Abstract:Delayed separation of survival curves is a common occurrence in confirmatory studies in immuno-oncology. Many novel statistical methods that aim to efficiently capture potential long-term survival improvements have been proposed in recent years. However, the vast majority do not consider stratification, which is a major limitation considering that most (if not all) large confirmatory studies currently employ a stratified primary analysis. In this article, we combine recently proposed weighted log-rank tests that have been designed to work well under a delayed separation of survival curves, with stratification by a baseline variable. The aim is to increase the efficiency of the test when the stratifying variable is highly prognostic for survival. As there are many potential ways to combine the two techniques, we compare several possibilities in an extensive simulation study. We also apply the techniques retrospectively to two recent randomized clinical trials.

Abstract:Weighted logrank tests are a popular tool for analysing right-censored survival data from two independent samples. Each of these tests is optimal against a certain hazard alternative, for example, the classical logrank test for proportional hazards. But which weight function should be used in practical applications? We address this question by a flexible combination idea leading to a testing procedure with broader power. Besides the test's asymptotic exactness and consistency, its power behaviour under local alternatives is derived. All theoretical properties can be transferred to a permutation version of the test, which is even finitely exact under exchangeability and showed a better finite sample performance in our simulation study. The procedure is illustrated in a real data example.

A Class of Weighted Log-Rank Tests for Survival Data when the Event is Rare

Abstract:In many epidemiological and medical follow-up studies, a majority of study subjects do not experience the event of interest during the follow-up period. An important example is the ongoing prostate, lung, colorectal, and ovarian cancer screening trial of the National Cancer Institute. In such a situation, the widely used G(rho) family of weighted log-rank statistics essentially reduces to the special case of the (unweighted) log-rank statistics. We propose a simple modification to the G(rho) family that adapts to survival data with rare events, a concept that we formulate in terms of a small number of events at the study endpoint relative to the sample size. The usual asymptotic properties, including convergence in distribution of the standardized statistics to the standard normal, are obtained under the rare event formulation. Semiparametric transformation models forming sequences of contiguous alternatives are considered and, for each rho, a specific such model is identified so that the corresponding modified G(rho) Statistic is asymptotically efficient. Simulation studies show that the proposed statistics do behave differently from the original G(rho) statistics when the event rate during the study period is low and the former could lead to a substantial efficiency gain over the latter. Extensions to the G(rho,gamma) family and to the regression problem are also given.

Abstract:A common feature of many recent trials evaluating the effects of immunotherapy on survival is that non-proportional hazards can be anticipated at the design stage. This raises the possibility to use a statistical method tailored towards testing the purported long-term benefit, rather than applying the more standard log-rank test and/or Cox model. Many such proposals have been made in recent years, but there remains a lack of practical guidance on implementation, particularly in the context of group-sequential designs. In this article, we aim to fill this gap. We discuss how the POPLAR trial, which compared immunotherapy versus chemotherapy in non-small-cell lung cancer, might have been re-designed to be more robust to the presence of a delayed effect. We then provide step-by-step instructions on how to analyse a hypothetical realisation of the trial, based on this new design. Basic theory on weighted log-rank tests and group-sequential methods is covered, and an accompanying R package (including vignette) is provided.

Abstract:Background: Most randomized controlled trials with a time-to-event outcome are designed assuming proportional hazards (PH) of the treatment effect. The sample size calculation is based on a logrank test. However, non-proportional hazards are increasingly common. At analysis, the estimated hazards ratio with a confidence interval is usually presented. The estimate is often obtained from a Cox PH model with treatment as a covariate. If non-proportional hazards are present, the logrank and equivalent Cox tests may lose power. To safeguard power, we previously suggested a 'joint test' combining the Cox test with a test of non-proportional hazards. Unfortunately, a larger sample size is needed to preserve power under PH. Here, we describe a novel test that unites the Cox test with a permutation test based on restricted mean survival time. Methods: We propose a combined hypothesis test based on a permutation test of the difference in restricted mean survival time across time. The test involves the minimum of the Cox and permutation test P-values. We approximate its null distribution and correct it for correlation between the two P-values. Using extensive simulations, we assess the type 1 error and power of the combined test under several scenarios and compare with other tests. We investigate powering a trial using the combined test. Results: The type 1 error of the combined test is close to nominal. Power under proportional hazards is slightly lower than for the Cox test. Enhanced power is available when the treatment difference shows an 'early effect', an initial separation of survival curves which diminishes over time. The power is reduced under a 'late effect', when little or no difference in survival curves is seen for an initial period and then a late separation occurs. We propose a method of powering a trial using the combined test. The 'insurance premium' offered by the combined test to safeguard power under non-PH represents about a single-digit percentage increase in sample size. Conclusions: The combined test increases trial power under an early treatment effect and protects power under other scenarios. Use of restricted mean survival time facilitates testing and displaying a generalized treatment effect.

