Abstract:In this issue of Endoscopy , Soriani et al. [1] evaluate the efficacy of over-the-scope (OTS) clips versus through-the-scope (TTS) clips as a primary treatment in Forrest Ia–IIb peptic ulcer bleeding. They conducted a multicenter, randomized controlled trial (RCT) in four Italian and one Spanish center. This was the first trial to provide head-to-head comparison between two mechanical modalities in endoscopic hemostatic treatment for high risk peptic ulcer bleeding. The negative result of the 30-day rebleeding rate after successful initial hemostasis was 1.6% (1/61) and 3.9% (2/51) for patients treated with OTS clips and TTS clips, respectively (RR = 0.42, 95%CI 0.04–4.53; P =0.47). However, the successful initial hemostasis and overall clinical success rates were higher in the OTS clip group than in the TTS clip group (98.4% vs. 78.4%, P =0.001; 96.7% vs. 74.5%, P =0.001, respectively) with an absolute risk reduction of 20%. "These data support the recommendation of the ESGE guideline to use over-the-scope clips as the primary treatment only in selected patients with peptic ulcer bleeding who have a high risk of hemostatic failure with through-the scope clips." This study emphasizes an important factor for successful initial hemostasis and overall clinical success, which is the expertise of the endoscopist. The study included only experienced endoscopists who had already placed 30 or more OTS clips, resulting in near perfect initial hemostasis and overall clinical success in the OTS clip group. This information was supported by our previous trial, which concluded that OTS clips could be helpful as a primary hemostasis modality in large (≥1.5cm) bleeding peptic ulcers only in experienced hands [2]. Other previous RCTs on this topic [3] [4] have also emphasized the need for endoscopist experience in OTS clip use by setting a protocol of a 2-week OTS clip training program [3] or including only endoscopists who performed ≥20 OTS clip applications per year [4] in the study. Therefore, the results of the Soriani et al. study might not be applicable to general endoscopists. The learning curve for the use of OTS clips is warranted to ensure excellent outcomes for initial hemostasis. Compared with TTS clips, OTS clips can capture a larger amount of tissue and anchor onto the base of the fibrotic ulcer, and therefore have a higher clip retention rate [5]. The nitinol clip has a high tensile strength and compresses the bleeding artery firmly [5]. Brandler et al. reported three characteristics of actively bleeding ulcers that would make OTS clip use appropriate at the index endoscopy: 1) size >2cm plus a vessel ≥2mm; 2) location around the major arteries (posterior wall of the duodenal bulb and lesser curvature of the body); and 3) presence of a deep, excavated, or fibrotic base [6]. Consequently, the European Society of Gastrointestinal Endoscopy (ESGE) suggest using a cap-mounted clip in these three scenarios as first-line treatment (weak recommendation, low quality of evidence) [7]. In the current study, among patients with failed TTS clip hemostasis (n=11), the majority (7/11; 63.6%) had fibrotic-based ulcers, 3/11 (27.3%) had ulcers at the posterior wall of the duodenal bulb, and one patient (9.1%) had a large vessel [1]. Subsequently, all 11 patients underwent successful endoscopic hemostasis with OTS clip application [1]. These data support the recommendation of the ESGE guideline to use OTS clips as the primary treatment only in selected patients who have a high risk of hemostatic failure with TTS clips. In summary, OTS clips should not be used routinely as a first-line endoscopic hemostatic method for peptic ulcer bleeding. Patients with fibrotic-based or large ulcers sitting on top of a large vessel or ulcers at a high risk location may be selected for OTS clip application as an initial treatment, as these patients would benefit from this approach without being overtreated. Over-the-Scope Clips Versus Standard Endoscopic Treatment for First Line Therapy of Non-variceal Upper Gastrointestinal Bleeding: Systematic Review and Meta-Analysis

