Abstract:ABSTRACT Objectives: This study describes the performance of the Multilingual Naming Test (MINT) by Chinese American older adults who are monolingual Chinese speakers. An attempt was also made to identify items that could introduce bias and warrant attention in future investigation. Methods: The MINT was administered to 67 monolingual Chinese older adults as part of the standard dementia evaluation at the Alzheimer’s Disease Research Center (ADRC) at the Icahn School of Medicine at Mount Sinai (ISMMS), New York, USA. A diagnosis of normal cognition ( n = 38), mild cognitive impairment ( n = 12), and dementia ( n = 17) was assigned to all participants at clinical consensus conferences using criterion sheets developed at the ADRC at ISMMS. Results: MINT scores were negatively correlated with age and positively correlated with education, showing sensitivity to demographic factors. One item, butterfly , showed no variations in responses across diagnostic groups. Inclusion of responses from different regions of China changed the answers from “incorrect” to “correct” on 20 items. The last five items, porthole , anvil , mortar , pestle , and axle , yielded a high nonresponse rate, with more than 70% of participants responding with “I don’t know.” Four items, funnel , witch , seesaw , and wig , were not ordered with respect to item difficulty in the Chinese language. Two items, gauge and witch , were identified as culturally biased for the monolingual group. Conclusions: Our study highlights the cultural and linguistic differences that might influence the test performance. Future studies are needed to revise the MINT using more universally recognized items of similar word frequency across different cultural and linguistic groups.

The multilingual naming test in Alzheimer's disease: clues to the origin of naming impairments

Abstract:The current study explored the picture naming performance of patients with Alzheimer’s disease (AD). First, we evaluated the utility of the multilingual naming test (MINT; Gollan et al., 2011), which was designed to assess naming skills in speakers of multiple languages, for detecting naming impairments in monolingual AD and amnestic mild cognitive impairment (MCI). If the MINT were sensitive to linguistic impairment in AD, using it in clinical practice might have advantages over using tests exclusively designed for English monolinguals. We found that the MINT can be used with both monolinguals and bilinguals: A 32-item subset of the MINT is best for distinguishing monolingual patients from controls, while the full MINT is best for assessing degree of bilingualism and language dominance in bilinguals. We then investigated the cognitive mechanisms underlying naming impairment in AD. To this end, we explored which MINT item characteristics best predicted performance differences between monolingual patients and controls. We found that contextual diversity and imageability, but not word frequency (nor words’ number of senses), contributed unique variance to explaining naming impairments in AD. These findings suggest a semantic component to the naming impairment in AD (modulated by names’ semantic richness and network size).

Abstract:OBJECTIVE Dysnomia, or complaints of word-finding difficulties for names, places, and objects is common in patients with Alzheimer's Disease (ad). This common characteristic makes confrontational naming a fundamental component of neuropsychological assessment in aging populations. The Multilingual Naming Test (MINT) detects naming impairments at various stages of cognitive decline. However, our understanding of how performance varies based on education, is sparse. Authors evaluated the sensitivity of the MINT in predicting naming impairments in AA in relation to education levels. METHODS 842 cognitively unimpaired (CU) and 413 mild cognitive impairment (MCI) AA elders were administered the MINT as part of the National Alzheimer's Coordinating Center neuropsychological battery. The MINT consists of 32 items of ascending difficulty that participants must name spontaneously. Semantic and phonemic cues are given in the absence of immediate responses. RESULTS A 5-number MEANS analysis was used to measure differences among CU and MCI stratifying by education level. Overall naming scores were consistently lower in cohorts with lower education across diagnosis. Unsurprisingly CU participants outperformed MCI counterparts. CONCLUSION More sensitive tools are needed to detect cognitive functioning changes in individuals with lower education levels. The MINT offers a novel approach in detecting confrontational naming deficits in those at risk of developing ad, but norms accounting for scholastic attainment are necessary to accurately diagnose minority populations with varying education levels. Further research is required to validate the MINT in participants with diverse backgrounds, low literacy levels and other factors that could affect quality of formative education.

