Abstract:Orthodontic Mini-Implants have a high success rate, but it is crucial to assess the load that they bear in order to maintain their primary stability. Increasing the diameter can improve this stability, but there are limitations due to the proximity of the tooth roots. To avoid damage, smaller diameters are used, which can decrease resistance and cause permanent deformations. Objective: The objective of this study is to evaluate the influence of the diameter of Mini-Implants through bending force tests, taking into account primary stability after one and two insertions. Methods: Here, 40 Ti6AI4V alloy Mini-Implants of two different brands and diameters were divided into eight groups, half of which received one insertion in the artificial bone, and the rest received two. All were subjected to a constant bending force using an INSTRON-Electropuls E10000LT (Norwood, MA, USA) until fracture. Results: The smaller-diameter Mini-Implants were less resistant to fracture, but both were able to withstand the necessary loads produced by orthodontic movements. As for the inserts, there were no statistically significant differences. Conclusions: There is an advantage to using 1.6 mm Mini-Implants over 2.0 mm ones, as a smaller diameter does not lead to fracture due to the forces used in orthodontic treatment. Having one or two inserts did not have a statistically significant effect.

What is the impact of patient attributes, implant characteristics, surgical techniques, and placement location on the success of orthodontic mini-implants in young adults? A Systematic Review and Meta-Analysis

Abstract:Background: Temporary anchorage devices (TADs) address challenges in traditional orthodontic anchorage like patient compliance and precision, showing significantly improved clinical outcomes, particularly for cases requiring maximum anchorage. Materials and methods: A systematic electronic search was performed in five research databases, focusing on studies published between 2015 and 2023. The ROBINS-I tool from the Cochrane Bias Methods Group assessed the risk of bias. Data analysis included categorical and numerical variables, with categorical variables analyzed using Cohen's method in a random effects model to account for variability. Sensitivity and heterogeneity were evaluated using a 'leave-one-out' approach and the I 2 statistic, respectively. At the same time, publication bias was checked using Egger's test, with findings presented through Forest and Funnel plots. Numerical variables were subjected to weighted regression analysis. Results: Examination of 15 studies involving 1981 patients and 3272 orthodontic mini-implants identified key factors affecting implant stability. Failure rates varied significantly, influenced by factors such as the characteristics and insertion site of the orthodontic mini-implants (OMIs), patient-specific variables, and operator experience. Notably, the insertion site and implant characteristics like size did not significantly affect failure rates, but there was a negative correlation between the magnitude of force applied and failure rates. Conclusion: The success of orthodontic mini-implants is broadly consistent across patient demographics and is not significantly impacted by gender or age, though failure rates were higher in males and when implants were placed in the maxilla. These findings suggest that higher applied forces might reduce failure rates. Clinical significance: This review underlines mini-implant efficacy across varied patient demographics, emphasizing the importance of site selection, jaw location, and force application in enhancing success rates and guiding tailored treatment strategies.PROSPERO ID CRD42023411955.
Er:YAG laser, piezosurgery, and surgical drill for bone decortication during orthodontic mini-implant insertion: primary stability analysis—an animal study

