Abstract:Glioma is the most prevalentand dangerous type of brain tumor which can be life-threatening when its grade is high. The early detection of these tumors can improve and save the life of the patients. The automatic segmentation of brain tumor from magnetic resonance imaging (MRI) plays a vital role in treatment planning and timely diagnosis. Automatic segmentation is a challenging task due to the massive amount of information provided by MRI and the variation in the location and size of the tumor. Therefore, a reliable and authentic method to segment the tumorous region from healthy tissues accurately is an open challenge in the field of deep learning-based medical image analysis. This research paper presents an end-to-end framework for automatic 3D Brain Tumor Segmentation (BTS). The proposed model is a hybrid of the deep residual network and U-Net model (dResU-Net). The residual network is used as an encoder in the proposed architecture with the decoder of the U-Net model to handle the issue of vanishing gradient. The proposed model is designed to take advantage from low-level and high-level features simultaneously for making the prediction. In addition, shortcut connections are employed between residual network to preserve low-level features at each level. Furthermore, skip connections between residual and convolutional blocks in the proposed architecture are used to accelerate the training process. The proposed architecture achieved promising results with the average dice score for the tumor core (TC), whole tumor (WT), and enhancing tumor (ET) on the BraTS 2020 dataset of 0.8357, 0.8660, and 0.8004, respectively. To demonstrate the robustness of the proposed model in real-world clinical settings, validation of the trained model on an external cohort is performed on randomly selected 50 patients of the BraTS 2021 benchmark dataset. The achieved dice scores on the external cohort are 0.8400, 0.8601, and 0.8221 for TC, WT, and ET, respectively. The comparison of results of the proposed approach with the state-of-the-art techniques indicates that dResU-Net can significantly improve the segmentation performance of brain tumor sub-regions.

Abstract:Accurate segmentation of different sub-regions of gliomas such as peritumoral edema, necrotic core, enhancing, and non-enhancing tumor core from multimodal MRI scans has important clinical relevance in diagnosis, prognosis and treatment of brain tumors. However, due to the highly heterogeneous appearance and shape of these tumors, segmentation of the sub-regions is challenging. Recent developments using deep learning models has proved its effectiveness in various semantic and medical image segmentation tasks, many of which are based on the U-Net network structure with symmetric encoding and decoding paths for end-to-end segmentation due to its high efficiency and good performance. In brain tumor segmentation, the 3D nature of multimodal MRI poses challenges such as memory and computation limitations and class imbalance when directly adopting the U-Net structure. In this study we aim to develop a deep learning model using a 3D U-Net with adaptations in the training and testing strategies, network structures, and model parameters for brain tumor segmentation. Furthermore, instead of picking one best model, an ensemble of multiple models trained with different hyper-parameters are used to reduce random errors from each model and yield improved performance. Preliminary results demonstrate the effectiveness of this method and achieved the 9th place in the very competitive 2018 Multimodal Brain Tumor Segmentation (BraTS) challenge. In addition, to emphasize the clinical value of the developed segmentation method, a linear model based on the radiomics features extracted from segmentation and other clinical features are developed to predict patient overall survival. Evaluation of these innovations shows high prediction accuracy in both low-grade glioma and glioblastoma patients, which achieved the 1st place in the 2018 BraTS challenge.

Abstract:Tumor segmentation from multi-modal brain MRI images is a challenging task due to the limited samples, high variance in shapes and uneven distribution of tumor morphology. The performance of automated medical image segmentation has been significant improvement by the recent advances in deep learning. However, the model predictions have not yet reached the desired level for clinical use in terms of accuracy and generalizability. In order to address the distinct problems presented in Challenges 1, 2, and 3 of BraTS 2023, we have constructed an optimization framework based on a 3D U-Net model for brain tumor segmentation. This framework incorporates a range of techniques, including various pre-processing and post-processing techniques, and transfer learning. On the validation datasets, this multi-modality brain tumor segmentation framework achieves an average lesion-wise Dice score of 0.79, 0.72, 0.74 on Challenges 1, 2, 3 respectively.

