Abstract:Background The membership of the North American Skull Base Society (NASBS) has grown considerably in recent years with diversity in subspecialty training, gender, and geography. The academic relationships and contributions of its membership have not been studied. Objectives This study aimed to (1) measure academic contributions of NASBS membership; (2) identify influential nodes of academic collaboration; (3) identify opportunities for future collaboration and mentorship. Methods Peer-reviewed publications of members of NASBS (2019 NASBS website) were identified using Scopus author name search. Network structures were constructed and analyzed using the graph-tool python library to produce a weighted coauthorship network and compute centrality measures. Visual network maps were then produced using Gephi network visualization software. Results The coauthor network contained 952 members with found publications and 4,996 connections. A total of 846 (88.9%) members were contained in a single connected giant component; 102 members were unconnected, and 64 members had a single connection. Girvan–Newman clustering identified 267 communities, where 13 contained at least 1% of the total membership each. The three largest communities contained 23.3, 8.4, and 6.9% of members. There were 111 published members identified as women; 5.4% of women were unconnected versus 11.4% of men. Average publication count for women was 32.3 (standard deviation [SD] 38.5) versus 70.5 (SD 106) for men. Average citation count for women was 543 (SD 1,012) versus 1,389 (SD 2,893) for men. Conclusion Network mapping of membership of the NASBS helps to visualize the academic activities and relationships of the NASBS and reveals areas of concentration and influence within the specialty. Network Analysis of Past Presidents of the North American Skull Base Society

