Abstract:OBJECTIVE: The aim of this study was to perform a comparative evaluation of the effectiveness of ultrasonic tips versus the Terauchi file retrieval kit (TFRK) for the removal of broken endodontic instruments.MATERIALS AND METHODS: A total of 80 extracted human first mandibular molars with moderate root canal curvature were selected. Following access cavity preparation canal patency was established with a size 10/15 K-file in the mesiobuccal canals of all teeth. The teeth were divided into 2 groups of 40 teeth each: the P group (ProUltra tips) and the T group (TFRK). Each group was further subdivided into 2 smaller groups of 20 teeth each according to whether ProTaper F1 rotary instruments were fractured in either the coronal third (C constituting the PC and TC groups) or the middle third (M constituting the PM and TM groups). Instrument retrieval was performed using either ProUltra tips or the TFRK.RESULTS: The overall success rate at removing the separated instrument was 90% in group P and 95% in group T (p > 0.05) The mean time for instrument removal was higher with the ultrasonic tips than with the TFRK (p > 0.05).CONCLUSION: Both systems are acceptable clinical tools for instrument retrieval but the loop device in the TFRK requires slightly more dexterity than is needed for the ProUltra tips.
Temperature changes of tooth surfaces under ultrasonic tip working against the wall of root canals-an in vitro study

Abstract:Objective To investigate the temperature changes of tooth surfaces under ultrasonic tip working against the wall of root canals,and the effect of power setting with or without water spray cooling on it.Methods Eight human premolars were used in an in vitro tooth model to use Satelec P5 ultrasonic system and ET40 tips working against the walls of root canal.Combinations of power settings(6 and 10) and with or without water spray were used to test their effect on temperature changes at the tooth surfaces using a thermocouple meter.Results The cutting of ultrasonic tip against the wall of root canal would cause significantly temperature rising on the tooth surfaces.An increase in temperature was observed with increasing power setting and a decrease in water spray.Conclusion Heat generation could be minimized by using low power settings and water spray.Care must be taken while using ultrasonic tips with high power setting and without water spray continued for a long time.
Exploring morphometric transformations in temperature‐sensitive NiTi instruments alongside real‐time intracanal temperature dynamics

Abstract:Aim This study was twofold: (i) it aimed to investigate the morphometric changes of three temperature‐sensitive nickel–titanium (NiTi) instruments at different temperatures, and (ii) to conduct an in vivo real‐time analysis of intracanal temperature changes. Methods Changes in the shape and length of XP‐Endo Shaper, XP‐Endo Finisher, and XP‐Endo Finisher‐R were evaluated in real time whilst heated in a temperature‐controlled water bath from 22 to 45°C. Instruments were fixed to a laminated water‐resistant 1 mm graph paper attached to a stone block. Instruments were imaged whilst subjected to increasing temperature using a digital camera attached to an operating microscope. From recorded videos, still frames were extracted at 10‐s intervals and changes in the length and shape of each instrument were measured and changes were plotted against time. Moreover, the intracanal temperature of distal roots of lower molars was measured in vivo for patients attending the clinic for non‐surgical root canal treatments. The temperature was measured using a K‐type thermocouple probe inserted into the mid‐root level after irrigating the canal with a solution set at room temperature (22°C) or heated to 45°C. The intraoral and intracanal temperatures were recorded using a video camera for 180 s at 5‐s intervals to plot the change in the intraoral and intracanal temperature, after both irrigation solution temperatures, with time. Results The shape transformation of XP‐Endo Shaper began at 31.5 ± 2.0°C and reached its optimal transformation at 35.1 ± 1.0°C. For the Finisher and Finisher‐R, shape transformations began at 29.2 ± 1.9 and 26.9 ± 2.2°C reaching the optimal transformation at 33.9 ± 1.4 and 32.7 ± 1.7°C, respectively. The average decreases in lengths of XP‐Endo Shaper, Finisher, and Finisher‐R after full transformation were 0.43 ± 0.23, 1.07 ± 0.22, and 1.15 ± 0.22 mm, respectively. The intracanal temperature reached 32.9 ± 0.8 and 33.2 ± 1.0°C after 3 min of application of irrigation solutions set at 22 or 45°C, respectively. Conclusion The tested instruments exhibited diverse changes in their shapes and lengths at varying temperatures. Despite the temperature of the irrigation solution, the intracanal temperature consistently remained lower than the intracanal temperature once equilibrium was reached. This highlights the importance of considering the temperature of irrigation solution during in vitro testing of endodontic instruments.

