Abstract:Changes in tooth mobility after surgical crown lengthening procedures were examined with a modern electro-mechanical instrument (Periotest) in a total of 37 teeth in 14 patients between the ages of 18 and 55 years. Seven teeth were incisors, three canines, thirteen pre-molars and fourteen were molars. Periotest values, probing depths, bleeding indices and plaque indices were recorded after the initial preparation stage of the periodontal treatment, and further Periotest values and plaque and bleeding indices were recorded at intervals up to 3 months after surgery. In each category of teeth there was a progressive increase in tooth mobility starting immediately after surgery and peaking about the seventh day. The mobility then declined steadily to approach the base line again at about 3 months. There were statistically significant differences in mobility after surgery between tooth types (p < 0.01). Gender and age did not significantly affect the values. The pattern of post-surgical tooth mobility can be closely correlated with the known biological events of post-surgical healing.

Abstract:OBJECTIVE: The use of periodontal probes harbors the risk of measurement errors. The aim of this study was to investigate the accuracy of the digital determination of gingival recession and papilla height on virtual jaw models, given the hypothesis that they show lower intrarater and interrater variability than conventional linear measurements taken clinically or on cast models.MATERIALS AND METHODS: Gingival recessions and papilla heights were measured at 30 sites by five examiners using the following methods: (A) direct measurements using a periodontal probe intraorally or (B) on cast models using a caliper, (C) digital measurements on virtual models obtained by optical scans taken intraorally, or (D) made of cast models using the same software. Measurements were repeated after 1 week and intraclass, intrarater, and interrater correlations of the measurements using the four different methods were analyzed.RESULTS: The greatest disagreement between the 1st and 2nd measurement was identified for method A. Recessions were less reliably measured than papillae. The best agreement between methods was found in the digital ones (C and D). Regarding papilla height, increased values were obtained when method D was applied as compared with both clinical evaluations. For gingival recession, method A measured the highest values.CONCLUSION: In the present study, the use of digital technologies by intraoral scanning or scanning of cast models improved the reproducibility and lowered the variance of measurements within one individual and between different investigators.CLINICAL SIGNIFICANCE: Providing a more reliable and examiner-independent technique for measurements of the soft tissue architecture could improve data quality in periodontal research and in the analysis of different clinical treatment modalities.

An In Vitro Evaluation of Periotest Implant Stability Measurements Taken on Implant Retained Crowns and Healing Abutments

Abstract:Objective: To assess the reliability of implant stability measurements recorded with the Periotest device and to investigate the differences in values when these measurements were taken on implant retained crowns and healing abutments. Materials and methods: Fifty-six implants in eight synthetic bone blocks were used to carry out implant stability measurements using the Periotest device by two different operators. Each block constituted an example of bone of density D1, D2, D3, or D4, and two blocks of each density were used. The healing abutments placed were of a height to allow approximately 6 mm of the implant-abutment complex to be supracrestal and temporary crowns were made to match the dimensions of an average central incisor. Descriptive statistics were used to describe the perio test values (PTVs) at each of the different heights on the implant abutments and implant crowns. Means for each site were calculated and distribution of data assessed using the Kruskal Wallis test. The interclass correlation coefficient (ICC) was used to determine the relationship between the PTVs recorded on the implant abutments and implant crowns. Results: The mean PTV (±standard devidation) recorded across all sites was 5.57 ± 11.643 on the implant abutments, and 12.27 ± 11.735 on the temporary crowns. Excellent/good inter-operator ICCs were recorded for the mid-abutment site in all bone blocks D1-D4 (ICC = 0.814, p < 0.001, ICC = 0.922, p < 0.001, ICC = 0.938, p < 0.001, ICC = 776, p < 0.001). For mid crown sites, ICC between operators was excellent/good only for recordings in D2 bone (ICC = 0.897, p < 0.001). Conclusions: Periotest device seems to be able to reliably measure implant stability across all types of bones when the implant stability is assessed at approximately 3 mm coronal to the implant platform for abutments and 4.5 mm for implant supported single crowns.

