Abstract:Background To study the clinical characteristics, early diagnosis and treatment of traumatic renal artery thrombosis so as to recommend how to diagnose, treat and prevent traumatic renal artery thrombosis. Methods Summarize the clinical data of 10 patients with traumatic renal artery thrombosis and analyzed pathogenesis, clinical characteristics and the diagnosis and treatment procedure. Results The left renal artery thrombosis was 6 cases and the right renal artery thrombosis was 4 cases. Ultrasound reported that renal has reduced blood flow signal in one case and was confirmed by enhanced CT. The others were directly diagnosis renal artery embolization by enhanced CT scan. 4 cases of patients were treated with low molecular weight heparin calcium. Although the follow-up CT scan showed that injured renal had no obvious blood perfusion, the renal function was in normal range. 2 patients had renal hypertension, 1 case had poor control of blood pressure with drugs, so nephrectomy was performed. Conclusions Clinical symptoms and laboratory examinations of traumatic renal artery thrombosis had no specificity, color ultrasound is the early diagnosis method and enhanced CT scan is the effective mean of diagnosis. The early recovery of renal blood is effective treatment. Major concerns are renal function and blood pressure in follow up.
PixelPrint4D: A 3D printing method of fabricating patient-specific deformable CT phantoms for respiratory motion applications.

Abstract:All in-vivo medical imaging is impacted by patient motion, especially respiratory motion, which has a significant influence on clinical workflows in diagnostic imaging and radiation therapy. Many technologies such as motion artifact reduction and tumor tracking algorithms have been developed to compensate for respiratory motion during imaging. To assess these technologies, respiratory motion phantoms (RMPs) are required as preclinical testing environments, for instance, in computed tomography (CT). However, current RMPs are highly simplified and do not exhibit realistic tissue structures or deformation patterns. With the rise of more complex motion compensation technologies such as deep learning-based algorithms, there is a need for more realistic RMPs. This work introduces PixelPrint4D, a 3D printing method designed to fabricate lifelike, patient-specific deformable lung phantoms for CT imaging. The phantom demonstrated accurate replication of patient lung structures, textures, and attenuation profiles. Furthermore, it exhibited accurate nonrigid deformations, volume changes, and attenuation changes under compression. PixelPrint4D enables the production of highly realistic RMPs, surpassing existing models to offer more robust testing environments for a diverse array of novel CT technologies.

Abstract:Abstract The fabrication of multi-material medical phantoms with both patient-specificity and realistic mechanical properties is of great importance for the development of surgical planning and medical training. In this work, a 3D multi-material printing system for medical phantom manufacturing was developed. Rigid and elastomeric materials are firstly combined in such application for an accurate tactile feedback. The phantom is designed with multiple layers, where silicone ink, Thermoplastic Polyurethane (TPU), and Acrylonitrile Butadiene Styrene (ABS) were chosen as printing materials for skin, soft tissue, and bone, respectively. Then, the printed phantoms were utilized for the investigation of needle-phantom interaction by needle insertion experiments. The mechanical needle-phantom interaction was characterized by skin-soft tissue interfacial puncture force, puncture depth, and number of insertion force peaks. The experiments demonstrated that the manufacturing conditions, i.e. the silicone grease ratio, interfacial thickness and the infill rate, played effective roles in regulating mechanical needle-phantom interaction. Moreover, the influences of material properties, including interfacial thickness and ultimate stress, on needle-phantom interaction were studied by finite element simulation. Also, a patient-specific forearm phantom was printed, where the anatomical features were acquired from Computed Tomography (CT) data. This study provided a potential manufacturing method for multi-material medical phantoms with tunable mechanical properties and offered guidelines for better phantom design.

Abstract:A twenty-year-old white man, with acute dissection of the ascending aorta, underwent emergency resection with dacron graft replacement, utlizing cardiopulmonary bypass. Subsequent repair of coarctation of the thoracic aorta was performed. No blood was administered due to the patient's religious preference. The problems of emergency surgery in Jehovah's Witness patients are discussed.

