Abstract:Objective To assess the image quality (IQ) of an iterative reconstruction (IR) technique(sinogram affirmed iterative reconstruction,SAFIRE) for upper limb intravascular fistula CT angiography on a second generation dual-source CT scanner and determine the optimal dose level using IR with IQ comparable to filtered back projection(FBP).Methods The experiment was a prospective study.A total of 150 patients were averagely divided into 5 groups to perform upper limb intravascular fistula CT angiography.The control group was scanned with conventional tube output (120 kV,110 mAs) and images were reconstructed with FBP.The tube current of the other 4 groups (reconstructed with SAFIRE) was successively reduced to 70％,60％,50％ and 40％ of that of control group.Quantitative measurements of signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were obtained in two regions of interest (ROI) in each group.The ROIs were placed at the subclavian artery near shoulder joint,and the stoma of radial artery and cephalic vein.Analysis of variance (ANOVA) was used for comparisons of objective evaluation parameters (SNR,CNR) and radiation dose (CTDIvol,DLP,E) among 5 groups.Two radiologists graded IQ in a blinded fashion on a 4-point scale (4-excellent,3-good,2-fair and 1-poor).The Kruskal-Wallis nonparametric test was performed for detection of differences in subjective evaluation of IQ among groups.Results The radiation doses of groups 77,66,55 and 44 mAs were 70％,55％,40％ and 25％ of the control group respectively.The differences of SNR and CNR among groups 77,66,44 mAs and the control group were statistically significant in ROI1 (F =24.018,20.386,P ＜ 0.05),SNR and CNR of group 77 and 66 mAs were higher than the control group while group 44 mAs was lower.The difference of SNR and CNR among group 55 mAs and the control group was not statistically significant in ROI1 (P ＞ 0.05).The differences of SNR and CNR among groups 77,66,55 mAs and the control group were statistically significant in ROI2 (F =15.934,13.818,P ＜ 0.05),and groups 77,66 and 55 mAs had higher SNR and CNR.The difference of SNR and CNR between group 44 mAs and the control group was not statistically significant in ROI2 (P ＞ 0.05).Groups 77,66,55 mAs and the control group had higher scores of the subjective IQ (≥ 3) which was descended in group 44 mAs.The differences of contrast,sharpness,noise and general impression of images among the 5 groups were statistically significant by Kruskal-Wallis nonparametric test(H =10.268,14.542,15.840,11.014,P ＜ 0.05).Using the criterion of excellent IQ(score 4),the ROC curve of dose levels and IQ acceptability established a reduction of 60％ of effective dose [Group 55 mAs:(3.545 ± 1.396) mSv] as an optimum cutoff point (AUC:0.844,95％ CI:0.705-0.982).Conclusions Iterative reconstruction technique could provide a dose reduction to 40％ (3.545 mSv) compared with the filtered back projection in upper limb intravascular fistula CT angiography without reducing equivalent image quality.

Abstract:Rationale and Objectives This study aims to compare the diagnostic confidence of photon-counting detector CT angiography (PCD-CTA) depending on the used vascular reformatting kernels with digital subtraction angiography (DSA) as diagnostic reference standard in peripheral arterial occlusive disease (PAOD). Material and Methods In 39 patients, 45 lower extremity PCD-CTA with subsequent DSA were analyzed. Advanced PAOD (Fontaine stage 4) was ascertained in 77.8% of patients. CTA post-processing comprised three vascular kernels (Bv36/48/56). Objective image quality assessment included vessel attenuation, image noise, contrast-to-noise (CNR) and signal-to-noise ratios (SNR). Subjective evaluation of calcium blooming, vessel sharpness, luminal attenuation and image noise was performed by three radiologists. Diagnostic performance and concordance to DSA were assessed. Results The luminal attenuation remained kernel-independent constant. With sharper kernels, image noise increased substantially, while SNR and CNR decreased. Subjective reduction of calcium blooming and increased vessel sharpness were noted for the sharp Bv56 kernel. While sensitivity in stenosis quantification was comparable between kernels (81.6% vs. 81.5% vs. 81.0%, p = 0.797), specificity increased slightly higher sharpness (71.1% vs. 76.9% vs. 79.6%, p = 0.067). Diagnostic concordance of stenosis ratings compared to DSA increased likewise (Bv36 vs. Bv56, p = 0.002). Severe crural vessel calcifications had no influence on sensitivity, regardless of kernel selection. Contrarily, specificity was substantially worse in severely calcified tibial vessels but could be improved by using the sharp Bv56 kernel (Bv36 vs. Bv56 p = 0.024). Diagnostic confidence was highest for Bv56. Conclusion In lower leg PCD-CTA, sharp convolution kernels increase diagnostic confidence compared to DSA by improved vessel delineation and reduced calcium blooming with acceptable image noise.