Abstract:Brain tumor segmentation from Magnetic Resonance Images (MRI) is considered a big challenge due to the complexity of brain tumor tissues, and segmenting these tissues from the healthy tissues is an even more tedious challenge when manual segmentation is undertaken by radiologists. In this paper, we have presented an experimental approach to emphasize the impact and effectiveness of deep learning elements like optimizers and loss functions towards a deep learning optimal solution for brain tumor segmentation. We evaluated our performance results on the most popular brain tumor datasets (MICCAI BraTS 2020 and RSNA-ASNR-MICCAI BraTS 2021). Furthermore, a new Bridged U-Net-ASPP-EVO was introduced that exploits Atrous Spatial Pyramid Pooling to enhance capturing multi-scale information to help in segmenting different tumor sizes, Evolving Normalization layers, squeeze and excitation residual blocks, and the max-average pooling for down sampling. Two variants of this architecture were constructed (Bridged U-Net_ASPP_EVO v1 and Bridged U-Net_ASPP_EVO v2). The best results were achieved using these two models when compared with other state-of-the-art models; we have achieved average segmentation dice scores of 0.84, 0.85, and 0.91 from variant1, and 0.83, 0.86, and 0.92 from v2 for the Enhanced Tumor (ET), Tumor Core (TC), and Whole Tumor (WT) tumor sub-regions, respectively, in the BraTS 2021validation dataset.

Abstract:Brain tumor image segmentation plays a significant auxiliary role in clinical diagnosis. Recently, deep learning has been introduced into multimodal segmentation tasks, which construct various Convolutional Neural Network (CNN) structures to achieve excellent performance. However, most CNN-based segmentation methods have poor capability for global feature extraction. Transformer is good at modeling long-distance dependencies, but it can cause local information loss and usually has a high computational complexity. In addition, it is difficult to fully exploit the brain tumor features of different modalities. To address these issues, in this paper, we propose a region–attention fusion (RAF) network that combines a dual-branch vision Transformer (DVT), called RFTNet. In RFTNet, the DVT is exploited to capture the delicate local information and global semantics separately by two branches. Meanwhile, a novel RAF is employed to effectively fuse the images of the different modalities. Finally, we design a new hybrid loss function, called region-mixed loss function (RML) to calculate the importance of each pixel and solve the problem of class imbalance. The experiments on BrasTS2018 and BraTS2020 datasets show that our method obtains a higher segmentation accuracy than other models. Furthermore, ablation experiments prove the effectiveness of each key component in RFTNet.

A deep learning model integrating FCNNs and CRFs for brain tumor segmentation

Abstract:Accurate and reliable brain tumor segmentation is a critical component in cancer diagnosis, treatment planning, and treatment outcome evaluation. Build upon successful deep learning techniques, a novel brain tumor segmentation method is developed by integrating fully convolutional neural networks (FCNNs) and Conditional Random Fields (CRFs) in a unified framework to obtain segmentation results with appearance and spatial consistency. We train a deep learning based segmentation model using 2D image patches and image slices in following steps: 1) training FCNNs using image patches; 2) training CRFs as Recurrent Neural Networks (CRF-RNN) using image slices with parameters of FCNNs fixed; and 3) fine-tuning the FCNNs and the CRF-RNN using image slices. Particularly, we train 3 segmentation models using 2D image patches and slices obtained in axial, coronal and sagittal views respectively, and combine them to segment brain tumors using a voting based fusion strategy. Our method could segment brain images slice-by-slice, much faster than those based on image patches. We have evaluated our method based on imaging data provided by the Multimodal Brain Tumor Image Segmentation Challenge (BRATS) 2013, BRATS 2015 and BRATS 2016. The experimental results have demonstrated that our method could build a segmentation model with Flair, T1c, and T2 scans and achieve competitive performance as those built with Flair, T1, T1c, and T2 scans.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and Objective</h3><p>Manual brain tumor segmentation by radiologists is time consuming and subjective. Therefore, fully automatic segmentation of different brain tumor subregions is essential to the treatment of patients. In this paper, we propose a neural network for automatically segmenting the enhancing tumor (ET), whole tumor (WT), and tumor core (TC) brain tumor subregions.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The network is based on a U-Net with encoding and decoding structure, a residual module, and a spatial dilated feature pyramid (DFP) module, namely, DFP-ResUNet. First, we propose using a spatial DFP module composed of multiple parallel dilated convolution layers to extract the multiscale image features. This spatial DFP structure improves the ability of the neural network to extract and utilize the multiscale image features. Then, we use the residual module to deepen the network structure. Further, we propose using a multiclass Dice loss function to suppress the impact of class imbalance on brain tumor segmentation. We carried out a large number of ablation experiments to verify the feasibility and superiority of our approach using the Multimodal Brain Tumor Segmentation (BraTS) challenge dataset.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The mean Dice score of different subregions was ET 0.8431, WT 0.897 and TC 0.9068 using the proposed method on the BraTS 2018 challenge validation set and 0.7985, 0.90281, 0.8453 on the BraTS 2019 challenge, respectively. Further, we got a high Sensitivity and Specificity and low Hausdorff distance.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Through the analysis of the experimental results, it can be seen that the proposed approach DFP-ResUNet has a great potential in segmenting different subregions of brain tumors and can be applied in clinical practice.</p>

