Abstract:A 43-year-old woman was scheduled to undergo cardiac pacemaker implantation because of sick sinus syndrome. Following percutaneous access of the left subclavian vein, the guide wire traveled straight across to the right subclavian vein, and could not be turned acutely downward into the left persistent superior vena cava despite multiple attempts with lead reshaping. Injection of contrast medium showed the persistent left superior vena cava and absent right superior vena cava (Figure 1). A right subclavian approach was used to accomplish a bipolar lead with active fixation (Medtronic 5076-58, Minneapolis, MN) implantation in the right atrium, via the coronary sinus (Figure 2). The RV lead could not enter the right ventricle. A singlechamber pacing system (AAI) was selected, due to the normal atrioventricular conduction. Overcoming Lead Penetration Failure in Left Bundle Branch Area Pacing

Abstract:Left bundle branch area pacing is an innovative technique employed to maintain left ventricular synchrony. However, the operator can face challenges in penetrating the lead into the septum during screwing. Two pitfalls—helix-only penetration and shallow penetration—hinder lead penetration. The research, conducted in 2 phases on patients and swine hearts, tested 2 techniques to overcome these issues. The results highlight the impact of endocardial involvement and suggest that the optimal technique involves intentionally creating an endocardial breach rather than simply unscrewing and rescrewing. This solution could facilitate the procedure by minimizing the number of attempts to reach the pacing site and thus reducing the risk of complications.

Abstract:As an emerging pacing technique, left bundle branch area pacing (LBBAP) has served as a physiological pacing modality that overcomes the limitations of His bundle pacing (HBP) or right ventricular pacing. Three patients with terminal heart failure who were waiting for heart transplantation and met the indications of pacemaker implantations received LBBAP. Symptoms were relieved and stabilized and eventually received heart transplantation. Diseased hearts from the recipients were dissected post-transplantation, and the direct visual of pacing lead locations in the interventricular septum were evaluated, and the histopathological examination around the lead was conducted for the first time in human. As a result, we found that the locations of LBBAP leads were matched with fluoroscopic views during the procedure and Masson's staining showed extensive fibrosis occur around the lead but did not result in high thresholds.

Abstract:Left bundle branch pacing has recently emerged as a significant alternative to right ventricular pacing. The rate of implanted stylet-driven septal leads is expected to increase substantially in the coming years, along with the need to manage long-term complications. Experience in extracting these leads is currently very limited; however, the number of complex extractions is anticipated to increase in the future. We report a complex case involving the extraction of a long-dwelling Solia lead used for left bundle branch pacing in a 21-year-old man. The lead was extracted through the implant vein 27 months after implantation, using a methodology that involved a locking stylet and compression coil. The new lead insertion was challenging due to venous occlusion but after successful venoplasty, the His lead was successfully implanted. The postoperative course was uneventful, demonstrating the feasibility of extraction without complications.

Abstract:Introduction This case report highlights the novel role of His‐bundle pacing (HBP) from right atrium, not just for preserving cardiac function, but also for avoiding interference with TriClip devices. Methods and Results A 78‐year‐old female with severe tricuspid regurgitation received two TriClip devices. Postprocedure, frequent significant sinus pauses required a pacemaker. HBP was chosen to avoid lead complications. Under local anesthesia, a His pacing lead was inserted via the axillary vein using specialized catheter. Follow‐ups over 2.5 years showed stable parameters with no complications. Conclusion HBP is effective for patients with TriClip devices, ensuring optimal cardiac function and lead stability.

Abstract:A 77-year-old man was referred for pacemaker implantation. Cephalic vein cutdown failed, and subclavian vascular access was obtained only after several puncture attempts. Both leads (5.6 F) were inserted through the same puncture site. Lead measurements were as expected. Before the patient's discharge, left cardiac implantation was suspected on chest X-ray and electrocardiogram (ECG), and confirmed by echocardiography, with the atrial lead being screwed into the left main coronary artery ( Panel A ). The patient was transferred to our hospital for further therapy. Following wiring the left anterior descending coronary artery and placing a bailout stent, we first withdrew the atrial lead. As there was no angiographic evidence of a coronary lesion, we removed the guidewire and the stent. Next, we withdrew both leads completely and implanted immediately a covered stent into the subclavian artery. Finally, two pacing leads were implanted through the vena axillaris 1 into the right atrial appendage and right-sided interventricular septum ( Panel B ). No pleural or pericardial effusion was noted, and the patient was discharged free of any symptoms on the following day.

