Abstract:Background: Nowadays, there is a growing need for competence in bronchoscopy. However, traditional mentorship-based training presents limitations in learning consistency, subjective evaluation, and patient safety concerns. Simulation-based training has gained attention for its potential to enhance skill acquisition and objective assessment. This study explores the effectiveness of self-driven bronchoscopy simulation training using high-fidelity (HFM) and low-fidelity (LFM) models on novice medical students. Methods: Medical students without prior bronchoscopy experience were randomly assigned to a self-learn using either HFM or LFM for 4 hours. Pre and posttests were conducted to evaluate the students' knowledge and skill gains with a modified Bronchoscopy Skills and Tasks Assessment Tool. After their test results, students were asked to answer a questionnaire based on a Likert Scale to assess their satisfaction with self-learning. Results: The results revealed significant progression in both groups, but the HFM group outperformed the LFM group in terms of total score, knowledge, skills, and procedure duration. Participant satisfaction with the educational process was generally high in both groups. However, this study has certain limitations, such as a small sample size, a short training period, and the absence of real evaluation in patients. Conclusion: This study demonstrates the efficacy of self-driven bronchoscopy simulation training using HFM and LFM of bronchoscopy, with HFM offering superior gain for skill and knowledge. Larger-scale and long-term studies are recommended to further investigate the effectiveness of this self-learning and retention of the bronchoscopy knowledge.
Development of an In Situ Thoracic Surgery Crisis Simulation Focused on Nontechnical Skill Training

Abstract:Background: Our vision was to develop an inexpensive training simulation in a functional operating room (in situ) that included surgical trainees and nursing and anesthesia staff to focus on effective interprofessional communication and teamwork skills. Methods: The simulation scenario revolved around an airway obstruction by residual tumor after pneumonectomy. This model included our thoracic operating room with patient status displayed by an open access vital sign simulator and a reversibly modified Laerdal airway mannequin (Shavanger, Norway). The simulation scenario was run seven times. Simulations were video recorded and scored with the use of Non-Technical Skills for Surgeons (NOTSS) and TeamSTEPPS2. Latent safety threats (LSTs) and feedback were obtained during the debriefing after the simulation. Feedback was captured with the Method Material Member Overall (MMMO) questionnaire. Results: Several LSTs were identified, which included missing and redundant equipment and knowledge gaps in participants' roles. Consultant surgeons received a higher overall score than thoracic surgery fellows on both NOTSS (3.8 versus 3.3) and TeamSTEPPS2 (4.1 versus 3.2) evaluations, suggesting that the scenario effectively differentiated learners from experts with regards to nontechnical skills. The MMMO overall simulation experience score was 4.7 of 5, confirming a high-fidelity model and useful experiential learning model. At the Canadian Thoracic Bootcamp, the MMMO overall experience score was 4.8 of 5, further supporting this simulation as a robust model. Conclusions: An inexpensive in situ intraoperative crisis simulation model for thoracic surgical emergencies was created, implemented, and demonstrated to be effective as a proof of concept at identifying latent threats to patient safety and differentiating the nontechnical skills of trainees and consultant surgeons.

Abstract:Abstract This study examined the relationship between organizational support, work-family conflict, and job and life satisfaction among coaches. Data from collegiate head coaches with families (N = 253) were gathered through a mailed questionnaire. Results from a series of covariance structure models indicated that a partially mediated model was the best fitting model, χ2 = (255, N = 253) 461.20, p < .001; root mean error of approximation = .05; comparative fit index = .95; parsimonious normed fit index = .71. In partial support of the study hypotheses, the results supported full mediation of the direct effect from organizational support to life satisfaction. Work-family conflict partially mediated the relationship between organizational support and job satisfaction. Job satisfaction partially mediated the effect of organizational support and work-family conflict to life satisfaction.

Abstract:INTRODUCTION AND OBJECTIVES: Three-dimensional (3D) model simulation provides the opportunity to manipulate real devices and learn intervention skills in a realistic, controlled, and safe environment. To ensure that simulators provide a realistic surrogate to real procedures they must undergo scientific validation. We aimed to evaluate the 3D-printed simulator SimulHeart® for face and content validity to demonstrate its value as a training tool in interventional cardiology (IC).METHODS: Health professionals were recruited from sixteen Portuguese IC units. All participants received a 30-minute theoretical introduction, 10-minute demonstration of each task and then performed the intervention on a 3D-printed simulator (SimulHeart®). Finally, a post-training questionnaire focusing on the appearance of the simulation, simulation content, and satisfaction/self-efficacy was administered.RESULTS: We included 56 participants: 16 "experts" (general and interventional cardiologists), 26 "novices" (cardiology residents), and 14 nurses and allied professionals. On a five-point Likert scale, the overall mean score of face validity was 4.38±0.35 and the overall mean score of content validity was 4.69±0.32. There was no statistically significant difference in the scores provided by "experts" and "novices". Participants reported a high level of satisfaction/self-efficacy with 60.7% considering it strongly improved their skills. The majority (82.1%) "agreed" or "strongly agreed" that after the simulation they felt confident to perform the procedure on a patient.CONCLUSION: The 3D-printed simulator (SimulHeart®) showed excellent face and content validity. 3D simulation may play an important role in future IC training programs. Further research is required to correlate simulator performance with clinical performance in real patients.