Abstract:Background: Despite clinical interventional studies on the influence of diet on periodontal inflammatory parameters, there has been no practice-based cross-sectional study from a German population to date that has conducted both a comprehensive dental and periodontal examination and a thorough validated assessment of dietary behavior. Therefore, the aim of this pilot study was to evaluate, in a proof of concept, whether there is a correlation between the overall periodontal inflammatory surface area (PISA), periodontal clinical parameters (pocket probing depths (PPD), clinical attachment loss (CAL), bleeding on probing (BOP), furcation involvement (FI), tooth mobility (TM)), and the dietary behavior of patients with periodontal disease when utilizing a practice-based research network. The primary outcome was the correlation between the periodontal inflammatory surface (PISA) and the dietary assessment data. Materials and Methods: The practice-based research network, consisting of eight Master's graduates, recruited patients who met the inclusion and exclusion criteria and performed a periodontal examination together with the assessment of dietary behavior using a digital version of the validated retrospective dietary recall (DEGS/RKI). Statistical analyses included linear regression models adjusted for age and smoking and unpaired t-tests, conducted using STATA 17.0 with a significance level of 5%. In addition, the data obtained were classified according to the currently recommended amounts of daily intake. Results: A total of 1283 teeth were analyzed, with 60.25% (773 teeth) requiring treatment. The average PISA was 753.16 mm2 (SD ± 535.75 mm). Based on dietary guidelines, the studied population consumed excessive amounts of extrinsic sugars and fats, while their fiber and legume intake was insufficient. The intake of certain nutrients, including water-soluble fibers, specific fatty acids, vitamins (D, B1, B2, B6, and B12), iron, and zinc, was associated with reduced PISA, PPD, CAL, and BOP. Conclusion: Within the limits of the current study, including its cross-sectional design and cohort size, the outcomes demonstrated the influence of nutrition on periodontal health.

Abstract:Background/purpose: Interproximal contact loss may lead to food impaction and result in subsequently periodontal complications. The purpose of this prospective study was to investigate the peri-implant parameters of posterior implant-supported single crowns (SCs) with and without mesial proximal contact loss after 2 years of follow-up. Material and methods: Twenty-six patients with a total of 40 posterior implant-supported SCs with mesial adjacent natural teeth were observed for 24 months after crown insertion. The mesial proximal contacts were assessed by dental floss, then were classified as tight, weak, and open contacts. The following peri-implant parameters were evaluated, including modified plaque index (MPI), modified gingival index (MGI), and probing depth (PD) were conducted at six sites per tooth (mesiofacail, midfacial, distofacial, mesiolingual, mid-lingual and distolingual) in the 6-, 12-, 18- and 24-month following visits. Furthermore, radiographs were taken regularly in 12- and 24-month recall sections for measuring the marginal bone loss (MBL). Results: At 12-month observation, the incidence rates of weak and open contacts were 22.5 % and 12.5 %; whereas after 24 months of clinical service, the rates came up with 12.9 % and 25.6 %, respectively. No significant differences were found between the tight, weak, and open contact groups in the parameters of MPI, MGI, or PD (P > 0.05) at 12- and 24-month follow-up. None of the mean differences of the peri-implant parameters: MPI, MGI, PD and MBL had significant differences between the tight, weak, and open contact groups after 1 and 2 years of clinical service (P > 0.05). Conclusion: The presence of open, weak, and tight mesial proximal contacts had no significant effects on the peri-implant tissue conditions.

Abstract:Background: The aim of this clinical study was to compare the occlusal caries detection (OCD) performance of the intraoral scanners (IOSs) Trios 4 (TIO, 3Shape) and Emerald S (EME, Planmeca) and the Diagnocam (DIA, KaVo) with the established visual (WHO) examination (VIS, reference method). Methods: Between 08/2022 and 02/2023, 60 children (mean age 9.6 ± 2.5 years) were examined as part of their regular dental checkups. OCD was performed at the tooth level, separately for primary and permanent unrestored teeth. Furthermore, two thresholds were analyzed: sound versus overall caries (pooled data of enamel and dentin caries, TH1) and pooled data of sound and enamel caries versus dentin caries (TH2). Results: The best agreement with the reference method (reliability) in both dentitions was obtained for DIA (ĸ = 0.829/ĸ = 0.846; primary/permanent teeth), followed by EME (ĸ = 0.827/ĸ = 0.837) and TIO (ĸ = 0.714/ĸ = 0.680). Similar results were shown for the diagnostic quality (sensitivity, specificity and area under the curve of the receiver operating characteristic curve), with higher values for TH1 than for TH2. Both IOSs and the DIA showed worse results than the reference method VIS. Conclusions: Currently, IOS should be used as an additional caries detection tool, especially for visualization, and cannot be recommended as a basic tool for diagnosis or invasive/noninvasive therapy decisions in OCD.