Abstract:Objectives. This study sought to evaluate food consumption patterns in the context of food security in two Yukon First Nations communities. Study design. Twenty-nine members of Vuntut Gwitchin households in Old Crow and 33 members of Tlingit households in Teslin participated in individual interviews. Methods. Food frequency questionnaires were used to quantify traditional food consumption throughout the spring 2007 and winter 2008 and to identify potential temporal trends through a comparison with data from the early 1990s. Additional questions, including the Health Canada Household Food Security Survey Module, sought to assess food security concerns in each community. Results. Overall frequency of traditional food consumption did not change in either community from the 2 time-point analyses. There was, however, a difference in frequency of consumption of certain groups of foods, and this highlighted the degree to which environmental variability affects the availability of foods Conclusion. The importance of traditional foods in the diet of Yukon First Nations has not changed over the past 15 years. However, limited availability of food species, access to harvesting equipment and decrease in available time to go out on the land to harvest are food security challenges facing households today.
Lifelike PixelPrint phantoms for assessing clinical image quality and dose reduction capabilities of a deep learning CT reconstruction algorithm

Abstract:Deep learning CT reconstruction (DLR) has become increasingly popular as a method for improving image quality and reducing radiation exposure. Due to their nonlinear nature, these algorithms result in resolution and noise performance which are object-dependent. Therefore, traditional CT phantoms, which lack realistic tissue morphology, have become inadequate for assessing clinical imaging performance. We propose to utilize 3D-printed PixelPrint phantoms, which exhibit lifelike attenuation profiles, textures, and structures, as a better tool for evaluating DLR performance. In this study, we evaluate a DLR algorithm (Precise Image (PI), Philips Healthcare) using a custom PixelPrint lung phantom and perform head-to-head comparisons between DLR, iterative reconstruction, and filtered back projection (FBP) with scans acquired at a broad range of radiation exposures (CTDIvol: 0.5, 1, 2, 4, 6, 9, 12, 15, 19, and 20 mGy). We compared the performance of each resultant image using noise, peak signal to noise ratio (PSNR), structural similarity index (SSIM), feature-based similarity index (FSIM), information theoretic-based statistic similarity measure (ISSM) and universal image quality index (UIQ). Iterative reconstruction at 9 mGy matches the image quality of FBP at 12 mGy (diagnostic reference level) for all metrics, demonstrating a dose reduction capability of 25%. Meanwhile, DLR matches the image quality of diagnostic reference level FBP images at doses between 4 - 9 mGy, demonstrating dose reduction capabilities between 25% and 67%. This study shows that DLR allows for reduced radiation dose compared to both FBP and iterative reconstruction without compromising image quality. Furthermore, PixelPrint phantoms offer more realistic testing conditions compared to traditional phantoms in the evaluation of novel CT technologies. This, in turn, promotes the translation of new technologies, such as DLR, into clinical practice.

Abstract:Arthroplasty phantoms, including total knee replacement (TKR) phantoms, have been frequently used to test metal artefact reduction methods applied to positron emission tomography/computed tomography (PET/CT) images. These phantoms generally simulate either simple anatomical features or simple activity distribution around the metal inserts in the PET/CT scans. 3D printing has been used recently to fabricate fillable anthropomorphic phantoms that accurately simulate volume and geometry. This study aims to describe the process of image segmentation, phantom modelling, 3D printing and validation of a population-based fillable TKR phantom that simulates human TKR PET/CT metal artefacts.

Abstract:Recently, 3D printing has been widely used to fabricate medical imaging phantoms. So far, various rigid 3D printable materials have been investigated for their radiological properties and efficiency in imaging phantom fabrication. However, flexible, soft tissue materials are also needed for imaging phantoms for simulating several clinical scenarios where anatomical deformations is important. Recently, various additive manufacturing technologies have been used to produce anatomical models based on extrusion techniques that allow the fabrication of soft tissue materials. To date, there is no systematic study in the literature investigating the radiological properties of silicone rubber materials/fluids for imaging phantoms fabricated directly by extrusion using 3D printing techniques. The aim of this study was to investigate the radiological properties of 3D printed phantoms made of silicone in CT imaging. To achieve this goal, the radiodensity as described as Hounsfield Units (HUs) of several samples composed of three different silicone printing materials were evaluated by changing the infill density to adjust their radiological properties. A comparison of HU values with a Gammex Tissue Characterization Phantom was performed. In addition, a reproducibility analysis was performed by creating several replicas for specific infill densities. A scaled down anatomical model derived from an abdominal CT was also fabricated and the resulting HU values were evaluated. For the three different silicone materials, a spectrum ranging from −639 to +780 HU was obtained on CT at a scan setting of 120 kVp. In addition, using different infill densities, the printed materials were able to achieve a similar radiodensity range as obtained in different tissue-equivalent inserts in the Gammex phantom (238 HU to −673 HU). The reproducibility results showed good agreement between the HU values of the replicas compared to the original samples, confirming the reproducibility of the printed materials. A good agreement was observed between the HU target values in abdominal CT and the HU values of the 3D-printed anatomical phantom in all tissues.

