Abstract:The origins of castration can be traced back to the times of Classical Mythology. THe reasons for this kind of operation were extremely varied.Up into the 20th century it was performed as punishment and revenge upon conquered enemies, on religious grounds and for acquiring slaves and harem guards. THere are very few diseases to date giving rise to a medical indication for castration. According to the excessively strict interpretation of the words of St. Paul that "women should remain silent in Church", women were not permitted to participate in signing for several voices in the church. The high-pitched voices of young boys were therefore adopted, whose skill no longer met the continuously rising demands placed upon singers. The vocal performance of men signing in the falsetto registers ("falsettos") progressively failed to suffice in church singing and even more so in the opera, which started to become established as a new art form towards the end of the 16th century. Young boys' voices and flasettos were therefore gradually replaced by singing castratos. The "heyday" of this type of singing was in the 18th century and one of its best known representatives was Carlo Broschi, known as Farinelli. THe high pitches and aria di bravura corresponded to the taste of the audiences. In the 19th century the demand for castratos declined. One of the last was Alessandro Moreschi (1858-1922), who sang for over 30 years in the Sistine Chapel, in later years mainly conducting. According to present-day knowledge, he is the only castrato of whom several recordings were made between 1902 and 1904. They were published on a record, now available as a CD.

Castration is responsible pour morphologic changes not only of the larynx, but also of the skeleton and visceral tissues. These anatomic particularities mixed with a stupefying vocal technic, will create one of the most extraordinary vocal phenomenon of the history of singing, but the price to pay was so high.

Abstract:Trotula de Ruggiero is supposed to be one of the first female physician of the history, or at least the first who practiced, taught, and wrote medical texts inside the illustrious medieval Medical School of Salerno around the XI-XII centuries. Here we retrace the steps of her fascinating history from historical cues to legendary anecdotes, through the analysis of the medical texts which were ascribed to her in the Middle Ages and that were very popular around Europe for several centuries, prevalently dealing with all the aspects of women's medical problems, with a focus on dermatology, cosmetic science, and obstetrics/gynecology.

Abstract:Introduction: The predominant alterations in voice of patients with multiple sclerosis (MS) are phonatory instability, vocal asthenia and roughness, shortness of breath, hypophonia, and hypernasality. However, research on alterations of acoustic parameters has few studies and disparate results. The objective of this study was to investigate voice disturbances in patients with MS, both with objective measures (analysis of biomechanical) and subjective measures (scales and questionnaires). Methods: This is an experimental study with a total of 20 participants with MS. Voice samples were collected, and biomechanical correlates were analyzed through the Clinical Voice Systems program, Online Lab App. The VHI-30 (Voice Handicap Index) questionnaire, the GRBAS (grade, roughness, breathiness, asthenia, strain) scale, and the Hospital Anxiety and Depression Scale were used as subjective measures. Results: Ninety-five percentages of participants feel and describe dysphonic difficulties. Self-perception of vocal disability correlated with auditory vocal perceptual analysis in the sample of women. Conclusion: The biomechanical parameters showed alterations in the strength of the glottic closure, the efficiency index, and the structural imbalance index.

Abstract:We present the case of a 70-year-old woman with poikiloderma vasculare atrophicans (PVA), which demonstrated a benign course over 35 years, raising questions about its classification as early-stage mycosis fungoides (MF) or as a distinct entity. Despite receiving no treatment, the patient’s condition remained stable, with characteristic erythematous papules observed on clinical examination. Histopathological and immunohistochemical analyses supported the diagnosis, emphasizing the importance of longitudinal studies and patient preferences in treatment decisions for PVA.

Abstract:The purpose of the article is to trail regularities of vocal evolution which has formed the transition of stage operatic development from Rossini to Verdi and which has predetermined the performing tradition of Lucia‘s role in Donizetti’s opera “Lucia Lammermoor “(1835) by light coloratura soprano in the second half of XIX century until the present day. The methodology of the study is based on the intonation approach to interaction of music and spoken word in B. Asafiev”s school in Ukraine and which corresponds to the trend of hermeneutics research of the musicology study, as offered in works of L. Zima, E. Markova, A. Roshchenko. The scientific novelty of the work is determined by originality of historical approach to vocal intonation change which has been given birth by Italian romanticism in its dissociation from the R. Wagner approach and which has been accepted as leading approach in operatic performing tradition. This concept coincides with the author's independent analysis of the main heroine in Donizetti’s opera. Conclusion. The role of Lucia in Donizetti‘s opera with its particular style of libretto and manifestation of both femininity and outstanding power resulting in madness forms the parallel to preceding operatic images of hers as well as contemporary images in the days of Donizetti. The composer’s approach to F. Takkinardi-Persiani’s interpretation of this role, however, points to a presentiment of the replacement of "Rossini" soprano by lyrical coloratura in the operas of Gounod. Still this replacement drove the performing of the dramatic role of Lucia by "light" coloratura sopranos - from A. Patti to A. Galli-Kurci, from A. Nezhdanova to E. Miroshnichenko and last not least author of the essay in reference. It is sincerity and faithfulness to the beloved one rather than power of nature which determine the core values of Lucia, forming an analogy to heroines in psychodramas of V. Rebikov, whose genius as a treasure of the Ukraine South became a discovery at the musical theater nowadays. As a consequence ” Lucia di Lammermoor” continuously occupies a significant place in the repertoire of Ukrainian opera houses.
On Doctors and Their Operas: A Critical (and Lyrical) Analysis of Medicine in Opera

