Abstract:Background Asian women are often faced with difficult intubation with double-lumen tube (DLT). Previous studies have illustrated that cricoid and left main bronchus diameter together can predict the DLT size better, but it will take extra computed tomography (CT) scans for the patients. We hypothesized that predicting the best-fit left double-lumen tubes (LDLTs) size, ultrasound will show better efficacy than CT without increasing intubating complications for Asian women. Methods Our study enrolled 152 female patients undergoing elective thoracic surgery between March 2021 to October 2021. Patients were randomized into two groups: 75 in ultrasound group and 77 in the computed-tomography group. Patients in ultrasound group (US group) chosen the LDLT size according to the transverse diameter of the cricoid cartilage measured by ultrasound (TD-US). Controlled group based on the equivalent diameter of the left main bronchus (ED-LMB) measured by airway rebuilding of the helix CT (CT group). Results The overall accuracy of LDLT was compared. The proportion of the appropriate DLTs was similar between two groups (78.7 vs. 67.5%, P =0.161). The number of inappropriate cases in CT group increased when height of Asian women is under 160 cm (P <0.05). US group showed less resistance to pass the glottis (P <0.001) and took shorter time in intubating (P =0.002). Conclusion: Using ultrasound alone to select LDLT size has higher accuracy among Asian women less than 160 cm compared with CT. Trial registration Chictr.org.cn, registration number: ChiCTR2100043764
Combination of computed tomography measurements and flexible video bronchoscope guidance for accurate placement of the right-sided double-lumen tube: a randomised controlled trial

Abstract:Objective: To compare the modified strategy for the right-sided double-lumen tube (R-DLT) placement using a combination of CT measurements and flexible video bronchoscopy guidance with traditional bronchoscopy technique. Trial design, setting and participants: Double-blind, parallel randomised control trial at a tertiary care medical centre in China. 100 patients undergoing video-assisted thoracoscopic surgery and requiring R-DLT were randomly allocated to the control group and the intervention group. Intervention: The control group used the traditional bronchoscopy-guided technique. In the intervention group, the length and anteroposterior diameter of the right main bronchus (RMB) were measured on CT images to select the side and size of the Rüsch tube, and then a black depth marker was placed on the tube according to the difference between the length of the RMB and the bronchial cuff. Under the guidance of bronchoscopy, the depth marker should be placed parallel to the tracheal carina and a characteristic white line on the tube should be parallel to the midline of the tracheal carina. Main outcomes: The primary endpoint was the positioning of right upper lobe (RUL) ventilatory slot and RUL bronchial orifice. The secondary endpoints included intubation data and perioperative adverse events. Results: Compared with the control group, our modified strategy significantly increased the optimal and acceptable position rate (76% vs 98%, respectively; p<0.039), decreased the replacement rate (80% vs 94%; p=0.042), shortened the intubation time (101.4±7.3 s vs 75.2±8.1 s; p=0.019) and reduced the incidence of transient hypoxaemia (25% vs 6%; p=0.022), subglottic resistance (20% vs 6%; p=0.037), tracheobronchial injury (35% vs 13%; p=0.037) and postoperative RUL collapse (15% vs 2%; p=0.059). Conclusion: This study demonstrates the superiority of our strategy and provides a new viable method for R-DLT placement. Goal-directed Fluid Optimization Based on Stroke Volume Variation and Cardiac Index During One-Lung Ventilation in Patients Undergoing Thoracoscopy Lobectomy Operations: a Pilot Study

Abstract:OBJECTIVES: This pilot study was designed to utilize stroke volume variation and cardiac index to ensure fluid optimization during one-lung ventilation in patients undergoing thoracoscopic lobectomies. METHODS: Eighty patients undergoing thoracoscopic lobectomy were randomized into either a goal-directed therapy group or a control group. In the goal-directed therapy group, the stroke volume variation was controlled at 10%±1%, and the cardiac index was controlled at a minimum of 2.5 L.min-1.m-2. In the control group, the MAP was maintained at between 65 mm Hg and 90 mm Hg, heart rate was maintained at between 60 BPM and 100 BPM, and urinary output was greater than 0.5 mL/kg-1/h-1. The hemodynamic variables, arterial blood gas analyses, total administered fluid volume and side effects were recorded. RESULTS: The PaO2/FiO2-ratio before the end of one-lung ventilation in the goal-directed therapy group was significantly higher than that of the control group, but there were no differences between the goal-directed therapy group and the control group for the PaO2/FiO2-ratio or other arterial blood gas analysis indices prior to anesthesia. The extubation time was significantly earlier in the goal-directed therapy group, but there was no difference in the length of hospital stay. Patients in the control group had greater urine volumes, and they were given greater colloid and overall fluid volumes. Nausea and vomiting were significantly reduced in the goal-directed therapy group. CONCLUSION: The results of this study demonstrated that an optimization protocol, based on stroke volume variation and cardiac index obtained with a FloTrac/Vigileo device, increased the PaO2/FiO2-ratio and reduced the overall fluid volume, intubation time and postoperative complications (nausea and vomiting) in thoracic surgery patients requiring one-lung ventilation.