Abstract:Background and aims: Over-The-Scope Clips (OTSC) use have shown promising results for first line treatment of non-variceal upper gastrointestinal bleeding (NVUGIB). We conducted this meta-analysis to compare outcomes in patients treated with OTSC versus standard endoscopic intervention for first line endoscopic treatment of NVUGIB. Methods: We reviewed several databases from inception to December 9, 2022 to identify studies comparing OTSC and standard treatments as the first line treatment for NVUGIB. The outcomes assessed included re-bleeding, initial hemostasis, need for vascular embolization, mortality, need for repeat endoscopy, 30 day readmission rate, and need for surgery. Pooled risk ratios (RR) with 95% confidence intervals (CI) were calculated using random effect model. Heterogeneity was assessed by I2 statistic. Results: We included 11 studies with 1608 patients (494 patients in OTSC group and 1114 patients in control group). OTSC use was associated with significantly lower risk of re-bleeding (RR, 0.58; 95% CI 0.41-0.82). We found no significant difference in rates of initial hemostasis (RR, 1.05; 95% CI 0.99- 1.11), vascular embolization rates (RR, 0.93; 95% CI 0.40- 2.13), need for repeat endoscopy (RR, 0.78; 95% CI 0.40-1.49), 30 day readmission rate (RR, 0.59; 95% CI 0.17-2.01), need for surgery (RR, 0.81; 95% CI 0.29-2.28) and morality (RR, 0.69; 95% CI 0.38-1.23). Conclusions: OTSC are associated with significantly lower risk of re-bleeding compared to standard endoscopic treatments when used as first line endoscopic therapy for NVUGIB.
Over-the-Scope Clip Applications as First-Line Therapy in the Treatment of Upper Non-variceal Gastrointestinal Bleeding, Perforations, and Fistulas

Abstract:Background: The over-the-scope clip (OTSC) is an innovative device and has been successfully used in endoscopic treatment, however, there is a lack of clinical data from China. The aim of this study is to investigate the OTSC applications in the treatment of upper non-variceal gastrointestinal bleeding (UNVGIB), perforations, and fistulas in China. Methods: In total, 80 patients were treated with one OTSC respectively as first-line therapy in our endoscopy center between January 2016 and November 2020. Among them, 41 patients had UNVGIB, 34 patients had perforations, and five patients had fistulas. The technical and clinical success rates were used to assess the efficacy of OTSC on the above diseases. In addition, we compared the hemostatic efficacy of OTSC with the standard endoscopic therapy in ulcer bleeding and Dieulafoy's lesion by propensity score matching analysis. Results: In general, the OTSCs were applied successfully in all patients and achieved 100% (80/80) technical success. The clinical success of all patients was 91.3% (73/80). Among 41 patients with UNVGIB, the clinical success was 85.4% (35/41); 6 patients presented with recurrence. For patients of Dieulafoy's lesion and under antithrombotic therapy, we found that OTSC treatment had both efficient and reliable hemostasis effects. In addition, according to the characteristics of ulcers, site of bleeding lesion, and Blatchford score, all patients received similar and reliable clinical success rates. After propensity score matching, we found that OTSC treatment had low rebleeding rates when compared with standard endoscopic therapy in both Dieulafoy's lesion (15.0 vs 30.0%) and ulcer bleeding (17.6 vs 29.4%). Among 34 patients with perforations, the clinical success was 100% (34/34). Among five patients with fistulas, only one patient failed in maintaining the OTSC before esophageal fistula healing, and the clip achieved an overall clinical success of 80% (4/5). Conclusion: The OTSC represents a safe and effective endoscopic therapy for UNVGIB, perforations, and fistulas as first-line treatment, especially for Dieulafoy's lesion or patients under antithrombotic therapy for UNVGIB, etc. However, OTSC application in these specific lesions or patients lacks adequate evidence as first-line treatment. Therefore, further larger sample and multi-center clinical trials are required to improve its indications in clinical treatment.

