Abstract:STATEMENT OF PROBLEM:Additively manufactured surgical templates are commonly used for computer-guided implant placement. However, their accuracy, reproducibility, and dimensional stability have not been thoroughly investigated with the different 3D printers and materials used for their fabrication.PURPOSE:The purpose of this in vitro study was to evaluate the accuracy, reproducibility, and dimensional stability of additively manufactured surgical templates fabricated by using different 3D printers.MATERIAL AND METHODS:Thirty surgical templates were designed and additively manufactured from 3 different 3D printers as follows: group SLA (n=10) was fabricated by using a desktop stereolithography (SLA) 3D printer and photopolymerizing resin; group PolyJet (n=10) was fabricated by using a PolyJet 3D printer and photopolymerizing resins; and group DMP (n=10) was fabricated by using a direct metal printing (DMP) system and Co-Cr metal alloy. All surgical templates were scanned by using a laser scanner within 36 hours of production and digitalized again 1 month later. All scanned files were compared with the corresponding designed files in a surface matching software program. The mean deviation root mean square (RMS, measured in mm, representing accuracy), percentage of measurement data points within 1 standard deviation of mean RMS (in %, representing reproducibility), and dimensional changes were determined and compared.RESULTS:At the postproduction stage, group PolyJet was most accurate with the lowest RMS value of 0.10 ±0.02 mm and highest reproducibility with 93.07 ±1.54% of measurement data points within 1 standard deviation of mean RMS. After 1-month storage, group PolyJet(1month) remained the most accurate with the lowest RMS value of 0.14 ±0.03 mm and the highest reproducibility value of 92.46 ±1.50%. For dimensional stability, group SLA versus group SLA(1month) comparison showed a significant decrease in accuracy (RMS values of 0.20 ±0.08 mm versus 0.25 ±0.08 mm, P<.001) and reproducibility (88.16 ±3.66% versus 86.10 ±4.16%, P=.012). Group PolyJet versus group PolyJet(1month) comparison only showed significant changes in accuracy (RMS values of 0.10 ±0.02 mm versus 0.14 ±0.03 mm, P=.011). Group DMP versus group DMP(1month) comparison showed no significant changes in accuracy (RMS values of 0.19 ±0.03 mm versus 0.20 ±0.04 mm, P=.981) or reproducibility (89.77 ±1.61% versus 89.74 ±2.24%, P=1.000).CONCLUSIONS:Printed resin surgical templates produced by using the PolyJet 3D printer showed higher accuracy and reproducibility than those produced by using the desktop SLA 3D printer and printed Co-Cr surgical templates at both the postproduction stage and after 1-month storage. The level of accuracy and reproducibility in printed Co-Cr surgical templates was not affected by 1-month storage.

Abstract:Objectives To evaluate the influence of the three‐dimensional (3D) printing technology, material, sterilization, and disinfection on the accuracy of guided surgical templates. Material and Methods Fifty printed resin surgical templates were designed and fabricated using a digital light processing 3D printer with a photopolymerizing resin, and 50 printed metal surgical templates were designed and fabricated using a selective laser melting 3D printer with a titanium alloy. Templates from both groups were randomly divided into five subgroups involving different sterilization and disinfection procedures. The group without any sterilization or disinfection procedure served as the control group, whereas the other groups were used as the study groups (hydrogen peroxide gas plasma sterilization, 5% povidone‐iodine disinfection, 75% ethyl alcohol disinfection, and steam autoclave sterilization). Implant simulations were performed on the 3D‐printed resin models, and postoperative impressions were acquired with scan bodies attached to the implants. All surgical templates were digitally scanned. The root mean square was used to determine and quantify fabrication accuracy and reproducibility, and the definitive and planned implant positions were compared. Results The printed resin templates exhibited lower fabrication accuracy and reproducibility, as well as higher 3D deviations, after steam autoclave sterilization (p 0.05). Conclusions Printed metal surgical templates are viable alternatives for guided implant surgery. Preoperative steam or gas plasma sterilization is recommended, especially for metal templates, as resin templates show deformation and decreased accuracy after steam sterilization. Influence of 3D-printing method, resin material, and sterilization on the accuracy of virtually designed surgical implant guides

