Abstract:Hands-on courses utilizing preserved human tissues for educational training offer an important pathway to acquire basic anatomical knowledge. Owing to the reevaluation of formaldehyde limits by the European Commission, a joint approach was chosen by the German-speaking anatomies in Europe (Germany, Austria, Switzerland) to find commonalities among embalming protocols and infrastructure. A survey comprising 537 items was circulated to all anatomies in German-speaking Europe. Clusters were established for "ethanol"-, formaldehyde-based ("FA"), and "other" embalming procedures, depending on the chemicals considered the most relevant for each protocol. The logistical framework, volumes of chemicals, and infrastructure were found to be highly diverse between the groups and protocols. Formaldehyde quantities deployed per annum were three-fold higher in the "FA" (223 L/a) compared to the "ethanol" (71.0 L/a) group, but not for "other" (97.8 L/a), though the volumes injected per body were similar. "FA" was strongly related to table-borne air ventilation and total fixative volumes ≤1000 L. "Ethanol" was strongly related to total fixative volumes >1000 L, ceiling- and floor-borne air ventilation, and explosion-proof facilities. Air ventilation was found to be installed symmetrically in the mortuary and dissection facilities. Certain predictors exist for the interplay between the embalming used in a given infrastructure and technical measures. The here-established cluster analysis may serve as decision supportive tool when considering altering embalming protocols or establishing joint protocols between institutions, following a best practice approach to cater toward best-suited tissue characteristics for educational purposes, while simultaneously addressing future demands on exposure limits.

Abstract:The relationship between a static measure of dorsal first ray mobility and dynamic motion of the first ray, midfoot, and hindfoot during the stance phase of walking was investigated in healthy, asymptomatic subjects who represented the spectrum of static flexibility. Static first ray mobility of 15 subjects was measured by a load cell device and ranged from stiff (3.1 mm) to lax (8.0 mm). Using three-dimensional motion analysis, mean first ray dorsiflexion/eversion and mid-/hindfoot eversion peak motion, time-to-peak, and eversion excursion were evaluated. Subjects with greater static dorsal mobility of the first ray demonstrated significantly greater time-to-peak hindfoot eversion and eversion excursion (p >.01), and midfoot peak eversion and eversion excursion (p > .01). No significant association was found between static first ray mobility and first ray motion during gait. This research provides evidence that the dynamic response of the foot may modulate the consequences of first ray mobility and that compensory strategies are most effective when static measures of dorsal mobility are most extreme.

Abstract:Mold growth on body donations remains an underreported yet serious issue in anatomical teaching. Bacterial and fungal growth pose health risks to lecturers and students, alongside with ethical and aesthetic concerns. However, limited information exists on the presence of bacteria and fungi on body donations and their underlying causes. To investigate the potential impact of airborne germs on body donation contamination, we conducted indoor air measurements before, during, and after our anatomical dissection course, with outdoor measurements serving as a control. Tissue samples from the dissected body donations were collected to assess the germ load, with qualitative and quantitative microbiological analyses. Air samples from the dissection hall contained no fungi, but various fungal species were identified in the adjacent stairways and outdoors which implies that fungal occurrence in the dissection hall air was independent of lecturers' and students' presence. Moreover, our results indicate that adequate ventilation filters can effectively reduce indoor fungal germs during courses, while the bacterial load in room air appears to increase, likely due to the presence of lecturers and students. Additionally, the tissue samples revealed no bacterial or fungal germs which implies that our ethanol-formalin-based embalming solution demonstrates an effective long-term antimicrobial preservation of corpses.