Abstract:The Mini-Mental State (MMS) is a brief structured test of cognitive function. The purpose of this study was to adapt and normalise MMS for the Spanish population. The test was administered to 450 subjects (253 control volunteers, 86 mild memory/cognitive impairment without dementia subjects - CIWD and 111 Alzheimer's Disease patients - AD). A cross-sectional statistical study in a population stratified by age and education was conducted. A more accurate diagnosis is provided by scores that have been adjusted for age and level of education. The recommended cut-off in our study was 24/25 (non-demented above 24). The adaptation and normalisation of MMS provides the Spanish population with a highly valuable screening tool.

Abstract:Background: The Cross-Cultural Neuropsychological Test Battery (CNTB) is a novel test battery specifically designed to reduce the impact of multiculturality in cognitive assessment. Objective: We aimed to validate the CNTB in Spaniards in patients with Alzheimer's disease (AD), including patients at mild cognitive impairment (MCI) and mild dementia stages, and Parkinson's disease with MCI (PD-MCI). Methods: Thirty patients with AD-MCI, 30 with AD-dementia (AD-D), and 30 with PD-MCI were recruited. Each clinical group was compared against a healthy control group (HC) with no differences in sex, age, or years of education. Intergroup comparisons, ROC analysis, and cut-off scores were calculated. Results: AD-MCI scored lower than HC in those subtests associated with episodic memory and verbal fluency. AD-D also showed lower scores in executive functions and visuospatial tests. Effect sizes for all the subtests were large. PD-MCI showed lower performance than HC in memory and executive functions, particularly on error scores, with large effect sizes. Comparing AD-MCI and PD-MCI, AD-MCI had lower memory scores, while PD-MCI showed the worst performance in executive functions. CNTB showed appropriate convergent validity with standardized neuropsychological tests measuring the same cognitive domains. We obtained similar cut-off scores to previous studies performed in other populations. Conclusions: The CNTB showed appropriate diagnostic properties in AD and PD, including those stages with mild cognitive impairment. This supports the utility of the CNTB for the early detection of cognitive impairment in AD and PD.

Effects of arm frequency during synchronous and asynchronous wheelchair propulsion on efficiency.

Abstract:To further understand the possible underlying mechanisms of the low efficiencies in hand rim wheelchair propulsion, this study examined efficiency indices at different arm frequencies during two propulsion modes (synchronous and asynchronous). Fourteen male able-bodied participants performed VO2PEAK tests for both propulsion modes. Subsequently two sub-maximal exercise tests examining synchronous and asynchronous propulsion were completed at an individualised velocity (60% of VO2PEAK). The freely chosen arm frequency (FCF), followed by four counter-balanced trials at 60, 80, 120, and 140% of FCF were performed. Gross, net, and work efficiency were determined. Gross efficiency was significantly lower (p<0.05) at arm frequencies >100%, and participants were more efficient between 60 to 100% FCF. These arm frequencies corresponded to 76+/-22 to 126+/-36 and 70+/-18 to 116+/-30 pushes x min(-1) (synchronous and asynchronous respectively). Trends in VO2, gross and work efficiency suggest that 80% of FCF produced the best economy and efficiency during both propulsion modes (non-significant). Gross and work efficiency at 80% FCF were 6.8+/-0.7% and 13.0+/-4.6% for synchronous and 7.0+/-0.8% and 11.5+/-1.6% for asynchronous respectively. The results suggest that during both modes of propulsion the FCF is not necessarily the most efficient.

Abstract:Background: The Boston Naming Test (BNT) is a commonly used neuropsychological test of confrontation naming that aids in determining the presence and severity of dysnomia. Many short versions of the original 60-item test have been developed and are routinely administered in clinical/research settings. Because of the common need to translate similar measures within and across studies, it is important to evaluate the operating characteristics and agreement of different BNT versions. Methods: We analyzed longitudinal data of research volunteers (n = 681) from the University of Kentucky Alzheimer's Disease Center longitudinal cohort. Conclusions: With the notable exception of the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) 15-item BNT, short forms were internally consistent and highly correlated with the full version; these measures varied by diagnosis and generally improved from normal to mild cognitive impairment (MCI) to dementia. All short forms retained the ability to discriminate between normal subjects and those with dementia. The ability to discriminate between normal and MCI subjects was less strong for the short forms than the full BNT, but they exhibited similar patterns. These results have important implications for researchers designing longitudinal studies, who must consider that the statistical properties of even closely related test forms may be quite different.

