Abstract:This study aimed to compare the effect of a bioceramic sealer (iRoot SP) and a resin-based sealer (AH Plus) on the outcome of root canal treatment in a 2-year follow-up. Seventy-six teeth with irreversibly or necrotic pulp were recruited. After instrumentation and disinfection, the root canals were obturated using warm vertical compaction with iRoot SP (n = 43) or AH Plus (n = 33). Patients were followed up by clinical and radiographic examination at 6 12 and 24 months with recall rates of 84.2%, 65.8% and 48.7%, respectively. During each recall session, the success rates were 80%, 85.2% and 85% in the iRoot SP group and 82.8%, 91.3% and 88.2% in the AH Plus group. The success rates of the two groups did not differ significantly (p > 0.05). The bioceramic sealer resulted in a similar clinical performance and success rate to the resin-based sealer in endodontic treatment during a 2-year follow-up.
Evaluation of the Sealing Ability and Bond Strength of Two Endodontic Root Canal Sealers: An In Vitro Study

Abstract:Background: Obturation represents a critical step in endodontic treatment, which relies on a core material and a sealer. This study aims to evaluate the sealing ability and bond strength to the root canal walls of an epoxy resin-based sealer (AH-Plus®, Dentsply Sirona, Johnson City, TN, USA) and a bioceramic sealer (GuttaFlow Bioseal®, Coltène/Whaledent, GmbH + Co. KG, Langenau, Germany). Methods: Thirty-eight maxillary anterior teeth with single roots and identical round sections were separated into two experimental groups according to the root canal sealers used, namely, G1 = AH-Plus® and G2 = GuttaFlow Bioseal®, and two control groups, specifically, G3 = the negative control and G4 = the positive control. The sealing capacity was measured by the penetration of the radioactive isotope 99mTc. For the push-out test, the compressive force test was performed in a universal machine and the force was applied by exerting pressure on the surface of the material to be tested in the apical to the coronal direction and using three test points with different diameters. Results: GuttaFlow Bioseal® exhibited superior sealing ability compared to AH-Plus® (p = 0.003). Regarding the bond strength, AH-Plus® provided higher adhesion values than GuttaFlow Bioseal® in the three sections of the tooth root (p = 0.001). Conclusions: GuttaFlow Bioseal® had significantly better sealing ability than AH-Plus® but lower adhesion values in the three zones of the root canal, with statistically significant differences between the groups. However, it is important to note that for the action of endodontic sealers to be maximized, the root-filling technique must be most appropriate.

Abstract:Introduction: The purpose of this study was to compare the superiority of root fillings made with three different sealers after minimal instrumentation of maxillary Second Molar root canals utilising a modified single-cone method. The specimens were separated into three groups and filled with a size-fit gutta-percha master cone and Sealmax-R (MAARC), Ah Plus (Dentsply), and MTA Plus (PrevestDenpro) sealers. Before and after instrumentation, post-Gentle Wave, and after obturation, micro–computed tomographic images were collected. Starting at the top of the root, the mesiobuccal roots of the selected teeth were sectioned in 0.5-mm intervals. Light microscope was used to examine the cross sections. Results: In all three groups, was filled to 90%–99% of the canal space. Sealmax-R and Ah Plus groups used more filler material on average than MTA Plus group (P <.05). Apical, middle, and coronal thirds showed no significant differences. Cross-sectional scans indicated no obvious gaps in the Sealmax-R groups. After instrumentation, hard tissue debris was discovered in the root canal system in 49 of the 189 canal thirds (25.9%). After Gentle Wave cleaning, only 4 of 63 canals (6.3%) and 4 of 189 canal thirds (2.1%) contained debris.