Abstract:Purpose To assess the impact of different quantum iterative reconstruction (QIR) levels on objective and subjective image quality of ultra-high resolution (UHR) coronary CT angiography (CCTA) images and to determine the effect of strength levels on stenosis quantification using photon-counting detector (PCD)-CT. Method A dynamic vessel phantom containing two calcified lesions (25 % and 50 % stenosis) was scanned at heart rates of 60, 80 and 100 beats per minute with a PCD-CT system. In vivo CCTA examinations were performed in 102 patients. All scans were acquired in UHR mode (slice thickness0.2 mm) and reconstructed with four different QIR levels (1–4) using a sharp vascular kernel (Bv64). Image noise, signal-to-noise ratio (SNR), sharpness, and percent diameter stenosis (PDS) were quantified in the phantom, while noise, SNR, contrast-to-noise ratio (CNR), sharpness, and subjective quality metrics (noise, sharpness, overall image quality) were assessed in patient scans. Results Increasing QIR levels resulted in significantly lower objective image noise (in vitro and in vivo: both p < 0.001), higher SNR (both p < 0.001) and CNR (both p 0.008). Subjective noise of in vivo images significantly decreased with increasing QIR levels, resulting in significantly higher image quality scores at increasing QIR levels (all pairwise p < 0.001). Qualitative sharpness, on the other hand, did not differ across different levels of QIR (p = 0.15). Conclusions The QIR algorithm may enhance the image quality of CCTA datasets without compromising image sharpness or accurate stenosis measurements, with the most prominent benefits at the highest strength level.

Abstract:Background: Calcium blooming causes stenosis overestimation on coronary CTA. Objective: To evaluate the impact of virtual monoenergetic imaging (VMI) reconstruction level on coronary artery stenosis quantification using a photon-counting detector (PCD) CT. Methods: A phantom containing two custom-made vessels (25% and 50% stenoses) underwent PCD CT acquisitions without and with simulated cardiac motion. A retrospective analysis was performed of 33 patients (7 female, 26 male; mean age, 71.3±9.0 years; 64 coronary artery stenoses) who underwent coronary CTA by PCD CT followed by invasive coronary angiography (ICA). Scans were reconstructed at nine VMI levels (40-140 keV). Percentage diameter stenosis (PDS) was measured, and bias was determined from the ground-truth stenosis percentage in the phantom and ICA-derived quantitative coronary angiography measurements in patients. Extent of blooming artifacts was measured in the phantom and in calcified and mixed plaques in patients. Results: In the phantom, PDS decreased for the 25% stenosis from 59.9% (40 keV) to 13.4% (140 keV), and for the 50% stenosis from 81.6% (40 keV) to 42.3% (140 keV). PDS showed lowest bias for the 25% stenosis at 90 keV (bias, 1.4%), and for the 50% stenosis at 100 keV (bias, -0.4%). Blooming artifacts decreased for the 25% stenosis from 61.5% (40 keV) to 35.4% (140 keV), and for the 50% stenosis from 82.7% (40 keV) to 52.1% (140 keV). In patients, PDS for calcified plaque decreased from 70.8% (40 keV) to 57.3% (140 keV), for mixed plaque decreased from 69.8% (40 keV) to 56.3% (140 keV), and for non-calcified plaque was 46.6% at 40 keV and 54.6% at 140 keV. PDS showed lowest bias for calcified plaque at 100 keV (bias, 17.2%), for mixed plaque at 140 keV (bias, 5.0%), and for non-calcified plaque at 40 keV (bias, -0.5%). Blooming artifacts decreased for calcified plaque from 78.4% (40 keV) to 48.6% (140 keV), and for mixed plaque from 73.1% (40 keV) to 44.7% (140 keV). Conclusion: For calcified and mixed plaque, stenosis severity measurements and blooming artifacts decreased at increasing VMI. Clinical Impact: PCD CT with VMI reconstruction helps overcome current limitations in stenosis quantification on coronary CTA.

