Abstract:AimChild Death Review (CDR) is a method in which every child death is systematically and multidisciplinary examined to (1) improve death statistics, (2) identify factors that give direction for prevention, (3) translate the results into possible interventions, and (4) support families. The aim of this study was to determine to what extent procedures of organizations involved in the (health) care for children in The Netherlands cover these four objectives of CDR.Subject and methodsOrganizations in the Eastern part of The Netherlands and Dutch umbrella organizations involved in child (health) care were asked to provide their protocols, guidelines or other working agreements that describe their activities and responsibilities in case of a child’s death. Eighteen documents and nine interview reports were made available. For the analyses we used scorecards for each CDR objective.ResultsThe procedures of Perined, the National Cot Death Study Group, Dutch Cot Death Foundation and Child Protection Service cover the largest part of the objectives of CDR. Organizations pay most attention to the translation of results into possible interventions. Family support gets the least attention in protocols, guidelines and other working agreements.ConclusionDutch organizations separately cover parts of CDR. When the procedures of organizations are combined, all CDR objectives are covered in the response to only specific groups of child deaths, i.e., perinatal deaths, Sudden Unexpected Deaths in Infants and fatal child abuse cases. Further research into the conditions that are needed for an optimal implementation of CDR in The Netherlands is necessary. This research should also evaluate the recently implemented NODOK procedure (Further Examination of the Causes of death in Children), directed to investigate unexplained deaths in minors 0–18 years old.

Abstract:Background: Globally, the proportion of child deaths that occur in the neonatal period remains a high level of 37-41%. Differences of cause in neonate death exist in different regions as well as in different economic development countries. The specific aim of this study was to investigate the causes, characteristics, and differences of death in neonates during hospitalization in the tertiary Neonatal Intensive Care Unit (NICU) of China. Methods: All the dead neonates admitted to 26 NICUs were included between January 1, 2011, and December 31, 2011. All the data were collected retrospectively from clinical records by a designed questionnaire. Data collected from each NICU were delivered to the leading institution where the results were analyzed. Results: A total of 744 newborns died during the 1-year survey, accounting for 1.2% of all the neonates admitted to 26 NICUs and 37.6% of all the deaths in children under 5 years of age in these hospitals. Preterm neonate death accounted for 59.3% of all the death. The leading causes of death in preterm and term infants were pulmonary disease and infection, respectively. In early neonate period, pulmonary diseases (56.5%) occupied the largest proportion of preterm deaths while infection (27%) and neurologic diseases (22%) were the two main causes of term deaths. In late neonate period, infection was the leading cause of both preterm and term neonate deaths. About two-thirds of neonate death occurred after medical care withdrawal. Of the cases who might survive if receiving continuing treatment, parents' concern about the long-term outcomes was the main reason of medical care withdrawal. Conclusions: Neonate death still accounts for a high proportion of all the deaths in children under 5 years of age. Our study showed the majority of neonate death occurred in preterm infants. Cause of death varied with the age of death and gestational age. Accurate and prompt evaluation of the long-term outcomes should be carried out to guide the critical decision.
Socio-economic mortality differences in the Netherlands in 1950–1984: A regional study of cause-specific mortality

Abstract:The finding that mortality differences between occupational classes in England and Wales have widened during the postwar period raises the question whether a similar development has occurred in other industrialised countries. In this paper, a comparison is made with results from a geographical study on the Netherlands. This study compares four periods between 1950 and 1984 by means of a standard regional division, a single socio-economic index, uniform cause-of-death groups and a standard regression procedure. During the postwar period, the relationship between socio-economic level and all-cause mortality has become (more) negative. This development can to a large extent be attributed to 'negative' trends for lung cancer, diabetes mellitus, ischaemic heart disease, cerebrovascular disease and traffic accidents. High-level regions have fared better partly because favourable changes in national mortality trends seem to have begun first in these regions. The findings from this regional study agree to a large extent with evidence from Dutch studies at the individual level. It is concluded that socio-economic mortality differences in England and Wales and the Netherlands have probably developed similarly in various respects.

