Abstract:When it comes to old age, we are witnessing almost revolutionary changes at the present time. After decades of ignorance and lack of public interest, old age has fundamentally been re-negotiated. A diverse range of authors have diagnosed the growing bifurcation of old age into a rather independent and capable Third Age and a deep old Fourth Age that is characterized by sickness, frailty and dependency. Against this backdrop, many gerontologists claim that the so-called young-old are praised and valued for their (ongoing) "sameness" in terms of midlife-norms and capabilities, whereas the oldest old are increasingly excluded from humanity by radical "othering". Taking up this diagnosis, the article elaborates on this growing polarization within later life: Based on empirical research on the re-negotiation of old age in Germany, this contribution argues that the juxtaposition of "sameness" and "otherness" obscures the true character of the polarization, particularly with regard to the social role of the Third Age. Instead of sameness and otherness, we rather witness different processes of othering, with the young-old being valued as the other and the oldest old being disdained as the other. Despite the existence of profound critical analyses of the abjection associated with the Fourth Age as well as a considerable amount of literature on old age activation and the new role of the young-old, the specific point of this article's concern-the othering of the Third Age-has been completely neglected. The article discusses the reasons for this gap in more detail and will indicate to what extent concepts from Postcolonial Studies may help us to understand the dual process of othering-glorification and abjection.

Abstract:Fluorescence in situ hybridization (FISH) is used to determine amplification status of the Her-2/neu gene in specimens of newly diagnosed breast carcinoma. The Vysis kit for FISH analysis stipulates that the tissue be formalin-fixed and paraffin-embedded. Concerns regarding carcinogenicity of formalin and environmental effects of formalin waste have led to the development of formalin replacement products. An increasing number of breast biopsy specimens are being fixed in these substitutes. We tested 6 non-formalin–based fixatives to determine their impact on FISH testing for Her-2/neu gene amplification status by comparison with formalin-fixed control specimens from the same neoplasm. Specimens fixed in Pen-Fix, Prefer, Histochoice, UniFix, and GTF were associated with absent or technically compromised staining in at least one of the 3 neoplasms tested for each fixative when compared to the formalin-fixed control. O-Fix did not seem to compromise staining quality in 3 paired specimens tested.

Abstract:While most body image research has focused on young female populations, evidence has shown that as few as 12% of older women are satisfied with their body size. Recent studies have also highlighted how anti-aging discourses are promoting unrealistic body norms, which have shown to contribute to poor body image and altered health behaviors. A systematic review of empirical studies focused on older women's perspectives of health, body image, and the aging body is presented. Findings support that body image is a persistent, lifelong issue for women and should be considered when implementing healthy aging policies and practices.

‘This really takes it out of you!’ The senses and emotions in digital health practices of the elderly

Abstract:Wearables, fitness apps and home-based monitoring technology designed to help manage chronic diseases are generally considered in terms of their effectiveness in saving costs and improving the health care system. This article looks, instead, at the digital health practices of persons older than 65 years; it considers their actual health practices, their senses and emotions. In a qualitative study 27 elderly persons were interviewed about their digital health practices and accompanied while using the devices. The findings show that digital technologies and ageing bodies are co-productive in performing specific modes of health and the ageing process. The study shows that digital technologies not only encourage the elderly to remain physically active and enable them to age in place, but also that the use of these technologies causes the elderly to develop negative emotions that stand in a charged relationship to ageing stereotypes. Thereby, the sense of seeing has been place in pole position, while the faculty for introspection declines. This means that age-related impaired vision can result in particularly severe consequences. In the discussion it is debated in which concrete ways that digital health technologies have had a negative impact. The sociotechnical practices associated with wearables conform to the primacy of preventing ageing; passive and active monitoring technologies appear as subsystems of risk estimation, which in turn regulates diverse practices. The conclusion highlights the interrelation between notions of successful ageing and the digital practices of the elderly.