Abstract:Biomedical optical imaging is playing an important role in diagnosis and treatment of various diseases. However, the accuracy and the reproducibility of an optical imaging device are greatly affected by the performance characteristics of its components, the test environment, and the operations. Therefore, it is necessary to calibrate these devices by traceable phantom standards. However, most of the currently available phantoms are homogeneous phantoms that cannot simulate multimodal and dynamic characteristics of biological tissue. Here, we show the fabrication of heterogeneous tissue-simulating phantoms using a production line integrating a spin coating module, a polyjet module, a fused deposition modeling (FDM) module, and an automatic control framework. The structural information and the optical parameters of a "digital optical phantom" are defined in a prototype file, imported to the production line, and fabricated layer-by-layer with sequential switch between different printing modalities. Technical capability of such a production line is exemplified by the automatic printing of skin-simulating phantoms that comprise the epidermis, dermis, subcutaneous tissue, and an embedded tumor.

Abstract:Objective Deep learning reconstruction (DLR) algorithms exhibit object-dependent resolution and noise performance. Thus, traditional geometric CT phantoms cannot fully capture the clinical imaging performance of DLR. This study uses a patient-derived 3D-printed PixelPrint lung phantom to evaluate a commercial DLR algorithm across a wide range of radiation dose levels. Method The lung phantom used in this study is based on a patient chest CT scan containing ground glass opacities and was fabricated using PixelPrint 3D-printing technology. The phantom was placed inside two different size extension rings to mimic a small- and medium-sized patient and was scanned on a conventional CT scanner at exposures between 0.5 and 20 mGy. Each scan was reconstructed using filtered back projection (FBP), iterative reconstruction, and DLR at five levels of denoising. Image noise, contrast to noise ratio (CNR), root mean squared error (RMSE), structural similarity index (SSIM), and multi-scale SSIM (MS SSIM) were calculated for each image. Results DLR demonstrated superior performance compared to FBP and iterative reconstruction for all measured metrics in both phantom sizes, with better performance for more aggressive denoising levels. DLR was estimated to reduce dose by 25-83% in the small phantom and by 50-83% in the medium phantom without decreasing image quality for any of the metrics measured in this study. These dose reduction estimates are more conservative compared to the estimates obtained when only considering noise and CNR. Conclusion DLR has the capability of producing diagnostic image quality at up to 83% lower radiation dose, which can improve the clinical utility and viability of lower dose CT scans. Furthermore, the PixelPrint phantom used in this study offers an improved testing environment with more realistic tissue structures compared to traditional CT phantoms, allowing for structure-based image quality evaluation beyond noise and contrast-based assessments.

Abstract:Medical imaging phantoms are widely used for validation and verification of imaging systems and algorithms in surgical guidance and radiation oncology procedures. Especially, for the performance evaluation of new algorithms in the field of medical imaging, manufactured phantoms need to replicate specific properties of the human body, e.g., tissue morphology and radiological properties. Additive manufacturing (AM) technology provides an inexpensive opportunity for accurate anatomical replication with customization capabilities. In this study, we proposed a simple and cheap protocol to manufacture realistic tumor phantoms based on the filament 3D printing technology. Tumor phantoms with both homogenous and heterogenous radiodensity were fabricated. The radiodensity similarity between the printed tumor models and real tumor data from CT images of lung cancer patients was evaluated. Additionally, it was investigated whether a heterogeneity in the 3D printed tumor phantoms as observed in the tumor patient data had an influence on the validation of image registration algorithms. A density range between -217 to 226 HUs was achieved for 3D printed phantoms; this range of radiation attenuation is also observed in the human lung tumor tissue. The resulted HU range could serve as a lookup-table for researchers and phantom manufactures to create realistic CT tumor phantoms with the desired range of radiodensities. The 3D printed tumor phantoms also precisely replicated real lung tumor patient data regarding morphology and could also include life-like heterogeneity of the radiodensity inside the tumor models. An influence of the heterogeneity on accuracy and robustness of the image registration algorithms was not found.