Abstract:Statement of problem: Three-dimensional printing has introduced new manufacturing methods. However, information on the influence of the specific printing technology, material, sterilization, and the comparison between printing and milling on the accuracy of surgical guides is lacking. Purpose: The purpose of this in vitro study was to evaluate the influence of the manufacturing method (printing and milling), printing technology stereolithography (SLA) and digital light processing (DLP), material, and sterilization on the accuracy of digitally designed surgical implant guides. Material and methods: Resin patient replicas with a single edentulous space were used to place 132 implants with digitally designed surgical guides. The accuracy of postoperative implant position was analyzed for the manufacturing method (printing and milling), resin materials, and preoperative autoclaving. To determine 3D accuracy, angular displacement, mean horizontal crestal, apical displacement, and the linear vertical displacement at the apex were calculated separately for each group (n=12). In addition, the surgical guides were qualitatively analyzed by using field emission scanning electron micrograph. Results: The postoperative angular deviation ranged from 0.76 ±0.52 degrees (Rapidshape D20II with NextDent SG) to 2.43 ±0.64 degrees (Form2 with NextDent SG) (P<.001). Linear horizontal displacement at the crest was smallest for Rapidshape D20II with 3Delta Guide (0.27 ±0.08 mm) and highest for Form2 with NextDent SG (0.54 ±0.10 mm) (P<.001). Linear horizontal displacement at the apex ranged from 0.36 ±0.10 mm (SolFlex 350 with V-Print SG) to 0.89 ±0.32 mm (Form2 with NextDent SG) (P<.001). Considering the vertical position displacement was no more than 0.43 ±0.07 mm (Form2 with NextDent SG) short of the apex, none of the implant tips were displaced apically. Preoperative autoclaving differentially impaired the accuracy of surgical guides. Conclusions: The specific manufacturing technique, the 3D printing device, the resin material, and the application of preoperative sterilization all affected the accuracy of the postoperative implant position. Irrespective of the manufacturing method, all implants were placed within the commonly accepted safety distance.

Abstract:PURPOSE:To compare the accuracy of a chairside fused deposition modeling (FDM) 3D-printed surgical template with that of a light-cured template for implant placement.MATERIALS AND METHODS:Twenty standard mandibular resin models with missing teeth 36 and 46 were selected. Surgical templates were fabricated using a chairside FDM 3D-printer (test group) or a light-curing 3D printer (control group) (n = 20/group). Forty implants were placed by a clinician blinded to group allocation. The angular, 3D, mesiodistal, buccolingual, and apicocoronal deviations at the implant base and tip between preoperative design and postoperative implant position were recorded.RESULTS:The mean angular (test vs control groups: 3.22° ± 1.55° vs 2.74° ± 1.24°, p = 0.343) and 3D deviations at the implant base (test vs control groups: 0.41 ± 0.13 mm vs 0.35 ± 0.11 mm, p = 0.127) and tip (test vs control groups: 0.91 ± 0.34 mm vs 0.75 ± 0.28 mm, p = 0.150) were similar. The mesiodistal, buccolingual, and apicocoronal deviations at the implant base and tip also did not differ significantly between groups (p > 0.05).CONCLUSIONS:For single tooth gap indications, implant placement with an FDM 3D-printed surgical template was as accurate as that with a light-cured template, and more efficient.

