Abstract:Objective: The purpose of this case report is to describe a patient with post traumatic myositis ossificans (PTMO) of the anterior thigh following blunt trauma and discuss the incidence, clinical presentation, management, and imaging findings. Clinical features: An 18-year-old male presented to a chiropractic clinic with a chief complaint of left knee pain and reduced range of motion after an impact injury to his left anterior thigh during hurdling 6 weeks earlier. Immediately after the injury, he presented to the emergency department where radiography of the left knee was negative and he was diagnosed with a muscle sprain. Follow-up radiography and ultrasonography of the left knee in a chiropractic radiology department revealed ossification consistent with PTMO within his vastus intermedius. Intervention and outcome: The patient underwent a course of rehabilitation for 2 months including ice, class IV cold laser and vibration applied to his anterior thigh, and myofascial release of his quadriceps musculature with targeted and progressive rehabilitative exercises. His left knee pain resolved within 2 weeks of care. He resumed sports participation (American football) pain-free, while wearing protective padding over the affected thigh, 1 month after presentation, which was approximately 2 1/2 months following his injury. Conclusion: This case demonstrates that ultrasonography may have the capability to detect early phases of PTMO approximately 2 weeks prior to radiographic evidence and to monitor progression throughout its course.
[Effect of Bifidobacterial adhesin on lipopolysaccharide- and H2O2-induced proliferation and apoptosis of intestinal epithelial cells in vitro].

Abstract:OBJECTIVE To study the effect of Bifidobacterial adhesin on proliferation and apoptosis of intestinal epithelial cell induced by lypopolysaccharide (LPS) and H(2)O(2) in vitro. METHODS With (3)H-TdR incorporation method, flow cytometry and fluorochrome staining, the proliferation and apoptosis of intestinal epithelial cells induced by LPS and H(2)O(2) in vitro were studied. RESULTS LPS at the dose of 100 microg/L effectively stimulated the proliferation and apoptosis of cells, whereas H(2)O(2) at the dose of 200 micromol/L obviously restrained the proliferative ability while enhanced the apoptosis of the cells. Fluorochrome staining showed cell shrinkage, nuclear condensation and fragmentation under microscope. After treatment with Bifidobacterial adhesin, the cell proliferation and apoptosis decreased significantly in LPS group, and in H(2)O(2) group, cell apoptosis was significantly decreased. CONCLUSIONS Bifidobacterial adhesin can protect intestinal epithelial cells from the damage by LPS and H(2)O(2), and maintain the balance between the proliferation and apoptosis of the cells.

Abstract:Objective To evaluate the etiology, pathogenesis, clinical manifestation, imaging features, treatments and factors related to prognosis of acute spinal spontaneous hematoma.Methods The clinical data of 38 patients with acute spinal hematoma treated in our hospital from 2011 till now were analyzed retrospectively.Duration of follow-up was 6 months.The factors influencing the prognosis were analyzed.Results Acute epidural hematomas (n=29) were much more common than subdural (n=5), subarachnoid (n=1) and intramedullary (n=3).Most hematomas were located in the cervical and thoracic vertebra regions.The etiology of acute spinal spontaneous hematoma was unknown in most patients.Twenty-nine patients were dealt with surgical intervention and 9 patients were treated conservatively.After 6-month follow up, recovery rate measured by JOA score in patients of spinal injury ASIA level A and B was (51.26 ±38.97), and level C, D and E was (80.33 ±25.83), P<0.05.Recovery rate in patients with hematoma discovered in less then 24 hours treated with surgical decompression was (64.79 ±36.10), and that in those with hematoma present over 24 hours was (34.54 ±30.17), P<0.05.Conclusions Acute spinal hematoma always caused by unknown etiology, and usually manifests itself in a sudden onset of pain and neurological deficits.The early diagnosis mainly depends on MRI.Patients presenting with severe neurologic dysfunction or showing signs of progressive deficit should have immediate surgical intervention. The status of neurological deficits before surgery and the length of interval between onset and surgical intervention are associated with recovery.

Abstract:A woman in her 20s with a past medical history of surgical debulking of a right neck mass presented to the hospital for persistent and worsening right shoulder pain. The shoulder pain was associated with trismus and back and neck pain. A CT scan of the neck with contrast revealed post-surgical changes with increased heterotopic ossification throughout the surgical site extending to the supraclavicular soft tissues and the left sternocleidomastoid muscle, suggesting muscle ossification. A biopsy was performed, and the patient was diagnosed with myositis ossificans (MO). Initial treatment began with the administration of steroids and analgesics. She was scheduled for a follow-up with orthopedics, rheumatology, and genetics, but she was lost for follow-up. MO is a very rare medical condition usually associated with trauma, and in our patient, the symptoms started after a chiropractic adjustment.

