Abstract:Fragility fractures around the proximal end of the femur have increased in recent years due to an aging population, adding to the pressure on national health care systems and to hospital expenses. Peri-trochanteric fractures have historically been treated successfully with anatomic intramedullary nails, giving stable fixation in order to allow early for mobilisation of these frail patients. Some of these nails allow a second (anti-rotational) screw through the nail into the femoral head. We assessed the use of this additional screw in terms of quality of reduction, post-operative mobilization and complications.

Comparison of Percutaneous Compression Plating and Short Reconstruction Nail for Treatment of Intertrochanteric Fracture

Abstract:Objective: To compare percutaneous compression plating (PCCP) with Trigen short reconstruction nail (Trigen SRN) for the treatment of intertrochanteric hip fracture.Methods: Eighty‐four patients with intertrochanteric hip fracture admitted to our hospital from January 2007 to June 2008 were included in this retrospective study. Thirty‐six patients were treated with PCCP and 48 with Trigen SRN. Information regarding age, surgical time, blood loss, length of follow‐up, mortality, fracture impaction, time to union, complications, Harris score and visual analog scale (VAS) were all recorded.Results: The mean follow‐up time was 16.3 ± 3.2 months (13–19 months). The mean time to achieve radiological fracture healing was 3.6 ± 0.8 and 4.1 ± 1.0 months for the PCCP and Trigen SRN groups, respectively (P= 0.020); the mean Harris hip scores were 78.1 ± 4.8 and 74.1 ± 5.2 (P= 0.001); and the mean fracture impaction was 3.6 ± 2.3 and 3.3 ± 2.6 mm (P= 0.023). There were no statistical differences between the two groups in duration of surgery (P= 0.131) and blood loss (P= 0.268). The amount of nail in the inferior‐central quadrants was greater in the PCCP group than in the Trigen SRN group.Conclusion: PCCP achieves earlier pain release and better fracture reduction than Trigen SRN.

Abstract:Background The Trigen short reconstruction trochanteric antegrade nail (TAN) is considered most suitable for the small skeletons of Asian patients. We have evaluated the results of TAN for intertrochanteric fractures in Asian patients.Methods Between January 2006 and March 2009, 174 fractures in 164 patients were treated by TAN in our department. Epidemiological data, radiological changes, fracture types, cortical bone indices, clinical outcomes, and complications over a mean of 26 months were analyzed retrospectively.Results We observed a 17.0% fracture rate at the tip of the nail, a poor reduction rate of 15.9%, and a 41.4% cracking rate of the lateral trochanter for type 31-A3 fracture. Review of these cracking cases showed that they were all type 31-A3 unstable fractures combined with comminuted lateral trochanter fractures. Two shaft fractures required revision, with other fractures showing no delayed union, nonunion or infection. No screw breakage but one cut-out occurred, which received total hip arthroplasty revision. All other fractures healed at last. Risk of distal shaft fractures was not associated with patient age, gender, fracture type or cortical bone index.Conclusion Despite the theoretically better design of TAN and the healing of most fractures and good activity at last follow-up, poor reduction and coxa vara caused by insertion of the nail could not be completely avoided, and the design of the distal part of the nail had the potential to cause femoral shaft fracture. Future improvements are necessary to enable the use of this system in Asians.
Characteristics of intramedullary nail breakage in pertrochanteric femur fractures: a summary of 70 cases

