Abstract:Background: The use of Mechanical Circulatory Support (MCS) devices in cardiogenic shock (CS) is growing. However, the recent trends in using different MCS modalities and their outcomes in acute myocardial infarction associated CS (AMI-CS) are unknown. Methods: The national readmission database (2016-2020) was used to identify AMI-CS requiring MCS. Cohorts were stratified as ECMO compared to Impella. Propensity score matching (PSM) was used to remove confounding factors. Pearson's x2 test was applied to matched cohorts to compare outcomes. We used multivariate regression and reported predictive margins for adjusted trend analysis. Results: Among 20,950 AMI-CS hospitalizations requiring MCS, 19,628 (93.7 %) received Impella vs 1322 (6.3 %) were placed only on ECMO. ECMO group was younger (median age: 61 vs. 68 years, p < 0.001) and had a lower comorbidity burden. On propensity-matched cohorts (N 742), the ECMO cohort had higher adverse events, including mortality (51.6 % vs. 41.5 %), sudden cardiac arrest (SCA) (40.9 % vs. 31.8 %), acute stroke (9.2 % vs. 4.6 %) and major bleeding (16 % vs 12.2 %) [p < 0.05]. However, comparing ECPELLA (ECMO + Impella) to Impella alone, mortality (46.2 % vs. 39.4 %) and SCA (44 % vs. 36.4 %) rates were similar, though major bleeding was higher (18.2 % vs. 9.8 %). From 2016 to 2020, mortality trends for AMI-CS in the U.S. showed no significant change (p-trend: 0.071). Conclusion: Despite advances in MCS modalities, the overall mortality rate for AMI-CS remains unchanged. ECMO use without LV unloading showed higher mortality and adverse events compared to Impella. Prospective studies are needed to verify these findings.

Abstract:Cardiogenic shock complicating acute myocardial infarction (AMI-CS) remains a critical condition with high morbimortality. Despite advances, gender disparities persist in the management of this condition. Our aim was to evaluate gender differences in the utilization and outcomes of mechanical circulatory support (MCS) in AMI-CS. Additionally, we addressed the management strategies for AMI-CS stratified by race/ethnicity. We performed a retrospective study using the National Inpatient Sample 2016-2019 and including admissions with AMI-CS. The use of MCS and outcomes were compared between genders using inverse probability of treatment weighting (IPTW). Odds ratios (OR) with their 95% confidence interval (CI) were estimated. A total of 151,560 admissions for AMI-CS were analyzed, including 98,855 male and 52,705 female patients. After IPTW adjustment, females were significantly less likely to receive MCS (OR: 0.77, 95% CI: 0.73-0.81, p<0.001) and had higher in-hospital mortality rates (OR: 1.09, 95% CI: 1.00-1.18, p=0.045) than males. No significant gender differences were observed in major bleeding, renal replacement therapy, or mechanical ventilation. Racial disparities were also evident, with female patients across all racial groups being less likely to receive MCS compared to male counterparts. In conclusion, significant gender disparities exist in the utilization and outcomes of MCS in patients with AMI-CS. Female patients are less likely to receive MCS and have higher in-hospital mortality rates compared to male patients. Racial minorities, including Black and Hispanic patients, experience lower utilization of MCS compared to White patients. These findings underscore the complex interplay between gender, race/ethnicity, and healthcare access and outcomes.

Abstract:Introduction: Acute myocardial infarction complicated by cardiogenic shock (AMI-CS) mortality remains high despite revascularization and the use of the intra-aortic balloon pump (IABP). Advanced mechanical circulatory support (MCS) devices, such as catheter-based ventricular assist devices (cVAD), may impact mortality. We aim to identify predictors of mortality in AMI-CS implanted with IABP and the proportion eligible for advanced MCS in an Asian population. Methods: We retrospectively analyzed a cohort of Society for Cardiovascular Angiography and Intervention (SCAI) stage C and above AMI-CS patients with IABP implanted from 2017–2019. We excluded patients who had IABP implanted for indications other than AMI-CS. Primary outcome was 30-day mortality. Binary logistic regression was used to calculate adjusted odds ratios (aOR) for patient characteristics. Results: Over the 3-year period, 242 patients (mean age 64.1 ± 12.4 years, 88% males) with AMI-CS had IABP implanted. 30-day mortality was 55%. On univariate analysis, cardiac arrest (p < 0.001), inotrope/vasopressor use prior to IABP (p = 0.004) was more common in non-survivors. Non-survivors were less likely to be smokers (p = 0.001), had lower ejection fraction, higher creatinine/ lactate and lower pH (all p < 0.001). On multi-variate analysis, predictors of mortality were cardiac arrest prior to IABP (aOR 4.00, CI 2.28–7.03), inotrope/vasopressor prior to IABP (aOR 2.41, CI 1.18–4.96), lower arterial pH (aOR 0.02, CI 0.00–0.31), higher lactate (aOR 2.42, CI 1.00–1.19), and lower hemoglobin (aOR 0.83, CI 0.71–0.98). Using institutional MCS criteria, 106 patients (44%) would have qualified for advanced MCS. Conclusions: Early mortality in AMI-CS remains high despite IABP. Many patients would have qualified for higher degrees of MCS.

