Abstract:Whereas most companies use the century-old cost-plus pricing, this pricing method is especially inadequate for services on demand because these services have uncertain demand, high development costs, and a short life cycle. In this paper we propose a novel methodology, Price-at-Risk, that explicitly takes into account uncertainty in the pricing decision. By explicitly modeling contingent factors, such as uncertain rate of adoption or demand elasticity, the methodology can account for risk before the pricing decision is taken. The methodology optimizes the expected "net present value," subject to financial performance constraints, and thus improves on both the cost-based and value-based approaches found in the marketing literature.

Abstract:One of the most frequent, although widely understandable, reactions of people diagnosed with an incurable tumor is represented by incredulity, anger, and the denial of the impossibility of a definitive cure. Often, a picture of intense anxiety quickly takes over, overlapping the ever-growing collective hysteria of modern society, the result of a complex cultural mechanism in which technocracy often prevails over thought, introspection, and, in a broader sense, humanism. In this health drama, all actors often complain of formal inaccuracies while paying little attention to substantive ones. We argue that a more human emphatic patient-family-doctor relationship training to consider the undeniable progress of medicine and the fragility of all of us.

Bearing the mark of pain: mystery in medicine

Abstract:Dostoevsky wrote that love in action is a harsh and terrible thing compared to love in dreams. That reality is particularly evident in medicine, where there is an almost universal, involuntary participation of physicians and other healthcare workers in the suffering of their patients. This paper explores this phenomenon through the paradigm of 'mystery' as explained by the French existentialist philosopher Gabriel Marcel. A mystery is different from a problem in the sense that the former requires the active immersion of the person involved in order to be truly experienced. It is a 'meta-problem' that cannot be analyzed objectively and separately from the person that it affects, without changing the nature of the thing experienced. The authors contend that the human suffering encountered in medicine is one such phenomenon, and the paper draws on illustrations of this concept in art and literature. Awareness of the subtle but important difference between mystery and problem may help physicians better understand their personal entanglement with the suffering of patients.

Abstract:The phenomenon of death has accompanied human existence since the dawn of time. In the Middle Ages, it was something regarded as a natural, universal and almost daily occurrence. But it was not a concern for the body that absorbed the thinking of ordinary mortals. It was a belief in the afterlife and the immortality of the soul that made the phenomenon of death so essential. The after-death fate of human souls was to depend on that phenomenon, on the manner in which one's temporal life ended. But was only the welfare of the soul taken into account in the Old Polish period was well as more modern times? On the basis of old-time handbooks, guides and instructions manuals devoted to 'good dying', the author has presented various ways people once displayed a concern for the soul's further existence. Prayer, penance, alms and reconciliation with the temporal world were only some of them. A major role was assigned to the drafting of wills. It should be emphasised, that those not only concerned estates. The testaments drafted by Catholics contained requests for pre-paid prayers, masses and sums set aside for alms for the needy. A concern for one's earthly remains was by no means neglected. Both among Catholic and Protestant believers, the custom of holding often sumptuous funerals, referred to as pompa funebris, was a customary right, and requests for a 'quiet' funeral encountered in wills were not widely respected by heirs of the deceased.

Abstract:Abstract Synchrony and impact of the predators Laricobius nigrinus Fender and Laricobius rubidus LeConte, on hemlock woolly adelgid, Adelges tsugae Annand, were studied in an eastern hemlock field insectary in Virginia. First, a field insectary for propagation of the introduced L. nigrinus was established by planting hemlocks in 2001, infesting them with hemlock woolly adelgid in 2002 and 2003, followed by releasing 258 L. nigrinus in 2003. Initial sampling showed that the native L. rubidus was present in the area. Hemlock woolly adelgid and both Laricobius species populations increased annually, from which 305 F3 L. nigrinus adults were collected and redistributed to forests in 2007. Second, the phenology of hemlock woolly adelgid and Laricobius spp. life cycles were monitored in 2005 and 2006. Adult L. nigrinus (F2) and L. rubidus were active on hemlock from fall through mid-spring and overlapped with second-instar sistentes nymphs through progredientes eggs. The predators’ eggs were oviposited and larvae developed (i.e., F3 L. nigrinus) from late winter to mid-spring on progredientes eggs, indicating synchrony with suitable prey life stages. Third, a predator exclusion experiment was used to examine the relationships between the predators and prey in 2005 and 2006. When exposed to L. nigrinus (F2 adults and F3 larvae) and L. rubidus, hemlock woolly adelgid survival and ovisac density were lower and ovisac disturbance was higher than hemlock woolly adelgid protected in cages. The establishment and production of L. nigrinus at a field insectary, synchronization with, and impacts on hemlock woolly adelgid after a small release 2 yr earlier makes it an important potential biological control agent of hemlock woolly adelgid.
Nursing's professional dignity in palliative care: Exploration of an Italian context