Abstract:The way frailty is conceptualised and interpreted has profound implications for social responses, care practice and the personal experience of care. This paper begins with an exegesis of the concept of frailty, and then examines the dominant notions of frailty, including how ‘frailty’ operates as a ‘dividing-practice’ through the classification of those eligible for care. The definitions and uses of ‘frailty’ in three discursive locations are explored in: (a) the Oxford English Dictionary , (b) the international research literature, and (c) older women's accounts of their lived experience. Three distinctive discourses are found, and applying a Foucauldian analysis, it is shown that the differences reflect overlaps and tensions between biomedical concepts and lived experiences, as well as negative underlying assumptions and ‘subjugated knowledge’. The concept of frailty represents and orders the context, organisational practices, social representations and lived experiences of care for older people. The evidence suggests that if, as the older women's accounts recommended, socially- and emotionally-located expressions of frailty were recognised in addition to the existing conceptions of risk of the body, frailty might no longer be thought of primarily as a negative experience of rupture and decline. To encourage this change, it is suggested that practice focuses on the prevention of frailty and associated feelings of loss, rather than reinforcing the feelings and experiences which render a person ‘frail’.

Abstract:This essay speculates about how medical philosophy may change to accommodate the technologic, organizational, and--in particular--population challenges of the future. The historical roots of the dominant medical world view are drawn and its tenets outlined. The existing paradigm may be called the Western biomedical model, whose doctrines include body-mind dualism, physical reductionism, the mechanical analogy, specific etiology, and the body as the appropriate object of regimen and control. Some of the pressures straining the paradigm are discussed, especially the force of human and population aging as it accelerates the dominance of chronic illness and focus of health care. The tentative outlines of an emergent paradigm are described. The mind, biography, surrounding environment, and culture are a few considerations that become very significant in a non-Cartesian world.

Abstract:Exploring older people's evocation of their positive experiences of aging has been proposed as a counterweight to the Western stereotype of aging as a process of decline. The aim of this article is to explore how aging women, who participate regularly in group exercise classes, perceive their own bodies and the bodies of others. This article reports on the findings from interviews with 16 women between the age of 70 and 85. We analyzed the data using qualitative content analysis. Two overarching and interrelated themes concerning body perception emerged from the interviews: "The aging body and appearance" and "The body as subject and object." The binary discourse of old age, as either a decline or a success appears in our findings. The training contributes to a sense of well-being experienced through perceived increased physical abilities, self- image and self-esteem. Physical ability was perceived as being more important than appearance by the participants in this study, considering their preconception of an association between declining health, abilities, and older age. Involvement in physical activity appears to play a significant role in the perception of the women's own aging. Although physical attractiveness is a desirable outcome, the most important positive impact of the group exercise was related to increased social belonging and well-being, physical abilities, and capabilities. Thus implications for practice suggests that an intensive group training contributes to the opinion that an aging body is not necessarily a barrier to positive and successful aging.

Abstract:Neurodegenerative diseases (ND) pose significant challenges for biomedicine in the twenty-first century, particularly considering the global demographic ageing and the subsequent increase in their prevalence. Characterized as progressive, chronic and debilitating, they often result in higher mortality rates compared with the general population. Research agendas and biomedical technologies are shaped by power relations, ultimately affecting patient wellbeing and care. Drawing on the concepts of bio- and necropolitics, introduced by philosophers Foucault and Mbembe, respectively, this perspective examines the interplay between the territoriality and governmentality around demographic ageing, ND and death, focussing on knowledge production as a dispositif of power by highlighting the marginal role that the phenomenon of mortality plays in the ND research landscape. We propose a shift into acknowledging the coloniality of knowledge and embracing its situatedness to attain knowledge 'from death', understood as an epistemic position from which novel approaches and practices could emerge.