Abstract:Desktop three-dimensional (3D) printers (D3DPs) have become a popular tool for fabricating personalized consumer products, favored for low cost, easy operation and other advantageous qualities. This study focused on the potential for using D3DPs to successfully, rapidly and economically print customized implants at medical clinics. An experiment was conducted on a D3DP-printed anterior cruciate ligament surgical implant using a rabbit model. A well-defined, orthogonal, porous PLA screw-like scaffold was printed, then coated with hydroxyapatite (HA) to improve its osteoconductivity. As an internal fixation as well as an ideal cell delivery system, the osteogenic scaffold loaded with mesenchymal stem cells (MSCs) were evaluated through both in vitro and in vivo tests to observe bone-ligament healing via cell therapy. The MSCs suspended in Pluronic F-127 hydrogel on PLA/HA screw-like scaffold showed the highest cell proliferation and osteogenesis in vitro . In vivo assessment of rabbit anterior cruciate ligament models for 4 and 12 weeks showed that the PLA/HA screw-like scaffold loaded with MSCs suspended in Pluronic F-127 hydrogel exhibited significant bone ingrowth and bone-graft interface formation within the bone tunnel. Overall, the results of this study demonstrate that fabricating surgical implants at the clinic (fab@clinic) with D3DPs can be feasible, effective and economical.
How Accurate Is Oral Implant Installation Using Surgical Guides Printed from a Degradable and Steam-Sterilized Biopolymer?

Abstract:3D printed surgical guides are used for prosthetically-driven oral implant placement. When manufacturing these guides, information regarding suitable printing techniques and materials as well as the necessity for additional, non-printed stock parts such as metal sleeves is scarce. The aim of the investigation was to determine the accuracy of a surgical workflow for oral implant placement using guides manufactured by means of fused deposition modeling (FDM) from a biodegradable and sterilizable biopolymer filament. Furthermore, the potential benefit of metal sleeve inserts should be assessed. A surgical guide was designed for the installation of two implants in the region of the second premolar (SP) and second molar (SM) in a mandibular typodont model. For two additive manufacturing techniques (stereolithography [SLA]: reference group, FDM: observational group) n = 10 surgical guides, with (S) and without (NS) metal sleeves, were used. This resulted in 4 groups of 10 samples each (SLA-S/NS, FDM-S/NS). Target and real implant positions were superimposed and compared using a dedicated software. Sagittal, transversal, and vertical discrepancies at the level of the implant shoulder, apex and regarding the main axis were determined. MANOVA with posthoc Tukey tests were performed for statistical analyses. Placed implants showed sagittal and transversal discrepancies of <1 mm, vertical discrepancies of <0.6 mm, and axial deviations of ≤3°. In the vertical dimension, no differences between the four groups were measured (p ≤ 0.054). In the sagittal dimension, SLA groups showed decreased deviations in the implant shoulder region compared to FDM (p ≤ 0.033), whereas no differences in the transversal dimension between the groups were measured (p ≤ 0.054). The use of metal sleeves did not affect axial, vertical, and sagittal accuracy, but resulted in increased transversal deviations (p = 0.001). Regarding accuracy, biopolymer-based surgical guides manufactured by means of FDM present similar accuracy than SLA. Cytotoxicity tests are necessary to confirm their biocompatibility in the oral environment.

Abstract:Purpose To evaluate the dimensional stability over time of additively manufactured surgical templates, fabricated by different resins, and stored by different methods. Materials and Methods Using a 3D printer with DLS technology and two different resins (Surgical Guide (SG)‐WhipMix and Key Guide (KG)‐KeystoneIndustries), 96 surgical guides were additively manufactured. The guides were stored in three different environments: directly exposed to sunlight (S1), in normal interior room conditions (S2), and in darkness (S3). The guides were digitally scanned immediately after fabrication and post‐processing, and after 1, 3, and 6 months of storage. For each group, the mean deviation of the root mean square (RMS) between guide's intaglio surface, as well as the axial deviation between sleeves' housings were calculated. Results The mean axial variations of angular axis deviation of sleeves' housings ranged between 0.09° and 3.99°. Abstract:Three-dimensional (3D) printing has become an integral part of orthognathic surgery. However, there is a lack of studies evaluating accuracy of orthognathic surgical splints fabricated from subtractive milling versus additive 3D printing. The primary aim of this in-vitro study was to compare the differences in trueness between milled and 3D-printed splints, while the secondary aim was to compare the differences in clinical fit of these splints. A sample of eight patients was selected, and STL files of the final orthognathic surgical splint were used to fabricate three splints for each of the eight cases. The first splint was fabricated by subtractive milling (SM), whereas the second and third splints were 3D printed with Digital Light Processing (DLP) and Laser Stereolithography (SLA), respectively. Paired superimposition of scans was performed using a reference model. The clinical fit of the splints to the printed models was also assessed. The mean root mean square (RMS) deviations for the SM, SLA, and DLP were 0.11 ± 0.02, 0.16 ± 0.02 and 0.14 ± 0.02 respectively. The post-hoc analysis showed that the SM splints had the highest accuracy (p < 0.01). However, DLP splints showed the best clinical fit, followed by SM and SLA. In conclusion, splints fabricated by SM were more accurate than those fabricated by 3D printing, although this difference may not be clinically significant. The site, rather than the magnitude of the errors, may have a greater effect on the clinical usability of splints. In general, SM and DLP splints demonstrated a good clinical fit and were suitable for the fabrication of surgical splints.

