Abstract:We report the experimental validation of a detection module based on a LaBr3(Ce+Sr) scintillator crystal, read by a matrix of Silicon Photomultipliers (SiPMs), for the development of a SPECT system for dose monitoring in Boron Neutron Capture Therapy (BNCT). The goal of the system is to detect the gamma rays at 478keV emitted by the excited 7Li produced in the 10B(n, α)7Li reaction, to have a real-time localization and quantification of the local dose released to a patient during BNCT treatment. The good energy resolution of the detector (<3% at 662keV) allows to resolve the photopeak at 478keV from the adjacent annihilation photons photopeak at 511keV, originated from pair production of high energy gamma rays, such as the hydrogen neutron capture gamma rays at 2.2MeV. The results shown in this article demonstrate the suitability of our detector in view of future application in BNCT real-time dose monitoring. Good linearity (RMSE < 2.5cps) between the number of events detected at 478keV and the boron concentration of the 10B-loaded samples has been achieved, down to a boron concentration of 62ppm with a neutron flux of approximately 1 × 105 n/cm2/s.

Abstract:We present a study on the neutron activation of a gamma-ray detector for a BNCT-SPECT dose imaging system. The detector is based on a LaBr 3 (Ce+Sr) scintillator crystal, coupled with a matrix of Silicon Photomultipliers (SiPMs), read by a dedicated electronics system. This detector has successfully demonstrated to be capable to identify the 10 B compounds when irradiating borated vials with thermal neutrons. However, a background signal around 478 keV was detected, suggesting the activation of the detector itself. This study aims to determine the origin of this background signal by simulating the two main parts of the detector, which are the crystal and electronic boards, in order to assess their contribution to the background signal. The results of the FLUKA simulations show that the neutron capture reactions on both the crystal and electronic boards cause a relevant background nearby the BNCT signal, thereby limiting the detector's sensitivity. To address this issue, a customized cadmium shielding has been developed. This solution was tested at the TRIGA Mark II research nuclear reactor of Pavia University, where experimental measurements and corresponding FLUKA simulations proved its effectiveness.

Abstract:Within our activities on accelerator-based boron neutron capture therapy (BNCT) carried out at the Tandar Laboratory (Comisión Nacional de Energía Atómica, Argentina) we present here the study and design of a tomographic imaging system for the measurement of the spatial distribution of the absorbed dose during a BNCT session. The 10B (n,alpha) 7Li boron neutron capture reaction produces a 478 keV gamma ray in 94 % of the cases. In BNCT a large fraction of this radiation escapes from the patient body. Its detection is thus very attractive for a noninvasive boron concentration measurement and an online absorbed dose evaluation since the dose absorbed by the tumor and healthy tissue strongly depends on the boron uptake and the neutron flux. For this purpose, a dedicated tomographic imaging system based on SPECT (Single Photon Emission Computed Tomography, a diagnostic medical imaging modality used in nuclear medicine) is proposed since standard SPECT cameras cannot be used due to the photon energy ( 478 keV ) and the particular background gamma field in BNCT. A detection system (collimator and detectors) is proposed. Monte Carlo numerical simulations are used for an implementation of a statistical algorithm used for the tomographic image reconstruction.

Abstract:In boron neutron capture therapy (BNCT), patient dose is contingent upon boron concentration and neutron irradiation. This study details the creation and implementation of a real-time beam-monitoring system using a Eu:LiCaAlF 6 (LiCAF) scintillation detector. This system precisely regulates neutron irradiation according to the treatment plan. A two-step calibration process was employed for enhanced reliability. The initial calibration used an Americium–Beryllium (AmBe) source to isolate neutron signals from a mix of gamma and neutron signals. The second calibration involved a polymethyl methacrylate (PMMA) moderator within the BNCT beam, calibrated against the gold-foil activation method. This yielded a thermal neutron reaction rate with an uncertainty of approximately 2.62 % (k = 2). For continuous monitoring, the system includes a detector with a polyethylene (PE) moderator and an internal LiCAF placed inside the beam port. This monitoring system calculates and displays the epithermal neutron flux by comparing the flux at the beam port to the thermal neutron flux from the moderator. The performance of the installed system was evaluated, noting an uncertainty in epithermal neutron conversion of 3.3 % (k = 2) and a linearity exceeding 0.99 (R 2 > 0.99). Operational from December 2022 to April 2023 in clinical trials, the system was subjected to regular quality assurance checks on an annual, monthly, and daily basis, showing no performance degradation or unusual operation. Future work will focus on collecting data to determine the lifespan of the LiCAF detector through continued monitoring.

