in the initial course of the disease or even after surgery. This article reviews the most frequent clinical challenges in patients after foot and ankle surgery, including a description of the various surgical procedures, the different imaging options with their advantages and disadvantages, and aims to integrate bone SPECT/CT into the clinical diagnostic workup.
A specific oblique radiography, and treatment, of a medial coronoid process fracture

Abstract:Acute traumatic fracture of the medial coronoid process is rarely reported and has previously only been described in three dogs to date. A three-year-old Schapendoes was presented for an acute non weight-bearing lameness. The definitive diagnosis of a medial coronoid process fracture was delicate and based on the disto-medial proximo-lateral oblique radiographic view (Di35M-PrLO). The surgical removal of the fragment was performed using a medial arthroscopic approach. The outcome was rapidly favourable. Lameness disappeared and the follow-up radiographs three months later revealed minimal reactions of osteoarthrosis.

Abstract:Objective The objective of this prospective study was to evaluate the use of ultrasonography in the diagnosis of medial coronoid process disease in unclear cases. Study Design Fifteen elbows (on thirteen dogs) for which radiography and computed tomography did not lead to a clear diagnosis of medial coronoid process disease were included. On each elbow, ultrasonography was performed with a high frequency linear transducer (12–18Hz). Then, arthroscopic examination of the joint was performed by a surgeon who was unaware of ultrasonographic findings to confirm medial coronoid process disease. Results At least one ultrasonographic lesion was detected in 13 out of 15 elbows. The main reported ultrasonographic lesions were joint effusion (10/15 elbows) and an abnormal shape of the medial coronoid process (irregular, ill-defined or fragmented) (9/15). Conclusion Ultrasonography can be a helpful additional diagnostic tool to confirm medial coronoid process disease of the elbow joint before performing arthroscopy in unclear cases. Further studies will be needed to evaluate the use of higher frequency transducers and determine if it could improve the diagnostic value of ultrasonography. Abstract:Objective To study the value of 99Tcm-MDP planar bone scintigraphy combined with SPECT/CT in the diagnosis of stress fracture (SF).Methods Twelve male New Zealand white rabbits were randomly divided into two groups by random number table method: 8 in the study group and 4 in the control group.The rabbits in the study group were given electronic stimulation regularly to let them jump in a special device for 6 weeks.All rabbits underwent both 99Tcm-MDP bone planar imaging and SPECT/CT.The tibia was divided into three parts (upper,middle and lower) and the corresponding ROI and volume of interest (VOI) were generated and mean radioactive counts were measured.The average radioactive counts of the control group served as the non-fracture reference.The radioactive counts ratio of suspected SF/nonfracture tibia (SF/N) was calculated.SF/N of 1.46 was the cut-off value for positive SF lesions.Finally,pathological examination was performed in each tibia of the rabbits.Kappa test was used to analyze the consistency between the imaging and histological findings.Results The sensitivity,specificity,accuracy for the diagnosis of SF was 56.25％ (9/16),7/8,66.67％ (16/24) by planar bone scintigraphy,and 75.00％(12/16),8/8,83.33％ (20/24) by bone scintigraphy combined with SPECT/CT,respectively.The diagnosis by planar imaging combined with SPECT/CT was significantly more accurate than that of the planar imaging alone.The Kappa value between bone scintigraphy combined with SPECT/CT and pathological findings was 0.667 (P＜0.05).Conclusion Planar imaging combined with SPECT/CT can improve the diagnostic efficacy of SF.

Abstract:Summary Aim: The study investigates whether early dynamic PET/CT (edPET/CT) using 18F-fluoro- deoxyglucose (FDG) discriminates between affected versus non-affected sites in patients with complicated, protracted fracture healing and suspected COM in the lower extremities. Patients, methods: In nine consecutive patients (1 woman, 8 men; age 54 ± 13 years), before standard late FDG-PET/CT, altogether 10 edFDG-PET/CT examinations were performed in list mode over 5 min starting with radiopharmaceutical injection. Eight consecutive time intervals (frames), four 15-s, then four 60-s, were reconstructed. For every patient, several volumes-of-interest were selected. To measure early FDG influx and accumulation, maximum and mean ed standardized uptake values (respectively, ed SUVmax, edSUVmean) were calculated in each volume-of-interest during each frame. Results were compared between affected and non-affected (contralateral) bone. Results: Starting in the 31-45s frame, the affected bone area showed significantly higher ed- SUVmax and edSUVmean than did the healthy contralateral region. In conventional PET/CT, affected bone areas also significantly differed from non-affected contralateral regions. Conclusion: This pilot study suggests that edFDG- PET may offer a less time consuming add on to standard FDG-PET/CT while being equally accurate. The results should be validated pros- pectively in larger trials.

