Abstract:Abstract Aims Catheter ablations of complex cardiac arrhythmias are currently guided by electro-anatomic mapping (EAM) systems. The aim of this study was to compare two different approaches: remotely supported nurse-led EAM with standard onsite engineer support. Methods and results In this retrospective observational study, 166 patients with complex and non-complex procedures were included. A total of 82 patients benefited from EAM with remotely supported nurse-led mapping (mean age: 62 ± 16 years), while the approach for 84 patients was with standard onsite engineer support (mean age: 56 ± 19 years). Procedural characteristics, acute results, and complication rates were compared between both groups and showed similar results. Complex and non-complex procedures were conducted in both groups, including left atrial and ventricular procedures. As ventricular tachycardia and accessory pathway ablations were more frequently conducted with standard onsite engineer support, we separately analysed the largest subgroup, 105 patients with atrial fibrillation, left atrial flutter, and left atrial tachycardia. Patients in this subgroup had comparable baseline characteristics, procedure times, and procedural success. Nevertheless, there were longer ablation times and more utilization of fluoroscopy in the onsite group, most likely due to more complex procedures. Conclusion Our results underline the practicality of remotely supported nurse-led EAM. The latter approach proved to be a safe alternative to onsite engineer support. Due to its advantages, particularly for insular settings, it will likely play a greater role in the future.

Abstract:Background Rapid technologic development and expansion of procedural expertise have led to widespread proliferation of catheter-based electrophysiology procedures. It is unclear whether these advances come at cost to patient safety. Objective This meta-analysis aimed to assess complication rates after modern electrophysiology procedures during the lifetime of the procedures. Methods A comprehensive search was performed to identify relevant data published before May 30, 2023. Studies were included if they met the following inclusion criteria: prospective trials or registries, including comprehensive complications data; and patients undergoing atrial fibrillation ablation, ventricular tachyarrhythmia ablation, leadless cardiac pacemaker implantation, and percutaneous left atrial appendage occlusion. Pooled incidences of procedure-related complications were individually assessed by random effects models to account for heterogeneity. Temporal trends in complications were investigated by clustering trials by publication year (2000–2018 vs 2019–2023). Results A total of 174 studies (43,914 patients) met criteria for analysis: 126 studies of atrial fibrillation ablation (n = 24,057), 25 studies of ventricular tachyarrhythmia ablation (n = 1781), 21 studies of leadless cardiac pacemaker (n = 8896), and 18 studies of left atrial appendage occlusion (n = 9180). The pooled incidences of serious procedure-related complications (3.49% [2000–2018] vs 3.05% [2019–2023]; P < .001), procedure-related stroke (0.46% vs 0.28%; P = .002), pericardial effusion requiring intervention (1.02% vs 0.83%; P = .037), and procedure-related death (0.15% vs 0.06%; P = .003) significantly decreased over time. However, there was no significant difference in the incidence of vascular complications over time (1.86% vs 1.88%; P = .888). Conclusion Despite an increase in cardiac electrophysiology procedures, procedural safety has improved over time.