Abstract:This paper considers the concept of successful ageing by means of a dialogue with the concept of frailty. This dialogue reveals the limits and blindspots of both concepts as well as their apparent dichotomy and the importance of understanding both the objective and subjective experience of ageing. In particular, the dialogue highlights that both frailty and successful ageing are social constructs that derive their meaning from what society values (autonomy, youthful capacities). They cannot by themselves account for the fact that flourishing and frailty are often found together whilst conversely successful ageing may not bring satisfaction. I argue that the arts and humanities supply us with conceptual and methodological tools with which to revisit what it is to age well offering a holistic approach that combines sensitivity both to older people's lived experience and to the underpinning material conditions and embodied realities. For this purpose, and building on previous scholarship in cultural gerontology, I suggest the unique value of concepts and frameworks associated by Simone de Beauvoir. When brought into alignment with the tenets of cultural and narrative gerontology, these concepts facilitate a rich understanding of the nuances and paradoxes of flourishing in deep old age which can also aid in restorying both old age and the life course more generally. I illustrate this through two examples: a feminist self-help guide to ageing, which accepts both limitations and growth and a film that imagines the possibilities of authentic ageing even despite a context of poverty, patriarchy and Alzheimer's Disease.

Abstract:PURPOSE In a previous study we investigated the analgesic efficacy of a combination of metamizol plus diclofenac after general anaesthesia. After minor orthopaedic surgery postoperative opioid requirements were reduced by 73% during the first 24 h after surgery. In the present study, we have investigated the efficacy of this analgesic combination after minor orthopaedic operations performed in spinal anaesthesia. METHODS Seventy four patients, scheduled for minor orthopaedic surgery, participated in this double-blind, randomised, placebo-controlled study. The setting was comparable to our previous study. Before induction of spinal anaesthesia, verum-treated patients received a diclofenac suppository (100 mg), and metamizol (1 g/100 ml NaCl 0.9% intravenously over 15 min). These infusions were repeated at 6 h and 12 h. In addition to the third infusion, the patients received a further diclofenac suppository (100 mg). Cumulated doses of buprenorphine (PCA, patient-controlled analgesia), pain scores (0-10), blood pressure, heart rate and side effects were recorded during the first 6 h and again at 24 h. RESULTS After spinal anaesthesia had subsided, all patients required increasing doses of buprenorphine. Verum-treated patients required significantly lower doses during the first 24 h after surgery (median -29%). CONCLUSIONS The combination of metamizol and diclofenac causes a clinically relevant reduction in opioid requirements after minor orthopaedic surgery in spinal anaesthesia.

Abstract:Digital technologies have gained vast relevance in postmodern societies and digital infrastructures are substantially integrated into the everyday lives of older people. This digitization is reframing the norms and practices of later life as well as the social construct of age itself. Despite the increasing amount of studies in the field of aging and technologies, it still lacks theorizing. This paper addresses this deficit, suggesting that the study of aging and technologies could profit from a comprehensive integration of theories from the sociology of aging, critical gerontology, and science-and-technology studies. We aim to make a theoretical contribution to this issue, asking: how is age being done in a digitized world? Applying a praxeological approach to aging and technologies, we firstly examine how theoretical and empirical work has constructed aging with technologies so far and identify its shortcomings. Some of this work so far lacks a proper consideration of social inequalities within these processes, whereas other studies lack a thorough consideration of materialities. Secondly, in an attempt to equally “praxeologize” and “materialize” the study of aging and technologies we develop a theoretical model that aims to overcome these shortcomings. In what we frame as a material praxeology of aging with technology, we are concerned with how age is being done through discursive formations, set into practice through social and material practices and involved in the (re)production of social inequalities. Enriching a Bordieuan terminology of social fields with notions of non-human agency, this praxeology is founded on three assumptions: (1) Social fields constitute the contexts in which age as a social phenomenon is being done with and through technologies (2) Human and non-human agents are equally involved in this process (3) The actions of the involved agents emerge from an agency distributed among them, and are structured through the power relations between them. Thirdly, we exemplify the application of this model by reference to a research project in the field of Active and Assistive Living.