Abstract:Summary Many studies have identified regions within human ventral visual stream to be important for object identification and categorization; however, knowledge of how perceptual information is communicated within the visual network is still limited. Current theories posit that if a high correspondence between incoming sensory information and internal representations exists, then the object is rapidly identified, and if there is not, then the object requires extra detailed processing. Event-related responses from the present magnetoencephalography study showed two main effects. The N1m peak latencies were approximately 15 milliseconds earlier to familiar letters than to unfamiliar pseudoletters, and the N2m was more negative to pseudoletters than to letters. Event-related beamforming analyses identified these effects to be within bilateral visual cortices with a right lateralization for the N2m effect. Furthermore, functional connectivity analyses revealed that gamma-band (50–80 Hz) oscillatory phase synchronizations among occipital regions were greater to letters than to pseudoletters (around 85 milliseconds). However, during a later time interval between 245 and 375 milliseconds, pseudoletters elicited greater gamma-band phase synchronizations among a more distributed occipital network than did letters. These findings indicate that familiar object processing begins by at least 85 milliseconds, which could represent an initial match to an internal template. In addition, unfamiliar object processing persisted longer than that for familiar objects, which could reflect greater attention to inexperienced objects to determine their identity and/or to consolidate a new template to aid in future identification.
Accuracy of Multi-Implant Impressions Using 3D-Printing Custom Trays and Splinting Versus Conventional Techniques for Complete Arches.

Abstract:PURPOSE:To assess the three-dimensional (3D) accuracy of multi-implant impressions for complete arches obtained using 3D printing technology, and to determine the clinical feasibility of this approach by comparing the results obtained with those obtained with conventional impression techniques.MATERIALS AND METHODS:A maxillary model matrix with four implant analogs was used to fabricate 10 stone reference models (control) to simulate edentulous patients. Ten 3D-printed impressions were made using polylactic acid custom open trays with component splinting (3D-printed group). To make 10 conventional impressions of the reference models, the poly(methyl methacrylate) open trays with splinting technique was used (conventional group). Impressions were poured with a type IV dental stone, which had been digitalized utilizing a high-resolution laboratory scanner after connecting the polymer scan bodies. Standard tessellation language (STL) datasets from the two groups of stone models were superimposed with the corresponding reference models. The interimplant distances and interimplant angulations for each implant pair were measured for the reference models and for the two test groups' definitive models, to assess 3D deviations. In addition, the mechanical properties of polylactic acid and poly(methyl methacrylate) tray materials were compared by means of three-point loading and tensile bond tests. The Student t test was used to compare the differences in implant position and mechanical properties between the two groups, while implant angulations were analyzed with Wilcoxon's rank-sum test, at P = .05.RESULTS:There was less implant-position deviation for the 3D-printed group (mean ± SD: 56.37 ± 12.52 μm) than for the conventional group (71.94 ± 18.86 μm) (P = .014). No significant differences were found in angular deviation between the two groups (P > .05). Flexural strength results suggested that polylactic acid (112.7 ± 1.62 MPa) was stronger than poly(methyl methacrylate) (104.0 ± 2.17 MPa; P < .0001). The tensile bond strength of polylactic acid (0.07 ± 0.005 MPa) was higher than that of poly(methyl methacrylate) (0.03 ± 0.004 MPa; P < .0001).CONCLUSION:The 3D-printing multi-implant impression technique could be an alternative to conventional impression techniques for complete arches.
Comparative evaluation of accuracy of implants placed with thermoplastic and three-dimensional-printed surgical guides: A randomized controlled trial

