Abstract:Objective To analyze the manifestation and variation of interrelations in MRI with myositis ossificans,and improve the ability of diagnosis and differential diagnosis.Methods The MRI in 16 examinations of 13 cases(male 4,female 9,aged 11 ~ 65 years,mean age 39.69 years) with myositis ossificans proved pathologically were reviewed retrospectively.Results On the early phase(2 ~ 3 weeks,4 examinations),the lesions demonstrated a soft mass with poor defined margin showed moderate signal intensity on T1 weighted imaging and high signal intensity on T2 weighted imaging;On the middle phase(1 ~ 12 months,11 examinations),the signal of lesions changed to lower than before on T1and T2 weighted imaging,but the signal was heterogeneity,and the lesions became lobulated with surrounding lower signal zone on T2 weighted imaging.In one case,intralesional hematoma was found and demonstrated high signal on T1 and T2 weighted imaging and contained a fluid-fluid level.Surrounding muscle edema became narrowed.On the later phase(1 ~ 2 years,1 examination): the imaging of lesion mainly showed high density signal and entrapped with a band of low signal intensity.Conclusion The manifestation and variation of MRI in myositis ossificans have certain characteristics.Thus this understanding is helpful to improve diagnosis and differential diagnosis of myositis ossificans,and unnecessary operation might be avoided.

Abstract:Objective To investigate the imaging features of non-traumatic myositis ossificans (NMO).Methods The clinical demonstrations and X-ray,CT and MRI findings were retrospectively analyzed in 9 patients with NMO.Results Among these 9 patients,2 were in early stage,3 were in middle stage and 4 were in late stage.X-ray showed increased peripheral soft tissue density in 7 patients and soft tissue swelling in 2 patients.CT showed obvious edematous and continuously irregular peripheral enhancement around the lump in 4 patients,"eggshell-shaped" calcification shadow in 6 patients,and three-layer distribution and liquefied or necrotic mass in the center in 7 patients.MRI showed inhomogeneous high signal intensity on T1WI,obvious high signal intensity on T2WI,markedly enhanced soft tissue mass,and a halo of persistent enhancement of edema zone (halo sign) surrounding the mass in 2 patients,slightly low signal intensity on T1WI,mixed signal intensity on T2WI,high signal intensity in the center,a few slightly flocculent high signal band around,and slightly enhancement in 3 patients,and equal signal intensity on T1 WI and T2 WI,similar to the adjacent bone,clear boundary and not connecting with the adjacent skeleton in 4 patients.Conclusion NMO images are featured by obvious enhancement around the mass and mild enhancement in edema areas (halo sign) on MRI,three-layer distribution in the edge of lesion on CT,and "eggshell-shaped" calcification on X-ray.
X-ray and CT Diagnosis of Intraosseous Ganglion

Abstract:Objective To investigate the pathogenesis, clinical manifestations, imaging features, and differential diagnosis of intraosseous ganglion. Methods Clinical and imaging features of 15 cases (5 men, 10 women; mean age 39.7 years) with intraosseous ganglia were retrospectively analyzed. There were 17 lesions, including 6 acetabula, 4 lunate, 3 proximal ends of tibia, 1 major tuberculum of humeral, 1 femoral head, 1 scaphoid, and 1 phalange. Results (1) Common radiological features included a unilocular or multilocular cyst surrounded by a full and thin rim of sclerotic bone in the subchondral epiphysis without any signs of degenerative joint disease. (2) Lesions were displayed as well defined round radiolucent defect or multi cystic changes with surrounding bony sclerosis or cystic and expansile change with irregular shape on CT scans. (3) CT showed an intraosseous ganglion communicating with adjacent joint in 1 patient. (4) CT values of the lesions were between 15- 80 HU. (5) Gas in the cyst could be seen in 3 cases. Conclusion Combined with patient′s age, lesion distribution, clinical manifestations, and imaging features, it is possible to make a correct diagnosis of intraosseous ganglion.

