Abstract:A multicenter retrospective study. The purpose of this study was to explore risk factors of dysphagia after anterior cervical surgery and factors affecting rehabilitation of dysphagia 2 years after surgery. Patients who underwent anterior cervical surgery at 3 centers from January 2010 to January 2013 were included. The possible factors included 3 aspects: demographic variables-age, sex, body mass index (BMI): hypertension, diabetes, heart disease, smoking, alcohol use, diagnose (cervical spondylotic myelopathy or ossification of posterior longitudinal ligament), preoperative visual analogue scale (VAS), Oswestry Disability Index (ODI), Japanese Orthopaedic Association (JOA), surgical-related variables-surgical option (ACDF, ACCF, ACCDF, or Zero profile), operation time, blood loss, operative level, superior fusion segment, incision length, angle of C2 to C7, height of C2 to C7, cervical circumference, cervical circumference/height of C2 to C7. The results of our study indicated that the rate of dysphagia at 0, 3, 6, 12, and 24 months after surgery was 20%, 5.4%, 2.4%, 1.1%, and 0.4%, respectively. Our results showed that age (58.8 years old), BMI (27.3kg/m(2)), course of disease (11.6 months), operation time (103.2 min), blood loss (151.6 mL), incision length (9.1 cm), cervical circumference (46.8 cm), angle of C2 to C7 (15.3 degrees), cervical circumference/height of C2 to C7 (4.8), preoperative VAS (7.5), and ODI (0.6) in dysphagia group were significantly higher than those (52.0, 24.6, 8.6, 88.2, 121.6, 8.6, 42.3, 12.6, 3.7, 5.6, and 0.4, respectively) in nondysphagia group; however, height of C2 to C7 (9.9 vs 11.7cm) and preoperative JOA (8.3 vs 10.7) had opposite trend between 2 groups. We could also infer that female, smoking, diabetes, ossification of posterior longitudinal ligament, ACCDF, multilevel surgery, and superior fusion segment including C2 to C3 or C6 to C7 were the risk factors for dysphagia after surgery immediately. However, till 2 years after surgery, only 2 risk factors, smoking and diabetes, could slow rehabilitation of dysphagia. Many factors could significantly increase rate of dysphagia after anterior cervical surgery. Operation time as a vital factor markedly increases immediate postoperative dysphagia and smoking, as the most important factor, lower recovery of dysphagia. Further study is needed to prove if these factors could influence dysphagia.

We could also infer that female, smoking, diabetes, ossification of posterior longitudinal ligament, ACCDF, multilevel surgery, and superior fusion segment including C2 to C3 or C6 to C7 were the risk factors for dysphagia after surgery immediately. However, till 2 years after surgery, only 2 risk factors, smoking and diabetes, could slow rehabilitation of dysphagia. Many factors could significantly increase rate of dysphagia after anterior cervical surgery. Operation time as a vital factor markedly increases immediate postoperative dysphagia and smoking, as the most important factor, lower recovery of dysphagia. Further study is needed to prove if these factors could influence dysphagia.
Risk factors of dysphagia following cervical spine surgery

Abstract:Objective To analyze the incidence and risk factors of dysphagia after anterior and posterior cervical spine surgeries( AC and PC procedures). Methods Three hundred and fifty-four patients were reviewed in this study,including 172 patients undergoing AC procedure( AC group),and 182 patients with PC procedure( PC group). All patients were followed up for 28 months. The presence and severity of dysphagia were reported on the first day of surgery and at 1,3,6 and 12 months after the procedure. The incidence and risk factors of dysphagia after cervical spine surgery were studied by using univariate and binary Logistic regression model analysis. Results The incidence and risk factors of dysphagia after cervical spine surgery at different time points were not statistically significant between the two groups( all P 0. 05).The univariate and binary Logistic regression model analysis showed that the risk factors such as age,gender,BMI,operative time,blood loss,procedure type,revision surgery,most cephalic operative level and number of operative levels did not significantly influence the incidence of postoperative dysphagia in the two groups( all P 0. 05). Conclusions Postoperative dysphagia is common after anterior and posterior cervical spine surgery. The incidence and severity of dysphagia both decrease over time. Age,gender,BMI,operative time,blood loss,procedure type,revision surgery,most cephalic operative level and number of operative levels may not be risk factors of postoperative dysphagia.
1087 Large Incidence and Risk Factors Meta-analysis of Dysphagia in Over 132,000 Anterior, Posterior, and Hybrid Surgeries for Cervical Spondylosis Myelopathy

