Abstract:Background Advanced ankylosing spondylitis is often associated with thoracolumbar kyphosis, resulting in an abnormal spinopelvic balance and pelvic morphology. Different osteotomy techniques have been used to correct AS deformities, unfortunnaly, not all AS patients can gain spinal sagittal balance and good horizontal vision after osteotomy. Materials and Methods Fourteen consecutive AS patients with severe thoracolumbar kyphosis who were treated with two-level PSO were studied retrospectively. All were male with a mean age of 34.9 ± 9.6 years. The followup ranged from 1-5 years. Preoperative computer simulations using the Surgimap Spinal software were performed for all patients, and the osteotomy level and angle determined from the computer simulation were used surgically. Spinal sagittal parameters were measured preoperatively, after the computer simulation, and postoperatively and included thoracic kyphosis (TK), lumbar lordosis (LL), sagittal vertical axis (SVA), pelvic incidence, pelvic tilt (PT), and sacral slope (SS). The level of correlation between the computer simulation and postoperative parameters was evaluated, and the differences between preoperative and postoperative parameters were compared. The visual analog scale (VAS) for back pain and clinical outcome was also assessed. Results Six cases underwent PSO at L1 and L3, five cases at L2 and T12, and three cases at L3 and T12. TK was corrected from 57.8 ± 15.2° preoperatively to 45.3 ± 7.7° postoperatively ( P < 0.05), LL from 9.3 ± 17.5° to -52.3 ± 3.9° ( P < 0.001), SVA from 154.5 ± 36.7 to 37.8 ± 8.4 mm ( P < 0.001), PT from 43.3 ± 6.1° to 18.0 ± 0.9° ( P < 0.001), and SS from 0.8 ± 7.0° to 26.5 ± 10.6° ( P < 0.001). The LL, VAS, and PT of the simulated two-level PSO were highly consistent with, or almost the same as, the postoperative parameters. The correlations between the computer simulations and postoperative parameters were significant. The VAS decreased significantly from 6.1 ± 1.9 to 2.0 ± 1.1 ( P < 0.001). In terms of clinical outcome, 10 cases were graded “excellent” and 4 cases were graded “good.” Conclusion Two-level PSO using a preoperative computer simulation is a feasible, safe, and effective technique for the treatment of severe thoracolumbar kyphosis in AS patients with normal cervical motion.
[Use Sagittal Reconstruction CT for Making Decisions Regarding the Surgical Strategy for Cervical Ossification of the Posterior Longitudinal Ligament].

Abstract:OBJECTIVE:Use sagittal reconstruction CT to verify the surgical strategy for cervical ossification of the posterior longitudinal ligament (OPLL).METHODS:A retrospective study of 161 patients (106 males and 55 females) who had undergone surgery for OPLL from July 2007 to November 2010 was performed. The mean age at surgery was 54.5 years (range from 26 to 77 years). The mean follow-up period was 28 months (12 - 54 months). There were 40 patients accept anterior approach surgeries (anterior group) which include 14 cases of anterior cervical corpectomy and fusion and 26 cases of anterior cervical discectomy and fusion. There were 120 patients accept posterior approach surgeries (posterior group) which was spinous process-splitting laminoplasty for cervical myelopathy using coralline hydroxyapatite. One patient accepted combined anterior and posterior approach. According to the sagittal reconstruction CT, the main reason for spinal cord compression was cervical disc herniation in anterior group, and OPLL in posterior group. The level of spinal cord compression was 1 to 2 levels in anterior group, and 1 to 5 levels in posterior group with a major of 2 to 4 levels. As the classification of OPLL, segmental type and circumscribed type were major of segmental type in anterior group and all of the four types were in posterior group, the distribution of each type was average. The patients of posterior group were classified into two groups according to the modified K-line classification, and