Abstract:Objective To investigate the influence of meniscectomy on limb alignment. Methods From June 2000 to March 2004, a series of 45 patients was involved in this study, included 28 men and 17 women with a mean age of 27 years (range 15-42 years). All these patients underwent arthroscopic operation for meniscal injury, and they could straighten their knee in maintaining standing preoperatively. With arthroscopy the injury of medial meniscus was found in 6, the lateral injury in 21, lateral discoid meniscus in 12, synovial fold symptom or hypertrophy of subpatellar fat pad with normal menisci in 6. Partial meniscectomy was performed in 22, subtotal meniscectomy in 10 because of peripheral meniscal tear, total meniscectomy in 7, and excision of synovial fold or subpatellar fat pad in 6 cases with normal menisci. All these patients were examined with weight-bearing X-rays of the total lower extremities pre- and post-operation. The limb alignment was measured, the intrinsic varying distance (IVD) was defined as the distance from center of the knee to femoro-tibial mechanical axes. The difference of IVD between pre- and post-operation was calculated as ΔIVD. The correlation analysis was conducted by the amount of excision, the change of limb alignment and the ΔIVD. Results The change of limb alignment increased with the amount of meniscal excision. The difference of ΔIVD between the patients received total or subtotal meniscectomy and those received partial or no meniscectomy was significant statistically(P 0.01). Meniscectomy for lateral discoid meniscus resulted in more obviously change of limb alignment than that for injury of ordinarily lateral meniscus, and the difference also was significant(P 0.05). Conclusion Subtotal or total meniscectomy may result in the change of limb alignment with deformity of genu varum or valgum. The genu valgum increases more evidently after meniscectomy for lateral discoid meniscus. Prosthesis should be considered after meniscectomy, especially post total or subtotal meniscectomy, in cases of obvious change of limb alignment or degeneration of articular cartilage.
Identification of Kinetic Abnormalities in Male Patients after Anterior Cruciate Ligament Deficiency Combined with Meniscal Injury: A Musculoskeletal Model Study of Lower Limbs During Jogging

Abstract:There is little known about kinetic changes in anterior cruciate ligament deficiency (ACLD) combined with meniscal tears during jogging. Therefore, 29 male patients with injured ACLs and 15 healthy male volunteers were recruited for this study to investigate kinetic abnormalities in male patients after ACL deficiency combined with a meniscal injury during jogging. Based on experimental data measured by an optical tracking system, a subject-specific musculoskeletal model was employed to estimate the tibiofemoral joint kinetics during jogging. Between-limb and interpatient differences were compared by the analysis of variance. The results showed that decreased knee joint forces and moments of both legs in ACLD patients were detected during the stance phase compared to the control group. Meanwhile, compared with ACLD knees, significantly fewer contact forces and flexion moments in ACLD combined with lateral and medial meniscal injury groups were found at the mid-stance, and ACLD with medial meniscal injury group showed a lower axial moment in the loading response (p < 0.05). In conclusion, ACLD knees exhibit reduced tibiofemoral joint forces and moments during jogging when compared with control knees. A combination of meniscus injuries in the ACLD-affected side exhibited abnormal kinetic alterations at the loading response and mid-stance phase.
Residual Varus Alignment after Posterior-Stabilized Total Knee Arthroplasty Limits Medial Soft Tissue Remodeling