Nine extracted human lower premolars were standardized by cone-beam computed tomography and used. The root canal was instrumented with a Reciproc 40.06 file. Temperature measurement was performed using K-type thermocouple sensors attached to the middle, cervical, and apical thirds of the teeth. The teeth had their roots immersed in distilled water at 37oC, which were distributed into 3 experimental groups according to the mechanical agitation methods to be studied. US Group (n=3), Irrisonic Ultrasonic Tip activated through ultrasound; EC Group (n=3), Easyclean Tip coupled to a contra-angle low-speed handpiece; XP Group (n=3), XP-endo Finisher file coupled to an endodontic electrical motor. Temperature measurements were performed simultaneously with agitation and irrigation of intracanal irrigants. Statistical analysis was performed using SPSS software with a significance level of 5%. For multiple comparisons, the Tukey test was used. The association between mechanical agitation methods and root third was statistically significant. Regarding the temperatures recorded on the external surface of the roots, the ultrasonic tip was significantly higher than the XP-endo Finisher file and the Easyclean tip, which did not differ from each other. Regarding the ultrasonic tip, the external temperature in the middle third (39.46oC) of the root was significantly lower than in the cervical (40.41oC) and apical third (40.53oC). None of the agitation methods of irrigants studied presented heating above 47oC, and their use is safe for periodontal tissues.
Comparing Cyclic Fatigue Resistance and Free Recovery Transformation Temperature of NiTi Endodontic Single-File Systems Using a Novel Testing Setup

Abstract:The aim of this study was to assess the effect of body temperature (37 °C) on the cyclic fatigue resistance of three endodontic single-file systems using a new testing setup. One Shape® new generation (OS), WaveOneTM (WO) and WaveOne® GOLD (WOG), which are made from different NiTi alloys and operated in different motions (rotation/reciprocation), were evaluated. The study design included four groups. Each group comprised 30 files, 10 files of each of the three file systems, tested at 20 ± 2 °C (group 1 and 3) and at 37 ± 1 °C (group 2 and 4). All files were tested in a custom-made metal block with artificial canals of 60° angle, and a 5 mm and 3 mm radius of curvature, respectively. A heating element was attached to replicate a temperature of 37 °C. Files were introduced 18 mm into the canals and operated until failure. Transformation temperatures of five samples of each of the tested file systems were determined via the bend and free recovery (BFR) method. With the exception of WOG in canals with a 3 mm radius of curvature (p = 0.075), all the tested file systems showed statistically significantly less time needed to fracture when operated at 37 ± 1 °C compared to at 20 ± 2 °C in canals with a 5 mm and 3 mm radius of curvature using Mann–Whitney U test (p < 0.05). All file systems showed transformation temperatures below the body temperature. We concluded that body temperature directly affects the cyclic fatigue resistance of all tested file systems. Bend and free recovery can be suitable for the determination of austenite finish temperatures (Af) of endodontic instruments as it allows testing a longer portion of the instrument.

