Abstract:Objective:To evaluate the related factors of dysphagia following anterior cervical decompression and fusion.Method:490 cases undergoing anterior discectomy decompression and fusion from October 2002 to October 2004 were followed up,of these,there were 306 males and 184 females,with the mean age of 47.2 years(range,12 to 76 years).Of these 490 patients,415 had cervical spondylosis,43 had cervical fracture or dislocation,22 had cervical disc herniation,8 had cervical spine tumor,and 2 had cervical spine tuberculosis.490 patients were reviewed retrospectively postoperatively about their swallow dysfunction(objective)and swallow comfortlessness(subjective).All cases with dysphgia accepted dexamethasone used intravenously and correspondant therapy.Result:490 cases were followed up,and 96(19.6%)of them had postoperative dysphagia which lasted for 3 weeks to 2 years.Of 96 patients with dysphagia,with the mean age of 57.6 yeas(range,38 to 63 years).Of 96 patients with dysphagia,89 cases had titanium plates placement,7 cases had intervertebral cages placement.Those without plates or cages placement and those with artificial disc placement had no postoperative dysphagia.Female,elder and titanium plates planting was associated with postoperative dysphagia.After 14.2 months follow-up,90 cases(93.7%)with dysphagia cured or improved,while 6 cases had no improvement.Conclusion:Postoperative dysphagia is a common complication associated with anterior cervical discectomy and fusion,the causemay be multifactory.As a result,clinicians should pay more attention to this complication and make proper intervention.

The results of our study indicated that the rate of dysphagia at 0, 3, 6, 12, and 24 months after surgery was 20%, 5.4%, 2.4%, 1.1%, and 0.4%, respectively. Our results showed that age (58.8 years old), BMI (27.3kg/m(2)), course of disease (11.6 months), operation time (103.2 min), blood loss (151.6 mL), incision length (9.1 cm), cervical circumference (46.8 cm), angle of C2 to C7 (15.3 degrees), cervical circumference/height of C2 to C7 (4.8), preoperative VAS (7.5), and ODI (0.6) in dysphagia group were significantly higher than those (52.0, 24.6, 8.6, 88.2, 121.6, 8.6, 42.3, 12.6, 3.7, 5.6, and 0.4, respectively) in nondysphagia group; however, height of C2 to C7 (9.9 vs 11.7cm) and preoperative JOA (8.3 vs 10.7) had opposite trend between 2 groups. We could also infer that female, smoking, diabetes, ossification of posterior longitudinal ligament, ACCDF, multilevel surgery, and superior fusion segment including C2 to C3 or C6 to C7 were the risk factors for dysphagia after surgery immediately. However, till 2 years after surgery, only 2 risk factors, smoking and diabetes, could slow rehabilitation of dysphagia. Many factors could significantly increase rate of dysphagia after anterior cervical surgery. Operation time as a vital factor markedly increases immediate postoperative dysphagia and smoking, as the most important factor, lower recovery of dysphagia. Further study is needed to prove if these factors could influence dysphagia.
Risk Factors for Dysphagia in Patients with Acute and Chronic Ischemic Stroke: A Retrospective Cohort Study

Abstract:Background: Dysphagia is a common and clinically significant complication of ischemic stroke. The prevalence and risk factors for dysphagia may vary at different stages following an ischemic stroke. Methods: This study included patients with acute and chronic ischemic stroke who were treated at the Department of Rehabilitation, First Affiliated Hospital, Zhejiang University School of Medicine from 2019 to 2022. Various demographic, clinical, and laboratory parameters were collected, and statistical analyses were performed to investigate their association with dysphagia. Results: Among the 399 ischemic stroke patients included in the study, 165 (41.4 %) experienced dysphagia, with 72 (38.7 %) in the acute phase and 93 (43.7 %) in the chronic phase. Univariate analysis revealed significant associations (p < 0.05) between dysphagia and factors such as pulmonary infection, aphasia, NIHSS score, ADL score, NLR score, lower extremity Brunnstrom's stages, and sit-to-stand balance. Multiple logistic regression analysis, after adjusting for confounding factors, identified the ADL score as an independent predictor of dysphagia. These findings were consistent across three time-windows: the acute phase, the chronic phase, and 180 days after stroke onset. Additionally, the lymphocyte count and pulmonary infection were identified as potential independent indicators. Conclusions: This study investigated the prevalence and risk factors for dysphagia in ischemic stroke patients at different time-windows. A low ADL score (<40) may serve as a valuable and reliable predictor for poststroke dysphagia in clinical settings.