Abstract:The goal of this research was to analyze the opera repertoire in terms of contents of physician roles. An additional goal was to pinpoint significant characters and passages that exemplify the changing role and perception of medicine in society over five centuries. A systematic search was performed. Musical characteristics and performing roles, and their determinants, were tabulated and formally analyzed. Of 493 operas listed, 53 (10.7%) were identified as having either a patient and/or doctor as characters. These operas span 239 years, from 1777 to 2016. Beyond unspecified Family Medicine and Generalists, some operas could be attributed to medical specialties, including 3 (5.7%) to Respiratory. Most interestingly, the 34 operas (6.9%) with physician roles are all represented by male characters, distributed by voice register mostly in the grave chords. Overall, the composer that appears with the highest interest in reporting disease is Giuseppe Verdi, with nine operas that include medical doctors. Finally, a trend can be observed regarding the evolving role of doctors, from mostly minor, buffal roles, either with magical or comical components in the 18th century, to a professional/technical physician more recently. Opera depicts a changing perception of the roles of doctors throughout history, with a blatant gender bias that still persists.

Abstract:Viewing and analyzing fine art from a medical perspective increases our appreciation of the artist’s suffering and teaches an important lesson of the human aspects of medicine. Paul Klee (1879–1940), one of the most influential artists of the 20th century, died at the age of 60 from complications of systemic sclerosis (scleroderma). In this second, in a series of clinical vignettes, I discuss the musculoskeletal manifestations of systemic sclerosis and how they may have influenced the life and art of Paul Klee.
'Il Sonnambulo' by Michele Carafa: A Forgotten Romantic Opera with Sleepwalking

Abstract:Romantic operas provide a useful tool for historians to understand the perception of some medical disorders that existed during the nineteenth century. Somnambulism was still a mysterious condition during this time, since its pathogenesis was unknown. Hence, it comes as no surprise that somnambulism features in a number of operas, the best known of which are Verdi's 'Macbeth' and Bellini's 'La Sonnambula', both the subject of recent scholarship. Here we examine a more obscure opera in which sleepwalking is depicted. Dating from 1824, 'Il Sonnambulo' by the Italian composer Michele Carafa is based on a libretto by Felice Romani. Although it shares some features with the Verdi and Bellini operas, it also presents original elements. Our analysis of this forgotten opera supports the contention that studying operas can shed light on medical theories and practices, and on how ideas about mind and body disorders were transmitted to the laity in times past.

Abstract:This paper progresses the original argument of Richard Ratzan that formal presentation of the medical case history follows a Homeric oral-formulaic tradition.The everyday work routines of doctors involve a ritual poetics, where the language of recounting the patient’s ‘history’ offers an explicitly aesthetic enactment or performance that can be appreciated and given meaning within the historical tradition of Homeric oral poetry and the modernist aesthetic of Minimalism. This ritual poetics shows a reliance on traditional word usages that crucially act as tools for memorisation and performance and can be linked to forms of clinical reasoning; both contain a tension between the oral and the written record, questioning the priority of the latter; and the performance of both helps to create the Janus-faced identity of the doctor as a ‘performance artist’ or ‘medical bard’ in identifying with medical culture and maintaining a positive difference from the patient as audience, offering a valid form of patient-centredness.
Quality of the singing voice in sopranos. Influence of the text and the musical accompaniment in the opera singing