clinical results were compared between the two groups. The modified K-line was defined as a line that connects the midpoints of the spinal canal at C(2) and C(7) on sagittal CT myelography. Compression to the spinal cord did not exceed the K-line in the modified K-line(+) group and did exceed it in the modified K-line(-) group. Clinical data were compared using t-test or χ(2) test. Correlation analysis was used to determine the relationships of C(2)-C(7) angulation between sagittal reconstruction CT and neutral position X-ray.RESULTS:The patient of anterior group had better recovery rate of the JOA score (72% ± 27%) than the posterior group (59% ± 35%) at the latest follow-up (t = 2.238, P = 0.027). In posterior group, the patients of modified K-line(+) group had better recovery rate of the JOA score (63% ± 37%) than the K-line(-) group (49% ± 30%) at the latest follow up (t = 2.150, P = 0.034). The C(2)-C(7) angulation on sagittal reconstruction CT was 11° ± 9° which has significantly correlated with the C(2)-C(7) angulation on neutral position X-ray which was 10° ± 10° (r = 0.947, P < 0.01).CONCLUSIONS:Considering the selection of surgical approach, it should be combined with the main clinical diagnosis for spinal cord compression, the level of compression, the classification of OPLL and the kyphotic alignment of the cervical spine. The modified K-line is a simple and practical tool for making decisions regarding the surgical strategy for cervical OPLL patients.

Abstract:Objectives: To analyze the clinical characteristics and surgical results of posttraumatic kyphosis of the thoracolumbar segment. Methods: Thirty-four patients with posttraumatic kyphosis of the thoracolumbar segment underwent posterior osteotomy, kyphosis correction, and fixation at our hospital. The kyphosis apex was from T10 to L2. There were 15 male and 19 female patients. The mean age was 48.9 years. Full-spine standing radiographs were obtained before surgery, after surgery, and at follow-up. The sagittal spine balance status was evaluated, and the kyphotic angle, the lumbar lordotic angle, and the thoracic kyphotic angle were measured. The Frankel grading system for neurological functions and the Visual Analogue Scale for back pain were evaluated before surgery and at follow-up. The Oswestry Disability Index was applied for assessment of life quality and surgical results of patients. The Patient Satisfied Index (PSI) was also used for determination of surgical results. Patients were grouped according to the extent of kyphotic angle and the surgical technique. The difference in kyphosis correction and back pain between groups was observed before and after surgery. Relief from neurological symptoms was also observed. Results: The average kyphotic angle was 48.5 degrees, the average lumbar lordotic angle was 57.9 degrees, and the average thoracic kyphotic angle was 11.4 degrees before surgery. The sagittal spine balance was well maintained in 14 cases, and negative imbalance and positive imbalance were seen in 13 cases and 7 cases, respectively. There was significant difference in back pain between patients in the abnormal and normal groups of the lumbar lordotic angle. The average follow-up time after surgery was 32.1 months. The average kyphotic angle correction rate was 90.4% after surgery and 87.2% at follow-up. The average Oswestry Disability Index was 50.4% before surgery, which improved to 29.4% at follow-up, with an improvement rate of 41.5%. The Patient Satisfied Index result showed a total satisfactory rate of 94.1%. There were 16 cases of neurological deficit before surgery; 8 of them achieved various degrees of improvement after surgery. Conclusions: Surgical correction of posttraumatic kyphosis of the thoracolumbar segment can show good radiologic and clinical results with the appropriate procedure according to the extent of the kyphosis angle. Some patients with neurological deficit are still worthy of surgical treatment.