Abstract:Background Complex electrophysiological procedures are performed using three-dimensional (3D) electroanatomical mapping (EAM) systems, which are usually operated on site, by field technical engineers (FTE). Due to a lack of skilled personnel, the engineers usually must travel between different hospitals and the procedures have to be planed in advance. Remote access (RA) technologies were recently implemented in clinical practice, aiming to increase the flexibility in the EP lab and maximise healthcare resources use. Methods Electroanatomical mapping was performed remotely by means of an integrated audio-visual solution for RA. The operator and the FTE communicated exclusively using the RA system, while the latter worked only in home office. Consecutive procedures were prospectively enrolled and analysed in terms of indication, procedural characteristics, feasibility and saved travelled time by the FTA. Results Between September 2022 and July 2023, a total of 133 EP procedures were performed in a high volume, tertiary electrophysiological centre in Germany. The mean age was 67 (57.5, 75.5) years. Women represented 39.8% of the population. The median BMI was 26.2 (23.1, 30.3) Kg/m2. A total of 56 (42.1%) patients underwent an atrial fibrillation ablation procedure (37.5% paroxysmal), 22 (16.5%) an atrial tachycardia ablation, 13 (9.8%) cavotricuspid isthmus ablation, 15 (11.3%) supraventricular tachycardia ablation, 14 (10.5%) a premature ventricular contraction ablation, 12 (9%) ventricular tachycardia ablations and 1 patient an electrophysiological diagnostic study. The median time length of procedure was 130 (96.5, 168) minutes. The median number of radiofrequency applications was 63.5 (22.3, 109.5), while the median RF time was 14.2 (6.9, 25.1) minutes. The fluoroscopy time was 9.2 (6.5, 12.6) minutes, with a median radiation dose of 408.5 (233.0, 631.8) cGy x cm2. The median amount of contrast media was 40 (8.75, 50.0) ml and the median amount of heparin administered was 12000 (8000, 16000) UI. Protamine was used in 9 cases, with a median of 7000 (5000, 7000) UI. Except for one supraventricular procedure, where no arrhythmia induction was possible, all procedures were successful. One major complication was observed. In one case, the internet-based headphone connection was not stable. Therefore, the communication was established via a conventional smartphone. No change to on-site support was needed. The travel time reduction was available in 130 cases. In total, 290 travel hours could be saved. Conclusion The routine implementation of RA for complex EP procedures proved to be safe and efficient. This approach has the potential to increase the feasibility in case of emergency procedures, reduce travel times for FTE and streamline healthcare resources allocation.

Procedural factors associated with early postoperative arrhythmias after repair of congenital heart defects

Abstract:Objectives: Early postoperative arrhythmias frequently are a relevant problem in the early postoperative management after surgical intervention for congenital heart disease. Few data are available indicating risk factors for their occurrence. The hypothesis was tested that factors closely related to the surgical procedure itself were associated with a higher incidence of arrhythmias early in the postoperative course after repair of congenital heart disease. Methods: All consecutive patients undergoing 1 of 3 well-defined surgical procedures were prospectively evaluated for the occurrence of arrhythmias during the entire postoperative hospital stay by means of continuous electrocardiographic monitoring in the intensive care unit and use of 24-hour Holter monitors. Patients examined were those undergoing transatrial closure of a ventricular septal defect, repair of complete atrioventricular canal, and tetralogy of Fallot. The relation between procedural variables and the occurrence of arrhythmias was independently evaluated for each of these 3 heart defects. Results: Early postoperative arrhythmias occurred in 30% of patients with ventricular septal defect (n = 75), 35% of patients with tetralogy of Fallot (n = 52), and 47% of patients with atrioventricular canal (n = 45). Patients with arrhythmias tended to be younger (significant only in the ventricular septal defect group). In all 3 patient groups, there was a significant correlation between incidence of arrhythmias and longer extracorporeal bypass time (P <.05) and longer aortic crossclamp time (P <.01), as well as with higher maximum postoperative troponin serum levels (P <.01). In patients with atrioventricular canal, there was a significant relation between hemodynamically incomplete surgical results and the occurrence of arrhythmias (P <.01). Conclusions: The occurrence of early postoperative arrhythmias after repair of congenital heart disease was significantly associated with procedure-related risk factors in each of 3 independent patient groups undergoing well-defined surgical procedures.

