Abstract:OBJECTIVE:To evaluate the improvement of iterative reconstruction in image space (IRIS) technique in computed tomographic (CT) coronary stent imaging with sharp kernel, and to make a trade-off analysis. MATERIALS AND METHODS:Fifty-six patients with 105 stents were examined by 128-slice dual-source CT coronary angiography (CTCA). Images were reconstructed using standard filtered back projection (FBP) and IRIS with both medium kernel and sharp kernel applied. Image noise and the stent diameter were investigated. Image noise was measured both in background vessel and in-stent lumen as objective image evaluation. Image noise score and stent score were performed as subjective image evaluation. RESULTS:The CTCA images reconstructed with IRIS were associated with significant noise reduction compared to that of CTCA images reconstructed using FBP technique in both of background vessel and in-stent lumen (the background noise decreased by approximately 25.4% ± 8.2% in medium kernel (P <C; 0.0001) and 30.3% ± 3.4% in sharp kernel (P <C; 0.0001). The in-stent lumen noise decreased by approximately 14.2% ± 19.2% in medium kernel (P <C; 0.0001) and 27.0% ± 17.8% in sharp kernel (P <C; 0.0001)). Subjective image assessment showed that the noise of the images reconstructed with IRIS decreased compared to that with FBP. Moreover, the images with sharp kernel showed better visualization of the stent struts and in-stent lumen than that with medium kernel. CONCLUSIONS:Iterative reconstruction in image space reconstruction can effectively reduce the image noise and improve image quality. The sharp kernel images constructed with iterative reconstruction are considered the optimal images to observe coronary stents in this study.

Abstract:Purpose: The study aimed to assess the feasibility and image quality of dual-source photon-counting detector computed tomography (PCD-CT) in evaluating small-sized coronary artery stents with respect to different acquisition modes in a phantom model. Methods: Utilizing a phantom setup mimicking the average patient's water-equivalent diameter, we examined six distinct coronary stents inflated in a silicon tube, with stent sizes ranging from 2.0 to 3.5 mm, applying four different CT acquisition modes of a dual-source PCD-CT scanner: "high-pitch," "sequential," "spiral" (each with collimation of 144 × 0.4 mm and full spectral information), and "ultra-high-resolution (UHR)" (collimation of 120 × 0.2 mm and no spectral information). Image quality and diagnostic confidence were assessed using subjective measures, including a 4-point visual grading scale (4 = excellent; 1 = non-diagnostic) utilized by two independent radiologists, and objective measures, including the full width at half maximum (FWHM). Results: A total of 24 scans were acquired, and all were included in the analysis. Among all CT acquisition modes, the highest image quality was obtained for the UHR mode [median score: 4 (interquartile range (IQR): 3.67-4.00)] (P = 0.0015, with 37.5% rated as "excellent"), followed by the sequential mode [median score: 3.5 (IQR: 2.84-4.00)], P = 0.0326 and the spiral mode [median score: 3.0 (IQR: 2.53-3.47), P > 0.05]. The lowest image quality was observed for the high-pitch mode [median score: 2 (IQR: 1- 3), P = 0.028]. Similarly, diagnostic confidence for evaluating stent patency was highest for UHR and lowest for high-pitch (P < 0.001, respectively). Measurement of stent dimensions was accurate for all acquisition modes, with the UHR mode showing highest robustness (FWHM for sequential: 0.926 ± 0.061 vs. high-pitch: 0.990 ± 0.083 vs. spiral: 0.962 ± 0.085 vs. UHR: 0.941 ± 0.036, P = non-significant, respectively). Conclusion: Assessing small-sized coronary stents using PCD-CT technology is feasible. The UHR mode offers superior image quality and diagnostic confidence, while all modes show consistent and accurate measurements. Clinical significance: These findings highlight the potential of PCD-CT technology, particularly the UHR mode, to enhance non-invasive coronary stent evaluation. Confirmatory research is necessary to influence the guidelines, which recommend cardiac CT only for stents of 3 mm or larger.