Abstract:OBJECTIVE To determine the effectiveness of surgical correction for post-traumatic kyphosis of thoracolumbar spine. METHODS From 1996 to 2003, 33 consecutive patients with post-traumatic kyphosis of thoracolumbar spine were corrected surgically. The mean age was 40.3 years (range, 13 - 65 years). The mean time between the initial injury and surgical correction was 36.0 months (range, 6 - 220 months). The kyphotic deformity averaged 40.8 degrees (range, 20 degrees - 82 degrees ). All the patients had neurological deficits. Twelve patients had obvious back pain. Seven patients lost sphincter function completely and nineteen patients lost the function partly. Twenty-three patients had ever undergone laminectomy and/or instrumentation. The treatment procedure consisted of anterior release and posterior spinal osteotomy with instrumentation (15 patients), posterior closing wedge osteotomy with instrumentation (12 patients), anterior release and instrumentation (6 patients). RESULTS Kyphosis was corrected from an average of 40.8 degrees to an average of 5.7 degrees, the corrective rate was 86.0% (40.8 degrees - 5.7 degrees /40.8 degrees). There were no severe complications. The average follow-up period was 24.6 months (range, 6 - 84 months). There was no loss of correction at follow-up. Ten of these patients showed an improvement in neural function by one or two levels according to the classification. Sphincter function recovered partly in ten patients. Back pain was relieved significantly in all of twelve patients with back pain preoperatively. Bony fusion was achieved in thirty-two patients. One patient had nonunion and achieved bony fusion after revision. CONCLUSION Posterior closing wedge osteotomy was suitable to kyphosis less than 40 degrees. Anterior release and posterior spinal osteotomy was effective, especially to the patients with severe kyphosis deformity or with operation history. Patients with incomplete neurological deficits and/or severe back pain could get benefit from osteotomy of spine, even if their medical history was long.

Abstract:BACKGROUND CONTEXT: Radiographic realignment objectives for the surgical correction of adult spinal deformity (ASD) have been well-described. However, the optimal sagittal spinopelvic alignment after corrective osteotomy for thoracolumbar kyphosis secondary to ankylosing spondylitis (AS) is still unknown so far. PURPOSE: To comprehensively investigate the relationship between radiographic parameters and clinical outcomes measured by Oswestry Disability Index (ODI) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) in thoracolumbar kyphosis patients caused by AS following one-level pedicle subtraction osteotomy (PSO) and to determine the optimal sagittal alignment at a minimum of 2-year follow-up. STUDY DESIGN: A retrospective review of consecutive AS patients from one institution (2006-2016). PATIENT SAMPLE: One hundred AS-related thoracolumbar kyphosis patients who underwent one-level PSO with a minimum of 2-year follow-up (range, 2-10 years). OUTCOME MEASURES: Spinopelvic parameters at final follow-up (>= 2 years) included thoracic kyphosis, lumbar lordosis, osteotomized vertebra angle, pelvic tilt (PT), sacral slope (SS), pelvic incidence, sagittal vertical axis, spinosacral angle (SSA), T1 pelvic angle (TPA), spinopelvic angle (SPA), and global kyphosis. Clinical outcomes at final follow-up (>= 2 years) was evaluated by ODI and BASDAI, and ODI<20 was defined as good clinical outcome. METHODS: Pre- and postoperative radiographic and clinical outcomes were compared by paired samples t test. The correlation between the postoperative radiographic parameters and clinical outcomes was investigated by Pearson correlation analysis. Linear regression analysis was performed to construct predictive models of the clinically relevant radiographic parameters based on ODI scores and to determine the realignment goals. Multiple stepwise regression analysis was applied to figure out the major radiographic contributors of ODI score at the final follow-up. RESULTS: In total, 100 AS patients (92 males and 8 females) with a mean age of 34.7 +/- 9.5 years (range, 17-63 years) were followed up for 38.6 +/- 17.5 months (range, 24-120 months). At the final follow-up, PT and TPA were positively correlated with both ODI and BASDAI score (p<.05). Although SS, SSA, and SPA were negatively