Shear-wave elastography of canine patellar tendons in healthy dogs and the influence of stifle joint angle

Abstract:Elastography is a sonographic modality that measures tissue stiffness, a mechanical property of tissues, and a biomarker for disease. Canine musculoskeletal application to the patellar tendon has been limited to semiqualitative strain elastography. This prospective study aimed to quantitatively evaluate patellar tendon stiffness using shear-wave elastography with a color map superimposed over the tendon, a propagation map for quality control, and measurements at specific regions of interest in 16 clinically normal sedated dogs weighing 25 kg or greater. Tendon stiffness using shear-wave elastography (SWE) was assessed at different stifle angles and in three regions to determine if angle and location affected stiffness. All dogs were screened with general and orthopedic exams, lateral stifle radiographs, and patellar tendon 2D ultrasound. Shear-wave elastography was performed from a long axis at various stifle angles at the proximal, middle, and distal tendon segments. Quality diagnostic SWE results varied significantly with stifle angle, and 150° of extension was the only angle found to be clinically useful based on the ease of obtaining measurable results and a quality control propagation wave. Patellar tendons were primarily stiff with a red color elastogram. The proximal and middle segments, measured at various angles, had a mean SWE velocity of 7.32 ± 0.90 m/s. Tendon stiffness did not differ along tendon length when measured in greater extension. However, stiffness decreased in the middle segment of the tendon at 150° when compared with 120°. This study establishes a quantitative baseline of normal patellar tendon stiffness to compare with pathologic states.
Diagnostic accuracy of bone SPECT and SPECT/CT imaging in the diagnosis of unilateral condylar hyperplasia: A systematic review and meta-analysis

Abstract:Purpose Imaging with bone scans plays an important role in the diagnostic path of patients with unilateral condylar hyperactivity or unilateral condylar hyperplasia (UCH). The aim of this study is to perform a systematic review of the diagnostic performance of the bone SPECT and SPECT/CT scan for the diagnosis of UCH. Materials and methods PubMed, SCOPUS and EMBASE were searched electronically to identify diagnostic accuracy studies that assessed the diagnostic value of bone SPECT and SPECT/CT for the diagnosis of UCH, Meta-analyses were performed with Metadisc 1.4 and 2.0. Results A total of 14 studies, with a total number of 887 patients, were included in the qualitative analysis and 11 studies qualified for meta-analyses. The pooled sensitivity and specificity for the SPECT scan were 0.814 (95 % CI: 0.639-0.915) and 0.774 (95 % CI: 0.655-0.861), for the SPECT/CT scan these were 0.818 (95 % CI: 0.749-0.874) and 0.901 (95 % CI: 0.840-0.945). The summary receiver operating characteristics of the SPECT scan showed an area under the curve of 0.847 (95 % CI: 0.722-0.972) and that of the SPECT/CT scan was 0.928 (95 % CI: 0.876-0.980). Conclusion Both bone SPECT scan and SPECT/CT scan provide a high diagnostic accuracy for UCH. The added value of the SPECT/CT scan is questionable and given the potential disadvantages of the SPECT/CT scan, including the increased radiation dose and costs, the diagnostic modality of first choice in patients with UCH should be a SPECT scan.

Abstract:Abstract Background Medical infrared thermal imaging (MITI) is a noninvasive imaging modality used in veterinary medicine as a screening tool for musculoskeletal and neurological disease processes. An infrared camera measures the surface body heat and produces a color map that represents the heat distribution. Local trauma or disease can impair the autonomic nervous system, which leads to changes in the local dermal microcirculation and subsequent alteration of surface body heat. Disruption of autonomic flow to the cutaneous vasculature at deeper levels can also result in asymmetric thermographic results. The purpose of this study was to evaluate surface temperature differences between limbs affected by bone neoplasia and their normal contralateral limbs. Results A statistically significant difference in average temperature was noted between regions of interest of the two groups (paired difference: 0.53 C° ± 0.14; P = 0.0005). In addition, pattern recognition analysis yielded a 75–100% success rate in lesion identification. Conclusions Significant alterations noted with average temperature and thermographic patterns indicate that MITI can document discernible changes associated with the presence of canine appendicular bone tumors. While MITI cannot be used as the sole diagnostic tool for bone cancer, it can be used as a screening modality and may be applicable in early detection of cancer.

