Abstract:Background The declining number of electrophysiologists pursuing academic research careers could negatively impact innovation for patients with heart rhythm disorders in the coming decades. Objective To explore determinants of research engagement after graduation from EP fellowship programs and evaluate associated barriers and opportunities. Methods A mixed methods survey of EP fellows and early career electrophysiologists was conducted, drawing from Heart Rhythm Society members. The survey encompassed 20 questions on demographics, research involvement, perceived research barriers, and perspectives on research time and opportunities. Responses were analyzed with robust Poisson regression. Results Among 259 respondents, those with dedicated research blocks during their fellowship had a significantly higher interest in future research (RR 1.15, p=0.04). The number of peer-reviewed publications modestly influenced interest in continued research (RR 1.0034 per publication, p < 0.0001), but there was no relationship to gender or race. Educational resources, networking opportunities, mentorship, funding, and protected time to enhance research engagement were important themes in the qualitative analysis, while key barriers to post-fellowship research were lack of mentorship, insufficient resources and time constraints in that order, particular with respect to women in research. Notably, no significant differences in barriers were observed between community training programs and academic centers. Conclusions Research experience and mentorship during EP fellowship were key determinants of subsequent research success after training, with similar findings by sex and race. These findings explain how fellowship training influences a physician's research practice post training and highlights opportunities to modify EP fellowships and augment research retention.

Abstract:The postnatal reorganization of rat proprioreceptive muscle afferent spinal terminal fields was explored by labelling transganglionically afferents from extensor digitorum communis with cholera toxin B sub-unit at different ages. Immunocytochemistry revealed labelled afferents in all segments examined (C4-T2) as well as retrogradely labelled motoneurones (C5-T1). Dorsal horn innervation appeared similar at all ages, but there were striking changes in the ventral horn. Many afferent boutons were seen closely apposed to labelled motoneurone proximal dendrites at postnatal day 7 (P7) and P14, but in the adult such contacts were almost entirely confined to distal dendrites. Between P7 and adult, a significant decrease in bouton density was found in the area dorsomedial to the labelled motoneurones that contained labelled dendrites and antagonist motoneurones. This anatomical reorganization may explain both the increasing stretch reflex threshold and its concomitant decrease in magnitude with age, and the reduction in excitatory connections to antagonist motoneurones, previously described in developmental neurophysiological studies.
An unusual case of hepatic carcinosarcoma.

Abstract:We report a rare case of a hepatic carcinosarcoma with rabdomyosarcomatous differentiation in its sarcomatous component. A 71-year old Caucasian female patient underwent a liver resection for a 4-cm lesion developed on an underlying HCV-related cirrhosis. Post-operative course was uneventful and the patient was discharged 5 days after surgery. At pathology, the tumor presented the features of hepatocellular carcinoma and rhabdomyosarcoma Three months later the patient experienced a liver recurrence, dying 2 months later for systemic disease. The reported case presents several peculiarities, i.e. the female gender, the HCV-related cirrhotic status, and the European origin of the patient. However, the outcome of our case confirms that this neoplasm pursues a highly aggressive course with poor prognosis.

Abstract:The thermoelastic martensitic transformations in Ti 4.>Ni5, and Ti48Nij2 alloys after isothermal aging have been studied by means of transmission electron microscopy and dilatometry. The presence of TUNi4 precipitates and the difference in precipitates size affect the martensite nucleation and martensitic transformation temperatures and sequences in TiNi alloys. Highly coherent and densely dispersed TUNi4 particles ( B19'. When sizes of Ti-,Ni 4 particles run as large as several microns, they provide nucleation sites of the B19'-martensite. Transformation sequence is B2-»B19' as in solution treated specimen.

Abstract:BACKGROUND Neurology is rapidly evolving as a result of continuous diagnostic and therapeutic progress, which influences the daily work of neurologists. Therefore, updating residency training programmes is crucial for the future of neurology. Several countries are currently discussing and/or modifying the structure of their neurology residency training programme. A detailed and up-to-date overview of the available European residency training programmes will aid this process. METHODS A questionnaire addressing numerous aspects of residency training programmes in neurology was distributed among 38 national representatives of the Resident and Research Fellow Section of the European Academy of Neurology. RESULTS We obtained data from 32 European countries, response rate 84%. The median (range) duration of the residency training programmes was 60 (12-72) months. In the majority of countries, rotations to other medical disciplines were mandatory, mostly psychiatry (69%), internal medicine (66%) and neurosurgery (59%). However, the choice of medical fields and the duration of rotations varied substantially between countries. In 50% of countries, there are formal regulations regarding training in evidence-based medicine, teaching skills and/or leadership qualities. In many countries (75%), residents have to take an exam. CONCLUSIONS We found substantial variation among European countries in the duration of residency training programmes, and especially in the choice of obligatory rotations to external medical disciplines. Despite a presumably similar spectrum of patients, neurology residency training programmes across Europe are not harmonised. The structure of the programme should be determined by its relevance for neurologists today and in the future.

