Abstract:A rare congenital condition known as hydrometrocolpos (HMC) is characterized by the buildup of secretions in the vagina and uterus as a result of distal obstruction; this condition is frequently linked to imperforate hymen. This case report presents a prenatal diagnosis of isolated HMC in a 30-year-old primigravida, detected at 36 weeks gestation. Initial ultrasound revealed a large cystic mass, confirmed by fetal magnetic resonance imaging to be HMC. Postnatal examination of the new born confirmed imperforate hymen and HMC, which was effectively treated with hymenotomy, resulting in early resolution. This case underscores the importance of prenatal imaging in the early identification and management of HMC to prevent potential complications.
Microperforate Hymen: A Rare Case of Pregnancy and Miscarriage

Abstract:Microperforate hymen is a congenital defect where the hymenal ring covering the vaginal opening is abnormally small, leading to problems such as painful intercourse, difficulty inserting tampons, and complications during menstruation and pregnancy. The hymen, formed during fetal development, undergoes canalization before birth, with deviations in this process leading to conditions like imperforate or microperforate hymen. This case report discusses a 28-year-old primigravida diagnosed with a microperforate hymen during the management of a missed miscarriage. Following routine investigations, she underwent a hymenectomy and suction evacuation, which alleviated the immediate issue. Despite an uneventful post-operative period, the patient experienced dyspareunia three months later, necessitating further treatment with serial dilators. A microperforate hymen can cause significant reproductive health issues, including menstrual irregularities, recurrent infections, and infertility. Timely diagnosis and appropriate surgical intervention are crucial in managing this rare condition, improving patient outcomes and quality of life.

Abstract:Background: Imperforate hymen is an uncommon obstructive anomaly of the developing female reproductive tract. There are occasional case reports of imperforate hymen occurring in family clusters, suggesting a plausible familial mode of inheritance. We describe a set of monozygotic premature twins with imperforate hymen noted at birth, whose mother was diagnosed with the same condition as a teenager. We also elucidate the likely underlying mode of inheritance of imperforate hymen. Method: We utilized the CARE (Case Report) guideline in reporting the cases. Case presentation: These are monozygotic twins born prematurely at 30 weeks of gestation, noted at birth to have bulging cyst-like structures protruding from their vaginas. The twins were not dysmorphic and did not have any other congenital malformations. Over the next few weeks, these cyst-like structures (mucoceles) became less prominent. The genital anomaly was diagnosed as imperforate hymen. Their mother was also diagnosed with an imperforate hymen when she was 12 years old and was treated with hymenectomy. Discussion: This unique occurrence of imperforate hymen in a set of premature monozygotic twins and their mother suggests a plausible autosomal or X-linked dominant mode of inheritance. Given the role of genetic inheritance in imperforate hymen development, it is important to screen female relatives of an index case for this genital anomaly.

Neonatal pyocele originating from a urinary tract infection: a case report

Abstract:Background: The origins of pyocele are primarily idiopathic, with reports suggesting the spread from intraperitoneal or hematogenous infection. However, we found no information in the literature regarding the pathogen's spread from the urinary tract. Case presentation: We report here a case of a 23-day-old term Iranian boy (Fars ethnicity) with complaints of new-onset fever, irritability, poor feeding, right hemiscrotal erythema, and edema. Moreover, the physical examination revealed an enlarged, erythematous, tender right hemiscrotum and edematous scrotal walls. Along with leukocytosis and elevated C-reactive protein levels, the urine culture indicated an infection with Escherichia coli. However, as the color Doppler ultrasonographic examination was inconclusive, he underwent surgical exploration on which a hydrocele sac with reddish cloudy pus fluid was noted, with its culture indicating growth with the same mentioned pathogen. Therefore, an appropriate antibiotic regimen was administered, and the patient was discharged a few days later after achieving full recovery and demonstrating no urinary tract structural abnormalities. Conclusion: In neonatal pyocele, the spectrum of evaluating the source of the infection should also be extended to the urinary tract. Moreover, administering suitable antibiotics would produce favorable results in cases with no structural abnormalities.