Abstract:Purpose To assess the impact of different quantum iterative reconstruction (QIR) levels on objective and subjective image quality of ultra-high resolution (UHR) coronary CT angiography (CCTA) images and to determine the effect of strength levels on stenosis quantification using photon-counting detector (PCD)-CT. Method A dynamic vessel phantom containing two calcified lesions (25 % and 50 % stenosis) was scanned at heart rates of 60, 80 and 100 beats per minute with a PCD-CT system. In vivo CCTA examinations were performed in 102 patients. All scans were acquired in UHR mode (slice thickness0.2 mm) and reconstructed with four different QIR levels (1–4) using a sharp vascular kernel (Bv64). Image noise, signal-to-noise ratio (SNR), sharpness, and percent diameter stenosis (PDS) were quantified in the phantom, while noise, SNR, contrast-to-noise ratio (CNR), sharpness, and subjective quality metrics (noise, sharpness, overall image quality) were assessed in patient scans. Results Increasing QIR levels resulted in significantly lower objective image noise (in vitro and in vivo: both p < 0.001), higher SNR (both p < 0.001) and CNR (both p 0.008). Subjective noise of in vivo images significantly decreased with increasing QIR levels, resulting in significantly higher image quality scores at increasing QIR levels (all pairwise p < 0.001). Qualitative sharpness, on the other hand, did not differ across different levels of QIR (p = 0.15). Conclusions The QIR algorithm may enhance the image quality of CCTA datasets without compromising image sharpness or accurate stenosis measurements, with the most prominent benefits at the highest strength level.

Abstract:Background: Calcium blooming causes stenosis overestimation on coronary CTA. Objective: To evaluate the impact of virtual monoenergetic imaging (VMI) reconstruction level on coronary artery stenosis quantification using a photon-counting detector (PCD) CT. Methods: A phantom containing two custom-made vessels (25% and 50% stenoses) underwent PCD CT acquisitions without and with simulated cardiac motion. A retrospective analysis was performed of 33 patients (7 female, 26 male; mean age, 71.3±9.0 years; 64 coronary artery stenoses) who underwent coronary CTA by PCD CT followed by invasive coronary angiography (ICA). Scans were reconstructed at nine VMI levels (40-140 keV). Percentage diameter stenosis (PDS) was measured, and bias was determined from the ground-truth stenosis percentage in the phantom and ICA-derived quantitative coronary angiography measurements in patients. Extent of blooming artifacts was measured in the phantom and in calcified and mixed plaques in patients. Results: In the phantom, PDS decreased for the 25% stenosis from 59.9% (40 keV) to 13.4% (140 keV), and for the 50% stenosis from 81.6% (40 keV) to 42.3% (140 keV). PDS showed lowest bias for the 25% stenosis at 90 keV (bias, 1.4%), and for the 50% stenosis at 100 keV (bias, -0.4%). Blooming artifacts decreased for the 25% stenosis from 61.5% (40 keV) to 35.4% (140 keV), and for the 50% stenosis from 82.7% (40 keV) to 52.1% (140 keV). In patients, PDS for calcified plaque decreased from 70.8% (40 keV) to 57.3% (140 keV), for mixed plaque decreased from 69.8% (40 keV) to 56.3% (140 keV), and for non-calcified plaque was 46.6% at 40 keV and 54.6% at 140 keV. PDS showed lowest bias for calcified plaque at 100 keV (bias, 17.2%), for mixed plaque at 140 keV (bias, 5.0%), and for non-calcified plaque at 40 keV (bias, -0.5%). Blooming artifacts decreased for calcified plaque from 78.4% (40 keV) to 48.6% (140 keV), and for mixed plaque from 73.1% (40 keV) to 44.7% (140 keV). Conclusion: For calcified and mixed plaque, stenosis severity measurements and blooming artifacts decreased at increasing VMI. Clinical Impact: PCD CT with VMI reconstruction helps overcome current limitations in stenosis quantification on coronary CTA.

Abstract:While drastic improvements in CT technology have occurred in the past 25 years, spatial resolution is one area where progress has been limited until recently. New photon counting CT systems, are capable of much better spatial resolution than their (energy integrating) predecessors. These improvements have the potential to improve the evaluation obstructive coronary artery disease by enabling more accurate delineation between calcified plaque and coronary vessel lumen. A new set of vessel phantoms has been designed and manufactured for quantifying this improvement. Comparisons are made between an existing clinical CT system, a prototype photon counting system, with images from a micro CT system being used as the gold standard. Scans were made of the same objects on all three systems. The resulting images were registered and the luminal cross section areas were compared. Luminal cross-sections near calcified plaques were reduced due to blooming, but this effect was much less pronounced in images from the prototype photon counting system as compared to the images from the clinical CT system.