Abstract:Background: Dysphagia is a well-known complication following anterior cervical spine surgery. Although risk factors for dysphagia have been reported in the literature, they still remain controversial. This study aims to investigate the risk factors associated with dysphagia following anterior cervical spinal surgery. Methods: PubMed, EMBASE, and The Cochrane Library were searched up to June 2016 for studies examining dysphagia following anterior cervical spinal surgery. Risk factors associated with dysphagia were extracted. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated for outcomes. Data analysis was conducted with RevMan 5.3 and STATA 12.0. Results: The final analysis includes a total of 18 distinct studies. The pooled analysis reveals that there are significant differences in female gender (OR=2.30, 95% CI: 1.76-2.99, P<0.001), the use of anterior cervical plate (OR=1.66, 95% CI: 1.05-2.62, P=0.03), more than 1 surgical level (OR=2.07, 95% CI: 1.62-2.66, P<0.001), the upper surgical level at C3/4 (OR=3.08, 95% CI: 1.44-6.55, P=0.004), and the use of bone morphogenetic protein-2 (rhBMP-2) (OR=5.52, 95% CI: 2.16-14.10, P<0.001). However, no significant difference is found in revision surgery (OR=1.67, 95% CI: 0.60-4.68, P=0.33), the type of fusion (OR=1.02, 95% CI: 0.62-1.67, P=0.95), and cervical disc arthroplasty (OR=1.37, 95% CI: 0.75-2.51, P=0.30). Conclusion: Female gender, the use of anterior cervical plate, more than 1 surgical level, the upper surgical level at C3/4, and the use of rhBMP-2 are the risk factors for dysphagia following anterior cervical spinal surgery. However, revision surgery, the type of fusion, and cervical disc arthroplasty are unassociated with dysphagia. Considering the limited number of studies, this conclusion should be interpreted cautiously, and larger scale studies are required.

Abstract:Study Design: Prospective multi-center cohort study Objective: To explore the association between operative level and postoperative dysphagia after anterior cervical discectomy and fusion (ACDF). Summary of Background Data: Dysphagia is common following ACDF and has several risk factors including soft tissue edema. The degree of prevertebral soft tissue edema varies based upon the operative cervical level. However, the operative level has not been evaluated as a source of postoperative dysphagia. Methods: Adult patients undergoing elective ACDF were prospectively enrolled at three academic centers. Dysphagia was assessed using the Bazaz questionnaire, Dysphagia Short Questionnaire (DSQ), and Eating Assessment Tool-10 (EAT-10) preoperatively and at 2, 6, 12, and 24-weeks postoperatively. Patients were grouped based on inclusion of specific surgical levels in the fusion construct. Multivariable regression analyses were performed evaluating the independent effects of number of surgical levels and inclusion of each particular level on dysphagia symptoms. Results: A total of 130 patients were included. Overall, 24 (18.5%) patients had persistent postoperative dysphagia at 24 weeks and were older, female, and less likely to be drink alcohol. There was no difference in operative duration or dexamethasone administration. Patients with persistent dysphagia were significantly more likely to have C4-C5 included in the fusion construct (62.5% vs. 34.9%, P =0.024) but there were no differences based on inclusion of other levels. On multivariable regression, inclusion of C3-C4 or C6-C7 were associated with more severe EAT-10 (β:9.56, P =0.016 and β:8.15, P =0.040) and DSQ (β:4.44, P =0.023 and (β:4.27, P =0.030) at 6 weeks. At 12-weeks, C3-C4 fusion was also independently associated with more severe dysphagia (EAT-10 β:4.74, P =0.024). Conclusion: The location of prevertebral soft tissue swelling may impact the duration and severity of patient-reported dysphagia outcomes at up to 24 weeks postoperatively. In particular, inclusion of C3-C4 and C4-C5 into the fusion may be associated with dysphagia severity.