Intervention: Standard hemostatic treatment (n = 97) or OTSC (n = 93). Measurements: The primary outcome was 30-day probability of further bleeds. Other outcomes included failure to control bleeding after assigned endoscopic treatment, recurrent bleeding after initial hemostasis, further intervention, blood transfusion, and hospitalization. Results: The 30-day probability of further bleeding in the standard treatment and OTSC groups was 14.6% (14 of 97) and 3.2% (3 of 93), respectively (risk difference, 11.4 percentage points [95% CI, 3.3 to 20.0 percentage points]; P= 0.006). Failure to control bleeding after assigned endoscopic treatment in the standard treatment and OTSC groups was 6 versus 1 (risk difference, 5.1 percentage points [CI, 0.7 to 11.8 percentage points]), respectively, and 30-day recurrent bleeding was 8 versus 2 (risk difference, 6.6 percentage points [CI,-0.3 to 14.4 percentage points]), respectively. The need for further interventions was 8 versus 2, respectively. Thirty-day mortality was 4 versus 2, respectively. In a post hoc analysis with a composite end point of failure to successfully apply assigned treatment and further bleeds, the event rate was 15 of 97 (15.6%) and 6 of 93 (6.5%) in the standard and OTSC groups, respectively (risk difference, 9.1 percentage points [CI, 0.004 to 18.3 percentage points]). Limitation: Clinicians were not blinded to treatment and the option of crossover treatment. Conclusion: Over-the-scope clips, as an initial treatment, may be better than standard treatment in reducing the risk for further bleeding from nonvariceal upper gastrointestinal causes that are amenable to OTSC placement.
Over-the-scope-clips versus standard treatment in high-risk patients with acute non-variceal upper gastrointestinal bleeding: a randomised controlled trial (STING-2)

Abstract:Objective: Acute non-variceal upper gastrointestinal bleeding (NVUGIB) is managed by standard endoscopic combination therapy, but a few cases remain difficult and carry a high risk of persistent or recurrent bleeding. The aim of our study was to compare first-line over-the-scope-clips (OTSC) therapy with standard endoscopic treatment in these selected patients. Design: We conducted a prospective, randomised, controlled, multicentre study (NCT03331224). Patients with endoscopic evidence of acute NVUGIB and high risk of rebleeding (defined as complete Rockall Score ≥7) were included. Primary endpoint was clinical success defined as successful endoscopic haemostasis without evidence of recurrent bleeding. Results: 246 patients were screened and 100 patients were finally randomised (mean of 5 cases/centre and year; 70% male, 30% female, mean age 78 years; OTSC group n=48, standard group n=52). All but one case in the standard group were treated with conventional clips. Clinical success was 91.7% (n=44) in the OTSC group compared with 73.1% (n=38) in the ST group (p=0.019), with persistent bleeding occurring in 0 vs 6 in the OTSC versus standard group (p=0.027), all of the latter being successfully managed by rescue therapy with OTSC. Recurrent bleeding was observed in four patients (8.3%) in the OTSC group and in eight patients (15.4%) in the standard group (p=0.362). Conclusion: OTSC therapy appears to be superior to standard treatment with clips when used by trained physicians for selected cases of primary therapy of NVUGIB with high risk of rebleeding. Further studies are necessary with regards to patient selection to identify subgroups benefiting most from OTSC haemostasis. Trial registration number: NCT03331224.

Abstract:Moderate‐certainty evidence supports OTSC as a superior initial treatment for acute non‐variceal UGIB with high re‐bleeding risk. TSA strengthens the reliability of this conclusion. Further large‐scale studies are needed to confirm OTSC's role, by exploring other prognosis outcomes and assessing cost‐effectiveness and potential complications Summary Background Acute non‐variceal upper gastrointestinal bleeding (UGIB) is challenging in patients at high risk of re‐bleeding in whom standard endoscopic treatment (ST) has limited effectiveness. Over‐the‐scope clips (OTSC) have shown promise in these patients although their precise role remains uncertain. Aims To confirm the role of OTSC in patients with UGIB at high risk of re‐bleeding. Methods We systematically searched CENTRAL, MEDLINE and Embase from January 1st, 1970 to April 24, 2024 for randomised controlled trials (RCTs) comparing OTSC and ST in acute non‐variceal UGIB with high re‐bleeding risk. The GRADE framework assessed evidence certainty, while trial sequential analysis (TSA) controlled random errors and evaluated conclusion validity. Results We analysed four RCTs (319 patients); pooled risk ratio (RR) for clinical success at initial endoscopy favoured OTSC (RR = 1.30, 95% CI = 1.08–1.56, p = 0.006, I2 = 58%, moderate certainty of evidence). TSA showed the desired sample size was 410 and the cumulative Z curve crossing the trial sequential monitoring boundary. The pooled RR for re‐bleeding within 30 days favoured OTSC (RR = 0.53, 95% CI = 0.30–0.94, p = 0.03, I2 = 0%, moderate certainty of evidence). There was no significant difference in 30‐day mortality, or length of hospital or ICU stay. Conclusions Moderate certainty evidence supports OTSC as a superior initial treatment for acute non‐variceal UGIB with high re‐bleeding risk. Further large‐scale studies are needed to confirm OTSCs' role by exploring other prognostic outcomes and assessing cost‐effectiveness and potential complications.