Abstract:BackgroundPerinatal mortality is an important indicator of health. European comparisons of perinatal mortality show an unfavourable position for the Netherlands. Our objective was to study regional variation in perinatal mortality within the Netherlands and to identify possible explanatory factors for the found differences.MethodsOur study population comprised of all singleton births (904,003) derived from the Netherlands Perinatal Registry for the period 2000–2004. Perinatal mortality including stillbirth from 22+0 weeks gestation and early neonatal death (0–6 days) was our main outcome measure. Differences in perinatal mortality were calculated between 4 distinct geographical regions North-East-South-West. We tried to explain regional differences by adjustment for the demographic factors maternal age, parity and ethnicity and by socio-economic status and urbanisation degree using logistic modelling. In addition, regional differences in mode of delivery and risk selection were analysed as health care factors. Finally, perinatal mortality was analysed among five distinct clinical risk groups based on the mediating risk factors gestational age and congenital anomalies.ResultsOverall perinatal mortality was 10.1 per 1,000 total births over the period 2000–2004. Perinatal mortality was elevated in the northern region (11.2 per 1,000 total births). Perinatal mortality in the eastern, western and southern region was 10.2, 10.1 and 9.6 per 1,000 total births respectively. Adjustment for demographic factors increased the perinatal mortality risk in the northern region (odds ratio 1.20, 95% CI 1.12–1.28, compared to reference western region), subsequent adjustment for socio-economic status and urbanisation explained a small part of the elevated risk (odds ratio 1.11, 95% CI 1.03–1.20). Risk group analysis showed that regional differences were absent among very preterm births (22+0 – 25+6 weeks gestation) and most prominent among births from 32+0 gestation weeks onwards and among children with severe congenital anomalies. Among term births (≥ 37+0 weeks) regional mortality differences were largest for births in women transferred from low to high risk during delivery.ConclusionRegional differences in perinatal mortality exist in the Netherlands. These differences could not be explained by demographic or socio-economic factors, however clinical risk group analysis showed indications for a role of health care factors.

Dutch euthanasia revisited

Abstract:The results of a follow-up study of euthanasia by the Dutch government, five years after the first study, were published on November 26, 1996. This article provides a detailed review of the two reports comparing and contrasting the statistics cited therein. The author notes that the "rules of careful conduct" proposed by the courts and by the Royal Dutch Society of Medicine were frequently disregarded. Special topics included for the first time in the second study were the notification and non-prosecution procedure, euthanasia of newborns and infants, and assisted suicide in psychiatric practice. The authors of the follow-up report state that it would be desirable to reduce the number of "terminations of life without patients' request," but this must be the common responsibility of the doctor and the patient. They suggest that the person who does not wish to have his life terminated should declare this clearly, in advance, verbally and in writing, preferably in the form of a living will. Involuntary euthanasia was rampant in 1990 and equally rampant in 1995. The author concludes that Dutch doctors who practice euthanasia are not on the slippery slope. From the very beginning, they have been at the bottom.

Abstract:The decline in infant mortality played a crucial role in the health transition in the Western World. This decline among the vulnerable new-borns was however not an evenly dispersed process. Inequalities as a result of regional differences, cultural influences or socioeconomic status shaped the paths towards low mortality rates. The role of socioeconomic status in levels of infant mortality and its decline remain highly debated. In this article, we study the development of socioeconomic disparities in infant mortality in the Dutch town of Maastricht in the period 1864–1955. This study uses unique individual-level cause of death data in order to see when changes in disease patterns took place for different socioeconomic groups. The aim is to identify socioeconomic inequalities by mapping changing epidemiological patterns over time, according to age within the first year of life. By deploying a multinomial logistic regression analysis we can trace the different timing of changes in the epidemiological regime. The results reveal that for most infants the change in mortality patterns from water- and foodborne infectious diseases towards a predominance of airborne infectious diseases occurred simultaneously with the massive decline in infant mortality from the start of the First World War onwards. Infants from the upper classes, however, appeared to have gained an earlier advantage, followed by infants from unskilled workers. Finally, from qualitative data it becomes clear that the awareness of the problematic nature of infant mortality, knowledge on infant care, hygienic practices, breastfeeding practices and the economic situation of World War I were factors aiding to the uneven decline in infant mortality.