Abstract:Background and aims: The aim of this study was to investigate the imaging performance and quality differences of PCD-CT in standard resolution mode (SR) versus ultra-high resolution mode (UHR) in the lower extremity runoff of dose-matched CTAs in a human cadaveric model. Methods: Extracorporeal perfusion of the upper leg was established in one fresh-frozen human cadaver via inguinal and popliteal accesses using a peristaltic pump. Seven peripheral stents were deployed in the SFA. Photon-counting CTAs were performed under contrast perfusion in SR and UHR mode with dose-equivalent 120kVp acquisition protocols (low-/ medium-/ high-dose: CTDIVol=3, 5, 10 mGy) and reconstructed with four vascular convolution kernels. Lumen visibility and contrast-to-noise ratio were compared using analyses of variance. Subjective image quality was assessed using a pairwise, forced-choice comparison software. Results: Lumen visibility was equal for SR and UHR at the used dose levels. CNR increase by UHR was significant for (ultra-)sharp convolution kernels BV60 (3 mGy; UHR vs. SR, 19.9 ± 1.9 vs. 15.7 ± 1.6, p < 0.046) and BV76 (8.0 ± 0.6 vs. 5.4 ± 0.3, p < 0.001). The relative CNR increase was higher for low-dose than high-dose scans (BV76: 48% vs. 36% at high dose, p < 0.033). The CNR of the low-dose scan in UHR mode was comparable to the high-dose scan in SR mode when the ultra-sharp kernel was used (8.0 ± 0.6 vs. 9.1 ± 1.1, p > 0.760). Among UHR examinations, a significant increase in CNR could only be measured in BV76 (8.0 ± 0.6 (3 mGy) vs. 12.4 ± 0.9 (10 mGy), p < 0.001). Readers preferred subjective image quality of UHR for all kernels with BV76 being ranked highest. Conclusion: The CNR increase in UHR mode is highest when combining low radiation dose and ultra-sharp reconstructions. Meanwhile, the subjective image quality in UHR mode generally supersedes SR images, suggesting further dose reduction potential.

Abstract:BACKGROUND AND PURPOSE Neuroimaging of intracranial vessels with implanted stents (ICS) and flowdiverters (FD) is limited by artifacts. Photon-Counting-Detector-Computed Tomography (PCD-CT) is characterized by a higher resolution. The purpose of this study was to assess the image quality of ultra-high-resolution (UHR) PCD-CT-Angiography (PCD-CTA) and spectral reconstructions to define the best imaging parameters for the evaluation of vessel visibility in ICS and FD. MATERIALS AND METHODS Retrospective analysis of consecutive patients with implanted ICS or FD, who received a PCD-CTA between April 2023 and March 2024. Polyenergetic (PE), virtual monoenergetic imaging (VMI), pure lumen (PL) and iodine (I) reconstructions with different kiloelectron volt (keV) levels (keV 40, 60 and 80) and reconstruction kernels (Body vascular kernel (Bv) 48, Bv56, Bv64, Bv72, Bv76) were acquired to evaluate image quality and assessed by 2 independent radiologists using a 5-point Likert scale and regions of interest (ROI). The different kernels, keV and the optimized spectral reconstructions were compared in descriptive analysis. RESULTS In total, 12 patients with 9 FDs and 6 ICSs were analyzed. In terms of quantitative image quality, sharper kernels as Bv64 and Bv72 yielded increased image noise, and decreased signal to noise (SNR) and contrast to noise ratio (CNR) compared to the smoothest kernel Bv48, (p<0.01). Among the different keV levels and kernels, readers selected the 40 keV level (p<0.01) and sharper kernels (in the majority of cases Bv72) as the best to visualize the in-stent vessel lumen. Assessing the different spectral reconstructions virtual monoenergetic and iodine reconstructions proved to be best to evaluate in-stent vessel lumen (p<0.01). CONCLUSIONS Our preliminary study suggests that PCD-CTA and spectral reconstructions with sharper reconstruction kernels and a low keV level of 40 seem to be beneficial to achieve optimal image quality for the evaluation of ICS and FD. Low-dose Ultra-high-resolution Photon-Counting Detector CT for Visceral Artery CT Angiography: A Preliminary Study