Abstract:Background Little is known about the relative contributions and interactions of the past presidents of the North American Skull Base Society (NASBS) and skull base centers. Objectives (1) Measure academic contributions of past presidents; (2) identify influential nodes of academic collaboration; (3) identify opportunities for future collaboration. Methods Peer-reviewed publications of past presidents of NASBS from 1964 to July 2019 were identified using Scopus author name search. Network structures were constructed and analyzed using the graph-tool python library to produce a weighted co-authorship network base and compute centrality measures. Girvan–Newman clustering was applied to identify community structure. Network maps were then produced using Gephi network visualization software with force-directed layout algorithms. Results The coauthor network of 29 presidents was fully connected, with a maximum shortest-path distance between presidents of 5. The mean number of connections from each node without respect to weighting was 5.31 (standard deviation [SD]: 3.53), and the mean number of connections with weighting was 8.40 (SD: 7.28). The number of unweighted connections ranged from 1 to 14 and weighted connections ranged from 0.25 to 24.7. Girvan–Newman clustering identified three communities with two that covered 93% of the network. The largest communities contained 14 and 13 presidents. The number of connections was correlated with h-index, both unweighted ( r 2 = 0.34) and weighted ( r 2 = 0.26). Conclusion Network mapping of past presidents of the NASBS helps to capture the history of the NASBS and reveals areas of concentration and influence within the specialty. Abstract:Background There are increased efforts to promote gender diversity in skull base specialties. Aims/Objectives This study seeks to evaluate changes within neurotology, rhinology, and skull base neurosurgery (SBNS) over the past decade. Methods Information regarding fellows training in the past 10 years, current faculty members and fellowship directors, and grants and awards were extracted from the American Neurotology Society, American Rhinologic Society, and North American Skull Base Society Web sites, tabulated, and summarized. Chi-square analysis compared the proportional makeup of female fellows within related fields. Results In the past decade in neurotology, men constituted 89 of 128 (69.53%) fellows, 132 of 170 (77.64%) faculty members, and 25 of 29 (86.2%) fellowship directors. Within rhinology, men constituted 211 of 279 (75.63%) fellows, 113 of 141 (80.14%) faculty members, and 32 of 35 (91.4%) fellowship directors. In SBNS programs, men constituted 94 of 105 (90%) fellows, 120 of 133 (90%) faculty members, and 43 of 44 (97.73%) fellowship directors. In neurotology fellowships, the proportion of women increased (12%, p = 0.06) from 2012–2017 to 2018–2022. Similarly, SBNS and rhinology had statistically insignificant increases of 4.28% ( p = 0.86) and 8.70% ( p = 0.87). Conclusion The representation of women is consistent across all three skull base specialties. Although the overall number of women in each field is increasing, it changes slowly. Abstract:Introduction Fellowship training has become increasingly sought after by neurosurgeons aiming for academic careers over the last two decades. This study assesses American Board of Neurological Surgeons board-certified neurosurgeons specializing in skull base or open cerebrovascular surgery between 2013 and 2023, focusing on identifying academic career predictors through demographic and academic outputs. Methods The study utilized the American Association of Neurological Surgeons Neurosurgical Fellowship Training Program Directory to identify neurosurgeons certified from 2013 to 2023, gathering demographic details and academic productivity from Scopus. Results Among 173 neurosurgeons, 87.86% were male, 36.36% were graduates from top 40 National Institutes of Health-funded medical schools, and 49.42% completed their residency in highly ranked departments. In univariate analysis, predictors for an academic career included publishing in the field before residency ( p = 0.03054), a higher h-index before and after residency ( p = 0.03976 and 0.0003101), and increased publication volume during and up to 3 years post-fellowship (2.284e-06). Multivariate analysis found that publication volume during and up to 3 years post-fellowship (odds ratio [OR] = 4.98, 95% confidence interval [CI]: 2.07–11.9, p = 0.0003) and basic science publications (OR = 2.4, 95% CI: 1.05–5.49, p = 0.038) were the most significant predictors of academic career placement. Conclusion The study underscores the strong link between the academic career success of neurosurgeons trained in skull base and open cerebrovascular surgery and their research productivity, particularly publication volume during key career stages and involvement in basic science research. This highlight sustained research activity as a critical determinant of academic career achievement, surpassing the influence of training institution prestige. Abstract:We extracted and processed abstract data from the SFN annual meeting abstracts during the period 2001-2006, using techniques and software from natural language processing, database management, and data visualization and analysis. An important first step in the process was the application of data cleaning and disambiguation methods to construct a unified database, since the data were too noisy to be of full utility in the raw form initially available. The resulting co-author graph in 2006, for example, had 39,645 nodes (with an estimated 6% error rate in our disambiguation of similar author names) and 13,979 abstracts, with an average of 1.5 abstracts per author, 4.3 authors per abstract, and 5.96 collaborators per author (including all authors on shared abstracts). Recent work in related areas has focused on reputational indices such as highly cited papers or scientists and journal impact factors, and to a lesser extent on creating visual maps of the knowledge space. In contrast, there has been relatively less work on the demographics and community structure, the dynamics of the field over time to examine major research trends and the structure of the sources of research funding. In this paper we examined each of these areas in order to gain an objective overview of contemporary neuroscience. Some interesting findings include a high geographical concentration of neuroscience research in north eastern United States, a surprisingly large transient population (60% of the authors appear in only one out of the six studied years), the central role played by the study of neurodegenerative disorders in the neuroscience community structure, and an apparent growth of behavioral/systems neuroscience with a corresponding shrinkage of cellular/molecular neuroscience over the six year period.

Abstract:Background The current study presents the effort of a global collaborative group to review the management and outcomes of malignant tumors of the skull base worldwide. Patients and methods A total of 28 institutions contributed data on 3061 patients. Analysis evaluated clinical variables, survival outcomes, and multivariable factors associated with outcomes. Results The median age was 56 years (IQR 44–67). The open surgical approach was used in 55% (n = 1680) of cases, endoscopic resection was performed in 36% (n = 1087), and the combined approach in 9.6% (n = 294). With a median follow‐up of 7.1 years, the 5‐year OS DSS and RFS were 65%, 71.7% and 53%, respectively. On multivariable analysis, older age, comorbidities, histology, dural/intracranial involvement, positive margins, advanced stage, and primary site were independent prognostic factors for OS, DSS, and RFS. Adjuvant RT was a protective prognostic factor. Conclusion The progress across various disciplines may have contributed to improved OS and DSS in this study compared to previous reports.