Ensemble Outperforms Single Models in Brain Tumor Segmentation

Abstract:Brain Tumor Segmentation (BraTS) and Federated Tumor Segmentation (FeTS) are open and popular challenges, for which countless medical image segmentation models have been proposed. Based on the platform that BraTS challenge 2021 provided for researchers, we implemented a battery of cutting-edge deep neural networks, such as nnU-Net, UNet++, CoTr, HRNet, and Swin-Unet to compare performances amongst distinct models directly. To improve segmentation accuracy, we first tried several modification techniques (e.g., data augmentation, region-based training, batch-dice loss function, etc.). Next, the outputs from the five best models were averaged using a final ensemble model, of which four models in the committee were organized in different architectures. As a result, the strengths of every single model were amplified by aggregation. Our model took one of the best performing places in the BraTS 2021 competition amongst over 1200 excellent researchers from all over the world, and robustly produced outstanding segmentation results across different unseen datasets from various institutions in the FeTS 2022 Challenge, which achieved Dice score of 0.9256, 0.8774, 0.8576 and Hausdorff Distances (95%) of 4.36, 14.80, 14.49 for whole tumor, tumor core, and enhancing tumor respectively. Since our model is not successfully evaluated on the test data due to a bug, all the scores stated in this manuscript are based on the validation dataset.
Automated Ensemble-Based Segmentation of Adult Brain Tumors: A Novel Approach Using the BraTS AFRICA Challenge Data

Abstract:Brain tumors, particularly glioblastoma, continue to challenge medical diagnostics and treatments globally. This paper explores the application of deep learning to multi-modality magnetic resonance imaging (MRI) data for enhanced brain tumor segmentation precision in the Sub-Saharan Africa patient population. We introduce an ensemble method that comprises eleven unique variations based on three core architectures: UNet3D, ONet3D, SphereNet3D and modified loss functions. The study emphasizes the need for both age- and population-based segmentation models, to fully account for the complexities in the brain. Our findings reveal that the ensemble approach, combining different architectures, outperforms single models, leading to improved evaluation metrics. Specifically, the results exhibit Dice scores of 0.82, 0.82, and 0.87 for enhancing tumor, tumor core, and whole tumor labels respectively. These results underline the potential of tailored deep learning techniques in precisely segmenting brain tumors and lay groundwork for future work to fine-tune models and assess performance across different brain regions.