Abstract:Rationale and objectives: To validate the image quality of low-dose ultra-high-resolution (UHR) scanning mode of photon-counting detector CT (PCD-CT) for visceral artery computed tomography angiography (CTA). Material and methods: We prospectively enrolled 57 patients each in the full dose (FD) and low-dose (LD) protocols, respectively, to undergo abdominal CT scans using the UHR mode on a PCD-CT system (NAEOTOM Alpha), between April 2023 and September 2023. Both the FD data and LD data were then reconstructed into two series of images: (a) 0.2 mm slice thickness, reconstruction kernel Bv48, quantum iterative reconstruction (QIR) 4; (b)1 mm slice thickness, Bv40, QIR 3. The signal-to-noise ratios (SNRs) and contrast-to-noise ratios (CNRs) of seven arteries were objectively measured. The image noise, vessel sharpness, overall quality, and visibility of nine arteries were subjectively assessed by three radiologists. Results: The SNRs and CNRs of 0.2 mm reconstruction set was inferior to that of 1 mm reconstruction set (p < 0.001 for all the arteries and noise), however, the image quality of 0.2 mm reconstruction set was higher than that of 1 mm reconstruction set in qualitative evaluation especially for tiny arteries in Volume-rendered (VR) image (p < 0.001). The SNRs and CNRs were not significantly higher for FD group than LD group on the same slice thickness except for SNRs of common hepatic artery, splenic artery and bilateral renal arteries in 0.2 mm reconstruction set. In the comparison on image quality between normal weight and overweight patients within the same reconstruction set, the results showed that low-dose scan did not significantly impact the image quality in overweight patients. The ratings of visibility of nine visceral arteries were not significantly different among FD and LD at the same thickness reconstruction set except for superior mesenteric artery (p = 0.002 and 0.007 for 0.2 mm and 1 mm reconstruction set in axial image; p = 0.002 and 0.007 for 0.2 mm and 1 mm reconstruction set in coronal image, respectively) and left gastric artery (p = 0.002 and p < 0.001 for 0.2 mm and 1 mm reconstruction set in VR image, respectively). Conclusion: The low-dose UHR scanning mode of PCD-CT has proven to be adequate for the clinical evaluation of visceral arteries. Utilizing a reconstruction with a slice thickness of 0.2 mm could enhance arterial depiction, particularly for small vessels.