Abstract:Little is known about the impact of the downgrade of guideline recommendations for intra-aortic balloon pump (IABP) use and the approval of the Impella in Japan, where IABPs have been predominantly used. This study aimed to describe the annual trends in the mechanical circulatory support (MCS) use and outcomes in patients with cardiogenic shock (CS) requiring MCS. Using the Japanese Diagnosis Procedure Combination database from July 2010 to March 2021, we identified inpatients with CS requiring MCS. The patients were stratified into 3 groups: (1) IABP alone, (2) Impella alone, and (3) extracorporeal membrane oxygenation (ECMO), regardless of IABP or Impella use. The patient characteristics and outcomes were reported by the fiscal year. Of the 160,559 eligible patients, 117,599 (73.2%) used IABP alone, 1,465 (0.9%) Impella alone, and 41,495 (25.8%) ECMO. The prevalence of the use of an IABP alone significantly decreased from 80.5% in 2010 to 65.3% in 2020 (p for trend <0.001), whereas the prevalence of the use of an Impella alone significantly increased from 0.0% to 5.0% and ECMO from 19.5% to 29.6% (p for trend <0.001 for both). In-hospital mortality significantly increased from 29.3% in 2010 to 32.6% in 2020 in the overall patients with CS requiring MCS but significantly decreased in those requiring ECMO from 73.7% to 64.1% (p for trend <0.001 for both). In conclusion, there were significant annual changes in the patterns of MCS use and clinical outcomes in patients with CS requiring MCS.
Early vs. Delayed Mechanical Circulatory Support in Patients with Acute Myocardial Infarction and Cardiogenic Shock

Abstract:Abstract Background Despite increased temporary mechanical circulatory support (tMCS) utilization for acute myocardial infarction complicated by cardiogenic shock (AMI-CS), data regarding efficacy and optimal timing for tMCS support are limited. This study aimed to describe outcomes based on tMCS timing in AMI-CS and to identify predictors of 30-day mortality and readmission. Methods Patients with AMI-CS identified in the National Readmissions Database were grouped according to the use of tMCS and early (<24 hours) vs. delayed (≥24 hours) tMCS. The correlation between tMCS timing and inpatient outcomes was evaluated using linear regression. Multivariate logistic regression was used to identify variables associated with 30-day mortality and readmission. Results Of 294,839 patients with AMI-CS, 109,148 patients were supported with tMCS (8,067 veno-arterial extracorporeal membrane oxygenation, 33,577 Impella, and 79,161 intra-aortic balloon pump). Of patients requiring tMCS, patients who received early tMCS (n = 79,906) had shorter lengths of stay (7 days vs. 15 days, p < 0.001) and lower rates of ischemic and bleeding complications than those with delayed tMCS (n = 32,241). Patients requiring tMCS had higher in-hospital mortality (OR [95% CI]) (1.7 [1.7-1.8], p < 0.001). Among patients requiring tMCS, early support was associated with fewer complications, lower mortality (0.90 [0.85-0.94], p < 0.001), and fewer 30-day readmissions (0.91 [0.85-0.97], p = 0.005) compared to patients with delayed tMCS. Conclusion Among patients receiving tMCS for AMI-CS, early tMCS was associated with fewer complications, shorter lengths of stay, lower hospital costs, and fewer deaths and readmissions at 30 days.

Abstract:Background There is limited data on the safety outcomes of mechanical circulatory support (MCS) for Acute Myocardial Infarction related Cardiogenic shock (CS) among elderly patients. Methods This study queried the National Inpatient Sample (NIS) from 2016-2020 to identify AMI-related CS patients aged>80 supported with an Intra-aortic balloon pump (IABP), percutaneous ventricular assist device (pVAD) or Extracorporeal membrane oxygenation (ECMO). We analyzed short-term outcomes as well as resource utilization in these hospitalizations. Results A total of 114,265 AMI-CS hospitalizations were identified out of which 16010 were supported with the MCS. IABP was the most frequently used MCS (72.6%), followed by pVAD (25.9%) followed by ECMO (1.47%). The median age of the patients supported with MCS was 84 (82-87) mostly among the whites. ECMO use was associated with higher odds of mortality, and stroke (59.57%, 10.64%) compared to IABP (39.33%,3.14%) and pVAD (51.45%,2.77%) (p <.0001 for all). Moreover, ECMO use was associated with longer hospital stay (median, IQR 9 (3-19) vs IABP: 7 (3-12), pVAD: 6 (2-13)) and mean total hospitalization cost (119204.06$ vs IABP: 52224.85$ vs ECMO: 78288.30$). Conclusions IABP was frequently utilized MCS among the elderly, however, ECMO had the worst outcome in terms of higher odds of in-hospital mortality and stroke rates as expected since these patients are sicker. Prospective and long-term data are needed to evaluate the optimal safety of MCS for elderly AMI-CS patients.