Abstract:It is important to identify factors that affect primary stability of orthodontic mini-implants because it determines the success of treatment. We assessed mini-implant primary stability (initial mechanical engagement with the bone) placed in pig jaws. We also assessed mini-implant insertion failure rate (mini-implant fracture, mini-implants to root contact). A total of 80 taper-shaped mini-implants (Absoanchor® Model SH1312-6; Dentos Inc., Daegu, Korea) 6 mm long with a diameter of 1.1 mm were used. Bone decortication was made before mini-implant insertion by means of three different methods: Group G1: Er:YAG laser (LiteTouch®, Light Instruments, Yokneam, Israel) at energy of 300 mJ, frequency 25 Hz, fluence 38.2 J/cm2, cooling 14 ml/min, tip 1.0 × 17 mm, distance 1 mm, time of irradiation 6 s; Group G2: drill (Hager & Meisinger GmbH, Hansemannstr, Germany); Group G3: piezosurgery (Piezotom Solo, Acteon, NJ, USA). In G4 group (control), mini-implants were driven by a self-drilling method. The primary stability of mini-implants was assessed by measuring damping characteristics between the implant and the tapping head of Periotest device (Gulden-Medizinteknik, Eschenweg, Modautal, Germany). The results in range between − 8 to + 9 allowed immediate loading. Significantly lower Periotest value was found in the control group (mean 0.59 ± 1.57, 95% CI 0.7, 2.4) as compared with Er:YAG laser (mean 4.44 ± 1.64, 95% CI 3.6, 5.3), piezosurgery (mean 17.92 ± 2.73, 95% CI 16.5, 19.3), and a drill (mean 5.91 ± 1.52, 95% CI 5.2, 6.6) (p < 0.05). The highest failure rate (33.3%) during mini-implant insertion was noted for self-drilling method (G4) as compared with G1, G2, and G3 groups (p < 0.05). The small diameter decortication by Er:YAG laser appeared to provide better primary stability as compared to drill and piezosurgery. Decortication of the cortical bone before mini-implant insertion resulted in reduced risk of implant fracture or injury of adjacent teeth. The high initial stability with a smaller diameter of the mini-implant resulted in increased risk of fracture, especially for a self-drilling method.

Abstract:OBJECT Placement of covered stents has emerged as a promising therapeutic option for cerebrovascular diseases. However, the medium- and long-term efficacy and safety of covered stents in the treatment of these diseases remain unclear. The purpose of this study was to evaluate the medium-term clinical and angiographic outcomes of covered stent placement for the treatment of intracranial aneurysms. METHODS The authors' institutional review board approved the study. Thirty-four patients (13 females and 21 males; mean age 41.9 years) with 38 intracranial aneurysms were treated with the Willis covered stent. Clinical and angiographic follow-up were performed at 3 months, at 6-12 months, and annually thereafter. The initial procedural and follow-up outcomes were collected and analyzed retrospectively. RESULTS Forty-two covered stents were successfully implanted into the target artery in 33 patients with 37 aneurysms, and 1 covered stent navigation failed in 1 patient. A complete aneurysm exclusion was initially achieved in 24 patients with 28 aneurysms, and a minor endoleak occurred in 9 patients with 9 aneurysms. Postoperatively, 2 patients died of complications related to the procedure. Angiographic and clinical follow-up data are available in 30 patients. The angiographic follow-up (17.5 ± 9.4 months [mean ± SD]) exhibited complete occlusion in 28 patients with 31 aneurysms, and incomplete occlusion in 2 aneurysms, with an asymptomatic in-stent stenosis in 3 patients (10%). The clinical follow-up (26.7 ± 13 months [mean ± SD]) demonstrated that 16 patients (53.3%) experienced a full recovery, and 14 patients (46.7%) improved. No aneurysm rupture, thromboembolic events, or neurological deficits resulting from closure of a perforating vessel by covered stent placement occurred. CONCLUSIONS Endovascular reconstruction with the Willis covered stent represents a safe, durable, and curative treatment option for selected intracranial aneurysms, yielding an excellent medium-term patency of the parent artery and excellent clinical outcomes.

Abstract:Background: The infrazygomatic crest mini-screw has been widely used, but the biomechanical performance of mini-screws at different insertion angles is still uncertain. The aim of this study was to analyse the primary stability of infrazygomatic crest mini-screws at different angles and to explore the effects of the exposure length (EL), screw-cortical bone contact area (SCA), and screw-trabecular bone contact area (STA) on this primary stability. Methods: Ninety synthetic bones were assigned to nine groups to insert mini-screws at the cross-combined angles in the occlusogingival and mesiodistal directions. SCA, STA, EL, and lateral pull-out strength (LPS) were measured, and their relationships were analysed. Twelve mini-screws were then inserted at the optimal and poor angulations into the maxillae from six fresh cadaver heads, and the same biomechanical metrics were measured for validation. Results: In the synthetic-bone test, the LPS, SCA, STA, and EL had significant correlations with the angle in the occlusogingival direction (rLPS = 0.886, rSCA = -0.946, rSTA = 0.911, and rEL= -0.731; all P < 0.001). In the cadaver-validation test, significant differences were noted in the LPS (P = 0.011), SCA (P = 0.020), STA (P = 0.004), and EL (P = 0.001) between the poor and optimal angulations in the occlusogingival direction. The STA had positive correlations with LPS (rs = 0.245 [synthetic-bone test] and r = 0.720 [cadaver-validation test]; both P < 0.05). Conclusions: The primary stability of the infrazygomatic crest mini-screw was correlated with occlusogingival angulations. The STA significantly affected the primary stability of the infrazygomatic crest mini-screw, but the SCA and EL did not.

