Abstract:Ultrasound (US) imaging has been widely utilized for guiding puncture procedures in spinal interventions. However, conventional methods that confine puncture within the US plane may not adequately meet the demands of endoscopic spinal surgery (ESS), which requires extensive spatial orientation adjustments. In this study, we introduce a novel approach using a US robotic system (URS) integrated with a multi-approach puncture mechanism to aid in target localization and puncture assistance during ESS. The proposed mechanism incorporates a five-bar linkage structure with a remote center of motion (RCM) to drive the needle with two degrees of freedom, enabling significant orientation adjustments of $85^{\circ }$ in the transverse plane and $140^{\circ }$ in the coronal plane. Additionally, the probe holder module provides vertical adaptability of the probe and depth adjustment of the RCM on the US plane. Experimental validation on a cross-wire model demonstrates puncture accuracy of less than 3 mm, meeting clinical requirements for ESS. Furthermore, the system's clinical usability is demonstrated on a gel lumbar model, showcasing the entire process (imaging, targeting, navigating, and puncture assisting) from imaging to puncture assistance. The URS shows promise in significantly improving the puncture success rates in ESS while reducing technical dependencies.

Abstract:Minimally invasive pedicle screw placement (MIPSP) is a widely used treatment for spine diseases. When coupled with intraoperative navigation modalities, robots may help improve surgical outcomes and reduce complications. With such a system, the application of pedicle screws has been expanded from needle insertion to the spine surgery. This paper investigates the possibility and feasibility of robot-assisted MIPSP based on ultrasound (US) guidance. The proposed system is non-radiative and fiducial-free, using purely image information to close the registration loop. Then the system automatically positions the drill tip to a planned screw trajectory and executes the drilling operation. Experiments were conducted on both ex-vivo lamb and human cadaver spines. An entry point accuracy of 2.39±1.41 mm, and orientation accuracy of 2.82±1.85∘ was found for 24 drilled trajectories on three lamb spines. On the ex-vivo human spine, the position error averaged 3.08±2.43 mm at the entry point and 4.05±2.62 mm at the stop point across 16 drilling instances. Moreover, a 87.5% success rate was reported by using Gertzbein-Robbins grade. The experimental results demonstrate the potential for offering a radiation-free alternative. Although restricted to cadaver trials, this work encourages further exploration of this technology to assist surgeons in maximizing performance in clinical practice.
Design and application of a novel patient-specific 3D printed drill navigational guiding template in percutaneous thoracolumbar pedicle screw fixation: A cadaveric study

Abstract:The conventional surgical method of percutaneous pedicle screw fixation (PPSF) mainly uses X-ray fluoroscopy guidance to target the vertebral pedicle for screw placement. This study aimed to explore the feasibility of establishing a personalized drill guide template for PPSF based on a three-dimensional (3D) printing technique and to evaluate the accuracy and safety of the method for assisting screw insertion in cadaveric specimens. The T3-L3 trunk cadaveric specimens from six adults were subject to a computed tomography (CT) scan in the prone position. A three-dimensional model containing the back skin contour was reconstructed. A bilateral ideal pedicle screw in the T6-L1 segment was designed. Then, the reverse templates were designed. The two templates were fused and printed into an individualized guide template. PPSF was performed under the assistance of the guide template, and the CT scan was taken postoperatively to access the screw position. Ninety-six pedicle screws were successfully placed on the bilateral vertebral body of the T6-L1 segment with the assistance of a guide template. The guide plate was not loosened or displaced when operated by a single hand, and the operation time was 24.6 ± 7.9 s. The axial CT images after puncture indicated that in 96 puncture needles, 90 needles were grade I and 6 were grade II, with a puncture accuracy rate of 98.6%. In conclusion, an individualized PPSF navigation template was developed using Mimics software and 3D printing prototyping, which improved the accuracy of PPSF in cadaveric specimens.

Abstract:BACKGROUND The proper installation for pedicle screws by the traditional method of surgeons dependent on experience is not guaranteed, and educational solutions have progressed from chalkboards to electronic teaching platforms. We designed a case of 3-dimensional printing drill guide template as a surgical application, which can accurately navigate implantation of pedicle screws, and assessed its effect for simulative training. MATERIAL AND METHODS We randomly selected a set of computed tomography data for spondylolisthesis. A navigational template of pedicles and screws was designed by software Mimics and Pro-E, where trajectories of directions and angles guiding the nail way were manipulated for screwing based on anatomy, and its solid model was fabricated by a BT600 3D printer. The screws were integrated and installed to observe their stability. RESULTS The navigational model and custom spine implants were examined to be compatibly immobilized, because they are tolerant to radiation and stable against hydrolysis. The screw size and template were fit accurately to the vertebrae intraosseously, because the pilot holes were drilled and the trajectories were guided by cannulas with visible routes. During the surgical workflow, the patient reported appreciation and showed substantial compliance, while having few complications with this approach. Compared with fluoroscopy-assisted or free-hand techniques, the effect of simulative training during processing was excellent. CONCLUSIONS The surgical biomodel is practical for the procedural accuracy of surgical guides or as an educational drill. This fostering a style of "practice substituting for teaching" sets a paragon of keeping up with time and is worthy of recommendation.

