Abstract:Abstract Objective : To retrospectively analyze the characteristics and related factors of adjacent segmental disease (ASD) after oblique lumbar interbody fusion (OLIF). Methods : This was a retrospective study of the effect of single- or multiple-segment OLIF combined with Wiltse-approach pedicle screw fixation on adjacent segment. For single-segment (n=92), the preoperative and last follow-up measurements of lumbar lordosis angle (LL), disc degeneration grade(DD), disc height (DH), facet joint degeneration grade (FD), and segmental angle (SA) of adjacent segments were recorded and analyzed statistically. For multiple-segment (n=56), based on the presence of ASD at the final follow-up, they were divided into the ASD group (n = 10) and non-ASD (NASD) group (n = 46). Smoking, hypertension, diabetes, osteoporosis, body mass index (BMI), and preoperative radiological parameters, including DD, FD, DH, and SA of both cranial and caudal adjacent segments, LL, and invasion of the facet joint by pedicle screws were recorded and statistically analyzed in both groups. Results : For single-segment, there was no ASD occurred during the follow-up period. There was a statistically significant difference in the lumbar lordosis angle between preoperative and last follow-up measurements (t=8.2860, P<0.0001), but no statistical difference in the DD, DH, FD, or SA of caudal and cranial segments. For multiple-segment, ASD occurred only in the caudal segment, specifically L5/S1 segment. For the comparison of ASD and NASD, there were statistically significant differences in the preoperative caudal adjacent DD (χ2=12.2500, P=0.0156*) and DH (t=3.6430, P=0.0004*) , age (t=2.1090, P=0.0396), and proportion of patients with osteoporosis (χ2=12.7800, P=0.0003*), but no statistically significant difference in the LL, cranial adjacent DD or DH, and adjacent caudal or cranial FD, SA, proportion of patients with smoking or hypertension or diabetes or invasion of the facet joint. Conclusion : ASD is less likely to occur in the early stage after single-segment OLIF, while ASD after multiple-segment OLIF mainly occurs at the caudal end, especially when the caudal vertebral body of the surgical segment is L5. Age, preoperative caudal adjacent segment disc degeneration, and osteoporosis are potential factors related to ASD after multiple-segment OLIF.

Abstract:We have prepared iron doped TiO2 samples by the impregnation of TiO2 (Degussa—P25) with Fe(III) nitrate as an iron precursor. Obtained materials were then characterized by structural x-ray diffraction (XRD) and optical (Uv-vis, Raman) techniques. Later, we deposited very small gold nanoparticles (AuNPs; <5 nm) onto as-prepared supports by a colloidal deposition method, in which lysine was used as a capping agent and a short sonication time was applied to facilitate the dispersion and deposition of AuNPs. The presence of Fe+3 in the TiO2 structure was confirmed by an increase in the number of oxygen defect sites that react with O2 to form reactive oxygen species. Transmission electron microscopy (TEM) was used to determine the size of the gold particles before and after deposition. The AuNPs deposited on the FeTiO2 exhibited good thermal stability against sintering, even at 500 °C for 4 h.
Pelvic Incidence–Lumbar Lordosis Mismatch Is Predisposed to Adjacent Segment Degeneration After Single-Level Anterior Lumbar Interbody Fusion: A Retrospective Case-Control Study

Abstract:Objective: Spinopelvic parameters play important roles in clinical outcomes and disability after short-segment fusion surgery for degenerative spine disease. This study aimed to investigate the relationship between preoperative or postoperative spinopelvic parameters and symptomatic adjacent segment degeneration (ASD) after single-level anterior lumbar interbody fusion (ALIF) surgery at the L4–5 segments.Methods: All patients who underwent single-level ALIF at the L4–5 level from January 2010 to December 2013 and were followed up for 5 years were analyzed. We collected the degree of degeneration at adjacent segments and preoperative and postoperative spinopelvic parameters. We compared the preoperative and postoperative parameters between patients with and without symptomatic ASD.Results: Sixty-one patients were included in our study, and 30 patients had symptomatic ASD. Degeneration at adjacent segments and preoperative spinopelvic parameters did not affect the occurrence of symptomatic ASD. Patients with symptomatic ASD had higher postoperative pelvic tilt (PT) and a mismatch between lumbar lordosis (LL) and pelvic incidence (PI) compared to those without. Postoperative PT > 15° and PI–LL mismatch > 10° were significant risk factors for symptomatic ASD.Conclusion: High PT and PI–LL mismatch were significant risk factors for symptomatic ASD after single-level ALIF surgery. Spine surgeons should consider achieving proper LL to insert the cage at the appropriate angle and prevent a PI–LL mismatch value > 10° after single-level fusion surgery.