Abstract:Female genital tract abnormalities are relatively uncommon and are usually discovered accidentally. These types of abnormalities are usually asymptomatic and do not require special treatment. The hymen is a type of connective tissue membrane in the perineum, which closes the vaginal orifice. Hymen atresia is a condition in which the vagina is abnormally closed or absent. It usually occurs in infancy at an incidence of 0.014%~0.100%, [ 1 ] causing vaginal hemorrhage and fluid accumulation. Hymen prolapse is asymptomatic and is commonly found during the examination of the baby external genitals. So far, only a few studies reported on hymen prolapse. Here we reported a single case of congenital hymen prolapse in a baby girl, which was discovered by ultrasound of the fetus and consequently confirmed after the baby was born.

Abstract:Introduction: Hymeneal imperforation is a rare genital malformation often discovered during abdominopelvic emergencies. Hematocolpos typically presents with pelvic pain, a palpable mass, and primary amenorrhoea. The diagnosis is confirmed by clinical evaluation and imaging studies such as ultrasound. Case presentation: A 13-year-old girl presented a four-week history of abdominal distension and pain, which worsened over time, along with constipation, but without vomiting or fever. Examination revealed stable vital signs and abdominal guarding. A gynecological exam showed an imperforate hymen. Ultrasound identified a hypoechoic fluid collection in the retrovesical area. Hematocolpos was diagnosed and surgical intervention involved opening the hymen and releasing 800 cc of blood. The patient remained stable after the operation and was discharged painless after two days. Discussion: Imperforate hymen, resulting from incomplete resorption of the hymeneal membrane during embryonic development, is a common cause of hematocolpos. Symptoms often manifest in menarche, with cyclical pelvic pain and primary amenorrhoea. The diagnosis is based on physical examination and imaging, while early intervention prevents complications such as endometriosis and infertility. Surgical treatment varies from hymenotomy to more complex reconstructive procedures based on the underlying cause. Conclusions: Although rare, imperforate hymen is the most prevalent congenital anomaly of the vagina, often remaining asymptomatic until menarche. Accurate diagnosis and timely surgical intervention are essential to avoid severe complications. This case highlights the importance of comprehensive clinical evaluation and appropriate imaging in the management of hematocolpos.

Abstract:A rare disorder known as imperforate hymen is characterized by congenital obstruction in the hymen. It can occur once every 1000 women. The majority of cases are diagnosed in teenage years and present with symptoms of an expanding pelvic mass, bulging mass in the vagina, abdominopelvic pain and absence of mensturation. Ultrasonography, computed tomography (CT) and magnetic resonance imaging (MRI) are used to evaluate patients with hemato/hydrocolpos. The cystic dilatation is observed in the vagina and this dilatation resulting in notable compression on the anterior urinary bladder and the posterior rectum in the cases of imperforate hymen, typically. Dilatation in uterin cavity and fallopian tubes may be observed in some cases. Adult cases previously published in the literature are micro perforated cases. We reported a case of imperforate hymen in a 24-year-old adult patient with urinary retention symptoms had amenorrhea. Abstract:Background Imperforate hymen is a congenital anomaly of the female genitalia wherein the opening to the vagina is completely obstructed by hymenal tissue and affects 1/2000 patients. Several other hymenal variants, including septate and microperforate hymen, may mimic the presentation of imperforate hymen. This may lead to misdiagnosis of hymenal variants by providers who are less familiar with examination and diagnostic methods for proper diagnosis of congenital anomalies of the female reproductive tract. The objective of this study was to review how an initial diagnosis of hymenal variants may change after referral to pediatric and adolescent gynecology (PAG) specialists. Methods A retrospective chart review of patients who underwent a hymenectomy procedure from 2018-2023 was performed on the basis of CPT codes. IRB approval was obtained from our institution. Medical records were reviewed by two researchers. Data regarding age, sociodemographics, reason for referral, and specialty of the initial examining provider were collected. Descriptive statistics were performed. Comparisons were made between initial diagnosis and final diagnoses of hymenal variants among patients who underwent a hymenectomy. Results 143 patients were included in our study population. Average age of presentation was 14.2 years (SD=2.4 years). 104 patients were referred to PAG from outside specialties: Pediatrics (71.2%), Adult Ob/Gyn (9.6%), Emergency Medicine (5.8%), Self-referral/unknown (7.7%), Other subspecialties (5.8%). Reasons for referral included difficulty with tampon insertion/removal (38.5%), abnormal uterine bleeding (8.4%), dysmenorrhea (6.3%), and other reasons/asymptomatic (16.8%). Notably, when patients underwent initial evaluation by a non-PAG provider, the initial and final diagnoses differed 44% of the time after referral to a PAG specialist. Conclusions Misdiagnosis of hymenal variants occurred nearly half of the time when patients were initially seen by non-PAG providers in this study. PAG providers offer unique expertise in gynecologic examination and appropriate diagnostic work-up of pediatric patients. Our study highlights a disparity in appropriate gynecologic care of pediatric patients with complex gynecologic conditions, as well as a need for early involvement of PAG-trained providers in the assessment of congenital anomalies of the reproductive tract. Increased availability of PAG providers may improve rates of misdiagnosis, delayed diagnosis, and inappropriate surgical management of these complex gynecologic conditions.

