Abstract:Myositis ossificans, a benign tumor composed of spindle cells and osteoblasts, can clinically and radiologically mimic osteosarcoma. While recognition and accurate diagnosis of myositis ossificans can be a challenge, this is critical as it may allow a conservative surgical approach to maximize functional outcomes. Herein, we present a patient with surface myositis ossificans confirmed genetically by the presence of COL1A1::USP6 gene fusion, along with a literature review. Due to the enhanced visualization of the bone matrix, computed tomography (CT) imaging may be a superior imaging modality to magnetic resonance (MR) imaging. Staged biopsies with samples obtained from the periphery and center of the lesions may allow pathologists to discern the zonal distribution histologically. Furthermore, immunohistochemistry fluorescence in situ hybridization and molecular testing can aid in the distinction of myositis ossificans from mimics. Because of their resemblance to other bone tumors, these cases of myositis ossificans highlight the importance of a multidisciplinary approach integrating clinical, radiologic, and pathologic analysis and involving serial imaging, sampling, and judicious use of ancillary immunohistochemical and molecular testing.

Abstract:Myositis ossificans is delineated and distinguished by the generation and deposition of cartilaginous and osseous soft tissues. It generally occurs in the lower extremities and is caused by direct trauma. During the different developmental stages of maturation, the lesion has different radiological appearances that can be confused with sarcomas. Here, we present the case of a 38-year-old woman who presented to the outpatient clinic with a painful mass in the lateral chest wall that had rapidly expanded and increased in size. The patient had no history of trauma. Chest computed tomography revealed an intramuscular mass in the lateral chest wall; postcontrast images demonstrated heterogeneous enhancement and peripheral calcification. The patient was then referred to our center for subsequent assessment and examinations. Pathological examination findings confirmed the diagnosis of myositis ossificans. Surgical resection was performed after obtaining patient consent. The symptoms experienced by the patient were successfully relieved, and no evidence of recurrence was observed during the 2-year follow-up period. Knowledge of the atypical locations of myositis ossificans, calcification patterns at different stages, and radiopathological correlations can help accurately diagnose myositis ossificans and avoid unnecessary medical imaging and interventions.

Abstract:A series of conditions can mimic musculoskeletal infections on imaging, complicating their diagnosis and affecting the treatment. Depending on the anatomical location, different conditions can manifest with clinical and imaging findings that mimic infections. Herein we present a wide spectrum of the musculoskeletal disorders of the axial skeleton, long bones, peripheral joints, and soft tissue that may manifest as infectious processes, and we focus on the potential mimics of osteomyelitis, septic arthritis, and infectious spondylodiscitis that are common in clinical practice. We present the typical imaging characteristics of each musculoskeletal infection, followed by mimicking conditions.

Abstract:Circumscribed myositis ossificans is a benign process of focal heterotopic ossification of the soft tissues, occurring in young subjects, usually following trauma. We report a case of a 15-year-old patient who suffered a direct trauma to the thigh during a soccer match, and developed a hard mass in the anterior face of the thigh. The patient was diagnosed with myositis ossificans secondary to trauma. In this case report, we want to illustrate the different imaging aspects of this benign condition, both in conventional radiology, computed tomography and magnetic resonance imaging, as its clinical and radiological appearance can be misleading, suggesting a sarcomatous neoplastic process.

Abstract:In this study, we discuss the clinical, radiological, and histopathological characteristics of intramuscular myxomas (IMM), a rare form of benign soft tissue tumors. We report the case of a 47-year-old female patient presenting with a painless, non-inflammatory mass in the right thigh, which was mobile relative to both superficial and deep planes. Imaging, biopsy, and subsequent histopathological study established the diagnosis of intramuscular myxoma. The patient underwent surgical excision of the mass, with a straightforward postoperative course. It is important to distinguish IMM from malignant tumors, such as soft tissue sarcomas, through comprehensive examinations including imaging and biopsy. The recommended treatment is surgery for complete excision of the mass, with an exceptionally low recurrence rate.