Abstract:Practitioners and educators continually evaluate alternative teaching methodologies to increase the learning of anatomical and clinical based skills. Traditionally, anatomy and clinical skills are taught separately to students in both physical and occupational therapy curricula. Formaldehyde preservation is seen as the primary embalming methodology within anatomical education but has been described as lacking when utilized within educational institutions. This study aimed to evaluate the integration of clinical skills into anatomical education using an alternative preservation technique called Imperial College London‐ Soft Preservation (ICL‐SP). Physical and occupational therapy completing the Musculoskeletal Anatomy course were presented with donors preserved with either formaldehyde or ICP‐SP. The students were tasked with traditional dissection and with performing clinical tests on the different donors followed by the administration of a questionnaire. The inclusion of ICL‐SP donors showed there was a better model for the representation of the vascular system and the nervous system without showing significant deficits in any other categories when compared to formaldehyde. The integration of clinical skills and anatomical curriculum presented increased student perceived understanding of associated anatomy (p < 0.05), gave better understanding of how anatomy is important to their professions (p < 0.05) and increased motivation to learn anatomy (p < 0.05). Students noted ICL‐SP was more beneficial forclinical skill procedures (Lachman's test: p < 0.05; Valgus Stress Test of the Elbow: p < 0.05). In conclusion, the integration of clinical skills into anatomical education can potentially provide clinicians with higher confidence performing clinical skills and may be complemented by the utilization of the new ICL‐SP methodology. Ethical approval was granted by the Ohio State University Institutional Review Board under log number 2021E0469.

Abstract:Art of embalming as practised by Egyptian about 3000 years ago transformed into embalming science of modern ages with the use of formaldehyde as a preservative solution. Subsequently, the search for ideal embalming preservative solution continues to date because of the health hazards related to formaldehyde preservation of cadavers. Alternative preservative methods and solutions suitable for making different skill training models with the specific requirement of pliability have also experimented. The literature had documented various solutions like Thiel’s solution and technique, phenoxyethanol preservation, saturated sodium chloride solution, cryopreservation, N-Vinyl-2-pyrrolidone, Ethanol–glycerin and Fix 4life solution as alternatives to formaldehyde preservation. This review is an attempt to have an overview comparison of all the recent alternate embalming methods applicable for developing skill training cadaveric models with an aim of reducing formaldehyde usage in preservation.
A comparative study of vascular injection fluids in fresh-frozen and embalmed human cadaver forearms

Abstract:Over the years, various vascular injection products have been developed to facilitate anatomical dissections. This study aimed to compare the most commonly used vascular injection products in fresh-frozen and formalin-embalmed cadaver specimens. An overview of the properties, advantages and limitations of each substance was given, and a comparison of vascular infusion procedures in both preservation methods was made. A literature search was performed in order to identify the most commonly used vascular injection products. Acrylic paint, latex, gelatin, silicone, Araldite F and Batson's No. 17 were selected for the study. One fresh-frozen and one embalmed cadaver forearm were infused with each injection product according to a uniform protocol. The curing time, skin- and subcutaneous tissue penetration, degree of filling of the arterial tree, extravasations, consistency of the injected vessels during dissection, and the costs of each injection fluid were noted. There was a large variation between the injection fluids in processing- and curing time, colour intensity, flexibility, fragility, elasticity, strength, toxicity and costs. All fluids were suitable for infusion. The penetration of injection fluid into the skin and subcutaneous tissue was significantly better in fresh-frozen specimens (P = 0.002 and P = 0.009, respectively), with significantly smaller branches casted (P = 0.004). Vascular infusion of fresh-frozen cadaver specimens results in a significantly better filled coloured arterial tree, enabling more detail to be achieved and smaller branches casted. The biomechanical properties of fresh-frozen soft tissues are less affected compared with formalin fixation. All the injection fluids studied are suitable for vascular infusion, but their different properties ensure that certain products and procedures are more suitable for specific study purposes.
Are spinal cord and medulla samples from embalmed donors suitable for histological examination? A pilot study