Abstract:Validation of Chinese version of Montreal Cognitive Assessment (MOCA) for the elderly population in Eastern China 72 patients with AD, 84 patients with mild cognitive impairment (MCI) and146 cases of the normal population sample of elderly from community in Hangzhou. Each sample were evaluated for the Montreal Cognitive Assessment Scale (MoCA-C), Mini-mental state examination (MMSE), Hasegawa Dementia Scale (HDS), Clinical Dementia Scale (CDR) and other neuropsychological tests .Through by The use of internal consistency, test-retest reliability, content validity, criterion validity, the principal component / factor analysis to evaluate the test results. The mean age and education level for the total sample was 64.4 years and 9.4 years, respectively. No differences were found between groups in terms of education level, or gender, but on average patients with MCI (60.7 years) were significantly younger (p<0.001) than those with AD (68.4 years) or normal cognitive function (67.2 years). Gender was not related to MMSE or MoCA-C scores. The time required of MoCA and MMSE in the three groups were compared differences (tAD=5.77, tMCI=7.16, tNC=14.71, P<0.01). MoCA's internal consistency coefficient (Cronbach's alpha) was 0.867. Test-retest reliability ICC value was 0.912 (P<0.01). MoCA score and MMSE were highly correlated (r = 0.912, P <0.01). Each item with total score correlation were 0.686-0.884. Factor analysis to extract two common factors that can explain the total variance of 71.882%, Factor 1 called executive function/attention factor, Factor 2 called memory factor. Validity of the Verbal Naming Test and Boston Naming Test in a sample of older Veterans

Abstract:ObjectiveThe Verbal Naming Test (VNT) assesses word-finding ability, or naming, through the use of a naming-to-definition paradigm and has been shown to effectively detect naming deficits in older adults. This study evaluated the convergent and discriminant validity of the VNT, assessed how well it detects Mild Cognitive Impairment (MCI), and provided descriptive data stratified by three levels of age (60-69, 70-79, 80-89).MethodThe sample included 151 healthy older Veterans who were found to be cognitively intact and free of neurological and acute psychiatric disorders (mean age = 74.23, SD = 6.67; range: 60-89) and 39 older Veterans with MCI (mean age = 72.97, SD = 8.24; range 60-88). Correlations were calculated between the VNT, Boston Naming Test (BNT), and several other neuropsychological measures. ROC analyses determined how well the VNT detected MCI.ResultsThe VNT correlated with the Rey Auditory Verbal Learning Test and the Logical Memory test. The BNT did not correlate with any measure with the exception of Judgment of Line Orientation (JLO). The Area Under the Curve (AUC) for the detection of MCI was 0.69. Normative data for the VNT are presented and stratified by ages.ConclusionsThis study provides evidence of the convergent and discriminant validity of the Verbal Naming Test in a sample of cognitively normal older adults, as well as its validity in the detection of MCI. It also provides descriptive data on the Verbal Naming Test that can be used in the neuropsychological assessment of older adults.
Is the Montreal cognitive assessment culturally valid in a diverse geriatric primary care setting? Lessons from the Bronx