Abstract:Introduction The performance and mechanical properties of nickel-titanium (NiTi) instruments are influenced by factors such as cross-section, flute design, raw material, and manufacturing processes. Many improvements have been proposed by manufacturers during the past decade to provide clinicians with safer and more efficient instruments. Methods The mechanical performance of NiTi alloys is sensitive to their microstructure and associated thermomechanical treatment history. Heat treatment or thermal processing is one of the most fundamental approaches toward adjusting the transition temperature in NiTi alloy, which affects the fatigue resistance of NiTi endodontic files. The newly developed NiTi instruments made from controlled memory wire, M-Wire (Dentsply Tulsa Dental Specialties, Tulsa, OK), or R-phase wire represent the next generation of NiTi alloys with improved flexibility and fatigue resistance. The advantages of NiTi files for canal cleaning and shaping are decreased canal transportation and ledging, a reduced risk of file fracture, and faster and more efficient instrumentation. The clinician must understand the nature of different NiTi raw materials and their impact on instrument performance because many new instruments are introduced on a regular basis. Results This review summarizes the metallurgical properties of next-generation NiTi instruments, the impact of thermomechanical treatment on instrument flexibility, and the resistance to cyclic fatigue and torsion. Conclusions The aim of this review was to provide clinicians with the knowledge necessary for evidence-based practices, maximizing the benefits from the selection and application of NiTi rotary instruments for root canal treatment.
[Temperature Variation at the External Root Surface During Nd: YAG Laser Irradiation in the Root Canal in Vitro].

Abstract:OBJECTIVE:To assess the temperature variation of the root surface using Nd: YAG laser irradiation in the root canal with different power and to evaluate the safety of laser application on the periodontal region. METHODS:Thirty extracted human teeth with single-roots were collected. The teeth were cross-sectioned in the cervical portion, standardizing the roots at a 12-mm length. The roots were used as specimen. The roots were radiographed in the buccal-lingual direction to measure the thickness of the proximal walls, by means of a digital radiographic system. The specimens were divided into three groups according to the laser potency (1.5, 3.0, and 4.5 W). Each group was subdivided into two subgroups according to laser frequency (15 and 30 Hz). With the Nd: YAG laser irradiation for 20 s, the temperature variation of the root surface was monitored by thermocouples located at different parts of the root external wall and recorded by digital thermometers. RESULTS:The groups irradiated with 4.5 W presented the greatest temperature variation (above 10°C), followed by 3.0 and 1.5 W. The temperatures were statistically different (P < 0.01). The groups irradiated in the same potency, regardless of whether 15 Hz or 30 Hz was used, presented with no statistical difference (P > 0.05). The apical half of the root presented statistically higher temperature rises than the cervical half of the root (P < 0.01). CONCLUSION:The temperature variation of the root surface was associated with laser power, irradiation time, and the thickness of dentin. Application of Nd: YAG laser in the root at 1.5 W for 20 s can safely be used in endodontic treatment.
Endothelin does not interact with angiotensin II in the coronary vascular bed of anesthetized dogs.

Abstract:We postulated that individually subthreshold circulating levels of angiotension II (Ang II) and endothelin-1 (ET-1) might induce fulminant coronary vasoconstriction when both are present. In 16 pentobarbital-anesthetized, open-chest mongrel dogs, blood pressure, heart rate, and standard ECG were registered continuously. Coronary blood flow (CBF) was measured in the left anterior descending coronary artery (LAD) by an electromagnetic flow probe. Drugs were administered into the LAD via an indwelling catheter. Bolus injections of Ang II (7.8 x 10(-13) to 3.9 x 10(-11) M) and ET-1 (10(-12) to 10(-9) M) induced a dose-dependent decrease in CBF (delta CBFmax -82 +/- 10% for Ang II and -91 +/- 8% for ET-1). Simultaneous Ang II and ET-1 boluses had slightly smaller effects on CBF than the calculated additive figure. Five-minute infusions of Ang II (10(-12) to 10(-10) M/min) and ET-1 (5 x 10(-12) to 2 x 10(-10) M/min) induced a slight decrease in CBF (delta CBFmax -12 +/- 9% for Ang II and -19 +/- 9% for ET). Background ET-1 or Ang II infusions did not alter the dose-response curve of the other drug. Simultaneous Ang II and ET-1 infusions at different rates (10(-12) to 10(-10) M/min for Ang II and 5 x 10(-12) to 2 x 10(-10) M/min for ET-1) over 5 min had similar effects on CBF as the calculated additive figure (delta CBFmax -35 +/- 17% for the joint administration of the highest doses). We conclude that after simultaneous administration into the dog coronary artery, Ang II and ET-1 do not interact sufficiently to induce fulminant vasoconstriction.
Effects of ultrasound settings on temperature changes in NiTi implants