Effects of Lordotic Angle of a Cage on Sagittal Alignment and Clinical Outcome in One Level Posterior Lumbar Interbody Fusion with Pedicle Screw Fixation

Abstract:This study aims to assess the differences in the radiological and clinical results depending on the lordotic angles of the cage in posterior lumbar interbody fusion (PLIF). We reviewed 185 segments which underwent PLIF using two different lordotic angles of 4° and 8° of a polyetheretherketone (PEEK) cage. The segmental lordosis and total lumbar lordosis of the 4° and 8° cage groups were compared preoperatively, as well as on the first postoperative day, 6th and 12th months postoperatively. Clinical assessment was performed using the ODI and the VAS of low back pain. The pre- and immediate postoperative segmental lordosis angles were 12.9° and 12.6° in the 4° group and 12° and 12.0° in the 8° group. Both groups exhibited no significant different segmental lordosis angle and total lumbar lordosis over period and time. However, the total lumbar lordosis significantly increased from six months postoperatively compared with the immediate postoperative day in the 8° group. The ODI and the VAS in both groups had no differences. Cages with different lordotic angles of 4° and 8° showed insignificant results clinically and radiologically in short-level PLIF surgery. Clinical improvements and sagittal alignment recovery were significantly observed in both groups.

Validation of Age-Adjusted Pelvic Incidence Minus Lumbar Lordosis and Lordosis Distribution Index for Assessing Adjacent-Segment Disease after Short-Level Lumbar Fusion Surgery: Minimum 5 Years of Follow-Up.

Abstract:OBJECTIVE:The purpose of this study was to investigate the influence of sagittal alignment according to age-adjusted pelvic incidence minus lumbar lordosis (PI-LL) and lordosis distribution index (LDI) on the occurrence of adjacent-segment disease (ASD) after lumbar fusion surgery.METHODS:This study retrospectively reviewed 234 consecutive patients with lumbar degenerative diseases who underwent 1- or 2-level lumbar fusion surgery. Demographic and radiographic (preoperative and 3-month postoperative) data were collected and compared between ASD and non-ASD groups. Binary logistic regression analysis was performed to evaluate adjusted associations between potential variables and ASD development. A subanalysis was further conducted to assess their relationships in the range of different PI values.RESULTS:With a mean follow-up duration of 70.6 months (range 60-121 months), 118 patients (50.4%) were diagnosed as having cranial radiological ASD. Univariate analyses showed that older age, 2-level fusion, worse preoperative pelvic tilt and LL, lower pre- and postoperative LDI, and more improvement in sagittal vertical axis were significantly correlated with the occurrence of ASD. No significant differences in the PI-LL and age-adjusted PI-LL (offset) were detected between ASD and non-ASD groups. Multivariate analysis identified postoperative LDI (OR 0.971, 95% CI 0.953-0.989, p = 0.002); 2-level fusion (OR 3.477, 95% CI 1.964-6.157, p < 0.001); and improvement of sagittal vertical axis (OR 0.992, 95% CI 0.985-0.998, p = 0.039) as the independent variables for predicting the occurrence of ASD. When stratified by PI, LDI was identified as an independent risk factor in the groups with low and average PI. Lower segmental lordosis (OR 0.841, 95% CI 0.742-0.954, p = 0.007) could significantly increase the incidence of ASD in the patients with high LDI.CONCLUSIONS:Age-adjusted PI-LL may have limited ability to predict the development of ASD. LDI could exert an important effect on diagnosing the occurrence of ASD in the cases with low and average PI, but segmental lordosis was a more significant risk factor than LDI in individuals with high PI.