Abstract:Background Proper lower limb alignment and soft tissue balance are significant indicators to measure the success of total knee arthroplasty (TKA). Previous studies have confirmed that soft tissue relaxation around the knee after TKA will change over time; however, the relationship between lower limb alignment and soft tissue balance after TKA remains unclear. We studied (1) whether the change of soft tissue balance around the knee with time after posterior-stabilized (PS) TKA would affect the alignment of the lower limbs; (2) Whether the accuracy of lower limb alignment during PS TKA affects postoperative soft tissue remodeling. Methods In this study, 100 patients were recruited after PS TKA. Among them, 50 patients with a hip knee ankle (HKA) angle of ≤ ± 3° were set as the neutral group, and 50 patients with an HKA angle of > ± 3° were set as the deviation group. The imaging results measured the HKA angle before the operation as well as the HKA, varus, and valgus angles at 1, 3, 6, 12, and 24 months after TKA. Clinical assessment included range of motion (ROM), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and Knee Society Score (KSS). Results Eight people were excluded from the study. After the exclusion, the study enrolled 47 patients in the neutral group and 45 patients in the deviant group and were followed for up to 2 years. There was no statistical significance in mean varus angles as well as HKA angle changes during the follow-up phase of each groups ( P > 0.05). The mean valgus angles of the patients in the neutral group group were 2.47°, 3.45°, 3.63°, 3.60° and 3.63°, and in the deviation group were 2.45° ( P = 0.841), 2.88° ( P < 0.001), 3.07° ( P < 0.001), 3.06° ( P < 0.001), and 3.10° ( P < 0.001). ROM, WOMAC and KSS of the two groups were significantly improved after operation, with no difference between the two groups. Conclusion This study shows that whether the alignment is accurate or not in the early stage after TKA, the relaxation of the medial and lateral soft tissues of the knee joint change; however, this change will not significantly affect the alignment of the lower limbs. Postoperative residual varus deformity limits medial soft tissue remodeling. Level of evidence III.
Effect of Lower Limb Alignment in Medial Meniscus–Deficient Knees on Tibiofemoral Contact Pressure

Abstract:Background: Degenerative medial meniscal tears and subsequent partial meniscal resection compromise meniscal function and lead to an overload of the medial compartment. In addition, lower limb alignment plays a key role in load distribution between the medial and lateral knee compartments, and varus alignment is a potential risk factor for medial osteoarthritis. Purpose/Hypothesis: The purpose of this biomechanical study was to investigate the effect of valgus and varus alignment on peak pressure and contact area in knees with concomitant horizontal medial meniscal tears and subsequent leaflet resection. It was hypothesized that varus alignment in combination with meniscal loss leads to the highest peak pressure within the medial compartment. Study Design: Controlled laboratory study. Methods: Six fresh-frozen human cadaveric knees were axially loaded using a 1000-N compressive load in full extension with the mechanical axis rotated to intersect the tibial plateau at 40%, 45%, 50%, 55%, and 60% of its width (TPW) to simulate varus and valgus alignment. Tibiofemoral peak contact pressure and contact area of the medial and lateral compartments were determined using pressure-sensitive foils in each of 4 different meniscal conditions: intact, 15-mm horizontal tear of the posterior horn, inferior leaflet resection, and resection of both leaflets. Results: The effect of alignment on peak pressure (normalized to the neutral axis) within the medial compartment in cases of an intact meniscus was measured as follows: varus shift resulted in a mean increase in peak pressure of 18.5% at 45% of the TPW and 37.4% at 40% of the TPW, whereas valgus shift led to a mean decrease in peak pressure of 8.7% at 55% of the TPW and 23.1% at 60% of the TPW. Peak pressure changes between the intact meniscus and resection within the medial compartment was less in valgus-aligned knees (0.21 MPa at 60% TPW, 0.59 MPa at 50% TPW, and 0.76 MPa at 40% TPW). Contact area was significantly reduced after partial meniscal resection in the neutral axis (intact, 553.5 ± 87.6 mm 2 ; resection of both leaflets, 323.3 ± 84.2 mm 2 ; P < .001). This finding was consistent in any alignment. Conclusion: Both partial medial meniscal resection and varus alignment led to an increase in medial compartment peak pressure. Valgus alignment prevented medial overloading by decreasing contact pressure even after partial meniscal resection. A horizontal meniscal tear did not influence peak pressure and contact area even in varus alignment. Clinical Relevance: As a clinical consequence, partial meniscal resection should be avoided to maintain the original biomechanical behavior, and the mechanical axis should be taken into account if partial meniscectomy is necessary.

Abstract:ABSTRACT Introduction Knee meniscus injury is a common sports injury, and minimally invasive surgery under knee arthroscopy has become an ideal method to treat meniscus injuries. This surgery rehabilitation has been improved, and several studies on the effects of functional exercise in the range of treatment are still inconclusive. Objective Study the functional exercise rehabilitation effects in patients after sports meniscus injury. Methods Twenty patients with meniscus-medial injury being operated on were selected, including eight men and 12 women. They were randomly divided into neuromuscular and strength training groups (11). Signs and symptoms were assessed before and eight weeks after treatment. JOA score indices and gait tests were compared. The impact of rehabilitation differences was evaluated in each group. Results Eight weeks after rehabilitation in both groups, the scores of the strength training group were higher than the neuromuscular group; the difference between the groups was statistically significant (P<0.05). Conclusion Functional exercise accelerates joint recovery, reflected in increased strength of adjacent muscles. The muscle and joint training effects on postoperative meniscus injury are worthy of recognition. The baropodometry revealed distinctions in walking patterns between different rehabilitation methods. From the perspective of this research, rehabilitation methods combined with proprioceptive exercises are complementary. Evidence Level II; Therapeutic Studies - Investigating the result.

