Abstract:Introduction: Many adjuvant methods have been researched to enhance the immediate implant success in the recent times. The study's goal was to compare the aesthetic and biological results of immediate dental implants in the aesthetic zone to standard tapered root form implants that had not undergone pre-treatment with platelet-rich plasma or photofunctionalization. Materials and methods: The study used a randomized controlled trial as its design. Ninety subjects were chosen at random and placed into three groups: a control group, a case group, and a group that needed replacement of their maxillary anterior teeth right away following extraction. The interventional groups received the "Platelet-Rich Plasma (PRP)" or "Photofunctionalization (PF)" group before the implant placement. After the delayed loading approach, the prosthesis was given after 6 months. At the second and fourth weeks, as well as 2, 4, 6, and 12 months (P 0.05), follow-up was conducted. The success and survival rate, aesthetic, and biological outcomes were assessed. One-way ANOVA was used to compare outcomes, and repeated-measures ANOVA was used to assess intragroup variations over baseline and follow-up. Results: The distal, mesial, and mean marginal bone loss as well as the aesthetic scores for pink and white did not differ significantly between groups. The implant stability however was significantly higher in the PF and PRP groups as compared to the controls. A single implant failed in all the three groups. The success percentage was similar for all the groups. Conclusion: The stability of immediate dental implants pre-treated with PRP or PF differed statistically significantly than the subjects in the control group, while other parameters remained the similar between the groups.

Abstract:There is a growing tendency to place single tooth implants immediately after extracting a failing tooth in the posterior region. The aim of this prospective case series pilot study was to evaluate immediate implant placement in molar-premolar post-extraction sites during a 1-year follow-up period.Aim of the study: To evaluate the effect of using Autogenous Partially Demineralized Dentin Matrix (APDDM) versus xenograft around Immediate implant.Patients and methods: twelve patients who have bilateral badly decayed tooth in premolar-molar area in a split mouth study. Group I: includes 12 Immediate implant in one side with autogenous partially deminaralized dentin matrix (APDDM) as a grafting material around the implant.Group II : includes 12 Immediate implant in the other side with Xenograft as a grafting material around the implant. Following parameters will be evaluated before treatment and recorded at baseline and after 6 months Plaque index (PI), Bleeding index (BI), Gingival recession (GR), Probing depth (PD), Conebeam Ct radiograph at baseline,6 month and 12month. Results: Four out of 15 of the mobile implants had to be removed before the 1-year evaluation. The implant and restoration survival rates were 73.3% at the 1-year evaluation (n = 15). The mean marginal bone loss, from loading to the 12-month follow-up, was 0.17 mm (n = 11). The mean plaque, calculus, peri-implant mucosa, bleeding, and pocket probing depth scores were low, depicting healthy peri-implant conditions. The patients were very satisfiedConclusion: The placement of immediate implants in the posterior sector is a widespread procedure where the success and survival rates are similar to those of traditional protocols. Abstract:Objective The SafetyCrown workflow facilitates the immediate restoration of posterior single sites with the one‐abutment/one‐time concept. This randomized clinical trial aimed to assess the direct effect of immediate restoration on dental patient‐reported outcomes (dPROs), feasibility, implant accuracy, and time. Materials and Methods Participants with a single posterior edentulous site for late implant placement underwent optical impressions, shade selection, and cone beam computed tomography. After virtual treatment planning, they were randomized into the test group and the control group. For the test group, individual definitive hybrid abutments were prefabricated. The next step was a fully guided surgery with printed guides. After the implant was placed using guided surgery, the abutment was inserted. A chairside CAD/CAM workflow was used to provide the patient with a provisional restoration. Implants in the control group were left submerged to heal. Oral health‐related quality of life (OHRQoL) was assessed using the OHIP‐G14, and dPRO was measured using a 10‐item visual analog scale (VAS) questionnaire. Additional measurements of implant accuracy and time were performed. Follow‐up was performed 7 to 10 days after implant placement. Results Thirty‐nine participants with 45 restorations were included (test group: 23, control: 22). Immediate restoration was successful in 21 out of 23 implants (91.3%) in the test group. Both groups exhibited decreased OHRQoL without significant intergroup differences, while patient satisfaction was high overall. Test group participants perceived higher benefits and satisfaction with immediate loading than participants in the control group. Evaluation of the Clinical Success of Immediate Loading Implant in the Aesthetic Zone: An In Vivo Study