Abstract:Myositis ossificans (MO) is a benign condition where bone forms within muscles due to increased activity of the periarticular tissues. Trauma is the most common cause. Nontraumatic MO is exceedingly rare. We present a rare instance of nontraumatic MO affecting the hip in a 32-year-old patient. The patient had a known case of seizure disorder and also had a history of a cerebrovascular accident (CVA). Despite the absence of trauma or known predisposing factors, the patient developed a sizable mass in the left hip, causing pain and restricted range of motion (ROM). Surgical excision of the mass was successful, resulting in complete removal and subsequent improvement in hip function and pain relief during postoperative recovery. Histopathological examination confirmed the diagnosis of MO. The patient's ROM normalized, and there were no signs of recurrence at the one-year follow-up. This case highlights the importance of recognizing MO in hip pain cases without trauma. Timely surgery through the approach described effectively removes the mass, preventing recurrence without compromising vital structures. It showcases a successful multidisciplinary approach for rare musculoskeletal conditions, offering valuable insights into similar cases.

Abstract:Circumscribed myositis ossificans is a benign process of focal heterotopic ossification of the soft tissues, occurring in young subjects, usually following trauma. We report a case of a 15-year-old patient who suffered a direct trauma to the thigh during a soccer match, and developed a hard mass in the anterior face of the thigh. The patient was diagnosed with myositis ossificans secondary to trauma. In this case report, we want to illustrate the different imaging aspects of this benign condition, both in conventional radiology, computed tomography and magnetic resonance imaging, as its clinical and radiological appearance can be misleading, suggesting a sarcomatous neoplastic process.

Abstract:The δ13C value of a C3plant is influenced by the species’ response to its environment, which is subject to natural changes with space and time. This paper reviews the magnitude of the resulting variations in plant δ13C values—the differences between species, and the variations at different spatial and temporal scales—with particular emphasis on studies of tree wood. Because of these variations, any attempt to estimate the absolute δ13C values of a plant food resource at a particular place and time in the past, even when based on analysis of archaeological plant remains, will be subject to uncertainties. Uncertainties due to a single factor are commonly 1 per mil, and in many cases a combination of factors will lead to uncertainties of at least 2 per mil. Many of these factors are also relevant to archaeological studies based on differences between the δ13C values of different individuals or populations of animal/humans. Where such differences are only a few per mil it may be difficult to unambiguously ascribe them to palaeodietary, rather than to palaeoenvironmental, causes.

Abstract:Chronic osteomyelitis of the skull base is a commonly reported pathology in existing scientific literature, but chronic osteomyelitis of the skull vault (COSV) is a rarely documented disease. We report the case of a 38 years old Afghan male with a presenting complaint of irregular swelling on the skull vault for six months. The patient had a history of head trauma one year back with a compound depressed fracture which had been surgically managed then. Physical examination revealed a 15 x 15 cm hard, immobile swelling with an old scar mark over the scalp. Neuroimaging was consistent with diffuse, bilateral frontoparietal swelling of bony origin, sparing underlying brain parenchyma. Surgically excision of the lesion was done through a bicoronal skin flap and cranioplasty done at the same time. Histological findings of the specimen reported chronic osteomyelitis. However, microbiology revealed no growth in culture. Patient was discharged on the second postoperative day uneventfully. This case turns minds into keeping skull vault osteomyelitis as differential diagnosis besides other spontaneous bony lesions; e.g. fibrous dysplasia, osteoma and giant cell tumours. Typically, a person with spinal stenosis complains about developing tremendous pain in the legs or calves and lower back after walking. This is usually very reproducible and immediately relieved by sitting down, or leaning over. When the spine is bent forward, more space is available for the spinal canal, causing a reduction in symptoms. Although symptoms may arise from narrowing of the spinal canal, not all patients develop symptoms. Why some patients develop symptomatic stenosis and others do not remain unknown. Therefore, the term spinal stenosis refers not to the finding of spinal canal narrowing, but rather to manifestation of lower extremity pain caused by compression on the affected nerves.
The anatomical basis for surgical intervention in myositis ossificans—A systematic review