Abstract:background This article describes a technique for transplanting hair from the chest to the balding scalp. objective The objective is to describe an alternative route of harvesting donor hair in the situation when donor scalp hair is depleted. method A fusiform donor harvest was removed from the sternal region. The tissue was divided into one‐and two‐haired micrografts and subsequently transplanted. results The technique worked extremely well with good take of the hair grafts. The donor site healed inconspicuously. conclusion The technique of transplanting hair from the chest to the balding scalp can be a viable alternative when confronted with donor scalp hair depletion.

Abstract:Introduction Differences in psychiatry training vary substantially across Europe. Such organisations as the European Federation of Psychiatry Trainees (EFPT), the European College of Neuropsychopharmacology, and the European Psychiatry Association, have committed to offer international experiences based on the premise that it could foster international collaboration, aid early career professionals to progress professionally and spark discussion regarding different practices across Europe. To date, there are no studies that focus exclusively on the exchange experience in mental health professionals Objectives I present the synthesis of the ten years answers from 2012 to 2022 to the post-exchange online evaluation form, which trainees had to fill in in order to receive an attendance certificate. Methods The present study analysed the answers of 202 psychiatry trainees or recent graduates who took part in the EFPT exchange program during 2012 and 2022 and filled in the internet-based evaluation form. The inclusion criteria were currently in training or recently finished training as a psychiatrist in Europe and filling in the questionnaire. The exclusion criterion was participation in the EFPT exchange program for the second or subsequent time. All trainees were systemically asked to complete the online evaluation form after the exchange period. The form includes sociodemographic, training in host country-related, and exchange experience-related questions. Experience measures were evaluated using the 4-point Likert scale. Data was anonymized before the analysis. The study followed the principles of the Declaration of Helsinki. Results The majority of participants were females in the second half of their training. The average age was 29 years. The largest number of applicants were from Turkey, whereas the United Kingdom hosted the most participants. One-third of the participants had previous international exchange experience. Most trainees were exposed to both outpatient and inpatient treatment settings and were involved in educational or research activities. 96.7% of participants indicated that they were satisfied or very satisfied with the experience, 95.6% said that the exchange was useful or very useful, and 98.9% were likely or very likely to recommend exchange to colleagues. Conclusions To my knowledge, this study is the first to assess the experience of psychiatry trainees who went on exchange during their professional training. Vast majority of trainees were satisfied with their exchange, thought it would be useful for their clinical practice and would recommend it to their colleagues. Abstract:Background and objectives: Epilepsy education has been transformed over the past 2 decades, leading to a need for structured formative assessment tools. The American Epilepsy Society developed the Epilepsy Fellowship In-Training Examination (EpiFITE) to provide high-quality formative assessment for fellows, to stimulate program improvement, and to guide future learning and teaching. The aim of this study was to explore validity evidence for the EpiFITE in meeting these goals. Methods: Validity evidence was sought from multiple sources. The content of the examination was linked to the American Board of Psychiatry and Neurology blueprint for initial certification in epilepsy, and items were developed by trained experts. Internal structure was studied using internal consistency and item analysis. Surveys of fellows and fellowship directors focused on the examination experience (response process) and how results influenced fellow assessment, future learning, and program improvement (relationship to other variables and consequences). Results: The EpiFITE was first administered in 2020, with 172 examinees from 67 programs. By 2022 (year 3), the EpiFITE was completed by 195 epilepsy fellows from 77 programs. The overall mean score of the examination was stable from year to year, and the committee predicted the difficulty of individual items with a high degree of accuracy. The examination had high internal consistency (Cronbach α 0.76-0.81). The median item discrimination index ranged from 0.17 in 2020 to 0.21 in 2022. Discrimination indices were lower (mean ≤0.10) for items that were either very easy or very difficult and significantly higher (mean >0.20) for other items. Program directors and epilepsy fellows agreed the examination questions were appropriate and agreed that the EpiFITE helped them identify areas for self-directed learning. Program directors also found the examination helpful in identifying areas of strength and areas for improvement within their programs. Discussion: There are several sources of evidence of the quality and validity of the EpiFITE. By exploring this validity evidence, we have identified several best practices in the development and evaluation of a subspecialty examination, and this experience could be helpful for developers of in-training examinations in other subspecialties.
The development and efficacy of a paediatric cardiology fellowship online preparatory course