Abstract:Most statistical tests for treatment effects used in randomized clinical trials with survival outcomes are based on the proportional hazards assumption, which often fails in practice. Data from early exploratory studies may provide evidence of non-proportional hazards which can guide the choice of alternative tests in the design of practice-changing confirmatory trials. We study a test to detect treatment effects in a late-stage trial which accounts for the deviations from proportional hazards suggested by early-stage data. Conditional on early-stage data, among all tests which control the frequentist Type I error rate at a fixed $\alpha$ level, our testing procedure maximizes the Bayesian prediction of the finite-sample power. Hence, the proposed test provides a useful benchmark for other tests commonly used in presence of non-proportional hazards, for example weighted log-rank tests. We illustrate the approach in a simulations based on data from a published cancer immunotherapy phase III trial.

Abstract:In confirmatory clinical trials, survival outcomes are frequently studied and interim analyses for efficacy and/or futility are often desirable. Methods such as the log rank test and Cox regression model are commonly used to compare treatments in this setting. They rely on a proportional hazards (PH) assumption and are subject to type I error rate inflation and loss of power when PH are violated. Such violations may be expected a priori, particularly when the mechanisms of treatments differ such as immunotherapy vs. chemotherapy for treating cancer. We develop group sequential tests for comparing survival curves with covariate adjustment that allow for interim analyses in the presence of non-PH and offer easily interpreted, clinically meaningful summary measures of the treatment effect. The joint distribution of repeatedly computed test statistics converges to the canonical joint distribution with a Markov structure. The asymptotic distribution of the test statistics allows marginal comparisons of survival probabilities at multiple fixed time points and facilitates both critical value specification to maintain type I error control and sample size/power determination. Simulations demonstrate that the achieved type I error rate and power of the proposed tests meet targeted levels and are robust to the PH assumption and covariate influence. The proposed tests are illustrated using a clinical trial dataset from the Blood and Marrow Transplant Clinical Trials Network 1101 trial.

Abstract:In clinical trials with survival endpoint, it is common to observe an overlap between two Kaplan-Meier curves of treatment and control groups during the early stage of the trials, indicating a potential delayed treatment effect. Formulas have been derived for the asymptotic power of the log-rank test in the presence of delayed treatment effect and its accompanying sample size calculation. In this paper, we first reformulate the alternative hypothesis with the delayed treatment effect in a rescaled time domain, which can yield a simplified sample size formula for the log-rank test in this context. We further propose an intersection-union test to examine the efficacy of treatment with delayed effect and show it to be more powerful than the log-rank test. Simulation studies are conducted to demonstrate the proposed methods.

Abstract:Proportional hazards are a common assumption when designing confirmatory clinical trials in oncology. With the emergence of immunotherapy and novel targeted therapies, departure from the proportional hazard assumption is not rare in nowadays clinical research. Under non-proportional hazards, the hazard ratio does not have a straightforward clinical interpretation, and the log-rank test is no longer the most powerful statistical test even though it is still valid. Nevertheless, the log-rank test and the hazard ratio are still the primary analysis tools, and traditional approaches such as sample size increase are still proposed to account for the impact of non-proportional hazards. The weighed log-rank test and the test based on the restricted mean survival time (RMST) are receiving a lot of attention as a potential alternative to the log-rank test. We conduct a simulation study comparing the performance and operating characteristics of the log-rank test, the weighted log-rank test and the test based on the RMST, including a treatment effect estimation, under different non-proportional hazards patterns. Results show that, under non-proportional hazards, the hazard ratio and weighted hazard ratio have no straightforward clinical interpretation whereas the RMST ratio can be interpreted regardless of the proportional hazards assumption. In terms of power, the RMST achieves a similar performance when compared to the log-rank test.