Abstract:INTRODUCTION: Surgical management of Cervical Spondylotic Myelopathy (CSM) is associated with complications with devastating impacts. Dysphagia is one of these major complications with the incidence rate being not completely known in different surgical approaches. METHODS: PubMed/MEDLINE, SCOPUS, and Cochrane were systematically searched until January 2023. Rates and risk of complications were assessed. Meta-analysis of rates and risks was performed with generalized linear mixed model (GLMM) and inverse variance method, respectively, and compared between techniques. RESULTS: 1196 citations were screened and 39 studies with 60 instances of dysphagia incidence/risk factors reports from over 132,442 patients were identified. Dysphagia’s overall incidence rate was 4.98%[95% confidence interval 3.23-7.61] and was significantly different between surgery groups (Chi2 p < 0.01). It was found in 7.97% [4.95–12.57] of anterior surgeries, 1.52% [0.94–2.45] of posterior ones, and 11.25% [3.38–31.44] of hybrid techniques (statistical comparison: P < A 65) risked dysphagia significantly (OR 1.38 [95%CI 1.11-1.71]). CONCLUSIONS: Chances of posterior surgery being complicated with dysphagia is not negligible and should be discussed with patient before surgery, especially with older adults.

Abstract:PurposeTo conduct a systematic review of literature to determine risk factors and preventative measures of early and persistent dysphagia after anterior cervical spine surgery (ACSS).MethodsOn March 2017, we searched the database PubMed, Medline, EMBASE, the Cochrane library, Clinical key, Springer link and Wiley Online Library without time restriction using the term ‘dysphagia’, ‘swallowing disorders’, and ‘anterior cervical spine surgery’. Selected papers were examined for the level of evidence by published guidelines as level I, level II, level III, level IV studies. We investigated risk factors and preventative measures of early or persistent dysphagia after ACSS from these papers.ResultsThe initial search yielded 515 citations. Fifty-nine of these studies met the inclusion and exclusion criteria. Three of them were level I evidence studies, 29 were level II evidence studies, 22 were level III evidence studies, and 3 were level IV evidence studies. Preventable risk factors included prolonged operative time, use of rhBMP, endotracheal tube cuff pressure, cervical plate type and position, dC2–C7 angle, psychiatric factors, tobacco usage, prevertebral soft tissue swelling, SLN or RLN palsy or injury of branches. Preventative measures included preoperative tracheal traction exercise, maintaining endotracheal tube cuff pressure at 20 mm Hg, avoiding routine use of rhBMP-2, use of zero-profile implant, use of Zephir plate, use of new cervical retractor, steroid application, avoiding prolonged operating time, avoiding overenlargement of cervical lordosis, decreasing surgical levels, ensuring knowledge of anatomy of superior laryngeal nerve and recurrent laryngeal nerve, to comfort always, patients quitting smoking and doctors ensuring improved skills. Unpreventable risk factors included age, gender, multilevel surgery, revision surgery, duration of preexisting pain, BMI, blood loss, upper levels, preoperative comorbidities and surgical type.ConclusionAdequate preoperative preparation of the patients including preoperative tracheal traction exercise and quitting smoking, proper preventative measures during surgery including maintaining endotracheal tube cuff pressure at 20 mm Hg, avoiding routine use of rhBMP-2, use of zero-profile implant, use of Zephir plate, use of new cervical retractor, steroid application, avoiding prolonged operating time, avoiding overenlargement of cervical lordosis and decreasing surgical levels, doctors ensuring knowledge of anatomy, improved surgical techniques and to comfort always are essential for preventing early and persistent dysphagia after ACSS.