Abstract:Background: This case report outlines the presentation of an emerging complication arising from left bundle branch area pacing (LBBAP). Case summary: A 43-year-old male with no history of cardiac problems experienced recurrent episodes of syncope with no prodromal symptoms. During monitoring in the emergency department, the patient underwent an episode of asystole, leading to LBBAP implantation. The procedure encountered technical challenges, resulting in an interventricular septal hematoma and subsequent ventricular arrhythmias. Despite initial concerns, conservative management led to resolution, demonstrated through echocardiographic follow-ups. Discussion: This report underscores the significance of ventricular arrhythmias as indicators of interventricular septal hematoma, providing insights into its diagnosis, management, and implications for LBBAP procedures.

Abstract:Background Left bundle branch area pacing (LBBAP) requires deep septal lead deployment for left-sided conduction stimulation. Advancing leads towards deep septal positions might add additional mechanical stress on these leads. Concerns on lead performance and reliability remain an unanswered question. Objective In this study, we aimed to analyze lead performance and integrity of stylet-driven pacing leads (SDL) for LBBAP. Methods This study assessed lead fracture rates of SDL in a large single-center cohort of adult LBBAP patients. Fluoroscopic analysis of lead bending angulations at the septal insertion point and in-vitro bench testing of lead preconditioning were performed to simulate clinical use conditions. Lead performance was compared between LBBAP and conventional right ventricular apical pacing (RVp) sites. Results The study included 325 LBBAP patients (66% male, 71±15 years). During a median follow-up of 18 months, two patients (0.6%) experienced conductor fracture between tip housing and ring electrode, whereas no such fractures occurred with RVp (n=149) patients (p=0.22). X-ray analysis revealed that high lead bending angulations occurred in 1.3% of the patients. Accelerated bench testing of excessive preconditioned leads showcased a higher probability of early conductor fracture compared to standard preconditioned leads. Conclusion The incidence of early conductor failure in LBBAP seems higher than with conventional RVp sites. The most vulnerable lead part seems to be the interelectrode space between the tip housing and ring electrode. Excessive angulation and preconditioning might contribute to early fatigue fracture.

Abstract:: We report the case of a 63-year-old female patient who was urgently admitted from electrophysiology laboratory (EP Lab) to operating room (OR) with hemodynamic instability and hemothorax. The dual-chamber pacemaker had been implanted 15 years ago. After median sternotomy she was evaluated for perforations and raptures of cardiac chambers and status of right ventricle (RV) lead of permanent pacemaker (PPM) by the surgeon. Evaluations revealed that RV and right atrium (RA) have been ruptured and tricuspid valve has been damaged as a result of dislodgment of RV PPM lead secondary to manipulations in electrophysiology (EP) lab. This lead was withdrawn through SVC and ruptures were repaired and tricuspid valve was replaced. The epicardial pacemaker was placed by the surgeon. After surgery, the patient was transferred to ICU, after three days she was discharged to ward and after 5 days from surgery she was discharged from hospital in good condition.

Abstract:Permanent pacemaker implantation is a low-risk procedure. However, complications may occur at a rate of around 4–8%. We present a case where initial implantation resulted in complications that could have been avoided by meticulous assessment of lead position in different projections and early post-procedure X-ray that would have delineated other serious complications. We present a case where the right ventricular lead was placed in the coronary sinus, which resulted in the loss of pacing capture with further syncope after the pacemaker implant. This was apparent in the post-procedure electrocardiogram (ECG) with right bundle branch pacing and the lead was repositioned in the right ventricular apex the following day. Furthermore, the patient was discharged home without a chest X-ray (CXR), and she represented a week later with a haemo-pneumothorax and pericardial effusion. A chest drain was placed and was discharged after a slow recovery following several complications that could have been avoidable.

Abstract:Premature ventricular contractions with right bundle branch block morphology induced during left bundle branch (LBB) pacing (LBBP) lead implantation serve as a marker indicating that the lead is close to or has reached the LBB region. However, no reports to date have described accelerated idioventricular rhythm (AIVR) induced by LBBP lead deployment. We herein describe a patient who underwent LBBP for complete atrioventricular block. The patient's intrinsic escape rhythm was overtaken by AIVR induced immediately after LBBP lead deployment. AIVR is another marker indicating that the lead has reached the LBB region.