Abstract:BACKGROUND Acute nonvariceal upper gastrointestinal bleeding (ANVUGIB) is a frequent life-threatening acute condition in gastroenterology associated with high morbidity and mortality. Over-the-scope-clip (OTSC) is a new endoscopic hemostasis technique, which is being used in ANVUGIB and is more effective. AIM To summarize and analyze the effects of the OTSC in prevention of recurrent bleeding, clinical success rate, procedure time, hospital stay, and adverse events in the treatment of ANVUGIB, to evaluate whether OTSC can replace standard endoscopic therapy as a new generation of treatment for ANVUGIB. METHODS The literature related to OTSC and standard therapy for ANVUGIB published before January 2023 was searched in PubMed, Web of Science, EMBASE, Cochrane, Google, and CNKI databases. Changes in recurrent bleeding (7 or 30 days), clinical results (clinical success rate, conversion rate to surgery, mortality), therapy time (procedure time, hospital stay), and adverse events in the OTSC intervention group were summarized and analyzed, and the MD or OR of 95%CI is calculated by Review Manager 5.3. RESULTS This meta-analysis involved 11 studies with 1266 patients. Total risk of bias was moderate-to-high. For patients in the OTSC group, 7- and 30-days recurrent bleeding rates, as well as procedure time, hospital stay, and intensive care unit stay, were greatly inhibited. OTSC could significantly improve the clinical success rate of ANVUGIB. OTSC therapy did not cause serious adverse and was effective in reducing patient mortality. CONCLUSION OTSC may provide more rapid and sustained hemostasis, and thus, promote recovery and reduce mortality in patients with ANVUGIB. In addition, the safety of OTSC is assured.

Abstract:Aims: We compared the efficacy of a novel hemostatic clip (Resolution ClipTM, Boston Scientific) (RC) and combined injection with heater probe (HP) thermocoagulation in treatment of PUB. Methods: In this randomized controlled trial, consecutive PUB patients with active bleeding, non-bleeding visible vessel, or adherent clots (Forrest type I, IIa and IIb) were prospectively randomized to either (1) RC or (2) combined epinephrine injection with HP thermocoagulation. After successful initial hemostasis, both groups of patients were given intravenous esomeprazole infusion for 72 hours. The primary endpoint was treatment failure within 30 days, which was defined as (1) failure to achieve initial hemostasis, or (2) recurrent bleeding after initial hemostasis. Rebleeding was confirmed by urgent endoscopy, which showed arterial bleeding or fresh blood together with a non-bleeding visible vessel. Secondary endpoints included technical difficulty as determined by a visual analogue scale (VAS) and 30-day mortality. Results: Analysis was by ITT, 136 patients were screened and 91 (mean age: 69.2; male: 59.3%) were studied. 8 of 47 (17%) Vs 6 of 44 (14%) patients reached primary endpoint in RET and HP groups (P = 0.67), respectively. Failed hemostasis occurred in 6 (12.8%) patients in RC group Vs 2 (4.5%) in HP group (P = 0.17). After excluding patients with failed initial hemostasis, the 30-day recurrent bleeding rate was 2 (4.9%) and 4 (9.8%) in RC and HP groups (P = 0.37, log rank), respectively. Endoscopists reported higher degree of technical difficulty (VAS: 5.1 +/- 0.4 Vs 3.6 +/- 0.3, P = 0.006) with RC. There was no difference in 30-day mortality (4.3 Vs 4.5%, P = 0.95). Conclusions: Resolution Clip™ has similar efficacy as combined injection with HP in control of peptic ulcer bleeding with higher technical difficulty.
Systematic Review with Network Meta-Analysis: Dual Therapy for High-Risk Bleeding Peptic Ulcers

