Abstract:Despite modern surgical methods and antibiotics, the mortality of the intraabdominal abscess remains high. The patients are often debilitated and many have undergone recent major surgery. Percutaneous catheter drainage of abdominal abscess has proven to be a valuable alternative to surgical drainage [1 ] and has been used in patients too ill to undergo a surgical drainage [2]. We recently drained abdominal abscesses in three patients at the bedside whose unstable status prohibited even transfer to the radiology department.

Abstract:OBJECTIVE: Lower extremity injuries are the most frequent Abbreviated Injury Scale (AIS) 2 injury for drivers in frontal crashes. The objective was to reconstruct 11 real-world motor vehicle crashes (2 with AIS 2+ distal lower extremity injury and 9 without lower extremity injury) and to analyze the vehicle parameters and driver attributes that affect injury risk.METHODS: Eleven frontal crashes were reconstructed with a finite element simplified vehicle model (SVM) using a semi-automated optimization method. The SVM was tuned to each corresponding vehicle and the Total HUman Model for Safety (THUMS) Ver 4.01 was scaled and positioned in a baseline configuration to mimic the documented precrash driver posture. The event data recorder crash pulse was applied as the boundary condition for each case. Additionally, for the 2 cases with lower extremity injury, 120 simulations to quantify the uncertainty and response variation were performed varying the following parameters using a Latin hypercube design of experiment (DOE): seat track position, seatback angle, steering column angle, steering column position, and D-ring height. Injury metrics implemented within THUMS were calculated from the femur, tibia, and ankle and cross-compared among the 11 baseline cases using tibia index and multiple injury risk functions. Kinetic and kinematic data from the 120-simulation DOE were analyzed and fit to regression models to examine any causal relationship between occupant positioning and lower extremity injury risk.RESULTS: Of the 11 real-world crashes, both cases with lower extremity injuries resulted in elevated tibia axial forces and resultant bending moments, compared to the 9 cases without lower extremity injury. The average tibia index of the 2 cases with distal lower extremity injury (left: 1.79; right: 1.19) was higher than that in the 9 cases without lower extremity injury (left: 1.16, P =.024; right: 0.82, P =.024). An increased risk of AIS 2+ tibia shaft (33.6%), distal tibia and hindfoot (20.0%), as well as ankle malleolar (14.5%) fracture was also observed for the injured compared to the noninjured cases. Rearward seat track position, reclined seat back angle, and reduced seat height were correlated with elevated tibia axial force and increased tibia index, imposing additional lower extremity injury risk.CONCLUSIONS: This study provides a computational framework for assessing lower extremity injuries and elucidates the effect of precrash driver posture on lower extremity injury risk while accounting for vehicle parameters and driver attributes. Results from the study aid in the evaluation of real-world injury data, the understanding of factors contributing to injury risk, and the prevention of lower extremity injuries.

Abstract:Aim. We hypothesized that normal saline (NS) may have more deleterious effects compared with lactated ringer (LR) in kidney transplant recipients because of the higher risk of acidosis and higher levels of serum potassium. Thus, the aim of this study was to determine the safety of LR if used during a renal transplant. Methods. Adults undergoing kidney transplantations were enrolled in a double-blinded randomized prospective clinical trial. They were divided into two groups in order to receive NS and LR infusion as intraoperative IV fluid replacement therapy. Results. There was a significant difference in the serum potassium level (p = .000) and the PH (p = .007) between the two groups at the end of transplantations. Two patients in the LR group lost their kidneys due to vascular graft thrombosis. In other words, hyperkalemia and acidosis occurred more frequently in the NS group while thrombotic events may be of concern in the LR group. Conclusion. Compared with NS, LR infusion may lead to a lower serum potassium level and a lower risk of acidosis, while there is major concern of the hypercoagulable state in these patients.