Abstract:Aims and objectives: To determine the relevance of nursing's professional dignity in palliative care. Background: Dignity is a valued concept in the ethical discourse of health disciplines. Nursing's professional dignity, a concept related to professional identity, is not clearly defined nor have its characteristics been delineated for its clinical relevance in palliative care. Design: A qualitative methodological approach. Methods: Focus groups elicited dialogues of nursing's professional dignity among 69 nurses working in hospices and home-care in Italy. Data were content-analysed via an inductive process. The COREQ checklist for qualitative studies was used for reporting this research. Results: A central theme related to (a) "Intrinsic dignity of persons" was embedded in the essence of palliative care. Several corollary themes underscored this central theme: (b) Professional (intra- and inter) relationships and teamwork; (c) Nursing professionalism; (d) Ethical dilemmas; and (e) Relationships with patients and their significant persons. Conclusions: Nurses valued the essence of respect as persons and the essence of respect for their work as coherent with intrinsic dignity and work dignity in palliative care. Nurses perceived their psycho-social relationships with patients and their families as rewarding incentives amidst disputatious interactions with peers and/or other healthcare professionals. They experienced ethical dilemmas, which they perceived as inherent in palliative care. Relevance to clinical practice: Study findings corroborate the literature regarding the concept of nursing's professional dignity which is intrinsic in respect of the human person. The intrinsic dignity in palliative care manifests as nurses are working in juxtaposition of a demanding yet rewarding care ambience.

Abstract:OBJECTIVE The purpose of this study is to evaluate the clinical characteristics and surgical outcome of cerebellopontine angle (CPA) epidermoids presenting with trigeminal neuralgia. METHODS Between 1996 and 2004, 10 patients with typical symptoms of trigeminal neuralgia were found to have cerebellopontine angle epidermoids and treated surgically at our hospital. We retrospectively analyzed the clinico-radiological records of the patients. RESULTS Total resection was done in 6 patients (60%). Surgical removal of tumor and microvascular decompression of the trigeminal nerve were performed simultaneously in one case. One patient died due to postoperative aseptic meningitis. The others showed total relief from pain. During follow-up, no patients experienced recurrence of their trigeminal neuralgia (TN). CONCLUSION The clinical features of TN from CPA epidermoids are characterized by symptom onset at a younger age compared to TN from vascular causes. In addition to removal of the tumor, the possibility of vascular compression at the root entry zone of the trigeminal nerve should be kept in mind. If it exists, a microvascular decompression (MVD) should be performed. Recurrence of tumor is rare in both total and subtotal removal cases, but long-term follow-up is required.

Abstract:Poetry and medicine are related in multiple ways, including historical interests in healing, defined broadly, through words. More contemporary scholarship explores how poems, which include insights into the human condition, can enlarge our understanding of health, illness, mortality, and health care, including issues of diversity. Anesthesiology and poetry have particular affinities due to their structures, timeframes, and rhythms. Patients, physicians, and health care workers can benefit in terms of well-being by access to reading, reflecting on, and writing poetry.

Abstract:Traditionally regarded as high-art, poetry is often seen as a superior form of literary achievement consecrating in verse worldviews and lives connected to ideal, transcendental realms, the pursuance of which supposedly leads to some kind of ideal health and spiritual well-being. The poet WB Yeats (Nobel Prize in Literature, 1923), who believed in the power of poetry to reveal realities and states of such perfection, thereby giving purpose to mundane life, likened this effect of poetry to the fashioning of statues as monuments of unageing intellect. However, contradictorily, he also questioned the value of poetry thus conceived by questioning whether it is healthy to aspire to embody poetically consecrated ideals in real life. Yeats's dilemmatic negotiation between these two positions suggests that better personal well-being can be achieved in living an enlightening life by being mindful of the body's sensuality and materiality. In poetic explorations of the ways in which idealism and sensuality can affect how we live our lives, Yeats used real-life examples of people he knew, often important public figures in Irish social and political history.The present paper frames these explorations in terms of Yeats's concepts of living stream and stone/statuary, augmented with Bruno Latour's concepts of traditional subject and articulated body, discussed in relation to purpose in life and closeness and empathy, proposing that an overly idealistic 'poetic' lifestyle can have adverse effects, whereas poetry that increases one's awareness of oneself as articulated body is conducive to better health and well-being.

Abstract:Turbocodes have been introduced as a new encoding technique for error correction over noisy channels. Their performance is close to theoretical limits. In this paper, we describe the use of the cross-entropy principle in the turbocode domain and we show that it is possible to use it to obtain an additional gain in terms of error correction performance.