Abstract:BACKGROUND AND PURPOSE Neuroimaging of intracranial vessels with implanted stents (ICS) and flowdiverters (FD) is limited by artifacts. Photon-Counting-Detector-Computed Tomography (PCD-CT) is characterized by a higher resolution. The purpose of this study was to assess the image quality of ultra-high-resolution (UHR) PCD-CT-Angiography (PCD-CTA) and spectral reconstructions to define the best imaging parameters for the evaluation of vessel visibility in ICS and FD. MATERIALS AND METHODS Retrospective analysis of consecutive patients with implanted ICS or FD, who received a PCD-CTA between April 2023 and March 2024. Polyenergetic (PE), virtual monoenergetic imaging (VMI), pure lumen (PL) and iodine (I) reconstructions with different kiloelectron volt (keV) levels (keV 40, 60 and 80) and reconstruction kernels (Body vascular kernel (Bv) 48, Bv56, Bv64, Bv72, Bv76) were acquired to evaluate image quality and assessed by 2 independent radiologists using a 5-point Likert scale and regions of interest (ROI). The different kernels, keV and the optimized spectral reconstructions were compared in descriptive analysis. RESULTS In total, 12 patients with 9 FDs and 6 ICSs were analyzed. In terms of quantitative image quality, sharper kernels as Bv64 and Bv72 yielded increased image noise, and decreased signal to noise (SNR) and contrast to noise ratio (CNR) compared to the smoothest kernel Bv48, (p<0.01). Among the different keV levels and kernels, readers selected the 40 keV level (p<0.01) and sharper kernels (in the majority of cases Bv72) as the best to visualize the in-stent vessel lumen. Assessing the different spectral reconstructions virtual monoenergetic and iodine reconstructions proved to be best to evaluate in-stent vessel lumen (p<0.01). CONCLUSIONS Our preliminary study suggests that PCD-CTA and spectral reconstructions with sharper reconstruction kernels and a low keV level of 40 seem to be beneficial to achieve optimal image quality for the evaluation of ICS and FD. Iodine and virtual monoenergetic reconstructions were superior to pure lumen and polyenergetic reconstructions to evaluate in-stent vessel lumen

Abstract:Background: Stent implantation is the predominant therapy for non-surgical myocardial revascularization in patients with coronary artery disease. However, despite substantial advances in multidetector computed tomography (MDCT) coronary imaging, a reliable detection of coronary in-stent restenosis is currently not possible. Purpose: To examine the ability of 64-detector-row CT to detect and to grade in-stent stenosis in coronary stents using a newly developed ex-vivo vessel phantom with a realistic CT density pattern, artificial stenosis, and a thorax phantom. Material and methods: Four different stents (Liberté and Lunar ROX, Boston Scientific; Driver, Medtronic; Multi-Link Vision, Guidant) were examined. The stents were placed on a polymer tube with a diameter of 2.5, 3.0, 3.5, or 4.0 mm. Different degrees of stenosis (0%, 30%, 50%, 70-80%) were created inside the tube. For quantitative analysis, attenuation values were measured in the non-stenotic vessel outside the stent, in the non-stenotic vessel inside the stent, and in the stenotic area inside the stent. The grade of stenosis was visually assessed by two observers. Results: All stents led to artificial reduction of attenuation, the least degree of which was found in the Liberté stent (11.3+/-10.2 HU) and the Multi-Link Vision stent (17.6+/-17.9 HU; P = 0.25). Overall, the non-stenotic vessel was correctly diagnosed in 55.5%, the low-grade stenosis in 58.3%, the intermediate stenosis in 63.8%, and the high-grade stenosis in 80.5%. In the 3.0-, 3.5-, and 4.0-mm vessels, in none of the cases was a non-stenotic or low-grade stenotic vessel misdiagnosed as intermediate or high-grade stenosis. The average deviation from the real grade of stenosis was 0.40 for the Liberté stent, 0.46 for the Lunar ROX stent, 0.45 for the Driver stent, and 0.58 for the Multi-Link Vision stent. Conclusion: Our ex-vivo data show that non-stenotic stents and low-grade in-stent stenosis can be reliably differentiated from intermediate and high-grade in-stent stenosis in vessels with a diameter of 3 to 4 mm. With regard to artifacts and the grading of stenoses, the Liberté stent was best suited for CT coronary angiography.