Abstract:Objective To reveal noteworthy characteristics of intramedullary (IM) nail breakage in pertrochanteric femur fractures. Materials and methods The data from 6 patients with IM nail breakage in our hospital between August 2008 and May 2018 were reviewed retrospectively. With an additional 64 cases reported in articles in the PubMed database prior to October 2019, a total of 70 cases were reviewed and analysed; epidemiological patient data and data on the initial trauma, fracture type, nail selected for the original surgery, time from surgery to breakage, mechanism and location of breakage, status of fracture healing, salvage treatment and prognosis were assessed. Results Seventy patients with pertrochanteric fractures, including 2 stable fractures and 68 unstable fractures, experienced IM nail breakage at a mean of 9.4 months after the initial surgery. Among them, 9 (12.9%) occurred within 3 months, 23 (32.9%) between 3 and 6 months and 38 (54.3%) after 6 months. The mean age was 72.3 years, and 16 (22.9%) patients were younger than 65 years old. When nail breakage occurred, 66 fractures (94.3%) exhibited delayed union/non-union. Self-dynamisation was found in 12 cases (17.1%). The salvage procedures included 4 partial/total implant removal procedures, 17 hemi/total hip arthroplasty procedures, 3 conservative treatment procedures, and 46 revised osteosyntheses, of which 7 cases (15.2%) sustained secondary implant failure. No significant differences were found between the failure rates of IM nails and extramedullary(EM) devices (odds ratio [OR], 3.429; 95% confidence interval [CI], 0.632–18.877; p = 0.330). Conclusion IM nail breakage is a rare complication lack of time regularity and mostly occurs in unstable pertrochanteric fractures in the presence of delayed union/non-union. Osteosynthesis revision can be conduct by a new IM nail or EM device but considerable secondary failure rate is noteworthy. Self-dynamisation may be a warning sign of nail breakage.

Abstract:<h3 class="a-plus-plus">Abstract</h3> <span class="a-plus-plus abstract-section id-a-sec1"> <h3 class="a-plus-plus">Introduction</h3> <p class="a-plus-plus">Recent advances have led to the design of a new cephalomedullary nail, which aims to decrease the risk of failures in patients with intertrochanteric hip fractures by allowing for insertion of two interdigitating screws into the head segment. The goal of this study is to evaluate the safety and efficacy of this two-screw cephalomedullary nailing system.</p> </span> <span class="a-plus-plus abstract-section id-a-sec2"> <h3 class="a-plus-plus">Patients/participants</h3> <p class="a-plus-plus">Patients 18 years of age and older who underwent intramedullary nailing of their intertrochanteric femoral fracture using the InterTAN nailing system (Smith and Nephew, Memphis, TN) from 2012 to 2016 were included in this retrospective study which was performed at two urban certified level-1 trauma centers and one urban certified level-3 trauma center. The study data was collected through a retrospective chart review and review of the existing radiographic studies. Primary outcome measure was mechanical hardware failure and screw cutout. Secondary outcome measures included nonunion, malunion, medical and surgical complications.</p> </span> <span class="a-plus-plus abstract-section id-a-sec3"> <h3 class="a-plus-plus">Results</h3> <p class="a-plus-plus">A total of 264 patients were included in this analysis. Two patients (0.75%) were found to have a screw cut out requiring revision surgery. Two other revision surgeries were performed for malrotation (<em class="a-plus-plus">n</em> = 1) and malunion (<em class="a-plus-plus">n</em> = 1). Other implant-related complications occurred in 19 cases (7.9%), which included broken distal screws (<em class="a-plus-plus">n</em> = 9), distal screw loosening (<em class="a-plus-plus">n</em> = 8), and loose lag screws (<em class="a-plus-plus">n</em> = 2). There was a total of 10 (3.8%) surgical wound complications, including four deep and six superficial infections.</p> </span> <span class="a-plus-plus abstract-section id-a-sec4"> <h3 class="a-plus-plus">Discussion</h3> <p class="a-plus-plus">This modified cephalomedullary nail is a reliable, safe, and effective implant for management of intertrochanteric hip fractures. Surgical treatment of patients with intertrochanteric hip fractures can be performed in a safe fashion using this implant.</p> </span>
Treatment of Unstable Intertrochanteric Fractures with Percutaneous Non-Contact Bridging Plates