Abstract:The prevalence of varus knee malalignment among junior and adult football players (FP) has proven to be higher compared to other sports. No causal relationship has yet been found, as genu varum can be assumed to be an independent risk factor for the development of knee osteoarthritis. The purpose of this study is to compare knee alignment measurements and sport-specific data of adolescent football players and referees (REF). Knee alignment was detected by measuring the intercondylar/intermalleolar distance (ICD/IMD) as well as the Hip–Knee–Ankle angle (HKA) using a standardized digital frontal-plane photograph. Anthropometric and sports-related data (training/match exposure, seasons actively played, etc.) were collected by means of questionnaires (Clinical trial registration number: DRKS00020446). A total of 28 male FP and 29 male adolescent REF were included in the survey. The mean age was 17.4 ± 0.7 years. The two groups did not differ significantly in age, height, weight, BMI, and overall football/refereeing exposure per week (FP vs. REF: 274 vs. 285 min/week, p = 0.61). The HKA of the FP was significantly lower (toward varus) than that of the REF (177.6° ± 2.4° vs. 179.0° ± 2.4°; p < 0.001). However, ICD did not significantly differ (FP: 17 ± 25 mm, REF: 13 ± 27 mm; p = 0.55). The football environment with frequent football exposure seems to have an influence on leg axis deviation in FP compared to REF. For prevention of knee osteoarthritis in FP, an advanced understanding of leg axis development in adolescent players is essential and, therefore, needs further research.

Biomechanical Evaluation of Preoperative Rehabilitation in Patients of Anterior Cruciate Ligament Injury

Abstract:ObjectivesTo investigate the biomechanical characteristics of patients with anterior cruciate ligament (ACL) injury by gait analysis, surface electromyography (SEMG), and proprioception test, and provide rehabilitation suggestions according to the results. MethodsIn this retrospective cohort study, 90 adults with unilateral ACL injury, ranging in age from 19 to 45 years (66 men and 24 women, average age: 30.03 ± 7.91) were recruited for this study form May 2018 to July 2019. They were divided into three groups according to the time after the injury: group A (3‐week to 1.5‐month), group B (1.5‐month to 1 year), and group C (more than 1 year). The SEMG signals were collected from the bilateral rectus femoris (RF), vastus medialis (VM), and vastus lateralis (VL) and the root mean square (RMS) were used to assess muscular activity. SEMG were used to analyze muscles function, gait analysis was used to evaluate the walking stability, balance and location assessment were used to analyze the proprioception. ResultsThrough the comparison between bilateral limbs, all muscles strength shown decreased (RF: 239.94 ± 129.70 vs 364.81 ± 148.98, P = 0.001; VM: 298.88 ± 175.41 vs 515.79 ± 272.49, P = 0.001; VL:389.54 ± 157.97 vs 594.28 ± 220.31, P < 0.001) and the division of proprioception became larger (tandem position: 7.79 ± 1.57 vs 6.33 ± 1.49, P = 0.001; stance with one foot: 8.13 ± 0.84 vs 7.1 ± 0.57, P = 0.003; variance of 30°: 6.96 ± 3.15 vs 4.45 ± 1.67, P = 0.03; variance of 60°: 4.64 ± 3.38 vs 2.75 ± 1.98, P = 0.044) in the injured side when compared to the non‐injured and 26 gait parameters were shown difference in group A. In group B, the muscle strength of VL shown decreased (VL: 381.23 ± 142.07 vs 603.9 ± 192.72, P < 0.001) and the division of location of 30° became larger (7.62 ± 4.98 vs 4.33 ± 3.24, P = 0.028) in the injured side when compared to the non‐injured side and there were eight gait parameters that showed differences. In group C, the muscle strength and proprioception showed no differences and only 16 gait parameters showed differences between the bilateral limbs. ConclusionThe results proved the deterioration of proprioception in 30° of injured side will not recover and non‐injury side and will become worse after 1 year from the injury; among the VL, VM, and RF, the recovery rate of VL is the slowest and bilateral straight leg raising (SLR) (30°) is the best way to train it; the gait stability will be worse after 1 year from the injury. Therefore, we suggest that the training for proprioception in 30° and VL are important for the rehabilitation, and the ACL reconstruction should be performed within 1 year.