Abstract:Boron Neutron Capture Therapy (BNCT) is a binary cancer therapy where a low energy neutron beam is incident upon a patient who has been administered a tumour-seeking 10 B loaded compound. The neutron capture reaction on 10 B results in the production of two short range particles, 7 Li and 4 He, that deposit all of their energies within the targeted cell. However, accurate, online dosimetry during BNCT is challenging as it requires knowledge of both the neutron fluence and 10 B concentration in cells. An additional product in the neutron capture reaction on 10 B is a 478 keV prompt gamma ray, and if the production vertices of these gamma rays could be imaged by an external camera, the density of the vertices could be used to infer the dose delivered to the patient. In this study, the feasibility of using an array of LaBr 3 scintillators as a modified Compton camera for prompt gamma imaging during BNCT was investigated using Geant4 simulations. These simulations demonstrated that a phantom containing a 3 cm diameter region of 400 ppm 10 B could be reconstructed using clinically relevant neutron fluences. This result opens up more possibilities for future research to improve dosimetry during BNCT.

Abstract:Boron Neutron Capture Therapy (BNCT) is a radiotherapy whose effect is due to the combination of low-energy neutron irradiation prior chemical targeting of tumour tissues with boron-10. The context of this work is the design of a clinical BNCT facility based on a Radio-Frequency Quadrupole proton accelerator, manufactured by the Italian National Institute of Nuclear Physics (INFN). Such a machine can provide a neutron beam suitable for the treatment of deep-seated tumours when coupled to a beryllium target and a Beam Shaping Assembly whose main constituent is densified lithiated aluminium fluoride. This paper refines the design of the facility, already addressed in preliminary studies, from the point of view of dosimetric quantities in the room and in the patient. The presented calculations have been performed with different Monte Carlo codes, and assess the time evolution of the dose due to the neutron activation of the irradiated materials. A comprehensive evaluation of the dose absorbed in the healthy organs of the patient is provided, with focus on the residual radioactivity of the patient urine, for three different treatment positions. Borated concrete is confirmed to be the best material for constructing the walls of the treatment room. A lead shielding at the beam-port is proposed to reduce the gamma dose at the irradiation position due to the activation of the Beam Shaping Assembly.

Abstract:Boron Neutron Capture Therapy (BNCT) is a therapeutic treatment for malignant tumors that utilizes the nuclear reactions that happen when thermal neutrons are captured by boron-10 atoms to selectively destroy designated cells. Boron-10 atoms are biochemically accumulated inside the tumor target, which is then irradiated with thermal neutrons. In recent years, the possibility to obtain accelerator based intense neutron beams has given a boost to the diffusion of BNCT also in Europe, removing the need of nuclear reactors. In this contest, the monitoring and characterization of the epithermal neutron beams dedicated to BNCT becomes an important issue. The directional neutron spectrometer called NCT-WES (Neutron Capture Therapy Wide Energy Spectrometer) is a single moderator neutron spectrometer composed of a polyethylene cylinder embedding six semiconductor-based detectors positioned at different depths along the cylinder axes. The position of the six detectors is studied in order to maximize the response of each one in a selected neutron energy range. The unfolding of the six simultaneous readings allows to reconstruct the incoming neutron energy spectrum as in a parallelized Bonner Sphere System. A cylindrical collimator situated in the front of the spectrometer makes the instrument sensitive to neutrons coming only from a given direction, which allows to exclude the contribution of the room scattered radiation. The experimental validation of the spectrometer, obtained through several measuring campaigns, is reported and discussed.