Abstract:New Zealand's high mortality rate from sudden infant death syndrome (SIDS) prompted the development of the New Zealand Cot Death Study. A report of the analysis of the data from the first year has been published. This report now gives the major identified risk factors from the full 3 year data set. In this case-control study there were 485 infants who died from SIDS in the post-neonatal age group, and 1800 control infants, who were a representative sample of all hospital births in the study region. Obstetric records were examined and parental interviews were completed in 97.5% and 86.9% of subjects, respectively. As expected many risk factors for SIDS were confirmed including: lower socio-economic status, unmarried mother, young mother, younger school-leaving age of mother, younger age of mother at first pregnancy, late attendance at antenatal clinic, non-attendance at antenatal classes, Maori, greater number of previous pregnancies, the further south the domicile, winter, low birthweight, short gestation, male infant and admission to a special care baby unit. In addition, however, we identified four risk factors that are potentially amenable to modification.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:Background Increasing life expectancy in high-income countries has been linked to a rise in fall mortality. In the Netherlands, mortality rates from falls have increased gradually from the 1950s, with some indication of stabilisation in the 1990s. For population health and clinical practice, it is important to foresee the future fall mortality trajectories. Methods A graphical approach was used to explore trends in mortality by age, calendar period and cohorts born in the periods of 1915–1945. Population data and the numbers of people with accidental fall fatality as underlying cause of death from 1990 to 2021 were derived from Statistics Netherlands. Age-standardised mortality rates of unintentional falls per 100 000 population were calculated by year and sex. A log-linear model was used to examine the separate effects of age, period and cohort on the trend in mortality and to produce estimates of future numbers of fall deaths until 2045. Results While the total population increased by 17% between 1990 and 2021, absolute numbers of fall-related deaths rose by 230% (from 1584 to 5234), which was 251% (an increase of 576 deaths in 1990 to 2021 deaths in 2020) for men and 219% (from 1008 to 3213) for women. Age-standardised figures were higher for women than men and increased more over time. In 2020, 79% of those with death due to falls were over the age of 80, and 35% were 90 years or older. From 2020 to 2045, the observed and projected numbers of fall deaths were 2021 and 7073 for men (250% increase) and 3213 and 12 575 for women (291% increase). Conclusion Mortality due to falls has increased in the past decades and will continue to rise sharply, mainly caused by growing numbers of older adults, especially those in their 80s and 90s. Contributing risk factors are well known, implementation of preventive measures is a much needed next step. An effective approach to managing elderly people after falls is warranted to reduce crowding in the emergency care and reduce unnecessary long hospital stays.

A geographical analysis of the relationship between early childhood death and socio-economic environment in an English city

Abstract:The relationship between early childhood death and socio-economic conditions was studied by obtaining the home address, at the time of death, of 254 infants and children between the ages of 0 to 5 years who died within the City of Southampton, England, between 1977 and 1982. The addresses were grouped into 142 'super-enumeration' districts formed by combining enumeration districts with similar socio-economic characteristics. These 'super-enumeration' districts were then categorised into 15 cluster types which highlighted the major social characteristics of the areas within Southampton. The number of deaths for each cluster type was then ascertained. A strong relationship was found to exist between high rates of early childhood death and cluster types displaying adverse social conditions--in particular, high levels of unemployment, single-parent families and poor housing amenities. It is suggested that the approach taken by this study might be one way of pinpointing areas of cities where a disproportionately large number of babies and young children die. It might then be possible by a combined and concerted effort on the part of the medical profession, nurses, health visitors, social workers and local authority to save at least some very young babies and children who might otherwise die.

