Abstract:Introduction: Hymeneal imperforation is a rare genital malformation often discovered during abdominopelvic emergencies. Hematocolpos typically presents with pelvic pain, a palpable mass, and primary amenorrhoea. The diagnosis is confirmed by clinical evaluation and imaging studies such as ultrasound. Case presentation: A 13-year-old girl presented a four-week history of abdominal distension and pain, which worsened over time, along with constipation, but without vomiting or fever. Examination revealed stable vital signs and abdominal guarding. A gynecological exam showed an imperforate hymen. Ultrasound identified a hypoechoic fluid collection in the retrovesical area. Hematocolpos was diagnosed and surgical intervention involved opening the hymen and releasing 800 cc of blood. The patient remained stable after the operation and was discharged painless after two days. Discussion: Imperforate hymen, resulting from incomplete resorption of the hymeneal membrane during embryonic development, is a common cause of hematocolpos. Symptoms often manifest in menarche, with cyclical pelvic pain and primary amenorrhoea. The diagnosis is based on physical examination and imaging, while early intervention prevents complications such as endometriosis and infertility. Surgical treatment varies from hymenotomy to more complex reconstructive procedures based on the underlying cause. Conclusions: Although rare, imperforate hymen is the most prevalent congenital anomaly of the vagina, often remaining asymptomatic until menarche. Accurate diagnosis and timely surgical intervention are essential to avoid severe complications. This case highlights the importance of comprehensive clinical evaluation and appropriate imaging in the management of hematocolpos.

Abstract:Objective To review the clinical characteristics of urethral prolapse in female children and summarize our experience of treatment.Methods A retrospective analysis of the clinical characteristics of 102 patients with urethral prolapse from January 2007 to December 2017 was conducted at The Children's Hospital of Zhejiang University School of Medicine.The age of the patients ranged from 8-156 months with an median of 80 months.The presenting symptoms in the 102 girls were:bleeding in 57 patients (55.9％),mass in 31 patients (30.4％),and dysuria/urinary frequency,urgent and pain in 14 patients (13.4％).In all,58 patients were managed conservatively with Sitz baths as their masses were small,39 underwent prolapse reduction under topical anesthesia and Sitz baths because their mass were large,and 5 patients were treated by excision of the prolapsed urethral mucosa with four-quadrant excisional technique because thrombosed urethral prolapse at first visit.Results A total of 89 patients were cured after conservative treatment (87.3％),8 patients were converted to surgical treatment because frequent recurrence with conservative treatment.No urethral stricture,active hemorrhage and recurrent were found in 13 patients after operation.Conclusions The most common clinical manifestations of urethral prolapse are urethral mass and bleeding.Most patients can be cured by conservative treatment.The patients whose symptoms were severe or suffered from frequent recurrence of urethral prolapse should be managed with surgical excision.

Abstract:A rare disorder known as imperforate hymen is characterized by congenital obstruction in the hymen. It can occur once every 1000 women. The majority of cases are diagnosed in teenage years and present with symptoms of an expanding pelvic mass, bulging mass in the vagina, abdominopelvic pain and absence of mensturation. Ultrasonography, computed tomography (CT) and magnetic resonance imaging (MRI) are used to evaluate patients with hemato/hydrocolpos. The cystic dilatation is observed in the vagina and this dilatation resulting in notable compression on the anterior urinary bladder and the posterior rectum in the cases of imperforate hymen, typically. Dilatation in uterin cavity and fallopian tubes may be observed in some cases. Adult cases previously published in the literature are micro perforated cases. We reported a case of imperforate hymen in a 24-year-old adult patient with urinary retention symptoms had amenorrhea. Clinical analysis on 195 adolescent women with pelvic endometriosis