Abstract:PurposeTo conduct a systematic review of literature to determine risk factors and preventative measures of early and persistent dysphagia after anterior cervical spine surgery (ACSS).MethodsOn March 2017, we searched the database PubMed, Medline, EMBASE, the Cochrane library, Clinical key, Springer link and Wiley Online Library without time restriction using the term ‘dysphagia’, ‘swallowing disorders’, and ‘anterior cervical spine surgery’. Selected papers were examined for the level of evidence by published guidelines as level I, level II, level III, level IV studies. We investigated risk factors and preventative measures of early or persistent dysphagia after ACSS from these papers.ResultsThe initial search yielded 515 citations. Fifty-nine of these studies met the inclusion and exclusion criteria. Three of them were level I evidence studies, 29 were level II evidence studies, 22 were level III evidence studies, and 3 were level IV evidence studies. Preventable risk factors included prolonged operative time, use of rhBMP, endotracheal tube cuff pressure, cervical plate type and position, dC2–C7 angle, psychiatric factors, tobacco usage, prevertebral soft tissue swelling, SLN or RLN palsy or injury of branches. Preventative measures included preoperative tracheal traction exercise, maintaining endotracheal tube cuff pressure at 20 mm Hg, avoiding routine use of rhBMP-2, use of zero-profile implant, use of Zephir plate, use of new cervical retractor, steroid application, avoiding prolonged operating time, avoiding overenlargement of cervical lordosis, decreasing surgical levels, ensuring knowledge of anatomy of superior laryngeal nerve and recurrent laryngeal nerve, to comfort always, patients quitting smoking and doctors ensuring improved skills. Unpreventable risk factors included age, gender, multilevel surgery, revision surgery, duration of preexisting pain, BMI, blood loss, upper levels, preoperative comorbidities and surgical type.ConclusionAdequate preoperative preparation of the patients including preoperative tracheal traction exercise and quitting smoking, proper preventative measures during surgery including maintaining endotracheal tube cuff pressure at 20 mm Hg, avoiding routine use of rhBMP-2, use of zero-profile implant, use of Zephir plate, use of new cervical retractor, steroid application, avoiding prolonged operating time, avoiding overenlargement of cervical lordosis and decreasing surgical levels, doctors ensuring knowledge of anatomy, improved surgical techniques and to comfort always are essential for preventing early and persistent dysphagia after ACSS.

Abstract:Objective To analyze the incidence and risk factors of dysphagia after anterior and posterior cervical spine surgeries( AC and PC procedures). Methods Three hundred and fifty-four patients were reviewed in this study,including 172 patients undergoing AC procedure( AC group),and 182 patients with PC procedure( PC group). All patients were followed up for 28 months. The presence and severity of dysphagia were reported on the first day of surgery and at 1,3,6 and 12 months after the procedure. The incidence and risk factors of dysphagia after cervical spine surgery were studied by using univariate and binary Logistic regression model analysis. Results The incidence and risk factors of dysphagia after cervical spine surgery at different time points were not statistically significant between the two groups( all P 0. 05).The univariate and binary Logistic regression model analysis showed that the risk factors such as age,gender,BMI,operative time,blood loss,procedure type,revision surgery,most cephalic operative level and number of operative levels did not significantly influence the incidence of postoperative dysphagia in the two groups( all P 0. 05). Conclusions Postoperative dysphagia is common after anterior and posterior cervical spine surgery. The incidence and severity of dysphagia both decrease over time. Age,gender,BMI,operative time,blood loss,procedure type,revision surgery,most cephalic operative level and number of operative levels may not be risk factors of postoperative dysphagia.