Abstract:Background Efficacy and validity of the MoCA for cognitive screening in ethnoculturally and linguistically diverse settings is unclear. We sought to examine the utility and discriminative validity of the Spanish and English MoCA versions to identify cognitive impairment among diverse community‐dwelling older adults. Methods Participants aged ≥65 with cognitive concerns attending outpatient primary care in Bronx, NY, were recruited. MoCA and neuropsychological measures were administered in Spanish or English, and a neuropsychologist determined cognitive status (normal with subjective cognitive concerns [SCC], mild cognitive impairment [MCI], and dementia). One‐way ANOVA compared cognitive statuses. ROC analyses identified optimal MoCA cutpoints for discriminating possible cognitive impairment. Results There were 231 participants, with mean age 73, 72% women, 43% Hispanic; 39% Black/African American; 113 (49%) completed testing in English and 118 (51%) in Spanish. Overall MoCA mean was 17.7 (SD = 4.3). Neuropsychological assessment identified 90 as cognitively normal/SCC, average MoCA 19.9 (SD = 4.1), 133 with MCI, average MoCA 16.6 (SD = 3.7), and 8 with dementia, average MoCA 10.6 (SD = 3.1). Mean English MoCA average was 18.6 (SD = 4.1) versus Spanish 16.7 (SD = 4.3). The published cutpoint ≤23 for MCI yielded a high false‐positive rate (79%). ROC analyses identified ≤18.5 as the score to identify MCI or dementia using the English MoCA (65% sensitivity; 77% specificity) and ≤16.5 for the Spanish MoCA (64% sensitivity;73% specificity) in this sample of older adults with cognitive concerns. Conclusions Current MoCA cutpoints were inappropriately high in a culturally/linguistically diverse urban setting, leading to a high false‐positive rate. Lower Spanish and English MoCA cutpoints may improve diagnostic accuracy for identifying cognitive impairment in this group, highlighting the need for the creation and validation of accurate cognitive screeners for ethnoculturally and linguistically diverse older adults.

Abstract:Objective: Modified Mini-Mental State Examination (3MSE) is often used to screen for dementia, but little is known about psychometric validity in American Indians. Methods: We recruited 818 American Indians aged 65–95 for 3MSE examinations in 2010–2013; 403 returned for a repeat examination in 2017–2019. Analyses included standard psychometrics inferences for interpretation, generalizability, and extrapolation: factor analysis; internal consistency-reliability; test-retest score stability; multiple indicator multiple cause structural equation models. Results: This cohort was mean age 73, majority female, mean 12 years education, and majority bilingual. The 4-factor and 2nd-order models fit best, with subfactors for orientation and visuo-construction (OVC), language and executive functioning (LEF), psychomotor and working memory (PMWM), verbal and episodic memory (VEM). Factor structure was supported for both research and clinical interpretation, and factor loadings were moderate to high. Scores were generally consistent over mean 7 years. Younger participants performed better in overall scores, but not in individual factors. Males performed better on OVC and LEF, females better on PMWM. Those with more education performed better on LEF and worse on OVC; the converse was true for bilinguals. All differences were significant, but small. Conclusion: These findings support use of 3MSE for individual interpretation in clinic and research among American Indians, with moderate consistency, stability, reliability over time. Observed extrapolations across age, sex, education, and bilingual groups suggest some important contextual differences may exist.

NeuroBel: Spanish screening test for oral psycholinguistics disabilities in elderly people with mild cognitive impairment and early-stage Alzheimer's disease

Abstract:Introduction: The NeuroBel is a short test that allows for psycholinguistic assessment of basic processes of oral comprehension and language production deterioration in the elderly. The objective is to carry out a pilot study of the initial reference values and cut-off points of this battery using a sample of Spanish elderly adults, with and without cognitive impairment, and detecting performance differences among them. Method: NeuroBel consists of 8 tasks that analyze oral language functioning from the theoretical model proposed by the Psycholinguistic approach. Seventy-five Spanish monolingual adult-elderly participants of both genders. Of those, 25 with Alzheimer's disease (AD) in the initial phase, 25 with mild cognitive impairment (MCI) and 25 participants without cognitive impairment (Controls). All subjects were evaluated using NeuroBel. Results: There are significant differences between the three groups. The participants with AD are significantly worse in the total score of NeuroBel. A discriminant analysis shows that 86.7% of the cases appear correctly classified in the groups originally selected. Likewise, participants with MCI obtained results that are statistically significantly worse than the control group. NeuroBel shows a high correlation with the MMSE (.89) and Sensitivity (.96) in the determination of AD and cognitive deterioration (AD + MCI vs. Controls). The area under the ROC curve is .97 in the contrast of AD vs. Controls and .98 in the determination of cognitive deterioration (AD + MCI vs. Controls). The canonical discriminant functions and the precision cut-offs from the ROC analyses are also shown in the results. Conclusions: NeuroBel is shown as a "very good" test in the detection of cognitive-linguistic impairment in elderly-adults.
Validation of the Chinese Version of the Memory Binding Test for Distinguishing Amnestic Mild Cognitive Impairment from Cognitively Normal Elderly Individuals