Abstract:INTRODUCTION: This systematic review was undertaken to discuss factors that affect mini-implants as direct and indirect orthodontic anchorage.METHODS: The data were collected from electronic databases (Medline [Entrez PubMed], Embase, Web of Science, Cochrane Library, and All Evidence Based Medicine Reviews). Randomized clinical trials, prospective and retrospective clinical studies, and clinical trials concerning the properties, affective factors, and requirements of mini-implants were considered. The titles and abstracts that appeared to fulfill the initial selection criteria were collected by consensus, and the original articles were retrieved and evaluated with a methodologic checklist. A hand search of key orthodontic journals was performed to identify recent unindexed literature.RESULTS: The search strategy resulted in 596 articles. By screening titles and abstracts, 126 articles were identified. After the exclusion criteria were applied, 16 articles remained. The analyzed results of the literature were divided into 2 topics: placement-related and loading-related factors.CONCLUSIONS: Mini-implants are effective as anchorage, and their success depends on proper initial mechanical stability and loading quality and quantity.

Effect of Surface Topography on the Primary Stability of Miniscrew Implants in Orthodontics-A Systematic Review and Meta-Analysis

Abstract:This present study has the purpose of determining how surface topography of implants affects the initial stability of miniscrew implants (MSIs). Electronic databases like PubMed Central, Scopus, Web of Science, Embase, and Cochrane Library, as well as reference lists, were thoroughly searched up until September 2022. Clinical trials involving individuals who got anchorage through mini-implants, along with information on categories of mini-implants dimension, shape, thread design, and insertion site, were required as part of the eligibility criteria. Primary and secondary stability were also assessed. We carried out selection process for the study, extraction of data, quality assessment, and a meta-analysis. The qualitative synthesis included 10 papers: three randomized, four prospective, and four retrospective clinical investigations. The results of this meta-analysis demonstrate that the clinical state of MIs is controlled by their geometrical surface qualities, which are also influenced by their shape and thread design. According to the evidence this meta-analysis produced, this circumstance exists. The duration of the follow-up period and MI success rates did not correlate with one another.

Abstract:Objectives: Primary stability is a fundamental prerequisite in predicting the prognosis of a mini-implant (MI) as a skeletal anchorage. This study aims to evaluate the influence of implant site preparation technology on the primary stability of MI. Methods: A total of 108 bovine cortical bone samples were fabricated to three thicknesses (0.5, 1.0, and 1.5 mm). For each thickness group, the samples were divided into three subgroups: I (without site preparation), II (site preparation with a mechanical drill), and III (site preparation with femtosecond laser ablation). After MI insertion into these samples, the pull out strength of MI was measured by lateral pull out tests. Results: For the 0.5 mm thickness samples, the lateral pull-out strength was 9.9±2.7 N in subgroup I, 6.7±2.1 N in subgroup II, and 15.2±2.6 N in subgroup III. For the 1.0 mm thickness samples, the lateral pull-out strength was 39.3±2.5N in subgroup I, 38.2±2.7N in subgroup II, and 46.3±1.7 N in subgroup III. For the 1.5 mm thickness samples, the lateral pull-out strength was 73.9±4.8 N in subgroup I, 70.1±2.8 N in subgroup II, 75.0±2.2 N in subgroup III. No signs of carbonization or substantial cracking were visible in any of the bone samples. Conclusion: Site preparation with laser ablation significantly improved the lateral pull-out strength over mechanical preparation and control (no site preparation) in thinner cortical bone samples (1.0 and 0.5 mm). Such improvement in lateral pull-out strength decreases as the samples become thicker and diminishes in thick (1.5 mm) cortical bone samples.