Abstract:BACKGROUND: The purpose of the present study was to compare the biological and the physico-chemical properties of bioceramic-based root canal sealers, calcium hydroxide-based, MTA-based and epoxy resin-based root canal sealers.MATERIAL AND METHODS: Two bioceramic-based sealers, one calcium hydroxide-based sealer, one MTA-based sealer and two epoxy resin-based sealers were tested.RESULTS: EasySeal and MTA Fillapex showed severe citotoxic activity, AH Plus and SealapexTM moderate cytotoxicity, BioRoot™ RCS and TotalFill BC Sealer were both cytocompatible. Except for TotalFill BC Sealer, all root canal sealers caused inhibition zones when tested with E. faecalis. The highest inhibition zone was observed for EasySeal, followed by AH Plus. BioRoot™ RCS, SealapexTM and MTA Fillapex showed the lowest inhibition zone. All the tested materials showed different degree of antibacterial activity by using direct contact test (DCT). The highest values were observed for BioRoot™ RCS, TotalFill BC Sealer and EasySeal, followed by MTA Fillapex and SealapexTM. Except for BioRoot RCS and TotalFill BC Sealer, all the root canal sealers fulfilled the requirements of the ISO 6876 standard, demonstrating a weight loss less than 3%. Bioroot RCS, TotalFill BC Sealer and SealapexTM exhibited high alkaline pH with an increase both for BioRoot™ RCS and TotalFill BC Sealer after 24 hours.CONCLUSIONS: The new bioceramic-based sealers showed acceptable physico-chemical properties, but BioRoot™ RCS and TotalFill BC Sealer seems to be too soluble, not respecting ISO 6876 requirements. Field Emission Scanning Electron Microscope Analysis of the Marginal Adaptation of Various Root Canal Sealers at the Dentin-Sealer and Sealer-Guttapercha Interfaces at Three Root Canal Levels: An In Vitro Study

Abstract:Aims An in vitro evaluation of the marginal adaptation of four root canal sealer variants at the dentin-sealer and sealer gutta-percha interfaces in the coronal, middle, and apical thirds of root canals was conducted using field emission scanning electron microscope (FESEM) analysis. Materials and methods In total, 80 extracted human mandibular premolar teeth were used in this study. All teeth were decoronated to standardize the root length to 14 mm. A round bur was used to gain access into canal orifices and the working length was determined. Root canal instrumentation was performed using the crown-down technique with ProTaper Next rotary files up to size X3, along with the use of root canal irrigants. All specimens were then randomly divided into four groups, with 20 specimens per group, depending on the root canal sealer used: Group A consisted of AH Plus (Dentsply Maillefer, Ballaigues, Switzerland), Group B of MTA-Fillapex (Angelus Dental, Londrina, Brazil), Group C of Bio-C Sealer (Angelus Dental), and Group D of GuttaFlow-2 (Coltene Whaledent, Altstatten, Switzerland). All specimens were obturated with size X3 gutta-percha points uniformly coated with respective sealers in the single-cone technique and coronal access was sealed with glass ionomer cement. The specimens were incubated for seven days and then horizontally sectioned at the coronal, middle, and apical thirds of the root canals. On each sample obtained, three points were randomly chosen and both sealer-dentin and sealer gutta-percha interfaces were examined under FESEM at 1000x magnification. The marginal gaps of all four sealers at both interfaces and at three levels of root canals were measured in µm and values were recorded, tabulated, and used for data analysis. One-way analysis of variance (ANOVA) and post hoc Bonferroni tests were used for statistical analysis. A p-value ≤ 0.05 was considered statistically significant. Results Compared to the sealer gutta-percha interface, AH Plus, MTA-Fillapex, and Bio-C sealers at all three levels of the root canals showed more marginal gaps at the sealer-dentin interface with a significant difference (p<0.05). However, the GuttaFlow-2 sealer showed no significant difference (P > 0.05). Conclusions The marginal adaptation of the GuttaFlow-2 sealer is superior to both dentin and gutta-percha at the coronal, middle, and apical thirds of root canals compared to other sealers used in the study.
Physical Properties of 5 Root Canal Sealers

Abstract:INTRODUCTION:The aim of this study was to evaluate the pH change, viscosity and other physical properties of 2 novel root canal sealers (MTA Fillapex and Endosequence BC) in comparison with 2 epoxy resin-based sealers (AH Plus and ThermaSeal), a silicone-based sealer (GuttaFlow), and a zinc oxide-eugenol-based sealer (Pulp Canal Sealer). METHODS:ISO 6876/2001 specifications were followed. The pH change of freshly mixed and set sealers was evaluated during periods of 1 day and 5 weeks, respectively. The viscosity was investigated at different injection rates (72, 10, and 5 mm/min) at room temperature by using a syringe-based system that was based on the Instron 3360 series universal testing system. RESULTS:The flow, dimensional change, solubility, and film thickness of all the tested sealers were in agreement with ISO 6876/2001 recommendations. The MTA Fillapex sealer exhibited a higher flow than the Endosequence BC sealer (P < .05). The MTA Fillapex and Endosequence BC sealers showed the highest film thicknesses among the tested samples. The Endosequence BC sealer exhibited the highest value of solubility, which was in accordance with 3% mass fraction recommended by the ISO 6876/2001, and showed an acceptable dimensional change. The MTA Fillapex and Endosequence BC sealers presented an alkaline pH at all times. The pH of fresh samples of the AH Plus and ThermaSeal sealers was alkaline at first but decreased significantly after 24 hours. The viscosity of the tested sealers increased with the decreased injection rates. CONCLUSIONS:The tested sealers were pseudoplastic according to their viscosities as determined in this study. The MTA Fillapex and Endosequence BC sealers each possessed comparable flow and dimensional stability but higher film thickness and solubility than the other sealers tested.