Abstract:Background: Shape memory alloys (SMAs) are well-known for their unique ability to undergo a shape change in response to a thermal stimulus. A frequently-used SMA for biomedical devices is NiTi, although its superelastic features tend to be emphasized more than the ability to change shape. Minimally invasive NiTi implants which can reconfigure or adjust their shape across several temperature points could provide desirable surgical outcomes. For decades, therapeutic ultrasound has been used medically as a non-invasive method for tissue thermal therapy. Ultrasound's ability to quickly raise temperatures, and transcutaneously activate shape changes in NiTi implants is a novel approach for eliciting the martensitic thermoelastic transformation. Methods: The purpose of this study was to investigate the features of therapeutic ultrasound that correspond with temperature changes in different NiTi specimens. For this purpose, ultrasound was applied to two NiTi specimens for two minutes each at varying low- and high-frequency and power settings using a Sonicator 740 and a Dynatron 150. Findings: The baseline temperature for all 32 trials was room temperature (23.0 ± 1.7°C). This study successfully increased the specimen temperature with the application of Sonicator 740 and Dynatron 150 therapeutic ultrasound machines (2.2 ± 2.4°C and 1.5 ± 1.15°C, respectively). From the statistical analyses of the experimental data, it was clear that there is a significant difference between low- and high-power settings on mean temperature change using the Dynatron 150 (ANCOVA; p = 0.013). Interpretation Of clinical relevance, NiTi implants can quickly and easily increase in temperature when applying therapeutic ultrasound. Ultrasound power causes temperature changes and should be accounted for when designing orthopedic implants for applications where dimensional changes are desirable.
Phase Transformation and Mechanical Behaviour of Different Heat-Treated Nickel-Titanium Rotary Instruments

Abstract:Objectives: This study evaluated the phase composition, phase transformation behaviour, and mechanical properties of five heat-treated NiTi instruments. Methods: ProTaper NEXT (M-wire, PTN), ProTaper Gold (Gold-wire, PTG), One Curve (C-wire, OC), EdgeTaper Platinum (Fire-wire, ETP), NeoNiTi (electrical discharge machining-wire, NNA), and ProTaper Universal (conventional wire, PTU, control) with #25-tip size were tested (n = 12/group) for cyclic fatigue resistance (number of cycles to failure; NCF) and torsional resistance (angle of rotation to fracture and maximum torque at fracture [ultimate torsional strength]). The geometry and fracture surfaces of the tested instruments were examined by scanning electron microscopy. The phase transformation temperature and phase composition of the instruments were evaluated using differential scanning calorimetry and X-ray diffraction. Data were statistically analysed using one-way ANOVA and Tukey's post hoc test, with the significance level set at 5%. Results: PTG showed the highest NCF (P < .05) at 37°C, while ETP exhibited the highest angle of rotation to fracture, ultimate torsional strength, and stiffness (P < .05). Scanning electron microscopy demonstrated typical clusters of fatigue striations and numerous cracks after cyclic fatigue fracture, whereas there was a concentric abrasion pattern with a dimple and microvoids at the centre after torsional fracture. In differential scanning calorimetry curves, austenite-finishing temperatures of heat-treated instruments were higher than 37°C, whereas that of PTU was lower than 37°C. PTU showed strong peaks of austenite at 25 and 37°C, whereas ETP showed a strong peak of R-phase at 25°C, but mostly austenite phase at 37°C in X-ray diffraction. Conclusions: Geometry, alloy type, and phase transformation temperatures of NiTi instruments affected their mechanical behaviour. Clinical relevance: PTG showed the highest NCF, suitable for markedly curved canals. ETP had the highest torsional resistance, appropriate for narrow and constricted canals.