Abstract:Aim: The aim of the current study was to assess the clinical success of immediate loading implant in the esthetic zone. Materials and methods: 20 patients with 20 teeth to get replaced were selected for the study. All patients underwent a pre-surgical preparation followed by draping. Following stringent asepsis, necessary precaution was taken to not to injure the socket wall while removal of tooth. After removal, socket was thoroughly cleaned and curetted with saline and betadine. Using physiodispenser and drills, socket was properly prepared for implant insertion taking care of irrigation with saline to avoid heating of socket while drilling. Implants were then placed with initial stability with hand motion followed by complete insertion with hand ratchet. Black silk sutures were used to close the socket. Patients underwent the first control one week after surgery. They were recalled at the 1st, 3rd and 6th months to evaluate the following parameters to assess the success of immediate implant placement: mobility, soft tissue conditions (gingival index and probing pocket depth (PPD)), and a graded scale ranging from "very satisfied" to "very unsatisfied" were employed to subjectively evaluate patient contentedness. Results: During the 1st and 3rd months, 100% mobility was absent. But on the 6th month, the implant of 3 patients (15%) was mobile. The maximum gingival index score was noted in the 1st month (1.02 ± 0.01) and reduced more in the 3rd month (0.74 ± 0.08). The probing depth was more in the 1st month (3.88 ± 0.10) and it was reduced in the 3rd month (3.02 ± 0.12). Significant difference was not found between different times of intervals. 14 patients were very satisfied, 5 patients were fairly satisfied, and 1 was fairly unsatisfied. Conclusion: The current study concluded that immediate implant placement in the esthetic zone has a better success rate with good patient acceptance.
Early implant placement with or without alveolar ridge preservation in single tooth gaps renders similar esthetic, clinical and patient-reported outcome measures: One-year results of a randomized clinical trial

Abstract:Objectives: To test whether early implant placement with alveolar ridge preservation (ARP) results in different esthetic, clinical and patient-reported outcome measures (PROMs) compared with early implant placement without ARP. Material and methods: Seventy-five patients requiring single tooth extraction in the anterior maxilla were recruited. Following tooth extraction, the patients were randomly allocated to three groups: (a) ARP using demineralized bovine bone mineral containing 10% collagen (DBBM-C) covered by a collagen matrix (CM) (n = 25), (b) ARP using DBBM-C covered with a palatal graft (PG) (n = 25) and (c) spontaneous healing (control) (n = 25). Eight weeks after tooth extraction, a CBCT was taken and early implant placement was performed in all patients. Esthetic, clinical and PROMs were evaluated one year post-loading. Results: A total of 70 patients were available for re-examination at one year post-loading. The median mid-facial mucosal margin change amounted to -0.02 mm (IQR -0.27-0.46) in the CM group, -0.13 mm (IQR -0.44-0.25) in the PG group and -0.14 mm (IQR -0.29-0.07) in the control group, with no significant differences between the groups. Mean PES scores amounted to 7.0 ± 1.4 in the CM group, 7.1 ± 1.5 in the PG group and 7.3 ± 1.7 in the control group without significant differences between the groups. Plaque, bleeding on probing and probing depth did not differ between treatment groups. PROMs in general revealed no significant differences between the groups. Conclusion: Early implant placement with ARP using either a collagen matrix or a palatal graft rendered similar esthetic, clinical and PROMs to early implant placement without ARP. When a failing tooth can be replaced with an implant within 2 months after tooth extraction, the added value of ARP might be clinically negligible.