Abstract:Objective:To evaluate the related factors of dysphagia following anterior cervical decompression and fusion.Method:490 cases undergoing anterior discectomy decompression and fusion from October 2002 to October 2004 were followed up,of these,there were 306 males and 184 females,with the mean age of 47.2 years(range,12 to 76 years).Of these 490 patients,415 had cervical spondylosis,43 had cervical fracture or dislocation,22 had cervical disc herniation,8 had cervical spine tumor,and 2 had cervical spine tuberculosis.490 patients were reviewed retrospectively postoperatively about their swallow dysfunction(objective)and swallow comfortlessness(subjective).All cases with dysphgia accepted dexamethasone used intravenously and correspondant therapy.Result:490 cases were followed up,and 96(19.6%)of them had postoperative dysphagia which lasted for 3 weeks to 2 years.Of 96 patients with dysphagia,with the mean age of 57.6 yeas(range,38 to 63 years).Of 96 patients with dysphagia,89 cases had titanium plates placement,7 cases had intervertebral cages placement.Those without plates or cages placement and those with artificial disc placement had no postoperative dysphagia.Female,elder and titanium plates planting was associated with postoperative dysphagia.After 14.2 months follow-up,90 cases(93.7%)with dysphagia cured or improved,while 6 cases had no improvement.Conclusion:Postoperative dysphagia is a common complication associated with anterior cervical discectomy and fusion,the causemay be multifactory.As a result,clinicians should pay more attention to this complication and make proper intervention.
Risk factors for dysphagia after anterior cervical spine surgery: A meta-analysis

Abstract:Background: Dysphagia is a well-known complication following anterior cervical spine surgery. Although risk factors for dysphagia have been reported in the literature, they still remain controversial. This study aims to investigate the risk factors associated with dysphagia following anterior cervical spinal surgery. Methods: PubMed, EMBASE, and The Cochrane Library were searched up to June 2016 for studies examining dysphagia following anterior cervical spinal surgery. Risk factors associated with dysphagia were extracted. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated for outcomes. Data analysis was conducted with RevMan 5.3 and STATA 12.0. Results: The final analysis includes a total of 18 distinct studies. The pooled analysis reveals that there are significant differences in female gender (OR=2.30, 95% CI: 1.76-2.99, P<0.001), the use of anterior cervical plate (OR=1.66, 95% CI: 1.05-2.62, P=0.03), more than 1 surgical level (OR=2.07, 95% CI: 1.62-2.66, P<0.001), the upper surgical level at C3/4 (OR=3.08, 95% CI: 1.44-6.55, P=0.004), and the use of bone morphogenetic protein-2 (rhBMP-2) (OR=5.52, 95% CI: 2.16-14.10, P<0.001). However, no significant difference is found in revision surgery (OR=1.67, 95% CI: 0.60-4.68, P=0.33), the type of fusion (OR=1.02, 95% CI: 0.62-1.67, P=0.95), and cervical disc arthroplasty (OR=1.37, 95% CI: 0.75-2.51, P=0.30). Conclusion: Female gender, the use of anterior cervical plate, more than 1 surgical level, the upper surgical level at C3/4, and the use of rhBMP-2 are the risk factors for dysphagia following anterior cervical spinal surgery. However, revision surgery, the type of fusion, and cervical disc arthroplasty are unassociated with dysphagia. Considering the limited number of studies, this conclusion should be interpreted cautiously, and larger scale studies are required.