Abstract:Myositis ossificans (MO) is a benign condition characterized by heterotrophic bone formation, most commonly within muscle tissue. Multiple types have been described, the most predominant being myositis ossificans circumscripta, which occurs in response to trauma. Myositis ossificans cases reported in the literature were reviewed systematically. The aim of the review was to examine how the condition and its management are influenced by the anatomical structures involved. The Medline and Ovid Embase online databases were searched for all papers relevant to MO between Jan 1972 and Dec 2020. Duplicate and unobtainable records were removed. The remaining records were manually assessed for relevance by three independent reviewers, initially by abstract and then by full‐text screening, to ensure that all papers were suitable. Acceptance of articles was not limited by case features or study design. In total, 77 papers from the literature search were included. They contained information on 89 patients diagnosed with MO. The average age of the patients was 26.17 years (range 13 weeks to 72 years, a 71.75 year range) and 65.17% were male. The lower limb was affected more than the upper limb or spine, the most common site being the thigh. Muscle was the most commonly affected tissue, the quadricep femoris being most often involved. Diagnostic imaging varied widely among cases; X‐ray alone was the most common method, followed by a combination of X‐ray and MRI. Lesions in the lower limb had larger diameters than those in the upper limb or spine. More cases in the upper limb (47.83%) than in the lower limb (33.33%) required surgery. All instances of MO in the jaw, hand, and foot were symptomatic, and all required surgical management. The findings were consistent with previous research showing MO to be a predominantly male disease, with most patients being under 30 years of age. Trauma was involved in 52.81%, lower than previously reported. In this study, the vastus lateralis of the quadriceps was most often affected. MO lesions were less common and smaller in the upper limb but more required surgery. Anatomically confined sites such as the hand and foot always required surgery, probably because of earlier onset or more severe functional effects of symptoms. Further work is needed to review the anatomical predilection of MO to help identify patients who could benefit from earlier consideration of surgery. A national registry of MO cases could contribute to further research on this disease.

Abstract:Background: Gelatin foam has been regularly used the complex neuro and spinal surgeries for a long time. Apart from their hemostatic properties, these are inert and provide form an inert membrane that prevents scar adhesions o vital structures such as the brain or spinal cord. Case presentation: We present a case of cervical myelopathy due to an ossified posterior longitudinal ligament that underwent the instrumented posterior decompression and had neurological worsening 48 h after the index surgery. An magnetic resonance imaging showed a hematoma compressing the spinal cord which on exploration was confirmed to be a gelatin sponge. It represents the rare phenomenon of mass effect due to their osmotic properties, especially in a closed space causing neurologic deterioration. Conclusion: We emphasize the rare cause of early onset quadriparesis after the posterior decompression due to the swollen gelatin sponge over the neural elements. The patient recovered with timely intervention.

Abstract:INTRODUCTION:Spinal subdural hematoma is rare and can cause serious neurological symptoms. Sometimes, idiopathic spinal subdural hematoma can spontaneously occur without any identifiable underlying etiologies. In this report, we present such an uncommon case of paraplegia caused by idiopathic spinal subdural hematoma that was successfully managed by laminectomy.CASE PRESENTATION:A 45-year-old Chinese woman presented with sudden onset of progressive asthenia and numbness in both lower extremities, accompanied by difficulty in micturition. An initial non-contrast spinal magnetic resonance imaging at a local hospital suggested a spinal subdural tumoral hematoma at the T9 level. She was referred to our hospital and an emergency laminectomy from T8 to T10 was performed 22 hours after onset of her initial symptoms. However, nothing but a hematoma was identified during the operation, and a final diagnosis of spontaneous acute spinal subdural hematoma was concluded. She had partial return of sensations and voluntary movement after the operation.CONCLUSIONS:On imaging findings, spinal subdural hematoma could manifest as focal and independent from the dura matter, and, therefore, it should be included in the differential diagnosis of medullary compressive lesions.

Abstract:Arachnoiditis ossificans (AO) is a rare and complex neurological condition characterized by pathological calcification or ossification of the arachnoid membrane. Arachnoiditis ranks as the third most frequent cause of failed back surgery syndrome (FBSS). This narrative review explores the evolving radiological approaches in its diagnosis and monitoring. The historical perspective traces the progression from plain radiographs to advanced imaging techniques. Current radiological modalities, including X-ray, computed tomography (CT), and magnetic resonance imaging (MRI), are discussed, highlighting their respective roles, advantages, and limitations. Emerging and advanced imaging modalities, such as high-resolution CT, 3T and 7T MRI, and PET/CT or PET/MRI, are examined for their potential to enhance diagnostic accuracy and monitoring capabilities. A comparative analysis of these imaging modalities considers their sensitivity, specificity, cost-effectiveness, and radiation exposure implications. The review also explores the crucial role of imaging in disease monitoring and treatment planning, including follow-up protocols, evaluation of disease progression, and guidance for interventional procedures. Future directions in the field are discussed, focusing on promising research areas, the potential of artificial intelligence and machine learning in image analysis, and identified gaps in current knowledge. The review emphasizes the importance of a multimodal imaging approach and the need for standardized protocols. It concludes that while significant advancements have been made, further research is necessary to fully understand the correlation between imaging findings and clinical outcomes. The continued evolution of radiological approaches is expected to significantly improve patient care and outcomes in AO.
Intractable Scapular Pain Due to Undiagnosed Osteoid Osteoma: A Case Report