Abstract:Background Considering the complexity of lung structures and the difficulty of thoracoscopic surgery, simulation-based training is of paramount importance for junior surgeons. Here, we aim to design a high-fidelity lung model through utilizing the three-dimensional (3D) printing technology combined with synthetic materials to mimic the real human lung. Methods The 3D printed lung model was manufactured based on the computed tomography images of a randomly selected male patient. Synthetic materials were used for the construction of lung parenchyma, blood vessels, and bronchi. Then, the model was assessed in terms of its visual, tactile, and operational features by participants (the senior surgeons, junior surgeons, and medical students), who were asked to complete the specially designed survey-questionnaires. Results A 3D printed model of the right lung made of synthetic materials was successfully fabricated. Thirty subjects participated in our study (10 senior surgeons, 10 junior surgeons, and 10 medical students). The average visual evaluation scores for senior surgeons, junior surgeons, and medical students were 3.970.61, 4.56 +/- 0.58, 4.76 +/- 0.49, respectively. The average tactile evaluation scores were 3.40 +/- 0.50, 4.13 +/- 0.68, 4.00 +/- 0.64, respectively. The average operation evaluation scores were 3.33 +/- 0.83, 3.93 +/- 0.66, 4.03 +/- 0.66, respectively. Significant lower scores were obtained in the group of the senior surgeons compared with the other two groups. Conclusion A high level of fidelity was exhibited in our 3D printed lung model and it could be applied as a promising simulator for the surgical training in the future.

Abstract:Objectives To improve our previous simulation-based training module by using sustainable material to mold an anatomically accurate terrain and reproducing major vascular injuries encountered during robot-assisted nephrectomy. Methods The simulator was built with a pump, gauge, and valve linked via silicone tubing. Artificial blood was made from cornstarch, water, and red dye, and pumped through 3D-Med artificial vessels with the dimensions of an average renal artery. Silicone was used to emulate the pliability of organic tissue and mold an anatomically accurate terrain. Eight urologic residents participated in the pilot simulation. We employed validated assessment tools including Non-Technical Skills for Surgeons and Objective Structured Assessment of Technical Skills forms to guide debrief sessions moderated by an expert physician after individual performance evaluations. Results The apparatus demonstrated high reproducibility across all simulation scenarios, enhancing resident problem-solving skills. Residents' pre-simulation surveys revealed significant concern regarding their acute hemorrhage management. Residents' post-simulation survey demonstrated average realism scores increased from 4.375 to 4.75. Residents also felt the simulator enhanced learning, offering valuable practice and knowledge applicable to their surgical specialty. Conclusions The management of acute hemorrhage during robot-assisted surgery remains a space for additional surgical education and training. Our simulation successfully provided a reliable, reproducible training for residents to practice their technical and non-technical skills in managing acute hemorrhage.

Abstract:Background One-lung ventilation in infants is a high-risk procedure. Complications include endotracheal tube occlusion, with grave consequences. Although there are commercially available bronchoscopy simulators, there are no realistic models of infant patients. This limits access to training opportunities that would ensure safe and efficient lung isolation. To bridge this gap, we developesd a realistic infant bronchial tree model for single lung intubation and evaluated preliminary validity evidence of its features and clinicians' ability to perform critical skills associated with pediatric one-lung ventilation. Methods Using computed tomography imaging, a stereolithography file of an infant airway was generated to 3D print a model. This model was inserted into a commercially available airway trainer to allow lung isolation using standard bronchoscopy techniques. Ten experienced pediatric anesthesiologists independently evaluated the simulator's physical attributes, realism, value, and relevance using a 29-item paper survey and rated using 4-point rating scales (4 = highest). Participants' ability to complete 5 critical tasks was self-reported using 5-point rating scales (5 = too easy). Item and domain mean ratings were calculated, and comments reviewed. Results Overall, reviews were positive, with mean scores indicating adequate realism and high value. Specific challenges were associated with right mainstem bronchus and upper lobe takeoff. Performance scores indicated that most tasks were "somewhat easy to perform," suggesting that the model's anatomy did not hinder physicians' ability to perform one-lung ventilation. Conclusion Preliminary findings indicate that the novel simulator holds promise for training in lung isolation techniques after refinement. Future research will target refinement, expanding evaluation, and developing a comprehensive curriculum and competency assessment program.