Abstract:Purpose To compare diagnostic accuracy between dual-energy CT lung perfused blood volume (Lung PBV) imaging and single photon emission computed tomography (SPECT) in detecting chronic thromboembolic pulmonary hypertension (CTEPH) with histopathological results as reference standard in a canine model. Materials and methods Eighteen CTEPH canines were included into this experimental study. All procedures including paracentesis, embolization, scanning, pressure measurement and feeding medicine were repeated each two weeks, until systolic/diastolic pressure in canines was ≥30/15 mmHg or mean pulmonary artery pressure ≥20 mmHg, and then sacrificed for histopathology examination. Two radiologists (readers 1 and 2) and two nuclear radiologists (readers 3 and 4) analyzed images of conventional CT pulmonary angiography in dual-energy CT mode, Lung PBV imaging and SPECT, respectively. The presence, numbers, and locations of pulmonary emboli (PE) were recorded on a per-lobe basis. Pathological examination was served as reference standard. Sensitivity, specificity and accuracy of Lung PBV and SPECT were calculated. Kappa statistics were used to quantify inter-reader agreement. Results With histopathological results as reference standard, the sensitivities of 72.2%, 78.8%, 81.2%, specificities of 75.9%, 87.5%, 84.8%, accuracies of 73.8%, 83.1%, 83.1%, for readers 1, 2 and both with Lung PBV, respectively. Readers 3, 4 and both had sensitivities of 14.3%, 25.7%, 33.3%, specificities of 90.0%, 86.7%, 93.3%, accuracies of 49.2%, 53.8%, 60.0% with SPECT for detecting CTEPH. Inter-reader agreements were good for dual-energy CT (kappa = 0.662) and SPECT (k= 0.706) for detecting CTEPH. Conclusion Dual-energy CT had a higher accuracy to detect CTEPH than SPECT in this canine model study.

Abstract:Abstract A 54-year-old man with Gleason 9 prostate cancer with reported nodal and skeletal metastases was referred to us. Outside hospital reports described abnormal left proximal humerus activity on bone scan concerning for metastasis; however, concurrent PSMA PET/CT did not show activity in this lesion. Further review of the PET/CT images revealed characteristic features of enchondroma in the left humeral lesion.

Abstract:Compared to positron emission tomography/computed tomography (PET/CT), the uptake of PET- magnetic resonance imaging (MRI) has been slow, even more so in clinical practice compared to the (pre-)clinical research setting. However, for applications in musculoskeletal (MSK) research, the combination of PET and MRI into a single modality offers attractive advantages over other imaging modalities. Most importantly, MRI has exquisite soft-tissue detail without the use of contrast agents or ionizing radiation, superior bone marrow visualization, and an extensive spectrum of distinct multiparametric assessment methods. In the preclinical setting, the introduction of PET inserts for small-animal MRI machines has proven to be a successful concept in bringing this technology to the lab. Initial hurdles in quantification have been mainly overcome in this setting. In parallel, a promising range of radiochemistry techniques has been developed to create multimodality probes that offer the possibility of simultaneously querying different metabolic pathways. Not only will these applications help in elucidating disease mechanisms, but they can also facilitate drug development. The clinical applications of PET/MRI in MSK are still limited, but encouraging initial results with novel radiotracers suggest a high potential for use in various MSK conditions, including osteoarthritis, rheumatoid arthritis, ankylosing spondylitis and inflammation and infection. Further innovations will be required to bring down the cost of PET/MRI to justify a broader clinical implementation, and remaining issues with quality control and standardization also need to be addressed. Nevertheless, PET/MRI is a powerful platform for MSK research with distinct qualities that are not offered by other techniques.
Ultrashort Echo Time Quantitative Magnetic Resonance Imaging of the Cruciate Ligaments in Normal Beagles

Abstract:Objective: The aim of this study was to provide normative ultrashort echo time magnetic resonance imaging (UTE MRI) data of the patellar ligament (PL), cranial cruciate ligament (CrCL) and caudal cruciate ligament (CdCL) in non-lame Beagles. Study design: Eight stifles from four subjects obtained immediately postmortem were imaged using UTE MRI in the true sagittal plane. Regions of interest were drawn manually and the total (T2*), short T2* (T2*S) and long T2* (T2*L) values of the signal decay were calculated to evaluate the bound and free water components of the tendon. The T2*S, T2*L and T2* values were compared between the PL, CrCL and CdCL RESULTS: The mean and standard deviation of T2*S, T2*L and T2* were as follows: 0.54 ± 0.13, 4.65 ± 1.08 and 8.35 ± 0.82 ms for the PL; 0.46 ± 0.14, 5.99 ± 0.52 and 8.88 ± 0.4 ms for the CrCL and 0.41 ± 0.13, 7.06 ± 0.57 and 9.26 ± 0.18 ms for the CdCL. Significant differences were found between the T2*L component of the PL and each CrCL/CdCL and a smaller difference was noted between the T2*L of the CrCL and CdCL (p = 0.05). No difference of the T2*S value was found between any of the ligaments. Conclusion: Establishing normative UTE data of the canine stifle is valuable for comparison in future studies in which normal and damaged ligaments may be evaluated, particularly in those affected limbs in which no instability is identified on physical examination in which normal and damaged ligaments may be evaluated.