Abstract:Background: There are critical disparities in the neurosurgical care provided around the globe due to challenges in resource allocation, training, and infrastructure. Global neurosurgical collaborations have replaced classical mission trips to address these disparities. However, the development of these collaborations and the impact of research funding on their growth have not yet been systematically studied. In this article, we use a graph theoretical approach to investigate trends in funding and co-authorship between and among authors from high-income countries (HICs) and authors from low- and middle-income countries (LMICs). Methods: A bibliometric search of the global neurosurgical literature returned 307 articles between 1985 and 2020. A connectivity analysis was conducted to compute the number of co-authorships between HIC-HIC, LMIC-HIC, and LMIC-LMIC authors. The number of connections, summarized as either a global sum of connections or an average number of connections per manuscript, were analyzed in the context of time and funding through parametric statistical tests. Results: An exponential increase in co-authorship collaboration was observed over time, especially after 2015. Notably, LMIC-LMIC collaborations appear to be rising at over twice the rate of other collaboration types. The presence of funding, in general, was associated with increased co-authorship of manuscripts by LMIC and HIC authors together (P = 0.033). A significant majority of the funding associated with LMIC-HIC co-authorships was supplied through charitable organizations and government grants (P = 0.034, P = 0.009, respectively). Most LMIC-LMIC co-authorships had no funding. Conclusions: This work shows significant and rapid growth in international neurosurgical partnerships, especially in HIC-LMIC and LMIC-LMIC collaborations. Also, a significant positive relationship exists between research funding and LMIC-HIC co-authorship trends. This work encourages us as a community to continue to expand our translational collaborations with LMIC neurosurgeons and establish funding mechanisms independent of HIC authors.

Abstract:Skull base surgery is a young surgical subspecialty currently led by its second generation of surgeons. At present, there is no literature that narrates the connection of the present to the past. An extended interview was held with Dr Jon H. Robertson, who helped establish the subspecialty in Memphis, TN, to aid in identifying and connecting sentinel events and key figures in the development of the discipline. The field drastically evolved during his era of practice (1975-present), with the advent of advanced imaging and technology, as well as the emergence of multidisciplinary skull base surgical teams. The intersection of the careers of Jon H. Robertson, James T. Robertson, Gale Gardner, Edwin Cocke, John Shea, Jr., and Jerrall Crook in Memphis catalyzed the standardization of a multidisciplinary approach to cranial base pathology. We report the findings of Dr Jon H. Robertson's extended interview, told against the backdrop of the history of the subspecialty. The story of the development of skull base surgery is told from the unique perspective of one who lived and shaped a pivotal segment in this historical timeline.

Residency Education Practices in Endoscopic Skull Base Surgery

Abstract:Background There has been increased interest in how residents train in the subspecialty of skull base surgery. Examining which training methods are popular and effective to optimize residency learning is necessary, especially with new training adjuncts available to the modern trainee. In this study, we survey North American Skull Base Society (NASBS) members to analyze endoscopic skull base surgery education methods. Methods The NASBS membership was surveyed regarding endoscopic skull base surgery teaching and feedback methods using a Likert scale via an anonymized REDCap form over 4 months. Results With a response rate of 10.1%, we found that informal teaching methods and verbal qualitative feedback were rated significantly more effective than other teaching and feedback methods ( p < 0.01). When comparing the opinions of otolaryngologists and neurosurgeons, otolaryngologists were less likely to believe feedback is most effective with a shared grading scale ( p < 0.01). Physicians with more than 10 years of experience posttraining felt model- and rubric-based teaching were used more frequently ( p < 0.01). Respondents indicated that standardization and use of simulation, artificial intelligence, and virtual reality should be at the forefront of educational practices used in the field in the coming 5 to 10 years. Conclusion Despite the current emphasis on informal training, respondents pointed to standardization and simulation as methods of endoscopic skull base surgery education that should be used more in the future. These results indicate an unmet need in skull base education. Future multi-institutional initiatives with NASBS membership participation are warranted.