Abstract:Study Design: Prospective multi-center cohort study Objective: To explore the association between operative level and postoperative dysphagia after anterior cervical discectomy and fusion (ACDF). Summary of Background Data: Dysphagia is common following ACDF and has several risk factors including soft tissue edema. The degree of prevertebral soft tissue edema varies based upon the operative cervical level. However, the operative level has not been evaluated as a source of postoperative dysphagia. Methods: Adult patients undergoing elective ACDF were prospectively enrolled at three academic centers. Dysphagia was assessed using the Bazaz questionnaire, Dysphagia Short Questionnaire (DSQ), and Eating Assessment Tool-10 (EAT-10) preoperatively and at 2, 6, 12, and 24-weeks postoperatively. Patients were grouped based on inclusion of specific surgical levels in the fusion construct. Multivariable regression analyses were performed evaluating the independent effects of number of surgical levels and inclusion of each particular level on dysphagia symptoms. Results: A total of 130 patients were included. Overall, 24 (18.5%) patients had persistent postoperative dysphagia at 24 weeks and were older, female, and less likely to be drink alcohol. There was no difference in operative duration or dexamethasone administration. Patients with persistent dysphagia were significantly more likely to have C4-C5 included in the fusion construct (62.5% vs. 34.9%, P =0.024) but there were no differences based on inclusion of other levels. On multivariable regression, inclusion of C3-C4 or C6-C7 were associated with more severe EAT-10 (β:9.56, P =0.016 and β:8.15, P =0.040) and DSQ (β:4.44, P =0.023 and (β:4.27, P =0.030) at 6 weeks. At 12-weeks, C3-C4 fusion was also independently associated with more severe dysphagia (EAT-10 β:4.74, P =0.024). Conclusion: The location of prevertebral soft tissue swelling may impact the duration and severity of patient-reported dysphagia outcomes at up to 24 weeks postoperatively. In particular, inclusion of C3-C4 and C4-C5 into the fusion may be associated with dysphagia severity.

Abstract:Anterior cervical spine surgery (ACSS) is a surgical intervention widely used for a myriad of indications including degenerative, oncologic, inflammatory, traumatic, and congenital spinal conditions. A primary concern for surgeons performing ACSS is the postoperative development of oropharyngeal dysphagia. Current literature reports a wide incidence of this complication ranging from 1 to 79%. Dysphagia after ACSS is multifactorial, with common risk factors being prolonged duration of operation, revision surgeries, multilevel surgeries, and use of recombinant human bone morphogenetic protein-2. Many technical strategies have been developed to reduce the risk of postoperative dysphagia, including the development of low-profile implants and retropharyngeal local steroid application. In this article, we review the most recent literature regarding the epidemiology and pathophysiology, diagnostic criteria, risk factors, and management of dysphagia after ACSS.

Anterior Approach Versus Posterior Approach for the Treatment of Multilevel Cervical Spondylotic Myelopathy: a Systemic Review and Meta-Analysis

Abstract:PURPOSE:To compare the clinical outcomes, complications, and surgical trauma between anterior and posterior approaches for the treatment of multilevel cervical spondylotic myelopathy. STUDY DESIGN:Systematic review and meta-analysis. METHODS:We searched MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials databases for randomized controlled trials or non-randomized controlled trials that compared anterior and posterior surgical approaches for the treatment of multilevel cervical spondylotic myelopathy. Exclusion criteria were non-controlled studies, combined anterior and posterior surgery, follow-up <1 year, cervical kyphosis >15°, and cervical myelopathy caused by ossification of the posterior longitudinal ligament. The main end points included: recovery rate; Japanese Orthopedic Association (JOA) score; reoperation rate; complication rate; blood loss; and operation time. Subgroup analysis was conducted according to the mean number of surgical segments. RESULT:A total of eight studies were included in the meta-analysis; none of which were randomized controlled trials. All of the selected studies were of high quality as indicated by the Newcastle-Ottawa scale. In five studies involving 351 patients, the preoperative JOA score was similar between the anterior and posterior groups [P > 0.05, WMD: -0.00 (-0.56, 0.56)]. In four studies involving 268 patients, the postoperative JOA score was higher in the anterior group compared with the posterior group [P < 0.05, WMD: 0.79 (0.16, 1.42)]. For recovery rate, there was significant heterogeneity among the four studies involving 304 patients, hence, only descriptive analysis was performed. In seven studies involving 447 patients, the postoperative complication rate was significant higher in the anterior group compared with the posterior group [P < 0.05, odds ratio: 2.60 (1.63, 4.15)]. Of the 245 patients in the 8 studies who received anterior surgery, 21 (8.57%) received reoperation. Of the 285 patients who received posterior surgery, only 1 (0.3%) received reoperation. The reoperation rate was significantly higher in the anterior group compared with the posterior group (P < 0.001). In the 3 studies involving 236 patients compared subtotal corpectomy and laminoplasty/laminectomy, blood loss and operation time were significantly higher in the anterior subtotal corpectomy group compared with the posterior laminoplasty/laminectomy group [P < 0.05, WMD: 150.10 (63.53, 236.66) and P < 0.05, WMD: 59.17 (45.69, 72.66)]. CONCLUSION:The anterior approach was associated with better postoperative neural function than the posterior approach in the treatment of multilevel cervical spondylotic myelopathy. There was no apparent difference in the neural function recovery rate. The complication and reoperation rates were significantly higher in the anterior group compared with the posterior group. The surgical trauma associated with corpectomy was significantly higher than that associated with laminoplasty/laminectomy.
Anterior cervical decompression with fusion for treating multilevel cervical myelopathy