Abstract:Introduction and objectives: Opera singing voice uses different technical tools for artistic purposes. We want to define if the quality of the sung sound is affected by an aware work on the musical accompaniment and the text. We analyse the acoustic signal and the subjective perception. The pitch studied in the soprano voice is A4 (880 Hz) with the vowel /a/. We have chosen a tone and a vowel that can be generated by different strategies in terms of phonoresonance adjustments. Materials and methods: We have carried out a prospective study with 20 sopranos, without voice pathology, who have sung a phrase from the aria Deh, vieni non tarda and another from the aria Dove sono i bei momenti from the opera Le nozze di Figaro, by W.A. Mozart. First, each phrase sung spontaneously was recorded and then a second time, after a proposed work on the content of the text and musical aspects of the accompaniment such as: rhythm, harmony, texture and direction of the phrase. The participants prolonged the emission of the A4 for more than three seconds, maintaining the context of the sentence. The acoustic signal has been analysed using the PRAAT programme and a VAS (Visual Analogue Scale) questionnaire has been used to collect subjective perceptions. Results: The mean age was 36 ± 11 years (range between 20 and 58) and the mean number of years singing was 17 ± 12 years (range between 3 and 35). No statistically significant differences were found, although an improvement in the VAS was seen in the second sentence after the intervention. Conclusions: The acoustic analysis parameters are stable, and the VAS tends to improve when an awareness of the text and the instrumental accompaniment is carried out.

Abstract:Modern historical research devoted to the development of medical science and the art of healing increasingly involves the analysis of sociocultural and sociocommunicative aspects, which become part of historical analysis. The article examines the relationship between fine art and human anatomy as an element of sociocultural translation (communication) of knowledge in the history of medicine, in particular anatomy. The art of the Renaissance, primarily portraiture from life, copying the image of a specific person, can reflect signs of his health, symptoms of illness, and medical knowledge in historical retrospect is actually part of the culture of society. The artists often turned out to be medically observant, accurate in small details. In this study, the authors show several examples of the reflection of specific medical pathologies in paintings and come to the conclusion that works of art can transmit information about the state of health of the depicted person, the presence of clinical manifestations of diseases captured by the brush of an artist but probably not yet known and understood by the doctors of that epoque.

Abstract:Summary Objectives/Hypothesis The purpose of this research was to study the associations of postural activities and knowledge of the voice of opera singers, recognized in the literature for the specificity of their posture. Additionally, the link between vocal pathologies and body pains on one hand, and posture control on the other hand was investigated. Methods A questionnaire including 90 questions was distributed to singers in France and overseas during 6 months. Ninety-eight opera singers participated in the survey. Data were analyzed using Excel and Stata software. Results The results showed that the singers who paid more attention to posture, postural work while singing, had knowledge of anatomy and postural role in singing voice, healthy lifestyle, and good body habits, had less vocal discomfort and pathologies, a better vocal preservation, and used more costo-abdominal breathing. On the other hand, knowledge of postural role and postural work was linked with pain and vocal fatigue. Furthermore, a preventive need was revealed concerning the providing of knowledge and skills to singers, but also to singing teachers, speech therapists, and stage workers. Conclusion Our study shows that benefits were found in the voice quality of opera singers with anatomical and postural knowledge and who work on posture as part of their vocal work compared to others. Postural work and knowledge increase attention to body pain and vocal fatigue. These results can inform health care providers, opera singers, and their teachers and performers of the benefits of posture on operatic voice quality. Accordingly, this study sparks new ideas for postural work and therapy in lyric voice.

The Role of Human Microbiota in Myasthenia Gravis: A Narrative Review

Abstract:Myasthenia gravis (MG) is an autoimmune neuromuscular disease characterized by fluctuating weakness of the skeletal muscles. Although antibodies against the neuromuscular junction components are recognized, the MG pathogenesis remains unclear, even if with a well-known multifactorial character. However, the perturbations of human microbiota have been recently suggested to contribute to MG pathogenesis and clinical course. Accordingly, some products derived from commensal flora have been demonstrated to have anti-inflammatory effects, while other have been shown to possess pro-inflammatory properties. In addition, patients with MG when compared with age-matched controls showed a distinctive composition in the oral and gut microbiota, with a typical increase in Streptococcus and Bacteroides and a reduction in Clostridia as well as short-chain fatty acid reduction. Moreover, restoring the gut microbiota perturbation has been evidenced after the administration of probiotics followed by an improvement of symptoms in MG cases. To highlight the role of the oral and gut microbiota in MG pathogenesis and clinical course, here, the current evidence has been summarized and reviewed.
Castrati singers: surgery for religion and art

Abstract:The act of castration was practiced from ancient times. In countries of Middle and Far East, castration was often done to provide eunuchs as guardians of the harems. In Europe and especially in Italy, it was carried out to preserve the male voice unbroken into adult life. From 16th century till the end of 18th century, castrati singers dominated opera with their supernatural voices. Boys were castrated mainly before the age of 9 years and when they grew up they had feminine characteristics, such as smooth, hairless bodies, breasts, infantile penis. The training procedure to become a castrato singer was very intense and lasted up to ten years. The most common surgical technique was either to sever the spermatic cords or crush the testis with the fingers. The voice of a castrato was the outcome of a larynx the size of a child's combined with the lung volume of an adult male. The castrati singers became superstars who dominated opera, singing both male and female roles for more than 200 years. Castrated for art, the beauty, range and flexibility of their voices raised them to mythical status.