Abstract:Objectives: This in vitro study aimed to evaluate the effect of placing two mini-dental implant (MDI) at different heights on the retention form of the lower over denture (OD). Materials and Methods: Sixteen acrylic mandibular blocks and their corresponding ODs were prepared and divided into four groups (four sets per group) to represent an atrophic mandibular ridge with two MDIs placed parallel to each other at inter-implant heights (IHs) of 0.0, 1.5, 3.0, and 4.5 mm. The OD blocks were mounted on a manual pulling stand and pulled vertically 12 times to measure the peak tension load (N). The data were analyzed using one-way analysis of variance (ANOVA), and the differences between the means were compared ( P < 0.05). Results: One-way ANOVA showed a statistically significant effect of IH on the retention force of the OD blocks ( P < 0.05). The mean retention force (N) for evenly placed MDIs was as follows: 32.2 ± 0.95, 1.5 mm IH: 30.1 ± 0.56, 3.0 mm IH: 26.1 ± 0.71, and 4.5 mm IH: 27.4 ± 0.90. Conclusion: The retention force of the OD blocks was significantly affected by the vertical inter-implant distance. A significant reduction in the mean retention force was observed for ODs retained by MDIs placed at an IH >1.5 mm.
Bone quality affects stability of orthodontic miniscrews

Abstract:Abstract The objective of this study was to evaluate the effect of bone–miniscrew contact percentage (BMC%) and bone quality and quantity on orthodontic miniscrew stability and the maximum insertion torque value (ITV). Orthodontic miniscrews of five different dimensions and several bovine iliac bone specimens were used in the evaluation. Miniscrews of each dimension group were inserted into 20 positions in bovine iliac bone specimens. The experiment was divided into three parts: (1) Bone quality and quantity were evaluated using cone-beam computed tomography (CBCT) and microcomputed tomography. (2) The 3D BMC% was calculated. (3) The ITVs during miniscrew insertion were recorded to evaluate the stability of the orthodontic miniscrews. The results indicated that longer and thicker miniscrews enabled higher ITVs. CBCT was used to accurately measure cortical bone thickness (r = 0.939, P < 0.05) and to predict the bone volume fraction of cancellous bone (r = 0.752, P < 0.05). BMC% was significantly influenced by miniscrew length. The contribution of cortical bone thickness to the ITV is greater than that of cancellous bone structure, and the contribution of cortical bone thickness to BMC% is greater than that of cancellous bone structure. Finally, the higher is BMC%, the greater is the ITV. This study concludes that use of CBCT may predict the mechanical stability of orthodontic miniscrews.

Abstract:Statement of problem Optimal implant stability and preventing complications such as screw loosening are paramount concerns for implant-supported prostheses. However, studies examining the influence of various internal connection designs on detorque values and removal forces, critical aspects of implant success, are lacking. Purpose The purpose of this in vitro study was to assess the impact of the shape and design of the internal connection in tissue-level and bone-level implants on the detorque value and the force required for abutment removal from the implant. Material and methods Forty dental implants were securely mounted in 10×6×20-mm acrylic resin blocks positioned perpendicular to the surface. The implants were divided into 4 groups (n=10): bone-level SM Torx, tissue-level PSI Torx, bone-level UF Hex, and tissue-level UF Hex implants. After exposure to a dynamic loading test at 31.2 N, 2 Hz and 10 6 cycles, measurements were made of both detorque values and removal forces. Statistical analyses, including 1-way ANOVA with a post hoc Tukey test and Kolmogorov-Smirnov test, were conducted to assess the results (α=.05). Results The differences in detorque values among the 4 groups were statistically similar ( P =.087). In terms of removal force values, tissue-level PSI implants exhibited the highest values, while bone-level UF implants had the lowest values, with significant differences in the removal forces among the 4 groups ( P <.001). Pairwise comparisons revealed significant differences among the groups ( P <.001), except for the comparison between tissue-level PSI and bone-level SM implants ( P =.108). Conclusions While detorque values remain consistent across implant types, the shape and design of the internal connection of implants has a significant impact on the removal force required for abutment detachment from the implant.