Abstract:Introduction Mechanical circulatory support (MCS) is often deployed in patients with refractory cardiogenic shock complicating acute myocardial infarction (AMI-CS). Nevertheless, the mortality rate in these patients remains utterly high, around 50%. Purpose The aim of this study was to identify patient characteristics associated with 30-day mortality in patients receiving MCS for AMI-CS. Methods In the Netherlands, all patients undergoing percutaneous coronary intervention (PCI) are prospectively entered in the Netherlands Heart Registration (NHR). Additional data were retrieved for all CS patients between 2017-2021 in 14 Dutch hospitals, including MCS usage. Survivors and non-survivors amongst MCS treated patients were compared to identify patient characteristics associated with 30-day mortality. In addition, multivariate logistic regression analysis on imputed data was performed to quantify these associations in adjustment for confounders. Results MCS was deployed in 516 (23.3%) of all AMI-CS patients. MCS patients had a mean age of 64.2 (± 12.5) years and 74.6% were male. An intra-aortic balloon pump (IABP) was used in 49.0% of the patients. Impella, extracorporeal membrane oxygenation (ECMO) and multiple devices were used in 18.2%, 13.2% and 18.4% respectively. The overall 30-day mortality rate was 55.0%. At baseline, MCS survivors were younger (62.8 vs 65.2, p=0.03), had diabetes less often (18.4 vs. 28.0, p=0.02), and presented with a higher mean arterial pressure (MAP) (77.9 vs. 71.7, p<0.01). An out-of-hospital cardiac arrest (OHCA) preceded in 34.3% of the survivors versus 42.0% of the non-survivors (p=0.09). Survivors also had lower admission levels of lactate (5.0 vs. 7.8, p<0.01), glucose (12.1 vs. 15.2, p<0.01) and creatinine (94 vs. 115, p<0.01). Multivessel disease (MVD) was present less frequently in survivors compared to non-survivors (62.1% vs. 74.2%, p<0.01). In-hospital cardiac arrest (IHCA) occurred less often in survivors both prior to PCI (8.2% vs. 18.0%, p<0.01) and during PCI (22.8% vs. 34.6%, p<0.01). After multivariate logistic regression, higher age (OR 1.04, 95%CI: 1.02-1.05), resuscitation prior to (OR 2.11, 95 %CI: 1.09-4.09) or during PCI (OR 1.58, 95% CI: 1.01-2.47) and the presence of MVD (OR 2.07, 95% CI: 1.32-3.25) were independently associated with 30-day mortality in AMI-CS patients who received MCS. Conclusion In AMI-CS patients who received MCS, older age, the occurrence of an IHCA prior to or during PCI, and the presence of MVD were independently associated with an increased 30-day mortality.Figure 1.Table 1.

Abstract:Introduction Intra-aortic balloon pump (IABP) has been used since 1960s. Randomized controlled trials demonstrated neither benefit nor harm with IABP use in cardiogenic shock complicating acute coronary syndrome (ACS-CS). However, the clinical trajectory of non-ischemic cardiogenic shock (NI-CS) differs significantly from ACS-CS. There has been increasing interest in the use of IABP to stabilize patients with NI-CS. We aimed to study trends in outcomes of IABP use in NI-CS. Method This is a retrospective cohort analysis using 2016 to 2020 National Inpatient Sample. The International Classification of Diseases, Tenth Revision (ICD- 10) Clinical Modification codes and Procedure codes were used to identify adult patients (equal or >18 years) with NI-CS who were managed with IABP solely. We excluded patients with STEMI, NSTEMI and any revascularization procedures during their hospitalization as well as patients that received other percutaneous ventricular assist devices including Impella or extracorporeal membrane oxygenation. Pearson Chi-square and t-test were used for data analysis. Results We identified 5,533 hospitalizations with NI-CS that were managed with IABP solely during the study period. The rate of admissions for NI-CS managed with IABP increased from 17.9% in 2016 to 21.5% in 2020, p trend<0.001. Mean age was 64 ±14 years. The majority were men (66%). Racial distributions were White (66%), Black (16%), Hispanic (9%) and Asian (4%). The 3,689 (67%) hospitalizations had a Charlson comorbidity index (CCI) of equal or >3. The most common comorbidities found were chronic heart failure (76%), hypertension (62%), atrial fibrillation (42%), chronic kidney disease (40%) and diabetic mellitus (40%). Other comorbidities were chronic pulmonary disease (16%), obstructive sleep apnea (11%) and pulmonary hypertension (7%). Most NI-CS admissions received IABP at urban teaching hospitals, compared to rural hospitals (84% vs 3%, p<0.001). The in-hospital mortality rates decreased from 31.1% in 2016 to 21.7% in 2020, p trend<0.001, however, the overall mortality was still remarkably high (26%). There was no statistically significant difference in trends of length of stay (14 days in 2016 vs 16 days in 2020, p =0.194). Conclusion During the study period, there was decreasing trends in in-hospital mortality despite increased hospitalizations with NI-CS that were managed with IABP solely. Further studies are needed to evaluate the role of IABP use in NI-CS.