Abstract:BACKGROUND Tracheal injuries, vocal cord injuries, sore throat and hoarseness are common complications of double-lumen tube (DLT) intubation. OBJECTIVE This study aimed to evaluate the effects of ’video double-lumen tubes’ (VDLTs) on intubation complications in patients undergoing thoracic surgery. DESIGN A randomised controlled study. SETTINGT Xuzhou Cancer Hospital, Xuzhou, China, from January 2023 to June 2023. PATIENTS One hundred eighty-two patients undergoing elective thoracic surgery with one-lung ventilation were randomised into two groups: 90 in the DLT group and 92 in the VDLT group. INTERVENTION VDLT was selected for intubation in the VDLT group, and DLT was selected for intubation in the DLT group. A fibreoptic bronchoscope (FOB) was used to record tracheal and vocal cord injuries. MAIN OUTCOME MEASURES The primary outcomes were the incidence of moderate-to-severe tracheal injury and the incidence of vocal cord injury. The secondary outcomes included the incidence and severity of postoperative 24 and 48 h sore throat and hoarseness, RESULTS The incidence of moderate-to-severe tracheal injury was 32/90 (35.6%) in the DLT group, and 45/92 (48.9%) in the VDLT group ( P = 0.077; relative risk 1.38, 95% CI, 0.97 to 1.95). The incidence of vocal cord injury was 31/90 (34.4%) and 34/92 (37%) in the DLT and VDLT groups, respectively ( P = 0.449). The incidence of postoperative 24 h sore throat and hoarseness was significantly higher in the VDLT group than in the DLT group (for sore throat: P = 0.032, relative risk 1.63, 95% CI, 1.03 to 2.57; for hoarseness: P = 0.018, relative risk 1.48, 95% CI, 1.06 to 2.06). CONCLUSION There was no statistically significant difference in the incidence of moderate-to-severe tracheal injury and vocal cord injury between DLTs and VDLTs. While improving the first-attempt success rate, intubation with VDLT increased the incidence of postoperative 24 h sore throat and hoarseness. Comparison of safety between double lumen-tube and endobronchial occluder for single-lung ventilation

Abstract:Background: Double lumen-tube is frequently used in clinic for single-lung ventilation. However, the tube placement and location takes much time and there are many complications such injured bronchus, vocal cord and throat postoperatively. Objective: To compare the safety and impact on the incidence and severity of postoperative hoarseness, vocal cord lesions, and sore throat between double lumen-tube and endobronchial occluder for single-lung ventilation. Design, Time and Setting: Randomized controlled observation was performed at Ningbo Medical Treatment Center Lihuili Hospital from January 2006 to October 2007. Participants: 100 patients who underwent resection of esophageal cancer by single-lung ventilation were randomly divided into endobronchial occluder and double lumen-tube groups (n=50). Methods: All patients were subjected to single-lung ventilation by endobronchial occluder or double lumen-tube, respectively. The tracheal intubation was performed by the same anesthetist. Main Outcome Measures: 1 The time for intubation; 2 Atelectasis and exposure extent in surgery at single-lung ventilation; 3 Postoperative hoarseness and sore throat were assessed at 24, 48 and 72 hours, and bronchial injuries and vocal cord lesions were examined by bronchoscopy immediately after surgery. Results: The time for intubation in double lumen-tube group was longer than endobronchial occluder group (P < 0.05). There were no significant differences in mean arterial blood pressure and basic vital sign before and after intubation between two groups (P > 0.05). There were no significant differences in atelectasis and exposure extent in surgery between two groups (P > 0.05). Postoperative hoarseness occurred significantly more frequently in the double-lumen group than in the endobronchial occluder group (P < 0.05). The incidence of bronchial injuries was nearly equivalent in two groups (P > 0.05). No major complications such as bronchial ruptures were observed in both groups. Conclusion: Single-lung ventilation can be achieved via either a double lumen-tube or an endobronchial occluder. However, occluder is recommended for single-lung ventilation because it can reduce the incidence of vocal cord injuries, postoperative hoarseness, and sore throat.
A Comparison of Conventional Rotating Method and Non-Rotating Method for Double-Lumen Tube Insertion Using a Customized Rigid J-Shaped Stylet for One-Lung Ventilation: A Randomized Controlled Trial