Abstract:Background: The current study was planned to evaluate the accuracy of dental implant placement with two different types of surgical guides: Thermoplastic and three-dimensional (3D) printed. Materials and methods: A total of 32 implants were placed in 20 healthy, partially dentate individuals with an isolated single missing tooth. The implant sites were randomly allocated into two treatment groups: Group A (thermoplastic implant surgical guide, n = 16 implants) and Group B (3D printed implant surgical guide, n = 16 implants). All the cases in both groups were digitally planned according to a defined protocol, and a comparison of the planned and actual implant positions was performed using the medical image analysis software. The differences in the outcome variables, i.e., angular deviation (AD), 3D error at the entry, 3D error at the apex (3D EA), vertical deviation (VD), and composite deviation, were statistically analyzed. Results: All the outcome variables showed improvements, but statistically significant improvement was shown by AD (P = 0.005), 3D EA (P = 0.01), and VD (P = 0.007). The mean and standard deviation (SD) for AD, (3D EA), and VD were 5.58° ±1.93°, 0.96 ± 0.32 mm, and 0.58 ± 0.36 mm, respectively, for group A. The mean and SD for AD, (3D EA), and VD were 3.94° ± 0.64°, 0.64 ± 0.35 mm, and 0.29 ± 0.13 mm, respectively, for group B (P < 0.05). Conclusion: Within the limits of the study, dental implants placed using 3D-printed surgical guides were positioned clinically with greater accuracy, and fewer deviations were observed from their presurgical planned positions as compared to the thermoplastic surgical guides.
Clinical applications and prospects of 3D printing guide templates in orthopaedics

Abstract:Background: With increasing requirements for medical effects, and huge differences among individuals, traditional surgical instruments are difficult to meet the patients' growing medical demands. 3D printing is increasingly mature, which connects to medical services critically as well. The patient specific surgical guide plate provides the condition for precision medicine in orthopaedics. Methods: In this paper, a systematic review of the orthopedic guide template is presented, where the history of 3D-printing-guided technology, the process of guides, and basic clinical applications of orthopedic guide templates are described. Finally, the limitations of the template and possible future directions are discussed. Results: The technology of 3D printing surgical templates is increasingly mature, standard, and intelligent. With the help of guide templates, the surgeon can easily determine the direction and depth of the screw path, and choose the angle and range of osteotomy, increasing the precision, safety, and reliability of the procedure in various types of surgeries. It simplifies the difficult surgical steps and accelerates the growth of young and mid-career physicians. But some problems such as cost, materials, and equipment limit its development. Conclusions: In different fields of orthopedics, the use of guide templates can significantly improve surgical accuracy, shorten the surgical time, and reduce intraoperative bleeding and radiation. With the development of 3D printing, the guide template will be standardized and simplified from design to production and use. 3D printing guides will be further sublimated in the application of orthopedics and better serve the patients. The translational potential of this paper: Precision, intelligence, and individuation are the future development direction of orthopedics. It is more and more popular as the price of printers falls and materials are developed. In addition, the technology of meta-universe, digital twin, and artificial intelligence have made revolutionary effects on template guides. We aim to summarize recent developments and applications of 3D printing guide templates for engineers and surgeons to develop more accurate and efficient templates.