Abstract:Objective:To find its important to detect and treat earlier pelvic endometri-osis by analyzing the data of adolescent patients. Methods:The data of 195 adolescent patients with pelvic endometriosis were collected and confirmed by surgery from 1999 to 2015 . They were devided into groups according to r-AFS( Ⅰ,Ⅱ,Ⅲ,Ⅳ) ,genital malformation and its severity in endometriosis,respectively. The age,age of menarche,dysmenorrhea,BMI,delayed diagnosis time,the diagnostic accuracy of ultrasound and misdiagnosis were retrospectively compared be-tween groups. Results:The data showed that older age,longer delayed diagnosis time,higher in-cidence rate of dysmenorrhea and diagnostic accuracy rate of ultrasound were detected in women with stage Ⅲ,Ⅳ endometriosis,compared with those with stage Ⅰ,Ⅱ (P<0. 05). Meanwhile,the younger age,lower incidence rate of dysmenorrheal and diagnostic accuracy rate of ultrasound, shorter delayed diagnosis time,less severe cases were found in the genital-malformation group, however,more misdiagnosed cases were in the genital-malformation group (P<0. 05). Conclu-sion:Pelvic endometriosis is a developing and severe disease in adolescents. Its early detection and treatment are important,by which the adolescent female fertility is protected.

Abstract:The author reports a case of vaginal calculi secondary to urethrovaginal fistula with vaginal stenosis in a 14-year-old girl. The initial urethrovaginal fistula and vaginal stenosis resulted from pelvic trauma after a traffic accident, with subsequent surgical urethral realignment and anterior colporrhaphy without success. The patient had continuous urinary incontinence for a duration of 11 years after surgery, and was finally diagnosed with urethrovaginal fistula with primary multiple vaginal calculi in our hospital. Surgeries were performed to remove the stones and repair the urethrovaginal fistula. Colpoplasty was also given by an obstetrician–gynecologist to solve the vaginal stenosis. After the procedure, the girl urinated normally. At her 3-month follow-up, the patient had no sign of urine leakage.

Abstract:Background Imperforate hymen is a congenital anomaly of the female genitalia wherein the opening to the vagina is completely obstructed by hymenal tissue and affects 1/2000 patients. Several other hymenal variants, including septate and microperforate hymen, may mimic the presentation of imperforate hymen. This may lead to misdiagnosis of hymenal variants by providers who are less familiar with examination and diagnostic methods for proper diagnosis of congenital anomalies of the female reproductive tract. The objective of this study was to review how an initial diagnosis of hymenal variants may change after referral to pediatric and adolescent gynecology (PAG) specialists. Methods A retrospective chart review of patients who underwent a hymenectomy procedure from 2018-2023 was performed on the basis of CPT codes. IRB approval was obtained from our institution. Medical records were reviewed by two researchers. Data regarding age, sociodemographics, reason for referral, and specialty of the initial examining provider were collected. Descriptive statistics were performed. Comparisons were made between initial diagnosis and final diagnoses of hymenal variants among patients who underwent a hymenectomy. Results 143 patients were included in our study population. Average age of presentation was 14.2 years (SD=2.4 years). 104 patients were referred to PAG from outside specialties: Pediatrics (71.2%), Adult Ob/Gyn (9.6%), Emergency Medicine (5.8%), Self-referral/unknown (7.7%), Other subspecialties (5.8%). Reasons for referral included difficulty with tampon insertion/removal (38.5%), abnormal uterine bleeding (8.4%), dysmenorrhea (6.3%), and other reasons/asymptomatic (16.8%). Notably, when patients underwent initial evaluation by a non-PAG provider, the initial and final diagnoses differed 44% of the time after referral to a PAG specialist. Conclusions Misdiagnosis of hymenal variants occurred nearly half of the time when patients were initially seen by non-PAG providers in this study. PAG providers offer unique expertise in gynecologic examination and appropriate diagnostic work-up of pediatric patients. Our study highlights a disparity in appropriate gynecologic care of pediatric patients with complex gynecologic conditions, as well as a need for early involvement of PAG-trained providers in the assessment of congenital anomalies of the reproductive tract. Increased availability of PAG providers may improve rates of misdiagnosis, delayed diagnosis, and inappropriate surgical management of these complex gynecologic conditions.