Abstract:This study aims to compare the cyclic fatigue resistance of nickel–titanium (NiTi) endodontic instruments, focusing on the impact of various alloy treatments and manufacturing processes across different generations of these instruments; Twenty instrumentation systems from different generations, comprising both continuous and reciprocating motion designs, were tested. Four hundred instruments underwent cyclic fatigue testing using an INSTRON machine, with the time and number of cycles to fracture (NCF) recorded for each instrument. Statistical analyses were performed to compare the fatigue resistance between systems, generations, and motion types; Instruments treated with advanced thermal processing, such as Excalibur, Reciproc Blue, and TruNatomy, demonstrated superior resistance to fracture, whereas systems like Protaper Universal, K3XF, and 2Shape showed the lowest resistance. Reciprocating instruments generally exhibited higher cyclic fatigue resistance than continuously rotating instruments; Technological advancements in NiTi instrument design, especially the implementation of heat-treated alloys, have improved cyclic fatigue resistance, enhancing the safety and efficiency of endodontic treatments. Reciprocating systems, in particular, exhibit superior fracture resistance, suggesting their greater utility in challenging clinical conditions.

Abstract:Background: The heat produced during implant site osteotomy can potentially interfere with and influence the osseointegration process of a dental implant. The objective of this in vitro investigation was to measure the temperature changes during simulated osteotomies in bovine rib bone. The measurements were made at the apical area of the osteotomies with steel implant drills compared to zirconia implant drills. Methods: Steel cylindrical drills (2 mm) and zirconia cylindrical drills (2 mm) were evaluated in vitro using bovine rib bone for a total of five groups based on the number of osteotomies performed with each drill: 10, 20, 40, 90, or 120 osteotomies. Bone and apical drill temperatures were measured by means of infrared thermography. The drilling time for each osteotomy was measured for each preparation. Results: Statistically significant differences were found in the temperature measurements in the bone and apical portion of the drills between the study groups (p < 0.05). A statistically significant difference was observed for drilling time preparation between steel cylindrical drill (2 mm) and zirconia cylindrical drills (2 mm) (p < 0.01). Conclusions: The drill material has an impact on the temperature changes that occur at its apical portion during bone preparation for implant placement.

Efficiency and Temperature Rise of File Ablation by Neodymium:Yttrium-Aluminum-Perovskite Laser in Vitro.

Abstract:Introduction: Retrieving a separated file in the root canal system requires a skilled clinician. A metal file can be removed by ablation via the neodymium:yttrium-aluminum-perovskite (Nd:YAP) laser; however, the efficiency and potential hazards of the ablation procedure are still unclear. The aim of this study was 2-fold: to investigate the efficiency of file ablation using the Nd:YAP laser and to evaluate the rise in temperature of the root surface in vitro. Methods: In part 1 of this study, #10 and #15 C-files (VDW GmbH, Munich, Germany) and ProTaper Next X1 and X2 files (Dentsply Maillefer, Ballaigues, Switzerland) were selected. Under 8.5x, the Nd:YAP laser was used to ablate the files starting at the tip of the file. The length of the file was recorded after each 5-pulse ablation. In part 2 of this study, 3 files each of the #15 C-file and the ProTaper Next X2 were separated in 6 distal roots of mandibular second molars with single canals. Two-pulse and 5-pulse modes were used to ablate the separated files, and the rise in root surface temperature was recorded with an infrared camera. Another 3 unaltered distal roots served as the negative control. For scanning electron microscopic evaluation, the teeth were split with bone scissors and observed under a scanning electron microscope to evaluate the surface change of the file and the root canal wall. Results: The average length of the ablated files after each 5-pulse interval was 0.113-0.158 mm with no statistical difference between the different files.The temperature rise of the negative control group (mean +/- standard deviation = 1.86 degrees +/- 0.11 degrees C and 5.81 degrees +/- 0.35 degrees C for the 2- and 5-pulse group) was significantly lower than the roots with separated files (mean +/- standard deviation = 3.74 degrees +/- 0.20 degrees C and 4.02 degrees +/- 0.11 degrees C for 2-pulse C-file and ProTaper Next X2; 8.47 degrees +/- 0.19 degrees C and 9.04 degrees +/- 0.20 degrees C for 5-pulse C-file and ProTaper Next X2). The temperature rise of all groups was lower than 10 degrees C. The 2-pulse group showed a lower temperature rise than the 5-pulse group. The diameter or the type of alloy had no effect on the temperature rise. Conclusions: Separated files (stainless steel or nickel-titanium alloy) can be ablated by the Nd:YAP laser. When the number of pulses is less than 5, the temperature rise of the root surface is considered safe to the surrounding periodontium.