Abstract:Aim: Tooth loss can affect the person’s life dramatically. Tooth loss is accompanied by bone loss in all dimensions, so preservation of the bone is mandatory for placement of dental implant. Immediate implant can preserve alveolar bone. Due to difference in shape between the implants and extracted roots a gap appears between the immediately placed implant and the extraction socket resulting in jumping distance, Bone grafts are commonly used to fill this gap, recent studies suggest that jumping gaps shouldn’t always be grafted. Low level bio stimulation has stimulatory effect on bone cells so it can be used in immediate implants to increase bone formation around the implants. Materials and methods: this study was carried in the hospital of future university. A total of twelve patients with non-restorable mandibular molar participated in this study. A total of twenty implants were placed immediately in mandibular molar region without bone grafting, thew were divided equally into two groups. Surgical technique was similar for both groups. After immediate implant placement with primary stability of 35 N, both groups received custom made healing collar using flowable composite to seal the jumping distance.The study group (intervention) received a total of 60J/cm2 .10J/cm2 3 days before extraction, another 10J/cm2 at the fresh socket, another 10 J/cm2 after osteotomy, another 10J/cm2 after implant placement, another 20J/cm2 at the follow up visit with 3 days interval. After six weeks secondary implant stability was measured for both groups using Resonance Frequency Analysis device (RFA). Prosthetic phases started for implants that showed sufficient secondary stability. Results: All implants enrolled in this study showed sufficient secondary stability except for one totally avulsed implant from the control group. Study group showed more stability upon measuring using RFA device after six weeks of implant placement. The study group has shown a higher secondary stability compared to control group, the mean ISQ value of study (71.89±2.67) was significantly higher than control (62.43±8.62). Conclusion: Based on the results of our study we concluded that low level diode laser 980nm has a stimulatory effect on bone formation when applied before, during and after immediate implant placement, According to this study 60J/cm2 is recommended dose and it also gave better soft tissue healing results

It is now recognized that some early loading of a nonocclusal nature can stimulate faster osseointegration, although full occlusal loading is still delayed for 3 or more months. Here, we test the hypothesis that earlier occlusal loading of mandibular premolar and molar immediate implants may be possible. In this retrospective case series study, 18 mandibular molar and nine mandibular premolar teeth were atraumatically extracted and immediate implants placed 1–2 mm apical to buccal and lingual crestal bone. Periimplant gaps received particular allograft covered with acellular dermal matrix barrier. Healing abutments were placed through puncture points in the membranes to help in stabilizing the latter and to permit nonsubmerged site healing. At 6−8 weeks, each implant was evaluated for stability using the Periotest® device and restored if the Periotest® (PTV) value seen was negative. Data were analyzed by t test and MannWhitney U at a significance level of P < 0.05. Retrospective assessment of all 27 implants after 5 years' period of follow up showed all implants to have survived. Overall mean crestal bone loss was determined to be−0.25 ± 0.54 mm. Individual mean bone levels for mesial and distal surfaces were−0.24 ± 0.77 mm and−0.26 ± 0.72 mm, respectively (P = 0.78). A statistically significant difference in bone loss between genders was detected. Overall mean probing depth was 2.09 ± 0.57 mm. Based on the widely used Albrektsson criteria, the overall survival and success rate was 100%. Immediate implants placed into mandibular premolar and molar extraction sockets and allowed nonsubmerged healing may be ready for restoration at earlier times than previously thought possible.