Abstract:We reviewed medical records of patients with unstable intertrochanteric fractures (AO/OTA type 31-A2, A3) treated with percutaneous non-contact bridging (NCB) plates and with gamma nails and compared operative data and outcomes. Fifty-two patients received NCB plates (27 males and 25 females; median age, 77.0 years) and 36 received gamma nails (15 males and 21 females; median age, 77.0 years). Fluoroscopy time in the NCB plate group was 34.0 s (IQR: 30.0, 37.5), and in the gamma nail group was 45.0 s (IQR: 39.0, 51.5, p < 0.001). Hospital stay in the NCB plate group was significantly shorter than the gamma nail group (15.0 days [IQR: 13.0, 16.5] vs. 17.0 days [IQR: 14.0, 19.0], respectively; p = 0.004). Harris hip scores were similar between the groups at 6 months and 1 year postoperatively. The percutaneous NCB plate provides satisfactory results for the treatment of unstable intertrochanteric fractures.
COMPARATIVE STUDY BETWEEN THE FAILURE RATES OF THE TROCHANTERIC FIXATION NAIL-ADVANCED (TFNA) AND GAMMA NAILS IN PROXIMAL FEMUR FRACTURE FIXATION

Abstract:Hip fractures frequently occur in elderly patients with osteoporosis and are rapidly increasing in prevalence owing to an increase in the elderly population and social activities. We experienced several recent presentations of TFNA nails failed through proximal locking aperture which requires significant revision surgery in often highly co-morbid patient population. The study was done by retrospective data collection from 2013 to 2023 of all the hip fractures which had been fixed with Cephalomedullary nails to review and compare Gamma (2013–2017) and TFNA (2017–2023) failure rates and the timing of the failures. Infected and Elective revision to Arthroplasty cases were excluded. The results are 1034 cases had been included, 784 fixed with TFNA and 250 cases fixed Gamma nails. Out of the 784 patients fixed with TFNA, 19 fixation failed (2.45%). Out of the 250 cases fixed with Gamma nails, 15 fixation failed (6%). Mean days for fixation failure were 323 and 244 days in TFNA and Gamma nails respectively. We conclude that TFNA showed remarkable less failure rates if compared to Gamma nails. At point of launch, testing was limited and no proof of superiority of TFNA over Gamma nail. Several failures identified with proximal locking aperture in TFNA which can be related to the new design which had Substantial reduction in lateral thickness at compression screw aperture.

Abstract:Trochanteric fracture id one of the commonest fractures in Orthopaedics and also among the most devastating injuries of the elderly. The incidence of this fracture increases with advancing age. Around 50% of the Peri-trochanteric variant of these fractures, are of the unstable type. PFNA2 deploys a helical blade which provides compressibility and an additional rotation control at the fracture site.The aim of this study is to assess the effectiveness of Intra-medullary xation of unstable peri-trochanteric fractures with inter-locking proximal femoral nail PFNA2, in a prospective study. This study was done in Department of Orthopaedics, Chettinad Hospital and Research institute, Kelambakkam, Chengalpattu district, Tamilnadu and was spread over 4 years from Jan 2017 to December 2020 with minimum follow-up period of 12 months. 66 patients participated in this study. Harris Hip Score was used to assess the functional outcome. The Harris Hip Score, mean at 6 months was 75.35, which improved to 82.02 by 12 months of follow-up. With regard to our complications 4 patients had a superior cut out of helical screw, which required re-operation by bipolar hemiarthroplasty of the cemented variant. 9 patients had a varus deformity of 10-12 degrees. Abductor lurch was noted in 11 patients. Supercial-wound infection occurred in 8 patients and deep wound infection, requiring surgical irrigation occurred in 3 patients. In conclusion, PFNA2 has better stability, better rotatory stress shielding, superior compaction at fracture site, less blood loss, shorter operation time, less radiation exposure and better clinico-radiological outcomes. PFNA2 is a good implant choice to tackle the complex problems encountered in displaced unstable trochanteric fractures