The impact of preoperative MRI-detected lateral meniscal intra-substance signal abnormalities on mid-term functional outcomes following mobile-bearing unicompartmental knee arthroplasty

Abstract:Purpose: There is a dilemma as to whether the presence of degenerative changes of lateral meniscus is a contraindication to medial unicompartmental knee arthroplasty (UKA). Therefore, the purpose of this study is to assess the influence of preoperative MRI-detected lateral meniscal intra-substance signal abnormalities on mid-term functional outcomes following mobile-bearing UKA. Methods: We performed a retrospective review of the record on a consecutive series of patients who have undergone mobile-bearing medial UKA from September 2020 to June 2023. The mean duration of follow-up was 2.34 years. All records were collected from case system. MRI assessment of lateral meniscus was performed with the use of the Stoller's classification system. Patients were categorized into two groups (grade 0 and ≥ grade 1). Patient-reported outcomes were assessed with the use of the American Knee Society functional Score (AKSS-F), American Knee Society Objective Score (AKSS-O), and Oxford Knee Score (OKS) preoperatively and at latest follow-up. Furthermore, range of motion (ROM) and hip-knee-ankle angle (HKA) were measured in preoperative and postoperative periods. Results: A total of 92 patients (101 knees) were included in our study. No differences in AKSS-F, AKSS-O, OKS, HKA, or ROM were found between those who showed normal or abnormal signal change of lateral meniscus preoperatively (P < 0.05). Furthermore, there were also no significant differences between two groups concerning AKSS-F, AKSS-O, OKS, HKA, or ROM at latest postoperative follow-up (P < 0.05). Age and duration of disease were independent predictors of low postoperative AKSS-F and AKSS-O (P < 0.05). Nevertheless, ROM was an independent predictor of high postoperative AKKS-O (P < 0.05). Age and female were independent predictors of high postoperative OKS (P < 0.05). Conclusion: The presence of preoperative MRI-detected lateral meniscal degenerative changes did not affect mid-term functional outcomes in patients who underwent mobile-bearing medial UKA. On the basis of the results of the current study, we believe that the presence of degenerative changes of lateral meniscus is acceptable in mobile-bearing medial UKA.
Effects of preoperative valgus deformity in patients undergoing neutrally aligned total knee arthroplasty: A retrospective cohort study with a minimum five-year follow-up

Abstract:Objectives: This study aimed to investigate the relationship between the severity of preoperative valgus deformity and clinical outcomes of neutrally aligned total knee arthroplasty (TKA). Patients and methods: A total of 376 knees with valgus deformity who underwent TKA from January 2006 to March 2014 were retrospectively screened, and 287 knees (242 patients; 32 males, 210 females; mean age: 64.5±8.8 years; range, 35 to 83 years) aligned neutrally after the operation were included. Patients were divided into four groups based on the preoperative hip-knee-ankle (HKA): mild (0°< HKA ≤5°, n=94), moderate (5°< HKA ≤10°, n=74), severe (10°< HKA ≤15°, n=75), and very severe (HKA >15°, n=44) groups. Range of motion (ROM), Knee Society Score (KSS), Visual Analog Scale (VAS) dynamic pain scores, and Forgotten Joint Score (FJS) were evaluated. Mechanical failures were recorded to assess prosthesis survival. A survival rate analysis was performed using Kaplan-Meier survival analysis. Results: The degree of preoperative valgus deformity did not have a significant impact on the postoperative ROM, KSS, VAS dynamic pain scores, or FJS at the last follow-up. There were no significant differences in cumulative survival rates of neutrally aligned TKAs at 10 years between the four groups (p=0.513). Conclusion: The severity of preoperative valgus deformity did not affect the clinical outcomes of neutrally aligned TKAs in the minimum five-year follow-up. Additionally, it did not impact the survival rates of neutrally aligned TKAs over 10 years.
Finite element study of stress distribution in medial UKA under varied lower limb alignment