Abstract:Transverse Vaginal Septum is a rare congenital anomaly of the female genital tract. It may be obstructive or non-obstructive, and patients with obstructive features are diagnosed earlier. Common presentations include lower abdominal pain, urinary retention, and constipation in the background of primary amenorrhoea. Herein, we present a case of a 13-year-old adolescent girl who presented with acute lower abdominal pain and urinary retention. A mid-vaginal transverse septum was observed on vaginal examination. On sonography, signicant hematocolpos were visualised and a septate uterus was noted. The patient underwent a complete excision of the transverse septum. The postoperative period was uneventful, and the patient began to have normal menstruation. Early diagnosis, adequate surgical management, and proper follow-up are key to successful management of transverse vaginal septae. Further research is necessary to better classify and standardise treatment protocols due to the paucity of reported cases.
Beta-adrenergic receptor levels and function after growth of S49 lymphoma cells in low concentrations of epinephrine.

Abstract:Growth of S49 wild-type (WT) lymphoma cells for 24 hr with 3 nM epinephrine produces a very pronounced attenuation of cAMP accumulation in response to subsequent challenges with much higher concentrations of the catecholamine [Mol. Pharmacol. 36:459-464 (1989)]. We report here the effects of this treatment, in S49 WT, cyc-, and kin- cells, on the responsiveness of adenylate cyclase in partially purified membranes. The desensitization of adenylate cyclase in the S49 WT cells after 24-hr treatment was homologous, in that only responses to epinephrine were attenuated. The EC50 for epinephrine stimulation of adenylate cyclase was 54 +/- 8% (mean +/- standard error) higher in treated cells than in controls, and there was a 32 +/- 3% reduction in Vmax at supramaximal epinephrine concentrations. The treatment also caused a 34 +/- 9% reduction in the levels of the beta-adrenergic receptor (beta AR), which was of a sufficient magnitude to account for the homologous desensitization seen. The 24-hr treatment had similar effects in S49 kin- cells, where we observed a 28 +/- 4% decrease in Vmax, a 35 +/- 6% increase in EC50 for epinephrine stimulation of adenylate cyclase, and a 25 +/- 3% decrease in beta AR. In contrast, the 24-hr treatment had no measurable effect on adenylate cyclase activity in S49 cyc- cells. That is, the responsivity of adenylate cyclase reconstituted with Gs from S49 WT cells was not attenuated, although beta AR levels were significantly decreased. The desensitization of S49 cells with the 24-hr treatment was additive with that mediated by the cAMP-dependent protein kinase (cAPK). Further, unlike the cAPK-mediated attenuation, it was relatively insensitive to the levels of free Mg2+ in the adenylate cyclase reaction mixture. The characteristics of the desensitization produced by 24-hr treatment with 3 nM epinephrine, together with the observation that it is similar in S49 WT and kin- cells, demonstrates that the process in WT cells is, at least in part, independent of the rapid cAPK-mediated desensitization. It is also most likely that it is unrelated to the rapid cAMP-independent processes involving sequestration/internalization or the beta AR kinase, because those mechanisms require much higher receptor occupancies than the 0.2% occurring with 3 nM epinephrine. Thus, 24-hr treatment appears to produce attenuation of adenylate cyclase by causing down-regulation of beta AR, without involving any other known form of desensitization.