Abstract:Objective To evaluate the clinical value of the segemental methods of anterior cervical decompression with fusion for treating multilevel cervical myelopathy. Methods A retrospective review of 20 patients who had undergone segemental method of anterior cervical decompression with fusion for treating multilevel cervical myelopathy. All patiens'cervical plain films,magnetic resonance imagings(MRI) and computed tomographies(CT) on preoperation and postoperation were observed. The mJOA(modified JOA) scores of preoperation and 3 months postoperatively were collected.The implant graft fusion rate ,the degree of correcting kyphotic deformity in cervical vertebrae and the complications were recorded. Results 20 cases were followed up from 14 months to 42 months. All patients improved their functional status in different degree. The mean preoperative mJOA score of all patients was 9.2, the postoperative was 13.8,the average mJOA recovery rate was 59.4%(p<0.05).All patients'kyphotic deformity was corrected and some restored normally. No complications such as graft dislocation,screw and plates loosing or broken were happened.Conclusions The segemental method of anterior cervical decompression with fusion for treating multilevel cervical myelopathy appears to be effectively decompressing spinal cord,improving nerve functional status and restoring lordosis to the cervical spine with high fusion rate and fewer complications.

Abstract:Background: Early dysphagia is a frequent complication of anterior cervical (AC) spine surgery. However, there are no reports that have discussed the correlation between early dysphagia and the positional relationship between thyroid cartilage and the surgical level.Methods: We retrospectively enrolled 82 patients in our hospital who underwent single-level AC discectomy performed by the same surgeon using the same internal fixation apparatus from 2015 to 2017. Swallowing difficulty was rated during the first five postoperative days using a 10-point scoring system. The positional relationship between the thyroid cartilage and the surgical level was defined as discectomy within the thyroid cartilage (IN group) or outside the thyroid cartilage (OUT group) using preoperative computed tomography (CT) images. The confounding factors such as gender, age, body mass index (BMI), hypertension, diabetes mellitus, drinking, smoking, operative level, operative time, and blood loss were analyzed by a binomial logistic regression.Results: The thyroid cartilage was most commonly located above the C5 level (65.1%). Early dysphagia developed in 47.6% of the patients during the first five postoperative days. The IN and OUT groups each contained 41 cases. The difference in the cumulative postoperative early dysphagia score between the IN and OUT groups was statistically significant (p Conclusions: We found that early dysphagia, which is a self-limiting complication, was correlated with surgery performed at levels outside the thyroid cartilage region. Preoperative review of the positional relationship between the thyroid cartilage and the surgical level can predict the incidence of postoperative transient dysphagia.