Abstract:Objectives The incidence of VTE in patients undergoing EPS and RFCA is considerable high. Procedure duration is potential to be a risk factor of TE. Better pharmacological thromboprophylaxis is considered to be essential, particularly in those patients with potential risk factors. Methods A prospective study was conducted from February 2003 to August 2006 in our clinic. A total of 348 patients with EPS (n=231) and RFCA (n=117) were analysed. For the diagnosis of DVT, Duplex ultrasonography evaluation of the bilateral femoral veins was performed after the procedure. Lung ventilation/perfusion scintigraphy was carried out in patients suspected to have PTE. An initial bolus of 3000 IU heparin was injected to all patients, and additional heparin was supplied when the activated clotting time (ACT) was less than 300 s during the procedure. Results There were 38 patients with DVT (10.9%) and one patient of them was diagnosed as PTE (0.3%). With univariate analysis, the procedure duration and the number of the introducer sheaths were identified as a significant risk factor in the whole study population. When analysed by the stepwise multiple logistic regression model, only the procedure duration risk factor was found to be significant (OR=1.008, 95% CI 1.001 to 1.015). While, both ablation (OR=1.330, 95% CI 0.666 to 2.658) and adipositas (OR=0.808, 95% CI 0.402 to 1.621) did not appear to be related to TE. Smoking habit influenced negatively the incidence of VTE (OR=0.311, 95% CI 0.107 to 0.904). Pretreatment of aspirin had no relation to the occurrence of VTE (OR=1.055, 95% CI 0.5372 to 2.072). Additional heparin was required most in the patients with RF ablation, more introducer sheaths and longer length of duration to maintain the ACT more than 300 s during the procedure. The incidence of VTE in patients with additional heparin was significant high than the patients without additional heparin. Certain attention should be drawn on patients with potential risk factors, and the ACT measurement should be repeated to maintenance at an adequate level. Conclusions The incidence of VTE in patients undergoing EPS and RFCA is considerable high. Procedure duration is potential to be a risk factor of TE. Better pharmacological thromboprophylaxis is considered to be essential, particularly in those patients with potential risk factors.

Abstract:Background: Paediatric cardiac electrophysiologists are essential in CHD inpatient care, but their involvement is typically limited to consultation with individual patients. In our integrated heart centre, an electrophysiologist reviews all cardiac inpatient telemetry over the preceding 24 hours and participates in daily multidisciplinary morning report. This study investigates the impact of the strategy of consistent, formalised electrophysiologist presence at multidisciplinary morning report. Methods: This is a single-centre, prospective, observational study of electrophysiologist participation in patient encounters during heart centre multidisciplinary morning report from 10/20/2021 to 10/31/2022. Multidisciplinary morning report includes discussion of all intensive care and non-intensive care cardiac patients. An encounter was defined as reporting on one patient for one day. Electrophysiologists were initially blinded to observations. Results: Two electrophysiologists were observed over 215 days encompassing 6413 patient encounters. Electrophysiologists made comments on 581(9.1%) encounters in 234 unique patients with diverse diagnoses, equating to a median of 3[interquartile range:1–4] encounters per day. These included identifications of arrhythmias and describing electrocardiographic findings. Recommendation to change management occurred in 282(48.5%) encounters, most commonly regarding medications (n = 142, 24.4%) or pacemaker management (n = 48, 8.3%). Of the 581 encounters, there were 61(10.5%) in which they corrected another physician's interpretation of rhythm or electrocardiogram. Conclusion: Routine electrophysiologist involvement in multidisciplinary morning report provides significant, frequent, and timely input in patient management by identifying precise rhythm-related diagnoses and allowing nuanced, patient-specific medication and pacemaker management of all cardiac patients, not just those consulted. Electrophysiologist presence at multidisciplinary morning report is a vital resource and this practice should be considered at integrated paediatric cardiac centres.

Effect of Resident Involvement on Morbidity and Mortality Following Thoracic Endovascular Aortic Repair