Abstract:Some details of the uhrastructure of the syncytial stomach cell of the rotifer have been presented by Lansing (1). He has described the luminal membrane of the stomach cell as consisting of a thick reticular layer beneath a unit membrane 60 A thick. He noted a similarity between this layer and the terminal web of the intestinal epithelium. Lansing further observed goblet-shaped pockets in the gastric luminal surface. Ingested colloidal gold particles were observed in these pockets and in cytoplasmic food vacuoles which led Lansing to postulate that the gold particles entered the cell cytoplasm through the gobletlike structures. This communication will present details of a fine "crystalline" organization of a region immediately surrounding the gobleflike projection. A Philodina species was selected from a mixed rotifer culture obtained from Turtox Products General Biological Supply House, Inc. Organisms were fixed in a contracted state by 1% osmium tetroxide buffered to pH 7.5 in McEwen's saline (2). Some of these rotifers were prefixed in 6% glutaraldehyde and postfixed in osmium tetroxide. In order to fix rotifers in an extended state, some animals were "paralyzed" in 0.5% neosynephrine (3), and subsequently fixed in 2 % unbuffered osmium tetroxide. It has been somewhat more difficult to obtain details of ultrastructure with the last procedure as compared to the former methods; however, fixation in extension is desirable for orientation purposes. All fixed specimens were dehydrated in ethanol and embedded in Maraglas (4). Thin sections were cut on a Porter Blum microtome, stained with lead citrate (5), and examined in an RCA EMU3E. Fig. 1 depicts an oblique section through the lumen of the syncytial stomach of a rotifer. The lumen of the gut contains cilia, and it is bounded by a unit membrane (Fig. 2) as described by Lansing (1). Beneath the membrane is a relatively homogeneous region which stains rather lightly. In cross-sections of the gut (Fig. 2), this region appears finely granular or amorphous, whereas in longitudinal sections (Fig. 1) it appears to consist of thin fibers approximately 30 A in diameter. In keeping with the terminology employed by Lansing (1), this area will be called the terminal web. Scattered within the terminal web are the roughly cylindrical "goblet-shaped" bodies described by Lansing (1). Their appearance is somewhat variable. Some appear to open into the stomach lumen (Fig. 1), whereas others appear separated from the lumen by one or two unit membranes (Fig. 2). Still others appear to be extruding small vacuoles into the syncytial cytoplasm by pinocytosis, these vacuoles apparently being food vacuoles, also described by Lansing (1) (Fig. 2). Surrounding the goblet-shaped vacuoles is a very densely staining cylindrical collar of material which, at high magnification, can be resolved into elements periodically spaced at 50 to 150 A (Figs.

Abstract:Anterior cervical discectomy and fusion (ACDF) is a widely performed surgical procedure for treating cervical disease. [ 1 , 2 ] Cervical disc herniation is a degenerative disease of the cervical spine in which a protruded disc compresses the exiting nerve and incurs segmental instability. ACDF via an anterior approach to the neck would be the most commonly implemented surgical option for this cervical spinal condition. In addition to common postoperative complications such as bleeding, infection, and pseudoarthrosis after ACDF, damage such as laryngeal nerve, muscle, soft tissue damage, and dysphagia may result from the anterior approach. [ 3 ] Dysphagia is a disabling condition that may have a detrimental effect on quality of life. The incidence of dysphagia varies among studies owing to differences in the surgical method, individual patient characteristics, evaluation method, and follow-up period. In a previous report, the percentage of patients who experienced difficulty in swallowing immediately after ACDF surgery was relatively high. [ 4 ] The rate of dysphagia reported after surgery ranges from 1% to 79%, but in most cases, these symptoms are in the short term and tend to improve naturally over time. [ 4 ] Despite the limited information available on rehabilitative strategies for treating dysphagia following ACDF, this case report aims to evaluate and recommend an effective rehabilitation strategy for patients experiencing post-ACDF dysphagia.