Abstract:Boron neutron capture therapy (BNCT) is a selective biological targeted nuclide technique for cancer therapy. It has the following attractive features: good targeting, high effectiveness, and causes slight damage to surrounding healthy tissue compared with other traditional methods. It has been considered as one of the promising methods for the treatment of various cancers. Measuring 10B concentrations is vital for BNCT. However, the existing technology and equipment cannot satisfy the real-time and accurate measurement requirements, and more efficient methods are in demand. The development of methods and imaging applied in BNCT to help measure boron concentration is described in this review.

Abstract:Boron Neutron Capture Therapy (BNCT) is a radiation treatment modality for cancer based on the 10 B(n,α) 7 Li (BNC) reaction. Knowledge of the spatial location of boron atoms is essential when considering a BNCT treatment, since the BNC reaction products cause lethal damage in a range comparable to cell diameter. Neutron autoradiography is a technique able to provide this information with high resolution at relatively low cost. First a boron-loaded biological matrix is laid on a nuclear track detector (NTD). By irradiating this assembly with thermal neutrons, the emitted particles of the BNC reaction produce damaged zones in the NTD along each incident ion trajectory, and by mapping these nuclear tracks boron spatial distribution in the sample can be assessed. In our laboratory, we have established different approaches: high fluence autoradiography (HFA), to observe boron microdistribution through differences in shades of gray; quantitative autoradiography (QTA), which allows the correlation between track density measurements and boron concentration; and UV-C sensitization of polymers (UVC-a), which produces an imprint of the biological material in the NTD, thus improving the spatial resolution. Moreover, we aim to develop a technique using commercial CMOS sensors in order to simultaneously detect the BNC events and the image of the biological sample during irradiation. The objective of this work is to describe the mentioned approaches and to discuss future perspectives.

Abstract:Boron Neutron Capture Therapy (BNCT) is a binary radiation therapy which is able to selectively destroy malignant cells while sparing the normal tissue. A 10B containing drug able to target neoplastic cells is administered to the patient which is then irradiated with thermal neutrons that induce the 10B(n,alpha)7Li capture reaction. Therefore BNCT therapy effectiveness is strongly dependent on the ability to induce a high concentration of 10B inside the neoplastic tissue. The correct evaluation of such quantity and of the dose deposited in the tumour is a key element to further enhance BNCT effectiveness. The current methods clinically used to estimate the boron concentration are indirect. To obtain a direct and real-time quantification of the dose delivered to the tumour a BNCT-SPECT system has been proposed. SPECT imaging in BNCT is based on the 478 keV photon emitted in the 94% of the cases due to the 10B thermal neutron capture. To develop a BNCT-SPECT imaging system a CdZnTe (CZT) detector was chosen because it has a high energy resolution and can be employed at room temperature. This study focused on a simulation study of a small 20x20x20 mm3 CZT detector as the base element of the SPECT imaging system. The system was first simulated in an ideal setup using a virtual collimator of 1 mm aperture and a 7 mm spatial resolution was obtained. Then it was modified to obtain more realistic cases using multiple sources. The more realistic cases showed a spatial resolution that could be improved by using a bigger sensitive volume and using smaller angular steps for the data acquisition, but it was still possible to reconstruct the image of the simulated source and to discriminate multiple sources within a phantom. This work showed the capabilities of a small 20x20x20 mm3 CZT detector as a SPECT imager in BNCT and it also showed that with some improvements, a CZT detector can be used as a base for a BNCT-SPECT imaging system.

Abstract:One important issue in Boron Neutron Capture Therapy is the delivered dose to the tissues outside the tumor. An international standard for light ion beam systems sets two recommended limits for out-of-field dose based on distance from the field edge: maximum absorbed dose from all radiation types shall not exceed 0.5 % of the maximum dose at distances 15 cm to 50 cm from the field edge. At distances >50 cm from the field edge, the maximum absorbed dose shall not exceed 0.1 %. This paper is a continuation of our previous works focused on the design of an accelerator-based neutron source for BNCT. We already designed a novel Beam Shape Assembly which meets the IAEA criteria for BNCT treatments. Using this BSA, in the present work, we characterize by Monte Carlo simulations the dose outside the neutron field. The out-of-field dose has been assessed via estimates using the ambient and equivalent dose. Also the boron uptake in healthy tissues has been analyzed for the equivalent dose computation. It is concluded that our design for a future accelerator-based source for BNCT meets reasonably well the criteria defined from other forms of radiotherapy on both equivalent and effective dose outside the field.