Abstract:Brain tumors are highly hazardous, and precise automated segmentation of brain tumor subregions has great importance and research significance on the diagnosis and treatment of diseases. Rapid advances in deep learning make accurate and efficient automatic segmentation more possible, but there are challenges. In this paper, an efficient 3D segmentation model (DPAFNet) based on dual-path (DP) module and multi-scale attention fusion (MAF) module is proposed. In DPAFNet, the dual path convolution is applied to broaden the network scale and residual connection is introduced to avoid network degradation. An attention fusion module is proposed to aggregate channel level global and local information, in which feature maps of different scales are fused to obtain features that are enriched in semantic information. This makes the object information of small tumors get full attention. Furthermore, the 3D iterative dilated convolution merging (IDCM) module expands the receptive field and improves the ability of context awareness. Ablation experiments verify the optimal combination of dilation rate for the dilated convolution merging module and demonstrate the enhancement of segmentation accuracy due to the post-processing method. Comparative experiments of this study on BraTS2018, BraTS2019 and BraTS2020 are promising and provide a promising precision and Dice score compared to related work. The proposed DPAFNet achieves Dice score of 79.5%, 90.0% and 83.9% in the enhancing tumor, whole tumor and tumor core on BraTS2018, respectively. On BraTS2019, it achieves Dice score of 78.2%, 89.0% and 81.2% in the enhancing tumor, whole tumor and tumor core, respectively.

Abstract:After introduction of computerized tomography (CT), we experienced 22 patients with traumatic extradural and intracerebral combined hematomas, of whom 15 underwent sequential CT scans. In 14 of the 22 patients or 13 of the 15 patients whose initial CT scans were performed early, within 6 hours after injury, intracerebral hematomas developed more slowly than extradural hematomas. In ten of the 13 patients, development of intracerebral hematomas was demonstrated only after removal of extradural hematomas, and in four patients acute brain swelling was observed during surgery. Therefore it is emphasized that the incidence of post-surgical intracerebral hematoma with extradural hemorrhages is high and that acute brain swelling during surgery for extradural hematomas is largely caused by the delayed intracerebral hematomas.
Clinical capability of modern brain tumor segmentation models

Abstract:Purpose: State-of-the-art automated segmentation methods achieve exceptionally high performance on the Brain Tumor Segmentation (BraTS) challenge, a dataset of uniformly processed and standardized magnetic resonance generated images (MRIs) of gliomas. However, a reasonable concern is that these models may not fare well on clinical MRIs that do not belong to the specially curated BraTS dataset. Research using the previous generation of deep learning models indicates significant performance loss on cross-institutional predictions. Here, we evaluate the cross-institutional applicability and generalzsability of state-of-the-art deep learning models on new clinical data. Methods: We train a state-of-the-art 3D U-Net model on the conventional BraTS dataset comprising low- and high-grade gliomas. We then evaluate the performance of this model for automatic tumor segmentation of brain tumors on in-house clinical data. This dataset contains MRIs of different tumor types, resolutions, and standardization than those found in the BraTS dataset. Ground truth segmentations to validate the automated segmentation for in-house clinical data were obtained from expert radiation oncologists. Results: We report average Dice scores of 0.764, 0.648, and 0.61 for the whole tumor, tumor core, and enhancing tumor, respectively, in the clinical MRIs. These means are higher than numbers reported previously on same institution and cross-institution datasets of different origin using different methods. There is no statistically significant difference when comparing the dice scores to the inter-annotation variability between two expert clinical radiation oncologists. Although performance on the clinical data is lower than on the BraTS data, these numbers indicate that models trained on the BraTS dataset have impressive segmentation performance on previously unseen images obtained at a separate clinical institution. These images differ in the imaging resolutions, standardization pipelines, and tumor types from the BraTS data. Conclusions: State-of-the-art deep learning models demonstrate promising performance on cross-institutional predictions. They considerably improve on previous models and can transfer knowledge to new types of brain tumors without additional modeling.