Abstract:Background Achieving adequate primary stability for dental implants in the posterior maxilla is challenging due to the low bone density and the presence of fine trabecular bone in the area. Low primary implant stability can compromise the long-term success of the implant. Purpose the study aims to evaluate the effect of intentionally undersized osteotomy sites on the insertion torque of implants placed in type 4 bone density in the posterior maxilla, regardless of the surgeon's tactile sense of the bone density. Materials and methods Conventional implant placement technique was performed in the posterior maxilla by intentionally omitting the last drill of the recommended drilling sequence protocol. The insertion torque was measured using an integrated surgical motor software. Results The study included 72 implants in both genders and different age groups. The retrieved samples were divided into four groups with insertion torque ranging from 11 Ncm to 48 Ncm. The mean insertion torque was 26.51±0.18 Ncm for all cases (P=0.001). No significant difference was found in the mean insertion torque between different age groups (26.51±2.845, P=0.84). However, a significant difference was observed between male and female cases (P=0.84). Conclusions Most implant insertion torque measurements vary significantly between cases in the intentionally under-prepared posterior maxillary osteotomy site. Omitting the last drill should be applied for all the preparations of the posterior maxillary implant sites. Minimizing the osteotomy site intentionally is recommended for all posterior maxillary implant cases.

Abstract:Background: In order to obtain satisfactory treatment outcomes, orthodontic space closure frequently requires trustworthy anchoring devices. Because they are easier to install and smaller than regular implants, mini-implants have become a viable option. The purpose of this study was to evaluate the effectiveness of regular implants versus mini-implants as orthodontic anchoring devices for closing gaps in a tertiary care context. Methods: A total of 100 patients who needed space closure were split into two groups at random and given regular implants (group B, n = 50) or mini-implants (group A, n = 50). The rate of space closure, anchoring loss, and patient pain were measured using visual analog scales as the primary end measures. Resonance frequency analysis was used to assess implant stability. Based on implant survival and the lack of peri-implant problems, success rates were calculated. Findings: Mini-implants showed much lower rates of patient pain (P < 0.001) and anchoring loss (P < 0.05) than normal implants; however, they did show comparable rates of space closure (P = 0.42). In addition, mini-implants had comparable success rates (P = 0.78) and greater implant stability (P < 0.01) when compared to normal implants. Conclusion: In conclusion, mini-implants have benefits in anchorage preservation, patient comfort, and implant stability over regular implants when used as orthodontic anchoring devices for space closure. They also offer equivalent effectiveness. These results provide credence to the use of mini-implants as practical substitutes in orthodontic practice.
Factors affecting success and failure of orthodontic mini-implants: A retrospective review.

Abstract:Objective: To determine the factors associated with success and failure of orthodontic mini-implant (MI) in orthodontic patients. Study Design: Retrospective Review. Setting: Department of Orthodontic, Sharif Medical and Dental College. Period: September-November 2021. Material & Methods: The files and records of all patients from last 3 years fulfilling the selection criteria were reviewed by an orthodontic consultant and two post-graduate residents and data was recorded in a predesigned proforma. All mini-implants were inserted by a single operator (orthodontic consultant) with a hand driven autoclaved screwdriver compatible with Dentaurum Mini-Implant (Tomas Pin©). Length and Diameter of all mini-implants were chosen to be constant with 8mm length and 1.5mm diameter. All patient-related and location-related factors related to success of mini-implant were assessed. Results: The success rate in females (85.9%) was higher than in males (69.5%) and there was significant difference between male and females (p=0.023). The TADS inserted in higher bone quality (Q1=95.8%, Q2=94.4%) showed a significant difference (p=0.001) in success rate than those inserted in lower bone quality (Q3=38.1%, Q4=23.1%). Conclusion: Bone quality and gender were significant factors, which impact the clinical performance of orthodontic mini-implants. Other patient-related and location-related factors did not significantly impact the success rate of orthodontic mini-implants.