Abstract:The reflection of medicine in the universal arts has motivated several rheumatologists to discover features of rheumatic diseases depicted by the artist's eyes long before they were defined as specific pathologic entities. The result has been the identification of several pieces of art dating from the Middle Ages, the Renaissance, the Baroque and Post-Impressionist periods that depict clear features of several rheumatic diseases such as RA, OA, camptodactyly and temporal arteritis, among others. On the other hand, great artists such as Pierre-Auguste Renoir, Antoni Gaudí, Raoul Dufy, Paul Klee, Frida Kahlo and Niki de Saint Phalle are good examples of how rheumatic diseases such as RA, scleroderma and chronic pain can influence the artist's perspective, the technique used and the content of their work. Art can serve as a powerful resource to understand the natural course of diseases. By learning through the artist's eyes the way illnesses behave and evolve in time, rheumatologists can trace the history of several conditions.

Abstract:Representations of disease in Renaissance paintings have been discussed in medical literature, in the context of historical epidemiology, as potential sources of information about the incidence and appearance of particular conditions in earlier times. The present study seeks to show how Renaissance art can also contribute to historical nosology by casting light on the question of whether particular conditions recognized as abnormal today were understood as pathological in the past. The hands of two Renaissance Madonna figures are examined in sculptures produced by Francesco di Simone Ferrucci (1437-1493). Because the Virgin Mary was considered physically perfect by believers, and because Francesco was a successful producer of devotional sculptures for a wide audience, it is highly probable that any abnormal conditions found in the hands of Madonnas sculpted by him would not have been regarded as pathological at the time. The sculptures examined appear to depict camptodactyly and boutonniere deformity in the hands of Madonna figures. These uncommon conditions are also found in Renaissance artworks that show other individuals of high social status, but their presence in the hands of the Madonna gives the strongest indication that they were not considered pathological, due to religious belief in the Virgin's physical perfection. Examination of Madonna figures in late fifteenth century Renaissance art can contribute to historical nosology by identifying abnormal conditions that were not regarded as pathological at the time. The examples of such conditions identified in the present study are camptodactyly and boutonniere deformity.

Abstract:Opera is the most complete form of theatrical representation, characterized by musical accompaniment, both instrumental and vocal. It has played an important role in sociocultural spheres, affecting the various social strata and reflecting customs and ideas in different centuries. Composers have created pieces that have also shown the development of medicine. Since the birth of opera in seventeenth century in Italy, neuroscience has played an important role in influencing the representation of madness and neurological aspects. From the Folly of the Renaissance, a path toward a representation of madness was developed, initially linked to the myths of classical antiquity. In the seventeenth and eighteenth centuries, madness was represented as comical or funny, of a loving nature and influenced by the spread of the Commedia dell'Arte (Comedy of Art). In the nineteenth century, with the rise of the first scientific theories of the mind, insanity took more precise connotations and was separated from other psychiatric and neurological diseases. The operas of the twentieth century depicted psychiatric and neurological diseases, taking into account newer medical and scientific discoveries.

Abstract:This essay brings together critical perspectives from the discrete traditions of artists' books and the medical humanities to examine artists' books by three contemporary artists - Penny Alexander, Martha A. Hall and Amanda Watson-Will - that treat experiences of illness and wellbeing. Through its focus on a multimodal and multisensory art form that has allegiances with, but is not reduced to, narrative, the essay adds to recent calls to rethink key assumptions of illness narrative study and to challenge utilitarian approaches. In particular, it draws attention to the aesthetic and imaginative elements of illness communication by exploring how artists' books represent lived experiences in a distinctively palpable way and offer an "intimate authority" that extends beyond narrative legitimacy or a form of struggle against the medical gaze. By interrogating narrative's dominance in medical humanities research, the essay further expands awareness of illness experiences that resist conventional forms of representation (such as chronic illness), and of alternative reflective practices within healthcare education that encourage engagement with both mind and body.