Abstract:Knee osteoarthritis (KOA) has a high incidence among the elderly, significantly impacting their quality of life and overall health. Medial unicompartmental knee arthroplasty (UKA) is an excellent choice for treating knee single-compartment lesions, and lower limb alignment has a profound impact on medial UKA. To explore the influence of different lower limb alignments on medial UKA. In this study, we selected MR and CT data of healthy adult male knee joints to establish a complete finite element analysis (FEA) model of the knee joint. After validation, we established a finite element model of medial UKA. Subsequently, we created 60 sets of FEA models with different lower limb alignments to analyze the impact of different lower limb alignments on medial UKA. A vertical load of 1000 N was applied to the FEA models with different lower limb alignments. During the process of shifting the Mikulicz line from the midpoint of the knee joint towards the medial side, the lower limb load was primarily concentrated on the medial compartment. The stress values of the lateral meniscus, tibial cartilage, and femoral cartilage gradually decreased. ROI (region of interest) 1 and ROI 2 showed the maximum principal strain changes, while ROI 3 and ROI 4 exhibited less pronounced fluctuations, with the maximum principal strain roughly proportionally increased. During the process of shifting the Mikulicz line towards the lateral side from the midpoint of the knee joint, the stress on the lateral compartment increased observably. ROI 1, ROI 2, ROI 3, and ROI 4 showed decreased maximum principal strains, approximately inversely proportional changes, but the overall reduction was relatively small. Different lower limb alignments have a profound impact on the short- and long-term joint function after UKA. When the Mikulicz line is 10 mm inside the midpoint of the knee joint or slightly outside, there is a relatively lower risk of tibial component fractures, lower stress on the lateral compartment, and lower load on the prosthesis. During medial UKA, measures such as bone resection and prosthesis selection should be taken to ensure that the Mikulicz line is in the ideal position.

Dynamic Between-Leg Differences While Walking in Anterior Cruciate Ligament–Deficient Patients with and Without Medial Meniscal Posterior Horn Tears

Abstract:Background: Patients with anterior cruciate ligament-deficient (ACLD) knees with medial meniscal posterior horn tears (MMPHTs) have been reported to demonstrate a combined stiffening and pivot-shift gait pattern compared with healthy controls. Movement asymmetries are implicated in the development and progression of osteoarthritis. Purpose: To investigate the knee kinematics and kinetic asymmetries in ACLD patients with (ACLD + MMPHT group) and without (ACLD group) MMPHTs while walking on level ground. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 15 patients with isolated unilateral ACL ruptures, 10 with unilateral ACL ruptures and MMPHTs, and 22 healthy controls underwent gait testing between January 2014 and December 2016. Between-leg differences (BLDs) in knee kinematics and kinetics were compared among participants in all groups. Results: The ACLD + MMPHT group demonstrated significantly greater BLDs in knee moments in the sagittal plane during the loading response phase than the ACLD and control groups. Compared with the control group, the ACLD and ACLD + MMPHT groups demonstrated significantly greater BLDs in knee angles in the sagittal plane during the midstance and terminal stance phases. Compared with the control group, significantly greater BLDs in knee rotation moments were found throughout the stance phase in both the ACLD and the ACLD + MMPHT groups. BLDs in lateral ground-reaction forces (GRFs) in the ACLD + MMPHT and ACLD groups were both significantly greater than the control group during the loading response phase. BLDs in anterior GRFs in the ACLD + MMPHT and ACLD groups were both significantly greater than the control group during the loading response phase. Only the ACLD + MMPHT group demonstrated greater BLDs in vertical GRFs than the control group during the loading response phase, while no significant differences were observed between the ACLD and control groups. Conclusion: The ACLD + MMPHT group demonstrated significantly more knee flexion moment asymmetries than the ACLD and control groups during the loading response phase. Both the ACLD + MMPHT and the ACLD groups demonstrated significant knee angle and moment asymmetries in the sagittal plane during the terminal stance phase than the control group. Both the ACLD + MMPHT and the ACLD groups demonstrated knee rotation moment asymmetries during the midstance and terminal stance phases compared with the control group. A rehabilitation program for ACLD patients both with and without MMPHTs should take into consideration these asymmetric gait patterns.