Manual preoperative tracheal retraction exercise decreases the occurrence of postoperative oropharyngeal dysphagia after anterior cervical discectomy and fusion

Abstract:Objective: Preoperative tracheal retraction exercise (TRE) to minimize the occurrence of postoperative oropharyngeal dysphagia after anterior cervical spine surgery. Methods: A total of 220 patients admitted for elective anterior cervical spine surgery from January 2013 to December 2014 were retrospectively reviewed. The patients were allocated into two groups: TRE group and control group (without TRE). Modified dysphagia scoring system (MDSS) was used for evaluating the presence and severity of dysphagia symptoms at 1 week and 1, 3, and 6 months after surgery. Demographics such as age, gender, smoking, type of procedure, number of levels operated, duration of surgery, intraoperative blood loss, and instrumentation were analyzed. The clinical outcomes in both groups were compared with Neck Disability Index (NDI), Visual Analogue Scale (VAS) for arm and neck pain, and Odom's criteria for global outcome. Results: In the first week postoperatively, 86 patients (39.1%) developed dysphagia, which decreased to 72 (32.7%), 5 (2.3%), and 4 (1.8%) after 1, 3, and 6 months, respectively. The patients who received the TRE prior to surgery had significantly better MDSS scores ( p = 0.032 for second-level, 0.022 for third-level, and 0.009 for fourth-level fusions) than control group patients who did not receive TRE at the first week of surgery. At the 1-month follow-up, the followed-up patients for second- to fourth-level fusions in the TRE group had improved MDSS scores than those in the control group ( p = 0.041 for second-level, 0.025 for third-level, and 0.0011 for fourth-level fusions). MDSS scores showed no significant difference between both the groups at 1 and 3 months postoperatively for single level anterior cervical fusion. NDI and VAS scores didn't yield any significant difference. Global outcome by Odom's criteria was 88.6%. Conclusion: Preoperative TRE can significantly reduce the occurrence of postoperative dysphagia after ACDF surgery. During follow-up, the incidence of postoperative dysphagia was significantly lower and had resolved at 3 months in all patients.

Abstract:INTRODUCTION: Surgical management of Cervical Spondylotic Myelopathy (CSM) is associated with complications with devastating impacts. Dysphagia is one of these major complications with the incidence rate being not completely known in different surgical approaches. METHODS: PubMed/MEDLINE, SCOPUS, and Cochrane were systematically searched until January 2023. Rates and risk of complications were assessed. Meta-analysis of rates and risks was performed with generalized linear mixed model (GLMM) and inverse variance method, respectively, and compared between techniques. RESULTS: 1196 citations were screened and 39 studies with 60 instances of dysphagia incidence/risk factors reports from over 132,442 patients were identified. Dysphagia’s overall incidence rate was 4.98%[95% confidence interval 3.23-7.61] and was significantly different between surgery groups (Chi2 p < 0.01). It was found in 7.97% [4.95–12.57] of anterior surgeries, 1.52% [0.94–2.45] of posterior ones, and 11.25% [3.38–31.44] of hybrid techniques (statistical comparison: P < A 65) risked dysphagia significantly (OR 1.38 [95%CI 1.11-1.71]). CONCLUSIONS: Chances of posterior surgery being complicated with dysphagia is not negligible and should be discussed with patient before surgery, especially with older adults.

A comparison of dysphagia rates between long-segment anterior versus posterior cervical fusion