Abstract:There is good evidence that endotoxemia, sepsis, and septic shock are associated with the generation and release of reactive oxygen species (ROS) such as superoxide anion (O2−.), indicating that oxygen-derived free radicals play an important role in the pathogenesis of sepsis/shock. Studies on the application of free oxygen radical scavengers to limit the damage to tissues and organs have been recently attempted. A stable piperidine nitroxide of low molecular weight (Tempol) can permeate biological membranes and scavenge O2−. in vitro and in vivo. Thus, we investigated effects of Tempol on the circulatory failure and multiple organ injuries caused by a clinically relevant polymicrobial sepsis model in the rat-cecal ligation and puncture (CLP). CLP not only successfully induced circulatory failure but also substantially increased plasma concentrations of glutamate-oxalate-transferase and glutamate-pyruvate-transferase (indicators of liver injury), creatinine and blood urea nitrogen (indicators of kidney injury), and decreased base excess in arterial blood in the late stage, indicating the development of multiple organ injury in this study. These were also confirmed by a histologic examination showing that the CLP-induced sepsis accompanied increase of polymorphonuclear neutrophil (PMN) infiltration in the lung and sequestration in the liver. Our results demonstrated that Tempol not only ameliorated the deterioration of hemodynamic changes and renal and liver injuries but also attenuated PMN infiltration in the lung and sequestration in the liver (histology). In addition, Tempol improved the survival in CLP-induced septic rats. Moreover, Tempol reduced the plasma NO· and interleukin-1β and organ O2−. levels in CLP-treated rats. In conclusion, Tempol prevented circulatory failure and attenuated organ dysfunction/injury as well as decreased the mortality rate in CLP-treated animals. These beneficial effects of Tempol may be attributed to inhibition of ROS formation (e.g., NO· and O2−.), suggesting antioxidant (e.g., Tempol) is a potential therapeutic agent in the treatment of intraperitoneal septic shock.

Abstract:The log-rank test is most powerful under proportional hazards (PH). In practice, non-PH patterns are often observed in clinical trials, such as in immuno-oncology; therefore, alternative methods are needed to restore the efficiency of statistical testing. Three categories of testing methods were evaluated, including weighted log-rank tests, Kaplan-Meier curve-based tests (including weighted Kaplan-Meier and Restricted Mean Survival Time, RMST), and combination tests (including Breslow test, Lee's combo test, and MaxCombo test). Nine scenarios representing the PH and various non-PH patterns were simulated. The power, type I error, and effect estimates of each method were compared. In general, all tests control type I error well. There is not a single most powerful test across all scenarios. In the absence of prior knowledge regarding the PH or non-PH patterns, the MaxCombo test is relatively robust across patterns. Since the treatment effect changes overtime under non-PH, the overall profile of the treatment effect may not be represented comprehensively based on a single measure. Thus, multiple measures of the treatment effect should be pre-specified as sensitivity analyses to evaluate the totality of the data.
A modified weighted log-rank test for confirmatory trials with a high proportion of treatment switching

Abstract:In confirmatory cancer clinical trials, overall survival (OS) is normally a primary endpoint in the intention-to-treat (ITT) analysis under regulatory standards. After the tumor progresses, it is common that patients allocated to the control group switch to the experimental treatment, or another drug in the same class. Such treatment switching may dilute the relative efficacy of the new drug compared to the control group, leading to lower statistical power. It would be possible to decrease the estimation bias by shortening the follow-up period but this may lead to a loss of information and power. Instead we propose a modified weighted log-rank test (mWLR) that aims at balancing these factors by down-weighting events occurring when many patients have switched treatment. As the weighting should be pre-specified and the impact of treatment switching is unknown, we predict the hazard ratio function and use it to compute the weights of the mWLR. The method may incorporate information from previous trials regarding the potential hazard ratio function over time. We are motivated by the RECORD-1 trial of everolimus against placebo in patients with metastatic renal-cell carcinoma where almost 80\% of the patients in the placebo group received everolimus after disease progression. Extensive simulations show that the new test gives considerably higher efficiency than the standard log-rank test in realistic scenarios.