Abstract:Bone removal is commonly used in the extraction of third molars and the heat generated during the process can interfere with bone repair. The aim of this study was to evaluate the temperature variation presented in bone removal performed with a high speed turbine (400,000rpm), implant motor with straight piece (100,000rpm), low speed micromotor (20,000rpm) and piezoelectric saw (30kHz) in pig mandibles. For this, bone removal was carried out around 20 posterior teeth, under constant saline solution irrigation with syringe and needle. In addition, the time required to perform bone removal was recorded. The results indicated a mean temperature variation and standard deviation of 0,96°C (0,6°C) for the high speed turbine, 1,38°C (0,5°C) with the implant motor, 2,22°C (0,7°C) for the low speed micromotor and 2,90°C (1,3°C) for the piezoeletric. The conventional variance was calculated discounting the variation of time used for bone removal around the teeth. There was a statistically significant difference in temperature variation between the high speed turbine vs micromotor (p=0,00) and the high speed vs piezoeletric (p=0,04). We conclude that there is a statistically significant difference in temperature variation between the instruments used in oral and maxillofacial surgery, with higher rotation speeds resulting in lowest temperature variations and a reduced surgical time.

VSL#3 Probiotic-Mixture Induces Remission in Patients with Active Ulcerative Colitis

Abstract:BACKGROUND AND AIMS:Intestinal bacteria have been implicated in the initiation and perpetuation of IBD; in contrast, “probiotic bacteria” have properties possibly effective in treating and preventing relapse of IBD. We evaluated the safety and efficacy of VSL#3 and the components, and the composition of the biopsy-associated microbiota in patients with active mild to moderate ulcerative colitis (UC).METHODS:Thirty-four ambulatory patients with active UC received open label VSL#3, 3,600 billion bacteria daily in two divided doses for 6 wk. The presence of biopsy-associated bacteria was detected using a nucleic acid-based method and the presence of VSL#3 species confirmed by DNA sequencing of 16S rRNA.RESULTS:Thirty-two patients completed 6 wk of VSL#3 treatment and 2 patients did not have the final endoscopic assessment. Intent to treat analysis demonstrated remission (UCDAI ≤ 2) in 53% (n = 18); response (decrease in UCDAI ≥ 3, but final score ≥3) in 24% (n = 8); no response in 9% (n = 3); worsening in 9% (n = 3); and failure to complete the final sigmoidoscopy assessment in 5% (n = 2). There were no biochemical or clinical adverse events related to VSL#3. Two of the components of VSL#3 were detected by PCR/DGGE in biopsies collected from 3 patients in remission.CONCLUSION:Treatment of patients with mild to moderate UC, not responding to conventional therapy, with VSL#3 resulted in a combined induction of remission/response rate of 77% with no adverse events. At least some of the bacterial species incorporated in the probiotic product reached the target site in amounts that could be detected.
The accuracy of using guided endodontics in access cavity preparation and the temperature changes of root surface: An in vitro study