Abstract:Journal of Endovascular Therapy, Ahead of Print. Purpose:The aim of this study was to evaluate the radiation dose, image quality, and the potential of virtual monoenergetic imaging (VMI) reconstructions of high-pitch computed tomography angiography (CTA) of the thoracoabdominal aorta on a dual-source photon-counting detector–CT (PCD-CT) in comparison with an energy-integrating detector–CT (EID-CT), with a special focus on low-contrast attenuation.Methods:Consecutive patients being referred for an electrocardiogram (ECG)-gated, high-pitch CTA of the thoracoabdominal aorta prior to transcatheter aortic valve replacement (TAVR), and examined on the PCD-CT, were included in this prospective single-center study. For comparison, a retrospective patient group with ECG-gated, high-pitch CTA examinations of the thoracoabdominal aorta on EID-CT with a comparable scan protocol was matched for gender, body mass index, height, and age. Virtual monoenergetic imaging reconstructions from 40 to 120 keV were performed. Enhancement and noise were measured in 7 vascular segments and the surrounding air as mean and standard deviation of CT values. The radiation dose was noted and signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated. Finally, a subgroup analysis was performed, comparing VMI reconstructions from 40 keV to 70 keV in patients with at least a 50% decrease in contrast attenuation between the ascending aorta and femoral arteries.Results:Fifty patients (mean age 77.0±14.5 years; 31 women) were included. The radiation dose was significantly lower on the PCD-CT (4.2±1.4 vs. 7.2±2.2 mGy; p<0.001). With increasing keV, vascular noise, SNR, and CNR decreased. Intravascular attenuation was significantly higher on VMI at levels from 40 to 65, compared with levels of 120 keV (p<0.01 and p<0.005, respectively). On the PCD-CT, SNR was significantly higher in keV levels 40 and 70 (all p<0.001), and CNR was higher at keV levels 40 and 45 (each p<0.001), compared with scans on the EID-CT. At VMI ≤60 keV, image noise was also significantly higher than that in the control group. The subgroup analysis showed a drastically improved diagnostic performance of the low-keV images in patients with low-contrast attenuation.Conclusion:The ECG-gated CTA of the thoracoabdominal aorta in high-pitch mode on PCD-CT have significantly lower radiation dose and higher objective image quality than EID-CT. In addition, low-keV VMI can salvage suboptimal contrast studies, further reducing radiation dose by eliminating the need for repeat scans.Clinical ImpactECG-gated CT-angiographies of the thoracoabdominal aorta can be acquired with a lower radtiation dose and a better image quality by using a dual-source photon-countinge detector CT. Furthermore, the inherent spectral data offers the possiblity to improve undiagnostic images and thus saves the patient from further radiation and contrast application.

Abstract:We compared image quality of head and neck CT angiography (CTA) obtained with a photon-counting detector CT (PCD-CT), including virtual monoenergetic images and polyenergetic reconstructions, and conventional energy-integrating detectors CT (EID-CT) in three patients. PCD-CT monoenergetic reconstructions at 70 keV and lower provided excellent image quality, with improved signal-to-noise and contrast-to-noise compared to EID-CT and PCD-CT polyenergetic reconstructions. PCD-CT may enable radiation dose and iodinated contrast dose reduction for cerebrovascular imaging.

Abstract:Objective To assess the viability of using ultra-low radiation and contrast medium (CM) dosage in aortic computed tomography angiography (CTA) through the application of low tube voltage (60 kVp) and a novel deep learning image reconstruction algorithm (ClearInfinity, DLIR-CI). Methods Iodine attenuation curves obtained from a phantom study informed the administration of CM protocols. Non-obese participants undergoing aortic CTA were prospectively allocated into two groups and then obtained three reconstruction groups. The conventional group (100 kVp-CV group) underwent imaging at 100 kVp and received 210 mg iodine/kg in combination with a hybrid iterative reconstruction algorithm (ClearView, HIR-CV). The experimental group was imaged at 60 kVp with 105 mg iodine/kg, while images were reconstructed with HIR-CV (60 kVp-CV group) and with DLIR-CI (60 kVp-CI group). Student's t-test was used to compare differences in CM protocol and radiation dose. One-way ANOVA compared CT attenuation, image noise, SNR, and CNR among the three reconstruction groups, while the Kruskal–Wallis H test assessed subjective image quality scores. Post hoc analysis was performed with Bonferroni correction for multiple comparisons, and consistency analysis conducted in subjective image quality assessment was measured using Cohen's kappa. Results The radiation dose (1.12 ± 0.23 mSv vs. 2.03 ± 0.82 mSv) and CM dosage (19.04 ± 3.03 mL vs. 38.11 ± 6.47 mL) provided the reduction of 45% and 50% in the experimental group compared to the conventional group. The CT attenuation, SNR, and CNR of 60 kVp-CI were superior to or equal to those of 100 kVp-CV. Compared to the 60 kVp-CV group, images in 60 kVp-CI showed higher SNR and CNR (all P < 0.001). There was no difference between the 60 kVp-CI and 100 kVp-CV group in terms of the subjective image quality of the aorta in various locations (all P > 0.05), with 60 kVp-CI images were deemed diagnostically sufficient across all vascular segments. Conclusion For non-obese patients, the combined use of 60 kVp and DLIR-CI algorithm can be preserving image quality while enabling radiation dose and contrast medium savings for aortic CTA compared to 100 kVp using HIR-CV.