Abstract:Study design: Retrospective cohort study. Objective: Assess the impact of inadequate correction of L4-S1 lordosis during transforaminal lumber interbody fusion (TLIF) on adjacent segment disease and revision rates. Background: Restoring alignment is an important consideration in spinal fusions. Failure to correct to level-specific alignment goals could promote the development of adjacent segment disease. As such, it is crucial to investigate the role of sagittal segmental alignment on clinical outcomes following short segment lumbar fusion. Methods: Patients who underwent 1- to 3-level transforaminal lumbar interbody fusion (TLIF) and had two-year outcomes data were included in this retrospective cohort study. Segmental lumbar lordosis was assessed in all patients with L3-L4, L4-L5, and L5-S1 constructs. Demographics, radiographic spinopelvic alignment, and complications were compared in a sub-group of patients fused at L4-S1 with adequate (i.e., within 35-45°) and inadequate (i.e., <35°) L4-S1 lordosis. Results: Among the 168 included patients, mean age was 61.7 years, 56.0% were female, and mean follow-up was 32.3 months. Segmental lumbar lordosis did not change significantly after TLIF of L3-L4, L4-L5, or L5-S1 (P>0.05). Two-year postoperatively, 32.7% developed adjacent segment disease and 19.6% underwent revisions. After stratification by adequate (N=15) or inadequate (N=54) restoration of L4-S1 lordosis following initial TLIF surgery, adequately-restored patients had higher preoperative L4-S1 lordosis (Adequately-Restored=39.3° vs Inadequately-Restored=29.5°, P<0.001) and lower two-year postoperative adjacent segment disease (6.7% vs. 33.3%, P=0.032) and revision (5.7% vs. 25.9%, P=0.l33) rates. Adjacent segment disease patients had higher implant-related complications (Adjacent Segment Disease=36.8% vs No Adjacent Segment Disease=8.0%, P=0.008) and subsequent revisions (61.1% vs. 8.7%, P<0.001). Conclusions: Adjacent segment disease and revisions after degenerative lumbar spinal fusion are common. In this cohort, suboptimal restoration of L4-S1 lordosis was associated with higher rates of adjacent segment disease and subsequent revisions, thus highlighting the importance of restoring sagittal alignment in degenerative spine surgery. Abstract:OBJECTIVE This study aimed to investigate the impact of pelvic incidence (PI) and lumbar lordosis (LL) matching on health-related quality of life (HRQOL) outcomes in patients undergoing one- or two-level lumbar fusions for degenerative pathology. The study also examined changes in alignment and HRQOL over a 24-month follow-up period. METHODS A retrospective cohort study used data from a multicenter, prospectively collected database. Radiographic parameters were measured preoperatively and at 3-month and 24-month postoperative time points. Patients were categorized as having alignment (PI-LL ≤ 10°) or malalignment (PI-LL > 10°) at all time points. The Oswestry Disability Index scores were collected at the same time points. Statistical analyses assessed differences in HRQOL scores and radiographic parameters between the aligned and malaligned groups. RESULTS Seventy-six patients were included. Both the aligned and malaligned groups showed improved HRQOL scores after surgery, but patients with proper alignment (PI-LL ≤ 10°) had significantly better HRQOL scores at the 24-month follow-up. Alignment remained stable from 3 months to 24 months postoperatively, with minimal movement between the aligned and malaligned groups. CONCLUSIONS Proper PI-LL matching in one- and two-level lumbar fusions for degenerative pathology leads to improved HRQOL outcomes at the 24-month follow-up. Patients with maintained proper alignment after surgery experience continued improvement in disability levels. Surgeons should consider longer follow-up for patients who do not achieve proper alignment initially, as 24 months is crucial for assessing the consequences of malalignment in short-segment lumbar fusions.