Abstract:Selective accumulation of B-10 compound in tumour tissue is a fundamental condition for the achievement of BNCT (Boron Neutron Capture Therapy), since the effectiveness of therapy irradiation derives just from neutron capture reaction of B-10. Hence, the determination of the B-10 concentration ratio, between tumour and healthy tissue, and a control of this ratio, during the therapy, are essential to optimise the effectiveness of the BNCT, which it is known to be based on the selective uptake of B-10 compound. In this work, experimental methods are proposed and evaluated for the determination in vivo of B-10 compound in biological samples, in particular based on neutron radiography and gammaray spectroscopy by telescopic system. Measures and Monte Carlo calculations have been performed to investigate the possibility of executing imaging of the 10B distribution, both by radiography with thermal neutrons, using 6LiF/ZnS:Ag scintillator screen and a CCD camera, and by spectroscopy, based on the revelation of gamma-ray reaction products from B-10 and the H. A rebuilding algorithm has been implemented. The present study has been done for the standard case of B-10 uptake, as well as for proposed case in which, to the same carrier, is also synthesized Gd-157, in the amount of is used like a contrast agent in NMRI.

Abstract:Boron Neutron Capture Therapy (BNCT) is a promising cancer therapy, which has recently been in practical use in Japan especially using an accelerator. In BNCT real-time measurement of local boron dose is required. In the present study, a novel design of a SPECT system for BNCT (BNCT-SPECT) has been carried out to realize estimation of the local boron dose, i.e., treatment effect of BNCT. Necessary performance which BNCT community requires includes accuracy of 5% and spatial resolution of 5 mm, which are regarded to be difficult to realize. A possible design was investigated to meet the requirements. The design results we achieved are as follows: As for the elemental detection device, GAGG (3.5 × 3.5 × 30 mm<sup>3</sup>) was selected, and for the collimator, the collimator hole diameter was 3.5 mm, the collimator hole pitch was 4 mm and the collimator length was 26 cm. For the obtained performance with the design, the accuracy was 4.4% and the spatial resolution was 5.1 mm. Currently prior to production of the real system, a prototype of BNCT-SPECT is being developed to acquire real projection data to confirm the performance and examine our own image reconstruction method with the obtained projection data.

Abstract:One of the main problems associated with the implementation of neutron capture therapy in clinical practice, requiring a solution, is the determination of absorbed dose. The only method that directly allows measuring the absorbed does is gamma spectrometry based on the detection of gamma rays with an energy of 478 keV. This article considers the possibility of using a xenon gamma spectrometer, which has high radiation resistance to neutron fluxes. The GEANT4 software package was used to study the neutron capture therapy process and conduct corresponding calculations.

Abstract:This paper explores the adaptation and application of i-TED Compton imagers for real-time dosimetry in Boron Neutron Capture Therapy (BNCT). The i-TED array, previously utilized in nuclear astrophysics experiments at CERN, is being optimized for detecting and imaging 478 keV gamma-rays, critical for accurate BNCT dosimetry. Detailed Monte Carlo simulations were used to optimize the i-TED detector configuration and enhance its performance in the challenging radiation environment typical of BNCT. Additionally, advanced 3D image reconstruction algorithms, including a combination of back-projection and List-Mode Maximum Likelihood Expectation Maximization (LM-MLEM), are implemented and validated through simulations. Preliminary experimental tests at the Institut Laue-Langevin (ILL) demonstrate the potential of i-TED in a clinical setting, with ongoing experiments focusing on improving imaging capabilities in realistic BNCT conditions.