Abstract:An increasing amount of molecular imaging studies are ordered each year for an oncologic population that continues to expand and increase in age. The importance of these studies in dictating further care for oncologic patients underscores the necessity of differentiating benign from malignant findings, particularly for a population in whom incidental findings are common. The aim of this review is to provide pictorial examples of benign musculoskeletal pathologies which may be found on molecular imaging and which may be mistaken for malignant processes. Imaging examples are provided in the form of radiographs, bone scintigraphy, computed tomography, and fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) scans. Special attention is paid to specific features that help narrow the differential diagnosis and distinguish benign from malignant processes, with the goal of avoiding unnecessary invasive procedures.

Abstract:We present two pediatric cases of nontraumatic myositis ossificans (MO), which initially mimicked infections, leading to unnecessary treatments. The first case involves an 11-year-old boy with acute left hip pain and swelling, misdiagnosed as a hip joint infection and treated with antibiotics before histology confirmed MO. The second case is a two-year-old girl who presented with limping and restricted hip movement, initially suspected to have septic arthritis. Following MRI and clinical reassessment, her condition was diagnosed as MO. Both cases highlight the challenges of diagnosing MO in children and underscore the importance of including it in differential diagnoses for suspected infections.

Abstract:Actinomycosis is a rare infectious disease characterized by slowly progressive, chronic suppurative lesions, often mistaken for malignancies due to its ability to mimic them. It is caused by Actinomyces bacteria, which are part of the normal flora of the human oropharynx, gastrointestinal, and urogenital tracts. This case report describes a 51-year-old male with a history of mandibular rhabdomyosarcoma presenting with severe shoulder and hip pain, dysphagia, and headaches, initially suspected to be a cancer recurrence. However, after further investigation, including a PET-CT and tonsillectomy, the diagnosis of actinomycosis was confirmed through histopathological examination. The case highlights the diagnostic challenges of actinomycosis, especially in patients with complex clinical histories, emphasizing the importance of considering it as a differential diagnosis in similar presentations. The patient was treated with long-term antibiotic therapy, predominantly beta-lactams, demonstrating the necessity of a comprehensive diagnostic approach and the implications of a delayed diagnosis. This case underscores the critical need for high clinical suspicion and awareness among healthcare professionals regarding the potential for actinomycosis to mimic more common diseases, ensuring timely and accurate treatment.

Abstract:Objective To analyze the manifestation and variation of interrelations in MRI with myositis ossificans,and improve the ability of diagnosis and differential diagnosis.Methods The MRI in 16 examinations of 13 cases(male 4,female 9,aged 11 ~ 65 years,mean age 39.69 years) with myositis ossificans proved pathologically were reviewed retrospectively.Results On the early phase(2 ~ 3 weeks,4 examinations),the lesions demonstrated a soft mass with poor defined margin showed moderate signal intensity on T1 weighted imaging and high signal intensity on T2 weighted imaging;On the middle phase(1 ~ 12 months,11 examinations),the signal of lesions changed to lower than before on T1and T2 weighted imaging,but the signal was heterogeneity,and the lesions became lobulated with surrounding lower signal zone on T2 weighted imaging.In one case,intralesional hematoma was found and demonstrated high signal on T1 and T2 weighted imaging and contained a fluid-fluid level.Surrounding muscle edema became narrowed.On the later phase(1 ~ 2 years,1 examination): the imaging of lesion mainly showed high density signal and entrapped with a band of low signal intensity.Conclusion The manifestation and variation of MRI in myositis ossificans have certain characteristics.Thus this understanding is helpful to improve diagnosis and differential diagnosis of myositis ossificans,and unnecessary operation might be avoided.