Abstract:Abstract Background: Adjacent segment degeneration (ASD) is a major issue after posterior lumbar interbody fusion (PLIF). Several studies have reported the potential causes of ASD based on radiography. However, the postoperative dynamic changes in the adjacent segments are not clear. This study aimed to determine the effect of PLIF on ASD using a formetric 4D system and to compare the effectiveness of this system with that of traditional radiography for the prediction of ASD. Methods: Eighty-five consecutive patients who underwent PLIF of a single-segmen t were included. The formetric 4Dsystem was used to calculate the relative rotation angle between the fusion segment and the upper and lower adjacent vertebrae preoperatively and at 6, 12 and 24 months postoperatively. The range of motion (ROM) and disc height (DH) of the adjacent segments were measured using radiography preoperatively and at 24 months postoperatively. At the final follow-up, the visual analogue scale (VAS) and Oswestry disability index (ODI) were used to evaluate the surgical outcome. The patients were divided into two groups according to the occurrence of radiographic ASD: the ASD group with progression of degeneration and the N-ASD group without progression of degeneration. The clinical outcomes and measurement data between the two groups were compared and analyzed. Results: The index fusion segments included L2-3 to L5-S1. Preoperatively, the relative rotation angles between the fused segment and the upper and lower adjacent vertebrae were 5.1°±2.2°and 3.3°±2.0°,respectively, and both angles increased significantly at all time-points after surgery ( p <0.05). The angles changed most significantly during L2-3 fusion . Radiographic ASD was noted in 13 of 85 patients (15.3%) at 24 months. There was no significant difference in the DH, ROM, or clinical outcome between the two groups ( p >0.05), while the relative rotation angle with the upper adjacent vertebra was greater in the ASD group than in the N-ASD group ( p <0.05). Conclusion: The relative rotation angle with the adjacent vertebra increased significantly after lumbar fusion surgery. It may be a more sensitive predictor for the development ofradiographic ASD than flexion-extension ROM and DH.
Predictors of Postoperative Segmental and Overall Lumbar Lordosis in Minimally Invasive Transforaminal Lumbar Interbody Fusion: A Consecutive Case Series

Abstract:Study design: Retrospective Case-Series. Objectives: Due to heterogeneity in previous studies, the effect of MI-TLIF on postoperative segmental lordosis (SL) and lumbar lordosis (LL) remains unclear. Therefore, we aim to identify radiographic factors associated with lordosis after surgery in a homogenous series of MI-TLIF patients. Methods: A single-center retrospective review identified consecutive patients who underwent single-level MI-TLIF for grade 1 degenerative spondylolisthesis from 2015-2020. All surgeries underwent unilateral facetectomies and a contralateral facet release with expandable interbody cages. PROs included the ODI and NRS-BP for low-back pain. Radiographic measures included SL, disc height, percent spondylolisthesis, cage positioning, LL, PI-LL mismatch, sacral-slope, and pelvic-tilt. Surgeries were considered "lordosing" if the change in postoperative SL was ≥ +4° and "kyphosing" if ≤ -4°. Predictors of change in SL/LL were evaluated using Pearson's correlation and multivariable regression. Results: A total of 73 patients with an average follow-up of 22.5 (range 12-61) months were included. Patients experienced significant improvements in ODI (29% ± 22% improvement, P < .001) and NRS-BP (3.3 ± 3 point improvement, P < .001). There was a significant increase in mean SL (Δ3.43° ± 4.37°, P < .001) while LL (Δ0.17° ± 6.98°, P > .05) remained stable. Thirty-eight (52%) patients experienced lordosing MI-TLIFs, compared to 4 (5%) kyphosing and 31 (43%) neutral MI-TLIFs. A lower preoperative SL and more anterior cage placement were associated with the greatest improvement in SL (β = -.45° P = .001, β = 15.06° P < .001, respectively). Conclusions: In our series, the majority of patients experienced lordosing or neutral MI-TLIFs (n = 69, 95%). Preoperative radiographic alignment and anterior cage placement were significantly associated with target SL following MI-TLIF.