A case report of urethral prolapse in young girls: Clinical manifestations and surgical management

Abstract:Background and importance: Urethral Prolapse (UP), first described by Solinger in 1732, is a rare condition characterized by the circular protrusion of the distal urethral mucosa through the external meatus, forming a hemorrhagic, sensitive vulvar mass. This condition is most commonly observed in prepubertal black females. This case report details the clinical manifestations and surgical management of UP in three young girls. Cases presentation: Three girls, aged 4, 5, and 6 years, were admitted with symptoms of vaginal blood spotting. Physical examinations revealed moderate bleeding and a red ring of congested, edematous tissue prolapsing through the urethral meatus. Diagnostic procedures confirmed UP, and surgical management was undertaken. The surgical approach involved the complete excision of the prolapsed tissue and mucosal-to-mucosal anastomosis under general anaesthesia. Postoperative follow-up over a mean period of 11 years showed no recurrence or urethral stricture. Clinical discussion: UP is a rare benign condition primarily affecting the female urethra, with several hypothesized etiologies, including weak pelvic floor structures and increased intraabdominal pressure. The typical presentation includes vaginal bleeding and a doughnut-shaped mass around the urethral meatus. Conservative management may be considered for mild cases, but surgical excision is recommended for severe cases, offering a safe and effective solution with low recurrence rates. Conclusions: UP in children, although rare, should be considered in cases of unexplained vaginal bleeding. Diagnosis is primarily clinical, and surgical resection provides a definitive and cost-effective treatment.

Abstract:Acute urinary retention caused by uterine incarceration due to adenomyosis is a rare phenomenon with limited documented cases. This report details the case of a 38-year-old female with acute urinary retention. A pelvic MRI revealed an enlarged retroverted uterus with a mass on the posterior left of the myometrium, indicative of adenomyosis. The size and anatomical location of the mass suggested uterine incarceration with direct pressure on the urinary bladder neck, leading to urinary retention. The patient underwent total abdominal hysterectomy, bilateral salpingectomy, and adhesiolysis. Subsequent follow-ups at one and three months post-surgery showed the resolution of urinary symptoms, underscoring the importance of considering adenomyosis as a potential cause of urinary retention in women with lower urinary tract symptoms. Timely recognition and appropriate intervention are crucial for mitigating complications and improving outcomes in these patients, as illustrated in this case.

Abstract:Identifying the etiology of acute pelvic pain in girls is often clinically challenging. Particularly in young girls, it is often difficult to determine if acute pelvic symptoms are originating from a gynecologic source or from a genitourinary or gastrointestinal etiology based on the child's clinical examination alone. Therefore, imaging plays a key role in establishing a diagnosis and in directing medical and surgical treatment. Pediatric gynecologic conditions, which can present acutely with pain or mass or both include ovarian torsion, hematometrocolpos, pelvic inflammatory disease, inguinal hernias containing an ovary or the uterus or both, adnexal cysts, pregnancy, vaginal foreign bodies, and ovarian vein thrombosis. Sources of pelvic pain in girls from nongynecologic etiologies include appendicitis, distal ureterolithiasis, and Crohn's disease. Owing to the lack of ionizing radiation, widespread availability, and lack of need for sedation, gray scale, with color Doppler ultrasound examination is often the initial imaging obtained when there is suspicion of gynecologic pathology. Radiographs of the abdomen and pelvis are commonly used in the acute setting, when there is concern for a bowel obstruction or free intraperitoneal air. Cross-sectional imaging with computed tomography or magnetic resonance imaging is used, when the diagnosis remains unknown and to clarify findings found on ultrasound and radiographs. Correctly identifying and diagnosing the causes of acute pelvic pain in girls is crucial for the care of these patients. Here, we review the spectrum of causes of acute pelvic pain in female infants to teens with emphasis on imaging approach and age-related characteristics.

Abstract:Uro-genital bleeding in pre-menarcheal girls always raises concerns regarding child abuse. There are serious social issues involved; however, before labeling it as sexual abuse, all efforts must be made to rule out an organic pathology. We recently encountered two young girls of Afro-Caribbean origin who presented with vaginal bleeding with concerns from referring physicians about child/sexual abuse. On examination both had a rare condition of urethral prolapse. One of them underwent four-quadrant excision and the other was treated conservatively with urethral catherization.