Abstract:Abstract This paper argues that the philosophy of ambiguity, associated with Maurice Merleau-Ponty and Simone de Beauvoir, provides a helpful framework for understanding the complex and paradoxical nature of old age outside the dominant categories of the ‘third’ and ‘fourth’ ages. Building on philosophical literature in cultural gerontology including that focused on the ‘art of living’ and other approaches that draw on existentialist thought, it suggests ‘ambiguity’ as a theme that has been overlooked in such literature. The advantage of this approach is that it can accommodate contradictory elements and thus underpin an integrated approach to old age. At the same time, in distinguishing between ontological and social dimensions of ambiguity, the conceptual framework offers a new perspective on ageism that can help clarify the distinction between ageism as oppression and the negative aspects of deep old age itself. The paper is structured as follows. After making the case for the value and importance of ‘ambiguity’ as a framework for viewing old age, I set out the key aspects of this philosophy as found in the work of Merleau-Ponty and particularly Beauvoir. I then apply this framework to a discussion of two literary memoirs of deep old age, by Diana Athill and Florida Scott-Maxwell respectively. I argue that the picture of old age which emerges contrasts with the hegemonic discourses enframed by successful ageing and frailed old age and offer the possibility not only of a true appreciation of old age but also of a more meaningful life course itself.

Outdated and re-configured: Challenging linear conceptualizations of ageing through the case of revived obsolete technologies

Abstract:Taking as a starting point the conventional view of ageing as a linear process beginning in a youthful and productive stage but gradually deteriorating, this paper shifts the usual anthropocentric focal point towards technological artifacts which do not conform to this typical view. More specifically, three examples of technologies previously considered obsolete, but which have seen a revival in the last decade, are presented: the so-called dumbphones, analogue cameras, and vinyl players. Although very different at first glance, the three cases of these revived technologies show a similar evolution trajectory which breaks from the typical view of ageing in technological artifacts. Instead, they indicate how their revival does not simply entail a reconsideration of their initial value (such as it is often the case with antiques or heirlooms), but a transformation, hybridisation, and re-envisioned purpose. To this effect, the agential realism theory is applied to show how the revival of technological artifacts and practices once considered outdated attempts to dissolve binaries such as old/new, young/old, or slow/fast. Furthermore, such artifacts reveal trajectories of ageing that are unlike their human counterparts, but which can make way for new manners of articulating issues pertaining to ageing as a process in humans as well. The contribution of the paper lies in illustrating how adopting a non-linear view of ageing and fundamentally questioning its inherent binaries has the capacity to produce a much-needed nuanced view of ageing in humans, non-humans, and their sociomaterial entanglements.

Abstract:Abstract Aging has been given short shrift as a topic in philosophy. The aim of this article is to redress this neglect by revisiting some of the key philosophical issues in Simone de Beauvoir’s book, Old Age. In her notion of old age’s unrealizability, its impossibility of fully embodying a subject position, and the role played by the other in denying such subjectivity, she draws upon the work of both Heidegger and Sartre. The dilemma she repeatedly draws attention to, of always seeming to age in ways other than as one’s self, raises the question of whether any view of aging as an authentic subjectivity may be no more than, in Heidegger’s words, a “chimerical undertaking.” In examining how the concepts of bad faith and inauthenticity are used by Heidegger and Sartre, the article concludes that for both these writers, an authentic subject position can be maintained in later life, without ending up as the otherwise inauthentic subject of others’ collective imaginary of “a good age.”

Abstract:In this article, I analyze interviews with a diverse group of 30 women aged 46–71 to understand how they experience signs of aging, such as weight gain, gray hair, and facial hair, in everyday life. I find that some women’s responses are in line with normative femininity and appearance norms. Others, however, focus on different gendered meanings of the body that are connected to care-taking, work, ageist treatment, and past abuse. I argue that feminists should apply the theoretical concept of femininity more broadly than appearance and attraction issues to gain a deeper understanding of the multiple meanings of living in an aging female body in a gendered society. In the conclusion, I discuss the implications of this study for public health policies as well as future research on gender and the body.

Effects of long-term cocaine administration and exercise on cardiac metabolism and isomyosin expression.