Abstract:ObjectiveThis study aims to evaluate success factors implicated in clinical orthodontic miniscrew stability after their interradicular placement in maxilla.Materials and methodsSix hundred seventy-six miniscrews were inserted in maxillary interradicular sites in a sample of 276 patients (109 males and 167 females; mean age 19 ± 1.7 years) and immediately loaded. Percentage failure rate was recorded, and the influence of the following factors was investigated: structural (miniscrew length, diameter and body shape), operative (side of insertion site, pilot hole drilling or not) and biological (maximal insertion torque [MIT] and type of gingiva). A chi-square test with Monte Carlo correction was performed to detect the influence of these variables on the failure rate of orthodontic miniscrews. Then both multivariate logistic regression and post hoc analysis were performed, followed by classification and regression tree (CART) analysis.ResultsThe average success rate was 88%. The principal factors implicated in the failure rate were miniscrew length, MIT values and type of gingiva. Specifically, 8 mm miniscrew length, alveolar mucosa and 5–10 Ncm MIT values were linked to higher failure rates. According to CART, the main variable influencing failure is miniscrew length (≤ 8 mm for higher failure rates). For others, MIT values of 5–10 Ncm are linked to higher failure rates (p < 0.05).ConclusionOrthodontic miniscrews inserted in the maxilla display good success rates. However, clinicians should be discouraged from using miniscrews of length ≤ 8 mm and MIT values < 10 Ncm, even with longer miniscrews.Clinical relevanceInformation about factors related to failure rate of miniscrews placed at posterior maxillary interradicular sites is given.

Abstract:The present article describes data from systematic review and meta-analysis investigating the efficacy and safety outcomes comparing mini-implants (MIs) and conventional anchorage reinforcement in patients with maximum dentoalveolar protrusion. All relevant RCTs and non-RCTs published up to 2018 were collected from PubMed, Embase and Cochrane database. Thirteen studies assessing the effect of mini-implants were included, of which 4 were randomized controlled trials (RCTs) and 9 observational studies. The efficacy parameters include mesiodistal movements of molars and incisors and vertical movements of molars and incisors. Whereas, the safety parameters were angular and linear measurement of soft tissue change. Subgroup analysis data was provided in terms of patients average age (<18 years and >= 18 years) at the initiation of treatment. This dataset is suitable for research purpose in the field of orthodontics and also helps dental doctors to determine their treatment preferences in the choice of anchorage reinforcement. Effect of residual bone height and implant macro-design on primary stability in sinus floor elevation: an ex-vivo study

Abstract:Objectives: To evaluate the influence of residual bone height (RBH) and implant macro-design on primary stability (PS) of implants using a simultaneous sinus floor elevation (SFE) and implant insertion model. Method and Materials: Fresh bovine rib samples, resembled type-IV density which confirmed by computerized tomography, were prepared to represent four groups of varying RBHs (3,6,9, and 15 mm). To simulate simultaneous implant insertion with SFE, 120 implants in different macro-designs (group R: NobelReplace; group P: NobelParallel; and group A: NobelActive, Nobel Biocare, Gothenburg, Sweden) were randomly inserted at RBHs of 3, 6, 9, and 15 mm in each rib. Implant stability quotient (ISQ) were measured immediately after implant insertion. Results: RBH and implant macro-design have an impact on ISQ values (P < .001). ISQ values were the highest with RBH of 15 mm, followed by RBHs of 9, 6, and 3 mm. (P <.001). There was no statistically significant difference between different implant macrodesigns at RBHs of 3 and 15 mm regarding ISQ values (P = .111, P = .551). ISQ values of group P were higher than of group R and group A at a RBH of 6 mm (P =.049, P = .029). ISQ values were also higher in group P compared to group A at a RBH of 9 mm (P = .006). Conclusion: The higher PS may be expected in sites with higher RBH, regardless of the macro-design. In addition, cylindric implant design may enhance the PS at RBHs of 6 and 9 mm in simultaneous implant insertion with SFE.