Abstract:Objective: The goal of this study was to compare rates of dysphagia and patient-reported outcomes (PROs) following long-segment (≥ 3 levels) anterior cervical spinal fusion (ACF) and posterior cervical spinal fusion (PCF) at 3 and 12 months postoperatively. PROs were also compared for patients with dysphagia versus those without dysphagia. Methods: A prospectively collected quality improvement database was used to identify patients who had a long-segment cervical spinal fusion. Cohorts were divided into ACF and PCF groups. Eating Assessment Tool-10 scores and PROs were obtained for all patients preoperatively and at 3 and 12 months postoperatively to compare. Multivariate analysis was also performed to evaluate risk factors for dysphagia. Results: A total of 132 patients met the inclusion criteria, 77 of whom had undergone ACF and 55 of whom had undergone PCF. Dysphagia rates between ACF and PCF cohorts were similar at baseline (13.0% vs 18.2%, p = 0.4). New-onset dysphagia rates were also comparable at 3-month follow-up (39.7% vs 23.1%, p = 0.08) and 12-month follow-up (32.6% vs 32.4%, p > 0.99). Patients who underwent PCF had worse Neck Disability Index (NDI) scores at 3 months than did patients with ACF (13.67 ± 9.49 vs 10.55 ± 6.24, respectively; p = 0.03). There were significantly higher NDI scores for patients with dysphagia at 3 months in both the ACF and PCF groups and at 12 months for those in the PCF group. Analogously, EuroQol-5 Dimensions scores were worse for patients with dysphagia; however, this was only significant for patients in the ACF group at 3 months. There were no significant risk factors for the development of dysphagia found on multivariate analysis. Conclusions: Similar rates and severity of dysphagia were seen following ACF and PCF at 3- and 12-month follow-up. This suggests that long-term dysphagia following cervical fusion surgery may be due to structural changes from the fusion rather than the surgical approach. However, the ACF cohort was significantly younger, and this may have partially accounted for the findings. PROs were also compared for patients with and without dysphagia, demonstrating worsened outcomes in some domains for patients who presented with dysphagia at 3- and 12-month follow-up. This suggests that dysphagia may be associated with a decreased quality of life after cervical fusion.

Abstract:Superamphiphobic nanofibrous membranes exhibiting robust water/oil proof and breathable performances were prepared by the combination of a novel synthesized fluorinated polyurethane (FPU) containing a terminal perfluoroalkane segment and incorporated SiO(2) nanoparticles (SiO(2) NPs). By employing the FPU/SiO(2) NPs incorporation, the hybrid membranes possess superhydrophobicity with a water contact angle of 165° and superoleophobicity with an oil contact angle of 151°. Surface morphological studies have indicated that the wettability of resultant membranes could be manipulated by tuning the surface composition as well as the hierarchical structures. The quantitative hierarchical roughness analysis using N(2) adsorption method has confirmed a major contribution of SiO(2) NPs on enhancing the porous structure, and a detailed correlation between the fractal dimension and amphiphobicity is proposed. Furthermore, a designed concept test shows that the as-prepared membranes could load 1.5 kg water or oil at the same time maintained an extremely high air permeability of 2 L min(-1), suggesting their use as promising materials for a variety of potential applications in protective clothing, bioseparation, water purification, tissue engineering, microfluidic systems, etc., and also provided new insight into the design and development of functional hybrid membranes based on FPU.
Intraoperative Protection of Pharyngeal Autonomic Nerves: Preventing Dysphagia After Anterior Cervical Decompression and Fusion Surgery

Abstract:Objective: The study aims to investigate whether intraoperative protection of the pharyngeal autonomic nerve can effectively reduce the incidence of postoperative dysphagia following anterior cervical decompression and fusion surgery (ACDF). Methods: A retrospective analysis was conducted on 130 cases that underwent ACDF from January 2018 to June 2022 at our hospital. Divided into nonautonomic neuroprotection (NANP) group and autonomic neuroprotection group based on whether receive protective measures for the pharyngeal autonomic nerve during surgery. General data were recorded and compared between the 2 groups. Postoperative outcomes were evaluated using Neck Disability Index, Japanese Orthopaedics Association (JOA) score, and JOA improvement rate. The incidence and severity of postoperative dysphagia were assessed using Bazaz dysphagia assessment criteria and swallowing-quality of life questionnaire. Results: There were no significant differences in general data (P > 0.05). The average operation time and intraoperative blood loss also showed no significant differences (P > 0.05). Both groups showed significant improvements in Neck Disability Index and JOA scores at all follow-up time points compared to preoperative scores (P < 0.01). The incidence of postoperative dysphagia in the autonomic neuroprotection group was significantly lower than that in the NANP group at all follow-up time points (P < 0.05). Both group showed a significant reduction in scores 3 days postoperatively compared to preoperative scores (P < 0.01), and the NANP group also showed significant reductions in scores at 3 month and 1 year postoperative follow-up time points compared to preoperative scores (P < 0.01). Conclusions: The adoption of pharyngeal autonomic nerve protective measures during ACDF can effectively lower the probability of postoperative dysphagia.
Development and Validation of a Risk Prediction Model for Pseudarthrosis Following Transforaminal Lumbar Interbody Fusion: A Retrospective Analysis