Abstract:Background: Cardiogenic shock remains a major cause of morbidity and mortality in patients with ST-segment elevation myocardial infarction. We aimed to assess the current trends in cardiogenic shock management, looking specifically at the incidence, use of intra-aortic balloon pump therapy and outcomes in patients with ST-segment elevation myocardial infarction treated with primary percutaneous coronary intervention. Methods and results: We undertook an observational cohort study of 21,210 ST-segment elevation myocardial infarction patients treated between 2005–2015 at the eight Heart Attack Centres in London, UK. Patients’ details were recorded at the time of the procedure into local databases using the British Cardiac Intervention Society percutaneous coronary intervention dataset. There were 1890 patients who presented with cardiogenic shock. The primary outcome was all-cause mortality at a median follow-up of 4.1 years (interquartile range: 2.2–5.8 years). Increasing rates of cardiogenic shock were seen over the course of the study with consistently high mortality rates of 45–70%. A total of 685 patients underwent intra-aortic balloon pump insertion during primary percutaneous coronary intervention for cardiogenic shock with decreasing rates over time. Those patients undergoing intra-aortic balloon pump therapy were younger, more likely to have poor left ventricular function and less likely to have had previous percutaneous coronary intervention compared to the control group. Procedural success rates were similar (86.0% vs 87.1%, p=0.292) although crude, in-hospital major adverse cardiac event rates were higher (43.8% vs 33.7%, p<0.0001) in patients undergoing intra-aortic balloon pump therapy. Kaplan-Meier analysis demonstrated significantly higher mortality rates in patients receiving intra-aortic balloon pump therapy (50.9% intra-aortic balloon pump vs 39.9% control, p<0.0001) during the follow-up period. After multivariate Cox analysis (hazard ratio 1.04, 95% confidence interval 0.62–1.89) and the use of propensity matching (hazard ratio: 1.29, 95% confidence interval: 0.68–1.45) intra-aortic balloon pump therapy was not associated with mortality. Conclusion: Cardiogenic shock treated by percutaneous coronary intervention is increasing in incidence and remains a condition associated with high mortality and limited treatment options. Intra-aortic balloon pump therapy was not associated with a long-term survival benefit in this cohort and may be associated with increased early morbidity.

Abstract:Introduction Temporary mechanical circulatory support (tMCS) devices are increasingly used for the management of patients experiencing cardiogenic shock despite a paucity of randomized controlled data on their clinical efficacy. Complications associated with these devices are a common, but poorly quantified, occurrence that may negatively affect outcomes in cardiogenic shock patients receiving tMCS. Objective We sought to evaluate complication rates in patients with cardiogenic shock who receive tMCS and compare complication rates between different tMCS devices. Methods Consecutive patients with cardiogenic shock managed at a tertiary academic medical center from May 2015 to December 2021 were retrospectively reviewed (N=1,162). Our final study cohort comprised patients who developed cardiogenic shock and received tMCS after admission to our institution (N=187). Primary outcomes studied were complications using STS-INTERMACS definitions: neurologic complications (ischemic or hemorrhagic stroke, transient ischemic attack [TIA]), major bleeding events (type 3 and 4), and hemolysis, as well as acute kidney injury (AKI) requiring renal replacement therapy (RRT) and vascular compromise requiring vascular repair or amputation. Chi-squared tests were used to compare complication rates. Results Our study comprised 187 patients of whom 55 received VA-ECMO, 72 intra-aortic balloon pump, and 60 percutaneous ventricular assist device (pVAD - 90% Impella CP, 6.6% Impella 5.5, 3.3% Impella 2.5) as first tMCS. Patients receiving VA-ECMO had a higher proportion of SCAI stage E and higher lactate prior to the initiation of tMCS (Table- Panel A). Our analysis showed a vascular complication rate of 20% for VA-ECMO vs 6.9% for IABP vs 5% for pVAD, p=0.015; neurologic complication rate of 12.7% vs 8.3% vs 5%, p=0.34; hemolysis rate of 22.2% vs 2.8% vs 36.7%, p<0.001; AKI requiring RRT of 25.5% vs 9.7% vs 13.3%, p=0.045; and major bleeding event rate of 85.5% vs 34.7%% vs 53.3%, p<0.001 (Table- Panel B). Conclusions Overall complication rates increased from IABP to pVAD to VA-ECMO. VA-ECMO patients experienced more vascular complications, AKI requiring RRT, and major bleeding; pVAD patients had more hemolysis while the risk of stroke/TIA was comparable among the tMCS groups. Whether the observed differences are related to the specific device type and/or the different degrees of severity of shock warrants further investigation.