PRODUCT LINE MANAGEMENT AS DYNAMIC, ATTRIBUTE-LEVEL COMPETITION

Abstract:Product lines are composed of SKUs from the same manufacturer which compete directly with one another. Understanding how consumers choose among numerous alternatives of brand, size and format is complicated by the multitude of possibilities among which they can switch over time. We approach this problem in terms of dynamic attribute satiation: that households seek out specific attributes over time, and that their switching patterns will explicitly reflect this tendency. We formulate a dynamic model of sequential choice which relies on the following insight: although households may entirely disagree on the relative merits of two products, they are far more likely to agree on how similar they are. That is, while product preferences may differ markedly across households, product positionings will not. The resulting model offers a number of unique benefits over and above parsimoniousness. Operationalizing product similarity in terms of observed attributes – information typically available in existing scanner panel data – describes the data better than sidestepping such attribute information in lieu of unobserved attributes. Estimated “part worths” enable one to characterize how substitutable or complementary two products are from the consumption perspective of households: for example, in one category, we find brand name to be a crucial determinant of how similarly two products are perceived, though in another apparently similar category, size is the critical attribute. While on the one hand this suggests that manufacturers may be able to capitalize upon a strong brand name by umbrella branding, it also suggests that products may cannibalize one another’s sales if other attributes in the product category are perceived to be unimportant. Switching probabilities depend on household-level inertia as well as on inter-product similarities, allowing an understanding of both these effects as well as their interplay. This stands in contrast with state dependence models that ignore inter-product similarities, which force different products to be always complements for a variety-seeking household, and always substitutes for an inertial household. The model accounts for patterns of asymmetric substitutability, and suggests that complementarity and substitutability should not be construed solely at the category level. Urethral duplication as cause of incontinence in a 5-years-old female: Surgical approach

Abstract:Introduction: Duplication of the urethra is a rare congenital anomaly in females which, if left unidentified, might cause refractory urinary incontinence. This video demonstrates its identification and treatment. Patient & methods: A 5-year-old female was referred for primary refractory diurnal incontinence without any dry moment since birth. Initial clinical examination showed permanent urine leakage from an opening cranial to the clitoris. Ultrasound did not show any bladder or kidney abnormalities. A diagnostic cystoscopy under anesthesia revealed an accessory meatus located cranially to the orthotopic meatus and to the clitoris. Results: Lying in dorsal decubitus under general anesthesia, catheters were inserted in respectively the orthotopic and accessory meatus. The accessory meatus and urethra were dissected away into the bladder dome from a circumferential skin incision. Defects were closed and bladder drainage per urethram was left in situ for 10 days. P ostoperative recovery was uncomplicated. Complete resolution of the incontinence was immediately achieved at catheter removal. Daytime and night-time continence are observed with 3 years follow-up. Conclusion: Urethral duplication is a rare finding in females but should be excluded during the workup of primary incontinence. In girls with isolated urethral duplication, resection of the accessory urethra is the best surgical option.
Congenital Neonatal Intestinal Obstruction: Retrospective Analysis at Tertiary Care Hospital

Abstract:Background: The purpose of this study is to analyze the etiology, clinical presentation and outcome of neonatal intestinal obstruction at our institute.Materials and Methods: The medical record of all the patients, presented with intestinal obstruction in neonatal period during 2014 and 2015 was reviewed retrospectively for etiology, clinical features, investigations, management, and outcome.Results: Out of total 53 cases of neonatal intestinal obstruction, 27 were of intestinal atresia (9 cases (17%) were of duodenal atresia, 7 (13%) were of jejunal atresia and 8 (13%) were ileal atresias and 3 cases were found with colonic atresia); 7 were malrotation, 17 were Hirschsprung’s disease (HD). All the patients were investigated with abdominal radiography and sonography. All patients were managed surgically. Overall mortality was 10/53 (18.8%). Out of 27 cases of atresia, 9 patients died (33% mortality). Septicemia was the cause of death in 7 patients (58.3%). Anastomotic leak was present in one mortality case.Conclusion: The most common cause of neonatal intestinal obstruction is atresia. Duodenal atresia was the most common atresia in our study followed by ileal atresia. Postoperative complications like septicemia led to most of deaths in our series. Septicemia, wound infection, hypothermia, prematurity need special attention for survival of neonates.
Bowel Perforation Secondary to Ventriculoperitoneal Shunt: Case Report and Clinical Analysis.

Abstract:Bowel perforation is an unusual complication of ventriculoperitoneal shunting. This article describes a case of bowel perforation associated with a ventriculoperitoneal shunt inserted in an 8-month-old male infant for meningocoele and hydrocephalus. Ten months after insertion of the shunt the infant presented with the shunting tube protruding through the anus. There were no signs of meningitis or peritonitis. At laparotomy the tube was seen to enter the transverse colon and was encapsulated by the greater omentum. The tube was cut and the distal end removed via the anus. The transverse colon was repaired. The catheter continued to function effectively and the patient remained asymptomatic. The literature on this rare complication is reviewed and the therapeutic options are discussed.