Abstract:Background: Episodic memory starts to decline very early in the development of Alzheimer's disease (AD). Subtle impairments in memory binding may be detected in mild cognitive impairment (MCI). This study aims to examine the psychometric properties of the Chinese version of the memory binding test (MBT). Methods: One hundred and sixty-four subjects (26 individuals with AD, 67 individuals with amnestic MCI (aMCI), 30 individuals with subjective cognitive impairment (SCI), and 41 cognitively normal elderly individuals (NC)) participated in the study. Twenty-two subjects repeated the assessment of the MBT within 6 weeks (+/- 2 weeks). Pearson correlation was used to calculate the convergent validity. The test--retest reliability was determined by the calculation of the intraclass correlation coefficient (ICC). Discriminative validity was calculated to evaluate the receiver-operating characteristic curves. The optimal index was chosen by comparing the area under the curve for specificity and sensitivity >= 0.80. The optimal cutoff score of the index was chosen to maximize the sum of sensitivity and specificity. Results: The absolute value of the convergent validity of the direct indexes of MBT ranged from 0.443 to 0.684. The ICC for each of direct indexes was 0.887-0.958. Total delayed paired recall (TDPR) was the optimal index for discriminating aMCI from NC. The cutoff score for TDPR was <= 25 to distinguish aMCI from NC (sensitivity = 0.896, specificity = 0.707). Conclusion: The Chinese version of MBT is a valid and reliable instrument to detect MCI.
Diagnostic Value of Time-Constrained Naming Test in Mild Cognitive Impairment

Abstract:Background: Naming difficulties have recently garnered more interest in elderly individuals with mild cognitive impairment (MCI). We anticipate that naming tests with the consideration of response time can provide more informative and distinct neuropsychological profiles of individuals with MCI. Methods: Naming tests were administered to 76 elderly individuals with MCI and 149 healthy elderly (HE). We analyzed the impact of MCI on naming performance and occurrence of “delayed” response. We also validated the predictive power of naming tests with a time-constrained scoring system. Results: MCI participants performed poorer on the noun naming test than HE participants (p = 0.014). MCI was significantly associated with the occurrence of “delayed” response on the noun (odds ratio [OR] = 3.57; 95% confidence interval [CI] = 1.78-7.17) and verb naming tests (OR = 4.66; 95% CI = 2.07-10.46). The time-constrained naming scores were significantly better able to distinguish the MCI from the HE group than the conventional spontaneous naming score on both the noun (p < 0.001) and verb (p = 0.002) naming tests. Conclusions: Our findings broaden the knowledge related to the naming ability in individuals with MCI, with respect to the response time. We also confirmed the validity of the naming tests by applying the “delayed” responses as supplementary assessments in the diagnosis of MCI.

Abstract:Introduction: Digital solutions for cognitive assessment are currently not only widely used in experimental contexts but can also be useful in clinical practice for efficient screening and longitudinal follow-up. The "Guttmann Cognitest" ® , which includes seven computerized tasks designed to assess main cognitive functions, revealed in a previous validation study to be a potential useful tool to assess cognitive functioning in healthy middle-aged adults. Method: Here, we present results from a validation in two different populations: one consisting of older adults, and the other comprising young and middle-aged individuals, some of them affected by acquired brain injury. To perform a convergent validity test, older adults were also administered with the MOCA, while young and middle-aged individuals were administered with a short neuropsychological assessment including gold-standard neuropsychological tests. We also conducted sensitivity and specificity analysis to establish the utility of this instrument in identifying potential cognitive dysfunctions in the two groups. Results: Results demonstrated strong convergent validity as well as good specificity and sensitivity characteristics. Discussion: This tool is a valid and useful instrument to assess cognitive functioning and detecting potential cases of cognitive dysfunctions in older adults and clinical populations.