Abstract:Photon Counting CT allows the acquisition of high-resolution data sets with 0.2 mm reconstructed slice thickness. It is unknown whether the detection of coronary artery stenoses, of coronary plaque or of vulnerable plaque characteristics is improved in data sets that employ maximum resolution. We therefore compared the identification of above characteristics in data sets reconstructed with 0.2 mm and 0.4 mm slice thickness In 30 patients with suspected coronary artery disease, sinus rhythm, a heart rate ≤65 beats/min, and body weight ≤ 100 kg, high-resolution data sets were acquired by photon counting CT (60 ml contrast i.v. at 6 ml/s) after s.c. application of nitrates in inspiratory breathhold. Data were reconstructed with 0.2 mm slice thickness at 0.1 mm increment and 0.4 mm slice thickness at 0.2 mm increment. Next to quantitative analysis of image quality parameters, all data sets were evaluated by two independent expert reviewers concerning subjective image quality as well as the ability to determine the presence of coronary artery stenoses, calcified and non- calcified plaque, as well as parameters of plaque vulnerability (positive remodeling, necrotic core, density < 30 HU and napkin-ring sign) using a 5-point Likert scale (-2 to +2) and a dedicated 11-segment coronary model. Of the 30 patients, 47% were male (mean age 58±12 years, mean heart rate 53±6 bpm, mean body weight 80±15 kg). The median dose length product was 508 mGy*cm. Mean image noise was 42±7 HU vs. 34±7 HU for 0.2 mm vs. 0.4 mm reconstructions (p < 0.001). The median image quality score was significantly higher for 0.2 mm reconstructions (3.8±0.8 vs 3.6±0.7). Of 335 coronary segments, none were classified as "unevaluable" while 87% vs. 76% were classified as "perfect" in 0.2 mm vs. 0.4 mm reconstructions (p < 0.001). Full confidence to rule-in or rule out significant coronary stenoses was achieved in 93% of segments in both reconstructions. The number of non-calcified plaques identified in 0.2 mm reconstructions was numerically higher than in 0.4 mm reconstructions (77 vs. 71, p = n.s.). On the Likert scale, the confidence to identify parameters of plaque vulnerability was significantly higher for 0.2 mm reconstructions (1.93 vs. 1.85, p < 0.001). In a selected patient group, photon counting CT coronary angiography demonstrated excellent image quality and an extremely low rate of unevaluable coronary segments. High-resolution reconstructions improve subjective image quality as was as the assessment of vulnerable plaque characteristics.

Abstract:Purpose We aimed to identify the optimal reconstruction settings based on qualitative and quantitative image quality parameters on standard and ultra-high resolution (UHR) images using photon-counting CT (PCCT). Method We analysed 45 patients, 29 with standard and 16 with UHR acquisition, applying both smoother and sharper kernel settings. Coronary CT angiography images were performed on a dual-source PCCT system using standard (0.4/0.6 mm slice thickness, Bv40/Bv44 kernels, QIR levels 0–4) or UHR acquisition (0.2/0.4 mm slice thickness, Bv44/Bv56 kernels, QIR levels 0–4). Qualitative image quality was assessed using a 4-point Likert scale. Image noise (SD), signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated in both the proximal and distal segments. Results On standard resolution, larger slice thickness resulted in an average increase of 12.5 % in CNR, whereas sharper kernel led to an average 8.7 % decrease in CNR. Highest CNR was measured on 0.6 mm, Bv40, QIR4 images and lowest on 0.4 mm, Bv44, QIR0 images: 25.8 ± 4.1vs.8.3 ± 1.6 (p < 0.001). On UHR images, highest CNR was observed on 0.4 mm, Bv40, QIR4 and lowest on 0.2 mm, Bv56 and QIR0 images: 21.5 ± 3.9vs.3.6 ± 0.8 (p < 0.001). Highest qualitative image quality was found on images with Bv44 kernel and QIR level 3/4 with both slice thicknesses on standard reconstruction. Additionally, Bv56 with QIR4 on 0.2 mm slice thickness images showed highest subjective image quality. Preserved distal vessel visualization was detected using QIR 2–4, Bv56 and 0.2 mm slice thickness. Conclusions Photon-counting CT demonstrated high qualitative and quantitative image quality for the assessment of coronaries and stents.