Abstract:Objective: To investigate the correlation of spinopelvic alignment according to Roussouly classification on the occurrence of adjacent segment disease (ASD) in the patients undergoing fusion surgery for lumbar degenerative diseases. Methods: A cross-sectional study. Clinical data of 166 consecutive patients who had undergone lumbar fusion between January 2009 and January 2019 in the Affiliated Changzhou Second People's Hospital of Nanjing Medical University and Affiliated Drum Tower Hospital of Medical School of Nanjing University were retrospectively reviewed. There were 59 males and 107 females, with an average age of (58.6±9.5) years (ranged 41-78 years). Fusion length averagely spanned (1.7±0.7) levels (ranged 1-3 levels). The patients were classified by both "theoretical" (based on pelvic incidence (PI)) and "current"(based on sacral slope (SS)) Roussouly types. The patients were classified as "matched" if their "current" shape matched the "theoretical" type and otherwise as "unmatched". Multivariate logistic regression analysis of the variables recruited from univariate analyses was performed to identify the factors independently associated with the development of ASD after lumbar fusion. Results: The average follow-up duration after initial surgery was (49.2±20.7) months (ranged 25 to 134 months). Thirty (18.1%, 30/166) patients were diagnosed as ASD. Postoperatively, two thirds of the patients who suffered ASD after surgery were unmatched, while 36.8% (50/136) of the patients without ASD had unmatched type. Univariate analyses showed that older age, more fusion levels, float fusion, pre-and postoperative worse spinopelvic alignment, and postoperative unmatched Roussouly type were identified as risk factors of ASD. Multivariate logistic regression analysis identified postoperative Roussouly type mismatch (OR=3.310, 95%CI: 1.282-8.545, P=0.013), old age (OR=1.074, 95%CI: 1.019-1.131, P=0.008) and postoperative SS (OR=0.928, 95%CI: 0.865-0.995, P=0.036) as the independent risk factors of development of ASD after lumbar fusion. Conclusion: A significant association between postoperative sagittal malalignment and occurrence of ASD is detected, the evaluation of sagittal alignment by Roussouly classification could help predict the occurrence of ASD.

Abstract:BackgroundThe purpose of the current study was to investigate the long-term clinical outcomes of this technique for degenerative scoliosis (DS). MethodsThe records of 95 consecutive patients with DS who underwent selective segmental transforaminal interbody fusion combined with posterior-instrumented spinal fusion in our department from January 1999 to December 2007 were analysed retrospectively. Average follow-up was 7.8 years. Recorded clinical outcomes included Oswestry Disability Index (ODI), visual analogue scale (VAS) pain scores and overall patient satisfaction. Radiographic measurements included coronal Cobb angle, apical vertebra translation, Nash-Moe grade, lumbar lordosis (LL) and thoracolumbar kyphosis. Comparison of the clinical and radiographic parameters before surgery and at final follow-up was studied. Linear correlation analysis was applied to analyse the relationship between the clinical and radiological results. ResultsAverage ODI and VAS pain scores were significantly improved at final follow-up compared with baseline (P = 0.038; P = 0.005). Specifically, the average ODI score was 32.2 8.6 before surgery and 11.1 +/- 6.8 at final follow-up; the average VAS score was 8.9 +/- 2.0 before surgery and 2.0 +/- 1.2 at final follow-up; patient satisfaction was 88.2% (84/95) at final follow-up. In addition, Cobb angle, apical vertebra translation and Nash-Moe grade were all statistically significantly decreased compared with preoperative values (P = 0.019; P = 0.035; P = 0.001). Although LL had significantly increased (P = 0.022), thoracolumbar kyphosis did not exhibit a significant change (P = 0.64). There was significant correlation between LL and decreased ODI scores (r = 0.62, P = 0.01). Eleven patients (11.6%) underwent reoperation during the study period. ConclusionSelective segmental transforaminal interbody fusion combined with posterior-instrumented spinal fusion appears to have reasonable long-term clinical and radiographic outcomes for the treatment of DS.