Abstract:Background Photon‐counting CT (PCCT) is the next‐generation CT scanner that enables improved spatial resolution and spectral imaging. For full spectral processing, higher tube voltages compared to conventional CT are necessary to achieve the required spectral separation. This generated interest in the potential influence of thin slice high tube voltage PCCT on overall image quality and consequently on radiation dose. Purpose This study first evaluated tube voltages and radiation doses applied in patients who underwent coronary CT angiography with PCCT and energy‐integrating detector CT (EID‐CT). Next, image quality of PCCT and EID‐CT was objectively evaluated in a phantom study simulating different patient sizes at these tube voltages and radiation doses. Methods We conducted a retrospective analysis of clinical doses of patients scanned on a conventional and PCCT system. Average patient water equivalent diameters for different tube voltages were extracted from the dose reports for both EID‐CT and PCCT. A conical phantom made of polyethylene with multiple diameters (26/31/36 cm) representing different patient sizes and containing an iodine insert was scanned with a EID‐CT scanner using tube voltages and phantom diameters that match the patient scans and characteristics. Next, phantom scans were made with PCCT at a fixed tube voltage of 120 kV and with CTDIVOL values and phantom diameters identical to the EID‐CT scans. Clinical image reconstructions at 0.6 mm slice thickness for conventional CT were compared to PCCT images with 0.4 mm slice thickness. Image quality was quantified using the detectability index (d′), which estimated the visibility of a 3 mm diameter contrast‐enhanced coronary artery by considering noise, contrast, resolution, and human visual perception. Alongside d′, noise, contrast and resolution were also individually assessed. In addition, the influence of various kernels (Bv40/Bv44/Bv48/Bv56), quantum iterative reconstruction strengths (QIR, 3/4) and monoenergetic levels (40/45/50/55 keV) for PCCT on d′ was investigated. Results In this study, 143 patients were included: 47 were scanned on PCCT (120 kV) and the remaining on EID‐CT (74 small‐sized at 70 kV, 18 medium‐sized at 80 kV and four large‐sized at 90 kV). EID‐CT showed 7%–17% higher d′ than PCCT with Bv40 kernel and strength four for small/medium patients. Lower monoenergetic images (40 keV) helped mitigate the difference to 1%–6%. For large patients, PCCT's detectability was up to 31% higher than EID‐CT. PCCT has thinner slices but similar noise levels for similar reconstruction parameters. The noise increased with lower keV levels in PCCT (≈30% increase), but higher QIR strengths reduced noise. PCCT's iodine contrast was stable across patient sizes, while EID‐CT had 33% less contrast in large patients than in small‐sized patients. Conclusion At 120 kV, thin slice PCCT enables CCTA in phantom scans representing large patients without raising radiation dose or affecting vessel detectability. However, higher doses are needed for small and medium‐sized patients to obtain a similar image quality as in EID‐CT. The alternative of using lower mono‐energetic levels requires further evaluation in clinical practice.