Abstract:Introduction A team of biomedical engineers, neurosurgeons, and a medical educator, created and evaluated preliminary validity evidence of a high-fidelity simulator used to train ventriculostomy skills. Description A scale reproduction of an adult skull and relevant anatomy was created. Twelve neurosurgery residents, one fellow, and five attendings (n=17) from three academic medical centers performed the simulated creation of a ventricolostomy. Participants rated the simulator using a 37-item survey across five domains. Quality of the simulator was scored from “Not at all realistic” (1) to “Highly realistic” (4). Self-reported ability to perform relevant tasks was rated from “Too difficult to perform (0) to “Too easy to perform” (4). Validity evidence relevant to test content was evaluated using a Rasch model, while evidence relevant to internal structure (inter-item reliability, and inter-rater agreement) was evaluated using traditional methods. Conclusion Analyses indicated attendings had statistically higher ratings (M=3.4/4.0) than fellows (M=3.3), and residents (M=3.0), p=.02. Domain means were 3.9 (Value), 3.5 (Physical attributes), 3.4 (Realism of experience), 3.3 (Relevance), and 2.9 (Ability). Inter-item consistency across domains were estimated to be moderate-high (.65, .92), and inter-rater agreement regarding simulator characteristics was high [ICC(2,k)= .90]. Rating differences were found across institutions, p=.001, and are reviewed. The observed global rating (2.5) indicated the simulator can be considered for teaching ventriculostomies, but could be improved slightly. Ratings indicated the simulator was valuable as a learning tool, but could be improved with minor modifications. Abstract:The COVID-19 pandemic has drawn attention to aerosol-generating medical procedures (AGMPs) in health care environments as a potential mode of transmission. Many organizations and institutions have published AGMP safety guidelines, and several mention the use of simulation in informing their recommendations; however, current methods used to simulate aerosol generation are heterogenous. Creation of a high-fidelity, easily producible aerosol-generating cough simulator would meet a high-priority educational need across all medical specialties. In this communication, we describe the design, construction, and user study of a novel cough simulator, which demonstrates the utility of simulation in raising AGMP safety awareness for providers of all roles, specialties, and training levels.

Abstract:Mobile wayfinding and guide apps have become indispensable tools for navigating unfamiliar urban spaces. Such applications address targeted, "just-in-time" queries, but are not optimally designed for multi-point expeditions that can quickly build route and survey-level familiarity with a neighbourhood. We first conducted an experimental simulation involving a homebuying scenario to assess the usefulness of a popular mobile wayfinding and search application (Google Maps) for exploring a neighbourhood. We then designed a prototype application called Block Party that addresses a number of limitations of Google Maps for this purpose, and evaluated it in a second replica study. The results suggested that application designs that facilitate switching among distinct but synchronized navigation views such as Block Party might support more efficient usage and the selection of task-appropriate views, leading to better overall spatial awareness.

Abstract:Objective: Microvascular anastomosis is challenging, and training surgeons to develop and maintain skills is imperative. Current training models either miss the simulation of the surgical workflow, lack 3D key-hole space, need ethical approval, require special preparation, or lack realism. To circumvent these issues, this study describes the use of a mixed reality 3D printed model with integrated blood flow for training cerebral anastomosis and assesses its validity. Methods: Different-sized 3D-printed artificial micro artery models in a 3D brain space with a keyhole opening were used. The model was connected to a 4D simulator to induce pulsatile blood flow. Neurosurgical microscopes and exoscopes were used for visualization. Nine participants (n = 6 board-certified cerebrovascular neurosurgeons; n = 3 in-training) participated in the study and practiced anastomosis techniques with the simulator. Two senior, experienced vascular neurosurgeons mentored live teaching activity on the simulator. Participants completed a survey to score the face and content validity of the simulation on a 5-point Likert scale. Simulation time and anastomosis score differences between in-training and board-certified participants were compared for construct validity. Results: Participants scored the simulation difficulty similar to actual surgery, proving face validity. All participants agreed that practice on the provided simulator models would improve bypass techniques (μ = 4.67/5 ± 0.47) and instrument handling (μ = 4.56/5 ± 0.68). Board-certified participants had better anastomosis scores than in-training participants (non-significant difference). Conclusions: The 4D simulator and the high-fidelity artificial 3D printed model effectively simulated actual bypass surgery in a key-hole fashion with blood flow abilities. Limited resources and preparation time are needed for the training setup. The model provides benefits in learning and maintaining anastomosis skills and allows for easy adaptation to different microanatomical scenarios.