Abstract:Roughly one in six patients receiving conventional transvenous pacemaker systems experience significant complications within 1 year of implant, mainly due to the transvenous lead and subcutaneous pocket. A new helix-fixation single-chamber ventricular leadless pacemaker (LP) system capable of pre-deployment exploratory electrical mapping is commercially available. Such an LP may mitigate complications while streamlining the implantation. In this study, the initial real-world implant experience of the helix-fixation LP was evaluated following its commercial release.

Abstract:Leadless pacemakers offer the opportunity to avoid transvenous hardware among patients with tricuspid valve prostheses. We present the first case of a helix-based fixation leadless pacemaker implanted through valve-in-valve tricuspid prostheses in a 43-year-old female with extensive prior cardiac history. At the time of presentation, epicardial pacing was no longer a viable option in the setting of pacemaker dependence. Placement of a helix-fixed, leadless right ventricular pacemaker was performed as a bridge to dual-chamber leadless pacing. This was safely and effectively performed and highlighted favorable procedural characteristics that included RV cavity dimensions and prosthesis type.

Abstract:Isolated persistent left superior vena cava (PLSVC) is a rare congenital anomaly typically found incidentally due to its asymptomatic nature. However, it can present technical challenges for device implanters. We report a case involving a patient with PLSVC, for whom the implantation of a transcatheter pacing system proved to be the most effective long-term solution. Although this venous anomaly initially provided a safe pacing route, it eventually led to early complications. The patient, a 78-year-old Puerto Rican man with hypertension, diabetes mellitus, and complete atrioventricular block, experienced multiple complications with pacing devices. After a failed left-sided pacemaker implant, a right-sided single-chamber ventricular device was placed, but it led to right ventricular lead fractures and was eventually abandoned. A new pacing system implanted in the left chest lasted only a year. Venography revealed a patent PLSVC with a previously implanted device now obstructed by an occluded left brachiocephalic vein. After laser-assisted extraction, a dual-chamber device was successfully implanted through the PLSVC. Despite unremarkable physical and lab results, the patient later showed syncope and high lead impedances with fractures in both leads and total PLSVC occlusion. A transcatheter pacing system was chosen to address the complex anatomical issues and abandoned hardware. Atrial synchronized pacing was confirmed the morning after implantation, and the patient was safely discharged. Ensuring a stable ventricular rhythm is crucial for patients with complete heart block. When hemodynamic stability is compromised by recurrent lead fractures and rare anatomical variants, implanters must consider alternative solutions. In this case, a transcatheter system was selected to avoid further lead and pocket-related complications and mitigate the risks of additional laser-assisted extractions. At the end of the device's lifespan, a new device can be implanted without significant anatomical issues, and the epicardial route remains a viable option if necessary.

Abstract:Abstract Background The use of transvenous pacing leads is associated with the risk of developing tricuspid valve (TV) dysfunction. This develops through several mechanisms including the failure of leaflet coaptation or direct damage to the TV or to its sub-valvular apparatus and can result in significant tricuspid regurgitation (TR). Multiple approaches to pacemaker implantation after transvenous lead extraction (TLE) or surgical TV repair have been described. Placement of pacing leads across the TV is generally avoided in such circumstances. Case summary A 66-year-old woman presented with a year-long history of exertional dyspnoea, peripheral oedema, and postural neck pulsations. Her medical history included a dual-chamber pacemaker implantation for sinus node dysfunction 14 years ago. Echocardiography revealed severe lead-related TR. Her case was discussed in our multi-disciplinary team meeting. A decision was made to perform a TLE and implant a leadless pacemaker in an attempt to avoid open-heart surgery if possible. This was reserved as an option in the event of persistent severe TR. Transvenous extraction of the right ventricular lead was performed. The atrial lead was preserved and connected to and AAI device. A Micra AV was implanted allowing for atrioventricular (AV) synchronous pacing. Discussion We present the first case of successful implementation of AV sequential pacing using a dual-pacemaker approach involving the use of an AAI pacemaker and a Micra AV device. This was performed after TLE for severe lead-related TR.