Abstract:This is a retrospective cohort study, the purpose of this study was to compare the radiographic and clinical functional improvement of L4‐L5 lumbar degenerative spondylolisthesis (LDS) patients with anterior cage implantation and posterior cage implantation. This study reviewed 128 patients with L4‐L5 LDS between January 2016 and August 2019. The lumbar lordosis (LL), segmental lordosis (SL), sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT) and slope degree (SD) was measured for radiographic outcomes. We used the visual analog scale (VAS) and the oswestry disability index (ODI) score to assess functional outcomes. The LL, SL, PT, SS, SD and PI‐LL after operation showed great improvements, relative to the corresponding preoperative values in both groups. Compared to Group P, Group A exhibited far enhanced SL, LL outcomes and showed depressed PI‐LL outcomes at the final follow‐up. Meanwhile, the SL in Group A 1 months after operation were also higher than in Group P. We found that VAS and ODI at the final follow‐up in Group A were significantly lower than those in Group P. For patients with LDS, the anteriorly placed cage may provide better improvement of SL after PLIF surgery. Meanwhile, the anteriorly placed cage may achieve better sagittal balance and functional outcomes in the long run. Methods This study reviewed 128 patients with L4‐L5 LDS between January 2016 and August 2019. All patients underwent an open PLIF surgery that included intravertebral decompression, implantation of pedicle screws and cage. We grouped according to the position of the center of the cage relative to the L5 vertebral endplate. Patients with the center of the cage located at the anterior 1/2 of the upper end plate of the L5 vertebral body were divided into Anterior group, and located at the posterior 1/2 of the upper end plate of the L5 vertebral body were divided into Posterior group. The lumbar lordosis (LL), segmental lordosis (SL), sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT) and slope degree (SD) was measured for radiographic outcomes. We used the visual analog scale (VAS) and the oswestry disability index (ODI) score to assess functional outcomes. Paired t‐test was used to compare imaging and bedside data before and after surgery between the two groups, and independent sample t‐test, χ2 test and Fisher exact test were used to compare the data between the two groups. Result The mean follow‐up of Anterior group was 44.13 ± 9.23 months, and Posterior group was 45.62 ± 10.29 months (P > 0.05). Oblique Lumbar Interbody Fusion with Stand-Alone Cages for the Treatment of Degenerative Lumbar Spondylolisthesis: A Retrospective Study with 1-Year Follow-Up

Abstract:Patients with degenerative lumbar spondylolisthesis (DLS) often suffer from years of low back pain (LBP) due to instability of the lumbar spine and the reduction of disc height. Since January 2016, we have performed oblique lateral interbody fusion (OLIF) on 154 patients. Among these, 56 patients who suffered from DLS underwent OLIF with stand-alone cages. Forty-two patients with a follow-up time that exceeded 1-year were enrolled for this study. The forty-two patients were followed up for at least one year. Operation segments ranged from L3-4 to L4-5. All the patients were with 1-level fusion. The mean postoperative ventral-disc height and dorsal-disc height increased significantly compared with preoperative (P<0.05). A significant postoperative increase was also observed in the mean operative segmental lordotic angle and the whole lumbar lordotic angle (P<0.05). Compared with preoperative, the postoperative VAS significantly decreased with no significant increase in the VAS in the last follow-up. The LBP was significantly relieved. The mean postoperative VAS of LBP decreased significantly compared with the preoperative ((1.6 ± 0.8) vs. (7.8 ± 0.8)). Postoperative complications included psoas major abscess and intervertebral space infection (1/56). Except for one patient whose cage subsided during the last follow-up, the other patients had good cage position. The one whose cage collapsed complained no symptoms including LBP. OLIF with stand-alone cages should be considered as a safe and effective option which can effectively alleviate LBP for the treatment of DLS.

Abstract:The aim of this work was to assess the value of the physical status of human papillomavirus (HPV) DNA as a disease marker for cervical cancer development in a set of 248 DNA samples previously genotyped as HPV 16 or 18, by calculating the E2/E6 ratio through real-time PCR. There was a significant difference in integration status according to disease grade for both genotypes (p <0.001). Furthermore, especially for HPV 18, determining the DNA physical status could be a useful biomarker in predicting cervical cancer risk development, with a lower E2/E6 ratio clinically associated with the development of a precancerous lesion.