Abstract:Objective. To compare the clinical efficacy and safety of anterior cervical decompression and segmental fusion and posterior expansive canal plasty in the treatment of multisegment cervical myelopathy. Methods. Retrospective analysis was performed of 56 cases of multisegment cervical myelopathy patients admitted from July 2018 to June 2021, 32 male patients and 24 females, aged 56.9 ± 12.8 years with an average duration of 10.6 ± 3.2 years. All patients' preoperative imaging examination revealed multiple-segmented cervical disc herniation and had clinical manifestations of cervical myelopathy. Results. No neurovascular complications occurred in both groups, and 24 to 36 months of follow-up (mean 28.6 months) were obtained. The height of the cervical spondylosis segment was higher than that 2 weeks after surgery ( p < 0.05 ), and the curvature of the cervical spine was significantly lower than that before surgery. There was no statistical significance in the height of the anterior column and curvature of the cervical vertebra at 2 weeks after surgery and at the last follow-up ( p > 0.05 ). There were statistically significant differences in anterior curvature of the cervical spine between the two groups at 2 weeks after surgery and the last follow-up ( p < 0.05 ). Japanese Orthopaedic Association (JOA) scores in both groups recovered significantly after surgery. At 3 months and the last follow-up, the improvement rate of JOA score in the anterior approach group was significantly higher than that in the posterior approach group ( p < 0.05 ), and the improvement rate of JOA score in the anterior approach group was also better than that in the posterior approach group ( p < 0.05 ). Conclusion. This segmented anterior fusion procedure can effectively restore the anterior cervical column height and can significantly improve spinal cord function compared with posterior spinal canal enlargement plasty, thus could be considered an effective option for the treatment of multisegment cervical myelopathy.

Abstract:Superamphiphobic nanofibrous membranes exhibiting robust water/oil proof and breathable performances were prepared by the combination of a novel synthesized fluorinated polyurethane (FPU) containing a terminal perfluoroalkane segment and incorporated SiO(2) nanoparticles (SiO(2) NPs). By employing the FPU/SiO(2) NPs incorporation, the hybrid membranes possess superhydrophobicity with a water contact angle of 165° and superoleophobicity with an oil contact angle of 151°. Surface morphological studies have indicated that the wettability of resultant membranes could be manipulated by tuning the surface composition as well as the hierarchical structures. The quantitative hierarchical roughness analysis using N(2) adsorption method has confirmed a major contribution of SiO(2) NPs on enhancing the porous structure, and a detailed correlation between the fractal dimension and amphiphobicity is proposed. Furthermore, a designed concept test shows that the as-prepared membranes could load 1.5 kg water or oil at the same time maintained an extremely high air permeability of 2 L min(-1), suggesting their use as promising materials for a variety of potential applications in protective clothing, bioseparation, water purification, tissue engineering, microfluidic systems, etc., and also provided new insight into the design and development of functional hybrid membranes based on FPU.
Prevalence and risk factors of axial neck pain in patients undergoing multilevel anterior cervical decompression with fusion surgery

Abstract:Objectives The aim of this study was to explore the prevalence and risk factors for axial neck pain in patients undergoing multilevel anterior cervical decompression with fusion surgery. Methods In this study, 88 patients, who underwent multilevel anterior cervical decompression with fusion surgery from January 2012 to January 2017, were retrospectively reviewed. Based on the postoperative axial neck pain, the patients were classified into two groups: axial pain group and no axial pain group. The patients were followed up 3 weeks, 3 months, and 1 year after cervical anterior surgery for the early- and long-term clinical evaluation. The possible effect factors included demographic variables (age, sex, BMI, smoking, drinking, heart disease, hypertension, diabetes, preoperative kyphosis, preoperative axial neck pain, preoperative JOA scores, and ODI) and surgery-related variables (surgical option, vertebral lesions, spinal canal stenosis rate, superior fusion segment, presence of intramedullary high signal intensity). Results The prevalence of axial neck pain was 27.3% (24 cases of 88). Our results showed that preoperative axial neck pain (62% vs 23%, P < 0.001) and preoperative kyphosis (42% vs 21.9%, P < 0.001) were risk factors for axial pain after multilevel anterior cervical surgery. Additionally, for patients with preoperative cervical kyphosis, compared to no axial pain group, the axial neck group was significantly more likely to exist a higher preoperative angle of C2–7 (13.31 ± 2.33 vs 7.33 ± 2.56, P < 0.001) and a higher correction range for kyphosis (20.24 ± 4.12 vs 12.34 ± 3.12, P < 0.001). However, for all the patients with postoperative axial symptoms, the improvement rate of axial pain was significantly higher for patients without cervical kyphosis at the early-term follow-up (3 weeks) ( P = 0.032), no significant differences were found at the medium-term ( P = 0.554) and long-term follow-up ( P = 0.902), and improvements of clinical symptom have no obvious difference at the last follow-up. Conclusions Overall, preoperative axial neck pain and kyphosis could predict axial neck pain for patients undergoing multilevel anterior cervical decompression with fusion surgery, and recovery of cervical kyphosis may contribute to the long-term recovery of neural function, but may also suffer from risk of short-term axial pain, which could be reduced through moderate cervical curvature recovery.
Operation Methods for Multilevel Cervical Spondylotic Myelopathy and Comparison of Efficacy