Abstract:Objective To study the imaging diagnosis of intraosseous ganglia.Methods 36 cases of intraosseous ganglion proved pathologically were included in this study.Of them,19 cases were examined with CT,17 underwent MRI and 5 cases underwent both CT and MRI examination.Results On CT,the cysts appeared as round or irregular low density with internal partition(8/19),sclerosis edge(15/19),broken cortex(12/19),intact-cortex(7/19),and without soft issue mass and periosteal reaction(19/19).On MR,the lesions were sharply defined round or irregular adjacent to the cortical bone ,which were low signal intensity (13/17) on T1WI , high signal intensity(13/17) on T2WI and STIR,and the ganglion connected with the joint(16/17).Conclusion CT and MRI are important methods in detecting the intraosseous ganglion.

Abstract:Objective:To analysis the appearances and to evaluate the diagnostic value of X-ray CT and MR imaging in osteoid osteoma.Methods:22 cases 19 females and 3 males of osteoid osteoma proved by surgical pathology and their X-ray,CT and MRI were reviewed.The ability of X-ray,CT and MRI to demonstrate the nidus and surrounding reaction were analyzed.Results:The nidus were appeared as round or oval shape and the diameter was less than 2 cm.It was fomd in 17 cases of X-ray film，22 CT and 20 of MRI .There were different degree of bone sclerosis,periosteal reaction and soft tissues or bone marrow edema around the nidus.The diagnostic accuracy was 77.3% for X-ray，100% for CT and 90.9% for MRI.Conclusion:Most of osteoid osteoma have the typical appearances and is no difficult to make diagnosis.CT scan is the most accuracy method to demonstrate the nidus.It is possible to make misdiagnosis only with X-ray or MRI for the case that the hidus not be demonstodted.

Abstract:The imaging techniques have been widely used in the diagnosis of chronic osteomyelitis,including plain radiography,magnetic resonance imaging,computed tomography and radionuclide imaging.Plain radiography is useful in the early detection of suspected osteomyelitis,but its sensitivity is low.Due to its high resolution and sensitivity,MRI has been the most widely used but its specificity is relatively poor.CT is mainly recommended for diagnosis of chronic osteomyelitis in complicated anatomic regions.Radionuclide imaging will have prospects of broad application due to its diverse radiotracers and high sensitivity.This paper reviewed the research of imaging modalities in diagnosis of chronic osteomyelitis.
The Imaging Features of Intra-Articular Osteoid Osteoma

Abstract:Objective: To analyze clinical characteristic and the imaging features of intra-articular osteoid osteoma. Methods: The clinical and imaging features of 17 cases with intra-articular osteoid osteoma which were proved by surgical pathology were analyzed retrospectively. Results: The joint involved were hip(13 cases), knee(1 case), elbow(1 case), shoulder(1 case) and intervertebral joint(1 case). Only 5 cases had the typical symptom which consists of pain that worsened at night. With radiographic, the nidus were demonstrated in 6 cases, 6 cases had sclerosis around the nidus, 9 cases had associated joint swelling, none had periosteal reaction. In CT scan, the nidus were demonstrated in all cases, 12 cases had the sclerosis around the nidus, 11 cases had associated joint effusion, and with no periosteal reaction. In the 14 cases with MR imaging, the nidus were demonstrated in 6 cases, 11 cases had the associated joint effusion. Conclusion: Most intra-articular osteoid osteomas occur in the hip, the symptom was atypical. It was difficult unable to demonstrate the nidus and the associated joint effusion, on conventional X-ray or MRI and so it was possible to miss the diagnosis, CT scan is the best method to demonstrate the nidus and to achieve the diagnosis.

Abstract:Myositis ossificans is delineated and distinguished by the generation and deposition of cartilaginous and osseous soft tissues. It generally occurs in the lower extremities and is caused by direct trauma. During the different developmental stages of maturation, the lesion has different radiological appearances that can be confused with sarcomas. Here, we present the case of a 38-year-old woman who presented to the outpatient clinic with a painful mass in the lateral chest wall that had rapidly expanded and increased in size. The patient had no history of trauma. Chest computed tomography revealed an intramuscular mass in the lateral chest wall; postcontrast images demonstrated heterogeneous enhancement and peripheral calcification. The patient was then referred to our center for subsequent assessment and examinations. Pathological examination findings confirmed the diagnosis of myositis ossificans. Surgical resection was performed after obtaining patient consent. The symptoms experienced by the patient were successfully relieved, and no evidence of recurrence was observed during the 2-year follow-up period. Knowledge of the atypical locations of myositis ossificans, calcification patterns at different stages, and radiopathological correlations can help accurately diagnose myositis ossificans and avoid unnecessary medical imaging and interventions.