Abstract:PurposeProximal femoral fractures are among the most commonly sustained fractures. The current treatment of stable proximal femoral fractures located in trochanteric region primarily involves the use of two systems: extramedullary dynamic hip screws and intramedullary hip nails. Given that these fractures are mainly found in the elderly population, the necessity of a repeat, due to failure of the first, may jeopardize the patient’s life. Decisive factors contributing to the healing of a fracture (or the failure thereof) include fracture pattern, technical implementation of the operation (i.e., position of the implant), implant’s properties and its changes in relation to the surrounding bone tissue during loading. Each screw insertion variant results in damage to various load-bearing bone structures, which can be expected to influence healing quality and stability of newly formed bone.MethodWith the aid of a numerical model and finite element methods, the authors analyzed several different positions of IMHN/PFH-nails in the proximal femur, with the objective of determining positions with an increased risk of failure.Results and conclusionIn model situations, it has been shown that in stable fractures results do not depend on absolutely precise positioning and small deflections in the nails and neck screws positions do not significantly increase the risk of failure for the entire fixation. Damage to load-bearing structures relative to various implant placements does not impact the resultant overall fixation stability. Therefore, it is not necessary to re-introduce implants in the ideal position, which can lead to reduced patient radiation doses during surgery.

Abstract:Background: Mechanical failure of intramedullary nails is rare. This investigation was prompted by a series of cases of observed breakage of the recently introduced TFNA Proximal Femoral Nailing System (TFN [Trochanteric Fixation Nail]-Advanced; DePuy Synthes) in our region. Laboratory analysis and case data are presented, in contribution to post-market surveillance. Methods: Medical and imaging records from the 3 public tertiary orthopaedic trauma hospitals in Western Australia were reviewed. Relevant data of patients in whom breakage of the TFNA implant occurred between 2016 and 2018 were collected and analyzed. Laboratory review of retrieved implants was conducted at the Centre for Implant Technology and Retrieval Analysis (CITRA) in Western Australia.

Abstract:INTRODUCTION: The increasing socioeconomic need for optimal treatment of hip fractures in combination with the high diversity of available implants has raised numerous biomechanical questions. This study aims to provide a comprehensive overview of biomechanical research on the treatment of intertrochanteric fractures using cephalomedullary devices.METHODS: Following the PRISMA-P guidelines, a systematic literature search was performed on 31.12.2022. The databases PubMed/MEDLINE and Web of Science were searched. Scientific papers published between 01.01.2000 - 31.12.2022 were included when they reported data on implant properties related to the biomechanical stability for intertrochanteric fractures. Data extraction was undertaken using a synthesis approach, gathering data on criteria of implants, sample size, fracture type, bone material, and study results.RESULTS: The initial search identified a total of 1459 research papers, out of which forty-three papers were considered for final analysis. Due to the heterogeneous methods and parameters used in the included studies, meta-analysis was not feasible. A comprehensive assessment of implant characteristics and outcome parameters was conducted through biomechanical analysis. Various factors such as proximal and distal locking, nail diameter and length, fracture model, and bone material were thoroughly evaluated.CONCLUSION: This scoping review highlights the need for standardization in biomechanical studies on intertrochanteric fractures to ensure reliable and comparable results. Strategies such as avoiding varus, maintaining a sufficient tip-apex-distance, cement augmentation, and optimizing lesser trochanteric osteosynthesis enhance construct stability. Synthetic alternatives may offer advantages over cadaveric bone. Further research and meta-analyses are required to establish standardized protocols and enhance reliability.

Mechanical Complications of Proximal Femur Fractures Treated with Intramedullary Nailing: A Retrospective Study

