Abstract:Journal of International Medical Research, Volume 52, Issue 9, September 2024. The penetration of a peritoneal dialysis catheter into the intestinal cavity is a clinically rare complication. In the present retrospective clinical case series, 11 patients with uraemia who received continuous ambulatory peritoneal dialysis and attended hospital between 2019 and 2023 are described. The median patient age was 61.91 ± 11.33 years. All patients had previously experienced peritoneal dialysis-related peritonitis and were clinically cured by infusing sensitive antibiotics into the abdominal cavity. Colonoscopy was utilised to locate the penetrating catheter and close the perforation with a titanium clip once the catheter had been removed via an external approach. Following a 2–4-week fast, the perforations healed in all 11 patients. The present authors' experience illustrates that directly removing the catheter and clamping the perforation opening under the guidance of colonoscopy is simple to operate with few complications compared with traditional open surgery.

Abstract:The indwelling urinary catheter is the leading cause of complicated urinary tract infections and Gram-negative bacteraemia in this age group. It accounts for about 40% of life-threatening septicaemia. There is a progressive increase in mortality independently associated with the duration of catheterization. Polymicrobial bacteriuria is common. Urease-producing bacteria lead to encrusted and blocked catheters. The current challenges are to develop effective methods to sensitize healthcare workers to avoid the routine use of indwelling catheters, remove them when no longer needed, develop alternative methods for care of incontinence, employ non-invasive methods to measure urine output, and improve urine drainage systems. The research paradigm needs to focus on prevention of catheter-associated infections rather than on futile attempts to treat irreversible sepsis.

Abstract:Erosion of the peritoneal dialysis (PD) catheter into the bowel is very rare. Some patients experience serious consequences such as peritonitis, difficulties in draining, and feculent dialysate effluent with watery diarrhea, but some patients develop no symptoms. Most cases have occurred upon insertion of a stylet-catheter. Perforation by a PD catheter is distinctly unusual. Here, we report a 72-year-old man who presented with a plastic tube protruding from the anus after defecating. We diagnosed the patient with delayed rectal perforation by a PD catheter. We compare our patient with similar patients identified by a search of the English-language literature indexed at PubMed (keywords: “perforation,” “erosion,” “peritoneal dialysis,” “catheter”) and a search for references within articles identified in the primary search.

Abstract:PURPOSE:This is a single-center, descriptive report of the management and complications of venous catheter use in 19 severely burned passengers from a bus fire that occurred on July 5, 2014, in Hangzhou.METHODS:We recorded the parameters of the catheters insertion and indwelling. Sampling of each removed catheter was conducted to monitor for catheter-related infections. Bedside ultrasound screening was performed for recording central venous catheter (CVC)-related complications.RESULTS:Of the 174 venous accesses placed, 108 were CVCs. 27 (25.0%) catheter tip cultures (CTC) were positive; 12 (11.1%) were catheter-related blood stream infections (CRBSI). Acinetobacter baumannii was the most prominent bacterial infection for both CTC- (55.56%) and CRBSI- (75.00%) positive catheters. CTC- and CRBSI-positive rates were higher during the emergency stage, and both dropped rapidly after reform measures (chi-square test, p = 0.003), and all were negative after the no.8 catheters. Accumulative regression results indicated that total body surface area burned (TBSA), number of catheters, days of indwelling, and bloodstream infections were independently associated with CTC results, while gender and number of catheters were independently associated with CRBSI results. 1 femur vein thrombosis was detected and cured.CONCLUSIONS:Bedside ultrasound and professional IV team for CVC management are pivotal for massive burn victims. Their intervention helps control CVC-related infections and other complications. A. baumannii was the most frequent bacterial infection found in both CTC- and CRBSI-positive catheters. Several most important factors associated with catheter-related infections were concluded. This information alerts us to watch for patients with such warning factors.