Abstract:Boron Neutron Capture Therapy for liver malignancies is being investigated at the University of Mainz. One important aim is the set-up of a reliable dosimetry system. Alanine dosimeters have previously been applied for dosimetry of mixed radiation fields in antiproton therapy, and may be suitable for measurements in mixed neutron and gamma fields. Material and methods: Two experiments have been carried out in the thermal column of the TRIGA Mark II reactor at the University of Mainz. Alanine dosimeters have been irradiated in a phantom and in liver tissue. Results: For the interpretation and prediction of the dose for each pellet, beside the results of the measurements, calculations with the Monte Carlo code FLUKA are presented here. For the phantom, as well as for the liver tissue, the measured and calculated dose and flux values are in good agreement. Discussion: Alanine dosimeters, in combination with flux measurements and Monte Carlo calculations with FLUKA, suggest that it is possible to establish a system for monitoring the dose in a mixed neutron and gamma field for BNCT and other applications in radiotherapy.

Abstract:Boron neutron capture therapy (BNCT) is a high-dose-intensive radiation therapy that has gained popularity due to advancements in accelerator neutron sources. To determine the dose for BNCT, it is necessary to know the difficult-to-determine boron concentration and neutron fluence. To estimate this dose, we propose a method of measuring the prompt γ-rays (PGs) from the boron neutron capture reaction (BNCR) using a Compton camera. We performed a fundamental experiment to verify basic imaging performance and the ability to discern the PGs from 511 keV annihilation γ-rays. A Si/CdTe Compton camera was used to image the BNCR and showed an energy peak of 478 keV PGs, separate from the annihilation γ-ray peak. The Compton camera could visualize the boron target with low neutron intensity and high boron concentration. This study experimentally confirms the ability of Si/CdTe Compton cameras to image BNCRs.

Abstract:Recently, new high-resolution cadmium-zinc-telluride (CZT) drift strip detectors for room temperature gamma-ray spectroscopic imaging were developed by our group. The CZT detectors equipped with orthogonal anode/cathode collecting strips, drift strips and dedicated pulse processing allow a detection area of 6 × 20 mm2 and excellent room temperature spectroscopic performance (0.82% FWHM at 661.7 keV). In this work, we investigated the potentialities of these detectors for prompt gamma-ray spectroscopy (PGS) in boron neutron capture therapy (BNCT). The detectors, exploiting the measurement of the 478 keV prompt gamma rays emitted by 94% 7Li nuclides from the 10B(n, α)7Li reaction, are very appealing for the development of single-photon emission computed tomography (SPECT) systems and Compton cameras in BNCT. High-resolution gamma-ray spectra from 10B samples under thermal neutrons were measured at the T.R.I.G.A. Mark II research nuclear reactor of the University of Pavia (Italy).
Catalase Overexpression Fails to Attenuate Allergic Airways Disease in the Mouse1

Abstract:Oxidative stress is a hallmark of asthma, and increased levels of oxidants are considered markers of the inflammatory process. Most studies to date addressing the role of oxidants in the etiology of asthma were based on the therapeutic administration of low m.w. antioxidants or antioxidant mimetic compounds. To directly address the function of endogenous hydrogen peroxide in the pathophysiology of allergic airway disease, we comparatively evaluated mice systemically overexpressing catalase, a major antioxidant enzyme that detoxifies hydrogen peroxide, and C57BL/6 strain matched controls in the OVA model of allergic airways disease. Catalase transgenic mice had 8-fold increases in catalase activity in lung tissue, and had lowered DCF oxidation in tracheal epithelial cells, compared with C57BL/6 controls. Despite these differences, both strains showed similar increases in OVA-specific IgE, IgG1, and IgG2a levels, comparable airway and tissue inflammation, and identical increases in procollagen 1 mRNA expression, following sensitization and challenge with OVA. Unexpectedly, mRNA expression of MUC5AC and CLCA3 genes were enhanced in catalase transgenic mice, compared with C57BL/6 mice subjected to Ag. Furthermore, when compared with control mice, catalase overexpression increased airway hyperresponsiveness to methacholine both in naive mice as well as in response to Ag. In contrast to the prevailing notion that hydrogen peroxide is positively associated with the etiology of allergic airways disease, the current findings suggest that endogenous hydrogen peroxide serves a role in suppressing both mucus production and airway hyperresponsiveness.