Abstract:Although chronic cocaine use is cardiotoxic, its use remains problematic in athletics. Hence adaptive changes induced in the heart by superimposing chronic cocaine use on an exercise training are of interest but remain poorly understood. Therefore this study investigated the effects of cocaine treatment combined with exercise training on the metabolic and contractile properties of the heart. Male Sprague-Dawley rats were assigned to one of four groups: normal sedentary (NS, n = 6), cocaine sedentary (CS, n = 6), normal trained (NT, n = 6), and cocaine trained (CT, n = 6). Trained animals were sprint trained 4 times/week. CS and CT animals received cocaine (25 mg/kg, ip) 6 times/week, 15 min before each exercise bout and 2 additional times per week. After 12 weeks, all animals were sacrificed, and the hearts were removed and analyzed for citrate synthase activity, 3-hydroxyacyl-CoA dehydrogenase activity, Ca(2+)-activated myofibrillar ATPase activity, and myosin isoform distribution. None of the groups demonstrated altered cardiac metabolic properties, but cocaine alone and in conjunction with exercise reduced myofibrillar ATPase activity (p < 0.05) and increased expression of the low ATPase myosin isoform, V3. These data suggest that the potential of the citric acid cycle and beta-oxidation is not sensitive to chronic cocaine treatment, but the distribution of cardiac myosin among its three isoforms is affected. Furthermore, high-intensity treadmill training does not interact with cocaine to further alter these properties.

Abstract:The body is central to the experience of aging. In this metasynthesis, the authors located, analyzed, and reconceptualized phenomenological research on how life is experienced within and through the body after 85 years. Sandelowski and Barroso’s metasynthesis method was employed. Seven databases were searched for primary phenomenological studies. After systematic screening and quality appraisal, 19 studies were included for review. Analysis was guided by Maurice Merleau-Ponty’s existential philosophy of embodiment. Findings were organized using Max van Manen’s reflective lifeworld existentials. Four ontological dimensions emerged, namely, being in a body (corporeality), being in time (temporality), being in place (spatiality), and being with others (relationality). Each dimension captures a distinct experiential dimension of this stage of the life course. The results suggest vast heterogeneity in the ontological experience of aging, supporting the view that diversity grows with age. The ability to adapt to multidimensional losses was identified as a source of resilience.

Abstract:Soil organic matter (SOM) is essential for sustaining soil health and crop productivity. However, changes in SOM stocks in response to agronomic practices are slow and show years later when it is too late for adjustments in management. Identifying early indicators of SOM dynamics will allow early management decisions and quick remedial action. The objectives of this study were to evaluate long-term effects of tillage intensity and timing on SOM pools and determine the most responsive SOM pools to tillage practice. Soil from a long-term (53 yrs) winter wheat (Triticum aestivum L.) - spring pea (Pisum sativum L.) rotation and undisturbed grass pasture (GP) in inland Pacific Northwest (iPNW) was sampled to evaluate the effect of four tillage systems [no-till (NT), disk/chisel (DT/CT), spring plow (SP), and fall plow (FP)] on soil organic carbon (SOC, proxy for SOM), total nitrogen (TN), particulate organic matter carbon (POM-C) and nitrogen (POM-N), permanganate oxidizable carbon (POXC), water extractable organic carbon (WEOC), total dissolved nitrogen (TDN), KCl-extractable nitrogen (KEN), microbial biomass carbon (MBC) and nitrogen (MBN), basal respiration (BR), carbon mineralization (Cmin), and metabolic quotient (qCO2). GP had higher levels of SOC pools than cultivated treatments. On average, tillage significantly decreased SOC and TN by 28% and 26%, respectively, compared to GP. Among the cultivated soils, tillage had no significant effect on SOC and TN, except for DT/CT that had slightly higher SOC than FP (P=0.08). On the contrary, NT and DT/CT significantly (P<0.05) increased levels of POM-C, POM-N, POXC, WEOC, MBC, BR, Cmin, and qCO2 over FP or SP. However, tillage did not affect TDN, MBN, and KEN. The C-pools (POM-C, POXC, MBC, WEOC, BR, and Cmin) were more strongly correlated with SOM than the N-pools (TDN, MBN, and KEN), with an exception to POM-N. Under wheat-pea rotation in the iPNW, reduced tillage systems (NT and DT/CT) have a potential to maintain or increase SOM, which can be assessed early through its physical (POM), chemical (POXC, WEOC), and microbiological (MBC, BR, Cmin) indicators. POXC and WEOC were the most sensitive indicators of tillage-induced changes in SOM dynamics.