Boston Naming Test in Chinese Elderly, Patient with Mild Cognitive Impairment and Alzheimer's Dementia

Abstract:Objective: To investigate applicability for Boston naming test (30 items) (BNT) in Chinese elderly and identify effect for mild cognitive impairment (MCI) and Alzheimer's dementia(AD) using BNT. Methods:100 normal elderly, 38 amnesic MCI, 34 mild AD and 38 moderate AD were evaluated by neuropsychological tests (include BNT, mini mental state examination and auditory verbal memory test, etc). MMSE total score of 4 groups were 28.4±1.5, 26.1±2.6, 20.7±1.7, and 15.6±3.3, respectively. Results: Age, sex, level of education were found to be significant factors affecting overall scores of spontaneous naming in normal elderly group. Spontaneous naming score for participants of elementary, high school and college groups were 22.2±3.3, 25.5±2.5 and 26.3±1.8, respectively. Scoring of male participants higher than that of females. Spontaneous naming score of 4 groups were 24.9±3.0, 20.9±3.6, 18.7±4.0 and 15.7±4.2, respectively. As cut-off ≤22 score of spontaneous naming of BNT, the sensitivities for MCI, mild AD and moderate AD were 61%, 79% and 95% respectively; the specificities were all around 81%. Selective impairment of unfamiliar items occurred MCI and mild AD and hold of familiar item across diagnostic groups. Semantic cue naming and recognition ability by BNT showed there was progressive damage in AD patients, but less than that of spontaneous naming of BNT. Conclusion: The ability of naming is influenced by age, gender and educational level. Patients with MCI or mild AD have impairment in naming.

Abstract:Assessment, Ahead of Print. Experimental cognitive tests are designed to measure particular cognitive domains, although evidence supporting test validity is often limited. The Consortium for Neuropsychiatric Phenomics test battery administered 23 experimental and traditional neuropsychological tests to a large sample of community volunteers (n = 1,059) and patients with psychiatric diagnoses (n = 137), providing a unique opportunity to examine convergent validity with factor analysis. Traditional tests included subtests from the Wechsler and Delis–Kaplan batteries, while experimental tests included the Attention Networks Test, Balloon Analogue Risk Task, Delay Discounting Task, Remember–Know, Reversal Learning Task, Scene Recognition, Spatial and Verbal Capacity and Manipulation Tasks, Stop-Signal Task, and Task Switching. Several experimental cognitive measures were insufficiently related to other tests and were excluded from factor analyses. In the remaining 18 tests, exploratory factor analysis and subsequent multigroup confirmatory factor analysis supported a three-factor structure broadly corresponding to domains of verbal/working memory, inhibitory control, and memory. In sum, several experimental measures of inhibitory control had weak relationships with all other tests, while the convergent validity of most tests of working memory and memory was supported.

Subclinical naming errors in mild cognitive impairment

Abstract:Mild cognitive impairment (MCI) is the transitional stage between normal aging and Alzheimer's disease (AD). Impairments in semantic memory have been demonstrated to be a critical factor in early AD. The Boston Naming Test (BNT) is a straightforward method of examining semantic or visuo-perceptual processing and therefore represents a potential diagnostic tool. The objective of this study was to examine naming ability and identify error types in patients with amnestic mild cognitive impairment (aMCI). Methods: Twenty aMCI patients, twenty AD patients and twenty-one normal controls, matched by age, sex and education level were evaluated. As part of a further neuropsychological evaluation, all subjects performed the BNT. A comprehensive classification of error types was devised in order to compare performance and ascertain semantic or perceptual origin of errors. Results: AD patients obtained significantly lower total scores on the BNT than aMCI patients and controls. aMCI patients did not obtain significant differences in total scores, but showed significantly higher semantic errors compared to controls. Conclusion: This study reveals that semantic processing is impaired during confrontation naming in aMCI.