Abstract:Pelvic organ prolapse (POP) is a common condition mainly affecting postmenopausal women, characterized by the descent of pelvic organs through the vaginal canal. While often asymptomatic, POP can manifest with various symptoms such as a painless bulge or pressure sensation, abdominal pain, urinary complaints, and discomfort during intercourse. Severe cases can lead to urinary tract obstruction, hydronephrosis, and renal dysfunction. This case study presents an elderly female with bilateral severe hydronephrosis and pyelonephritis due to undiagnosed POP. Imaging revealed obstructive uropathy resulting from bilateral ureteric compression caused by cystocele and uterine prolapse. The patient's condition improved with antibiotics and supportive management. A vaginal hysterectomy was performed, which led to the resolution of the urinary tract obstruction. This case emphasizes the importance of considering POP in elderly women with urinary symptoms and the need for proactive screening. It highlights the significance of appropriate management to prevent irreversible renal damage. Different treatment modalities, including surgery and pessaries, are discussed, to emphasize the significance of tailoring treatments to individual patient characteristics.

Abstract:A rare congenital condition known as hydrometrocolpos (HMC) is characterized by the buildup of secretions in the vagina and uterus as a result of distal obstruction; this condition is frequently linked to imperforate hymen. This case report presents a prenatal diagnosis of isolated HMC in a 30-year-old primigravida, detected at 36 weeks gestation. Initial ultrasound revealed a large cystic mass, confirmed by fetal magnetic resonance imaging to be HMC. Postnatal examination of the new born confirmed imperforate hymen and HMC, which was effectively treated with hymenotomy, resulting in early resolution. This case underscores the importance of prenatal imaging in the early identification and management of HMC to prevent potential complications.

Abstract:Blunt abdominal injury is not uncommon in the paediatric population. The presentation can be immediate or delayed, and if not promptly recognized may be attended with complications such as septicaemia, shock or death. Simple, readily available and cheap imaging techniques such as abdominal radiograph and ultrasound scan still remain relevant in the diagnosis of blunt abdominal injuries where abdominal computerized tomography is unavailable or unaffordable. This can expedite the decision for an early surgical exploration. In girls, complete bladder neck transection with vaginal wall injury is a rarely reported and unusual association of blunt abdominal injury more so with delayed presentation in the absence of an associated pelvic fracture. This rare trauma entity in a child can pose a diagnostic difficulty leading to delayed presentation. This is a case of 3 year old girl with 3 weeks history of trauma to the abdomen from collapsed building fence during a heavy downpour. She later developed progressive abdominal distension with pain, constipation, vomiting, fever and drainage of clear fluid per vaginam. She had visited several hospitals but was managed non-operatively. Over the period the patient had some relief of symptoms until she presented to our facility with poorly defined features of peritonitis following a referral from a peripheral hospital. Abdominal ultrasound scan revealed intraperitoneal fluid collection. She was resuscitated and had exploratory laparotomy with primary reconstruction of the bladder neck transection and vaginal wall laceration. She did well immediate postoperative except for the superficial surgical site infection and during follow-up visits. Primary repair of traumatic bladder neck transection and vaginal wall laceration in paediatric patients with delayed presentation of blunt abdominal injury is a feasible option of management. However, long term follow up is recommended as secondary procedures may become necessary.