Abstract:Background: Adding a second endoscopic therapy to epinephrine injection might improve hemostatic efficacy in patients with high-risk bleeding ulcers but the optimum modality remains unknown. We aimed to estimate the comparative efficacy of different dual endoscopic therapies for the management of bleeding peptic ulcers through random-effects Bayesian network meta-analysis.Methods: Different databases were searched for controlled trials comparing dual therapy versus epinephrine monotherapy or epinephrine combined with another second modality until September, 30 2016. We estimated the ORs for rebleeding, surgery and mortality among different treatments. Adverse events were also evaluated.Results: Seventeen eligible articles were included in the network meta-analysis. The addition of mechanical therapy (OR 0.19, 95% CrI 0.07-0.52 and OR 0.10, 95% CrI 0.01-0.50, respectively) after epinephrine injection significantly reduced the probability of rebleeding and surgery. Similarly, patients who received epinephrine plus thermal therapy showed a significantly decreased rebleeding rate (OR 0.30, 95% CrI 0.10-0.91), as well as a non-significant reduction in surgery (OR 0.47, 95% CrI 0.16-1.20). Although differing, epinephrine plus mechanical therapy did not provide a significant reduction in rebleeding (OR 0.62, 95% CrI 0.19-2.22) and surgery (OR 0.21, 95% CrI 0.03-1.73) compared to epinephrine plus thermal therapy. Sclerosant failed to confer further benefits and was ranked highest among the 5 treatments in relation to adverse events.Conclusions: Mechanical therapy was the most appropriate modality to add to epinephrine injection. Epinephrine plus thermal coagulation was effective for controlling high risk bleeding ulcers. There was no further benefit with sclerosants with regard to rebleeding or surgery, and sclerosants were also associated with more adverse events than any other modality.

Abstract:Primary prevention of esophageal variceal (EV) bleeding includes endoscopic variceal ligation (EVL) or use of nonselective beta blockers. If bleeding occurs, EVL is the first treatment. Post banding ulceration is uncommon but may develop at the ligation site within 1-2 weeks due to spontaneous slippage prior to thrombus formation, leading to post banding ulcer bleeding (PBUB). Endoscopic hemostasis for PBUB is challenging due to ulcerated and macerated areas from prior banding. Traditional techniques such as EVL leads to slippage and conventional clips shear through friable mucosa. Over-the-scope clips (OTSC) approximate large areas with a sturdy grasp making it an ideal tool in this setting. We present two patients with PBUB treated successfully with OTSC.

Over-the-scope clip as a rescue treatment for massive bleeding due to Dieulafoy lesion at the colorectal anastomosis: A case report

Abstract:Rationale: The bleeding of Dieulafoy lesion predominantly involves the proximal stomach and leads to severe gastrointestinal bleeding. However, these lesions have also been reported in the whole gastrointestinal tract. Bleeding of Dieulafoy lesions at the anastomosis was seldomly reported and was very easy to be ignored clinically. Patient concerns: We describe a 72-year-old woman with a past history of surgery for rectal carcinoma hospitalized with chief complaint of massive rectal bleeding. No gross bleeding lesion was found during the first emergency colonoscopy. Despite multiple blood transfusions, her hemoglobin rapidly dropped to 5.8 g/dL. Diagnosis: She was diagnosed with Dieulafoy lesion at the colorectal anastomosis during the second emergency colonoscopy. Interventions: Primary hemostasis was achieved by endoscopic hemostatic clipping. However, she experienced another large volume hematochezia 3 days later, and then received another endoscopic hemostatic clipping. She was improved and discharged. However, this patient underwent hematochezia again 1 month later. Bleeding was arrested successfully after the over-the-scope clip (OTSC) was placed during the fourth emergency colonoscopy. Outcomes: This patient underwent 4 endoscopic examinations and treatments during 2 hospitalizations. The lesion was overlooked during the first emergency colonoscopy. The second and third endoscopes revealed Dieulafoy lesion at the colorectal anastomosis and performed endoscopic hemostatic clippings, but delayed rebleeding occurred. The bleeding was stopped after the fourth emergency colonoscopy using OTSC. There was no further rebleeding during hospitalization and after 2-year of follow-up. Lessons: As far as we know, there is no reported case of lower gastrointestinal bleeding caused by Dieulafoy lesion at the colorectal anastomosis, OTSC is a safe and effective rescue treatment for Dieulafoy lesions.