Abstract:Abstract Accurate segmentation of brain tumors has a vital impact on clinical diagnosis and treatment, and good segmentation results are helpful for the treatment of this disease, which is a serious threat to human health. High‐precision segmentation of brain tumors remains a challenging task due to their diverse shapes, sizes, locations, and complex boundaries. Considering the special structure of medical brain tumor images, many researchers have proposed a brain tumor segmentation (BraTS) network based on 3D U‐Net. However, there are also problems such as insufficient receptive fields and excessive computing costs. In this paper, we propose an efficient BraTS model based on group normalization (GN) and 3D U‐Net (3D‐EffUNet). First, according to the characteristics of brain tumor images, the medical image of the whole case is input into the model, and 3D convolution layers are used to extract features and filter irrelevant information. Then, using 3D U‐Net as the main framework, an efficient convolutional module is designed for more precise processing of brain tumor features. Moreover, an efficient convolution module based on GN and an attention mechanism is introduced to reduce the complexity of the network without affecting the segmentation performance and to increase the awareness of voxels between adjacent dimensions and the local space. Finally, the decoder was used to reconstruct high‐precision BraTS information. The model is trained and tested on the BraTS2021 dataset, and the experimental results show that it can maintain good segmentation performance and greatly reduce the calculation cost.

SARFNet: Selective Layer and Axial Receptive Field Network for Multimodal Brain Tumor Segmentation

Abstract:Efficient magnetic resonance imaging (MRI) segmentation, which is helpful for treatment planning, is essential for identifying brain tumors from detailed images. In recent years, various convolutional neural network (CNN) structures have been introduced for brain tumor segmentation tasks and have performed well. However, the downsampling blocks of most existing methods are typically used only for processing the variation in image sizes and lack sufficient capacity for further extraction features. We, therefore, propose SARFNet, a method based on UNet architecture, which consists of the proposed SLiRF module and advanced AAM module. The SLiRF downsampling module can extract feature information and prevent the loss of important information while reducing the image size. The AAM block, incorporated into the bottleneck layer, captures more contextual information. The Channel Attention Module (CAM) is introduced into skip connections to enhance the connections between channel features to improve accuracy and produce better feature expression. Ultimately, deep supervision is utilized in the decoder layer to avoid vanishing gradients and generate better feature representations. Many experiments were performed to validate the effectiveness of our model on the BraTS2018 dataset. SARFNet achieved Dice coefficient scores of 90.40, 85.54, and 82.15 for the whole tumor (WT), tumor core (TC), and enhancing tumor (ET), respectively. The results show that the proposed model achieves state-of-the-art performance compared with twelve or more benchmarks.
RFNet: Region-aware Fusion Network for Incomplete Multi-modal Brain Tumor Segmentation

Abstract:Most existing brain tumor segmentation methods usually exploit multi-modal magnetic resonance imaging (MRI) images to achieve high segmentation performance. However, the problem of missing certain modality images often happens in clinical practice, thus leading to severe segmentation performance degradation. In this work, we propose a Region-aware Fusion Network (RFNet) that is able to exploit different combinations of multi-modal data adaptively and effectively for tumor segmentation. Considering different modalities are sensitive to different brain tumor regions, we design a Region-aware Fusion Module (RFM) in RFNet to conduct modal feature fusion from available image modalities according to disparate regions. Benefiting from RFM, RFNet can adaptively segment tumor regions from an incomplete set of multi-modal images by effectively aggregating modal features. Furthermore, we also develop a segmentation-based regularizer to prevent RFNet from the insufficient and unbalanced training caused by the incomplete multi-modal data. Specifically, apart from obtaining segmentation results from fused modal features, we also segment each image modality individually from the corresponding encoded features. In this manner, each modal encoder is forced to learn discriminative features, thus improving the representation ability of the fused features. Remarkably, extensive experiments on BRATS2020, BRATS2018 and BRATS2015 datasets demonstrate that our RFNet outperforms the state-of-the-art significantly.
ResUNet + + : a comprehensive improved UNet + + framework for volumetric semantic segmentation of brain tumor MR images