Abstract:Objectives This study investigates effects of surgical guide manufacturing process on 3D transfer accuracy of planned dental implant position, using three production methods: additive 3D‐printed (3DF), subtractive milled (MF), and analog laboratory fabricated templates (LF). Material and Methods Implant position for a single‐tooth gap (#26) planned digitally. 3DF and MF templates were designed digitally, while LF templates were analogously created. For each manufacturing type, 10 surgical guides were fabricated. Each guide was used for template‐guided implant placement in model replicas. For evaluation of implant placement, cone beam computed tomography scans of all implanted models were superimposed, and implant positions were determined. Deviations at implant shoulder/apex were measured, and median and inter‐quartile range (IQR) were determined for mesio‐distal, oro‐facial, coronal apico, 3D spaces, and angles. Results At implant shoulder, vertical components dominated position deviations (up to 1.04 mm, IQR 0.28 mm for 3DF). Horizontal deviations were much lower (mesio‐distally up to 0.38 mm, IQR 0.36 mm (LF)). Implant apex shows similar vertical deviations, while horizontal deviations clearly increased compared to shoulder, especially in mesio‐distal direction. Median angular deviations were between 2.1° (IQR 2.0 mm, max. 4.2°) for 3DF and 3.3° (IQR 1.9 mm, max. 5.3°) for MF. No statistical differences were found between manufacturing types (Kruskal–Wallis test, p = .05). Conclusions The study showed the method of implant guide fabrication did not affect the accuracy of implant placement within the limits of an in vitro environment. All methods resulted in implant placement which did not exceed the accepted safety deviation envelope (1.5–2.0 mm).

Abstract:Statement of problem. Three-dimensionally printed custom open trays have become a popular option in clinical implant dentistry because of advantages such as individualization, efficiency, and effectiveness. However, clinical evidence on their accuracy and feasibility is lacking.Purpose. The purpose of this clinical study was to evaluate the accuracy and feasibility for impressions of multiple implants by using 3D-printed custom open trays versus conventional custom open trays.Material and methods. Twenty-two partially edentulous individuals needing impression making for restorations supported by multiple implants were enrolled. Two types of custom open trays were made for each participant, a 3D-printed tray (test) and a conventional tray (control). With a splinted technique, silicone definitive impressions were obtained with the 2 custom open trays and poured with Type IV dental stone. Impression accuracy (primary outcome) was evaluated by measuring linear distances and the marginal gaps between the implant replicas and verification devices on the test and control casts. Clinical tray fit, impression quality, and cast quality were rated by an independent technician through a visual analog scale (VAS). The fabrication time and cost of the 2 types of custom open trays were recorded. The feasibility of 3D-printed trays was determined from these outcomes. The paired Student t test and Wilcoxon rank-sum tests were used for statistical analysis (a=.05). Results. For impression accuracy, no statistically significant difference was found between test and control groups (P>.05). In terms of clinical tray fit, impression quality, and cast quality, no statistically significant difference was found (all P>.05). Regarding fabrication time and cost, the test group (57.65 +/- 6.49 minutes, 0.37 +/- 0.07 United States dollars [USD]) exhibited superiority over the control group (101.96 +/- 2.92 minutes, 4.41 +/- 0.37 USD) (both P<.001).Conclusions. Within the limitations of this study, the 3D-printed custom open trays were clinically accurate, efficient, and cost-effective for impressions of multiple implants. (J Prosthet Dent 2022;128:396-403)
Effect of 3D printer, implant analog and angulation on the accuracy of analog position in implant casts