Abstract:Objective: To evaluate the effect of resident involvement in thoracic endovascular aortic repair (TEVAR). Summary of background data: Although the influence of resident intraoperative involvement in several types of surgical procedures has been reported, the effect of resident participation in TEVAR is unknown. We evaluated patient outcomes in resident-involved TEVAR procedures. Methods: The American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database was analyzed for TEVAR performed from 2010 to 2012. Current procedural terminology codes were used to identify adult patients (≥18 y) who underwent TEVAR. Patients were grouped into those with and without resident involvement. Descriptive and binomial logistic statistics were used to determine the effect of resident involvement on post-TEVAR outcomes. p values < 0.05 were considered statistically significant. Results: A total of 676 patients met inclusion criteria for this study. Of these, 517 (76.5%) had residents involved. Overall mortality was 9.8%, with no significant difference between the 2 groups (p = 0.88). Resident involvement was not a significant predictor of any post-TEVAR complication. Postoperative pneumonia (3.5% vs 6.9%, p = 0.06), prolonged mechanical ventilation (11.8% vs 11.9%, p = 0.96), stroke (2.7% vs 5.7%, p = 0.07), urinary tract infection (3.3% vs 4.4%, p = 0.50), progressive renal insufficiency (1.2% vs 2.5%, p = 0.22), acute renal failure (4.1% vs 5.0%, p = 0.60), cardiac arrest (2.9% vs 5.0%, p = 0.20), myocardial infarction (1.7% vs 1.9%, p = 0.91), deep venous thrombosis (1.7% vs 1.3%, p = 0.67), red blood cells transfusions (29.2% vs 36.5%, p = 0.08), sepsis (2.9% vs 4.4%, p = 0.35), septic shock (1.9% vs 3.8%, p = 0.18), and unplanned reintubation (8.7% vs 9.4%, p = 0.78) were not significantly affected. Additionally, resident involvement did not significantly affect operative time (176.1 ± 122.8 min vs 180.3 ± 119.1 min, p = 0.71) and anesthesia time (282.1 ± 146.6 min vs 278.3 ± 140.5 min, p = 0.78). Conclusions: The participation of residents in TEVAR did not significantly affect all 30-day patient outcomes. Resident involvement in TEVAR is safe and should be encouraged. Mini abstract: This study evaluated the effect of resident participation on postoperative outcomes of thoracic endovascular aortic repair (TEVAR) using the American College of Surgeons National Surgical Quality Improvement (ACS-NSQIP) database. Abstract:Abstract Funding Acknowledgements Type of funding sources: None. Introduction Follow-up in patients with cardiovascular implantable electronic devices (CIED) is essential to avoid adverse events and complications, such as arrhythmias, device dysfunction, and infections, among other events. Prevention plays a fundamental role, especially when these patients undergo non-cardiological invasive tests and/or procedures (e.g., surgeries) or magnetic resonance imaging (MRI). These situations sometimes require the reprogramming of the device. In our Arrhythmia's Unit, after developing a healthcare management model based on the Lean methodology, 2 processes were identified that implied a reduction in activity, a threat to efficiency and a risk to patients who would undergo invasive non-cardiological procedures or MRI. One of them was a significant number of "urgent" consultations and calls to evaluate and/or reprogram CIED in patients that underwent invasive non-cardiological procedures the same day, many of them without indication of reprogramming according to the type of procedure. Another one was the increase in consultations of people with CIED who require MRI, was often contraindicated because it produces adverse effects on the patient and/or device. Objectives: To implement action protocols, depending on the type of device and dependant on stimulation, for patients with CIED who undergo surgery and invasive non-cardiac procedures and MRI. To evaluate the number of urgent calls and adverse events before and after implementation. Methods: A single-centre, not randomized study has been designed. A year before and after the implementation of the protocol the following variables were quantified a) number of urgent calls b) number of adverse events (persistence of the special programming of the device more than 24 h after the end of the procedure: asynchronous stimulation, ICD therapies off, CIED dysfunction or unnecessary programming). Two protocols were developed: a) management of CIED carriers undergoing invasive non-cardiological procedures according to recent clinical practice guidelines. b) risk of dysfunction after MRI based on type/compatibility with MRI of the device, implanted leads, and stimulation dependence, using a colour scale, from safe (in green) to high risk of dysfunction (in black) Pictures 1,2. The protocols for action in the anaesthesia and radiology services were disseminated through sessions given by the arrhythmia unit staff. Results: In the year prior to implementing the protocol, 185 urgent calls/year were quantified, (15.1 calls/month) and the number of adverse events was 4 (0.2 events/month). One year after the implementation of the protocol, urgent calls were reduced to 4calls/year, representing 0.33 calls/month (p <0.001) and there was an absence of adverse events (zero events/month). Conclusions. The implementation of protocols based on recent clinical practice guidelines can reduce urgent calls and adverse events related to reprogramming in our centre, thus increasing patient safety.