Abstract:ObjectivesThe aim of the present study was to evaluate the adaptation of a calcium silicate bioceramic (BC) sealer with either BC or conventional gutta-percha compared with that of AH Plus sealer in different root canal sections.Materials and methodsSeventy-two extracted mandibular premolars were divided randomly into six groups. After standardised chemomechanical preparation, four groups were obturated with the BC sealer and BC gutta-percha or conventional gutta-percha, and the other two groups were obturated with AH Plus sealer and conventional gutta-percha either in lateral compaction or in a single cone technique. Each root was sectioned into three sections. An impression was made from each section, and replicas were then made for scanning electron microscopy (SEM) analysis. Areas and interfacial gaps were identified using image analysis software. In addition to descriptive and explorative data analyses, linear regression analysis was performed.ResultsAll specimens had measurable interfacial gaps. Significantly fewer gaps were found between conventional gutta-percha and sealer compared to those observed when using the BC gutta-percha (p < 0.001). However, minor interfacial gaps between sealer and dentin were observed with the BC sealer (p = 0.04). The technique of obturation in different root canal sections did not significantly affect the sealer adaptability.ConclusionThe type of gutta-percha as well as the sealer had a noticeable impact on the adaptability.Clinical relevanceDifferent obturation techniques will result in similar outcomes. However, within the limitations of the study, there seems to be no advantage in using the BC gutta-percha.

Occurrence and clinical correlates of psychiatric comorbidity in patients with psychotic disorders.

Abstract:BACKGROUND The aim of this study was to explore patterns and clinical correlates of psychiatric comorbidity in patients with schizophrenia spectrum disorders and mood spectrum disorders with psychotic features. METHOD Ninety-six consecutively hospitalized patients with current psychotic symptoms were recruited and included in this study. Index episode psychotic diagnosis and psychiatric comorbidity were assessed using the Structured Clinical Interview for DSM-III-R-Patient Version (SCID-P). Psychopathology was assessed by the SCID-P, Brief Psychiatric Rating Scale, Scale for the Assessment of Negative Symptoms, and Hopkins Symptom Checklist. Awareness of illness was assessed with the Scale to Assess Unawareness of Mental Disorders. RESULTS The total lifetime prevalence of psychiatric comorbidity in the entire cohort was 57.3% (58.1% in schizophrenia spectrum disorders and 56.9% in mood spectrum psychoses). Overall, panic disorder (24%), obsessive-compulsive disorder (24%), social phobia (17.7%), substance abuse (11.5%), alcohol abuse (10.4%), and simple phobia (7.3%) were the most frequent comorbidities. Within the group of mood spectrum disorders, negative symptoms were found to be more frequent among patients with psychiatric comorbidity than among those without comorbidity, while such a difference was not detected within the group of schizophrenia spectrum disorders. Social phobia, substance abuse disorder, and panic disorder comorbidity showed the greatest association with psychotic features. An association between earlier age at first hospitalization and comorbidity was found only in patients with unipolar psychotic depression. Patient self-reported psychopathology was more severe in schizophrenia spectrum patients with comorbidity than in those without, while such a difference was less pronounced in mood spectrum psychoses. CONCLUSION These findings suggest that psychiatric comorbidity is a relevant phenomenon in psychoses and is likely to negatively affect the phenomenology of psychotic illness. Further studies in larger psychotic populations are needed to gain more insight into the clinical and therapeutic implications of psychiatric comorbidity in psychoses.
In vitro genotoxicity of root canal sealers