Abstract:Medical practice and the field of humanities frequently intersect. It is uncanny how problems presented or described in literature that are several hundred years old still present themselves to us on a regular basis. Often, our answers to these dilemmas are not perfect, but we continue our attempts at providing solutions through an enlightened evolution of our thought and approaches. Leo Tolstoy's novella, The Death of Ivan Ilych, is a classic piece of literature that allows a view of the dying process in an ordinary human being, and presents us with an opportunity to observe, not only the intersection of medicine and humanities, but also that of critical care and palliative medicine. Here Tolstoy, through his keen observation of the human condition at the end of life, allows us an opportunity to view a 19(th) century perspective that has an all too familiar persistence that needs a 21(st) century intervention.

Abstract:Background and purpose: This article will convey a new understanding of health and disease as a coherent dimension where the whole body interacts with its natural environment. This understanding is linked to a theoretical framework and illustrated by reference to research with patient experiences with inflammatory bowel disease (IBD), with the disorders Crohn's disease and ulcerative colitis. Creating ecological sustainability towards a greener health service where the body has a higher intrinsic value is a growing concern. Material and methods: The methodological approach is hermeneutic, whereby the results of one's own research are interpreted and reconceived through reflection towards a new understanding. Themes from patients' experiences are interpreted in light of the theoretical frame of reference of this article in order to achieve a new understanding. Results: The further interpretation of the state-of-the-art article and empirical articles resulted in the following themes: 1. The lived body is understood as a coherent whole and a place of inner freedom. 2. The body in a lowered state of stress helps the patient to experience dignity and vitality. 3. An integrative understanding of health in terms of IBD brings dignity and wholeness to the body. Conclusion: This article sheds light on the connections between body knowledge, nutrition, dignity, and integrative understanding of health in acute and chronic IBD. In clinical practice, the connections may happen when giving the patient the opportunity for recovery by learning to listen to the body. Dignity is linked to both coping and expert help from clinically competent health professionals.

Abstract:The authors use data from a finance-related theme park to explore how financial education changes investment, financing, and consumer behavior. Students were assigned fictitious life situations and asked to create household budgets. Some students received a 19-hour financial literacy curriculum before going to the park, and some did not. After controlling for demographic variables, the authors show that the treatment effects of the financial literacy program are strong. Students were more frugal, delayed gratification, paid off debt faster, and relied less on credit financing after training. Students who attended training showed greater uptake of decision support that was offered in the park, which indicates that decision support and financial literacy training are complements, not substitutes.

Abstract:Abstract One of the major themes of discussion in the art and especially the literature of the 18th and 19th centuries was the body rather than the soul. In the beginning this seemed to be the case mostly because of the natural processes related to the transforming events of maturation and death of the human body and mind. However, towards the end of the 18th century and well into the 19th century, a certain shift took place from the common perspective on the body to a rather scientific literary approach. Our attempt is to notice and make the necessary connections between the concepts of nature (both human and external/physical nature) and the innovative technologies implemented in the then society, with a later reference to the new accidental and commercial facets of death felt as destruction of nature especially in the work of the American Romantic writers Ralph Waldo Emerson and Henry David Thoreau. Being aware that they are highly spoken of in view of their transcendentalism as a particular philosophy dealing with the bond between man and nature, we will slowly come to terms with this type of concerns and connect them to the conflicting reality of industrialization as a sudden and repressive phenomenon within the society of men. Finally our point is that this very phenomenon caused the two writers to make a historical detour and use their formation as naturalist thinkers in order to make sense of their century’s deaths and diseases. This is to say that even from a Romantic perspective, the concern for the body is surprisingly concrete, while these writers’ transcendentalist concerns for the soul are prolix though without substance.

Abstract:Palliative Medicine, Ahead of Print. Background:Every advanced cancer diagnosis brings enormous challenges to patients and their relatives on numerous levels: be it physical, practical, social challenges, or on a more personal level. While specific aspects have been researched before, an overarching approach is lacking.Aim:To understand the lived experiences of people with advanced cancer, to identify gaps along the cancer care continuum, to identify potential opportunities for meaningful interventions and to develop a theoretical framework for practitioners and researchers.Design:A qualitative study using in-depth interviews with patients, relatives, and care professionals. Interviews were transcribed verbatim and analysed using a conventional content analysis.Setting/participants:Fifty-four interviews with 17 patients from a university oncology department and palliative care service, 15 relatives and 22 care professionals from physicians to funeral directors. All interviewees were recruited by a German university hospital.Results:We developed a novel model describing the diagnosis with advanced cancer as a highly disruptive experience that threatens to challenge the integrity of personhood in cancer patients through four areas: communication, knowledge, relationships, and confidence. We were able to identify factors leading to disintegration in these areas and factors supporting a restoration of integrity of personhood.Conclusions:The developed model provides a more thorough understanding of patients lived experiences. It can help to develop new interventions along the cancer care continuum to support patients in the complex challenges they face. These interventions should focus on supporting the integrity of personhood.