Randomized controlled trial of hemostatic powder versus endoscopic clipping for non-variceal upper gastrointestinal bleeding

Abstract:Background and aims: Despite advances in pharmacological and endoscopic management of non-variceal upper gastrointestinal bleeding (NVUGIB), mortality is still relevant. TC-325 (Hemospray-Cook Medical) is a mineral powder with adsorptive properties, designed for endoscopic hemostasis. There are still no comparative trials studying this new hemostatic modality. The objective of this research was to compare the use of TC-325 (associated with epinephrine injection) with the combined technique of endoscopic clipping and epinephrine injection for the treatment of patients with NVUGIB. Methods: We conducted a pilot randomized controlled trial with patients that presented NVUGIB with an actively bleeding lesion at the endoscopic evaluation. Patients were randomized either to the Hemospray or Hemoclip group. The randomization list was generated by a computer program and remained unknown throughout the entire trial. All patients underwent second-look endoscopy. Results: Thirty-nine patients were enrolled. Peptic ulcer was the most frequent etiology. Primary hemostasis was achieved in all Hemospray cases and in 90% of Hemoclip group (p = 0.487). Five patients in Hemospray group underwent an additional hemostatic procedure during second-look endoscopy, while no patient in the Hemoclip group needed it (p = 0.04). Rebleeding, emergency surgery and mortality rates were similar in both groups. No toxicity, allergy events, or gastrointestinal obstruction signs were observed in Hemospray group. Conclusions: TC-325 presents similar hemostatic results when compared with conventional dual therapy for patients with NVUGIB. Hemospray's excellent primary hemostasis rate certifies it as a valuable tool in arduous situations of severe bleeding or difficult location site.
Endoscopic hemostasis for peptic ulcer bleeding: systematic review and meta-analyses of randomized controlled trials

Abstract:BackgroundPeptic ulcer represents the most common cause of upper gastrointestinal bleeding. Endoscopic therapy can reduce the risks of rebleeding, continued bleeding, need for surgery, and mortality. The objective of this review is to compare the different modalities of endoscopic therapy.MethodsStudies were identified by searching electronic databases MEDLINE, Embase, Cochrane, LILACS, DARE, and CINAHL. We selected randomized clinical trials that assessed contemporary endoscopic hemostatic techniques. The outcomes evaluated were: initial hemostasis, rebleeding rate, need for surgery, and mortality. The possibility of publication bias was evaluated by funnel plots. An additional analysis was made, including only the higher-quality trials.ResultsTwenty-eight trials involving 2988 patients were evaluated. Injection therapy alone was inferior to injection therapy with hemoclip and with thermal coagulation when evaluating rebleeding and the need for emergency surgery. Hemoclip was superior to injection therapy in terms of rebleeding; there were no statistically significant differences between hemoclip alone and hemoclip with injection therapy. There was considerable heterogeneity in the comparisons between hemoclip and thermal coagulation. There were no statistically significant differences between thermal coagulation and injection therapy, though their combination was superior, in terms of rebleeding, to thermal coagulation alone.ConclusionsInjection therapy should not be used alone. Hemoclip is superior to injection therapy, and combining hemoclip with an injectate does not improve hemostatic efficacy above hemoclip alone. Thermal coagulation has similar efficacy as injection therapy; combining these appears to be superior to thermal coagulation alone. Therefore, we recommend the application of hemoclips or the combined use of injection therapy with thermal coagulation for the treatment of peptic ulcer bleeding.