Abstract:Background: Guided endodontics is a successful technique that has been gradually applied to endodontic therapy in recent years without being affected by the operator's experience. However, the guided bur produces excessive heat during continuous rotation and friction with root canal walls, it is not clear whether the degree of temperature increase may lead to the periodontal ligament and alveolar bone damage. Methods: A total of 58 teeth were used, of which 40 teeth were not grouped, all used to evaluate the accuracy. 40 single-rooted premolars were scanned using CBCT and an intra-oral scanner, and 3D-printed guided plates were made with the pre-designed access. A custom-made guided bur was used to prepare the access cavities. The postoperative CBCT data and pre-designed pathways were matched to evaluate the deviation between the planned and virtual paths. The other 18 teeth were randomly divided into three groups (ET20 and ProTaper F3 as the control group, guided endodontics as the test group), with 6 teeth in each group. The temperature changes on the root surfaces were inspected with a thermocouple thermometer. Results: The average deviation on the tip and the base of the bur was 0.30 mm and 0.28 mm (mesial/distal), and 0.28 mm and 0.25 mm (buccal/lingual). The average angle deviation was 3.62°. The mean root surface temperature rise of the guided endodontics group was the lowest (5.07 °C) (P < 0.05). Conclusions: The access cavity preparation performed with guided endodontics has feasible accuracy and low-temperature rise on the root surfaces. Due to the limitations of the study, whether it has high reliability and safety in clinical applications needs to be further studied in vivo.
Intra-pulpal and subsurface temperature rise during tooth irradiation with 808 nm diode laser: an in vitro study

Abstract:Aim: This in vitro study evaluated the pulpal and subsurface temperatures during proximal tooth surface irradiation with different settings of an 808 nm diode laser. Materials and methods: The elevation of pulpal and subsurface temperature during irradiation was measured using thermocouples positioned in the centre of the pulp chamber (n=30) and in the centre of a prepared 1.5 mm deep coronal cavity (n=30). Each sample was irradiated 3 times within one-week interval using different exposure settings. A temperature increase of 3.5°C was regarded as critical value for pulpal heath. Results were analysed with one-way ANOVA and Duncan post hoc tests. Micromorphological investigation by digital microscopy was carried out for the irradiated and non-irradiated tooth surface. RESULTS Measurable temperature increase within the pulp chamber (delta PT) and the subsurface (delta ST) were observed with all laser settings tested. The highest recorded delta PT and delta ST were 3.1°C and 8.5°C, respectively. Delivery mode, beam diameter, and exposure time influenced the temperature rise. No alterations on the enamel surface were observed when inspected by the digital microscope after undergoing irradiation with the tested parameters. Cconclusion: From the thermal point of view, under the conditions of the present study the application of an 808 nm diode laser on the outer surface of the tooth at 1W in the continuous mode and at 5W in the pulsed mode for two cycles of 30 s each proved to be safe.

[Heat propagation in dentin during cavity preparation in vitro with oscillating instruments]

Abstract:The purpose of this work was to study the temperature rise caused by preparation of human dentin using two different types of oscillating instruments. A newly designed tungsten carbide tip (Cariex TC, KaVo) and a diamond-coated tip (Cariex D, KaVo) in combination with an airscaler (Sonicflex 2003L, KaVo) and two different flow rates of two coolants were investigated. For both tips significant differences in temperature rise were found between cooling with waterspray or with Plakout Gel (Kerr, HaWe) and using no coolant. A flow of 7.3 ml water/min was found to be sufficient to avoid any critical temperature rise, an excavation of longer duration without coolant cannot be recommended. The use of water-based highly viscous substances over a defined period during the caries excavation process seems to be a viable alternative.