Abstract:Myositis ossificans, a benign tumor composed of spindle cells and osteoblasts, can clinically and radiologically mimic osteosarcoma. While recognition and accurate diagnosis of myositis ossificans can be a challenge, this is critical as it may allow a conservative surgical approach to maximize functional outcomes. Herein, we present a patient with surface myositis ossificans confirmed genetically by the presence of COL1A1::USP6 gene fusion, along with a literature review. Due to the enhanced visualization of the bone matrix, computed tomography (CT) imaging may be a superior imaging modality to magnetic resonance (MR) imaging. Staged biopsies with samples obtained from the periphery and center of the lesions may allow pathologists to discern the zonal distribution histologically. Furthermore, immunohistochemistry fluorescence in situ hybridization and molecular testing can aid in the distinction of myositis ossificans from mimics. Because of their resemblance to other bone tumors, these cases of myositis ossificans highlight the importance of a multidisciplinary approach integrating clinical, radiologic, and pathologic analysis and involving serial imaging, sampling, and judicious use of ancillary immunohistochemical and molecular testing.

Abstract:Objective: To explore the imaging features of chondromyxoid fibroma of bone and evaluate their clinical value. Methods: Eleven cases with chondromyxoid fibroma of bone confirmed by surgical pathology underwent radiography, CT plain scanning and MR SE-T1WI, SE-T2WI and SE-T1WI enhanced scanning before operation, the imaging data was reviewed and analysed retrospectively in comparison with surgical and pathological results. Results: The lesions in seven cases of 11 chon- dromyxoid fibroma with well defined osteolytic destruction were located at the metaphysis of the long tubular bone, in 2 cases the lesion was in pelvis, 1 case in skull, the other case in vertebrae. The diameter of the tumor ranged from 3.2cm to 16.9cm, with a mean diameter of 4.7cm. When located in a long tubular bone, chondromyxoid fibroma were generally eccen- trically situated at the metaphysis, radiolucent and elongated in shape, cortical expansion, exuberant endosteal sclerosis, and coarse trabeculation, with small area of ring- and arc-like calcification, thin sclerotic rim separated the tumor from adjacent normal bone, there may be little periosteal reaction and small soft tissue masses. The CT value was similar to muscle and CT is optimal to detect the matrix mineralization. MR imaging depicted low signal intensity with well defined rim on T1WI, high signal intensity on T2WI. Conclusion: The imaging manifestations of chondromyxoid fibroma were specific to some extent. Combined utilization of plain X-ray, CT, and MRI is helpful for the diagnosis and differential diagnosis of chondromyxoid fi- broma.
Radiographic Examination of 41 Cases of Odontogenic Myxomas on the Basis of Conventional Radiographs

Abstract:Objective: There are some different radiographic appearances of odontogenic myxoma that have not been reported in the published literature. The purpose of this article is to offer rare views of odontogenic myxomas and analyse the characteristics of the radiographic appearances.Materials and methods: We reviewed the radiographic appearances of 41 cases of odontogenic myxomas on conventional radiographs, recruited from patients who visited the Department of Oral Radiology, West China College of Stomatology, Sichuan University between 1964 and 2005. The diagnoses of the cases were verified by histopathology.Results: On conventional radiographs, odontogenic myxomas presented varying radiographic appearances, which could be divided into six types as follows: Type I-unilocular; Type II-multilocular (including honeycomb, soap bubble and tennis racquet patterns); Type III-involvement of local alveolar bone; Type IV-involvement of the maxillary sinus; Type V-osteolytic destruction and Type VI-a mix of osteolytic destruction and osteogenesis.Conclusion: Odontogenic myxoma has variable radiographic appearances on conventional radiographs. This classification system helps us better understand the radiographic appearances of odontogenic myxoma on conventional radiographs.
An analysis of imaging characteristics of X-ray, computed tomography and magnetic resonance imaging of intraosseous lipoma