Trends in Mechanical Circulatory Support utilization, Left Ventricular Assist Device implantation and Transplant during Cardiogenic Shock Hospitalizations, after the New Heart Allocation Policy

Abstract:Background In October 2018, a new heart allocation policy was implemented to risk stratify patients listed for transplant, prioritizing patients supported with temporary mechanical circulatory support (MCS). The policy changes may have had an impact on the management of cardiogenic shock (CS). We sought to determine the changes in use of temporary MCS, durable left ventricular assist device (LVAD) and transplant in patients hospitalized before and after the new policy. Methods A retrospective analysis was conducted using the National Inpatient Sample (NIS) between 2017-2020. Hospitalizations for cardiogenic shock were identified, and stratified based on whether patients were admitted before or after the policy change. Baseline characteristics were compared between cohorts, and the primary outcome of interest was the use of MCS, transplant and LVAD before and after the policy change. Subgroup analyses included patients hospitalized at transplant and non-transplant centers, LVAD recipients as well as those who underwent transplant. Results A total of 643,655 hospitalizations were included, of which 260,340 (40.4%) were before the policy change, and 383,315 (59.6%) were after. In all patients with CS, there was a decrease in the use of LVAD (adjusted OR 0.73, p<0.01) and an increase in cardiac transplant (adjusted OR 1.45, p<0.01). While IABP use declined for the general CS population (adjusted OR 0.81, p<0.01), it increased significantly in cardiac transplant recipients (adjusted OR 2.55; p<0.01). Impella and VA-ECMO also increased in transplant recipients. No uptrend was seen in any other subgroup including LVAD recipients or CS patients managed in transplant centers. Conclusion Our study showed that the allocation policy change had a direct impact on MCS use in the first two years after implementation, but this effect was isolated to patients who underwent transplantation. It will be important to study how policy changes influence the management of other shock populations over time.

Abstract:BACKGROUND The use of Mechanical Circulatory Support (including Left ventricular assist devices) in patients with advanced Heart failure as a bridge-to-transplant or as a destination therapy has been linked to clinically significant 1- and 2- year survival rates. This study examines the trends of racial differences in the use of mechanical circulatory support devices (MCS) as well as some in-hospital outcomes. METHODS We identified all patients with Mechanical Circulatory Support (including Left ventricular assist devices) using ICD 10 codes from the national inpatient sample of the Health Care Utilization Project database, years 2016 - 2020. We classified our population into subgroups based on RACE (white, blacks, Hispanics and Asians). Using the chi-square test, we compared the differences in baseline characteristics amongst the racial subgroups and further used multivariate logistic regression to identify outcomes. RESULTS We observed about 95,430 patients with MCS. 73.2% were Whites, 14.3% Blacks, 9.4% Hispanics and 3.1% Asians and of these, Blacks were noted to have a younger mean age of 57 years. The population was composed highly of males than females across all races. AKI (64.1%), anemia (64.9%), obesity (21.33%) and OSA (13.0%), were all reported to be more common in Blacks than in other racial groups. Whites had greater rates of hypertension (14.9%) and nicotine use (25.8%) while Asians had higher rates of dyslipidemia (54.4%). Hispanics had higher rates of Diabetes (14.4%). Our black population had higher rates of cardiogenic shock (aOR: 0.88; 0.78 - 0.99, p=0.033), significantly longer mean length of stay (24.7 days vs 14.5 days, p<0.001) and higher hospital costs incurred ($750,083 vs $517,196, p<0.001) when compared to other racial groups. The most important independent predictor for the utilization of MCS was cardiogenic shock (aOR: 30.67; CI: 28.8 - 32.67, p<0.001); Asians (3.52%), Hispanics (2.58%) and blacks (2.20%) had higher rates than whites (2.13%). Between 2017 and 2020, Whites had the highest rates of Mechanical Circulatory support usage, with 0.78 per 1000 patients in 2020. Asians had the second highest usage rate with 0.69 per 1000 patients in 2020, while Blacks and Hispanics had lower but similar and stable rates during this period. CONCLUSION Despite having higher rates of cardiogenic shock (which is the independent predictor of MCS utilization) Blacks, Hispanics and Asians had lesser rates of MCS utilization. The findings from out study suggests that racial disparities persist in the use and outcomes of MCS, and efforts should be made to address these disparities and improve the care of patients with advanced heart failure across all racial groups.