Abstract:Background: Robotic neurosurgery is a rapidly advancing field with numerous applications in various subspecialties, including spine, functional, skull base, and cerebrovascular. This study aims to provide a comprehensive analysis of the most-cited articles on robotic neurosurgery. Methods: The Web of Science database was used to collect data, and bibliometric analysis was performed using VOSviewer and RStudio. Network analysis techniques such as co-occurrence, coauthorship, bibliographic coupling, and thematic mapping analyses were used to identify the top 100 most cited articles, major contributors, emerging trends, and noteworthy themes in the field. Results: The results showed that there has been a steady increase in the number of publications on robotic neurosurgery since 1991, with an exponential growth in the number of citations. The United States was the most common country of origin for articles, followed by Canada. The most productive authors in this field were Burton S.A. and Gerszten P.C., while the University of Pittsburgh was the most productive institution, and Neurosurgery was the most productive journal. Themes such as robotics, back pain, and prostate cancer, as well as trends in developing new technologies and improving the precision of surgical procedures, were identified. Conclusions: This study provides a comprehensive analysis of the most-cited articles on robotic neurosurgery. The broad range of topics and techniques explored emphasize the importance of continued innovation and investigation. Ultimately, the study's findings provide valuable guidance for future research and contribute to advancing our understanding of this critical area of study.

Abstract:Objective: This study aims to comprehensively analyze the global landscape of oral and maxillofacial surgery (OMFS) literature through bibliometrics. The research focuses on evaluating publication trends, influential topics, and the impact of scholarly works within the field. Methods: Following the principles of the Leiden Manifesto, a literature search was conducted using the Web of Science database, encompassing seven OMFS journals. A total of 18,218 publications were analyzed, with indicators such as publication language, geographic location, specialty, year, citation, and authorship examined. Statistical analyses, including Pearson correlation tests and angular coefficient calculations, were performed in RStudio. Results: The study revealed a strong upward trend in publications related to orthognathic surgery, trauma, and infections. Geographic analysis indicated a dominance of European and Asian contributions, with the USA leading in overall publications. The impact of self-citations was explored, emphasizing nuanced perspectives on their role in scholarly impact metrics. The data provided insights into the scholarly impact of OMFS publications, including the H-index, number of citations, and citing articles. Conclusion: The findings underscored potential global discrepancies in OMFS research, both geographically and in terms of topics covered. The study discussed the influence of language and journal impact on citation counts, emphasizing the role of English publications. Specific topics, such as trauma and infections, exhibited high scholarly interest and citation rates. Recommendations were made to encourage submissions on specific topics, such as cosmetic surgery and pathology, to enhance the diversity of OMFS literature. The study aims to foster international collaboration and leverage bibliometrics for evidence-based decision-making, educational strategies, and advancements in the dynamic field of oral and maxillofacial surgery.
Differences in productivity and collaboration patterns on spine-related research between neurosurgeons and orthopedic spine surgeons: Bibliometric analysis