Abstract:Abstract Purpose Spinal instrumentation with pedicle screw placement (PSP) is an important surgical technique for spinal diseases. Accurate screw trajectory is a prerequisite for PSP. Ultrasound (US) imaging with robot‐assisted system forms a non‐radiative alternative to provide precise screw trajectory. This study reports on the development and assessment of US navigation for this application. Methods A robot‐assisted US reconstruction was proposed and an automatic CT‐to‐US registration algorithm was investigated, allowing the registration of screw trajectories. Experiments were conducted on ex‐vivo lamb spines to evaluate system performance. Results In total, 72 screw trajectories are measured, displaying an average position accuracy of 2.80 ± 1.14 mm and orientation accuracy of 1.38 ± 0.61°. Conclusion The experimental results demonstrate the feasibility of proposed US system. This work, although restricted to laboratory settings, encourages further exploration of the potential of this technology in clinical practice.

Objective The incidence of degenerative diseases of the lumbar spine has increased in recent years. Unilateral pedicle screw combined with contralateral translaminar facet screw fixation offers the advantages of less trauma, better stability, and fewer complications. However, the surgical difficulty and suboptimal pinning accuracy of translaminar facet screw placement in clinical practice limit its use. Therefore, in this study, we designed a novel suspended 3D‐printed navigation module to facilitate fast and accurate intraoperative screw placement. The aim of this study is to investigate the digital design, precise implementation, and evaluation methods for placing unilateral pedicle screws in the lumbar spine combined with translaminar facet screw placement using a new suspended 3D navigation module. Methods This retrospective study included 46 patients with single‐level lumbar lesions who underwent spine surgery at the Affiliated Hospital of Putian University between June 2022 and December 2023. The suspended navigation module was designed digitally. Preoperative screw placement was simulated using 3D printed models, followed by an intraoperative accurate screw placement facilitated by the navigation module and a postoperative evaluation of the accuracy of screw placement. The absolute difference in three‐dimensional coordinates of the inlet and outlet points of the preoperative design and the postoperative screw‐nail channel served as the precision index. Results In a study involving 46 patients, surgery was successful with 92 pedicle screws and 46 translaminar facet screws placed without any penetration of the cortex. The difference in coordinates before and after screw insertion was minimal, with entry points varying between 1.21 to 1.36 mm and exit points between 1.97 to 2.46 mm. When screw accuracy met certain thresholds, there was no significant difference between preoperative design and postoperative coordinates, indicating precise replication of the surgical plan. Conclusion The new suspended 3D navigation module enables the precise placement of unilateral pedicle screws in the lumbar spine combined with translaminar pedicle screws for precise surgery.

Abstract:PurposePosterior spinal fusion surgery is required to correct severe idiopathic scoliosis. The surgery involves insertion of screws which requires high accuracy to prevent neurologic damage to the spinal cord. Although conventional CT navigation can reduce this risk, 3D-ultrasound-based navigation could achieve this without added ionizing radiation and usage of expensive and bulky equipment. This study aimed to evaluate the accuracy of a 3D ultrasound navigation system for posterior spine surgery.MethodsA custom 3D ultrasound (3DUS) with model-to-surface registration algorithm was developed and integrated into a 3D navigation environment. A CT scan of an adolescent spine (T3-T11) was segmented and 3D printed for experiments. A probe with reflective markers was placed in vertebral pedicles 684 times in varying levels, positions in the capture space and orientation of vertebra, and the entrypoint and trajectory accuracies were measured.ResultsAmong 684 probe placements in vertebral levels T3 to T11 in the phantom spine, 95.5% were within 1 mm and 5° of accuracy, with an average accuracy of 0.4 ± 0.4 mm and 2.1 ± 0.9°, requiring 8.8 s to process. Accuracies were statistically significantly affected by vertebral orientation and position in the capture volume, though this was still within the targeted accuracies of 1 mm and 5°.ConclusionThis preliminary ultrasound-based navigation system is accurate and fast enough for guiding placement of pedicle screws into the spine in posterior fusion surgery. The current results are limited to phantom spines, and future study in animal or human cadavers is needed to investigate soft tissue effects on registration accuracy.