Abstract:Background: The conventional double-lumen tube (DLT) insertion method requires a rotatory maneuver that was developed using direct laryngoscopy and may not be optimal for video laryngoscopy. This study compared a new non-rotatory maneuver with the conventional method for DLT insertion using video laryngoscopy. Methods: Patients scheduled for thoracic surgery requiring one-lung ventilation were randomly assigned to either the rotating (R) or non-rotating (NR) method groups. All patients were intubated using a customized rigid J-shaped stylet, a video laryngoscope, and a left-sided silicone DLT. The conventional rotatory maneuver was performed in the R group. In the NR group, the stylet was inserted with its tip oriented anteriorly (12 o'clock direction) while maintaining the bronchial lumen towards the left (9 o'clock direction). After reaching the glottic opening, the tube was inserted using a non-rotatory maneuver, maintaining the initial orientation. The primary endpoint was the intubation time. Secondary endpoints included first-trial success rate, sore throat, hoarseness, and airway injury. Results: Ninety patients (forty-five in each group) were included. The intubation time was significantly shorter in the NR group compared to the R group (22.0 [17.0, 30.0] s vs. 28.0 [22.0, 34.0] s, respectively), with a median difference of 6 s (95% confidence interval [CI], 3-11 s; p = 0.017). The NR group had a higher first-attempt success rate and a lower incidence of sore throats. Conclusions: The non-rotatory technique with video laryngoscopy significantly reduced intubation time and improved first-attempt success rate, offering a viable and potentially superior alternative to the conventional rotatory technique.
Application of Minor Auscultation Method in Positioning of Right Double Lumen-endobronchial Tube

Abstract:Objective To present a minor auscultation method and evaluate its feasibility in positing of right double lumen-endobronchial tube(R-DLT).Methods A total of fourty patients(ASA Ⅰ-Ⅱ,150-178 cm,aged 29-79 years old) requiring elective thoracic surgery undergoing general anesthesia were enrolled in this study.All patients were intubated with a R-DLT.they were randomly divided into control group(Group C) and experimental group(Group E)(n=20,each group).The former was applied with traditional positioning method,and the latter was adopted with minor auscultation method which was intubated with R-DLT gradually and received auscultation on the left anterior of chest simultaneously,then the R-DLT was introduced another 3.4/3.5cm when the breath sounds suddenly weakened.In this study,the followings were measured:①time required to position each tube satisfactorily;②frequency of malposition with fiberoptic bronchoscopy(FOB) before and during surgery;③arterial blood gas(ABG) after DLT and OLV10min;④glottal edema,trachyphonia and damage of airway mucosa.Results We found no difference in general condition,PaO2,PaCO2,pH,malposition frequency after lateral position and numbers of patients with glottal edema,damage of mucosa after tube withdrawing between the groups(P>0.05).Time required for tube posiing of Group E(154.5s) was significantly lower than Group C(216.7s)(P<0.05),and had higher success rates(95% vs 10%)(P<0.05).Conclusion Minor auscultation method is superior to traditional positioning method in the required time for initial positioning,and is characterized by simplicity of operation as well as cause little hurt to mucosa and glottis.
Evaluation of Endotracheal Intubation with a Flexible Fiberoptic Bronchoscope in Lateral Patient Positioning: A Prospective Randomized Controlled Trial