Abstract:Background: The transition from residency to paediatric cardiology fellowship is challenging due to the new knowledge and technical skills required. Online learning can be an effective didactic modality that can be widely accessed by trainees. We sought to evaluate the effectiveness of a paediatric cardiology Fellowship Online Preparatory Course prior to the start of fellowship. Methods: The Online Preparatory Course contained 18 online learning modules covering basic concepts in anatomy, auscultation, echocardiography, catheterisation, cardiovascular intensive care, electrophysiology, pulmonary hypertension, heart failure, and cardiac surgery. Each online learning module included an instructional video with pre-and post-video tests. Participants completed pre- and post-Online Preparatory Course knowledge-based exams and surveys. Pre- and post-Online Preparatory Course survey and knowledge-based examination results were compared via Wilcoxon sign and paired t-tests. Results: 151 incoming paediatric cardiology fellows from programmes across the USA participated in the 3 months prior to starting fellowship training between 2017 and 2019. There was significant improvement between pre- and post-video test scores for all 18 online learning modules. There was also significant improvement between pre- and post-Online Preparatory Course exam scores (PRE 43.6 ± 11% versus POST 60.3 ± 10%, p < 0.001). Comparing pre- and post-Online Preparatory Course surveys, there was a statistically significant improvement in the participants' comfort level in 35 of 36 (97%) assessment areas. Nearly all participants (98%) agreed or strongly agreed that the Online Preparatory Course was a valuable learning experience and helped alleviate some anxieties (77% agreed or strongly agreed) related to starting fellowship. Conclusion: An Online Preparatory Course prior to starting fellowship can provide a foundation of knowledge, decrease anxiety, and serve as an effective educational springboard for paediatric cardiology fellows.

Abstract:Introduction Every resident or fellow has unique factors that are most crucial in guiding their career decisions. When investigating specialty selection on a system level, many fields have data suggesting core factors that motivate trainees toward career decisions. Given the annual decline of fellows pursuing Advanced Heart Failure and Transplant Cardiology (AHFTC), factors impacting this trend must be better understood to support the needs of the growing field. Methods An extensive literature review was conducted to investigate factors that residents and fellows use to decide upon a specific specialty. A total of 1469 studies were screened via PubMed. All studies designs were included, as the goal of the review was to elicit qualitative outcomes. Results A total of 103 studies from 33 specialties were assessed. The literature spanned 88 unique journals, ranging in impact factors from 0.538 to 44.54. Most publications employed a survey as their study design (81.6%, n=84), surveying over 62,530 individuals predominately in the United States (70.5%). Four predominant factors emerged and were investigated more thoroughly: early exposure to the field (52 of 103 studies, 50.4%), clinical stimulation by the field (62 of 103 studies, 59.2%), inclusion and advocacy within the field (69 of 103 studies, 66.9%), and team-based mentorship (69 of 103 studies, 66.9%). Conclusions A systematic review of 103 manuscripts was conducted investigating factors that motivate trainees to pursue certain careers. There was consistency of motivating factors across all specialties, regardless of a medical or surgical focus. Within the AHFTC community, the next step is assessing what interventions can have the most significant impact on the future of trainees pursuing the field. Based on the themes that were seen, the mnemonic “ExCITe” (Figure 1) was developed to group interventions based upon the four key categories. Using this framework, leaders in the AHFTC community can identify ways to motivate and support trainees interested in heart failure and, more broadly, cardiology. By using these interventions, the field may more successfully “catch” residents at more appropriate times in their career and retain fellows most interested in AHFTC.