Abstract:Background: Pseudarthrosis is a common complication of transforaminal lumbar interbody fusion (TLIF) that can affect the long-term prognosis and increase revision surgery risk. Therefore, it is crucial to investigate the risk factors associated with pseudarthrosis and develop a predictive model. Methods: Patients who underwent TLIF at a single hospital between January 2019 and June 2021 were included. Each patient's fusion status was determined based on thin-section lumbar spine computed tomography scans performed at least 1 year postoperatively. A nomogram was subsequently established to predict the probability of pseudarthrosis based on the results of least absolute shrinkage and selection operator and multivariable analysis. Receiver operating characteristic curve, calibration curve, and decision curve analysis were applied to evaluate the prediction effect and clinical value of the model. Results: In total, 284 patients (131 men [46.1%]; mean [standard deviation, SD] age, 63.0 [12.1] years) were included in the analysis (mean [SD] follow-up, 19.2 [9.2] months). The incidence of pseudarthrosis was 13.4% (38/284). In least absolute shrinkage and selection operator and multivariable analysis, independent risk factors for pseudarthrosis included history of smoking (odds ratio [OR]:2.966, 95% confidence interval [CI]: 1.135-7.750, P = 0.027), osteoporosis (OR = 6.362, 95% CI: 2.433-16.636, P < 0.001), and the change of mean disc height (OR = 2.401, 95% CI: 1.638-3.519, P < 0.001). The areas under the curves in the training and validation cohorts were 0.870 and 0.809, respectively. Conclusions: History of smoking, osteoporosis, and change of mean disc height are all independent risk factors of pseudarthrosis following TLIF surgery; a nomogram based on these may help predict the probability of pseudarthrosis.

Abstract:The procedure of modified tracheal traction exercise (MTTE). The patient is in supine position and a U‐shaped neck cushion is used to achieve cervical hyperextension. The operator confirms the location of thyroid cartilage, and then the thyroid cartilage is pushed to the left side at least 1 cm across the midline of the anterior neck. Before returning to the starting position, this status should be held for 5 s, swallowing when the pushing is performed. Objectives Dysphagia, an impairment in swallowing, is a frequent and debilitating complication for patients undergoing anterior cervical discectomy and fusion (ACDF), a common surgical treatment for degenerative cervical myelopathy (DCM). This retrospective study aimed to assess the efficacy of modified tracheal traction exercise (MTTE) in alleviating postoperative dysphagia and improving clinical outcomes for these patients. Methods A cohort of 143 patients underwent multilevel fusions, equally distributed between MTTE (n = 75) and traditional tracheal traction exercise (TTTE) (n = 68) groups. Demographic parity was observed in gender distribution, age averages (MTTE: 51.43 ± 11.25 years; TTTE: 52.35 ± 10.43 years), body mass index (BMI), comorbidities, fusion segments, and preoperative hospitalization days. Surgical duration differences were assessed. Clinical outcomes, dysphagia incidence, blood loss, postoperative complications, Cervical Japanese Orthopedic Association (c‐JOA) scores, and functional outcome swallowing scale evaluations were conducted. Univariate and multivariate logistic regression analyses were used to explore factors influencing dysphagia.