[Evaluation of periodontal health of prosthetic abutments. Pilot study about 100 abutments of crowns and bridges]

Abstract:Background: The confection of a fixed prosthesis finds its indication in response to the patient's aesthetic and/or functional whose requirements are stronger in terms of satisfaction. This involves maintaining the stability of prosthetic reconstructions on a sufficiency long time. To achieve this, the preservation of periodontal health of the prosthetic abutments occupies a special choice. Purpose: The aim of this study was to evaluate the periodontal health of fixed prosthesis abutment. Materials and methods: Thirty-nine patients having fixed prosthesis in the mouth for more than one year participated in our study. Ultimately, 38 single crowns and 27 seven bridges based on 100 abutments have been recorded. After an interview, clinical and radiographic examination was performed. It was to assess the level of oral hygiene, the health of the superficial and deep periodontal abutment using OHIS index of Green and Vermilion, periodontal index of Russel and mobility index of Mühlemann. Results: On the 100 abutments examined, gingivitis (14%), gingival recession (13%), bone osteolysis (29%) and mobility (5%) were recorded. In addition, about 60% of patients had inadequate oral hygiene and less than 30% of them had a regular prosthetic maintenance. Conclusion: From this study, it appears that complications affecting periodontal prosthetic abutments are essentially bone lysed (29%), gingivitis (14%) and gingival recessions (13%).

Abstract:Electrical Properties Tomography (EPT) technique utilizes measurable radio frequency (RF) coil induced magnetic fields (B1 fields) in a Magnetic Resonance Imaging (MRI) system to quantitatively reconstruct the local electrical properties (EP) of biological tissues. Information derived from the same data set, e.g., complex numbers of B1 distribution towards electric field calculation, can be used to estimate, on a subject-specific basis, local Specific Absorption Rate (SAR). SAR plays a significant role in RF pulse design for high-field MRI applications, where maximum local tissue heating remains one of the most constraining limits. The purpose of the present work is to investigate the feasibility of such B1-based local SAR estimation, expanding on previously proposed EPT approaches. To this end, B1 calibration was obtained in a gelatin phantom at 7 T with a multi-channel transmit coil, under a particular multi-channel B1-shim setting (B1-shim I). Using this unique set of B1 calibration, local SAR distribution was subsequently predicted for B1-shim I, as well as for another B1-shim setting (B1-shim II), considering a specific set of parameter for a heating MRI protocol consisting of RF pulses plaid at 1% duty cycle. Local SAR results, which could not be directly measured with MRI, were subsequently converted into temperature change which in turn were validated against temperature changes measured by MRI Thermometry based on the proton chemical shift.

[Coronal leakage reconsidered]

Abstract:The aim of the present study was to evaluate the impact of the quality of the coronal restoration and of the root filling on periapical health. In a sample of 937 root filled teeth which had not received restorative treatment in the previous year, the technical quality of the coronal restorations and of the root fillings was radiographically scored; in addition the technical quality of the coronal restorations was clinically scored as well. The data showed that the technical standard of both coronal restoration and root filling were essential to periapical health. The study also showed the relatively limited value of epidemiologic findings. Epidemiologic research is descriptive as well as analytical in approach. According to the present data, endodontic failure cannot be fully explained by radiographic analysis only. Nevertheless, epidemiologic data remain a valuable component to the source of knowledge regarding etiologic factors and proper treatment procedures in order to improve the results of endodontic practice.

Abstract:OBJECTIVE Long non-coding RNA CRNDE (CRNDE) recently emerged as a carcinogenic promoter in various cancers including gastric cancer (GC). However, the functions and molecular mechanisms of CRNDE to GC are still largely unclear. The aim of this study was to investigate the clinical significance and functional mechanisms of CRNDE expression in GC. PATIENTS AND METHODS The expression of CRNDE was detected by quantitative Real-time PCR (qRT-PCR) in GC specimens and cell lines. The correlation between the CRNDE expression and clinicopathological parameters was investigated. Survival rate was determined with Kaplan-Meier and statistically analyzed with the log-rank method between groups. Subsequently, the significance of survival variables was analyzed using the Cox multivariate proportional hazards model. Then, MTT and Transwell assays were used to assess cell proliferation, migration and invasion capacity. Finally, Western blot analysis was performed to explore the effects of CRNDE knockdown on the PI3K/Akt pathway. RESULTS We observed that expression of CRNDE was higher in GC tissues and cells compared with the normal gastric tissue and normal gastric cell lines. High expression of CRNDE was correlated with invasion depth (p = 0.006), TNM stage (p = 0.010) and lymph node metastasis (p = 0.005). Furthermore, high CRNDE expression was associated with shorter overall survival (p = 0.0066) of GC patients. Multivariate analysis confirmed that high CRNDE expression was a significant independent predictor of poor survival in GC. In vitro assay indicated that knockdown of CRNDE inhibited cell proliferation, migration and invasion of GC. Finally, the data of Western blot showed that CRNDE exerted its oncogenic role by affecting PI3K/AKT signaling pathways. CONCLUSIONS Our findings indicate that CRNDE plays an important role in promoting GC progression and may represent a novel prognostic biomarker in GC.