Abstract:Prenatal diagnosis of left multicystic dysplastic kidney (MCDK) and ureterocele was made in a male fetus at 24-weeks gestation. The initial postnatal course was uneventful. At four weeks of age he developed urosepsis and respiratory distress. Ultrasound and MR urogram demonstrated a duplicated left collecting system with massive upper pole dilation and left ureterocele. Ureterocele puncture was performed, though failed to improve the clinical picture. Upper pole nephrostomy tube placement and subsequent upper pole heminephrectomy were performed and symptoms resolved. This case highlights the complexity of diagnosing and managing congenital anomalies of the kidney and urinary tract (CAKUT).

Abstract:An indirect inguinal hernia is a commonly seen congenital condition that can impact infants within their first year of life. An inguinal hernia arises when a portion of the intestines, omentum, or reproductive structures herniate into the scrotal sac or labia through the patent processus vaginalis. These hernias occur more frequently in preterm newborns. The contents of a hernia often consist of the small bowel, colon, omentum, and either the ovary or testicle. The occurrence of a uterus in a herniated sac is uncommon. The presence of a uterus, ovary, and fallopian tube is exceptionally rare, with only a few cases reported in the literature. We present a unique case of a 10-day-old female neonate who was delivered at 37 weeks of gestation and brought to the Paediatric Outpatient Department with swelling in the left inguinal region that had been present for the past five days. During the clinical examination, an irreducible mass was found in the left inguinal region. The ultrasound scan showed the uterus, ovary, fallopian tube, and minimal free fluid in the herniated sac. Colour Doppler evaluation of the uterus and ovaries revealed good vascularity. A diagnosis of a left inguinal hernia containing the uterus, left ovary, and fallopian tube, with no signs of ovarian torsion, was established. We performed a surgical procedure in which the likelihood of adhesions was taken into consideration, and the organs were removed from the hernial sac. We conducted the reintegration of the organs back into the pelvis, ligation of the high sac, and further repair of the internal inguinal ring to prevent the recurrence of the hernia. The surgical procedure was successful, and the postoperative period was without any complications. After the surgery, the patient was advised to have clinical and radiological follow-up for a period of one year. We recommend that a high-resolution ultrasound (HRUS) scan be routinely performed in neonates with an asymptomatic or symptomatic palpable mass in the inguinal region for early diagnosis and characterization of the herniated structures, as well as to assess their viability.
Primary Reconstruction of Bladder Neck Transection and Vaginal Laceration Post Blunt Abdominal Injury a Case Report

Abstract:Blunt abdominal injury is not uncommon in the paediatric population. The presentation can be immediate or delayed, and if not promptly recognized may be attended with complications such as septicaemia, shock or death. Simple, readily available and cheap imaging techniques such as abdominal radiograph and ultrasound scan still remain relevant in the diagnosis of blunt abdominal injuries where abdominal computerized tomography is unavailable or unaffordable. This can expedite the decision for an early surgical exploration. In girls, complete bladder neck transection with vaginal wall injury is a rarely reported and unusual association of blunt abdominal injury more so with delayed presentation in the absence of an associated pelvic fracture. This rare trauma entity in a child can pose a diagnostic difficulty leading to delayed presentation. This is a case of 3 year old girl with 3 weeks history of trauma to the abdomen from collapsed building fence during a heavy downpour. She later developed progressive abdominal distension with pain, constipation, vomiting, fever and drainage of clear fluid per vaginam. She had visited several hospitals but was managed non-operatively. Over the period the patient had some relief of symptoms until she presented to our facility with poorly defined features of peritonitis following a referral from a peripheral hospital. Abdominal ultrasound scan revealed intraperitoneal fluid collection. She was resuscitated and had exploratory laparotomy with primary reconstruction of the bladder neck transection and vaginal wall laceration. She did well immediate postoperative except for the superficial surgical site infection and during follow-up visits. Primary repair of traumatic bladder neck transection and vaginal wall laceration in paediatric patients with delayed presentation of blunt abdominal injury is a feasible option of management. However, long term follow up is recommended as secondary procedures may become necessary.