Abstract:Objective. The present research is aimed at determining the efficacy of immediate implantation (II) and delayed implantation (DI) for single-tooth restoration of maxillary anterior teeth. Methods. From February 2019 to June 2020, 80 patients who received single-tooth restoration of maxillary anterior teeth in Suzhou Kowloon Hospital, Shanghai Jiaotong University School of Medicine, were included, among which 38 cases with DI restoration were used as the control group (CG), and the remaining 42 cases with II were used as the research group (RG). The complications that occurred were recorded. Besides, subjective satisfaction (Visual Analogue Scale (VAS)), aesthetic effect after anterior teeth trauma restoration (Pink Esthetic Score (PES)), aesthetics of dental hard tissue (White Esthetic Score (WES)), pocket depth assessed by pure titanium periodontal probe, implant stability (Implant Stability Quotient (ISQ)), and oral health-related quality of life (Oral Health Impact Profile- (OHIP-) 14) were evaluated. Attachment height, general look, color, and chewing function were all much higher in RG than in CG, according to the evaluation results. Furthermore, at 3 months, 6 months, and 12 months after surgery, RG had greater PES, WES, ISQ, and OHIP-14 scores, while the periodontal depth was decreased. In both groups of patients, the incidence of complications was similar, with no discernible differences.

Clinical Outcomes of Maxillary Anterior Postextraction Socket Implants with Immediate Provisional Restorations Using a Novel Macro-Hybrid Implant Design: An 18- to 24-Month Single-Cohort Prospective Study

Abstract:Extraction and immediate implant placement/restoration in the esthetic zone is clinically challenging; benefits include fewer surgical appointments and maintenance of peri-implant soft tissues throughout the treatment period, and limitations include gingival recession and bone dehiscence during surgery. Macro-hybrid implants (large-diameter apical/narrow-diameter occlusal) were placed in 19 patients immediately following the extraction of hopeless maxillary anterior teeth. Immediate restorations were fabricated without occlusal contacts. Pre- and postplacement cone beam computed tomography (CBCT) scans were taken. Nineteen implants were available for recall 13 to 25 months postoperatively. The overall implant cumulative survival rate was 100% (range: 13 to 25 months, mean: 19 months), and mean insertion torque value was 65 Ncm. Mean Pink Esthetic Score was 12.63 at 6 months, and was 13 at the 18- to 24-month follow-up. Mean mesial and distal tooth-to-implant distances immediately after implant placement were 2.55 ± 1.29 mm and 2.29 ± 0.82 mm, respectively. Interproximal bone crest width, distance, and height were maintained at implant platforms, mesially and distally, 18 to 24 months postoperative. The results of this study indicated that the macro-hybrid implant geometry for this immediate surgical/restorative protocol provided excellent and stable 2-year results relative to implant survival (100%), labial plate thickness via CBCT evaluations, tooth-to-implant distances immediately post-implant placement, PES, and interproximal bone crest width, distance, and heights, which were maintained at the implant platforms.
Immediate loading of multiple splinted implants via complete digital workflow: A pilot clinical study with 1-year follow-up.

Abstract:Background Complete digital workflow attracts more attention in implant dentistry. Objectives To explore the feasibility and short-term clinical results of immediate loading of multiple implants with fixed temporary bridges (2-4 teeth span) by complete digital workflow, and to evaluate the three-dimensional (3D) deviation of digital impression comparing with traditional impression method. Material and Methods A total of 31 partial edentulous patients (16 females and 15 males) were recruited in this study. Digital impressions were taken immediately after implant placement, and implant-supported splinted temporary bridges were fabricated through a full digital approach (model free) and delivered within 24 hours. Final restorations were finished 4 months after surgery via traditional impression technique. Subjects were followed 1 year after treatment. 3D impression deviations were analyzed by comparing the digital and conventional impression methods. Time costs for the full digital approach were recorded. Implant survival rate, marginal bone levels were evaluated. Results All the recruited subjects finished this study. Seventy-four implants were surgically placed and immediately loaded with 34 temporary bridges fabricated through a full digital approach. Digital impression deviation compared with traditional impression method was 27.43 +/- 13.47 mu m. Time costs for chair side and laboratory were 32.55 +/- 4.73 and 69.30 +/- 10.87 minutes, respectively. Marginal bone alterations were -1.58 mm and -1.69 mm at the time of 4 and 12 months after surgery. The implants had a survival rate of 100% at the 1-year follow-up time. Conclusions Immediate loading of multiple implants in partial edentulous (2-4 teeth span) patients with full digital approach is clinically applicable. The 3D discrepancy between digital and traditional impression is within clinical acceptable range.
Influence of Immediate Implant Placement and Provisionalization with or without Soft Tissue Augmentation on Hard and Soft Tissues in the Esthetic Zone: A One-Year Retrospective Study