Abstract:Background: There is an unmet need for a reliable method of airway management for patients in the lateral position. This prospective randomized controlled two-center study was designed to evaluate the feasibility of intubation using a flexible fiberoptic bronchoscope in the lateral position during surgery. Methods: Seventy-two patients scheduled for elective nonobstetric surgery in the lateral decubitus position requiring tracheal intubation under general anesthesia at Lishui Central Hospital of Zhejiang Province and Jiaxing First Hospital of Zhejiang Province from April 1, 2015, to September 30, 2015, were enrolled in this study. Patients were randomly assigned to the supine position group (Group S, n=38) and the lateral position group (Group L, n=34). Experienced anesthetists performed tracheal intubation with a fiberoptic bronchoscope after general anesthesia. The time required for intubation, intubation success rates, and hemodynamic changes was recorded. Between-group differences were assessed using the Student's t-test, Mann-Whitney U-test, or Chi-square test. Results: The median total time to tracheal intubation was significantly longer in Group S (140.0 [135.8, 150.0]s) compared to Group L (33.0[24.0, 38.8]s) (P<0.01). The first-attempt intubation success rate was significantly higher in Group L (97%) compared to Group S (16%). Hemodynamic changes immediately after intubation were more exaggerated in Group S compared to Group L (P=0.02). Conclusion: Endotracheal intubation with a flexible fiberoptic bronchoscope may be an effective and timesaving technique for patients in the lateral position.

Abstract:Summary An attempt was made to demonstrate acquired resistance to Schistosoma mansoni infection in mice and hamsters. In one series of experiments, mice with light bisexual S. mansoni infections were challenged with an exposure great enough to kill uninfected mice. The survival pattern of the superinfected mice was significantly different from that of the controls in that the deaths were spread out over a much longer period. Apparently some of the mice were sufficiently immunized so that they were able to survive longer than the controls. In another set of experiments infected mice and hamsters were similarly challenged. Later, total worm counts were made and it was found that significantly fewer new worms developed in the superinfected animals than in the controls. These findings demonstrate that mice and hamsters develop resistance to reinfection with S. mansoni.
Does the periportal end of a double-lumen endobronchial tube need to be fixed to prevent dislocation of the cuffed end caused by a change in position? A randomized controlled trial

Abstract:This study aimed to evaluate the effects on the dislocation and misalignment of the cuffed end of a double-lumen endobronchial tube (DLT) when a patient moves from a horizontal to a lateral position without fixation. A total of 148 patients who had undergone video-assisted thoracoscope surgery were enrolled and randomly divided into two groups: a group in which the periportal end of the DLT was fixed with tape (group I; n = 74) and a group in which the periportal end of the DLT remained unfixed (group II; n = 74). Both groups were given an intravenous induction for double-lumen endobronchial intubation and then moved from a horizontal position to a lateral position, after which the alignment of the bronchial cuffed end of the DLT was assessed using a fiberoptic bronchoscope. After lateral position, the dislocation rate of group I and group II was 44.6% and 20.2%, and the misalignment rate was 27.0% and 8.1%, respectively, the incidence of dislocation and misalignment was significantly lower in group II than in group I after the change to a lateral position ( p < 0.05). After lateral position, the total rate of airway injury was 25.7% in group I and 5.4% in group II, the incidence of airway injury was significantly lower in group II than in group I ( p < 0.05), as was the incidence of sore throat, hoarseness, and cough on postoperative day 1 ( p < 0.05). The average outward dislocation of the periportal end of the DLT in group II was 1.5 cm. A DLT without periportal fixation is less likely to be displaced and poorly aligned when the patient moves from a horizontal to a lateral position, which could facilitate intra-operative management and reduce the incidence of postoperative complications. Through a randomized controlled trial, this study innovatively found that no double-lumen endobronchial tube (DLT) peripheral tape binding can prevent the dislocation and misalignment of the DLT bronchial cuffed end in patients undergoing thoracic surgery from horizontal to lateral position.

Abstract:Introduction With the growing emphasis on swift recovery, minimally invasive thoracic surgery has advanced significantly. Video-assisted thoracoscopic surgery (VATS) has seen rapid development, and the double-lumen tube (DLT) remains the most dependable method for tracheal intubation in VATS. However, hypoxaemia during DLT intubation poses a threat to the perioperative safety of thoracic surgery patients. Recently, transnasal high-flow nasal oxygen (HFNO) has shown promise in anaesthesia, particularly in handling short-duration hypoxic airway emergencies. Yet, its application in the perioperative period for patients undergoing pulmonary surgery with compromised cardiopulmonary function lacks evidence, and there are limited reliable clinical data. Methods and analysis A prospective, randomised, controlled, single-blind design will be employed in this study. 112 patients aged 18–60 years undergoing elective VATS-assisted pulmonary surgery will be enrolled and randomly divided into two groups: the nasal high-flow oxygen group (H group) and the traditional mask transnasal oxygen group (M group) in a 1:1 ratio. HFNO will be used during DLT intubation for the prevention of asphyxia in group H, while conventional intubation procedures will be followed by group M. Comparison will be made between the two groups in terms of minimum oxygen saturation during intubation, hypoxaemia incidence during intubation, perioperative complications and postoperative hospital days. Ethics and dissemination Approval for this study has been granted by the local ethics committee at Shenzhen Second People's Hospital. The trial results will be disseminated through peer-reviewed journals and scientific conferences. Trial registration number NCT05666908.