Effect of body size and enhanced helmet systems on risk for motorsport drivers

Abstract:Objective: Computational modeling has been shown to be a useful tool for simulating representative motorsport impacts and analyzing data for relative injury risk assessment. Previous studies have used computational modeling to analyze the probability of injury in specific regions of a 50th percentile male driver. However, NASCAR drivers can represent a large range in terms of size and female drivers are becoming increasingly more common in the sport. Additionally, motorsport helmets can be outfitted with external attachments, or enhanced helmet systems (EHS), whose effect is unknown relative to head and neck kinematics. The current study expands on this previous work by incorporating the F05-OS and M95-OS into the motorsport environment in order to determine correlations between metrics and factors such as PDOF, resultant ΔV occupant size, and EHS. Methods: A multi-step computational process was used to integrate the Global Human Body Models Consortium family of simplified occupant models into a motorsport environment. This family included the 5th percentile female (F05-OS), 50th percentile male (M50-OS), and 95th percentile male (M95-OS), which provide a representative range for the size and sex of drivers seen in NASCAR's racing series'. A series of 45 representative impacts, developed from real-world crash data, and set of observed on-track severe impacts were conducted with these models. These impacts were run in triplicate for three helmet configurations: bare helmet, helmet with visor, helmet with visor and camera. This resulted in 450 total simulations. A paired t-test was initially performed as an exploratory analysis to study the effect of helmet configuration on 10 head and neck injury metrics. A mixed-effects model with unstructured covariance matrix was then utilized to correlate the effect between five independent variables (resultant ΔV, body size, helmet configuration, impact quadrant, and steering wheel position) and a selection of 25 metrics. All simulations were conducted in LS-Dyna R. 9.1. Results: Risk estimates from the M50-OS with bare helmet were used as reference values to determine the effect of body size and helmet configuration. The paired t-test found significance for helmet configuration in select head-neck metrics, but the relative increase in these metrics was low and not likely to increase injury risk. The mixed-effects model analyzed statistical relationships across multiple types of variables. Within the mixed-effects model, no significance was found between helmet configuration and metrics. The greatest effect was found from resultant ΔV, body size, and impact quadrant. Conclusions: Overall, smaller drivers showed statistically significant reductions in injury metrics, while larger drivers showed statistically significant increases. Lateral impacts showed the greatest effect on neck metrics and, on average, showed decreases for head metrics related to linear acceleration and increases for head metrics related to angular velocity. HBM parametric studies such as this may provide an avenue to assist injury detection for motorsport incidents, improve triage effectiveness, and assist in the development of safety standards.
Comparison of Vehicle-Based Crash Severity Metrics for Predicting Occupant Injury in Real-World Oblique Crashes

Abstract:Transportation Research Record, Ahead of Print. The flail space model (FSM) is currently used in U.S. roadside hardware crash testing as a means of assessing occupant injury risk using observed vehicle kinematics data. European roadside hardware crash tests use an FSM variant along with a variant of the acceleration severity index (ASI). Although the FSM and ASI are currently used in roadside hardware testing, other vehicle-based crash severity metrics exist. Previous research has focused on examining the ability of these metrics to predict injury in frontal crashes. Despite the Manual for Assessing Safety Hardware prescribing a significant number of oblique crash tests, there has been little research on how well these metrics predict real-world oblique crash injury. This study compared the ability of six different vehicle-based metrics to predict occupant injury in oblique crashes: maximum delta-v, occupant impact velocity, ridedown acceleration, ASI, occupant load criterion, and vehicle pulse index. The crash severity metrics were calculated from real-world crash pulse data recorded by event data recorders. Oblique crashes from the National Automotive Sampling System Crashworthiness Data System were used to train logistic regression models that predict moderate to fatal injuries. The models were then compared on a dataset of oblique crashes from the Crash Investigation Sampling System. The results of this study confirmed that vehicle-based metrics provide a reasonable means of predicting real-world occupant injury risk in oblique crashes and suggest little difference between the investigated metrics. In addition to the vehicle-based metrics, belt use and vehicle damage location were found to influence injury risk.