Abstract:Aim: To evaluate the effect of leakage on differences in genotoxicity of root canal sealers ex vivo according to their main components using two different cytogenetic assays. Methodology: Six materials of different composition (GuttaFlow, Epiphany, Diaket, IRM, SuperEBA and Hermetic) were tested on human peripheral blood lymphocytes using the comet assay and chromosomal aberration analysis. Prepared materials were eluted in physiological solution for 1 h, 1 day, 5 and 30 days. Thereafter cultures were treated with 8 microg, 4 microg and 2 microg of each sealer. Frequencies of chromatide and chromosome breaks and accentric fragments were determined. Comet assay was used to evaluate primary DNA damage by measuring tail length and tail intensity. Chi-square, Fisher's PLSD (Protected Least Significant Difference) and Kruskall-Wallis non parametric tests were used for statistical analysis. Results: After 1-h elution only the highest dose of Diaket, Hermetic and SuperEBA significantly (P = 0.035, P = 0.048, P = 0.037 respectively) affected the measured cytogenetic parameters. The migration ability of DNA was more strongly affected than induction of chromosomal aberrations. After elutions longer than 24 h none of the tested sealers exhibited a genotoxic effect. Conclusion: Under the conditions used in the study all sealers had acceptable biocompatibility in terms of genotoxicity.
Evaluation of sealing ability of four bioceramic root canal sealers and an epoxy resin-based sealer: An in vivo study

Abstract:Introduction: The aim of the study was to evaluate the bacterial leakage of four different bioceramic root canal sealers and to compare it to the conventional epoxy resin-based sealer. Materials and Methods: The study sample consisted of 94 single-rooted human teeth with one round root canal, which were instrumented and randomly distributed into five experimental groups according to the root canal sealer: Group 1: TotalFill bioceramic sealer; Group 2: BioRoot root canal sealer; Group 3: mineral trioxide Aggregate (MTA) Fillapex; Group 4: MTA Plus; Group 5: epoxy resin-based sealer AH Plus and positive and negative control group. The prepared samples were fixed in a model designed for the Enterococcus faecalis leakage evaluation and were sterilized in plasma. The bacterial penetration through the filled root canals was checked by the color change in the lower tube during 60 days. The scanning electron microscopy was used to check the bond between the sealer and the dentine wall. Data were statistically analyzed. Results: Total Fill bioceramic sealer (BCS) group presented significantly lower number of samples that leaked (14.2%) compared to the AH Plus group (78.6%) ( P = 0.002) and MTA Plus group (50%) ( P = 0.046). There was no statistically significant difference between other groups ( P > 0.05). Conclusion: The TotalFill BCS root canal sealer provided better sealing ability compared to the AH Plus and the MTA Plus sealers.
Comparative evaluation of sealing ability of three bioactive obturation materials: A bacterial leakage study

Abstract:Aim: The aim of this study was to evaluate and compare the sealing ability of three obturating materials used with single-cone technique: C Point/Endosequence BC sealer, gutta-percha/Endoseal MTA and gutta-percha/GuttaFlow bioseal using bacterial leakage model with Enterococcus faecalis. Materials and Methods: Seventy-six extracted human single-rooted mandibular premolars with straight canals prepared using Hyflex CM up to 45/04 and assigned to 3 experimental groups according to the root canal filling material as Group A - C Point/Endosequence BC sealer; Group B - guttapercha/Endoseal MTA; Group C - guttapercha/GuttaFlow bioseal. Split-chamber bacterial leakage model was prepared and microleakage analysis was made every 24 h for up to 9 weeks. The presence of E. faecalis was confirmed by light microscopy and Gram staining. Results: A survival analysis was performed using Kaplan–Meier plots and a log-rank test and it was found that Group A showed the best apical seal followed by Group C and then Group B at the end of the observational period. The difference in bacterial leakage among Group A and C versus Group B was statistically significant (P < 0.05), but there was no significant difference between Group A and Group C. Conclusion: C Point/Endosequence BC sealer and GP/Guttaflow bioseal sealer has shown better apical seal with single-cone technique as compared to GP/Endoseal MTA.
Effects of Two Different Root Canal Preparation and Two Different Sealers on Apical Microleakage