Abstract:Introduction The sexual experience ought to be pleasant and desirable, this case however demonstrates one of such unusual sexual events with catastrophic ending that may have resulted in fatality but for timely surgical intervention. The first coital experience for women may be associated with discomfort but when it is non-consensual the damaging effect of the trauma is deeper and may last for a lifetime. Objective To demonstrate unusual presentation of coital injuries with hemoperitoneum and its management in low resource settings to reduce immediate and long-term effects including death. Methods A case report of an 18-year old nulliparous lady with no prior sexual exposure who presented with vagina bleeding of 18hours following non-consensual penetrative sexual intercourse perpetrated by her religious cleric. Bleeding was in excess of 500mLs with associated lower abdominal pain and dizziness. She presented in hypovolaemic shock with severe anemia (hematocrit - 15%). There was ongoing vagina bleeding with mild abdominal distension. No skin injuries. Results She had genital tract exploration with findings of a 3cm peri-urethral vaginal laceration and a 6cm longitudinal rent on the posterior upper vagina extending to the cervix. The apex of the vaginal laceration could not be visualized from the vagina, necessitating exploratory laparotomy. Findings at surgery include 1.7L of hemoperitoneum and a transverse laceration on the posterior cervix extending into the lower uterine segment about 5cm long. The lacerations were repaired per vagina and per abdomen respectively with hemostasis secured. She had other supportive care including antibiotics, blood transfusion, analgesics, tetanus prophylaxis, Post-exposure prophylaxis for HIV and Hepatitis B immunoglobulin as well as emergency contraceptives. She was counselled with empathy and referred to the clinical psychologist for mental health support to prevent long term mental health effects. The assailant was arrested by the police and charged to court Conclusions Sexual assault may be associated with significant morbidities including unusual presentation such as hemoperitoneum and cervico-uterine laceration even in the absence of obvious skin injuries. Early presentation and appropriate intervention can help limit the adverse outcomes that may be associated as well as long term sequelae. Disclosure No.

Abstract:BACKGROUND: Complex female genital tract malformations account for 1.2% of all female genitourinary malformations. Although exceedingly rare, they can cause severe gynecologic symptoms in young women and lead to fertility problems.CASE: We present the case of a 13-year-old girl with primary amenorrhea referred for cyclic abdominal lower pain and menouria. Detailed diagnostics revealed uterus didelphys, transverse vaginal septum, and bilateral vesicovaginal fistulas. Laparoscopic left hemi-hysterectomy and salpingectomy were performed. The vesicovaginal fistula on the right side was excised, and the proximal vagina was anastomosed with the distal dimple. Since the operation, the patient has been pain-free and menstruating regularly from the right uterus.SUMMARY AND CONCLUSION: Preservation of the uterus should be considered in any case of complex female genital tract malformation and, as successful laparoscopic treatment advocates, a minimally invasive approach is feasible.

Abstract:PURPOSE The purpose of this 1-year follow-up study was to evaluate the behavior/progression of early carious lesions (active noncavitated carious lesions) by surface and type of tooth in early childhood. METHODS A total of 179 3-and 4-year-old preschoolers took part in this study. Clinical examinations were conducted by a calibrated examiner using a mirror, ball-ended probe, gauze for cleaning and drying of teeth, and artificial light. The World Health Organization criteria, with an added measurement of early carious lesions (ECLs), were employed for the caries examinations. Descriptive statistics and logistic regression were used in the statistical analysis. RESULTS After a 1-year follow-up, the study population developed 1.60 ± 1.64 new carious lesions. Children with caries activity at baseline showed much higher risks of developing new lesions than caries-free children (odds ratio=17.3 for ECL development, OR=24.5 for cavitations/fillings). Most ECLs remained active/unchanged after 1 year, whereas approximately 36% were arrested. Approximately 10% of the ECLs became cavitated, were filled, or were missing due to caries at follow-up. ECLs turned into cavities or fillings more frequently in the posterior region. CONCLUSION This study's findings support the conservative management of ECLs since, after 1 year, the majority of lesions were active/unchanged or were arrested on the smooth surfaces of primary teeth.

Abstract:We present a case of acute urinary retention (AUR) with hyper-angulation of the urinary bladder neck secondary to uterine leiomyoma. Our patient is a 45-year-old female who presented with AUR and suprapubic pain requiring catheterization. CT images highlight the level of obstruction secondary to suspension of the urinary bladder rather than direct urethral compression. This case highlights this unique manifestation of AUR demonstrating the necessity for understanding its different mechanisms. Clinicians should maintain a high index of suspicion for AUR in patients with leiomyoma and lower urinary tract symptoms. Heightened awareness and timely intervention are crucial in preventing potential complications.