Abstract:Inflammatory pseudotumor (IPT) of a peripheral nerve is a rare non-neoplastic tumefactive inflammatory condition, often mimicking malignancy. The etiology of this condition is still unknown. Clinically and radiologically, the lesion can mimic a malignant tumor. This case report represents, as far as we know, the first publication describing the ultrasonography findings and the results of advanced dynamic contrast-enhanced magnetic resonance imaging (MRI) and diffusion-weighted MRI of IPT in a peripheral nerve. Abstract:Pelvic cystic masses are frequently observed in women. Most lesions are benign and of ovarian origin. However, non-ovarian lesions can be easily confused with cystic ovarian masses on imaging, which hampers diagnostic and therapeutic management. In this report, a rare case of mesenteric lymphangioma mimicking an ovarian cystic mass, discovered as an incidental finding on orthopaedic MRI in an adult female, is presented. The report highlights the sometimes difficult diagnostic process of pelvic cystic masses, due to an extensive differential diagnosis and the fact that imaging is often inconclusive. Even though most cystic masses are of ovarian origin, non-ovarian causes can mimic ovarian masses and should be considered as differential diagnoses. Surgical exploration may be necessary to exclude malignant causes.

Abstract:Introduction: Myositis ossificans (MO) is the most frequent type of heterotopic ossification, occurring in individuals aged 20-40 years within major muscles, and roughly 75% of cases are triggered by trauma. It is most commonly present in locations with a high risk of injury, such as the quadriceps femoris and the arm's flexor muscles, and it is infrequently found in intra-articular sites. Case report: A 19-year-old male with a history of pain, swelling, and restriction of movements of the right elbow for the last 6 years with a history of difficulty in eating and combing hair with the right hand. History of trauma to his right elbow 6 years ago for which he underwent conservative native treatment with massage. Physical examination found a swelling over the right medial elbow joint of size approximately 7 cm × 6 cm × 3 cm, with no local rise of temperature or tenderness. There was a fixed flexion deformity of 75°. There were no sensory or vascular deficits. Under general anesthesia, through the anterior approach, an irregular bony mass was visualized which was excised. Through a posterior approach, a skin incision was made, subcutaneous tissue dissected, muscles retracted and then a calcified mass visualized over the olecranon process which was excised. The post-operative period was uneventful. Elbow joint mobilization was started from day 2 which showed significant correction of fixed flexion deformity. Conclusion: It is necessary to rule out non-traumatic MO when a patient exhibits radiologically detectable calcification and complains of a painful lump forming in a muscle. The degree of calcification and mature bone growth varies depending on the stage of the lesion. A biopsy of the lesion can rule out malignant transformation. Surgical excision is required for the management of mature lesions.
Nontraumatic Myositis Ossificans After Spontaneous Subarachnoid Hemorrhage: A Case Report

Abstract:Myositis ossificans is uncommon in patients with nontraumatic brain injuries. This report presents a challenging case in which myositis ossificans was diagnosed and treated by medical management in a patient who was unable to complain of any symptoms due to akinetic mutism that occurred after nontraumatic subarachnoid hemorrhage. The patient had intermittent high-grade fever, and laboratory tests showed elevated C-reactive protein and D-dimer levels without clinical signs of infection two months after subarachnoid hemorrhage. Lower-extremity venography using computed tomography was performed to rule out deep venous thrombosis. There was no thrombus, but right vastus medialis muscle showed inflammatory change with faint multilayered curvilinear hyperdense rims. The administration of indomethacin helped prevent abnormal bone formation. For the early detection of myositis ossificans, careful observation of clinical presentation and a high index of clinical suspicion is necessary in brain-injured patients. Further, elevated serum inflammatory markers accompanied by elevated alkaline phosphatase can be a critical clue. Early computed tomography helps identify early 'string sign' prior to characteristic ossification. Our report highlights that the myositis ossificans is remediable by early detection and appropriate nonsurgical management.
Intraosseous myofibroma mimicking an odontogenic lesion: case report, literature review, and differential diagnosis

Abstract:Background: Intraosseous myofibroma of the jaw is a rare neoplasm of mesenchymal origin with limited comprehensive understanding. It typically affects patients in the first two decades of life with a male predilection. Case presentation: This study presents a rare case of myofibroma mimicking an odontogenic lesion in a 2-year-old boy. The patient presented with an incidental finding of a painless swelling of the right mandibular ramus of unknown etiology. Imaging analysis revealed a solid, expansile lesion adjacent to the germinal zone of the right mandibular first molar. Histopathologic analysis and immunohistochemistry after incisional biopsy suggested a possible central odontogenic fibroma, and the patient underwent total enucleation, leading to the final diagnosis of intraosseous myofibroma. Follow-up examinations showed no evidence of recurrence. Conclusions: This report contributes to the understanding of myofibroma in pediatric patients and underscores the critical role of meticulous histopathologic examination for effective surgical planning and optimal patient outcomes.