Effect of Metrics-Based Simulation Training to Proficiency on Procedure Quality and Errors Among Novice Cardiac Device Implanters: The IMPROF Randomized Trial

Abstract:Importance: In cardiac device implant training, there is no common system to objectively assess trainees' ability to perform tasks at predetermined performance levels before in vivo practice; therefore, patients are potentially exposed to risks related to operators' early learning curve. Objective: To assess the effect on implant performance quality of novel metrics-based simulation training to proficiency (proficiency-based progression [PBP]) vs traditional simulation-based training (SBT). Design, setting, and participants: In this prospective randomized trial, conducted between March 8, 2022 and November 24, 2022, novice implanters were randomized (blinded) 1:1 to participate in an SBT curriculum (procedural knowledge e-learning and in-person simulation training) at an international skills training center, with proficiency demonstration requirements at each training stage for advancing (PBP approach) or without the requirements. Ultimately, trainees performed a cardiac resynchronization therapy (CRT) implant using virtual reality simulation. The procedure was video-recorded and subsequently scored using previously validated metrics by 2 independent assessors blinded to group. Physicians who had already implanted more than 20 pacemakers or defibrillators and fewer than 200 CRT systems as the first operator were eligible. Thirty-two implanters from 10 countries voluntarily enrolled in the training program and were randomized; 30 (15 per group) started and completed training. Data analysis was performed from November 27 to December 22, 2022. Intervention: Training with PBP vs SBT. Main outcome and measures: The primary outcome comprised 4 objectively assessed performance metrics derived from the video-recordings: number of procedural steps completed, errors, critical errors, and all errors combined. Results: Baseline experience of the 30 participants (19 [63%] male; mean [SD] number of years in implant practice, 2.0 [1.8]; median [IQR] number of implanted pacemakers or defibrillators, 47.5 [30.0-115.0]; median [IQR] number of implanted CRT systems, 3.0 [1.25-10.0]) was similar between study groups. Compared with the SBT group, the PBP group completed 27% more procedural steps (median [IQR], 31 [30-32] vs 24 [22-27]; P < .001) and made 73% fewer errors (median [IQR], 2 [1-3] vs 7 [5-8]; P < .001), 84% fewer critical errors (median [IQR], 1 [0-1] vs 3 [3-5]; P < .001), and 77% fewer all errors combined (errors plus critical errors) (median [IQR], 3 [1-3] vs 11 [8-12]; P < .001); 14 of the 15 PBP trainees (93%) demonstrated the predefined target performance level vs 0 of the 15 SBT trainees. Conclusions and relevance: In this randomized trial, the PBP approach to novice implanter training generated superior objectively assessed performance vs SBT. If implemented broadly and systematically, PBP training may ensure safe and effective performance standards before trainees proceed to (supervised) in vivo practice. Abstract:Introduction Training in clinical cardiac electrophysiology (CCEP) involves the development of catheter handling skills to safely deliver effective treatment. Objective data from analysis of ablation data for evaluating trainee of CCEP procedures has not previously been possible. Using the artificial intelligence cloud‐based system (CARTONET), we assessed the impact of trainee progress through ablation procedural quality. Methods Lesion‐ and procedure‐level data from all de novo atrial fibrillation (AF) and cavotricuspid isthmus (CTI) ablations involving first‐year (Y1) or second‐year (Y2) fellows across a full year of fellowship was curated within Cartonet. Lesions were automatically assigned to anatomic locations. Results Lesion characteristics, including contact force, catheter stability, impedance drop, ablation index value, and interlesion time/distance were similar over each training year. Anatomic location and supervising operator significantly affected catheter stability. The proportion of lesion sets delivered independently and of lesions delivered by the trainee increased steadily from the first quartile of Y1 to the last quartile of Y2. Trainee perception of difficult regions did not correspond to objective measures. Conclusion Objective ablation data from Cartonet showed that the progression of trainees through CCEP training does not impact lesion‐level measures of treatment efficacy (i.e., catheter stability, impedance drop). Data demonstrates increasing independence over a training fellowship. Analyses like these could be useful to inform individualized training programs and to track trainee's progress. It may also be a useful quality assurance tool for ensuring ongoing consistency of treatment delivered within training institutions.