Abstract:Background and Objectives: This retrospective cohort study analyzes mechanical complications in hip fracture surgery using the Trochanteric Fixation Nail-Advanced (TFNA) implant. It investigates the correlation of these complications with demographic, intraoperative, and radiological factors, aiming to identify associated risk factors and suggest improvements in clinical surveillance and treatment strategies. Materials and Methods: We enrolled 253 patients diagnosed with pertrochanteric hip fractures treated between 2017 and 2021, with 126 meeting the criteria for a minimum 6-month follow-up. Data on demographics, American Anesthesia Association Classification (ASA), comorbidities, AO/OTA [AO (Arbeitsgemeinschaft für Osteosynthesefragen)/OTA (Orthopedic Trauma Association)] fracture classification, procedural details, and time to failure were collected. Radiographs were evaluated for reduction quality, the tip-apex distance (TAD), progressive varus deviation, and identification of mechanical complications. Statistical analysis was performed using SPSS software. Results: The predominant AO/OTA fracture classification was 31A2 in 67 cases (52.7%). Reduction quality was deemed good or acceptable in 123 cases (97.6%). The mean time to failure was 4.5 months (range: 2.2-6). The average TAD was 18 mm (range: 1.2-36), with a mean progressive varus deviation of 2.44° (range: 1.30-4.14). A good or acceptable reduction quality was observed in 97.6% of cases. Mechanical complications occurred in 21.4% of patients, with significant associations found with the lateral cortex fracture, use of a TFNA implant with a 130° angle, open reduction, and absence of prior osteoporosis treatment. Conclusions: The study provides insights into mechanical complications in proximal femur fractures treated with the TFNA nail, emphasizing the need for enhanced clinical and radiographic surveillance, especially in patients without osteoporosis treatment. Our findings support the necessity for further clinical studies comparing these outcomes with other implant designs and underscore the importance of personalized treatment strategies to reduce complication rates.
Hip Replacement As Alternative to Intramedullary Nail in Elderly Patients with Unstable Intertrochanteric Fracture: A Systematic Review and Meta-Analysis

Abstract:Objective To evaluate the efficacy and safety of hip replacement and intramedullary nails for treating unstable intertrochanteric fractures in elderly patients. Methods Randomized clinical trials (RCTs) to compare hip replacement with intramedullary nail in the management of elderly patients with unstable intertrochanteric femur fracture were retrieved from Cochrane Library (up to January 2018), CNKI (China National Knowledge Infrastructure), Wanfang Data, PubMed, and Embase. The methodological quality of the included trials was assessed using the Cochrane risk of bias assessment tool, and relevant data was extracted. Statistical analysis was performed by Revman 5.3. Where possible, we performed the limited pooling of data. Results Fourteen trials including a total of 1067 participants aged 65 and above were included for qualitative synthesis and meta-analysis. The methodological quality of the included study was poor. The meta-analysis indicated that the hip replacement group benefited more than the intramedullary nail group in terms of the bearing load time (WMD -14.61, 95% CI -21.51 to -7.7, P < 0.0001), mechanical complications (OR 0.34, 95% CI 0.21 to 0.57, P < 0.0001), and post-operative complications (OR 0.46, 95% CI 0.22 to 0.93, P = 0.03). While the intramedullary nail was superior to arthroplasty regarding the intraoperative blood loss (WMD 58.36, 95% CI 30.77 to 85.94, P < 0.0001). However, there were no statistical significances in the length of surgery (WMD 5.27, 95% CI 4.23 to 14.77, P = 0.28), units of blood transfusion (WMD 0.34, 95% CI -0.16 to 0.85, P = 0.18), length of hospital stay (WMD -1.00, 95% CI -2.93 to 0.93, P = 0.31), Harris hip score (WMD 0.31, 95% CI -0.39 to 1.01, P = 0.38), and mortality (OR 1.24, 95% CI 0.12 to 13.10, P = 0.86). Conclusions This systematic review and meta-analysis provided evidence for the efficacy and safety of hip replacement and intramedullary nail in treating unstable intertrochanteric fractures. However, the results should be interpreted cautiously because of methodological limitations and publication bias.