Abstract:PURPOSE: To investigate the image quality of lower extremity computed tomography angiography (LE-CTA) using a reconstruction algorithm for monoenergetic images (MEIs) to evaluate peripheral arterial disease (PAD) at different kiloelectron volt (keV) levels.METHODS: A total of 146 consecutive patients who underwent LE-CTA on a dual-energy scanner to obtain MEIs at 40, 50, 60, 70, and 80 keV were included. The overall image quality, segmental image quality of the arteries and PAD segments, venous contamination, and metal artifacts from prostheses, which may compromise quality, were analyzed.RESULTS: The mean overall image quality of each MEI was 2.9 ± 0.7, 3.6 ± 0.6, 3.9 ± 0.3, 4.0 ± 0.2, and 4.0 ± 0.2 from 40 to 80 keV, respectively. The segmental image quality gradually increased from 40 to 70-80 keV until reaching its highest value. Among 295 PAD segments in 68 patients, 40 (13.6%) were scored at 1-2 at 40 keV and 13 (4.4%) were scored at 2 at 50 keV, indicating unsatisfactory image quality due to the indistinguishability between high-contrast areas and arterial calcifications. The segments exhibiting metal artifacts and venous contamination were reduced at 70-80 keV (2.6 ± 1.2, 2.7 ± 0.5) compared with at 40 keV (2.4 ± 1.1, 2.5 ± 0.7).CONCLUSION: The LE-CTA method using a reconstruction algorithm for MEIs at 70-80 keV can enhance the image quality for PAD evaluation and improve mitigate venous contamination and metal artifacts.

Abstract:Abstract Objectives Photon-counting detector CT (PCD-CT) promises a leap in spatial resolution due to smaller detector pixel sizes than implemented in energy-integrating detector CTs (EID-CT). Our objective was to compare the visualization of smallest bone details between PCD-CT and EID-CT using a mouse as a specimen. Materials and methods Two euthanized mice were scanned at a 20-slice EID-CT and a dual-source PCD-CT in single-pixel mode at various CTDI Vol values. Image noise and signal-to-noise ratio (SNR) were evaluated using repeated ROI measurements. Edge sharpness of bones was compared by the maximal slope within CT value plots along sampling lines intersecting predefined bones of the spine. Two readers evaluated bone detail visualization at four regions of the spine on a three-point Likert scale at various CTDI Vol ’s. Two radiologists selected the series with better detail visualization among each of 20 SNR-matched pairs of EID-CT and PCD-CT series. Results In CTDI Vol -matched scans, PCD-CT series showed significantly lower image noise (Noise CTDI = 5 mGy : 16.27 ± 1.39 vs. 23.46 ± 0.96 HU, p < 0.01), higher SNR (SNR CTDI = 5 mGy : 20.57 ± 1.89 vs. 14.00 ± 0.66, p < 0.01), and higher edge sharpness (Edge Slope lumbar spine : 981 ± 160 vs. 608 ± 146 HU/mm, p < 0.01) than EID-CT series. Two radiologists considered the delineation of bone details as feasible at consistently lower CTDI Vol values at PCD-CT than at EID-CT. In comparison of SNR-matched reconstructions, PCD-CT series were still considered superior in almost all cases. Conclusions In this head-to-head comparison, PCD-CT showed superior objective and subjective image quality characteristics over EID-CT for the delineation of tiniest bone details. Even in SNR-matched pairs (acquired at different CTDI Vol ’s), PCD-CT was strongly preferred by radiologists. Key Points • In dose-matched scans, photon-counting detector CT series showed significantly less image noise, higher signal-to-noise ratio, and higher edge sharpness than energy-integrating detector CT series . • Human observers considered the delineation of tiny bone details as feasible at much lower dose levels in photon-counting detector CT than in energy-integrating detector CT . • In direct comparison of series matched for signal-to-noise ratio, photon-counting detector CT series were considered superior in almost all cases .