Abstract:Objective To compare the apical sealability between two different root canal preparation and two different root canal sealers.Methods Fresh extracted permanent teeth were randomly divided into one control group and A,B,C,D four test groups.The root canals in control group were prepared manually with the stainless steel instruments and not filled.The test groups: group A: Prepared with rotarory hero 642 Nickel-Titanium instruments and the sealer was cortisomol.Group B: prepared manually with the stainless steel instruments and the sealer was cortisomol.Group C: prepared manually with the stainless steel instruments and the sealer was vitapex.Group D:prepared with rotarory hero 642 Nickel-Titanium instruments and the sealer was vitapex.All canals were obturated by cold lateral compaction of Gutta Percha with sealer.All the teeth were dyed in 2% methylene blue for 72 hours.The linear lengths of dye were measured to evaluate the degree of microleakage.Results All of the canals in four test groups showed some degrees of apical microleakage.The microleakage in group A was significant lower than that in group C and group D.No significant difference was found in B,C,D,groups.Conclusion Engine-driven prepared with Hero 642 Nickel-Titanium instruments and filled with Cortisomol could reduce the apical microleakage and increase the effects of RCT effectively.Sealers may play more important roles in the apical microleakage.

Abstract:Introduction: Airtight sealing of the root canal is the foremost desirable result of any root canal treatment, but getting the perfect filling of the root canal is always problematic, which could be due to multiple reasons. One such majorly reason impacting the obturation is the root canal sealer utilized. Objective: The present study was done for evaluating microleakage in zinc oxide eugenol cement (ZOE) and epoxy resin-based sealer (AH Plus) root canal sealers using linear dye leakage method. Methods: This in-vitro study used twenty-two extracted human single canal mandibular premolar teeth comprising 2 groups (11 in each), where sealers used for microleakage evaluation were ZOE cement sealer, and AH Plus. All the samples once cleaned, shaped and obturated were soaked in methylene blue dye for forty-eight hours with two millimeter (mm) exposed apex. The samples were then split into halves and inspected under a stereomicroscope for microleakage. The generated data were statistically analyzed using an independent t-test in SPSS version 11. Results: All the test samples showed microleakage. However, the mean microleakage measured was considerably higher in the ZOE cement sealer at 6.18mm and 2.95mm for the Epoxy resin-based sealer (AH Plus sealer). Conclusions: Epoxy resin-based sealer has better performance than ZOE cement sealer with minimal microleakage. Thus, Epoxy resin-based sealer showed a better prognosis and it can be considered to be adapted in clinical practice.
Antibacterial activity of different root canal sealers against Enterococcus faecalis

Abstract:BACKGROUND: The aim of the present study was to compare in vitro the antimicrobial activity of different root canal sealers against Enterococcus faecalis, prior and subsequent to setting.MATERIAL AND METHODS: Agar diffusion test (ADT) was used for evaluating the antibacterial activity of non-set sealer while the direct contact test (DCT) was used for after setting.RESULTS: ADT: Except for TotalFill BC Sealer all the others sealers tested showed antibacterial activity. BioRoot™RCS, MTA Fillapex and Sealapex Root Canal Sealer showed the lowest antibacterial activity, a significant increase in antibacterial effect for both Pulp Canal Sealer™ and AH plus sealers were found. Significantly higher were the mean diameters of the bacterial inhibition zone by both EasySeal or N2 sealers. DCT: AH plus and Sealapex Root Canal Sealer doesn't show any bactericidal effect after 6 min of contact. After 15 and 60 min of contact a significant increment for AH plus and for Sealapex Root Canal Sealer of the bactericidal effect was found. Significantly much higher was the antibacterial effect of Sealapex Root Canal Sealer compare to that observed for AH plus. BioRootTMRCS, MTA Fillapex, Pulp Canal Sealer™ and N2 showed at least means of the number of colonies formed in milliliter after 6 min of contact. Except for N2, a significant increase in bactericidal effect after 15 and 60 min for the other compared sealers (BioRootTMRCS, MTA Fillapex and Pulp Canal Sealer™).CONCLUSIONS: For every contact times considered, both TotalFill BC Sealer and EasySeal were bactericidal against E. faecalis and killed all bacteria. Abstract:INTRODUCTION Microleakage continues to be a main reason for failure of root canal treatment where the challenge has been to achieve an adequate seal between the internal structure and the main obturating material. The objective of this study is to compare the sealing ability of 3 newer obturating materials GuttaFlow, Resilon/Epiphany system (RES) and Thermafil, using silver nitrate dye and observing under stereomicroscope. METHODOLOGY Thirty single rooted teeth were divided into following groups. Group I : GuttaFlow ;Group II : Resilon /Epiphany sealer Group III : Thermafil with AH-Plus sealer. Teeth were decoronated and instrumented with profile rotary system and obturated with specified materials. Apical seal was determined by dye penetration method using silver nitrate. Then the specimens were transversely sectioned at each mm till 3 mm from the apex. Dye leakage was determined using stereomicroscope. Statistical analysis of the results was performed using Kruskall-Wallis test. RESULTS The results showed that Group II i.e., Resilon with Epiphany sealer showed the least amount of microleakage when compared to Group I i.e., GuttaFlow and Group III i.e., Thermafil with AH-plus sealer. CONCLUSION Based on the results of this study it can be concluded that RES had higher sealing ability followed by Thermafil and GuttaFlow in vitro but further studies have to be carried out to make a direct correlation between these results and invivo situation. Evaluation of cytotoxicity and up-regulation of gelatinases in human fibroblast cells by four root canal sealers