Abstract:The purpose of this clinical research was to evaluate peri-implant marginal changes around immediate implants placed either with the application of SCTG or XCM or without soft tissue grafting. A total of 48 patients requiring a single implant-supported restoration in the anterior jaw were selected for inclusion. Three surgical procedures were performed, as follows: type 1 implant with subepithelial connective tissue graft (SCTG), type 1 implant with xenogenic collagen matrix (XCM), and type 1 implant without soft tissue augmentation (NG) (control group). The marginal change of peri-implant soft tissue, facial soft tissue thickness (FSTT), peri-implant health status, esthetics, and patient satisfaction were assessed at one year after surgery. All of the placed implants showed a survival rate of 100%. No significant differences in FSTT were recorded between the SCTG group and the XCM group after treatment ( P > 0.05 ), while the NG group presented a significant difference ( P < 0.05 ). Patients in the NG group lost significantly more in the buccal marginal level than did patients in the SCTG group and those in the XCM group ( P < 0.05 ). The favourable success rate recorded in all groups confirmed immediate tooth replacement as a choice of treatment for a missing anterior single tooth. The NG group presented significant changes of FSTT and buccal marginal level, while XCM constituted a viable alternative to SCTG.

Abstract:Aim To compare the marginal bone level of immediately placed implants, with either immediate or delayed provisionalization (IP or DP), in the maxillary aesthetic zone after 10 years of function. Materials and Methods Participants with a failing tooth in the maxillary aesthetic zone were randomly assigned to immediate implant placement with either IP (n = 20) or DP (n = 20) after primary wound closure with a free gingival graft. The final restoration was placed 3 months after provisionalization. The primary outcome was change in marginal bone level. In addition, implant survival, restoration survival and success, peri‐implant tissue health, mucosa levels, aesthetic indices, buccal bone thickness and patient satisfaction were evaluated. Results After 10 years, the mean mesial and distal changes in marginal bone level were −0.47 ± 0.45 mm and −0.49 ± 0.52 mm in the IP group and −0.58 ± 0.76 mm and −0.41 ± 0.72 mm in the DP group (p = .61; p = .71). The survival rate was 100% for the implants; for the restorations, it was 88.9% in the IP group and 87.5% in the DP group. Restoration success, according to modified USPHS criteria, was 77.8% in the IP group and 75.0% in the DP group. The prevalence of peri‐implant mucositis was 38.9% and 35.7% and of peri‐implantitis 0.0% and 6.3%, respectively, in the IP group and DP group (p = 1.0; p = .40). The Pink Esthetic Score and White Esthetic Score was 15.28 ± 2.32 in the IP group and 14.64 ± 2.74 in the DP group, both clinically acceptable (p = .48). The buccal bone thickness was lower in the DP group. Patient satisfaction was similar in both groups (p = .75). Conclusions The mean marginal bone level changes after immediate implant placement with IP were similar to those after immediate placement with DP. Abstract:PURPOSE: Several procedures are performed to achieve optimal esthetic results in single-tooth implants. However, there is discordance regarding the potential benefit and risks of immediate implant loading/provisionalization. The aim of this prospective case series is to investigate the effect of immediate provisionalization of single-tooth implants at healed sites for periimplant soft-tissue conditions, focusing on papilla formation around single implants.MATERIALS AND METHODS: Twelve patients received a total of 12 implants in the incisor, canine or premolar region of the upper and lower jaw at healed sites with immediate chair-side provisionalization. Four months later, the temporary crown was replaced by the permanent crown. After 40±13.1 months, clinical follow-up was conducted, assessing Probing pocket depth (PPD); Bleeding on Probing(BoP); Mucosal recession (MR) and Width of Keratinized Mucosa (KM). Papilla index (PI) was determined immediately after implant placement (baseline), before removing the temporary crown (t1), 4 weeks after insertion of the definitive crown (t2) and at the final follow-up examination (t3) to evaluate papilla formation and its change over time.RESULTS: None of the implants were lost. The mean PPD was 2.5±0.39 mm, BoP of 25% and 3.5 mm of KM were observed at the final follow-up. No implants showed MR. PI increased in all patients from 1.5±0.45 at baseline to 2.4±0.56 at t1, 2.6±0.47 at t2 and 3.02.6±0 at t3. The increase in PI between t0 and each individual timepoint from t1-t3 showed statistical significance.CONCLUSION: The present results indicate the suitability and benefit of immediate provisionalization to achieve favorable peri-implant soft-tissue conditions and papilla formation.