Feasibility and Accuracy of Computer-Assisted Individual Drill Guide Template for Minimally Invasive Lumbar Pedicle Screw Placement Trajectory

Abstract:Objective: To discuss the feasibility and accuracy of a specific computer-assisted individual drill guide template (CIDGT) for minimally invasive lumbar pedicle screw placement trajectory (MI-LPT) through a bovine cadaveric experimental study. Design: A 3-D reconstruction model, including lumbar vertebras (L1-L5), was generated, and the optimal MI-LPTs were determined. A drill guide template with a surface made of the antitemplate of the vertebral surface, including the spinous process and the entry point vertebral surface, was created by reverse engineering and rapid prototyping techniques. Then, MI-LPTs were determined by the drill guide templates, and the trajectories made by K-wires were observed by postoperative CT scan. Setting: General Hospital of Shenyang Military Area Command of Chinese PLA. Results: In total, 150 K-wires for MI-LPTs were successfully inserted into L1-L5. The required mean time and fluoroscopy times between fixation of the template to the spinous process, entry point vertebral surface, and insertion of the K-wires for minimally invasive lumbar pedicle screw placement trajectories into each vertebra were 79.4 + 15.0 s and 2.1 + 0.8 times. There were no significant differences between the preoperative plan and postoperative assessment in the distance from the puncture to the midline and inclination angles according to the different levels (P > 0.05, respectively). The mean deviation between the preoperative plan and postoperative assessment in the distance from the puncture to the midline and inclination angles were 0.8 +/- 0.5 mm and 0.9 +/- 0.5 degrees, respectively. Conclusions: The potential use of the novel CIDGT, which was based on the unique morphology of the lumbar vertebra to place minimally invasive lumbar pedicle screws, is promising and could prevent too much radiation exposure intraoperatively. Abstract:The purpose of this study was to understand the acoustic properties of human vertebral cancellous bone and to study the feasibility of ultrasound-based navigation for posterior pedicle screw fixation in spinal fusion surgery. Fourteen human vertebral specimens were disarticulated from seven un-embalmed cadavers (four males, three females, 73.14 ± 9.87 years, two specimens from each cadaver). Seven specimens were used to measure the transmission, including tests of attenuation and phase velocity, while the other seven specimens were used for backscattered measurements to inspect the depth of penetration and A-Mode signals. Five pairs of unfocused broadband ultrasonic transducers were used for the detection, with center frequencies of 0.5 MHz, 1 MHz, 1.5 MHz, 2.25 MHz, and 3.5 MHz. As a result, good and stable results were documented. With increased frequency, the attenuation increased (P<0.05), stability of the speed of sound improved (P<0.05), and penetration distance decreased (P>0.05). At about 0.6 cm away from the cortical bone, warning signals were easily observed from the backscattered measurements. In conclusion, the ultrasonic system proved to be an effective, moveable, and real-time imaging navigation system. However, how ultrasonic navigation will benefit pedicle screw insertion in spinal surgery needs to be determined. Therefore, ultrasound-guided pedicle screw implantation is theoretically effective and promising.

Abstract:Study Design. To develop a high-accuracy, easy-manipulated, and low-cost cervical pedicle three-dimensional locator for guiding cervical pedicle screw placement. Objectives. To improve the safety of cervical pedicle screw insertion, simplify its manipulation, and offer safety guarantees in clinical application. Summary of Background Data. Because of the complicacy of the anatomic structure of cervical pedicle, clinical application of cervical pedicle screw remained hard to popularize. And recent advances on cervical pedicle location did not simplify the manipulation of procedure significantly. Without aid of locator, distance and angle errors were difficult to control. So far, the only instrument for the location of pedicle "computer-assisted image-guided surgical system" is too expensive for clinical use. Furthermore, it has not yet been documented on location instrument for insertion of cervical pedicle screws. Methods. The high-accuracy three-dimensional locator, made of stainless steel was designed to aid cervical pedicle nail placement. The cadavers were inserted nails under guidance of the locator that has been input the corresponding parameters collected from CT images. Results were evaluated by CT scans. Results. A total of 90 nails were inserted into cervical pedicles, 90% of which were found right in the pedicles; 10% had pedicle cortex breaches. Parallel offset error of the locator in cross-sectional and lateral CT image that was 0.56 +/- 0.70 and 1.04 +/- 0.99; angular offset error was 1.69 +/- 2.41 and 6.54 +/- 7.08. Conclusion. Three-dimensional locator of cervical pedicle and its localization technique achieved good results for individuated screws insertion into cervical pedicles, having such advantages as easy manipulation, high accuracy, and low cost. The system presented safety guarantee to the application and advanced the prevalence of pedicle techniques. Its principium and design were demonstrated feasible.

Abstract:Methods of interactive image generation using processed video contents, and its 3D model capture are proposed; we mainly stress the key technique of the system and the implementation of moving 3D object extraction. In order to reflect a user's intention for generating new multimedia contents, we discuss the methods of storage, retrieval, and presentation. This system is an example of the novel creation of multimedia contents by using the technologies of computer vision and virtual reality. The user is able to generate novel multimedia contents without shape modeling by a human. The proposed low-cost extraction and interaction algorithm can track objects at 8 frames/sec with sufficient accuracy.