Abstract:Objectives: To evaluate the accumulative effect of 3D printer, implant analog systems, and implant angulation on the accuracy of analog position in implant casts. Methods: A reference cast, presenting a case of a three-unit implant-supported prosthesis, was scanned with a coordinate measurement machine, producing the first reference data set (CMM, n = 1). The second reference data set (n = 10) was prepared using an intraoral scanner (IOS) (Trios4). Test quadrant casts were produced using three DLP type 3D printers, Max (MAX UV385), Pro (PRO 4K65 UV), and Nex (NextDent 5100), and three implant analog systems, El (Elos), Nt (Nt-trading), and St (Straumann) (n = 90). Stone casts were also produced via analog impressions (Stone, n = 10). After digitization, the accuracy of 3D distance, local angulation (angle between implants) and global angulation (angle between the implant center axis and an axis perpendicular to the global plane) was evaluated by comparing the reference (CMM, IOS), test (3D print), and control (Stone) groups using metrology software. Data were statistically analyzed using three-way ANOVA and Tukey`s tests (α = 0.05). Results: IOS was truer in 3D implant distance and more precise in capturing local angulation than Stone (p ≤ 0.05). Other measurements were similar between both groups (p > 0.05). The amount of error introduced in the workflow by IOS and 3D printing was mostly similar (p > 0.05). 3D printed casts had similar or even higher accuracy than Stone group (p > 0.05). In most cases, higher trueness was achieved when using PRO 4K65 UV 3D printer and Elos implant analog system (p ≤ 0.05). Conclusion: 3D printer, implant analog system, and implant angulation have a significant effect on the accuracy of analog position in implant casts. Limited-span implant-supported cases could be reproduced digitally with similar accuracy as conventional methods. Clinical significance: A fully digital workflow with a carefully selected 3D printer and implant analog system can increase the accuracy of digitally produced implant casts with comparable accuracy to conventional workflow.

Abstract:Different printing technologies can be used for prosthetically oriented implant placement, however the influence of different printing orientations and steam sterilization remains unclear. In particular, no data is available for the novel technology Continuous Liquid Interface Production. The objective was to evaluate the dimensional accuracy of surgical guides manufactured with different printing techniques in vertical and horizontal printing orientation before and after steam sterilization. A total of 80 surgical guides were manufactured by means of continuous liquid interface production (CLIP; material: Keyguide, Keyprint), digital light processing (DLP; material: Luxaprint Ortho, DMG), stereolithography (SLA; Surgical guide, Formlabs), and fused filament fabrication (FFF; material: Clear Base Support, Arfona) in vertical and horizontal printing orientation (n = 10 per subgroup). Spheres were included in the design to determine the coordinates of 17 reference points. Each specimen was digitized with a laboratory scanner after additive manufacturing (AM) and after steam sterilization (134 °C). To determine the accuracy, root mean square values (RMS) were calculated and coordinates of the reference points were recorded. Based on the measured coordinates, deviations of the reference points and relevant distances were calculated. Paired t-tests and one-way ANOVA were applied for statistical analysis (significance p 0.05). Regarding reference points and distances, CLIP showed larger deviations compared to SLA in both printing orientations after steam sterilization, while DLP manufactured guides were the most dimensionally stable. In conclusion, the different printing technologies and orientations had little effect on the manufacturing accuracy of the surgical guides before sterilization. However, after sterilization, FFF surgical guides exhibited significant deformation making their clinical use impossible. CLIP showed larger deformations due to steam sterilization than the other photopolymerizing techniques, however, discrepancies may be considered within the range of clinical acceptance. The influence on the implant position remains to be evaluated.

Abstract:Purpose: To evaluate the effects of two base types and three restoration designs on the resin consumption and trueness of the 3D-printed dental casts. Additionally, the study explored the dimensional stability of these 3D-printed dental casts after 1 year of storage. Materials and methods: Various types of reference dental casts were specifically designed to represent three types of dental restoration fabrications, including full-arch (FA), long-span (LS), and single-unit (SU) prostheses. The reference casts were digitized with a dental laboratory scanner and used to create flat and hollow base designs (N = 18) for the 3D-printed study casts. The 3D-printed study casts were digitized and evaluated against their corresponding references immediately after 3D printing and again after 1 year of storage, with the trueness quantified using the root mean square error (RMSE) at both time points. Volumes of resin used were recorded to measure resin consumption, and the weights of the 3D-printed study casts were also measured. The data were analyzed using two-way ANOVA and a Tukey post hoc test, α = 0.05. Results: Volumetric analysis showed the flat-base design had significantly higher resin consumption with weights for the FA group at 42.51 ± 0.16 g, the LS group at 31.64 ± 0.07 g, and the SU group at 27.67 ± 0.31 g, as opposed to 26.22 ± 1.01 g, 22.86 ± 0.93 g, and 20.10 ± 0.19 g for the hollow designs respectively (p < 0.001). Trueness, assessed through two-way ANOVA, revealed that the flat-base design had lower RMSE values indicating better trueness in the LS (54 ± 6 µm) and SU (59 ± 7 µm) groups compared to the hollow-base design (LS: 73 ± 5, SU: 99 ± 11 µm, both p < 0.001), with no significant difference in the FA group (flat-base: 50 ± 3, hollow: 47 ± 5 µm, p = 0.398). After 1 year, the flat-base design demonstrated superior dimensional stability in the LS (flat base: 56 ± 6 µm, hollow base: 149 ±45 µm, p < 0.001) and SU groups (flat base: 95 ± 8 µm, hollow base: 183 ±27 µm, p < 0.001), with the FA group showing no significant difference in the base design (flat base: 47 ± 9, hollow base: 62 ± 12 µm, p = 0.428). Conclusions: The hollow-base design group showed lower resin consumption than the flat-base design group. However, the flat-base designs exhibited superior trueness and less distortion after 1 year of storage. These findings indicate that despite the higher material usage, flat-base designs provide better initial accuracy and maintain their dimensional stability over time for most groups.