A Novel Tubeless Urinary Catheter Protocol Enhanced Recovery after Minimally Invasive Lung Surgery

Abstract:Objectives: Although previous studies have shown the feasibility of non-intubated techniques, it is unknown whether avoiding urinary catheters can enhance recovery. This study aimed to determine whether the tubeless urinary catheter protocol is feasible and beneficial for minimally invasive lung surgery. Methods: Patients were randomized to the control group, completely tubeless group, and partially tubeless group. A propensity score-matched (PSM) analysis was performed to balance the non-random baseline characteristics. Complications and postoperative recovery were compared. Regression analysis was performed to identify the independent predictors of complications. A nomogram for predicting the risk of non-automatic micturition was constructed and internally validated. Results: One hundred fifty-nine patients were enrolled. The incidence rates of urinary irritation and urinary tract infection (UTI) were significantly lower in the tubeless groups (74.4 vs. 39.5%, p < 0.001; 28.2 vs. 8.6%, p = 0.001, respectively). The tubeless group had a higher proportion of 0-degree discomfort (81.5 vs. 30.8%, p = 0.001) and shorter duration of postoperative hospital stay than the control group (4.59 vs. 5.53 days, p < 0.001). No difference was observed in terms of urination retention and urinary incontinence between the tubeless group and the control group. After PSM, the advantages of the tubeless group still existed, and comparing to the partially tubeless group, the completely tubeless group was of even less UTI and more 0-degree discomfort (18.5 vs. 0.0%, p = 0.019; 96.3 vs. 59.3%, p = 0.002). The tubeless protocol was the only independent protective factor of urinary complications. A nomogram was constructed and showed good predictive ability. Conclusions: The tubeless catheterization protocol led to fewer complications, better compliance, and shorter hospital length of stay. The advantages were more significant with the completely tubeless protocol. The utility of our nomogram can assist clinicians in avoiding risks in performing the tubeless protocol.

In this study, the fluid-sealing characteristics of the endobronchial cuffs of three different commercially available DLTs (Broncho-Cath with a polyvinylchloride cuff, Broncho-Cath with a polyurethane cuff, and Human Broncho with a silicone cuff) and two different bronchial blockers (Arndt and Coopdech bronchial blockers) were evaluated using a benchtop model. The lateral decubitus position for the surgical management of bleeding from the right lung was simulated. The artificial tracheobronchial tree was placed horizontally, with the left bronchus in the dependent position and the right bronchus in the non-dependent position. In the DLT experiments, the tracheobronchial tree was intubated with left-sided DLTs, and the endobronchial cuff was inflated to maintain the intracuff pressure at 25 cmH 2 O. In the experiments with bronchial blockers, each bronchial blocker was inserted into the right bronchus, and the endobronchial cuff was inflated to seal the main bronchus. A fluid leakage test around the endobronchial cuff was performed using three different types of DLT (size 35, 37, and 41 Fr, each) and two different types of bronchial blockers (9 Fr). The 5 mL of colored water was poured into the right bronchus to simulate the blood flow from the operative side, and the times to the first and 100% leakage around the endobronchial cuff were recorded. Each bronchial blocker showed significantly less leakage over time than the other DLTs ( P < 0.05). Fluid was not fully leaked around the cuffs for 24 h with either bronchial blocker. The times to first and 100% leakage were not significantly different among different types of DLTs. The times to first and 100% leakage did not also differ among the three different sizes of each type DLT. There was no significant difference in the time to first leakage around the endobronchial cuffs between Arndt and Coopdech bronchial blockers. Bronchial blockers provided a more effective seal against lung-to-lung aspiration than DLTs in the lateral decubitus position for thoracotomy in the benchtop model.