Although typically they are based on ecological data (e.g., species, habitats, ecological processes), increasingly they also include nonecological data, mostly on the vulnerability of valued features and economic costs of implementation. Nevertheless, the effectiveness of conservation actions implemented through conservation‐planning initiatives is a function of the human and social dimensions of social‐ecological systems, such as stakeholders’ willingness and capacity to participate. We assessed human and social factors hypothesized to define opportunities for implementing effective conservation action by individual land managers (those responsible for making day‐to‐day decisions on land use) and mapped these to schedule implementation of a private land conservation program. We surveyed 48 land managers who owned 301 land parcels in the Makana Municipality of the Eastern Cape province in South Africa. Psychometric statistical and cluster analyses were applied to the interview data so as to map human and social factors of conservation opportunity across a landscape of regional conservation importance. Four groups of landowners were identified, in rank order, for a phased implementation process. Furthermore, using psychometric statistical techniques, we reduced the number of interview questions from 165 to 45, which is a preliminary step toward developing surrogates for human and social factors that can be developed rapidly and complemented with measures of conservation value, vulnerability, and economic cost to more‐effectively schedule conservation actions. This work provides conservation and land management professionals direction on where and how implementation of local‐scale conservation should be undertaken to ensure it is feasible.
Impact of clinical variables on outcomes in refractory cardiac arrest patients undergoing extracorporeal cardiopulmonary resuscitation

Abstract:Background: In the past two decades, extracorporeal resuscitation (ECPR) has been increasingly used in the management of refractory cardiac arrest (CA) patients. Decision algorithms have been used to guide the care such patients, but the effectiveness of such decision-making tools is not well described. The aim of this study was to compare the rate of survival with a good neurologic outcome of patients treated with ECPR meeting all criteria of a clinical decision-making tool for the initiation of ECPR to those for whom ECPR was implemented outside of the algorithm. Methods: All patients who underwent E-CPR between January 2014 and December 2021 at the Montreal Heart Institute were included in this retrospective analysis. We dichotomized the cohort according to adherence or non-adherence with the ECPR decision-making tool, which included the following criteria: age ≤65 years, initial shockable rhythm, no-flow time <5 min, serum lactate <13 mmol/L. Patients were included in the "IN" group when they met all criteria of the decision-making tool and in the "OUT" group when at least one criterion was not met. Main outcomes and measures: The primary outcome was survival with intact neurological status at 30 days, defined by a Cerebral Performance Category (CPC) Scale 1 and 2. Results: A total of 41 patients (IN group, n = 11; OUT group, n = 30) were included. A total of 4 (36%) patients met the primary outcome in the IN group and 7 (23%) in the OUT group [odds ratio (OR): 1.88 (95% CI, 0.42–8.34); P = 0.45]. However, survival with a favorable outcome decreased steadily with 2 or more deviations from the decision-making tool [2 deviations: 1 (11%); 3 deviations: 0 (0%)]. Conclusion and relevance: Most patients supported with ECPR fell outside of the criteria encompassed in a clinical decision-making tool, which highlights the challenge of optimal selection of ECPR candidates. Survival rate with a good neurologic outcome did not differ between the IN and OUT groups. However, survival with favorable outcome decreased steadily after one deviation from the decision-making tool. More studies are needed to help select proper candidates with refractory CA patients for ECPR.