Abstract:Abstract Background Upper gastrointestinal bleeding (UGIB) is a leading cause of morbidity and is associated with a 2 % – 17 % mortality rate in the UK and USA. Bleeding peptic ulcers account for 50 % of UGIB cases. Endoscopic intervention in a timely manner can improve outcomes. Hemostatic spray is an endoscopic hemostatic powder for GI bleeding. This multicenter registry was created to collect data prospectively on the immediate endoscopic hemostasis of GI bleeding in patients with peptic ulcer disease when hemostatic spray is applied as endoscopic monotherapy, dual therapy, or rescue therapy. Methods Data were collected prospectively (January 2016 – March 2019) from 14 centers in the UK, France, Germany, and the USA. The application of hemostatic spray was decided upon at the endoscopist’s discretion. Results 202 patients with UGIB secondary to peptic ulcers were recruited. Immediate hemostasis was achieved in 178/202 patients (88 %), 26/154 (17 %) experienced rebleeding, 21/175 (12 %) died within 7 days, and 38/175 (22 %) died within 30 days (all-cause mortality). Combination therapy of hemostatic spray with other endoscopic modalities had an associated lower 30-day mortality (16 %, P < 0.05) compared with monotherapy or rescue therapy. There were high immediate hemostasis rates across all peptic ulcer disease Forrest classifications. Conclusions This is the largest case series of outcomes of peptic ulcer bleeding treated with hemostatic spray, with high immediate hemostasis rates for bleeding peptic ulcers.

Abstract:Here we report a new type of microelectrode sensor for single-cell exocytotic dopamine release. The new microsensor is built by forming a gold-nanoparticle (AuNP) network on a carbon fiber microelectrode. First a gold surface is obtained on a carbon fiber microdisk electrode by partially etching away the carbon followed by electrochemical deposition of gold into the pore. The gold surface is chemically functionalized with a sol-gel silicate network derived from (3-mercaptopropyl)trimethoxysilane (MPTS). A AuNP network is formed by immobilizing Au nanoparticles onto the thiol groups in the sol-gel silicate network. The AuNP-network microelectrode has been characterized by scanning electron microscopy (SEM) and steady-state voltammetry. The AuNP-network microelectrode has been used for amperometric detection of exocytotic dopamine secretion from individual pheochromocytoma (PC12) cells. The results show significant differences in the kinetic peak parameters including shorter rise time, decay time, and half-width as compared to a bare carbon fiber electrode equivalent. These results indicate AuNP-network microelectrodes possess an excellent sensing activity for single-cell exocytotic catecholamine release, specifically dopamine. Moreover, key advantageous properties inherent to bare carbon fiber microelectrodes (i.e., rigidity, flexibility, and small size) are maintained in addition to an observed prolonged shelf life stability and resistance to cellular debris fouling and dopamine polymerization.
Efficacy of New Hemostatic Techniques in Nonvariceal Gastrointestinal Bleeding: A Systematic Review and Network Meta‐analysis

Abstract:Objectives Nonvariceal gastrointestinal bleeding (NVGIB) is a common medical condition with significant mortality and morbidity. Several types of hemostatic modalities are currently available in clinical setting. This systematic review and network meta‐analysis aimed to assess the efficacy of these modalities in treating NVGIB. Methods PubMed, EMBASE, and the Cochrane Library databases were searched for studies that compared the efficacy of different hemostatic techniques (over‐the‐scope clip [OTSC], hemostatic powder [HP] and conventional endoscopic treatment [CET]) for NVGIB published up to June 2022. The 30‐day rebleeding rate was regarded as the primary outcome. We performed pairwise and network meta‐analyses for all treatments. The heterogeneity and transitivity were evaluated. Results Twenty‐two studies were included. Regarding the 30‐day rebleeding rate, OTSC and HP plus CET showed superior efficacy in treating NVGIB compared with CET (OTSC vs CET: relative risk [RR] 0.42, 95% confidence interval [CI] 0.28–0.60; HP plus CET vs CET: RR 0.40, 95% CI 0.17–0.87), while OTSC and HP plus CET had comparable efficacy (RR 0.95, 95% CI 0.38–2.31). HP plus CET ranked the highest in the network ranking estimate. The sensitivity analysis showed that it was not robust that OTSC was superior to CET regarding the short‐term rebleeding rate and the initial hemostasis rate. While all‐cause mortality, bleeding‐related mortality and necessity of surgical or angiographic salvage therapy showed no statistically significant difference. Conclusion OTSC and HP plus CET significantly reduced 30‐day rebleeding rate compared to CET and had comparable efficacy in the treatment of NVGIB.