The Impact of Congenital Cardiac Surgery Fellowship on Training and Practice

Abstract:BACKGROUND In 2007, congenital cardiac surgery became a recognized fellowship by the American Council of Graduate Medical Education (ACGME). In 2022, the fellowship is transitioning from a one-year to a two-year program. Our objective is to provide current benchmarks by surveying current training programs and assessing characteristics contributing to career success. METHODS This was a survey-based study in which tailored questionnaires were distributed to program directors (PD) and graduates of the ACGME accredited training programs. Data collection included responses to multiple-choice and open-ended questions relevant to didactics, operative training, training center characteristics, mentorship, and employment characteristics. Results were analyzed using summary statistics, subgroup, and multivariable analyses. RESULTS The survey yielded responses from 13 of 15 (86%) PDs and 41 of 101 (41%) graduates from ACGME accredited programs. Perceptions among PDs and graduates were somewhat discordant, with PDs more optimistic than graduates. Of PDs, 77% (N=10) believed current training adequately prepares fellows and is successful in securing employment for graduates. The responses from graduates demonstrated 30% (N=12) were dissatisfied with operative experience and 24% (N=10) with overall training. Being supported during the first 5 years of practice was significantly associated with retention in congenital cardiac surgery and greater practicing case volumes. CONCLUSIONS Dichotomous views exist between graduates and program directors regarding success in training. Mentorship during early career was associated with increased case volumes, career satisfaction, and retention in the congenital cardiac surgery field. Educational bodies should incorporate these elements during training and post-graduation.

European fellowship training opportunities for hepato-pancreato-biliary surgery: an international survey of fellows and program directors

Abstract:BACKGROUND: There are no established training pathways for hepato-pancreato-biliary (HPB) surgery in Europe. This study aims to overview the current status of fellowship training from both fellows' and institutions' perspectives.METHODS: A web-based snapshot survey was distributed to all members of the European-African Hepato-Pancreato-Biliary Association (E-AHPBA) to reach for former fellows and program directors of European HPB surgery fellowships held between 2013 and 2023.RESULTS: A total of 37 fellows and 13 program directors replied describing 32 different programs in 13 European countries. The median (range) age at fellowship start was 34 (30-45 years). Fellowship duration was most commonly one (36 %) or two (40 %) years. Fellowships were funded in 70 % and fellows were required to learn a new language in 27 %. Most fellows performed between none and 10 pancreatic (68 %), major (67 %) and minor (60 %) liver resections as 1st surgeon, while the number of operations performed as 1st assistant were more heterogeneous. Program directors estimated a higher number of operations performed by fellows as first surgeons. The percentage of procedures performed minimally invasively did not exceed 10 %.CONCLUSION: There is substantial heterogeneity between HPB fellowship programs in Europe. A wider standardization of clinical curriculum, including minimally invasive surgery, is desirable.

European otorhinolaryngology training programs: results of a European survey about training satisfaction, work environment and conditions in six countries

Abstract:ORL-students and residents have an ongoing debate about the "best" programme in Europe. Aim of this study was to comparatively assess differences among programmes in training, satisfaction, quality of life (QoL) of residents and recent otorhinolaryngologist (ORL) specialists in France, Germany, Spain, Italy, Austria, and Belgium. A self-administered anonymous questionnaire, structured in ten sections including general information, provided guidance, working environment, training structure, teaching of medical students, publication work, QoL, and satisfaction with training, were emailed to residents and recent ORL specialists. 476 returned questionnaires from 6 countries revealed that daily work hours were the highest in France and Belgium with 11 and 10.4 h on average, respectively. QoL, work conditions, and salary were best in Germany followed by Austria in terms of possibility of part-time contracts, better respect for post-duty day off, and compensation for overtime. Satisfaction with training including support and guidance of seniors was lowest in Italy, but, on the other hand, the publication work and support had a more important place than in other countries. In Belgium, there was some gap between the quality of teaching and feedback from seniors as well as apprenticeship. The highest satisfaction with training was in France and Spain followed by Austria. The study results provide guidance before choosing an ORL training programme in Europe. Country-specific strengths could be included into future harmonization efforts to improve all programmes, facilitate professional exchange and, finally, establish standards-of-care carried out by well-trained doctors also looking after a satisfying work-life balance.
Trainee Insight into Pain Fellowship Programs: A Critical Evaluation of the Current Educational System by the APPD