Abstract:Aim: To investigate the effects of root canal sealers on the cytotoxicity and gelatinolytic activity of matrix metalloproteinases (MMPs) in human fibroblasts. Methodology: Human fibroblasts (MRC5, 3×10(5) cells per well) were incubated directly or indirectly with AH Plus, Endomethasone N, Pulp Canal Sealer EWT or Sealapex for 30 min, 1, 4 or 24 h (time-points). The cytotoxicity of all root canal sealers was determined by counting viable cells using the trypan blue exclusion assay. Supernatants of cell cultures incubated with root sealers directly or indirectly were collected after each time-point to determine the levels of MMP-2 and MMP-9 gelatinolytic activity by gelatin zymography. Data were analysed using anova and the Tukey's tests. Results: Cells secreted MMP-2 after periods of 4 and 24 h; however, there were no significant differences between the sealers. Secretion of gelatinases was elevated by root canal sealers in direct contact with the cell monolayer when compared to indirect contact (P < 0.05). At the time-points tested, no gelatinolytic activity could be detected in the control group without the sealers. The cytotoxicity results revealed that all sealers were cytotoxic in both contact forms. Sealapex had the lowest cytotoxicity and AH Plus the most cytotoxicity. Conclusions: All root canal sealers induced the expression of MMP-2 in MRC5 fibroblasts. AH Plus had the highest cytotoxicity amongst the tested sealers, but all were associated with cytotoxic effects.
Clinical outcome of non-surgical root canal treatment using different sealers and techniques of obturation in 237 patients: A retrospective study

Abstract:Objectives: The aim of this retrospective study was to compare the clinical results of two root canal sealers and three obturation techniques used for non-surgical root canal treatment. Materials and methods: A total of two hundred eighty-three root canal treated teeth in two hundred thirty-seven patients with minimum a 6-month follow-up was included for this study. The canals were filled with three different modes: 1) cold lateral condensation (CLC) and AH Plus Sealer; 2) continuous wave condensation technique (CWC) and AH Plus Sealer, and 3) sealer-based obturation technique (SBO) and AH Plus Bioceramic Sealer. The treatment outcome was analysed based on clinical signs and symptoms, and periapical radiograph (periapical index, PAI). Results: There were no significant differences in treatment outcome between various sealers and filling techniques applied. The sealer extrusion was found most frequently in the CWC group (60.67%), followed by SBO (59.21%) and CLC (21.19%) with statistically significant differences (p < .05). The initial diagnosis, previous treatment and sealer extrusion (p < .05) were prognostic factors that affected treatment outcome. Conclusions: Based on the findings of this study, neither the sealer type nor the filling technique affected the treatment success while preoperative diagnosis, previous treatment and sealer extrusion had significant effect on the outcome. Clinical relevance: A bioceramic sealant applied along with the single-cone technique might be considered as an alternative method in root canal obturation.