Abstract:STUDY DESIGN: Prospective study.PURPOSE: The goal of this study was to evaluate the impact of anterior lumbar interbody fusion (ALIF) on L5-S1 level for restitution of distal segmental lordosis and to investigate its consequences on spino-pelvic parameters and the global sagittal balance.OVERVIEW OF LITERATURE: Lumbar surgery must be adapted to the spinal morphology in order to restore an adequate relation between pelvic and spinal parameters and especially to the pelvic incidence.METHODS: An observational, prospective study was conducted between January 2013 and May 2017. Eighty-six patients were treated by L5-S1 ALIF procedure regardless of disc replacement above L5-S1 level. Thirty-seven patients were included and subset analyses were performed on 25 patients operated on an isolated ALIF L5-S1 (group 1), and 12 patients with hybrid surgery consisting of an L5- S1 ALIF procedure and a L4-L5 lumbar disc replacement (group 2). Clinical parameters were analyzed using Visual Analog Scale (VAS) and Oswestry Disability Index (ODI) at M0 (preoperative) and M12 (12 months). Lumbo-pelvic parameters were assessed on a standing full-spine X-ray, preoperatively and at M12 after surgery.RESULTS: We observed a significant evolution of L1-S1 lumbar lordosis (p <0.001) with a significant increase of the distal arch L4-S1 lordosis (p <0.001) and decrease of the proximal arch lordosis (p =0.03). Preoperatively, 27% of the patients were unbalanced. Significant variation in sagittal balance parameters was observed, with a decrease of the sagittal vertebral axis (p <0.001). VAS and ODI improved significantly but no correlation was found. An evolution in the same direction was found in the two subgroup analyses.CONCLUSIONS: ALIF procedure on L5-S1 level allowed a reconstruction of lumbosacral segmental lordosis, modification of global lordosis, without variation of spino-pelvic parameters except an improvement in sagittal balance.

Abstract:Background Context Lateral lumbar interbody fusion (LLIF) is an effective technique for fusion and sagittal alignment correction/maintenance. Studies have investigated the impact on the segmental angle and lumbar lordosis (and pelvic incidence-lumbar lordosis mismatch), however not much is documented regarding the immediate compensation of the adjacent angles. Purpose To evaluate acute adjacent and segmental angle as well as lumbar lordosis changes in patients undergoing a L3-4 or L4-5 lateral lumbar interbody fusion for degenerative pathology. Study Design/Setting Retrospective cohort study Patient Sample Patients included in this study were analyzed pre- and post-LLIF performed by one of three fellowship-trained spine surgeons, 6 months following surgery. Outcome Measures Patient demographics (including body mass index, diabetes diagnosis, age, and sex) as well as VAS and ODI scores were measured. Lateral lumbar radiograph parameters: lumbar lordosis (LL), segmental lordosis (SL), infra and supra-adjacent segmental angle, and pelvic incidence (PI). Methods Multiple regressions were applied for the main hypothesis tests. We examined any interactive effects at each operative level and used the 95% confidence intervals to determine significance: a confidence interval excluding zero indicates a significant effect. Results We identified 84 patients who underwent a single level LLIF (61 at L4-5, 23 at L3-4). For both the overall sample and at each operative level, the operative segmental angle was significantly more lordotic post-op compared to pre-op (all ps ≤ .01). Adjacent segmental angles were significantly less lordotic post-op compared to pre-op overall (p = .001). For the overall sample, greater lordotic change at the operative segment led to more compensatory reduction of lordosis at the supra-adjacent segment. At L4-5, more lordotic change at the operative segment led to more compensatory lordosis reduction at the infra-adjacent segment. Conclusion The present study demonstrated that LLIF resulted in significant increase in operative level lordosis and a compensatory decrease in supra- and infra-adjacent level lordosis, and subsequently no significant impact on spinopelvic mismatch.