Abstract:Background : Three-dimensional (3D) printing is an emerging tool in the creation of anatomical models for surgical training. Its use in endoscopic sinus surgery (ESS) has been limited because of the difficulty in replicating the anatomical details. Aim : To describe the development of a patient-specific 3D printed multi-material simulator for use in ESS, and to validate it as a training tool among a group of residents and experts in ear-nose-throat (ENT) surgery. Methods : Advanced material jetting 3D printing technology was used to produce both soft tissues and bony structures of the simulator to increase anatomical realism and tactile feedback of the model. A total of 3 ENT residents and 9 ENT specialists were recruited to perform both non-destructive tasks and ESS steps on the model. The anatomical fidelity and the usefulness of the simulator in ESS training were evaluated through specific questionnaires. Results : The tasks were accomplished by 100% of participants and the survey showed overall high scores both for anatomy fidelity and usefulness in training. Dacryocystorhinostomy, medial antrostomy, and turbinectomy were rated as accurately replicable on the simulator by 75% of participants. Positive scores were obtained also for ethmoidectomy and DRAF procedures, while the replication of sphenoidotomy received neutral ratings by half of the participants. Conclusion : This study demonstrates that a 3D printed multi-material model of the sino-nasal anatomy can be generated with a high level of anatomical accuracy and haptic response. This technology has the potential to be useful in surgical training as an alternative or complementary tool to cadaveric dissection.

Abstract:Objectives It has been known for centuries that crystalline silica induces silicosis. However, there are different entities of crystalline silica. Siliceous earth is a worldwide unique blend of amorphous and cryptocrystalline silica and lamellar kaolinite. This retrospective cohort study is the first that investigated the silicosis risk of siliceous earth. Methods The cohort consists of employees exposed to siliceous earth in one company in Bavaria, Germany. Their job activities were collected from personnel files. Workers were included, if they had worked at least one year from the date of recruitment to the end of the study in 2008. The disease data were collected from occupational disease records of the Raw Materials and Chemical Industry Employers’ Liability Insurance Association (BG RCI). Two pneumologists reviewed the radiographic findings independently on the basis of reports of radiological findings according to ILO 2000. Workers with an ILO profusion ≥ 1/1 (p, q, r) were considered to have silicosis. Using the existing 709 measurement records of the years 1961–2008 exposure to silica was assessed for each subject with a job-exposure matrix. To determine the risk of silicosis Poisson regression was performed and to determine the dose-response relationship cubic splines were applied. Results In total 675 persons were enumerated, including 84 women. Among women, there were no silicosis but 55 among men. The evaluation of the exposed men showed a silicosis risk at a cumulative exposure of more than 6 mg/m3xyears (RR = 25; 95% CI 5.3–111.5) and especially in underground mining (RR = 12; 95% CI 4.9–28.6). In addition, a dose-response relationship with increasing cumulative exposure was found. Conclusions We found a dose-response relationship between silicosis and cumulative exposure to siliceous earth. The silicosis risk of siliceous earth is according to these results not higher than that described in the literature for crystalline or cryptocrystalline silica.

Abstract:INTRODUCTION: Patient simulators are increasingly used in the education and training of healthcare professionals. This paper describes the history of human patient simulator development, the features of contemporary simulators, the acquisition of basic and advanced clinical skills using patient simulators, and the benefits, cost, limitations and effectiveness of this innovative learning modality.SIMULATOR DEVELOPMENT: The development of human patient simulators began in the late 1960s, and accelerated in the late 1980s and early 1990s. Several simulator systems are now professionally manufactured, commercially available, and used at hundreds of medical centres, universities and colleges in the USA and throughout the world. Contemporary patient simulators have many clinical features, and look and respond to interventions with ever-increasing degrees of realism because sophisticated physiological and pharmacological models automatically control many features.SIMULATOR USE IN MEDICAL EDUCATION: Simulators are used to teach basic skills, such as respiratory physiology and cardiovascular haemodynamics, and advanced clinical skills, e.g. management of difficult airways, tension pneumothorax, pulmonary embolism and shock.BENEFITS, COSTS AND LIMITATIONS: The simulation laboratory offers distinct educational advantages, especially for learning how to recognise and to treat rare, complex, clinical problems. Costs of simulator-based educational programmes include facility, equipment and personnel. Current limitations include clinical realism of the patient manikin and faculty development.