Efficacy of antimicrobial photodynamic therapy in the management of chronic periodontitis: a randomized controlled clinical trial

Abstract:Aim: To evaluate the potential of antimicrobial photodynamic therapy (aPDT) as an adjunct to scaling and root planing (SRP) in the treatment of chronic periodontitis. Material and methods: In a single-centred randomized and controlled clinical trial, 90 patients (51 females and 39 males) with untreated chronic periodontitis were randomly assigned to receive SRP with aPDT (test group) or SRP alone (control group). Clinical parameters and halitosis were recorded for 6 months after treatment by a periodontist who was blinded to the procedure. Results: Inter-group and intra-group statistical analyses were performed. Significant difference between the two groups with respect to each variable was assessed using non-parametric Rank Order ancova. Probing pocket depth and clinical attachment levels showed statistically significant reduction in the test group on evaluation at 3 months and 6 months as compared to the control group (p < 0.05). A statistically significant improvement in gingival index and gingival bleeding index was seen for the test group after 2 weeks and 1 month of aPDT (p < 0.01), whereas the improvement in gingival index and gingival bleeding index at 3 months and in plaque index at 2 weeks after aPDT was less (p < 0.05). Also, a significant difference was detected for the test group at 1 month in terms of halitosis (p < 0.05), which did not persist for long. Conclusions: Antimicrobial photodynamic therapy acts as a beneficial adjunct to SRP in non-surgical treatment and management of chronic periodontitis in short-term. Further studies are required to assess the long-term effectiveness of aPDT.

Abstract:Purpose: To compare the outcomes of immediate and delayed implant placement with bone-level tapered implants. Materials and methods: In this post-market, multicenter prospective randomized controlled study with a primary endpoint of 1 year, 53 patients were randomized to receive either immediate implant placement (test group) or delayed implant placement (control group). The mean crestal bone level changes from implant loading to 12 months postloading were measured using standardized digital periapical radiographs. Changes in facial plate thickness (as measured on CBCT images), implant success and survival, implant stability, soft tissue changes, patient-centered outcomes, and adverse events were measured to assess outcomes between the test and control treatments at 12 months postloading. Results: Of the original 53 patients, 46 patients completed the study (23 in each group). Mean bone changes from loading to the 12-month follow-up were recorded with no statistically significant difference (P = .950) between the groups. The hypothesis was confirmed that immediate implant placement (test) in extraction sockets produces in similar outcomes as delayed placement (control). The test group was found to be similar to the control group (P = .022) in terms of mean changes in facial plate thickness. Implant survival and success were 95.8% in the test group and 92% in the control group. Stability in the control group was superior at the time of surgery, but there was no difference between the groups at implant loading, producing a nonsignificant P value of .563). Conclusions: This randomized controlled multicenter study showed comparable outcomes 1 year after prosthetic loading in the immediate and delayed implant placement groups.