Abstract:Rationale: Currently, percutaneous catheter drainage (PCD) is regarded as the first-line treatment modality of pyogenic liver abscess. Severe complications associated with PCD were uncommon. Hepatic rupture is an uncommon but life-threatening liver trauma with high mortality. Its management is challenging because a delay in the diagnosis may lead to fatal hemorrhagic shock. To our knowledge, PCD-associated hepatic rupture has never been reported. Patient concerns: We report herein a rare case of PCD-associated hepatic rupture. Its clinical courses and our therapeutic approaches are presented. Moreover, the clinical significance, underlying causes, and current views on severe liver trauma management will be discussed briefly. Diagnoses: A diabetic patient suffering from fever and malaise was diagnosed with a pyogenic liver abscess. PCD was performed because intravenous antibiotics were ineffective. The patient developed a liver rupture following PCD, with clinical and imaging confirmation but without further progression. Interventions: Surgical repair and vascular intervention were both inappropriate. As a result, medical treatments with supportive care were adopted and were found to be effective. Outcomes: The patient's condition improved gradually, with stabilized imaging and laboratory performance. He recovered uneventfully during follow-ups. Lessons: Hepatic rupture should be listed as an extremely rare but severe complication of PCD. Immediate suspicion and effective intervention may avoid an unfavorable consequence.

Abstract:Background: An infected aortic aneurysm is a rare and life-threatening vascular condition with a high incidence of arterial rupture and recurrence even after treatment. One of the most common causes of an infected aortic aneurysm is catheter-related bloodstream infection. Although infection due to indwelling catheters is possible, the incidence of this is rare, especially for long-term implanted arterial catheters. Case presentation: A 78-year-old Japanese man with a past medical history of rectal cancer with metastasis to the liver presented to our hospital as a result of low back pain. Remission had been achieved following surgery and adjuvant chemotherapy via an implanted catheter for arterial infusion. However, the original catheter that was inserted from the femoral artery to the hepatic artery via the celiac artery was still present more than 10 years after diagnosis, without being replaced, in case of a recurrence. On the day of admission, computed tomography scan of the chest and abdomen with contrast revealed an irregularly shaped aortic aneurysm at the origin of the celiac artery and a partially expanded common hepatic artery with disproportionate fat stranding along the implanted arterial catheter without extravasation. Although the initial impression was an impending rupture of the acute thoracoabdominal aortic aneurysm, a catheter-related infection was considered as a differential diagnosis. Surgery was performed, which revealed a catheter-related infected aortic aneurysm based on images along the catheter, pus cultures, and tissue pathology examination results. Conclusions: This is an extremely rare case of an infectious aneurysm caused by prolonged implantation of an arterial catheter for chemotherapy. It should be noted that an indwelling arterial catheter not only causes bloodstream infections but can also cause an infection of a thoracoabdominal aortic aneurysm.

Abstract:BACKGROUND: Spontaneous bladder rupture is relatively rare, and common causes of spontaneous bladder rupture include bladder diverticulum, neurogenic bladder dysfunction, gonorrhea infection, pelvic radiotherapy, etc. Urinary bladder perforation caused by urinary catheterization mostly occurs during the intubation process.CASE SUMMARY: Here, we describe an 83-year-old male who was admitted with 26 h of middle and upper abdominal pain and a history of long-term catheterization. Physical examination and computed tomography of the abdomen supported the diagnosis of diffuse peritonitis, most likely from a perforated digestive tract organ. Laparoscopic exploration revealed a possible digestive tract perforation. Finally, a perforation of approximately 5 mm in diameter was found in the bladder wall during laparotomy. After reviewing the patient's previous medical records, we found that 1 year prior the patient underwent an ultrasound examination showing that the end of the catheter was embedded into the mucosal layer of the bladder. Therefore, the bladder perforation in this patient may have been caused by the chronic compression of the urinary catheter against the bladder wall.CONCLUSION: For patients with long-term indwelling catheters, there is a possibility of bladder perforation, which needs to be dealt with quickly.

Abstract:Background: Patients in the urology ward often require peripheral intravenous catheters for cancer chemotherapy, antibiotics to treat urinary tract infections, surgery, and other treatments. Moreover, as the elderly population within urology has a high incidence of cancer, many patients also have fragile vasculature. Proper intravenous catheter insertion and management are crucial in such cases. However, no observational studies have revealed specific practices and patient outcomes related to intravenous catheter insertion and management in urology wards. This study aimed to clarify the actual situation of peripheral intravenous catheter placements in patients hospitalized in a general urology ward and investigate the risk factors for patient outcomes such as difficult intravenous access (DIVA) and catheter failure (CF). Methods: This was a prospective observational study. Medical record data were collected, and the actual catheter placement situation was described. A multivariate logistic regression analysis was performed to investigate the risk factors for DIVA and CF. Results: This study analyzed 180 catheters in 99 patients. The median age of the group was 70 years. The prevalence of DIVA and CF were 9.9% and 35.2%, respectively. A history of intravenous chemotherapy for cancer was positively associated with both DIVA and CF. Conclusion: This study's results demonstrate the crucial need for careful insertion and management of intravenous catheters in patients admitted to urology wards. These findings can help identify patients at high risk of DIVA and CF, while hospitalized in the urology ward.