Effect of a Neuromuscular Training on Knee Valgus, Range of Motion and Muscle Strength of Soccer Players With Dynamic Knee Valgus

Abstract:Background and Aims The present study aims to investigate the effect of a neuromuscular training program on knee valgus angle, and range of motion (ROM) and muscle strength of hip and ankle in male adolescent football players with dynamic knee valgus (DKV). Valgus knee is one of the most common neuromuscular abnormalities that is involved in causing non-contact damage to the anterior cruciate ligament (ACL). Methods This is a quasi-experimental study. Participants were 25 adolescent male football players with DKV who were purposefully selected from Shahin Football Academy in Amol, Iran and randomly divided into two groups of control and training. The existence of DKV was assessed by Squats test and the valgus angle were assessed by jump-landing test and Kinovea software. The abduction and external rotation strength and ROM of hip and the dorsiflexion and plantarflexion strength and ROM of ankle were evaluated with Manual Muscle Testing and a goniometer, respectively. After performing the pre-test assessments, the subjects in the training group performed neuromuscular training for 8 weeks, while the control group also participated in the physical fitness program of Shahin Football Academy. At the end, assessments were performed again in two groups. To examine the differences, repeated measures analysis of variance and Bonferroni post hoc test were used in SPSS software, version 24. The significance level was set at 0.05. Results The results showed a significant difference in knee valgus angle (P=0.001), abduction ROM of hip (P=0.01), external rotation ROM of hip (P=0.01), dorsiflexion ROM of ankle with knee extension (P=0.001), dorsiflexion ROM of ankle with knee flexion (P=0.001), hip abduction strength (P=0.002), hip external rotation strength (P=0.01), and plantarflexion strength of ankle (P=0.03) between the control and training groups. However, the results showed no significant difference in plantarflexion ROM of ankle and dorsiflexion strength of ankle (P>0.05). After one month, the results showed the sustainability of the effect of training on abduction and external rotation ROM of hip, dorsiflexion ROM of ankle with knee extension and flexion, and dorsiflexion and plantarflexion strength of ankle. Conclusion The neuromuscular training can reduce the risk of non-contact ACL injury in male football players with DKV by improving strength and ROM of hip and ankle.
Biomechanical effects of femoral prosthesis misalignment on the structure of the lateral compartment during medial unicompartmental knee arthroplasty in osteoporotic patients

Abstract:Background: This study aims to investigate the impact of varying coronal alignments of femoral prostheses on stress and strain distributions within the lateral compartment following unicompartmental knee arthroplasty (UKA) in patients with normal bone density and osteoporosis using finite element analysis. Additionally, it examines the relationship between osteoporosis and the progression of osteoarthritis in the lateral compartment postoperatively. Methods: UKA models were developed for both normal bone and osteoporotic conditions using a validated finite element model of the knee. Seven alignment conditions for the femoral prosthesis were simulated: 0° (neutral alignment), varus angles of 3°, 6°, and 9°, and valgus angles of 3°, 6°, and 9°, resulting in a total of 14 scenarios. Stress and strain distributions in the meniscus, tibial cartilage, and femoral cartilage of the lateral compartment were evaluated. Results: The results indicated that stress and strain in the meniscus, tibial cartilage, and femoral cartilage of the lateral compartment increased with greater varus alignment and decreased with greater valgus alignment in both normal and osteoporotic models. At equivalent alignment angles, stress and strain were consistently higher in the osteoporotic model (M2) compared to the normal bone model (M1), although the peak equivalent stress in the tibial cartilage was lower in the M2 model than in the M1 model. Conclusions: In patients with osteoporosis undergoing fixed-bearing medial UKA, varus malalignment of the femoral prosthesis can lead to increased stress and strain in the lateral compartment's meniscus, tibial cartilage, and femoral cartilage. These findings suggest that osteoporosis may contribute to abnormal stress and strain distributions in the lateral compartment following UKA, potentially accelerating the progression of osteoarthritis in this region postoperatively.