Abstract:With the technological development of forward-looking sensors, researchers are exploring their use not only for advanced driver assistance systems but also to gain important pre-crash information. Based on this pre-crash information, if the occupant motion inside the vehicle structure can be predicted for the oncoming crash scenario, then an optimal restraint strategy can be planned before the crash. This paper introduces a two-step FEM simulation based methodology for predicting the occupant severity in head-on crash scenarios. In the first step, we simulate the vehicle level model with different impact positions and relative approach angles. The results of these simulations, linear velocities in the longitudinal and lateral direction and angular velocities (roll, pitch, and yaw) during in-crash phase are the loading conditions for next simulation step (occupant level). This step simulates the motion of the driver in different crash scenarios. In this paper, we investigate the head, neck, and chest injury risks from vehicle-to-vehicle crash both traveling at 50 kilometers per hour. Prediction of the head injury criterion, identifying the cases where additional deployment of side-airbags and discussion of injury criteria with contour plots are the main outcome of this paper.
Numerical assessment of occupant responses during the bus rollover test: A finite element parametric study

Abstract:Rollover crashes of buses are usually associated with multiple impacts that can result in complex interactions between passengers and a bus superstructure. Although there have been a few field data studies that provide some insights into occupant injuries (e.g. severity and distribution of injuries) during the real-world bus rollover crash, because they had used post-crash data, the occupant kinematics and injury mechanisms were not completely detailed in their results. Based on a literature review, available numerical and experimental studies on a bus rollover safety have mainly focused on structural integrity rather than considering occupant responses in their assessment. In addition, their results about occupant responses in bus rollover crashes show some discrepancies in terms of the estimated injury distribution, severity, and causes. Therefore, the main objective of this study was to provide a more detailed understanding of the occupant kinematics and associated injury risk during the ECE R66 tilt table bus rollover test using validated finite element (FE) models. The ECE R66 tilt table rollover was simulated using a full finite element model of the bus. A 50th percentile male Hybrid III Anthropomorphic test device (ATD) and EuroSID-2re FE models were selected to simulate the occupant’s motion. Each ATD was seated adjacent to the impacted side wall and restrained with a 2-point seatbelt. Simulation parameters included two impact surface friction values and different side window conditions. The results indicated that both ATD estimated the highest injury risk when the partial ejection occurred. They predicted a similar injury risk for the head and thorax. The ES-2re estimated a very low risk of neck injury in all simulations, whereas the Hybrid III estimated the high risk of a neck injury. Finally, recommendations to potentially reduce the injuries were provided and possible future works were suggested.
Injury Biomechanics Evaluation of a Driver with Disabilities during a Road Accident-A Numerical Approach

Abstract:Numerical methods are often a robust way to predict how external mechanical loads affect individual biological structures. Computational models of biological systems have been developed over the years, reaching high levels of detail, complexity, and precision. In this study, two cases were analysed, differing in the airbag operation; in the first, the airbag was normally activated, and in the second case, the airbag was disabled. We analysed a model of a disabled person without a left leg who steers a vehicle using a specialized knob on the steering wheel. In both cases, a head-on collision between a car moving at an initial speed of 50 km/h and a rigid obstacle was analysed. We concluded that the activated airbag for a person with disabilities reduces the effects of asymmetries in the positioning of the belts and body support points. Moreover, all the biomechanical parameters, analysed on the 50th percentile dummy, i.e., HIC, seat belt contact force and neck injury criterion (Nij) support the use of an airbag. The resulting accelerations, measured in the head of the dummy, were induced into a finite element head model (YEAHM) to kinematically drive the head and simulate both accidents, with and without the airbag. In the latter, the subsequent head injury prediction revealed a form of contrecoup injury, more specifically cerebral contusion based on the intracranial pressure levels that were achieved. Therefore, based on the in-depth investigation, a frontal airbag can significantly lower the possibility of injuries for disabled drivers, including cerebral contusions.