Abstract:Background: Root canal irrigants play an essential role in removing the smear layer, pulp tissue, and microorganisms in the root canal system. The irrigation solution can remove the smear layer, which can interfere with the bond between the sealers. As a result of sealer penetration into dentin tubules following smear layer removal, an apical seal was obtained. One of the most recent innovations in root canals consists of a combination of silver citrate (BioAKT, New Tech Solution, Brescia, Italy) and chlorhexidine-ethylenediaminetetraacetic acid (CHX-EDTA; SmearOFF, Vista Dental, Racine, USA). These irrigants were used as the final irrigant for root canal disinfection. Epoxy sealers (AH26, Dentsply Maillefer, Ballaigues, Switzerland) and bioceramic sealers (ceraseal, Meta Biomed Co., Cheongju, Korea) have excellent adhesive properties and sealing ability and are commonly used as obturation materials. Objective: The aim of this study was to compare the effectiveness of a root canal combination irrigant between silver citrate and CHX-EDTA on the apical sealing ability of epoxy sealer and bioceramic sealer. Materials and Methods: Forty extracted mandible single-root premolars were used in this study. The root canal preparation was done with a rotary instrument until no. 30.06. Irrigation was performed using 5.25% NaOCl between instrumentations. The final irrigant and sealers were Group I: BioAKT + ceraceal; Group II: BioAKT + AH26; Group III: SmearOFF + ceraceal; and Group IV: SmearOFF + AH26. Obturation was then performed using the warm vertical compaction technique. The sealing ability of endodontic sealers was evaluated using the dye penetration method under the magnification of the stereomicroscope. The data were statistically analyzed using a two-way analysis of variance (ANOVA) (SPSS, IBM, USA). Results: No significant difference (P > 0.05) was observed between the four experimental groups. Conclusion: Both combination irrigants and sealers showed comparable effectiveness in smear layer removal and a similar sealing ability.

Abstract:Background: New and innovative materials are being marketed for the treatment of coronal pulpotomy. It is crucial to compare their efficacy with already-established materials. TheraCal PT (TP) is such a new material that studies are scarce. Aim: This study aim to compare and evaluate the outcome of coronal pulpotomy using Biodentine and a newly introduced calcium silicate-based dual-cure resin cement, TP, in patients with symptoms of irreversible pulpitis. Materials and methods: Sixty patients with exposed carious pulp and symptomatic irreversible pulpitis were included, aged 18-40, randomly allocated to two groups: TP (group I) and Biodentine (group II). Coronal pulpotomy was performed following a standardized protocol, with TP or Biodentine applied accordingly. The pain was recorded using Visual Analog Scale preoperatively for up to 1 week. Success was assessed clinically and radiographically for up to 12 months. Statistical analysis used: The data were analyzed using the Friedman test and the Mann-Whitney U-test. Intragroup pain was analyzed using the Wilcoxon signed-rank test. Results: Among 60 patients, intervention was done in 53. By removing dropout patients, 47 were analyzed, with 38 available for follow-up at 3, 6, and 12 months. The Biodentine group exhibited a 12-month success rate of 84%, while the TP group revealed 77.3%, with statistically insignificant difference (P = 0.563). Conclusion: TP can be effectively utilized as a pulpotomy material in cases of symptomatic irreversible pulpitis in mature permanent teeth, offering rapid setting and ease of use, although Biodentine yielded slightly better results in this study.

Abstract:Background: Root canal retreatment aims to eliminate or substantially reduce the microbial load from the root canal and removal of previous root filling materials is the primary goal of nonsurgical retreatment. Aim: To comparatively evaluate the effectiveness of ProTaper retreatment files, Mtwo retreatment files, and Hedstrom files along with Gates Glidden (GG) Drill by using a stereomicroscope for the detection of remaining gutta-percha after root canal retreatment using a stereomicroscope. Materials and methods: Forty-five test samples were divided into three experimental groups (15 samples each), such as Group A-ProTaper retreatment files; Group B-Mtwo retreatment files; Group C-Hedstrom files along with Gates Glidden Drill. After routine endodontic treatment in the test samples, gutta-percha was removed from the test samples as per the division of different retreatment file systems. Further, test samples were divided longitudinally and were evaluated under the stereomicroscope. Stereomicroscopic images were captured and analysed under Image Analyser Software-Image J. Statistical analysis: One-way ANOVA test was used for comparison of gutta-percha removal between the groups and Tukey's post hoc test was used for pairwise comparisons. Results: The least remaining filling material was seen in specimens retreated with the ProTaper universal system. The highest remaining filling material was seen in specimens retreated with Hedstrom file with Gates Glidden Drill. Conclusion: All experimental test techniques left gutta-percha remnants within the root canal. ProTaper universal rotary retreatment system proved to be an efficient method of removing gutta-percha and left relatively less volume in comparison with other test systems.