Abstract:Background: Spinal surgeries are commonly performed by neurosurgeons and orthopedic spine surgeons, with many spine-related articles published by them. However, there has been limited research that directly compares their research achievements. This article conducted a comparative analysis of spine-related research achievements between neurosurgeons and orthopedic spine surgeons. This study examines differences in productivity and impact on spine-related research between them using these measures, particularly with a novel clustering algorithm. Methods: We gathered 2148 articles written by neurosurgeons and orthopedic spine surgeons from the Web of Science core collections, covering the period from 2013 to 2022. To analyze author collaborations, we employed the follower-leader clustering algorithm (FLCA) and conducted cluster analysis. A 3-part analysis was carried out: cluster analysis of author collaborations; mean citation analysis; and a category, journal, authorship, L-index (CJAL) score based on article category, journal impact factors, authorships, and L-indices. We then utilized R to create visual displays of our findings, including circle bar charts, heatmaps with dendrograms, 4-quadrant radar plots, and forest plots. The mean citations and CJAL scores were compared between neurosurgeons and orthopedic spine surgeons. Results: When considering first and corresponding authors, orthopedics authors wrote a greater proportion of the articles in the article collections, accounting for 75% (1600 out of 2148). The CJAL score based on the top 10 units each also favored orthopedic spine surgeons, with 71% (3626 out of 6139) of the total score attributed to them. Using the FLCA, we observed that orthopedic spine surgeons tended to have more collaborations across countries. Additionally, while citation per article favored orthopedic spine surgeons with standard mean difference (= -0.66) and 95%CI: -0.76, -0.56, the mean CJAL score in difference (= 0.34) favored neurosurgeons with 95%CI: 0.24 0.44. Conclusion: Orthopedic spine surgeons have a higher number of publications, citations, and CJAL scores in spine research than those in neurosurgeons. Orthopedic spine surgeons tend to have more collaborations and coauthored papers in the field. The study highlights the differences in research productivity and collaboration patterns between the 2 authors in spine research and sheds light on potential contributing factors. The study recommends the use of FLCA for future bibliographical studies.

Abstract:Objective Recent years have witnessed a rapidly growing interest in CVJ bony abnormalities, and a qualitative and quantitative analysis of relevant literatures is necessary. This study aims to identify and summarize the published articles related to craniovertebral junction bony abnormalities, to analyze and visualize the current research trends and major contributors. Methods We collected data from Web of Science, excluding certain article types. Two researchers screened articles for relevance. Data was organized with EndnoteX9, and analyzed using VOSviewer and CiteSpace for co-authorship, co-occurrence, keyword burst, and co-citation analyses to identify research trends and collaborations. Results A total of 2,776 articles were included, revealing an increasing trend in annual publications of CVJ bony abnormalities. The USA was the leading country. The Spine is the most popular journal with the highest number of publications and citations. Professor Goel Atul from India emerged as the most influential pioneer in this field. Keyword analysis highlighted surgical techniques, diagnosis, and anatomy as the primary research hotspots and Fixation, Placement, and Basilar invagination gradually become the new research trend. However, there is a relative weakness in basic research and epidemiology. Conclusion This study provides valuable insights into the current research trends and critical contributors in CVJ bony abnormalities, guiding evidence-based decisions and fostering international collaborations to advance knowledge in this field.

Abstract:Despite the globalization of health information, collaborations between high-income countries (HICs) and low/middle-income countries (LMICs), while present, could still increase. This study builds on previous research highlighting LMIC underrepresentation in neurosurgery literature. We conducted a comprehensive bibliometric analysis using the Scopus database to investigate collaborative neurosurgical research between HIC institutions and those in low-income country (LIC)/LMICs. Articles published between 2018 and 2020 were examined. Articles were categorized into 3 groups: guidelines, conferences, and consensus statements; articles related to training and collaborations; and other articles. We categorized articles and authors by country, role, and specific subtopic. We included 238 reports from 34 neurosurgical journals for analysis. Geographic distribution indicated that India led LIC/LMIC contributions (25.21%). Among HICs, the United States had the highest contribution (47.76%). In collaborative studies, Uganda, Cameroon, Tanzania, Indonesia, and Nigeria made significant contributions. LICs and LMICs accounted for 446 authors, while HICs contributed with 592. India has presented the highest number of authors in significant positions. In HICs, significant positions are recognized in USA articles. When scoring authors' position in collaborative papers, still HICs had a clear prevalence. The highest number of collaborations between HICs and LICs/LMICs has been observed in articles related to training and collaborations. Kenya matched India's contributions in training and collaborations. Global guidelines and consensus papers can enhance patient care, but LMICs' involvement remains limited. Further attention to training and collaboration initiatives is needed. This study emphasizes the importance of promoting collaboration and training between countries with varying resources to advance neurosurgical care globally.