Abstract:Objective To investigate the imaging characteristics of X-ray, computed tomography ( CT ) and magnetic resonance imaging ( MRI ) of intraosseous lipoma and to improve the accuracy of diagnosis and differential diagnosis.Methods The imaging characteristics of X-ray, CT and MRI in 38 cases of intraosseous lipoma conifrmed by percutaneous puncture biopsy and pathological test were analyzed retrospectively, which was compared with pathological outcomes.Results Intraosseous lipoma was noticed in the calcaneus (n=11 ), in the femur (n=8 ), in the tibia (n=5 ), in the ilium (n=6 ), in the humerus (n=6 ), in the radius ( n=1 ) and in the talus (n=1 ) respectively. There were 38 cases of single cystic type in all. Based on the X-ray and CT, calciifcation was found in 11 cases, bone crest in 19 cases, double sclerotic band in 4 cases, sclerotic border in 21 cases and bone expansion in 16 cases. Lipo density could be determined by CT in all the cases. The CT value was -40HU - -133HU. The MRI showed there was fat component in all the cases. Among them, cyst formation was detected in 9 cases, and inhomogeneous signal in 20 cases. According to Milgram’s stage, 4 cases were at stage I, 4 cases at stage II and 30 cases at stage III. Conclusions The X-ray and CT can show the lesion site, morphology, calcification, bone crest, sclerotic border, bone expansion and with or without boundary. The MRI can make clear fat component, fat necrosis, cyst formation, myxoid change, inlfammation, range and border of the lesion. The X-ray, CT and MRI relfect imaging characteristics of intraosseous lipoma from different aspects.
Diagnostic features of osteitis condensans ilii by MRI-a systematic literature review

Abstract:Objective: To describe and evaluate the current knowledge of MRI characteristics of osteitis condensans ilii (OCI) in the diagnostics and differentiation of OCI from other conditions. Materials and methods: The databases PubMed, EMBASE, Scopus, and Web of Science were searched from their inception to March 2024 using the search terms "Magnetic Resonance Imaging" (MESH term in PubMed) and "osteitis condensans ilii" and limited to English language. Two reviewers independently screened titles, abstracts, and full-text eligibility and assessed the risk of bias according to Quality Assessment of Diagnostic Accuracy Studies, QUADAS-2. Results: The search identified 53 records. Case reports, letters/notes, and conference abstracts were excluded, resulting in 24 reports assessed by full-text, 9 research articles, 14 reviews, and a book chapter. Five retrospective research studies were found eligible for the review. Detailed MRI features of OCI were only described in two studies of patients with pain where they encompassed manifest subchondral iliac sclerosis often accompanied by bone marrow edema (BME) located peripheral to the sclerosis and displaying a continuous distribution and frequently accompanied by sacral BME. Erosions were rare and ankylosis did not occur. Fat deposition in the bone marrow was frequent and similar to BME often located to anterior strain-related joint areas. The QUADAS-2 assessments revealed risks of bias in all studies analyzed, especially regarding general applicability of the MRI features. Conclusion: There is a lack of valid data describing characteristic MRI features in general groups of OCI patients with and without pain.

Abstract:Objective To explore the MRI imaging features of Dermatomyositis/Polymyositis.Methods The imaging features of 12 cases were retrospectively analyzed.Results The fascial and muscle of 2 cases were demonstrated comprehensive hyperintensity on fat suppression images.Some fascial and muscle of 10 cases were demonstrated hyperintensity on fat suppression images.Fat suppression images showed enlargement and diffuse reticular pattern of the subcutaneous fat.The spatium intermusculare of hyperimtense area were not clear of the 12 patients.Conclusion MRI can effectively reveal these imaging appearances and provide more reliable evidence for clinical diagnosis.
Muscle imaging in myositis: MRI, US, and PET