Abstract:The patient presented to the clinic with painful muscle swelling in the right lower extremity, which improved with immunosuppressive therapy. Initially, the condition was diagnosed as polymyositis but recurred soon after. After imaging and biopsy, the final diagnosis was primary skeletal muscle peripheral T-cell lymphoma, not otherwise specified (PSM-PTCL, NOS). In this report, we discuss the challenges in diagnosing and treating this aggressive malignancy and review the literature on PSM-PTCL, NOS.

Intramuscular myxoma: unusual observation of spontaneous tumor size shrinkage

Abstract:Soft tissue tumors, whether benign or malignant, may grow over time or remain stable, but they usually do not spontaneously decrease in size. However, there are exceptions, such as inflammatory conditions, desmoid tumors, or benign cysts. Intramuscular myxomas are benign soft tissue tumors that typically present as a solitary, slow-growing, painless mass. They are generally treated by surgical resection, after which recurrence is rare. Here, we present a brief series of three unusual cases of intramuscular myxomas that spontaneously decreased in size. They were located in the cervical region, the right lower extremity, and the paravertebral lumbar region. Imaging findings and percutaneous biopsies confirmed the diagnosis in all cases. Follow-up imaging showed a spontaneous reduction in lesion volume over time, far exceeding the amount of tissue sample removed during biopsy. This unusual observation of spontaneous shrinkage may call into question the subsequent therapeutic approach to these lesions.
Malignancy mimics- diagnostic perplexities for oral and maxillofacial pathologists

Abstract:Objective: In pathology practice, one frequently encounters benign lesions which superficially resemble malignancy clinically and histopathologically. The diagnostic pitfalls can be avoided if the approach exemplified in the present study is followed. We expect that familiarity of these cases will be helpful for pathologists at the beginning of their career. Methods: Clinical case records of all the pathological specimens reported in our laboratory from January 2018 to September 2019 were queried. Cases displaying pseudotumor features were reviewed along with the special stains were performed and immunohistochemistry (IHC) studies. A working classification of pseudotumors presenting in the oral cavity was proposed. Results: Immunoglobulin G4-related disease, nodular fasciitis, fibrolipoma, odontogenic keratocyst with giant cell granuloma, juvenile ossifying fibroma with central giant cell granuloma and tumor-induced osteomalacia were the most common diagnoses where the tissue specimens resembled malignancies on routine clinicoradiological evaluation and light microscopy of tissue specimens. Their differential diagnosis and the pathological diagnostic dilemmas are explained. We have also highlighted the importance of correlating clinical, radiological and microscopic details with the findings deduced from advanced pathological aids to establish the final diagnosis. Conclusions: Pathologists should be aware of the conditions where the diagnosis of malignancy needs stricter evaluation to rule out malignant mimics. In such scenarios, correlation of light microscopy findings with clinical and radiological details cannot be overemphasized. Advanced pathological aids such as IHC, where necessary are often indispensable for reaching the accurate diagnosis in these cases.
Fibromatosis mimicking relapse of a neuroendocrine tumor at 68Ga-DOTATOC positron-emission tomography/computed tomography

Abstract:Mesenteric fibromatosis (desmoid tumor) is a locally aggressive fibroblastic lesion, characterized by a high recurrence rate that makes treatment challenging. Currently, there is no evidence-based treatment approach. We report the case of a female patient with a history of neuroendocrine tumor, who underwent 68Ga-DOTATOC positron-emission tomography/computed tomography (PET/CT), showing increased focal abdominal uptake suggestive of disease relapse. Histological examination revealed typical findings of fibromatosis. These findings indicate the expression of staining for somatostatin receptors (SSTRs) on fibromatosis cell surface and suggest to include fibromatosis among the potential causes of false-positive results at 68Ga PET/CT. Moreover, SSTRs expressed in desmoid tumors could be further investigated as a therapeutic target.