Background/aimsFor cancers with low incidence, low event rates, and a time-to-event endpoint, a randomized non-inferiority trial designed based on the logrank test can require a large sample size with significantly prolonged enrollment duration, making such a non-inferiority trial not feasible. This article evaluates a design based on a non-inferiority test of proportions, compares its required sample size to the non-inferiority logrank test, assesses whether there are scenarios for which a non-inferiority test of proportions can be more efficient, and provides guidelines in usage of a non-inferiority test of proportions.MethodsThis article describes the sample size calculation for a randomized non-inferiority trial based on a non-inferiority logrank test or a non-inferiority test of proportions. The sample size required by the two design methods are compared for a wide range of scenarios, varying the underlying Weibull survival functions, the non-inferiority margin, and loss to follow-up rate.ResultsOur results showed that there are scenarios for which the non-inferiority test of proportions can have significantly reduced sample size. Specifically, the non-inferiority test of proportions can be considered for cancers with more than 80% long-term survival rate. We provide guidance in choice of this design approach based on parameters of the Weibull survival functions, the non-inferiority margin, and loss to follow-up rate.ConclusionFor cancers with low incidence and low event rates, a non-inferiority trial based on the logrank test is not feasible due to its large required sample size and prolonged enrollment duration. Empirical power comparison of statistical tests in contemporary phase III randomized controlled trials with time-to-event outcomes in oncology

Abstract:Background: More than 95% of recent cancer randomized controlled trials used the log-rank test to detect a treatment difference making it the predominant tool for comparing two survival functions. As with other tests, the log-rank test has both advantages and disadvantages. One advantage is that it offers the highest power against proportional hazards differences, which may be a major reason why alternative methods have rarely been employed in practice. The performance of statistical tests has traditionally been investigated both theoretically and numerically for several patterns of difference between two survival functions. However, to the best of our knowledge, there has been no attempt to compare the performance of various statistical tests using empirical data from past oncology randomized controlled trials. So, it is unknown whether the log-rank test offers a meaningful power advantage over alternative testing methods in contemporary cancer randomized controlled trials. Focusing on recently reported phase III cancer randomized controlled trials, we assessed whether the log-rank test gave meaningfully greater power when compared with five alternative testing methods: generalized Wilcoxon, test based on maximum of test statistics from multiple weighted log-rank tests, difference in t-year event rate, and difference in restricted mean survival time with fixed and adaptive τ. Methods: Using manuscripts from cancer randomized controlled trials recently published in high-tier clinical journals, we reconstructed patient-level data for overall survival (69 trials) and progression-free survival (54 trials). For each trial endpoint, we estimated the empirical power of each test. Empirical power was measured as the proportion of trials for which a test would have identified a significant result (p value < .05). Results: For overall survival, t-year event rate offered the lowest (30.4%) empirical power and restricted mean survival time with fixed τ offered the highest (43.5%). The empirical power of the other types of tests was almost identical (36.2%-37.7%). For progression-free survival, the tests we investigated offered numerically equivalent empirical power (55.6%-61.1%). No single test consistently outperformed any other test. Conclusion: The empirical power assessment with the past cancer randomized controlled trials provided new insights on the performance of statistical tests. Although the log-rank test has been used in almost all trials, our study suggests that the log-rank test is not the only option from an empirical power perspective. Near universal use of the log-rank test is not supported by a meaningful difference in empirical power. Clinical trial investigators could consider alternative methods, beyond the log-rank test, for their primary analysis when designing a cancer randomized controlled trial. Factors other than power (e.g. interpretability of the estimated treatment effect) should garner greater consideration when selecting statistical tests for cancer randomized controlled trials.