Abstract:Imaging is an important tool in the evaluation of idiopathic inflammatory myopathies. It plays a role in diagnosis, assessment of disease activity and follow-up, and as a non-invasive biomarker. Among the different modalities, nuclear magnetic resonance imaging (MRI), ultrasound (US), and positron emission tomography (PET) may have the most clinical utility in myositis. MRI is currently the best modality to evaluate skeletal muscle and provides excellent characterization of muscle edema and fat replacement through the use of T1-weighted and T2-weighted fat suppressed/STIR sequences. Although MRI can be read qualitatively for the presence of abnormalities, a more quantitative approach using Dixon sequences and the generation of water T2 parametric maps would be preferable for follow-up. Newer protocols such as diffusion-weighted imaging, functional imaging measures, and spectroscopy may be of interest to provide further insights into myositis. Despite the advantages of MRI, image acquisition is relatively time-consuming, expensive, and not accessible to all patients. The use of US to evaluate skeletal muscle in myositis is gaining interest, especially in chronic disease, where fat replacement and fibrosis are detected readily by this modality. Although easily deployed at the bedside, it is heavily dependent on operator experience to recognize disease states. Further, systematic characterization of muscle edema by US is still needed. PET provides valuable information on muscle function at a cellular level. Fluorodeoxyglucose (FDG-PET) has been the most common application in myositis to detect pathologic uptake indicative of inflammation. The use of neurodegenerative markers is now also being utilized for inclusion body myositis. These different modalities may prove to be complementary methods for myositis evaluation.

Abstract:Objective To report the clinical and magnetic resonance imaging(MRI)features of 5 eases with idiopathic orbital myositis.Methods Four females and one male,aged 27 to 57 years,presented department of neurology in the First Hospital of Peking University in October 2008 to September 2009.The duration of disease Was between 3 months and 4 years.Recurrent course appeared in 3 of them.0rbital MRI Was performed in all of them.After diagnosis they underwent long.term corticosteroid treatment.Results All patients presented ocular pain,asymmetrical and incomplete ophthalmoplegia and mild proptosis.EMG revealed no significant decline in repetitive stimulation.Muscle biopsies of limb muscle were unremarkable.Creatine kinase and thyroid function test were in normal limits.MRI revealed unilateral.focal or difluse enlargement and enhancement of extraocular muscles,involving 1 extraocular muscle in 2 cases,2extraocular muscles in 2 cases,more extraocular muscles in 1 case.No evidence indicated bone destruction or cavernous sinus abnormalities.Five Cases showed improvement and remission after long-term administration of steroids.Conclusion Persistent and asymmetrical ophthalmoplegia is connnon in orbital myositis.Extraocular muscle swelling characterized the MRI changes.

Abstract:Objective To investigate the characteristic MRI features of sporadic inclusion?body myositis(sIBM). Methods Clinical and MR imaging data of 6 patients with sIBM diagnosed by muscle biopsy from May 2013 to November 2014 were retrospectively analyzed. All patients showed insidious onset of lower limb muscle weakness and diagnosed as sIBM by muscle biopsies. All patients were evaluated by the score of the severity of fatty infiltration, inflammation and atrophy in MRI. Results All patients were observed fatty infiltration with different degrees. The fatty infiltration in thighs was characterized in a decreasing order of frequency:gluteus maximus (6 cases), vastuslateralis (6 cases), vastusintermedius (6 cases), vastusmedialis (6 cases), sartorius (5 cases), adductor magnus (5 cases), rectus femoris (4 cases), semi?membranosus (4 cases), semi?tendinosus (4 cases), biceps femoris (4 cases), gracilis (3 cases), adductor longus(2 cases).The fatty infiltration in thighs was characterized in a decreasing order of severity:vastuslateralis (3.2 points), vastusintermedius (3.2 points), vastusmedialis (3.0 points), adductor magnus (3.0 points), gluteus maximus (2.7 points), bicepsfemoris (2.2 points), semi?membranosus (2.1 points), semi?tendinosus (2.1 points), rectus femoris (1.5 points), sartorius (1.3 points), gracilis (0.8 points), adductor longus (0.7 points). All patients showed the features of distal distribution andsymmetry. Inflammation was observed in 3 patients. 1 patient only involved the vastuslateralis, the other 2 patients were observed muscle inflammation with different degrees in 12 muscles. Atrophy was observed in 5 patients. The atrophy in thighs was characterized in a decreasing order of frequency:vastuslateralis (5 cases), vastusintermedius (5 cases), vastusmedialis (4 cases), adductor magnus (4 cases), semi?membranosus (2 cases), rectus femoris (1 cases), sartorius (1 cases) and gluteus maximus;there was no atrophy in adductor longus, gracilis,semi?tendinosus, biceps femoris. Conclusion The MRI characteristic manifestations of sIBM is fatty infiltration and atrophy in the distal portion, particularly involving the vastuslateralis, vastusintermedius, vastusmedialis and adductor magnus.