Abstract:Background: Temporary mechanical circulatory support devices (tMCS) are increasingly being used in patients with infarct-associated cardiogenic shock (AMICS). Evidence on patient selection, complications and long-term outcomes is lacking. We aim to investigate differences in clinical characteristics, complications and outcomes between patients receiving no tMCS or either intra-aortic balloon pump (IABP), veno-arterial extracorporeal membrane oxygenation (V-A ECMO) or Impella® for AMICS, with a particular focus on long-term outcomes. Methods: Using health claim data from AOK—Die Gesundheitskasse (local health care funds), we retrospectively analysed complications and outcomes of all insured patients with AMICS between 1 January 2010 and 31 December 2017. Results: A total of 39,864 patients were included (IABP 5451; Impella 776; V-A ECMO 833; no tMCS 32,804). In-hospital complications, including renal failure requiring dialysis (50.3% V-A ECMO vs. 30.5% Impella vs. 29.2 IABP vs. 12.1% no tMCS), major bleeding (38.1% vs. 20.9% vs. 18.0% vs. 9.3%) and sepsis (22.5% vs. 15.9% vs. 13.9% vs. 9.3%) were more common in V-A ECMO patients. In a multivariate analysis, the use of both V-A ECMO (HR 1.57, p < 0.001) and Impella (HR 1.25, p < 0.001) were independently associated with long-term mortality, whereas use of IABP was not (HR 0.89, p < 0.001). Kaplan–Meier estimates showed better survival for patients on IABP compared with Impella, V-A ECMO and no-tMCS. Short- and long-term mortality was high across all groups. Conclusions: Our data show noticeably more in-hospital complications in patients on tMCS and higher mortality with V-A ECMO and Impella. The use of both devices is an independent risk factor for mortality, whereas the use of IABP is associated with a survival benefit.

Abstract:Report of mechanical circulatory support in cardiogenic shock patients during the peripartum period. Background Cardiomyopathies account for more than half of the cardiovascular disease during the peripartum period. In the extreme, patients may present with cardiogenic shock (CS) requiring mechanical circulatory support (MCS). The aim of this study was to report our experience with CS requiring MCS in the peripartum period. Methods We present a single‐center retrospective analysis of all CS cases involving MCS during the peripartum period that occurred between 2012 and 2023. Results Eleven cases were included. Median age was 33, median BMI was 30.4, and 73% underwent a caesarian‐section for delivery. CS presentation occurred in 36.4% during pregnancy and in 63.6% after delivery. Most patients were in Society for Cardiovascular Angiography & Interventions (SCAI) Stage C shock and in 37% the suspected etiology was peripartum cardiomyopathy. MCS usage included intra‐aortic balloon pump (4), Impella microaxial blood pump (2), veno‐arterial extracorporeal membrane oxygenation (6), and temporary right ventricle assist devices (2), with some patients having multiple MCS devices. The rate of major complications was 36.4%. During a median follow‐up of 4.5 years, 7 patients had sustained cardiac recovery (63.6%), 1 patient (9.1%) underwent cardiac transplantation, 2 patients (18.2%) received a durable LVAD, and 2 (18.2%) have died. Conclusion MCS in severe CS cases during the peripartum period is rare and associated with favorable outcomes. High recovery rates suggest favoring first MCS/LVAD over transplant.

Sex Differences in Management, Time to Intervention, and In-Hospital Mortality of Acute Myocardial Infarction and Non-Myocardial Infarction Related Cardiogenic Shock

Abstract:Background Limited data are available on sex differences in the time to treatment of cardiogenic shock (CS) with and without acute myocardial infarction (AMI). Methods For this retrospective cohort study, we used nationally representative hospital survey data from the National Inpatient Sample (years 2016-2021) to assess sex differences in interventions, time to treatment (within versus after 24 hours of admission), and in-hospital mortality for AMI-CS and non-AMI-CS, adjusting for age, race, income, insurance, comorbidities, and prior cardiac interventions. Results We identified 1,052,360 weighted CS hospitalizations (60% non-AMI-CS; 40% AMI-CS). Women with CS had significantly lower rates of all interventions. For AMI-CS, women had a higher likelihood of in-hospital mortality after: revascularization (adjusted odds ratio (aOR) 1.15 [95% CI 1.09-1.22]), mechanical circulatory support (MCS) (1.15 [1.08-1.22]), right heart catheterization (RHC) (1.10 [1.02-1.19]) (all p<0.001). Similar trends were found for the non-AMI-CS group. Women with AMI-CS were less likely to receive early (within 24 hours of admission) revascularization (0.93 [0.89-0.96]), MCS (0.76 [0.73-0.80]), or RHC (0.89 [0.84-0.95]) than men; women with non-AMI-CS were less likely to receive early revascularization (0.78 [0.73-0.84]), IABP (0.85 [0.78-0.94]), pLVAD (0.88 [0.77-0.99]) or RHC (0.83 [0.79-0.88]) than men (all p<0.001). For both types of CS, in-hospital mortality was not significantly different between men and women receiving early ECMO, pLVAD, or PCI. Conclusions Sex disparities in the frequency of treatment of CS persist on a national scale, with women being more likely to die following treatment and less likely to receive early treatment. However, when comparing patients who received early treatment, in-hospital mortality does not differ significantly when men and women are treated equally within 24 hours of admission. Early intervention if clinically indicated could mitigate sex-based differences in CS outcomes and should be made a priority in the management of CS.