Abstract:Objective:To evaluate the prevalence of peri-implant disease and identify the risk indicators for periimplantitis in Chinese population.Methods:One thousand six hundred and twelve implants in 736 patients were enrolled which were placed and restored in the second dental center of Peking University School and Hospital of Stomatology,mean loading period (22.64±0.92) months.The periodontal parameters including plaque index (PLI),probing depth (PD),bleeding index (BI),keratinized gingiva width (KGW),residual cement,and bone resorption were recorded.Full mouth periodontal examinations were investigated including PD,BI,and missing teeth.The diagnosis of peri-implantitis and peri-implant mucositis were according to the definition in the consensus report of the Seventh European Workshop on Periodontology.Periodontal status around implants was compared among different implant systems and different loading periods.The associations between PLI,PD,BI,KGW,cement around implant,periodontal status of residual teeth,missing teeth,and peri-implantitis were analyzed by logistic regres sion.Results:Peri-implant mucositis (bleeding on probing (BOP) and no bone loss) occurred in 81.90％ of the subjects and 83.60％ of the implants.Peri-implantitis (BOP and bone loss) occurred in 4.50％ of the subjects and 3.70 ％ of the implants.The prevalence of peri-implantitis showed no significant difference among Straumann,Bicon,3i and Nobel Replace system.The prevalence of peri implantitis in group loading 0.5-1 year and 5 7.5years were significant lower than those in groups loading1 2,2 3,3-4,and 4-5years (P＜0.05).In the multivariate analysis,missing teeth,mean PD round implant,and mean BI round implant were significant risk indicators for peri-implantitis,after adjusting gender,age,smoking,residual cement,and KGW.Conclusion:The prevelence of peri-implant mucositis was high,the prevelence of peri-implantitis was not increased according to the extension of loading period.It is important to control the probing depth and bleeding on probing around implant to avoid peri-implantitis.

Abstract:Objective: To compare the clinical effects of periodontal treatment using periodontal endoscope with that of conventional treatment method for patients with periodontitis. Methods: Eleven periodontal patients with moderate to advanced periodontal destruction were recruited and treated with scaling and root planing in a split-mouth design randomly with (test group) or without (control group) periodontal endoscope. Changes of such clinical parameters as plaque index (PLI), bleeding index (BI), probing depth (PD) and attachment loss (AL) before and after treatments as well as the difference between test and control groups were examined and compared. Results: Both groups showed significant reductions in PD, BI and AL values at 6 weeks and 3 months after treatment (P <0.001). Only in the test group, for sites with PD≥6 mm at baseline, PD value ([4.0±1.2] mm) and AL value ([3.8±0.9] mm) at the end of 3 months were significantly lower than that at the end of 6 weeks (PD[4.4±1.3] mm, P<0.001; AL[4.1±1.1] mm, P< 0.05). There were no significant differences between two groups at the baseline, at the end of 6 weeks and 3 months, respectively. However, for the sites with PD≥6 mm in anterior teeth, PD value in test group at the end of 3 months was significantly lower than that in control group ([3.2±0.9] mm vs [3.7±0.9] mm, P <0.05), while AL value tend to be lower ([2.9 ± 1.2] mm vs [3.6 ± 1.3] mm, P=0.061). Conclusions: Periodontal treatment using endoscope is obviously effective, especially for the medium and long term prognosis of deep pockets and single rooted teeth.

Abstract:Objective To obtain the epidemiological information on disease status in untreated chronic periodontitis patients.Methods Two hundred untreated chronic periodontitis patients were selected from employed adults in an institute located in urban area. They were aged from 22 to 64. Periodontal measurements were performed on all teeth except the third molars, 6 sites for each tooth. Probing depth, attachment loss, and bleeding on probing were measured.Results Mean attachment loss in this population was 1.87mm. The severity of attachment loss increased with patients' age. It was more severe in male than in female, and more severe in bleeding sites than in non-bleeding sites. The most severe attachment loss occurred in mandibular incisors and the maxillary first molars. Maxillary canines were the least in AL severity.Conclusion The severity of chronic periodontitis was associated with patient age, gender, gingival inflammation, and manifested site-specificity.