Abstract:Background: Trochanteric fractures almost invariably occur as a result of a trivial fall involving both direct and indirect forces. Fixation in the geriatric population generally consists of weakened, osteoporotic bone; intramedullary devices (PFNA) carry an advantage over other load sharing devices by not having to depend on plate fixation with bone screws purchasing a compromised lateral cortex. The purpose of the present study is to verify the theoretical advantages of the proximal femoral nail in elderly osteoporotic patients and eventually functional outcome of the patient. Method: A total of 30 patients with age >60 years with intertrochanteric femur fractures managed with proximal femoral nail anti-rotation for prospective study. Harris hip score had been used in our study for regular follow up and evaluation at each and every follow up visit. Results: Average Harris hip score at the end of study showed mean value of 87, ranged from 65 to 94 with almost 73% patients showing excellent or good outcome. And 100% fractures got united with a good component position and average time to bone healing was 14 weeks. Conclusions: PFNA are now favored in western countries and there are multiple studies coming from that region to support this. Due to advantages of high union rate, early postoperative mobilization, and short operation time, PFNA osteosynthesis is the method of choice for surgical treatment of stable and unstable osteoporotic intertrochanteric femoral fractures.

Antegrade vs Retrograde Intra-Medullary Nailing in Femoral Shaft Fractures: A Systematic Review and Meta-Analysis

Abstract:Objectives: Femoral shaft fractures are one of the most prevalent fractures found in clinical practice. Numerous operative and non-operative options are readily available for the treatment of such fractures with intra-medullary nailing being the gold standard. To date, no consensus has been reached favoring one approach over the other. Thus, this meta-analysis aims to compare the outcomes between an antegrade and retrograde intra-medullary nailing for the treatment of femoral shaft fractures. Methods: PubMed, Cochrane, Google Scholar (page 1-20), and Embase were searched till January 2024. The clinical outcomes evaluated were the incidence of adverse events, reoperations, hip and knee pain, and surgery-related parameters. Results: Higher rates of hip pain, and heterotopic ossification (p=0.0003, and p=0.0002 respectively) was observed with antegrade nailing. However, a higher rate of knee pain (p=0.02) was appreciated in retrograde nailing. There was no statistically significant difference in the remaining analyzed outcomes such as operative time, reoperation rate or other complications. Conclusion: Despite a higher rate of heterotopic ossification using the antegrade nailing technique, both the antegrade and retrograde nailing techniques yield overall similar outcomes. Therefore, the decision to choose one or the other should be based on patient-related factors, and the surgeon's experience and preference.
Anterior Cortical Impingement With 240 mm Cephalo-Medullary Nail (PFN and PFNA-2) in Intertrochanteric Femur Fracture Fixation in the Indian Population: A Problem Statement

Abstract:Trochanteric fractures are common in the elderly population, and their incidence increases twice every decade after age 50. Intramural fixation has achieved good clinical efficacy in the treatment of unstable trochanteric fractures, but there have been complications reported in the literature in the Asian population. Most complications arise from a mismatch between the increase in the anterior femoral bow with advancing age and the proximal femoral nails (PFN) on the market, which still have straight designs on the sagittal plane. The non-anatomic shapes of the PFNs sometimes make the surgeries difficult or may lead to an inadvertent intraoperative fracture around the tip of the nail, particularly if they impinge on the anterior cortex of the femur. The entry point on the greater trochanter was divided into three equal parts, i.e., anterior 1/3rd, middle 1/3rd, and posterior 1/3rd on the lateral X-rays. Patients with posterior 1/3rd entry were excluded from the study as it is known that posterior positioning of nail entry can cause an increased incidence of anterior nail impingement. The AI was measured using the best available preoperative lateral roentgenogram of the femur using the incidence cortex (AI cortex) angle. This angle was measured using two tangential lines drawn parallel to the anterior cortex of the femur, proximal and distal to the most bowed point of the femur. We recommend that there is a need to introduce anterior curvature in the sagittal plane corresponding to the femoral bow in a 240 mm cephalomedullary nail to decrease complications. We also consider the use of either a short (i.e., 180 mm) or a long cephalomedullary nail in the Indian population, as the height of the population is shorter as compared to the western population, and the role of a 240 mm cephalomedullary nail is doubtful in the Indian population.