Abstract:The difference between two arms in the restricted mean survival time is an alternative to the hazard ratio. It provides a more easily understood measure of the treatment effect of an intervention in a controlled clinical trial with a time to event endpoint. In this article, we consider the distribution of the estimate for small or moderate sample sizes or when the event rate is low and compare the asymptotic relative efficiency (ARE) and relative efficiency for practical sample sizes of the test to the standard logrank test for Weibull parent distributions. We find that for small sample sizes, the distribution can depart markedly from the normal distribution. We propose a method to approximate the correct null distribution using the Cornish–Fisher expansion and provide an example. For Weibull parent distributions, we consider two scenarios with either proportional hazards or nonproportional hazards where the hazard functions and survival distribution curves crossover. The ARE is compared for these two scenarios.

Abstract:The classical approach to analyze time-to-event data, e.g. in clinical trials, is to fit Kaplan-Meier curves yielding the treatment effect as the hazard ratio between treatment groups. Afterwards commonly a log-rank test is performed in order to investigate whether there is a difference in survival, or, depending on additional covariates, a Cox proportional hazard model is used. However, in numerous trials these approaches fail due to the presence of non-proportional hazards, resulting in difficulties of interpreting the hazard ratio and a loss of power. When considering equivalence or non-inferiority trials, the commonly performed log-rank based tests are similarly affected by a violation of this assumption. Here we propose a parametric framework to assess equivalence or non-inferiority for survival data. We derive pointwise confidence bands for both, the hazard ratio and the difference of the survival curves. Further we propose a test procedure addressing non-inferiority and equivalence by directly comparing the survival functions at certain time points or over an entire range of time. We demonstrate the validity of the methods by a clinical trial example and by numerous simulation results.

Abstract:Clinical studies sometimes provide clustered data with censored failure times. A crucial factor of the randomized design that lessens selection bias is the random allocation rule. Given this, the weighted rank tests' p-values for stratified survival clustered sampling based on the random allocation rule are approximated using the double saddle-point approximation technique. For tests of significance and confidence intervals for the treatment effect, this approximation can be utilized. Through simulation experiments, the accuracy of the saddle-point approximation is examined by comparing saddle-point and normal approximations to the exact underlying permutation distribution.

Abstract:Non-proportional hazards (NPH) have been observed recently in many immuno-oncology clinical trials. Weighted log-rank tests (WLRT) with suitably chosen weights can be used to improve the power of detecting the difference of the two survival curves in the presence of NPH. However, it is not easy to choose a proper WLRT in practice when both robustness and efficiency are considered. A versatile maxcombo test was proposed to achieve the balance of robustness and efficiency and has received increasing attentions in both methodology development and application. However, survival trials often warrant interim analyses due to its high cost and long duration. The integration and application of maxcombo tests in interim analyses often require extensive simulation studies. In this paper, we propose a simulation-free approach for group sequential design with maxcombo test in survival trials. The simulation results support that the proposed approaches successfully control both the type I error rate and offer great accuracy and flexibility in estimating sample sizes, at the expense of light computation burden. Notably, our methods display a strong robustness towards various model misspecifications, and have been implemented in an R package for free access online.

Abstract:The one-sample log-rank test is the preferred method for analysing the outcome of single-arm survival trials. It compares the survival distribution of patients with a prefixed reference survival curve that usually represents the expected outcome under standard of care. However, classical one-sample log-rank tests assume that the reference curve is known, ignoring that it is frequently estimated from historical data and therefore susceptible to sampling error. Neglecting the variability of the reference curve can lead to an inflated type I error rate, as shown in a previous paper. Here, we propose a new survival test that allows to account for the sampling error of the reference curve without knowledge of the full underlying historical survival time data. Our new test allows to perform a valid historical comparison of patient survival times when only a historical survival curve rather than the full historic data is available. It thus applies in settings where the two-sample log-rank test is not applicable as method of choice due to non-availability of historic individual patient survival time data. We develop sample size calculation formulas, give an example application and study the performance of the new test in a simulation study.