Abstract:A 36-year-old female patient presented to our stroke neurology clinic for progressively worsening intractable, sharp, shooting interscapular pain radiating to the right shoulder and neck, which she had experienced for 4 years. She had previously seen an orthopedist and was referred to a neurosurgeon for surgical intervention after an MRI of the cervical spine showed the C3-C4 right vertebral artery loop protruding into the right C3-C4 neural foramen and compressing the exiting C4 nerve root. MR neurography showed a stable tortuous right vertebral artery loop, causing a mass effect on the dorsal root ganglion. A neuroforaminal decompression surgery was planned. However, the patient visited our stroke neurology clinic for a second opinion before surgery. An MRI of the thoracic spine showed an enhancing soft tissue mass at the right T4-T5 pedicles and adjacent body. A chest CT with contrast showed a 1 cm radiolucent lesion in the superior articular facet of T5, which represented a nidus. A technetium bone scan showed focal increased uptake within the right T5 pedicle, which is indicative of osteoid osteoma. The patient underwent laminectomy/resection and was pain-free at a 6-month follow-up; biopsy confirmed osteoid osteoma. This case illustrates the importance of neurolocalization during diagnostic testing.

Abstract:The occurrence of true bone in muscle has always held something of mystery, and even at the present time there is no generally accepted belief as to its method of origin.Although comparatively rare, more than five hundred cases, each the result of a single trauma, have been reported in the literature. Up to 1907, Strauss1 was able to collect reports of only 150 cases.The malady results from a severe traumatism such as the kick of a horse, a blow from a sharp-cornered object, or dislocation of the elbow, without break in the skin or subsequent suppuration. Nearly all cases occur in the brachialis anticus or the quadriceps extensor, rarely in other muscles. It is found most commonly in vigorous middle-aged men. Porter and Quinby2 state that it is most commonly a result of posterior dislocation of the elbow. Machol3 records sixteen cases following immediate reposition

Abstract:A case of chronic multifocal osteomyelitis was described in terms of its clinical manifestations, serological and imaging examinations, diagnostic criteria, treatment options, and follow-up evaluation after discharge. The pathogenesis, diagnosis, differential diagnosis and treatment of chronic multifocal osteomyelitis were reviewed, and the characteristics of autoinflammatory osteopathy were reviewed. The patient with onset from youth had developed severe skin lesions, progressive arthralgia and rachialgia. The clinical manifestation and the auxiliary examination of the patient accorded with the diagnosis of chronic multifocal osteomyelitis. After poor anti-inflammatory and analgesic effects, the switch to tumor necrosis factor alpha (TNF-α) inhibitor resulted in pain relief, normalization of inflammation indexes, and significant improvement in rash and imaging examination. Chronic recurrent multifocal osteomyelitis was a kind of autoinflammatory bone disease of multiple genes in disease with low incidence, unknown mechanism and unified diagnostic criteria. It was also known as chronic nonbacterial osteomyelitis, which was a rare, noninfectious inflammatory disorder that caused multifocallytic bone lesions characterized by periodic exacerbations and remissions. The exact pathophysiology or mechanism of the sterile bone inflammation was poorly understood, although chronic nonbacterial osteomyelitis was probably an osteoclast-mediated disease. In addition, an imbalance between pro- and anti-inflammatory cytokines was suspected to play a role. The available data so far pointed to the interplay among genetics, environmental, and immunologic factors as the causes of chronic nonbacterial osteomyelitis. Infectious etiology did not seem to play a crucial role in the pathogenesis of chronic nonbacterial osteomyelitis. It was often confused with metabolic bone disease, infection, tumor and other diseases. Its clinical manifestations were bone pain, fever, rash, fracture and so on. Laboratory examination showed significant increase in inflammatory markers. Radiographic examination revealed osteolytic or sclerosing changes. Magnetic resonance imaging was very useful for identifying bone lesions and tissue edema and was more accurate than bone emission computed tomography (ECT). Most of the patients begin to use non-steroidal anti-inflammatory drugs (NSAIDs) for treatment, but they are prone to relapse and new lesions appear.