Abstract:Background Patients with ST-elevation myocardial infarction who develop cardiogenic shock (STEMI-SHOCK) face significantly increased risk compared to those without shock. Mechanical circulatory support (MCS) devices are commonly utilized in STEMI-SHOCK patients, but the benefits of MCS remain uncertain. This study aims to apply the Society for Cardiovascular Angiography and Interventions (SCAI) shock classification system to STEMI-SHOCK patients, examining outcomes and identifying subgroups that may benefit from MCS. Purpose Our study assessed SHOCK severity in STEMI-SHOCK patients using the updated 2021 SCAI-SHOCK classification system and determined survival at various endpoints (30 days, 1 year, and 3 years), along with other outcomes such as vascular access complications and major adverse cardiac events, based on the severity class. Methods Patients with angiograms and indications of STEMI-SHOCK between 2012 and 2021 admitted to Liverpool Hospital Australia, were included after adjudication they had STEMI-SHOCK. Angiograms were eligible for adjudication if they met criteria for both STEMI and SHOCK. Each patient underwent individual adjudication by three researchers to confirm STEMI-SHOCK status. Patients were followed from the time of the first SHOCK diagnosis until censoring at the last follow-up. Data were retrieved from electronic and paper-based patient records. The primary outcome was 30-day survival, and secondary outcomes were 1-year and 3-year survival. Results The SCAI stages of 160 (Estimated 220) STEMI-SHOCK patients were determined, with 17 in Stage A, 39 in Stage B, 53 in Stage C, 25 in Stage D, and 26 in Stage E. Worse SCAI stages were associated with significantly lower 30-day, 1-year, and 3-year survival. No difference in survival between the time periods 2013-2015 and 2018-2021 was found. An eGFR < 60 millilitres/minute/1.73 meters^2 (hazard ratio [HR] 3.41; 95% confidence interval [CI] 1.18 – 9.92, p=0.024) and lactate greater than 4.15 millimoles/liter (HR 6.48; 95% CI [2.84 – 51.64], p=0.001) at shock were associated with lower 30-day and 3-year survival. Patients in the SCAI C subgroup treated with an IABP had significantly higher 1-year (p=0.001) and 3-year (p=0.001) survival compared to those not treated with an IABP. Conclusion The SCAI classification system successfully predicted short and long-term survival in STEMI-CS patients, with worse stages having progressively worse outcomes. No improvement in survival outcomes was observed in the overall SCAI C-E group of patients treated with mechanical support. However, patients in the SCAI C subgroup of SHOCK treated with mechanical support showed a statistically significant improvement in survival outcomes compared to those treated without mechanical support. Further studies with adequate sample sizes are required to investigate sub-populations of SCAI classes that may benefit from MCS devices.

Abstract:Introduction Epidemiology and outcomes of cardiogenic shock (CS) in patients with end stage renal disease (ESRD) have not been well studied. The objective of this study is to analyze temporal trends of incidence and outcomes of cardiogenic shock and utilization of percutaneous left ventricular assist device (pLVAD) in patients with ESRD. Methods We identified all hospitalized patients with CS and ESRD between January 2006 to December 2018 utilizing the United States Renal Data System (USRDS) database. We analyzed trends in the incidence, use of mechanical circulatory support, in-hospital mortality, and 1-year all-cause mortality. Results There were a total of 38,679 patients with ESRD admitted for CS during the study period. Mean age was 67.8 years (IQR; 59.4, 75.9) and 59% were men. The annual incidence of CS in patients with ESRD increased from 0.27% to 0.57% from 2006 to 2018 (Ptrend<0.001) (Figure 1A). There was no significant change in in-hospital mortality (53.91% in 2006 and 41.41% in 2018; Ptrend =0.38) and 1-year all-cause mortality (28.91% in 2006 and 35.13% in 2017; Ptrend =0.1) during the study period (Figure 1B). There was decrease in the use of intra-aortic balloon pump from 17.86 % in 2006 to 8.25% in 2018 (Ptrend < 0.001) (Figure 1C) and a significant increasing trend in the use of Impella from 0.14% in 2006 to 4.8% in 2018 (Ptrend <0.001) (Figure 1D). On multivariable regression analysis, age, peripheral vascular disease (OR: 1.13, 95% CI: 1.08 to 1.18; p<0.0001), diabetes mellitus (OR: 1.11, 95% CI: 1.06 to 1.16; p<0.0001) and duration on dialysis are independent predictors of in-hospital mortality. The risk of CS increases with increasing duration on dialysis: time on dialysis 2-5 years (OR: 1.26, 95% CI: 1.20 to 1.32; p=0.02), 6-10 years (OR: 1.4, 95% CI: 1.32 to 1.5; p=0.003), and ≥11 years (OR: 1.69, 95% CI: 1.53 to 1.86; p<0.0001). Conclusion The incidence of CS in patients with ESRD has doubled between 2006 and 2018. There has been significant decrease in use of IABP with concomitant increase in utilization of Impella during this period but there was no significant change in in-hospital and 1-year all-cause mortality.