Abstract:Background Catheter ablation of cardiac arrhythmias is an established treatment strategy. However, the optimal femoral vascular access strategy and post-interventional management in the electrophysiology (EP) setting is still under debate. Purpose To assess the impact of ultrasound-guided versus conventional femoral vascular puncture and of a modified post-interventional groin-site management strategy on complication rates. Method Patients undergoing EP procedures at the our university heart and vascular center are included in an institutional prospective registry. Conventional vascular access as well as post-interventional groin site pressure-bandage and bed rest of 6 hours (group I) was compared to a new strategy including ultrasound-guided vascular access, venous closure by a Z-suture and groin site pressure-bandage for only 2 hours plus additional 4 hours bed rest (group II). Incidences of minor and major groin site complications for both groups were compared. Hematoma, bleeding, arteriovenous fistula or pseudoaneurysm of the femoral artery without need for intervention (e.g. transfusion, thrombin injection, surgery) were classified as minor complications. Complications requiring intervention or retroperitoneal hematoma were defined as major complications. Results A total of 1535 procedures in 1381 patients (779 (51 %) procedures in 705 (51%) patients in group I and 756 (49%) procedures in 676 (49%) patients in group II) were analyzed. In group I and group II, respectively, 39%/44% procedures were index pulmonary vein isolations, 30%/29% repeat atrial fibrillation, 16 %/15% supraventricular tachycardia, 12%/11% PVC or VT ablations, and 4%/2% left atrial appendage occlusions. The total groin site complication rate was 9% (70/779 procedures) for group I, and 5% (38/756 procedures) for group II (OR 0.54 [95%-CI 0.35-0.82], p=0.003). Major groin site complications occurred in 2% (19/779 procedures) in group I and were reduced to 1% (10/756 procedures) in group II (OR 0.53 [95%-CI 0.22-1.22], p=0.156). Conclusion The overall groin site complication rate in EP-procedures was significantly reduced by implementing a novel institutional standard for vascular access management including ultrasound-guided puncture and a modified post-interventional groin site management.

Abstract:background This article describes a technique for transplanting hair from the chest to the balding scalp. objective The objective is to describe an alternative route of harvesting donor hair in the situation when donor scalp hair is depleted. method A fusiform donor harvest was removed from the sternal region. The tissue was divided into one‐and two‐haired micrografts and subsequently transplanted. results The technique worked extremely well with good take of the hair grafts. The donor site healed inconspicuously. conclusion The technique of transplanting hair from the chest to the balding scalp can be a viable alternative when confronted with donor scalp hair depletion.

Abstract:Objective: The safety of early post-operative cardiac catheterisation has been described following congenital heart surgery. Optimal timing of early post-operative cardiac catheterisation remains uncertain. The aim of this study was to describe the safety of early post-operative cardiac catheterisation and its impact on cardiac ICU and hospital length of stay, duration of mechanical ventilation, and extracorporeal support. Methods: This single-centre retrospective cohort study compared clinical and outcome variables between "early" early post-operative cardiac catheterisation (less than 72 hours after surgery) and "late" early post-operative cardiac catheterisation (greater than 72 hours after surgery) groups using Chi-squared, Student's t, and log-rank test (or appropriate nonparametric test). Results: In total, 132 patients were included, 22 (16.7%) "early" early post-operative cardiac catheterisation, and 110 (83.3%) "late" early post-operative cardiac catheterisation. Interventions were performed in 63 patients (51.5%), 7 (11.1%) early and 56 (88.9%) late. Complications of catheterisation occurred in seven (5.3%) patients, two early and five late. There were no major complications. Patients in the late group trended towards a longer stay in the cardiac ICU (19 days [7, 62] versus 11.5 days [7.2, 31.5], p = 0.6) and in the hospital (26 days [9.2, 68] versus 19 days [13.2, 41.8], p = 0.8) compared to the earlier group. Conclusion: "Early" early post-operative cardiac catheterisation was associated with an overall low rate of complications. Earlier catheterisations trended towards shorter cardiac ICU and hospital length of stays. Earlier catheterisations may lead to earlier recovery for patients not following an expected post-operative course.

Abstract:Background: Remote magnetic navigation (RMN)-guided ablation has become an inspiring method of catheter ablation for tachyarrhythmias. Hypothesis: Data from a large-scale single center may provide further insight into the safety of and the learning curve for RMN-guided ablation. Methods: A total of 1003 catheter ablation procedures using RMN for conditions including supraventricular ventricular tachycardia, atrial tachyarrhythmias, and premature ventricular contraction/ventricular tachycardia (PVC/VT) were retrospectively analyzed from an ablation registry. Procedural outcomes, including procedure time, mapping time, X-ray time, and RF time, were assessed. The complications were classified into two categories: major and minor. A subanalysis was used to illustrate the learning curve of RMN-guided ablation by assessing procedure time and total X-ray time of 502 atrial fibrillation (AF) ablation procedures. Results: Among these procedures, 556 (55.4%) were AF and 290 (28.9%) were PVC/VT. Electrical pulmonary vein isolation was achieved in 99.0% of AF procedures, and acute success reached 90.3% in PVC/VT procedures. The overall complication rate was 0.5%. In the subanalysis of AF procedures, the overall procedure time and X-ray time of procedures were short (125.9 ± 54.6 and 5.3 ± 3.9 minutes, respectively) and proceeded to decrease from the initial 30 procedures to about 300 procedures, where the learning curve reached plateau, demonstrating maximum procedure efficiency. Conclusions: RMN-guided ablation is safe, as verified by very low overall complication rate and reduced X-ray time. In our study, even the first AF procedures had a relatively low procedure time and total X-ray time, and procedure efficiency improved during the learning curve.