Abstract:BACKGROUND Colorectal foreign bodies are commonly encountered during surgery. They are frequently observed in men 20 to 90 years of age and have bimodal age distribution. Surgical management is necessary for cases of rectal perforation. However, surgical site infections are the most common complications after colorectal surgery. CASE SUMMARY We discuss a case of rectal perforation in a patient who presented to our hospital 2 d after its occurrence. The perforation occurred as a result of the patient inserting a sex toy in his rectum. Severe peritonitis was attributable to delayed presentation. CONCLUSION Vacuum-assisted closure was performed to treat the wound, which healed well after therapy. No complications were noted.

Abstract:We studied prospectively the incidence of symptomatic infections of presumed urinary tract origin requiring antimicrobial therapy among 54 male nursing home patients with chronic indwelling bladder catheters. During 514 patient-months at risk there were 106 episodes of symptomatic infection, for an incidence of 0.21 per patient-month at risk. Of the patients 80 per cent had at least 1 episode and 48 per cent had 2 or more. None of the clinical factors we examined, including age, nutritional status, stool incontinence, diabetes mellitus, episodes of catheter blockage and the use of chronic suppressant antimicrobial therapy, was associated with the development of symptomatic infection. Further research on host and pathogen-related factors that increase the risk for symptomatic infection, and improvements in infection control and catheter care protocols are necessary to decrease catheter-associated morbidity among male nursing home patients who must be managed by chronic indwelling catheterization.

Abstract:Peritoneal dialysis (PD), offering the benefits of stable hemodynamics, a low risk of bloodstream infection, preservation of residual renal function, and enhancement of quality of life, [ 1 ] has emerged as the preferred modality for renal replacement therapy in patients with end-stage renal disease. Percutaneous puncture catheterization, surgical catheterization, and laparoscopic catheterization are the most common methods for PD catheterization. Advancements in PD catheterization technology have led to a gradual decrease in the incidence of complications related to catheterization. However, these complications continue to be a primary cause of treatment failure in PD. The main complications include infection-related issues (exit site and tunnel infections) and mechanical problems (catheter displacement or occlusion, dialysate leakage, hernia, internal organ injury, and bleeding). [ 2 ] Hence, timely identification and correction of catheter-related complications are essential for enhancing the prognosis of PD patients.

Abstract:BACKGROUND Peritoneal dialysis (PD) is an important renal replacement therapy in patients with end-stage renal disease. PD catheters remain the lifeline for patients undergoing PD. The catheter technique survival rate is considered a core PD outcome domain. CASE SUMMARY The PD catheter spontaneously dislodged in a patient undergoing PD during regular fluid exchange without pain. Abdominal computed tomography showed a tunnel infection. A double-cuff straight Tenckhoff catheter had been inserted using the Seldinger technique. Before this incident, the patient had a history of tunnel infections. We speculate that recurrent tunnel infections and catheter insertion using the Seldinger technique may have led to catheter dislodgement. CONCLUSION The present case suggests that clinicians should more rigorously assess the persistence of catheter-related infections concerning the potential complications arising from catheter dislodgement associated with the Seldinger technique.

Abstract:In 1929 Forssman, a German physician, carried out the first peripherally inserted central catheter (PICC) line insertion when he catheterized his own heart. Although he suffered no ill-effects, myriad complications of central venous access have subsequently been reported. In 1956 Brown and Kent reported embolization of a femoral catheter that resulted in a perforated right ventricle and cardiac tamponade.1 By 1982 twenty-eight cases of cardiac tamponade complicating central venous cannulation had been reported.2 Insertion-related complications also include intravascular breakage and embolization of the guidewire, air embolism, hydrothorax, arrhythmias, arterial puncture and nerve damage. Post-insertion complications include catheter-related sepsis, thrombosis and catheter damage.3–7 We report a unique complication of PICC line insertion.