Abstract:Background: Since 1992, when the Accreditation Council of Graduate Medical Education (ACGME) acknowledged pain medicine as a subspecialty, the field has experienced significant growth in its number of programs, diversity of sponsoring specialties, treatment algorithms, and popularity among applicants. These shifts prompted changes to the educational model, overseen by program directors (PDs) and the ACGME. The pool of pain fellowship applicants also changed during that period. Objectives: This study aims to investigate trainees' reasons for applying to pain medicine fellowship programs as well as the applicants' specific expectations, interests, and motivations, thereby contributing to the remodeling and universal improvement of programs across the country. Study design: Online survey via SurveyMonkey. The online questionnaire targeted pain fellowship applicants in 2023 and current fellows in the US. Methods: Our study was designed by board members of the Association of Pain Program Directors (APPD). The board disseminated a survey to those who applied to ACGME Pain Medicine fellowships in 2023 as well as to existing fellows. The survey was emailed to residency and fellowship PDs for dissemination to their trainees. The participants answered a 12-question survey on their reasons for pursuing pain medicine fellowships, expectations of and beyond those fellowships, and educational adjustments. Results: There were 283 survey participants (80% applicants in residency training and 20% fellows). Participants ranked basic interventional procedures and a strong desire to learn advanced procedures as the most significant factors in pursuing a pain fellowship. Most trainees (70%) did not wish to pursue a 2-year fellowship, and 50% desired to go into private practice. Limitations: The relatively small number of respondents is a limitation that could introduce sampling error. Since most of the respondents were from the fields of physical medicine and rehabilitation (PM&R) and anesthesia, the use of convenience sampling reduced our ability to generalize the results to the wider community. Furthermore, approximately 80% of the trainees were residents, who might have had less experience in or knowledge of the survey's particulars than did the fellows. Conclusion: This survey demonstrated that procedural volume and diversity were important factors in trainees' decisions to apply to the field of pain medicine; however, extending the duration of a pain fellowship was not an option survey participants favored. Therefore, PDs and educational stakeholders in pain fellowship training need to develop creative strategies to maintain competitive applicants' interest while they adapt to our evolving field.

Fellows' perspective of HPB training programs in North America: results of a survey

Abstract:Background: Sixteen hepatopancreatobiliary fellowship programs in North America are accredited by the Fellowship Council. This study aims to assess fellows' perceptions of their training program. Methods: A multiple-choice questionnaire was sent to 35 fellows to assess how they perceived their training: academics, research, operative experiences, autonomy, mentorship, program quality and weaknesses. The survey was developed using the SurveyMonkey® tool. Results: Twenty-four of 35 fellows completed the survey. Sixteen fellows reported structured didactics; 10 reported mandatory research. As to operative experiences; 9 fellows reported exposure to minimally-invasive liver surgery; 5 reported exposure to robotics. Fourteen fellows reported using ablation; 5 reported using ablation laparoscopically; 8 reported using mostly radiofrequency ablation; 1 reported using irreversible electroporation. Eighteen fellows reported excellent training; 20 reported mentorship; 19 reported operative autonomy. Limited exposure to medical oncology/multidisciplinary care, portal hypertension surgery, and robotics surgery were perceived as program weaknesses by 7, 9, and 7 fellows, respectively. Conclusion: Most fellows ranked their program quality and academic content as excellent, but they perceived a need for more exposure to medical oncology, portal hypertension surgery, and minimally-invasive surgery, with an emphasis on robotics. Fellowship training may need to integrate fellows' desires for enhanced proficiency in these clinical areas.

Abstract:Background: There is limited data on the organisation of paediatric echocardiography laboratories in Europe. Methods: A structured and approved questionnaire was circulated across all 95 Association for European Paediatric and Congenital Cardiology affiliated centres. The aims were to evaluate: (1) facilities in paediatric echocardiography laboratories across Europe, (2) accredited laboratories, (3) medical/paramedical staff employed, (4) time for echocardiographic studies and reporting, and (5) training, teaching, quality improvement, and research programs. Results: Respondents from forty-three centres (45%) in 22 countries completed the survey. Thirty-six centres (84%) have a dedicated paediatric echocardiography laboratory, only five (12%) of which reported they were European Association of Cardiovascular Imaging accredited. The median number of echocardiography rooms was three (range 1–12), and echocardiography machines was four (range 1–12). Only half of all the centres have dedicated imaging physiologists and/or nursing staff, while the majority (79%) have specialist imaging cardiologist(s). The median (range) duration of time for a new examination was 45 (20–60) minutes, and for repeat examination was 20 (5–30) minutes. More than half of respondents (58%) have dedicated time for reporting. An organised training program was present in most centres (78%), 44% undertake quality assurance, and 79% perform research. Guidelines for performing echocardiography were available in 32 centres (74%). Conclusion: Facilities, staffing levels, study times, standards in teaching/training, and quality assurance vary widely across paediatric echocardiography laboratories in Europe. Greater support and investment to facilitate improvements in staffing levels, equipment, and governance would potentially improve European paediatric echocardiography laboratories.

Abstract:The relations for position, spot size, and inclination of the major axis of an elliptical Gaussian beam to knife-edge scanning data are derived. A knife-edge whose scanning direction is adjustable to any angle has been employed to scan across a beam in at least three directions. Nonlinear least-squares fit programs have been developed to check whether a beam is Gaussian, and to evaluate the parameters, with errors, of such an elliptic spot. The evolution of an astigmatic beam in the tangential and sagittal plane is measured.