Abstract:It is generally believed that the fixation used for embalming donor tissue routinely allocated to anatomical teaching, is insufficient to preserve tissue architecture at the cellular level and, therefore, samples obtained from anatomical donors may not be suitable for microscopic investigation. This is particularly relevant for the Central Nervous System (CNS). However, if this was not the case, the biomedical knowledge currently gained from anatomical donations could greatly be expanded by in‐depth investigations on the cellular structures of the human CNS. In this study, we addressed this question and examined histologically spinal cord and medulla samples obtained from donors from the Anatomical Gift Programme of RCSI in Dublin. Our analysis revealed that several features were easily identifiable with good histological details. Therefore, spinal cord and medulla samples from embalmed donors can be suitable for histological examination. These findings open new avenues of study of the structure of the human CNS, thus expanding considerably the impact of the generosity of anatomical donors for the advancement of biomedical knowledge. A general belief exists that tissue from anatomical donors, especially from the central nervous system (CNS), may not be suitable for histological investigation. This is based on the idea that fixation routinely used in embalming whilst optimal for enabling dissection, insufficiently preserves tissue architecture at the cellular level. However, anatomical donors represent a precious resource for microscopical investigation, provided that preservation is sufficient to enable recognition of structures at the histological level. This could considerably extend the biomedical knowledge currently gained from donations. In this study, we addressed this question and examined histologically, samples of medulla and spinal cord from donors from the Anatomical Gift Programme of RCSI in Dublin. Samples of medulla and spinal cord in the cervical, thoracic and lumbar regions were obtained with appropriate permissions from four different donors that had been previously embalmed for routine anatomical examination. The tissue obtained was processed for paraffin embedding and routine histology. Analysis revealed that several features were identifiable with good histological detail. A scoring system was applied to evaluate the level of histological preservation in the various samples by assessing the following parameters: tissue integrity, presence of leptomeninges along the entire section surface, distinction/adherence grey/white matter, identification of neurons, other cell nuclei, neuropil pattern, capillaries and other intraneural vessels. Lumbar spinal cord and to a lesser extent medulla and cervical spinal cord showed on average good quality of preservation. Conversely, thoracic spinal cord tended, in general, to be less well preserved. We can conclude that spinal cord and medulla samples from embalmed donors can be suitable for histological examination. These findings open new avenues of study of spinal cord and brain stem anatomy on cadaveric tissue from donors already existing in anatomy donation programmes. These results also pave the way for the potential development of new human models of study useful to spinal cord and brain stem researchers. Finally, this expands considerably the impact of the generosity of anatomical donors for the advancement of biomedical knowledge.

Toxicological Analysis of Drugs in Human Mummified Bodies and Proposed Guidelines

Abstract:From palaeopathology to forensic taphonomy, mummified human bodies constitute biological archives of paramount importance. Toxicology analysis of endobiotics and xenobiotics has already shown value to archaeological mummies research with detecting heavy metals, sedative-hypnotic drugs, and stimulants. Thanks to the large window of drug detection in hair and nails, the information from such studies has increased the scientific community's knowledge regarding past populations' lifestyles. Still, few bibliographic references exist regarding toxicology reports in mummified bodies from forensic settings. Here, the authors aim to draw attention to the valuable contribution of toxicology analysis, taking into account previously conducted studies and their findings. Given that sample collection on mummified bodies from forensic scenarios may not always happen in laboratories or autopsy rooms, the authors also suggest guidelines for in situ sampling of forensic mummies. It is expected that the present technical note will encourage experts to perform toxicology analysis in mummified bodies and publish their case reports more often.
Critical appraisal of Genelyn soft embalming for cadaveric surgical skill training: A systematic review

Abstract:Genelyn is a proprietary embalming solution used for preserving cadavers for surgical skill training (SST) and undergraduate teaching. The aim of this review is to examine the Genelyn embalming method critically by analyzing scientific publications that have employed this method for SST and undergraduate education. The systematic review process involved searching for all relevant articles in PubMed, Google Scholar, and Embase using the keywords "Genelyn," Genelyn embalming," "Genelyn embalmed cadaver," and "surgical skill training. All studies on Genelyn embalming published up to August 14, 2024, in the English language were eligible for inclusion. The literature review yielded 92 studies, 43 of which met the inclusion criteria. The Anatomical Quality Assurance (AQUA) tool from the International Evidence‐Based Anatomy (iEBA) working group was used to assess and analyze the risk of bias in all the selected full‐text articles. The selected studies reported that Genelyn embalming provided life‐like joint range of motion, tissue pliability, and color. Cadaveric studies assessing embalming solutions typically had smaller sample sizes, and few of them compared Genelyn embalmed cadavers (GECs) with the other soft embalming solutions. Unlike Theil's embalming, Genelyn embalming is cost‐effective, with easy steps for the preparation and storage of the solution and convenient handling of cadavers. Therefore, GECs serve as an efficient tool for SSTs.