Abstract:Background: Intertrochanteric fracture of femur is a common fracture in the elderly, and the number of intramedullary nails used to manage intertrochanteric fractures has steadily increased, but the evidence for the clinical efficacy of this practice is lacking. The present study retrospectively compared the clinical outcomes and imaging features of patients with unstable intertrochanteric hip fractures treated with traditional extramedullary hip screws (AO/OTA 31-A2) and patients with the same injury treated with newer intramedullary screws. Methods: Lower limb measurement (LLM) for the main results of the hip-specific tools, functional independence measure (FIM), and "timed up and go" (TUG) to test the scale and timing of the 2-minute walk test were used as secondary tools for clinical outcomes. Other detailed radiological parameters to evaluate the fracture movement, such as heterotopic ossification, and implant failure, were also recorded. Results: There were no significant differences between the intramedullary and extramedullary treatment groups in terms of the measures acquired by the primary or secondary clinical outcome tools (P>0.05), but the radiographic parameters favored the intramedullary treatment group as it showed reduced femoral neck shortening. Conclusions: Although treatment with intramedullary nailing resulted in better radiographic assessment results, this did not translate into better functional recovery outcomes.

Abstract:Objective To analyze the clinical effect of INTERTAN intramedullary nail and PFNA-Ⅱ intramedullary nail in the treatment of the elderly femoral intertrochanteric fracture. Methods Since October 2010 to August 2014, there were 128 cases of the elderly patients with femoral intertrochanteric fracture in our orthopedics, they were treated. According to the AO classification standard to classify, type A1 50 cases, type A2 57 cases and type A3 in 21 cases. Among them 69 cases were treated by using Smith﹠Nephew company INTERTAN intramedullary nail fixation, 59 cases were treated by using Synthes PFNA-Ⅱ intramedullary nail fixation. Compared between two groups patients operation time, amount of bleeding during the operation, incision length, the number of cases of postoperative blood transfusion and volume of blood transfusion, healing time of fracture, internal fixation failure cases, after three months thigh pain and the last follow-up of hip joint function Harris score. Two groups of data, measurement data (operation time, amount of bleeding during the operation, incision length, volume of blood transfusion, healing time of fracture) used t test, numeration data (the number of the postoperative blood transfusion, internal fixation failure cases, after three months thigh pain and the last follow-up of hip joint function Harris score) used chi-square test. Results All the patients were followed up for 8 to 16 months, with an average (10.33±2.75) months. PFNA-Ⅱ group operation time were (50.00±7.82) min, blood loss were (74.00±48.35) ml and postoperative blood transfusion amount were (1.60±0.70) U, were less than that of INTERTAN group, the number of cases of internal fixation failure of INTERTAN group 6 cases was less than PFNA-Ⅱ group 1 case (P<0.05), two groups of length of incision INTERTAN group (4.00±0.94) cm and PFNA-Ⅱ group (3.80±0.79) cm, fracture healing time INTERTAN group (5.20±0.92) months and PFNA-Ⅱ group (5.30±0.95) months, postoperative blood transfusion cases INTERTAN group 41 cases and PFNA-Ⅱ group 38 cases, after 3 months of thigh pain degree INTERTAN group and PFNA-Ⅱgroup, the last follow-up of hip joint function Harris score INTERTAN group excellent 45 cases, good 19 cases, can 3 cases, poor 2 cases and PFNA-Ⅱ group excellent 35 cases, good 15 cases, can 5 cases, poor 4 cases were found no significant differences (P>0.05). Conclusions PFNA-Ⅱ intramedullary nail and INTERTAN intramedullary nail were intramedullary fixation, each has its advantages. PFNA-Ⅱintramedullary nail has the advantages of simple operation, it is suitable for more medical complications and more difficult for a long time operation, INTERTAN intramedullary nail theoretically has higher biomechanical advantages and stability, it can reduce postoperative pain in the thigh. For anti-intertrochanteric fractures, we suggest to use extended intramedullary nailing fixation, it can reduce the local stress concentration and the risk of re-fracture, A2.2 type and more unstable intertrochanteric fracture preferred INTERTAN intramedullary nailing, reduce nail to cut out.