Abstract:Predictive models in radiology can now be constructed with remarkable accuracy using an amalgamation of radiomics and artificial intelligence. To effectively prepare for surgical procedures and assess the progression of the tumor, accurate segmentation of gliomas is essential. The current study aims to address a segmentation of the whole tumor (WT), tumor core (TC), and enhancing tumor (ET), three partially overlapping regions of interest within the glioma with two variants, high-graded glioma (HGG) and low-graded glioma (LGG), made available through the BraTS 2019, 2020, and 2021 challenges. The traditional approach has been bypassed by focusing only on the network architecture, but rather the proposed research work is also concentrating on data pre-processing, augmentation, training, and testing strategies to improve the performance of the automatic brain tumor segmentation. UNet and its variants have recently been shown to be effective in automatically segmenting brain tumors from volumetric multi-modal magnetic resonance (MR) images. Motivated from the literature, an improved UNet + + framework (ResUNet + +) is proposed to segment multi-modal volumetric MR images of brain tumor. The ResUNet + + is a 3D (three-dimensional) encoder-decoder model where the encoder path is replaced with the pre-trained backbone of the ResNet50 model. Moreover, the standard convolutional blocks of the traditional UNet architecture are substituted with the 3D dense convolutional blocks, and in the decoder phase, convolutional layers are replaced by the convolutional transpose layers (ConvTranspose), outperforming the existing models in terms of segmenting the WT, TC, and ET in both HGG and LGG. The performance of the ResUNet + + framework is evaluated using five different performance parameters, and when compared with the state-of-the-art models, the results demonstrate the effectiveness of the framework.

Abstract:Glioma is a kind of brain tumor that can arise at a distinct location along with dissimilar appearance and size. The high-grade glioma (HGG) is a serious kind of cancer when compare to low-graded glioma (LGG). The manual diagnosis process of these tumors is tiring and time consuming. Therefore, in clinical practices, MRI is useful to assess gliomas as it provides essential information of tumor regions. In this manuscript, an active deep learning-based feature selection approach is suggested to segment and recognize brain tumors. Contrast enhancement is made in the primary step and supplied to SbDL for saliency map construction, which later converts into binarized form by applying simple thresholding. In the classification phase, the Inception V3 pre-trained CNN model is employed for deep feature extraction. These features are simply concatenated along with dominant rotated LBP (DRLBP) for better texture analysis. Later, the concatenated vector is optimized through particle swarm optimization (PSO), so as to classify using softmax classifier. The experiments are conducted in two phases. At first, the segmentation approach SbDL is validated on BRATS2017 and BRATS2018 datasets. The achieved dice score for the BRAST2017 dataset is 83.73% for core tumor, 93.7% for the whole tumor and 79.94% for enhanced tumor. For BRATS2018 dataset, dice score obtained is 88.34% (core), 91.2% (whole) and 81.84% (enhanced). At the second, the classification strategy is applied on BRATS2013, 2014, 2017 and 2018 with an average accuracy of more than 92%. The overall results show that the presented method outperforms for both segmentation and classification of brain tumors.

Abstract:Segmenting brain tumors in multi-parametric magnetic resonance imaging enables performing quantitative analysis in support of clinical trials and personalized patient care. This analysis provides the potential to impact clinical decision-making processes, including diagnosis and prognosis. In 2023, the well-established Brain Tumor Segmentation (BraTS) challenge presented a substantial expansion with eight tasks and 4,500 brain tumor cases. In this paper, we present a deep learning-based ensemble strategy that is evaluated for newly included tumor cases in three tasks: pediatric brain tumors (PED), intracranial meningioma (MEN), and brain metastases (MET). In particular, we ensemble outputs from state-of-the-art nnU-Net and Swin UNETR models on a region-wise basis. Furthermore, we implemented a targeted post-processing strategy based on a cross-validated threshold search to improve the segmentation results for tumor sub-regions. The evaluation of our proposed method on unseen test cases for the three tasks resulted in lesion-wise Dice scores for PED: 0.653, 0.809, 0.826; MEN: 0.876, 0.867, 0.849; and MET: 0.555, 0.6, 0.58; for the enhancing tumor, tumor core, and whole tumor, respectively. Our method was ranked first for PED, third for MEN, and fourth for MET, respectively.