Abstract:Study Design: Retrospective database study. Objectives: To compare the accuracy and safety of 2 types of a computer-assisted navigation system for percutaneous pedicle screw placement during endoscopic lumbar interbody fusion. Methods: From May 2019 to January 2020, data of 56 patients who underwent Endo-LIF with a robot-assisted system and with an electromagnetic navigation system were compared. The pedicles in all patients were subjected to postoperative CT scan to assess screw correction by measuring the perpendicular distance between the pedicle cortical wall and the screw surface. The registration and matching time, guide-wire insertion time, the entire surgery time, and X-ray exposure time were recorded. Results: In the robot-assisted group, 25 cases with 100 percutaneous pedicle screws were included, and the excellent and good rate was 95%. In the electromagnetic navigation group, 31 cases with 124 screws were included, and the excellent rate was 97.6%. There was no statistical difference between the two groups ( P > 0.05). The registration time and the total time for the surgery also showed no statistical differences ( P > 0.05). The main difference between the two groups was the guide-wire insertion time and the X-ray exposure time ( P < 0.05). Conclusions: Both electromagnetic navigation and robot-assisted are safe and efficient for percutaneous pedicle screw placement. Electromagnetic navigation system has obvious advantages over robot-assisted in terms of faster guide-wire placement and less X-ray exposure. Robot-assisted for percutaneous pedicle screw placement offers a preoperative planning system and a stable registration system, with obvious drawbacks of a strict training curve.

Abstract:Towards reducing the failure rate of spinal fixation surgical procedures in osteoporotic patients, we propose a unique biomechanically-aware framework for the design of a novel concentric tube steerable drilling robot (CT-SDR). The proposed framework leverages a patient-specific finite element (FE) biomechanics model developed based on Quantitative Computed Tomography (QCT) scans of the patient's vertebra to calculate a biomechanically-optimal and feasible drilling and implantation trajectory. The FE output is then used as a design requirement for the design and evaluation of the CT-SDR. Providing a balance between the necessary flexibility to create curved optimal trajectories obtained by the FE module with the required strength to not buckle during drilling through a hard simulated bone material, we showed that the CT-SDR can reliably recreate this drilling trajectory with errors between 1.7-2.2%

Abstract:This study aims to explore the real-time navigation with guide template using an augmented reality head-mounted device (ARHMD) for pedicle screw placement. The spatial coordinate relationships between augmented reality images and real objects were established through the custom-made guide template, and the registration and tracking were completed using an ARHMD. The feasibility and accuracy of this method were verified by pedicle screw placement in 2 lumbar models. According to the Gertzbein–Robbins grading scale, the accuracy of pedicle screw placement was assessed. The navigation errors were estimated by measuring the deviation values of entry point and trajectory angle. The principle of pedicle standard axis positioning is adopted to make the axis of the pedicle overlap with the x-ray axis of c-arm.Results Then the clinical operation is carried out to verify the clinical feasibility of the designed robot and positioning method. The experimental results show that a total of 26 pedicle screws were accurately implanted. The accuracy of Grade A is 96.15%. The positioning time of a single guide pin is about 154.77 s, and three x-ray films need to be taken on average.Conclusions The positioning accuracy is increased by using the present method. In addition, this method is simple in operation, short in operation time, low in X-ray exposure.
Percutaneous thoraco-lumbar-sacral pedicle screw placement accuracy results from a multi-center, prospective clinical study using a skin marker-based optical navigation system

Abstract:Study design: Prospective multi-center study. Objective: The study aimed to evaluate the accuracy of pedicle screw placement using a skin marker-based optical surgical navigation system for minimal invasive thoraco-lumbar-sacral pedicle screw placement. Methods: The study was performed in a hybrid Operating Room with a video camera-based navigation system integrated in the imaging hardware. The patient was tracked with non-invasive skin markers while the instrument tracking was via an on-shaft optical marker pattern. The screw placement accuracy assessment was performed by three independent reviewers, using the Gertzbein grading. The screw placement time as well as the staff and patient radiation doses was also measured. Results: In total, 211 screws in 39 patients were analyzed for screw placement accuracy. Of these 32.7% were in the thoracic region, 59.7% were in the lumbar region, and 7.6% were in the sacral region. An overall accuracy of 98.1% was achieved. No screws were deemed severely misplaced (Gertzbein grading 3). The average time for screw placement was 6 min and 25 secs (± 3 min 33 secs). The average operator radiation dose per subject was 40.3 µSv. The mean patient effective dose (ED) was 11.94 mSv. Conclusion: Skin marker-based ON can be used to achieve very accurate thoracolumbarsacral pedicle screw placements.