Abstract:Background: Three-dimensional (3D)-printed osteotomy guides and personalized 3D-printed prostheses have been used in pelvic tumor resection and reconstruction, and achieved good results. However, there is a lack of methods to evaluate its precision. This study aimed to establish a novel method to evaluate the precision of osteotomy and prosthesis installation using our 3D printing-based system in pelvic tumor surgeries. Methods: A retrospective analysis was conducted on 24 patients with malignant pelvic tumors who underwent surgery with 3D-printed osteotomy guides and personalized prostheses between April 2014 and June 2024. Data regarding the preoperative design plans and postoperative follow-up imaging were collected. Additionally, 3D reconstruction was conducted to compare and measure. The deviation was categorized into “Excellent” (±5 mm), “Good” (±10 mm), and “Fair” (±10 mm and above). The deviation distance of osteotomy was defined as the absolute value of the difference between the actual safe distance and preoperatively planned safety margin. The deviation distance of acetabular center was defined as the straight-line distance between preoperatively planned acetabular center and the reconstructed acetabular center. Results: The osteotomy satisfaction rate was 87.5%, with 21 cases classified as “Excellent” and 3 as “Good”. The average deviation distance of osteotomy using 3D-printed guides was 2.74 ± 2.36mm (range, 0.13–9.29 mm). The average deviation distance of the acetabular centers reconstructed with 3D-printed personalized prostheses was 5.57 ± 3.71mm (range, 0.91–15.94 mm). Conclusions: The study provides a theoretical basis for the postoperative precision evaluation of 3D printing‐based system for pelvic tumor resection and reconstruction.
A Review on 3D-Printed Templates for Precontouring Fixation Plates in Orthopedic Surgery

Abstract:This paper is a systematic review of the literature on 3D-printed anatomical replicas used as templates for precontouring the fixation plates in orthopedic surgery. Embase, PubMed, Cochrane, Scopus and Springer databases were consulted for information on design study, fracture anatomical location, number of patients, surgical technique, virtual modeling approach and 3D printing process. The initial search provided a total of 496 records. After removing the duplicates, the title and abstract screening, and applying exclusion criteria and citations searching, 30 papers were declared eligible and included in the final synthesis. Seven studies were identified as focusing on retrospective non-randomized series of clinical cases, while two papers presented randomized case control studies. Two main approaches were highlighted in developing 3D-printed anatomical models for precontouring fixation plates: (a.) medical reconstruction, virtual planning and fracture reduction followed by 3D printing the model; (b.) medical reconstruction followed by 3D printing the model of the mirrored uninjured side. Revised studies reported advantages such as surgical time and blood loss reduction, while the reduction quality is similar with that of the conventional surgery. During the last couple of years there was an increase in the number of studies focused on precontouring orthopedic plates using 3D printing technology. Three-dimensionally-printed templates for plate precontouring were mostly used for acetabular fractures. Knowledge on medical virtual modeling and reconstruction is mandatory.