Abstract:3D printing technology has rapidly spread for decades, allowing the fabrication of medical implants and human phantoms and revolutionizing healthcare. The objective of this study is to evaluate some radiological properties of commercially available 3D printing materials as potential tissue mimicking materials. Among fifteen materials, we compared their properties with nine human tissues. In all materials and tissues, exposure and energy absorption buildup factors were calculated for photon energies between 0.015 and 15 MeV and penetration depths up to 40 mean free path. Furthermore, the Geant4 Monte Carlo toolkit (version 10.5) was used to simulate their percentage depth dose distributions. In addition, equivalent atomic numbers, effective atomic numbers, attenuation coefficients, and CT numbers have been examined. All parameters were considered in calculating the average relative error ( σ ), which was used as a statistical comparison tool. With σ between 6 and 7, we found that Polylactic Acid (PLA) was capable of simulating eye lenses, blood, soft tissue, lung, muscle, and brain tissues. Moreover, Polymethacrylic Acid (PMAA) material has a σ value of 4 when modeling adipose and breast tissues, respectively. Aside from that, variations in 3D printing materials' infilling percentage can affect their CT numbers. We therefore suggest the PLA for mimicking soft tissue, muscle, brain, eye lens, lung and blood tissues, with an infill of between 92.7 and 94.3 percent. We also suggest an 89 percent infill when simulating breast tissue. Furthermore, with a 96.7 percent infill, the PMAA faithfully replicates adipose tissue. Additionally, we found that a 59 percent infill of Fe-PLA material is comparable to cortical bone. Due to the benefits of creating individualized medical phantoms and equipment, the results might be seen as an added value for both patients and clinicians.

Abstract:Clinical utility of many biomedical optical devices is limited by the lack of effective and traceable calibration methods. Optical phantoms that simulate biological tissues used for optical device calibration have been explored. However, these phantoms can hardly simulate both structural and optical properties of multi-layered biological tissue. To address this limitation, we develop a 3D printing production line that integrates spin coating, light-cured 3D printing and Fused Deposition Modeling (FDM) for freeform fabrication of optical phantoms with mechanical and optical heterogeneities. With the gel wax Polydimethylsiloxane (PDMS), and colorless light-curable ink as matrix materials, titanium dioxide (TiO2) powder as the scattering ingredient, graphite powder and black carbon as the absorption ingredient, a multilayer phantom with high-precision is fabricated. The absorption and scattering coefficients of each layer are measured by a double integrating sphere system. The results demonstrate that the system has the potential to fabricate reliable tissue-simulating phantoms to calibrate optical imaging devices.
3D printing surgical phantoms and their role in the visualization of medical procedures

Abstract:The ongoing growth of three-dimensional (3D) printing has started to expand into the medical field. To date, the main challenge is a lack of information surrounding the materials which accurately mimic soft tissue, and how they can be reproduced into a surgical phantom for medical use. This study reports on successful materials for simulating soft tissue, and the methods in which they can be printed to create surgical phantoms. Noteworthy materials which have been reported in literature as having good concordance with soft tissue mechanical properties have been identified as silicone, gelatin, polyvinyl alcohol (PVA), and Stratasys-manufactured TangoPlus. Four printing techniques, namely material extrusion, material jetting, photopolymerization, and powder bed fusion, are discussed and reviewed on their suitability for fabricating phantoms, and a summary of their use in literature has been presented in tabular format. This study explores the current uses of 3D-printed phantoms for surgical training, surgical planning and patient understanding and discusses ways in which this could be advanced in the future.
Dosimetric characteristics of 3D-printed and epoxy-based materials for particle therapy phantoms