Abstract:Most medical curricula rely on human bodies for teaching macroscopic anatomy. Over the past 20 years, plastination has become an important means of preservation of organs, for well dissected specimens or for body slices. Here, several critical points regarding body donation with legal and ethical considerations for long-term preservation, the use of cadavers in teaching and the preparation of plastinates as an additional teaching tool will be discussed. Silicone S10 is the gold standard in the preparation of plastinates. An important point to respect is the preparation of specimens, since only very well dissected body parts or excellent tissue sections should be plastinated to show the extraordinary aspects of the human anatomy. The preparation of thin and transparent sections and preservation with P40 polyester provides an additional technique to prepare resistant body slices. A selection of samples prepared by S10 and P40 are shown and compared. In addition, Prussian or Berlin blue staining of brain slices is shown to discriminate better between gray and white matter and demonstrate neuroanatomical structures. These plastinates have been used for many years in teaching first- and second-year medical students and have not lost their appeal. Students and staff appreciate the use of such plastinates. One of the advantages is that their use is not restricted to the dissection hall; slices and body parts can be used in any lecture room or in small group teaching. Therefore, ethical and legal questions need to be addressed regarding their specific use. Plastinates do not replace the traditional dissection courses, since students learn best the anatomical features of a given region by hands-on dissection and by exploratory anatomy. Furthermore, plastinates are more rigid and do not allow demonstration of hidden structures; they also become more cumbersome for endoscopy or are too rigid for demonstrating mechanical features of joints. However, although not a replacement for traditional dissections, plastination provides an additional tool for long-term preservation and for teaching human anatomy.
The production of anatomical teaching resources using three-dimensional (3D) printing technology

Abstract:The teaching of anatomy has consistently been the subject of societal controversy, especially in the context of employing cadaveric materials in professional medical and allied health professional training. The reduction in dissection-based teaching in medical and allied health professional training programs has been in part due to the financial considerations involved in maintaining bequest programs, accessing human cadavers and concerns with health and safety considerations for students and staff exposed to formalin-containing embalming fluids. This report details how additive manufacturing or three-dimensional (3D) printing allows the creation of reproductions of prosected human cadaver and other anatomical specimens that obviates many of the above issues. These 3D prints are high resolution, accurate color reproductions of prosections based on data acquired by surface scanning or CT imaging. The application of 3D printing to produce models of negative spaces, contrast CT radiographic data using segmentation software is illustrated. The accuracy of printed specimens is compared with original specimens. This alternative approach to producing anatomically accurate reproductions offers many advantages over plastination as it allows rapid production of multiple copies of any dissected specimen, at any size scale and should be suitable for any teaching facility in any country, thereby avoiding some of the cultural and ethical issues associated with cadaver specimens either in an embalmed or plastinated form.
Capital punishment and anatomy: history and ethics of an ongoing association

Abstract:Anatomical science has used the bodies of the executed for dissection over many centuries. As anatomy has developed into a vehicle of not only scientific but also moral and ethical education, it is important to consider the source of human bodies for dissection and the manner of their acquisition. From the thirteenth to the early seventeenth century, the bodies of the executed were the only legal source of bodies for dissection. Starting in the late seventeenth century, the bodies of unclaimed persons were also made legally available. With the developing movement to abolish the death penalty in many countries around the world and with the renunciation of the use of the bodies of the executed by the British legal system in the nineteenth century, two different practices have developed in that there are Anatomy Departments who use the bodies of the executed for dissection or research and those who do not. The history of the use of bodies of the executed in German Anatomy Departments during the National Socialist regime is an example for the insidious slide from an ethical use of human bodies in dissection to an unethical one. There are cases of contemporary use of unclaimed or donated bodies of the executed, but they are rarely well documented. The intention of this review is to initiate an ethical discourse about the use of the bodies of the executed in contemporary anatomy.