Abstract:Objective To compare the DLT with the torque control blocker univent(TCBU) and to determine whether there are objective advantages of one over the other in one-lung ventilation during elective thoracic surgical cases.Methods 60 patients needing one-lung ventilation during elective thoracic surgical cases were randomly divided into Group D(DLT) and Group U(TCBU).The following variables were recorded and compared:①time required for intubation and position each tube,②ration of successful blind intubation,③ increase in airway peak pressure(PIP),④frequency of malpositions,⑤number of times that the fiberoptic was achieved,⑥surgical exposure,and ⑦incidence of sore throat or hoarseness.Results Statistical difference were found in increase of PIP and incidence of sore throat(P 0.05).No statistical difference were found in the time required to intubation and position each tube,the ration of successful blind intubation,the frequency of malpositions,the number of times that the fiberoptic was achieved and the surgical exposure(P 0.05).Conclusion TCB univent applied for one-lung ventilation is as good as DLT,and in some specific clinical situations it can offer more advantages over the DLT.
Clinical Evaluation of Univent tube in Airway Management during Video-Assisted Thoracoscopic Surgery

Abstract:Objective To evaluate the value of Univent tube in the airway management during one-lung ventilation. Methods A total of 40 patients with pneumothorax undergoing video-assisted thoracoscopic surgery (VATS) were randomly divided into Univent (U) and double-lumen tube (Robertshaw) (D) groups (20 in each). In U group, intubation was performed under the guidance of a fibreoptic bronchoscope; while in D group, the position of the tube was confirmed using fibreoptic bronchoscopy after intubation. The time of intubation, number of secondary dislodgements, quality of lung deflation, peak airway pressure during one-lung ventilation, and blood artery gas analysis data in the two groups were recorded 30 minutes after one-lung ventilation. Results The intubation time in U group was significantly longer than that in D group [(6.18±1.26) min vs (3.26±0.82) min, t=8.654, P=0.000]. The number of secondary dislodgements was 7 in U group, and 5 in D group(U=187.500, P=0.663). No significant difference was detected in the quality of lung deflation between the two groups, (U group: 15 excellent, 3 fair, and 2 poor; D group: 17 excellent, 2 fair, and 1 poor;U=179.500, P=0.583). The peak airway pressure during one-lung ventilation in U group was significantly lower than that in D group [(15.3±3.5) cm H2O vs (21.4±6.6) cm H2O, t=-3.649, P=0.001]; and the oxygen pressure in group U was significantly higher than that in group D [(303.8±65.7) mm Hg vs (258.4±72.9) mm Hg, t=2.066, P=0.046]. No significant difference was found in carbon dioxide pressure and oxygen saturation between the two groups. Conclusion During VATS, univent tube showed the same effectiveness and safety as DLT for one-lung ventilation.

Abstract:To assess the complications of conventional and fiberoptic endobronchial intubations using reusable (Leyland, London) and disposable (Rüsch, Waiblinger, Germany; Sheridan, Argyle, NY) double-lumen tubes (DLTs), endobronchial intubations occurring over a 12-month period were prospectively studied at this hospital. Residents working with staff anesthesiologists placed either left or right reusable (Leyland) or disposable (Rüsch or Sheridan) DLTs. The DLT used, the use of fiberoptic bronchoscopy (FOB), findings at FOB if used during the intubation or operation, and complications occurring during the case (SpO2 < 90%, peak inflation pressure > 40 cm H2O, air trapping, poor lung isolation, and airway trauma) were recorded. Two hundred thirty-four intubations were analyzed (102 right, 132 left; 70 Leyland reusable DLTs, 66 Rüsch disposable tubes, and 98 Sheridan tubes). Physical signs alone were used to confirm tube position more frequently when Leyland tubes were placed compared with disposable tubes (79% v 39%, P < 0.0001). Rüsch and Sheridan DLTs had similar rates of conventional placement. Nineteen percent of reusable tubes and 44% of disposable tubes required position adjustments using FOB during the initial intubation (P = 0.0002). Disposable tubes also more commonly required readjustment using FOB during the operation (30% v 7%, P < 0.0005). Complications occurred in 42/234 patients (18%). The frequency of specific complications was: decreased SpO2, 9%; increased airway pressures, 9%; poor lung isolation, 7%; air trapping, 2%, and airway trauma, 0.4%. Right-sided Sheridan DLTs had a statistically higher incidence of malposition, resulting in poorer lung isolation.(ABSTRACT TRUNCATED AT 250 WORDS)