Abstract:Background Severe and complex angular limb deformities in dogs require accurate morphological assessment using diagnostic imaging to achieve successful orthopedic surgery. Computed tomography (CT) is commonly used to overcome projection errors in two-dimensional angular measurements of dog hindlimb alignment. Three-dimensional volume rendering (VR) techniques permit virtual positioning and variable projection, but the final CT-image that defines the projection plane for angular measurements remains two-dimensional. Objective We wanted to develop a true three-dimensional open-source technique to measure the alignments of the hind limbs of dogs in CT scanners. Methods We developed an open-source 3D Slicer plug-in, to perform angular measurements using vector calculations in three-dimensional space. In 113 CT-scans of canine pelvic limbs, femoral torsion, femoral varus, femorotibial rotation, tibial torsion, tibial varus and tibiotalar rotation angles were calculated and compared to an already validated technique using VoXim®. Results Reference points were identified and measurements were possible in the 113 acquisitions. The greatest difference between the two techniques was 1.4° at only one tibial torsion angle. Mean values for all Bland-Altman plots did not show significant differences and were less than 0.07° for all comparisons. Discussion Based on these results we considered angular measurements of canine hind limb alignment in CT scans using the 3D Slicer extension program sufficiently accurate for clinical orthopedic and surgical purposes in veterinary medicine. Conclusion With our open-source 3D Slicer extension software, we provide a free accessible tool for veterinary orthopedic surgeons and thus we hope to improve angular measurements in CT-scans of canine hind limb deformities through true three-dimensionality.

Abstract:BACKGROUND:Dual-energy CT (DECT) has been used to detect myocardial infarct. However, few comparable studies with histopathological findings as gold standard have been published.PURPOSE:To investigate the accuracy of DECT iodine maps for detecting acute myocardial infarction compared with single photon emission computed tomography (SPECT) in a canine model using histopathological findings as the reference standard.MATERIAL AND METHODS:A model of myocardial ischemia was created by ligating the left anterior descending (LAD) coronary artery after thoracotomy in six dogs, while another three dogs undergoing thoracotomy without LAD ligature served as a control group. Contrast-enhanced DECT scans of the heart were performed, followed by resting 99mTc-MIBI SPECT myocardial perfusion imaging in all nine dogs before and 3 h after the procedure. Triphenyltetrazolium chloride (TTC) staining was performed and analyzed. In the short axis of the left ventricle, the wall surface was divided into 17 segments, which were assessed for infarcted myocardium on conventional CT from average-weighted data, DECT myocardial iodine maps, conventional CT plus DECT, SPECT, and histopathology. Inter-observer and inter-modality agreement for conventional CT, DECT myocardial iodine maps, and SPECT were calculated. CT value of infracted and non-infracted areas was measured.RESULTS:With the histopathological results as the reference standard, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were 75.0% (30/40), 92.0% (104/113), 76.9% (30/39), 91.2% (104/114), 87.6% (134/153) for conventional CT, 85.0% (34/40), 84.1% (95/113), 65.4% (34/52), 94.1% (95/101), 84.3% (129/153) for DECT myocardial iodine maps; 87.5% (35/40), 92.9% (105/113), 81.4% (35/43), 95.5% (105/110), 91.5% (140/153) for conventional CT plus DECT; 82.5% (33/40), 90.3% (102/113), 75.0% (33/44), and 93.6% (102/109), 88.2% (135/153) for SPECT, respectively. Excellent inter-observer agreement (Kappa value >0.8) and good inter-modality agreement (Kappa value >0.6) for each modality were found. CT values of infarcted myocardium (26 ± 22 HU, 36 ± 33 HU, 34 ± 16 HU) were lower than those of non-infarcted myocardium (115 ± 16 HU, 121 ± 28 HU, 123 ± 11 HU) on images of 140 kVp, 80 kVp, and average-weighted 120 kVp images (all P < 0.05).CONCLUSION:With histopathology as the reference standard, DECT myocardial iodine maps can detect acute myocardial infarction with diagnostic accuracy comparable to resting SPECT myocardial perfusion imaging in a canine model. DECT plus conventional CT had a potential to improve the detection of acute myocardial infarction.