Abstract:The review article attempts to focus on the practice of human cadaveric dissection during its inception in ancient Greece in 3rd century BC, revival in medieval Italy at the beginning of 14th century and subsequent evolution in Europe and the United States of America over the centuries. The article highlights on the gradual change in attitude of religious authorities towards human dissection, the shift in the practice of human dissection being performed by barber surgeons to the anatomist himself dissecting the human body and the enactment of prominent legislations which proved to be crucial milestones during the course of the history of human cadaveric dissection. It particularly emphasizes on the different means of procuring human bodies which changed over the centuries in accordance with the increasing demand due to the rise in popularity of human dissection as a tool for teaching anatomy. Finally, it documents the rise of body donation programs as the source of human cadavers for anatomical dissection from the second half of the 20th century. Presently innovative measures are being introduced within the body donation programs by medical schools across the world to sensitize medical students such that they maintain a respectful, compassionate and empathetic attitude towards the human cadaver while dissecting the same. Human dissection is indispensable for a sound knowledge in anatomy which can ensure safe as well as efficient clinical practice and the human dissection lab could possibly be the ideal place to cultivate humanistic qualities among future physicians in the 21st century.
Resin-embedded anatomical cross-sections as a teaching adjunct for medical curricula: is this technique an alternative to potting and plastination?

Abstract:With an ever-expanding use of cross-sectional imaging for diagnostic and therapeutic purposes, there has also been an increase in the need for exposure to such radiological and anatomical views at the undergraduate and postgraduate level to allow for early familiarisation with the relevant anatomy. Cadaveric cross-sections offer an excellent link between the two-dimensional radiological images and the three-dimensional anatomical structures. For such cross-sections to be useful and informative within educational settings, they need to be: (i) safe for students and trainees to handle and (ii) robust enough to withstand repeated handling; as well as (iii) displaying anatomy clearly and accurately. There are various ways in which cross-sections can be prepared and presented; plastinated, potted, vacuum-sealed or unmounted. Each of these approaches has advantages and disadvantages in terms of technical complexity, cost and quality. As an alternative to the above methods and their limitations, we propose the presentation of cadaveric cross-sections in a transparent polyester resin. This technique has been used extensively in craft and artistic industries, yet it is not publicised in anatomy teaching settings. The sections were layered in polyester resin contained within a mould. The set resin required finishing by sanding and polishing. The final cross-sections were safe to handle, durable and maintained excellent anatomical relationships of the contained structures. The transparency of the set resin was water-clear and did not obstruct the visibility of the anatomy. The cost of the process was found to be significantly lower, requiring less infrastructure when compared with alternative methods. The following trivial technical difficulties were noted during the resin-embedding process: trapped air causing organs to float; retained water in the anatomical specimens creating bubbles and discoloration; and microbubbles emerging from the solution affecting the finished surface. However, solutions to these minor limitations have been discussed within the paper with the aim of future proofing this technique. The sections have been used in undergraduate medical teaching for 4 years and they have shown no signs of degradation or discoloration. We believe that this method is a viable and cost-effective alternative to other approaches of displaying cross-sectional cadaveric material and will help students and trainees bridge the gap between the traditional three-dimensional anatomy and two-dimensional images.

Abstract:Background: Cadavers are essential for anatomical education, yet their scarcity plagues medical colleges in Kerala and India. However, the Government Medical College in Thrissur has experienced a rise in annual body donations. Aims and Objectives: This study aims to explore the motivating factors behind this increase among the population of Thrissur. Materials and Methods: This study employed a cross-sectional survey design conducted at the Department of Anatomy, Government Medical College in Thrissur, Kerala. The study utilized data from the Body Donation Register, which covered a period from May 1st, 2011 to April 30th, 2012, encompassing a total of 223 individuals. During this specific period, which was selected due to the notable surge in body donation trends, there was a significant increase observed. The sample size was set at 110 participants. Data collection was conducted through face-to-face interviews with the voluntary body donors and was analyzed using the statistical package for the social sciences, version 16.0. Results: Gender distribution: Among 120 donors, 56.7% were male and 26.7% were female. Age groups 61–70 had the highest percentage (36%) of body donors. Socioeconomic status: the majority were above the poverty line (78.2%). Religious affiliation: Hindus had the largest representation (40 individuals). Marital status: Most donors were married (92 out of 110). Number of children: Most donors had two children (64 individuals). Sources of information: Family and relatives were the primary sources. Awareness of dissection: An equal number of donors were aware and unaware. Motivations for donation varied, including social service and avoiding burden after death. Conclusion: A good proportion of donors had a socioeconomic status above the poverty line, and most of them were graduates or postgraduates. The majority of body donors did not have a religious affiliation. However, they had a strong family background and received support from their families. Interestingly, most of the donors were unaware of how their bodies would be used after donation.