Abstract:Musculoirritancy testing in the rabbit has been recognized as sufficiently sensitive to be highly predictive of human tolerance for intramuscularly administered drugs. A three-part study consisting of gross characterization of the lesion, microscopic observation and determination of serum activity of the enzyme creatine phosphokinase (CPK) has evolved as a standard test for evaluating single injection injury in the muscle. Injection and tissue processing procedures are described as well as a revised grading system for gross examination based on degrees of necrosis and hemorrhage, respectively. Based on 213 determinations, the control mean serum CPK activity was 262 +/- 113 I.U./liter. The importance of pretest acclimation of rabbits in establishing this value was emphasized. Designations of slight, moderate, or marked irritation are based on accumulative scoring of all rabbits at postinjection intervals of 1, 3, and 7 days. At 24 hr postinjection, serum CPK activities up to and including a range of 2000-3000 I.U./liter have been shown to be compatible with slight irritation and are considered predictive of human tolerance.
Preliminary injury risk estimation for occupants involved in frontal crashes by combining computer simulations and real crashes

Abstract:The fatality rate can be dramatically reduced with the help of emergency medical services. The purpose of this study was to establish a computational algorithm to predict the injury severity, so as to improve the timeliness, appropriateness, and efficacy of medical care provided. The computer simulations of full-frontal crashes with rigid wall were carried out using LS-DYNA and MADYMO under different collision speeds, airbag deployment time, and seatbelt wearing condition, in which a total of 84 times simulation was conducted. Then an artificial neural network is adopted to construct relevance between head and chest injuries and the injury risk factors; 37 accident cases with Event Data Recorder data and information on occupant injury were collected to validate the model accuracy through receiver operating characteristic analysis. The results showed that delta-v, seatbelt wearing condition, and airbag deployment time were important factors in the occupant’s head and chest injuries. When delta-v increased, the occupant had significantly higher level of severe injury on the head and chest; there is a significant difference of Head Injury Criterion and Combined Thoracic Index whether the occupant wore seatbelt. When the airbag deployment time was less than 20 ms, the severity of head and chest injuries did not significantly vary with the increase of deployment time. However, when the deployment time exceeded 20 ms, the severity of head and chest injuries significantly increased with increase in deployment time. The validation result of the algorithm showed that area under the curve = 0.747, p < 0.05, indicating a medium level of accuracy, nearly to previous model. The computer simulation and artificial neural network have a great potential for developing injury risk estimation algorithms suitable for Advanced Automatic Crash Notification applications, which could assist in medical decision-making and medical care.

Abstract:This study was designed to determine the success of various local anaesthesia techniques as assessed by the presence or absence of pain during restorative procedures performed by senior undergraduate dental students in a pedodontic clinic. Data gathered from the treatment of 151 patients by 43 students recorded the frequency of pain complaints during procedures for which local anaesthesia had been employed. It was found that 73% of the administrations were successful. The proportion of children who complained of pain during the operative procedure was significantly higher in those with a previous history of a painful dental experience.

Abstract:The paper presents the results of a numerical parametric study on the factors affecting passenger safety in a motorcoach frontal collision. The study is performed using the numerical model of the motorcoach sled test validated by the experimental data. The numerical model implements the rigid-body dynamics approach coupled with the finite-stiffness joints and deformable finite-element models of dummies. Three factors are parametrically studied: seating and restraint layout, seat stiffness and the shape of the input acceleration pulse. The positive effect of the seatbelt is demonstrated; the head injury criterion values for belted passenger do not exceed the safety limit for all considered seating layouts. The most dangerous seating layout is found to be the 50th percentile occupant with a 95th unbelted occupant sitting behind. Seat stiffness variation studies have shown the positive effect of plastic deformation in the seat structure. By varying the shape of the input acceleration pulse, the probability of a head injury is demonstrated to increase with the increase in the duration of maximum acceleration and time of its occurrence.