Abstract:Objective: This study aims to quantitatively assess the relationship between the patella alignment and morphology and knee osteoarthritis (KOA), as well as the kinematics and kinetics of the knee, using gait analysis. Methods: Eighty age-matched patients with KOA and control subjects were evaluated. Incident radiographic osteoarthritis (iROA) was identified using a Kellgren-Lawrence (KL) grade of ≥ 2. The modified Insall-Salvati ratio (Mod-ISR), patellar tilt angle (PTA), and patella index (PI) were utilized to evaluate the sagittal and transverse alignment of the patella and its morphology, respectively. Regression analyses were conducted to explore associations between patellar measurements and KOA, iROA, kinematics, and kinetics. Results: Significant differences were observed between the control and KOA groups in terms of KL grade, patella alta, abduction angle, and reaction force to the ground (P < 0.05, respectively). Following adjustment for covariates, a significant positive association was found between patella alta and KOA (OR = 0.307, 95%CI: 0.103 to 0.918, P = 0.035). Additionally, a significant negative association was observed between PTA and abduction angle (B = -0.376, 95%CI: -0.751 to -0.002; P = 0.049). The PI exhibited a statistically significant association with log-transformed vertical ground reaction force (B = 0.002, 95%CI: 0.001 to 0.003, P = 0.002). Furthermore, adjustment for covariates did not reveal any significant correlations with other indicators (P > 0.05, respectively). Conclusion: This study provides further evidence that proper alignment and morphology of the patella might be associated with maintaining normal biomechanical function. In addition, intervention measures targeting relevant patellar parameters, such as Mod-ISR, PTA, and PI, may positively impact KOA treatment outcomes.
Effects of proprioceptive training in the recovery of patients submitted to meniscus surgery: systematic review and meta-analysis

Abstract:Objective To evaluate the effects of proprioceptive training on rehabilitation of knee after arthroscopic partial meniscectomy (APM). Design PubMed, EMBASE, The Cochrane Library, Web of Science, China National Knowledge Infrastructure, Technology Periodical Database, WanFang Data and China Biology Medicine were searched until December 2021 for randomised controlled trials. Participants Patients who have undergone APM for meniscus injury caused by traumatic tear. Results A total of 9 studies with 453 patients were included in this study for meta-analysis, and 2/9 with high quality, 6/9 with moderate quality. Based on very low quality evidence, the pooled effect showed significant improvement for proprioceptive training group in proprioception test (p<0.05, I 2 =18%), knee extensor muscle strength (p<0.05, I 2 =29%), knee flexor muscle strength (p<0.05, I 2 =0%) and knee function score (p<0.05, I 2 =0%) compared with conventional training group in patients after APM. Conclusion Based on very low quality, adding proprioceptive training to conventional rehabilitation programmes might be beneficial to promote functional recovery for patients after APM. It is necessary to carry out more samples and higher quality large-scale studies to provide high evidence in the future. PROSPERO registration number CRD42020213201.

Predicting the Risk of Injuries Through Assessments of Asymmetric Lower Limb Functional Performance: A Prospective Study of 415 Youth Taekwondo Athletes

Background:The impact of interlimb asymmetries on sport injuries is unclear because of inconsistent findings, and there is a lack of research on youth athletes and the sport of taekwondo.Purpose:To examine the effects of functional interlimb asymmetries on noncontact lower limb injuries in youth athletes.Study Design:Cohort study; Level of evidence, 2.Methods:A total of 415 taekwondo athletes (318 boys and 97 girls) aged 6 to 17 years underwent baseline testing to determine interlimb asymmetries through the single-leg countermovement jump (CMJ), hop, and triple hop tests as well as the Star Excursion Balance Test. The athletes were then evaluated for 12 months to observe the occurrence of noncontact lower limb injuries.Results:During the study, 98 athletes (70 boys and 28 girls) sustained at least 1 noncontact lower limb injury. Athletes with higher interlimb asymmetries in single-leg CMJ height showed a significantly increased risk of noncontact lower limb injuries (boys: odds ratio [OR], 1.053 [95% CI, 1.027-1.080], P < .001; girls: OR, 1.070 [95% CI, 1.016-1.128], P = .011). Asymmetry in single-leg CMJ height of ≥15.28% was found to be the cutoff point for predicting noncontact lower limb injuries in boys (OR, 4.652 [95% CI, 2.577-8.398]; P < .001).Conclusion:This study highlights the utility of interlimb asymmetries in unilateral jump performance as a tool for assessing the risk of noncontact lower limb injuries in youth taekwondo athletes of both sexes. A proper evaluation of interlimb asymmetries may improve prevention strategies for youth athletes.