The processes of care and long-term mortality of acute myocardial infarction with cardiogenic shock survivors: A nationwide cohort study

Abstract:Background: Acute myocardial infarction with cardiogenic shock (AMI-CS) carries a significant risk of inpatient mortality compared with AMI alone, although it is unclear what the longer-term outcomes of AMI-CS survivors is, and whether the inpatient quality of care received influences this. Methods: Using the Myocardial Ischaemia National Audit Project (MINAP) registry, linked to Office for National Statistics (ONS) mortality data, we analyzed 330,517 UK AMI patients; 3330 (1 %) with CS. Patients dying within thirty-days of admission were excluded. Median follow-up for patients included was 1642 days. Cox regression models were fitted, adjusting for demographics and management strategy. Results: AMI-CS survivors were younger (median years) (67 vs. 69, p < 0.001), less often female (29 % vs. 32 %, p < 0.001) and more likely to present with STEMI (81 % vs. 37 %, p < 0.001). Mortality risk was highest at one-year for AMI-CS survivors compared to patients that did not suffer CS (adjusted hazard ratio [HR] 1.85; 95 % CI; 1.68-2.04, p < 0.001), and remained elevated at five-years (HR 1.55; 95 % CI; 1.43-1.68, p < 0.001). 'Excellent-care' according to mean opportunity-based quality indicator (OBQI) score compared to 'Poor-care', showed reduced risk of long-term mortality with AMI-CS (HR: 0.46, CI; 0.39-0.54, P < 0.001). Of patients that received "Excellent-care", AMI-CS survivors had elevated risk of long-term mortality (HR 1.45, 95 % CI; 1.34-1.57, P < 0.001). Conclusion: AMI-CS survivors have elevated risk long-term mortality risk when compared with AMI patients, which persists beyond five years. AMI-CS patients that receive higher-quality inpatient care have better longer-term survival compared to those with poorer inpatient care.

Abstract:This single-center, observational study assessed the impact of age, gender, and body mass index (BMI) in patients with cardiogenic shock (CS) on temporary mechanical circulatory support. All adult patients admitted to the Cleveland Clinic main campus Cardiac Intensive Care Unit (CICU) between December 1, 2015, to December 31, 2019, CICU with CS necessitating mechanical circulatory support (MCS) with intra-aortic balloon pump, Impella, or venous arterial-extra corporeal membrane oxygenation were retrospectively analyzed for this study. Baseline characteristics and 30-day outcomes were collected through physician-directed chart review. The impact of age, gender, and BMI on 30-day mortality was assessed using multivariable logistic regression. Kaplan-Meier survival curves were used to analyze the survival difference in specific subsets. A total of 393 patients with CS on temporary MCS were admitted to our CICU during the study period. The median age of our cohort was 63 years (interquartile range 54 to 70 years), median BMI was 28.50 kg/m2 (interquartile range 24.62 to 29.72) and 70% (n = 276) were men. In total, 22 patients >80 years had received MCS compared with 372 patients <80 years. Patients >80 years on MCS had significantly higher 30-day mortality compared with those <80 years (81.8% vs 49.3%, p = 0.006). Upon stratifying patients by BMI, 161 (41%) patients were found to have BMI ≥30 kg/m2 whereas 232 (59%) patients had BMI <30 kg/m2. Comparison of 30-day mortality revealed that patients with BMI ≥30 did significantly worse than patients with BMI <30 (59.6% vs 45.3%, p = 0.007). There was no difference in 30-day mortality between men and women. On multivariable logistic regression, both age and BMI had a positive linear relation with adjusted 30-day mortality whereas gender did not have a major effect. Advanced age and higher BMI are independently associated with worse outcomes in patients with CS on MCS. Utilizing a strict selection criterion for patients in CS is pertinent to derive the maximum benefit from advanced mechanical support.