Abstract:Abstract Aims Conduction system pacing (CSP) and atrioventricular junction ablation (AVJA) improve outcomes in patients with symptomatic, refractory atrial fibrillation (AF). Superior approach (SA) from the pocket, via axillary or subclavian vein, has been recently proposed as an alternative to the conventional femoral venous approach (FA) to perform AVJA. In this study we compared the impact of these alternative approaches on the nurse workload (NWL) and on patient satisfaction. Methods and results Prospective, observational study, enrolling consecutive patients undergoing simultaneous CSP and AVJA. ElectrophysiologyLaboratory (EP Lab) NWL was calculated with a self-developed model. Ward NWL was calculated using the MIDENF® validated scale. Patient satisfaction was collected using the Hospital Consumer Assessment of Healthcare Provider Systems (HCAHPS) questionnaire. A total of 119 patients were enrolled: in 50, AVJA was primarily attempted with SA, in 69 from FA. Compared to FA, SA was associated with a lower EP Lab NWL (169.8±26.7 vs. 202.7±38.9 minutes; p<0.001), and a lower Ward NWL (474.5±184.8 vs. 808.6±289.9 minutes; p<0.001). Multivariate analysis identified SA as an independent predictor of lower EP Lab NWL (hazard ratio 4.60; p=0.001), and of lower Ward NWL (hazard ratio 45.13; p<0.001). Compared to FA, SA was associated with a higher patient-reported rating regarding the experience during hospital stay (p=0.035), and the overall hospital evaluation (p=0.026). Conclusions In patients undergoing simultaneous CSP and AVJA, the use of a SA for ablation is a valid alternative to conventional FA. Compared to FA, this approach significantly reduces NWL, and is associated with greater patient satisfaction.

Abstract:Objectives: Critical CHD is associated with morbidity and mortality, worsened by delayed diagnosis. Paediatric residents are front-line clinicians, yet identification of congenital CHD remains challenging. Current exposure to cardiology is limited in paediatric resident education. We evaluated the impact of rapid cycle deliberate practice simulation on paediatric residents' skills, knowledge, and perceived competence to recognise and manage infants with congenital CHD. Methods: We conducted a 6-month pilot study. Interns rotating in paediatric cardiology completed a case scenario assessment during weeks 1 and 4 and participated in paired simulations (traditional debrief and rapid cycle deliberate practice) in weeks 2–4. We assessed interns' skills during the simulation using a checklist of "cannot miss" tasks. In week 4, they completed a retrospective pre-post knowledge-based survey. We analysed the data using summary statistics and mixed effect linear regression. Results: A total of 26 interns participated. There was a significant increase in case scenario assessment scores between weeks 1 and 4 (4, interquartile range 3–6 versus 8, interquartile range 6–10; p-value < 0.0001). The percentage of "cannot miss" tasks on the simulation checklist increased from weeks 2 to 3 (73% versus 83%, p-value 0.0263) and from weeks 2–4 (73% versus 92%, p-value 0.0025). The retrospective pre-post survey scores also increased (1.67, interquartile range 1.33–2.17 versus 3.83, interquartile range 3.17–4; p-value < 0.0001). Conclusion: Rapid cycle deliberate practice simulations resulted in improved recognition and initiation of treatment of simulated infants with congenital CHD among paediatric interns. Future studies will include full implementation of the curriculum and knowledge retention work.