Abstract:To investigate the effect of heart rate and virtual monoenergetic image (VMI) on coronary stent imaging in dual-source photon-counting detector computed tomography (PCD-CT). A dynamic cardiac phantom was used to vary the heart rate at 50 beats per minute (bpm), 70 bpm, and 90 bpm. Five types of stents (4.0 mm, 3.5 mm, 3.0 mm, 2.75 mm, and 2.5 mm diameter) were scanned at three different locations and reconstructed VMI at 70 keV. In addition, 50% stenosis was assessed for 3.0 mm and 4.0 mm stents. To assess in-stent stenosis, 40 keV, 70 keV, and 100 keV images were compared. Measurable lumen and contrast to noise ratio (CNR) from lumen to stenosis were evaluated quantitatively. A-4-point scale was used for the qualitative image quality of in-stent stenosis. The measurable lumen had no significant differences among heart rates in patent stents (p = 0.55). In-stent stenosis, the residual lumen was significantly larger in 40 keV [27.5% (20.8-32.3%)] than in 70 keV [11.5% (10.0-23.0%), p < 0.05] and 100 keV [0% (0-5.2%), p < 0.05]. The CNR was higher in 40 keV [12.5 (7.5-18.2)] than in 70 keV [5.3 (2.9-7.7), p < 0.05] and 100 keV [1.3 (0.5-2.7), p < 0.05]. The image quality was better in 40 keV (3.4 ± 0.7) than in 70 keV [(2.6 ± 0.8), p < 0.05] and 100 keV [(1.3 ± 0.4), p < 0.05]. Dual-source PCD-CT maintains a measurable lumen even at high heart rates. Adjusting the VMI can be helpful in visualizing the in-stent stenosis.
Coronary Atherosclerotic Plaque Characterization with Photon-counting CT: a Simulation-based Feasibility Study

Abstract:Recent development of photon-counting CT (PCCT) brings great opportunities for plaque characterization with much-improved spatial resolution and spectral imaging capability. While existing coronary plaque PCCT imaging results are based on detectors made of CZT or CdTe materials, deep-silicon photon-counting detectors have unique performance characteristics and promise distinct imaging capabilities. In this work, we report a systematic simulation study of a deep-silicon PCCT scanner with a new clinically-relevant digital plaque phantom with realistic geometrical parameters and chemical compositions. This work investigates the effects of spatial resolution, noise, motion artifacts, radiation dose, and spectral characterization. Our simulation results suggest that the deep-silicon PCCT design provides adequate spatial resolution for visualizing a necrotic core and quantitation of key plaque features. Advanced denoising techniques and aggressive bowtie filter designs can keep image noise to acceptable levels at this resolution while keeping radiation dose comparable to that of a conventional CT scan. The ultrahigh resolution of PCCT also means an elevated sensitivity to motion artifacts. It is found that a tolerance of less than 0.4 mm residual movement range requires the application of accurate motion correction methods for best plaque imaging quality with PCCT.

Ultra-high Versus Standard Resolution Photon-Counting Detector CT Angiography for Imaging of Femoral Stents in a Cadaveric Perfusion Model