Abstract:Objective: 3D printing has seen use in many fields of imaging and radiation oncology, but applications in (anthropomorphic) phantoms, especially for particle therapy, are still lacking. The aim of this work was to characterize various available 3D printing methods and epoxy-based materials with the specific goal of identifying suitable tissue surrogates for dosimetry applications in particle therapy. Methods: 3D-printed and epoxy-based mixtures of varying ratios combining epoxy resin, bone meal, and polyethylene powder were scanned in a single-energy computed tomography (CT), a dual-energy CT, and a μCT scanner. Their CT-predicted attenuation was compared to measurements in a 148.2 MeV proton and 284.7 MeV/u carbon ion beam. The sample homogeneity was evaluated in the respective CT images and in the carbon beam, additionally via widening of the Bragg peak. To assess long-term stability attenuation, size and weight measurements were repeated after 6–12 months. Results: Four 3D-printed materials, acrylonitrile butadiene styrene polylactic acid, fused deposition modeling printed nylon, and selective laser sintering printed nylon, and various ratios of epoxy-based mixtures were found to be suitable tissue surrogates. The materials' predicted stopping power ratio matched the measured stopping power ratio within 3% for all investigated CT machines and protocols, except for μCT scans employing cone beam CT technology. The heterogeneity of the suitable surrogate samples was adequate, with a maximum Bragg peak width increase of 11.5 ± 2.5%. The repeat measurements showed no signs of degradation after 6–12 months. Conclusion: We identified surrogates for soft tissue and low- to medium-density bone among the investigated materials. This allows low-cost, adaptable phantoms to be built for quality assurance and end-to-end tests for particle therapy.

Abstract:Abstract Objective . This work describes an approach for producing physical anthropomorphic breast phantoms from clinical patient data using three-dimensional (3D) fused-deposition modelling (FDM) printing. Approach . The source of the anthropomorphic model was a clinical Magnetic Resonance Imaging (MRI) patient image set, which was segmented slice by slice into adipose and glandular tissues, skin and tumour formations; thus obtaining a four component computational breast model. The segmented tissues were mapped to specific Hounsfield Units (HU) values, which were derived from clinical breast Computed Tomography (CT) data. The obtained computational model was used as a template for producing a physical anthropomorphic breast phantom using 3D printing. FDM technology with only one polylactic acid filament was used. The physical breast phantom was scanned at Siemens SOMATOM Definition CT. Quantitative and qualitative evaluation were carried out to assess the clinical realism of CT slices of the physical breast phantom. Main results . The comparison between selected slices from the computational breast phantom and CT slices of the physical breast phantom shows similar visual x-ray appearance of the four breast tissue structures: adipose, glandular, tumour and skin. The results from the task-based evaluation, which involved three radiologists, showed a high degree of realistic clinical radiological appearance of the modelled breast components. Measured HU values of the printed structures are within the range of HU values used in the computational phantom. Moreover, measured physical parameters of the breast phantom, such as weight and linear dimensions, agreed very well with the corresponding ones of the computational breast model. Significance . The presented approach, based on a single FDM material, was found suitable for manufacturing of a physical breast phantom, which mimics well the 3D spatial distribution of the different breast tissues and their x-ray absorption properties. As such, it could be successfully exploited in advanced x-ray breast imaging research applications.

Abstract:Mammography is currently the standard imaging modality used to screen women for breast abnormalities, and, as a result, it is a tool of great importance for the early detection of breast cancer. Physical phantoms are commonly used as surrogates of breast tissue to evaluate some aspects of the performance of mammography systems. However, most phantoms do not reproduce the anatomic heterogeneity of real breasts. New fabrication technologies, such as three-dimensional (3-D) printing, have created the opportunity to build more complex, anatomically realistic breast phantoms that could potentially assist in the evaluation of mammography systems. The reproducibility and relative low cost of 3-D printed objects might also enable the development of collections of representative patient models that could be used to assess the effect of anatomical variability on system performance, hence making bench testing studies a step closer to clinical trials. The primary objective of this work is to present a simple, easily reproducible methodology to design and print 3-D objects that replicate the attenuation profile observed in real two-dimensional mammograms. The secondary objective is to evaluate the capabilities and limitations of the competing 3-D printing technologies and characterize the x-ray properties of the different materials they use. Printable phantoms can be created using the open-source code introduced, which processes a raw mammography image to estimate the amount of x-ray attenuation at each pixel, and outputs a triangle mesh object that encodes the observed attenuation map. The conversion from the observed pixel gray value to a column of printed material with equivalent attenuation requires certain assumptions and knowledge of multiple imaging system parameters, such as x-ray energy spectrum, source-to-object distance, compressed breast thickness, and average breast material attenuation. To validate the proposed methodology, x-ray projections of printed phantoms were acquired with a clinical mammography system. The quality of the printing process was evaluated by comparing the mammograms of the printed phantoms and the original mammograms used to create the phantoms. The structural similarity index and the root-mean-square error were used as objective metrics to compare the two images. A detailed description of the software, a characterization of the printed materials using x-ray spectroscopy, and an evaluation of the realism of the sample printed phantoms are presented.