An examination of the performance of damaged energy-absorbing end terminals

Abstract:Objectives: Guardrail end terminals are designed to gradually decelerate vehicles during impact and protect vehicle occupants from severe injuries. It has been observed that some in-service end terminals are damaged, and it is unclear if their safety performance is still acceptable. The objectives of this study were to examine the conditions of in-service end terminals, and to evaluate the performance of damaged relative to undamaged end terminals in simulated impacts. Methods: Common damage patterns of guardrail end terminals were investigated by using post-crash pictures collected from the National Automotive Sampling System-Crashworthiness Data System (NASS-CDS). Conditions of in-service end terminals mounted along roads in portions of six U.S. states were examined by using a sample from the second Strategic Highway Research Program-Roadway Information Database (SHRP2-RID). Finite Element (FE) models of two minorly and three severely damaged ET-Plus systems, a commonly used energy-absorbing guardrail end terminal along U.S. roads, were developed. To evaluate the performance of the damaged ET-Plus systems, we performed impact simulations with vehicle-to-damaged ET-Plus models according to the National Cooperative Highway Research Program (NCHRP) 350, test conditions 3-30. Results: Of the 1000 in-service end terminal cases we investigated, 73% were undamaged, 18% had minor damage, and 8% had major damage. Increases in the average vehicle deceleration rates, maximum vehicle yaw angles, and vehicle local deformations were observed in simulated impacts with damaged ET-Plus end terminals relative to impacts with undamaged ET-Plus end terminals. For one damaged ET-Plus, a secondary collision was observed. Overall, we found that the damaged end terminals usually increased collision severity when compared with undamaged end terminals. Conclusions: The findings of this study point out the need for in-service performance evaluations and proper maintenance and repair practices of end terminals. The simulation models developed in this study could be further employed to investigate device performance in crash situations that are physically impractical to test and investigate the effects of site characteristics on device performance. The simulation models could also supplement crash tests to certify new hardware designs.

Abstract:OBJECTIVE: Serious head and cervical spine injuries have been shown to occur mostly independent of one another in pure rollover crashes. In an attempt to define a dynamic rollover crash test protocol that can replicate serious injuries to the head and cervical spine, it is important to understand the conditions that are likely to produce serious injuries to these 2 body regions. The objective of this research is to analyze the effect that impact factors relevant to a rollover crash have on the injury metrics of the head and cervical spine, with a specific interest in the differentiation between independent injuries and those that are predicted to occur concomitantly.METHODS: A series of head impacts was simulated using a detailed finite element model of the human body, the Total HUman Model for Safety (THUMS), in which the impactor velocity, displacement, and direction were varied. The performance of the model was assessed against available experimental tests performed under comparable conditions. Indirect, kinematic-based, and direct, tissue-level, injury metrics were used to assess the likelihood of serious injuries to the head and cervical spine.RESULTS: The performance of the THUMS head and spine in reconstructed experimental impacts compared well to reported values. All impact factors were significantly associated with injury measures for both the head and cervical spine. Increases in impact velocity and displacement resulted in increases in nearly all injury measures, whereas impactor orientation had opposite effects on brain and cervical spine injury metrics. The greatest cervical spine injury measures were recorded in an impact with a 15° anterior orientation. The greatest brain injury measures occurred when the impactor was at its maximum (45°) angle.CONCLUSIONS: The overall kinetic and kinematic response of the THUMS head and cervical spine in reconstructed experiment conditions compare well with reported values, although the occurrence of fractures was overpredicted. The trends in predicted head and cervical spine injury measures were analyzed for 90 simulated impact conditions. Impactor orientation was the only factor that could potentially explain the isolated nature of serious head and spine injuries under rollover crash conditions. The opposing trends of injury measures for the brain and cervical spine indicate that it is unlikely to reproduce the injuries simultaneously in a dynamic rollover test.

Abstract:This study developed a framework to generate injury prediction functions in order to evaluate the safety of occupants in traffic accidents, using human body model simulations to analyse the influences of crash severity, occupant stature, occupant posture, and restraint stiffness. The THUMS AM50-O Human Body Model (HBM) was used in a sled test setup. Six influencing parameters were included in injury prediction: impact velocity, occupant stature, Body Mass Index (BMI), torso angle, seating position, and seatbelt restraint force. Impact velocity was found as a significant influencing factor on skull injury, while BMI mainly affected chest injury. Sequential sampling method was used for generation of simulation matrix, and high-order interaction terms were identified. As injury prediction functions, quadratic regressions correlate better with the simulation results than other forms of regressions, the combinations of parameters in which also enhanced the robustness of injury prediction when considering uncertainty of input parameters. Using numerical models to generate injury prediction functions can better characterize occupant variations, such as sitting posture, than using traffic accident reconstruction. Such obtained functions can provide reasonable estimation on occupant injury risk based on accident information, and we also hope it may benefit vehicle safety improvement and triage protocol in medical treatments of traffic accident victims.
Analysis of finite element models for head injury investigation: reconstruction of four real-world impacts