Orthopaedic Academic Activity in the United States: Bibliometric Analysis of Publications by City and State

Abstract:Background: The purpose of this study was to conduct a bibliometric analysis of orthopaedic academic output in the United States. Methods: Publications based on city and state origin, corrected for population size, median household income, total number of surgeons, and the number of various subspecialties were evaluated. The 15 highest-ranked orthopaedic journals were audited from 2010 to 2014 and then subdivided into anatomic regions and 14 subspecialties. Results: A total of 8,100 articles were published during the study period. Most originated from New York, California, Pennsylvania, Massachusetts, and Minnesota. New York published the greatest number by city, followed by Philadelphia, Boston, Chicago, and Rochester. When adjusted for the number of publications per city, surgeons per population, publications per surgeon population, publications per population, and publications per median income per capita, Vail and New York led in two and Stanford in one of the metrics. Conclusions: New York was the leader for the total publications, greatest activity within subspecialties, and publications per surgeon/population and per median income/capita. Vail was the leader for publications/surgeon and population. The top four cities of New York, Philadelphia, Boston, and Chicago were responsible for 28% of the academic output over the 5-year study period.

Abstract:The landscape of the cranial neurosurgery has changed tremendously in past couple of decades. The main frontiers including introduction of neuro-endoscopy, minimally invasive skull base approaches, SRS, laser interstitial thermal therapy and use of tubular retractors have revolutionized the management of intracerebral hemorrhages, deep seated tumors other intracranial pathologies. Introduction of these novel techniques is based on smaller incisions with maximal operative corridors, decreased blood loss, shorter hospital stays, decreased post-operative pain and cosmetically appealing scars that improves patient satisfaction and clinical outcomes. The sophisticated tools like neuroendoscopy have improved light source, and better visualization around the corners. Advanced navigated tools and channel-based retractors help us to target deeply seated lesions with increased precision and minimal disruption of the surrounding neurovascular tissues. Advent of stereotactic radiosurgery has provided us alternative feasible, safe and effective options for treatment of patients who are otherwise not medically stable to undergo complex cranial surgical interventions. This paper review advances in treatment of intracranial pathologies, and how the neurosurgeons and other medical providers at the University of Missouri-Columbia (UMC) are optimizing these treatments for their patients.
A Review of Authorship Inflation and Multicenter Collaboration Trends in Orthopedic, Medical, and Surgical Journals Over the Last 60 Years

Abstract:Over the past six decades, authorship numbers in academic publications have increased significantly, a phenomenon known as authorship inflation. This study aims to analyze comparative authorship trends and the influence of multicenter collaborations across major orthopedic, medical, and surgical journals. We reviewed metadata from The New England Journal of Medicine (NEJM), Annals of Surgery (AS), and The Journal of Bone and Joint Surgery (JBJS) from January 1, 1960, to December 31, 2019. The number of authors per publication, the prevalence of multicenter studies, and their correlation were analyzed. Data was visualized using heat maps and box plots, and trends were statistically tested using the Jonckheere-Terpstra, Mann-Kendall, and generalized linear mixed models (GLMMs). A total of 73,062 articles were analyzed, with 1,190 articles identified as originating from multicenter studies. The number of multicenter trials was found to have increased significantly over time (p < 0.001), plateauing in NEJM but continuing to rise in JBJS and AS. There was a significant increase in authorship numbers per publication over time, across all journals (p < 0.0001). There was a significant statistical correlation (p < 0.0001) as indicated by the coefficient of determination (r2), for the association between the proportion of publications with >10 authors and the proportion of multicenter publications across all three journals. Authorship inflation in academic publishing may be attributable to the rise in multicenter collaborations. The rate of increase in authorship was more pronounced in medical and surgical journals compared to orthopedic journals, reflecting differing trends across specialties. These findings highlight the evolving nature of research collaboration and authorship practices in academic publishing.