Abstract:BACKGROUND: Adjacent segment disease (ASD) is a common problem after lumbar spinal fusions. Ways to reduce the rates of ASD are highly sought after to reduce the need for reoperation.OBJECTIVE: To find predisposing factors of ASD after lumbar interbody fusions, especially in mismatch of pelvic incidence and lumbar lordosis (PI-LL).METHODS: We conducted a retrospective cohort study of all patients undergoing lumbar interbody fusions of less than 4 levels from June 2015 to July 2020 with at least 1 year of follow-up and in those who had obtained postoperative standing X-rays.RESULTS: We found 243 patients who fit inclusion and exclusion criteria. Fourteen patients (5.8%) developed ASD, at a median of 24 months. Postoperative lumbar lordosis was significantly higher in the non-ASD cohort (median 46.4° ± 1.4° vs 36.9° ± 3.6°, P < .001), pelvic tilt was significantly lower in the non-ASD cohort (16.0° ± 0.66° vs 20.3° ± 2.4°, P = .002), PI-LL mismatch was significantly lower in the non-ASD cohort (5.28° ± 1.0° vs 17.1° ± 2.0°, P < .001), and age-appropriate PI-LL mismatch was less common in the non-ASD cohort (34 patients [14.8%] vs 13 [92.9%] of patients with high mismatch, P < .001). Using multivariate analysis, greater PI-LL mismatch was predictive of ASD (95% odds ratio CI = 1.393-2.458, P < .001) and age-appropriate PI-LL mismatch was predictive of ASD (95% odds ratio CI = 10.8-970.4, P < .001).CONCLUSION: Higher PI-LL mismatch, both age-independent and when adjusted for age, after lumbar interbody fusion was predictive for developing ASD. In lumbar degenerative disease, correction of spinopelvic parameters should be a main goal of surgical correction.

Abstract:Background: An abnormal postoperative lordosis distribution index (LDI), which quantifies the ratio between the lordosis at L4 to S1 and the lordosis at L1 to S1, contributes to the development of adjacent segment disease and increased revision rates in patients undergoing short-segment lumbar intervertebral fusions. Incorporating preoperative spinopelvic parameters and LDI into the surgical plan for short-segment fusion is important for guiding alignment restoration and preserving normal preoperative alignment in unfused segments. This study examined changes in LDI, segmental lordosis, and lordosis of the unfused levels in patients treated with personalized interbody cage (PIC) implants. Methods: This retrospective study evaluated radiographic measurements from 111 consecutively treated patients diagnosed with degenerative spinal conditions and treated with a short-segment fusion of L4 to L5, L5 to S1, or L4 to S1 using PIC implant(s) within 6 months of the fusion procedure. Comparisons of intervertebral lordosis for treated and untreated levels as well as LDI pre- and postoperatively were performed. Results: In patients with a preoperative hypolordotic distribution (LDI < 50%), statistically significant increases were found in LDI postoperatively, approaching the normal LDI range (LDI 50%-80%). Likewise, patients with hyperlordotic distribution preoperatively (LDI > 80%) experienced a decrease in LDI postoperatively, trending toward the normal range, although the changes were not statistically significant. Intervertebral lordosis for the L5 to S1 level increased significantly following the placement of a PIC in the normal and hypolordotic LDI groups. Changes in intervertebral lordosis for L5 to S1 were not significant for patients with preoperative hyperlordotic LDI. Reciprocal changes in intervertebral lordosis at L1 to L4 were not observed in any groups. Conclusions: PIC implants may provide a benefit for patients, particularly those with hypolordotic distributions preoperatively. They have the potential to further improve patient outcomes by helping surgeons to achieve patient-specific lordosis goals, which may help to reduce the risk of adjacent segment disease and revisions in patients undergoing short-segment lumbar intervertebral fusions. Clinical relevance: Personalized implants can help surgeons achieve patient-specific alignment goals, potentially prevent adjacent segment disease, and reduce long-term reinterventions.