Child care practices and cot death in Hong Kong

Abstract:Aims: To document child care practices in Hong Kong which has a very low SIDS rate of 0.3/1000 live births. Methods: Data were collected by interview and postal questionnaires using a protocol developed in southern New Zealand. 195 mothers were recruited at the Prince of Wales Hospital and 100 completed the study. Results: 81% babies slept in the parents room. 32% shared a bed with parents but only a third were described as being "in direct contact". Only 9% of infants were still breast feeding by 4 weeks of age. 78% of babies slept on their backs, 18% on their sides and 3% on the fronts. Sheepskins were not used and 56/58 described underbedding as firm or moderately firm. At the time of birth only 3% of mothers smoked. Conclusions: Certain SIDS risk factors (bedsharing, lack of breast feeding) are common in Hong Kong, whereas others (prone sleep position, soft underbedding, maternal smoking) appear uncommon.
Child safety in cars: An observational study on the use of child restraint systems in The Netherlands

Abstract:Objective: To minimize children's injuries due to car accidents, children must be transported in approved child restraint systems (CRS). The European Union optimized child protection by implementing R129 legislation for CRS in 2013. However, compliance with CRS recommendations after introduction of this newer standard has been scarcely evaluated. The main objectives of this study were to determine the prevalence of various types of CRS misuse and to investigate the use of ISOFIX and i-Size seats and parental knowledge regarding safe transportation of children in cars 5 years after the introduction of the newer R129 standard. Methods: During a cross-sectional observational study in the summer of 2018, parking lots of sites in the Netherlands were visited by researchers. Arriving or departing Dutch drivers who transported children under 9 years old were interviewed by means of a questionnaire and the misuse of CRS was directly observed using a checklist. Misuse was defined as CRS inappropriate for the child (based on height and weight) and/or CRS wrongly installed in the car and/or child improperly restrained in CRS. Results: In total, 392 drivers and 470 children were included in the study. Results showed that 83% of the children were transported with at least 1 misuse of their CRS: 7% of the CRS were inappropriate for the child, 49% of the CRS were wrongly installed in the car, and 59% of the children were improperly restrained in a CRS. Most CRS were installed using the seat belt (88%) compared to ISOFIX (12%). ISOFIX usage did not result in less CRS installation misuse (P = .338). The number of observed i-Size CRS was low (n = 13, 3%). Most drivers were familiar with ISOFIX (76%), but only 13% of the drivers had heard of i-Size. Conclusions: For 9 out of 10 children, CRS misuse was observed. Many children were transported in an appropriate CRS, but various mistakes were made when installing the CRS in the car and restraining children in the CRS. Moreover, ISOFIX usage did not result in statistically significant less CRS installation misuse compared to traditional seat belt usage, and i-Size CRS are still rather unknown 5 years after their introduction.

Regional differences in mortality from conditions amenable to medical intervention in The Netherlands: a comparison of four time periods.

Abstract:In The Netherlands, as in many other countries, important geographical variation in mortality from conditions amenable to medical intervention exists. Associations with a number of simple medical care supply characteristics (general practitioner density, hospital bed density, and percentage of regional hospital beds located in university and small hospitals) are generally weak and inconsistent, both before and after controlling for possible confounding factors. We explored one of the possible reasons for this lack of consistency, which is the time dependency of the relationship between medical care supply and avoidable mortality. A comparison of associations in four time periods (1950-54, 1960-64, 1970-74 and 1980-84) shows that the percentage of variance in regional mortality levels which can be "explained" by the medical care supply variables has changed over time. Although the patterns of change differ little from what one would expect on the basis of the time of introduction of medical care innovations, the exact nature of the associations is puzzling. Apart from some expected negative associations between mortality and the presence of university hospitals, we also found a few unexpected positive associations with general practitioner density. Possible explanations for these findings are discussed, and it is concluded that further study is necessary to reveal the causes of a higher or lower mortality level for conditions considered to be amenable to medical intervention.