Abstract:Background and aims: The aim of this study was to investigate the imaging performance and quality differences of PCD-CT in standard resolution mode (SR) versus ultra-high resolution mode (UHR) in the lower extremity runoff of dose-matched CTAs in a human cadaveric model. Methods: Extracorporeal perfusion of the upper leg was established in one fresh-frozen human cadaver via inguinal and popliteal accesses using a peristaltic pump. Seven peripheral stents were deployed in the SFA. Photon-counting CTAs were performed under contrast perfusion in SR and UHR mode with dose-equivalent 120kVp acquisition protocols (low-/ medium-/ high-dose: CTDIVol=3, 5, 10 mGy) and reconstructed with four vascular convolution kernels. Lumen visibility and contrast-to-noise ratio were compared using analyses of variance. Subjective image quality was assessed using a pairwise, forced-choice comparison software. Results: Lumen visibility was equal for SR and UHR at the used dose levels. CNR increase by UHR was significant for (ultra-)sharp convolution kernels BV60 (3 mGy; UHR vs. SR, 19.9 ± 1.9 vs. 15.7 ± 1.6, p < 0.046) and BV76 (8.0 ± 0.6 vs. 5.4 ± 0.3, p < 0.001). The relative CNR increase was higher for low-dose than high-dose scans (BV76: 48% vs. 36% at high dose, p < 0.033). The CNR of the low-dose scan in UHR mode was comparable to the high-dose scan in SR mode when the ultra-sharp kernel was used (8.0 ± 0.6 vs. 9.1 ± 1.1, p > 0.760). Among UHR examinations, a significant increase in CNR could only be measured in BV76 (8.0 ± 0.6 (3 mGy) vs. 12.4 ± 0.9 (10 mGy), p < 0.001). Readers preferred subjective image quality of UHR for all kernels with BV76 being ranked highest. Conclusion: The CNR increase in UHR mode is highest when combining low radiation dose and ultra-sharp reconstructions. Meanwhile, the subjective image quality in UHR mode generally supersedes SR images, suggesting further dose reduction potential.

Abstract:Background Photon‐counting CT (PCCT) is the next‐generation CT scanner that enables improved spatial resolution and spectral imaging. For full spectral processing, higher tube voltages compared to conventional CT are necessary to achieve the required spectral separation. This generated interest in the potential influence of thin slice high tube voltage PCCT on overall image quality and consequently on radiation dose. Purpose This study first evaluated tube voltages and radiation doses applied in patients who underwent coronary CT angiography with PCCT and energy‐integrating detector CT (EID‐CT). Next, image quality of PCCT and EID‐CT was objectively evaluated in a phantom study simulating different patient sizes at these tube voltages and radiation doses. Methods We conducted a retrospective analysis of clinical doses of patients scanned on a conventional and PCCT system. Average patient water equivalent diameters for different tube voltages were extracted from the dose reports for both EID‐CT and PCCT. A conical phantom made of polyethylene with multiple diameters (26/31/36 cm) representing different patient sizes and containing an iodine insert was scanned with a EID‐CT scanner using tube voltages and phantom diameters that match the patient scans and characteristics. Next, phantom scans were made with PCCT at a fixed tube voltage of 120 kV and with CTDIVOL values and phantom diameters identical to the EID‐CT scans. Clinical image reconstructions at 0.6 mm slice thickness for conventional CT were compared to PCCT images with 0.4 mm slice thickness. Image quality was quantified using the detectability index (d′), which estimated the visibility of a 3 mm diameter contrast‐enhanced coronary artery by considering noise, contrast, resolution, and human visual perception. Alongside d′, noise, contrast and resolution were also individually assessed. In addition, the influence of various kernels (Bv40/Bv44/Bv48/Bv56), quantum iterative reconstruction strengths (QIR, 3/4) and monoenergetic levels (40/45/50/55 keV) for PCCT on d′ was investigated. Results In this study, 143 patients were included: 47 were scanned on PCCT (120 kV) and the remaining on EID‐CT (74 small‐sized at 70 kV, 18 medium‐sized at 80 kV and four large‐sized at 90 kV). EID‐CT showed 7%–17% higher d′ than PCCT with Bv40 kernel and strength four for small/medium patients. Lower monoenergetic images (40 keV) helped mitigate the difference to 1%–6%. For large patients, PCCT's detectability was up to 31% higher than EID‐CT. PCCT has thinner slices but similar noise levels for similar reconstruction parameters. The noise increased with lower keV levels in PCCT (≈30% increase), but higher QIR strengths reduced noise. PCCT's iodine contrast was stable across patient sizes, while EID‐CT had 33% less contrast in large patients than in small‐sized patients. Conclusion At 120 kV, thin slice PCCT enables CCTA in phantom scans representing large patients without raising radiation dose or affecting vessel detectability. However, higher doses are needed for small and medium‐sized patients to obtain a similar image quality as in EID‐CT. The alternative of using lower mono‐energetic levels requires further evaluation in clinical practice.