Abstract:Previous studies have shown that both excessive linear and rotational accelerations are the cause of head injuries. Although the head injury criterion has been beneficial as an indicator of head injury risk, it only considers linear acceleration, so there is a need to consider both types of motion in future safety standards. Advanced models of the head/brain complex have recently been developed to gain a better understanding of head injury biomechanics. While these models have been verified against laboratory experimental data, there is a lack of suitable real-world data available for validation. Hence, using two computer models of the head/brain, the objective of the current study was to reconstruct four real-world crashes with known head injury outcomes in a full-vehicle crash laboratory, simulate head/brain responses using kinematics obtained during these reconstructions, and to compare the results predicted by the models against the actual injuries sustained by the occupant. Cases where the occupant sustained no head injuries (AIS 0) and head injuries of severity AIS 4, AIS 5, and multiple head injuries were selected. Data collected from a 9-accelerometer skull were input into the Wayne State University Head Injury Model (WSUHIM) and the NHTSA Simulated Injury Monitor (SIMon). The results demonstrated that both models were able to predict varying injury severities consistent with the difference in AIS injury levels in the real-world cases. The WSUHIM predicted a slightly higher injury threshold than the SIMon, probably due to the finer mesh and different software used for the simulations, and could also determine regions of the brain which had been injured. With further validation, finite element models can be used to establish an injury criterion for each type of brain injury in the future.
Parameter study for child injury mitigation in near-side impacts through FE simulations

Abstract:Objective: The objective of this study is to investigate the effects of crash-related car parameters on head and chest injury measures for 3- and 12-year-old children in near-side impacts. Methods: The evaluation was made using a model of a complete passenger car that was impacted laterally by a barrier. The car model was validated in 2 crash conditions: the Insurance Institute for Highway Safety (IIHS) and the US New Car Assessment Program (NCAP) side impact tests. The Small Side Impact Dummy (SID-IIs) and the human body model 3 (HBM3) (Total HUman Model for Safety [THUMS] 3-year-old) finite element models were used for the parametric investigation (HBM3 on a booster). The car parameters were as follows: vehicle mass, side impact structure stiffness, a head air bag, a thorax-pelvis air bag, and a seat belt with pretensioner. The studied dependent variables were as follows: resultant head linear acceleration, resultant head rotational acceleration, chest viscous criterion, rib deflection, and relative velocity at head impact. The chest measurements were only considered for the SID-IIs. Results: The head air bag had the greatest effect on the head measurements for both of the occupant models. On average, it reduced the peak head linear acceleration by 54 g for the HBM3 and 78 g for the SID-IIs. The seat belt had the second greatest effect on the head measurements; the peak head linear accelerations were reduced on average by 39 g (HBM3) and 44 g (SID-IIs). The high stiffness side structure increased the SID-IIs' head acceleration, whereas it had marginal effect on the HBM3. The vehicle mass had a marginal effect on SID-IIs' head accelerations, whereas the lower vehicle mass caused 18 g higher head acceleration for HBM3 and the greatest rotational acceleration. The thorax-pelvis air bag, vehicle mass, and seat belt pretensioner affected the chest measurements the most. The presence of a thorax-pelvis air bag, high vehicle mass, and a seat belt pretensioner all reduced the chest viscous criterion (VC) and peak rib deflection in the SID-IIs. Conclusions: The head and thorax-pelvis air bags have the potential to reduce injury measurements for both the SID-IIs and the HBM3, provided that the air bag properties are designed to consider these occupant sizes also. The seat belt pretensioner is also effective, provided that the lateral translation of the torso is managed by other features. The importance of lateral movement management is greater the smaller the occupant is. Light vehicles require interior restraint systems of higher performance than heavy vehicles do to achieve the same level of injury measures for a given side structure.