Demographic Transition in the Netherlands: A Statistical Analysis of Regional Differences in the Level and Development of the Birth Rate and of Fertility, 1850-1890

Abstract:Multivariate and causal analyses of the effects of 6 variables on the level and development of the Dutch birth rate between 1850 and 1890 help to explain the process of demographic transition in the Netherlands. Scholarly opinion has been divided about the influence of religion and the influence of regionally divergent attitudes on the process of modernization. In statistical analyses of 375 Dutch municipalities use is made of 6 variables that are indicators of geographic demographic economic and sociocultural phenomena. The preponderant weight of the purely demographic variable (death rate) stands out clearly. In every case the level and fall of the death rate showed the greatest correlation with the level and development of the birth rate. Regarding level of birth rate analyses showed the positive influence of economic (immigration surplus) cultural-geographic (agricultural systems as indicators of diverging attitudes) and cultural (religion and until 1871 illiteracy) variables. The geographic variable of population density showed no noticeable effect on level of birth rate. Before 1890 the regional factor was of much greater importance for the demographic transition process than the religious (Dutch Roman Catholicism) one. However in a later phase of the process (after 1900) the religious variable may have gained momentum and overtaken the regional explanation. In the analysis of the static situation (level of birth rate) religion played a slightly more decisive role in restraining birth rates than did the clay-sand distinction (agricultural systems). In the analysis of the dynamics (development of the birth rate) religion played no role. It seems that regional (geographically bounded patterns of culture have been decisive in the process of demographic transition in the Netherlands.

The frequency and types of resident relocations in Dutch nursing homes: a nationwide cohort study of electronic health record data

Abstract:We examined the occurrence of relocations of nursing home residents, the distribution of individual and group relocation types, the distribution of destinations, and trends over time. One third of Dutch nursing home residents relocated at least once during length of stay. Roughly 75 percent were individual relocations and the other 25 percent concerned group relocations. The average yearly number of individual relocations was about 3 times as high in the first 4 months after admission compared with later periods. A considerable proportion of Dutch long-stay nursing home residents experienced one or more relocations. One third of Dutch nursing home residents relocated at least once during length of stay. Roughly 75 percent were individual relocations and the other 25 percent concerned group relocations. The average yearly number of individual relocations was about 3 times as high in the first 4 months after admission compared with later periods. We performed an historical cohort study of pseudonymized registration data from a Dutch electronic health record. We selected data from nursing home residents aged 65 years and older who stayed on a ward for physical impairment or dementia and passed away between 2015 and 2019. Our study sample consisted of 26,060 long-stay nursing home residents from 67 nursing homes in the Netherlands. We examined the number of relocations, trends over time, individual versus group relocations and relocation destinations. We found that approximately one-third of long-stay nursing home residents relocated at least once with an average of 36 relocations per 100 residents per year. Roughly, 75 percent of relocations were individual relocations and 25 percent concerned group relocations. In the first 4 months after admission, the average number of individual relocations per 100 resident per year was about 3 times as often compared to later periods after admission. Most individual relocations were within the same type of care. A considerable proportion of Dutch long-stay nursing home residents experienced one or more relocations. Relocations for individual reasons occurred mostly in the first months after admission. Further investigation is warranted to explore which factors lead to relocations.

Abstract:Background: All women in the Netherlands should have equal access to obstetric care. However, utilization of care is shaped by demand and supply factors. Demand is increased in high risk groups (non-Western women, low socio-economic status (SES)), and supply is influenced by availability of hospital facilities (hospital density). To explore the dynamics of obstetric care utilization we investigated the joint association of hospital density and individual characteristics with prototype obstetric interventions. Methods: A logistic multi-level model was fitted on retrospective data from the Netherlands Perinatal Registry (years 2000-2008, 1.532.441 singleton pregnancies). In this analysis, the first level comprised individual maternal characteristics, the second of neighbourhood SES and hospital density. The four outcome variables were: referral during pregnancy, elective caesarean section (term and post-term breech pregnancies), induction of labour (term and post-term pregnancies), and birth setting in assumed low-risk pregnancies. Results: Higher hospital density is not associated with more obstetric interventions. Adjusted for maternal characteristics and hospital density, living in low SES neighbourhoods, and non-Western ethnicity were generally associated with a lower probability of interventions. For example, non-Western women had considerably lower odds for induction of labour in all geographical areas, with strongest effects in the more rural areas (non-Western women: OR 0.78, 95% CI 0.77-0.80, p<0.001). Conclusion: Our results suggest inequalities in obstetric care utilization in the Netherlands, and more specifically a relative underservice to the deprived, independent of level of supply.