Iterative reconstruction vs deep learning image reconstruction: comparison of image quality and diagnostic accuracy of arterial stenosis in low-dose lower extremity CT angiography

Abstract:Objective: To compare image quality and diagnostic accuracy of arterial stenosis in low-dose lower-extremity CT angiography (CTA) between adaptive statistical iterative reconstruction-V (ASIR-V) and deep learning image reconstruction (DLIR) algorithms. Methods: 46 patients undergoing low-dose lower-extremity CTA were enrolled. Images were reconstructed using ASIR-V (blending factor of 50% (AV-50) and 100% (AV-100)) and DLIR (medium (DL-M), and high (DL-H)). CT values and standard deviation of the aorta, psoas, popliteal artery, popliteal and ankle muscles were measured. The edge-rise distance and edge-rise slope were calculated. The degrees of granularity and edge blurring were assessed using a 5-point scale. The stenosis degrees were measured on the four reconstructions, and their mean square errors against that of digital subtraction angiography were calculated and compared. Results: For both ASIR-V and DLIR, higher reconstruction intensity generated lower noise and higher signal-to-noise ratio and contrast-to-noise ratio values. The standard deviation values in AV-100 images were significantly lower than other reconstructions. The two DLIR image groups had higher edge-rise slope and lower edge-rise distance (DL-M:1.79 ± 0.37 mm and DL-H:1.82 ± 0.38 mm vs AV-50:1.96 ± 0.39 mm and AV-100:2.01 ± 0.36 mm, p = 0.014) than ASIR-V images. The overall image quality of DLIR was rated higher than ASIR-V (DL-M:0.83 ± 0.61, DL-H:0.41 ± 0.62, AV-50:1.85 ± 0.60 and AV-100:2.37 ± 0.77, p < 0.001), with DL-H having the highest overall image quality score. For stenosis measurement, DL-H had the lowest mean-square-errors compared to digital subtraction angiography among all reconstruction groups. Conclusion: DLIR images had higher image quality ratings with lower image noise and sharper vessel walls in low-dose lower-extremity CTA, and DL-H provides the best overall image quality and highest accuracy in diagnosing artery stenoses. Advances in knowledge: DLIR provides high-quality images with sharper edges compared to ASIR-V during low-dose CTA of lower extremity arteries, and DLIR (high) provides the best overall image quality and highest accuracy in diagnosing artery stenoses among all reconstruction algorithms (ASIR-V and DLIR). ASIR-V with blending factor of 100% has the strongest noise reduction ability among all reconstruction algorithms (ASIR-V and DLIR); however, it generates the most blurred images.
Reduced-Dose Low-Voltage Chest CT Angiography with Sinogram-affirmed Iterative Reconstruction versus Standard-Dose Filtered Back Projection

Abstract:PURPOSE: To evaluate image quality of low-voltage chest computed tomographic (CT) angiography with raw data-based iterative reconstruction (sonogram-affirmed iterative reconstruction) in comparison with image quality of standard-dose standard-voltage filtered back projection (FBP) CT.MATERIALS AND METHODS: This prospective study was approved by the institutional review board, and the informed consent requirement was waived. Eighty consecutive patients who were referred for follow-up chest CT angiography underwent reduced-dose CT (hereafter, T2 examination) under technical conditions similar to those of the initial examination (hereafter, T1 examination), except the voltage selection was reduced by 20 kV with adaptation of the tube current to ensure a 50% reduction in CT dose index, and regular FBP was replaced by iterative reconstruction with sonogram-affirmed iterative reconstruction. The two techniques were compared by using paired tests (Student t test, Wilcoxon test, or McNemar test, according to the nature of variables).RESULTS: When compared with standard-dose T1 studies, reduced-dose T2 images showed: (a) significantly less objective noise at the level of the trachea on mediastinal and lung parenchymal images (P < .001) and no significant difference in objective noise at the level of the aorta on mediastinal images (P = .507); (b) significantly higher signal-to-noise and contrast-to-noise (P < .001) ratios; (c) similar visual perception of noise on mediastinal (P = .132) and lung (P = .366) images, mainly rated as moderate; and (d) similar overall subjective image quality (P = .405).CONCLUSION: Raw data-based iterative reconstruction yielded equivalent subjective and improved objective image quality of low-voltage half-dose CT angiograms compared with standard-dose FBP CT images for an average dose-length product of less than 80 mGy · cm in this population.SUPPLEMENTAL MATERIAL: http://radiology.rsna.org/lookup/suppl/doi:10.1148/radiol.12120414/-/DC1.