Abstract:A plane spiral orbital angular momentum (OAM) waves-based $4 \times 4$ multiple-in-multiple-out (MIMO) communication system for line-of-sight channel is proposed and experimentally demonstrated in comparison with conventional MIMO. The transmitting OAM waves are propagating along the transverse direction; hence there is no singularity or divergence problem. Compared with the conventional MIMO, this system could achieve lower spatial correlations and a higher capacity gain while the OAMantennas as the elements of the transmitting array are sorted in an ordered sequence. The measured bit-error-rate performance clearly demonstrates the feasibility of the proposed system, and the causation of the improved performance is also detailedly discussed. This technology can be applied in near-field communications or short-range MIMO systems to realize higher capacity communications.
Shoulder joint hemiarthroplasty for treatment of a severe osteochondritis dissecans lesion in a dog

Abstract:Introduction: Partial resurfacing of the humeral head has been reported in humans to treat humeral osteochondritis dissecans. The aim is to describe a custom-made humeral resurfacing prosthesis for treatment of severe humeral head osteochondritis disse-cans in a dog. Case report: A seven-month-old female entire St. Bernard dog was presented with a 10 week history of severe left thoracic limb lameness. Radiography, arthroscopy and magnetic resonance imaging confirmed an extensive osteochondritis dissecans lesion affecting the caudal, medial and central regions of the humeral head. A prosthesis designed from computed tomography (CT) images was manufactured in polished stainless steel alloy with a hydroxyapatite coated base and central finned humeral stem for cementless insertion. A standard caudal approach to the shoulder was used to place the prosthesis following reaming of the caudal humeral head. Results: Radiography and CT imaging revealed appropriate topographical placement on the humerus. Force plate analysis demonstrated initial reduction in ground reaction force at six weeks, followed by gradual improvement at three months. There was no radiographic evidence of implant loosening and the range of shoulder joint motion was comparable to the contralateral joint at three months; these findings were maintained to final follow-up at 24 months. Clinical significance: Placement of a humeral head resurfacing prosthesis was an effective technique for the management of severe osteochondritis dissecans-related shoulder lameness. To the authors' knowledge, this is the first report of the clinical use of shoulder hemiarthroplasty in a dog.
Added Value of Spect/Spiral Ct Versus Spect in Diagnosing Solitary Spinal Lesions in Patients with Extraskeletal Malignancies

Abstract:PurposeThe aim of this study was to investigate the added value of single-photon emission computed tomography (SPECT)/spiral computed tomography (CT) versus SPECT alone in the differential diagnosis of solitary spinal lesions in patients with extraskeletal malignancies. Materials and methodsA total of 90 patients who had a solitary spine ‘hot spot’ that could not be definitively diagnosed using planar scintigraphy were enrolled in the study. SPECT/spiral CT was performed on the indeterminate lesions in the spine. Images were independently interpreted by two experienced nuclear medicine physicians. Each spinal lesion was graded on a four-point diagnostic scale (1, benign; 2, likely benign; 3, likely bone metastasis; 4, bone metastasis). The final diagnosis of each lesion was based on pathological confirmation or follow-up. &kgr; scores were used to evaluate inter-reviewer agreement and agreement of the SPECT and SPECT/spiral CT diagnoses with the final diagnosis. ResultsFinal diagnoses revealed 25 bone metastases and 65 benign lesions. Forty percent (36/90) of the solitary spinal lesions were diagnosed as equivocal (likely benign or likely bone metastasis) according to SPECT, whereas only 5.6% (5/90) were diagnosed as equivocal according to SPECT/spiral CT. For SPECT/spiral CT-based and SPECT-based diagnoses, the &kgr; scores for inter-reviewer agreement were 0.889 (P<0.001, 95% confidence interval 0.824–0.954) and 0.504 (P<0.001, 95% confidence interval 0.401–0.607). The diagnostic accuracies of SPECT/spiral CT and SPECT images were 91.1% (82/90) and 58.9% (53/90), respectively (&khgr;2=24.919, P<0.001). ConclusionCompared with SPECT imaging alone, SPECT/spiral CT imaging was more accurate and valuable in the differential diagnosis of solitary spinal lesions in patients with extraskeletal malignancies.