Abstract:Objective To study the anterior and posterior operation approaches for the multilevel cervical spondylotic myelopathy and to compare the clinical results and mechanism of complication in follow-up.Methods The preoperative and postoperative data of 60 patients with multilevel cervical spondylotic myelopathy were reviewed.The follow-up ranged form 6 months to 8 years.The clinical results were evaluated by JOA score and VAS score.Results The JOA score improved from preoperative 7.3±1.5 to postoperative 12.2±3.0(P<0.01).Among them the excellent result was achieved in 37 patients(61.7%),good in 16(26.7%),accepted in 6(10%) and bad in 2(5%).Conclusion Both anterior approach and posterior approach are predominantly effective in the treatment of multilevel cervical spondylotic myelopathy.However,the appropriate method should be selected according to the clinical details to avoid the complications and reach the best operation results at last.

Abstract:Anterior cervical discectomy and fusion (ACDF) is a widely performed surgical procedure for treating cervical disease. [ 1 , 2 ] Cervical disc herniation is a degenerative disease of the cervical spine in which a protruded disc compresses the exiting nerve and incurs segmental instability. ACDF via an anterior approach to the neck would be the most commonly implemented surgical option for this cervical spinal condition. In addition to common postoperative complications such as bleeding, infection, and pseudoarthrosis after ACDF, damage such as laryngeal nerve, muscle, soft tissue damage, and dysphagia may result from the anterior approach. [ 3 ] Dysphagia is a disabling condition that may have a detrimental effect on quality of life. The incidence of dysphagia varies among studies owing to differences in the surgical method, individual patient characteristics, evaluation method, and follow-up period. In a previous report, the percentage of patients who experienced difficulty in swallowing immediately after ACDF surgery was relatively high. [ 4 ] The rate of dysphagia reported after surgery ranges from 1% to 79%, but in most cases, these symptoms are in the short term and tend to improve naturally over time. [ 4 ] Despite the limited information available on rehabilitative strategies for treating dysphagia following ACDF, this case report aims to evaluate and recommend an effective rehabilitation strategy for patients experiencing post-ACDF dysphagia.

Abstract:Anterior cervical spine procedures have been associated with satisfactory outcomes. However, the occurrence of troublesome complications, although uncommon, needs to be taken into consideration. The purpose of our study was to assess the actual incidence of anterior cervical spine procedure-associated complications and identify any predisposing factors. A total of 114 patients undergoing anterior cervical procedures over a 6-year period were included in our retrospective, case-control study. The diagnosis was cervical radiculopathy, and/or myelopathy due to degenerative disc disease, cervical spondylosis, or traumatic cervical spine injury. All our participants underwent surgical treatment, and complications were recorded. The most commonly performed procedure (79%) was anterior cervical discectomy and fusion (ACDF). Fourteen patients (12.3%) underwent anterior cervical corpectomy and interbody fusion, seven (6.1%) ACDF with plating, two (1.7%) odontoid screw fixation, and one anterior removal of osteophytes for severe Forestier's disease. Mean follow-up time was 42.5 months (range, 6-78 months). The overall complication rate was 13.2%. Specifically, we encountered adjacent intervertebral disc degeneration in 2.7% of our cases, dysphagia in 1.7%, postoperative soft tissue swelling and hematoma in 1.7%, and dural penetration in 1.7%. Additionally, esophageal perforation was observed in 0.9%, aggravation of preexisting myelopathy in 0.9%, symptomatic recurrent laryngeal nerve palsy in 0.9%, mechanical failure in 0.9%, and superficial wound infection in 0.9%. In the vast majority anterior cervical spine surgery-associated complications are minor, requiring no further intervention. Awareness, early recognition, and appropriate management, are of paramount importance for improving the patients' overall functional outcome.