associated with the score of ODI (p<.05), and BASDAI was negatively related to SPA (p<.05). Predicting regression models of the clinically-relevant radiographic parameters were built based on the ODI score at the last follow-up. CONCLUSIONS: Based on the regression models, the optimal sagittal alignment of AS patients satisfying good clinical outcome (ODI<20) at a minimum of 2-year follow-up was: PT<24 degrees, SSA>108 degrees, TPA<22 degrees, and SPA>152 degrees. Realizing the aforementioned realignment goals may contribute to satisfied clinical outcome for AS patients with thoracolumbar kyphosis undergoing one-level PSO. (C) 2019 Elsevier Inc. Pedicle subtraction osteotomy and disc resection with cage placement in post-traumatic thoracolumbar kyphosis, a retrospective study

Abstract:BackgroundIt is estimated that upwards of 50,000 individuals suffer traumatic fracture of the spine each year, and the instability of the fractured vertebra and/or the local deformity results in pain and, if kyphosis increases, neurological impairment can occur. There is a significant controversy over the ideal management. The purpose of the study is to present clinical and radiographic results of pedicle subtraction osteotomy and disc resection with cage placement in correcting post-traumatic thoracolumbar kyphosis.MethodsFrom May 2010 to May 2013, 46 consecutive patients experiencing post-traumatic thoracolumbar kyphosis underwent the technique of one-stage pedicle subtraction osteotomy and disc resection with cage placement and long-segment fixation. Pelvic incidence (PI), pelvic tilt (PT), sagittal vertical axis (SVA), and sagittal Cobb angle were measured to evaluate the sagittal balance. Oswestry disability index (ODI), visual analog scale (VAS), and general complications were recorded.ResultsThe average surgical time was 260 min (240–320 min). The mean intraoperative blood loss was 643 ml (400–1200 ml). The maximum correction angle was 58° with an average of 47°, and the SVA improved from +10.7 ± 3.5 cm (+7.2 to + 17.1 cm) to +4.1 ± 2.7 cm (+3.2 to + 7.6 cm) at final follow-up (p < 0.01). PT reduced from preoperative 27.2 ± 5.3° to postoperative 15.2 ± 4.7° (p < 0.01). The VAS changed from preoperative 7.8 ± 1.6 (5.0–9.0) to 3.2 ± 1.8 (2.0–5.0) (p < 0.01). Clinical symptoms and neurological function were significantly improved at the final follow-up. All patients completed follow-up of 41 months on average.ConclusionsPedicle subtraction osteotomy and disc resection with cage placement and long-segment fixation are effective and safe methods to treat thoracolumbar post-traumatic kyphosis.

Abstract:BACKGROUND CONTEXT: The head's center of gravity (COG) plumb line (PL) and C7 PL could be simultaneously positioned over the pelvis in adult spinal deformity with normal cervical mobility. However, the position of the head in relation to the global spinal alignment has yet to be investigated in ankylosing spondylitis (AS) patients with thoracolumbar kyphosis. PURPOSE: The objective of this study was to analyze the position of head in relation to the global spinal alignment in AS-related thoracolumbar kyphosis. STUDY DESIGN/SETTING: Retrospective single-center study. PATIENT SAMPLE: AS patients who underwent lumbar pedicle subtraction osteotomy for thoracolumbar kyphosis from January 2010 to August 2016 were reviewed. Only patients with a visible ear canal on the preoperative, immediate postoperative, and final follow-up radiographs were included. OUTCOME MEASURES: The chin-brow angle, cervical range of motion (ROM), lumbar lordosis (LL), thoracolumbar angle, thoracic kyphosis (TK), L1 pelvic angle, pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), TK+LL+PI, PI-LL, maximal kyphosis (MK), deformity angular rate, T1 pelvic angle, T1 tilt, spinosacral angle, sagittal vertical axis (SVA) COG-C7, SVA COG-femoral head (FH), and SVA C7-S were evaluated. Data regarding the health-related quality of life, including the Oswestry disability index (ODI) and visual analogue scale (VAS)-back score, were also collected preoperatively, 2 years postoperatively, and at the latest follow-up. METHODS: The cohort was divided into patients with occiput-trunk (OT) concordance or with OT discordance according to the SVA COG-C7 <= 30 mm or >30 mm, respectively. There was no funding in this study and there are no conflict of interest-associated biases. RESULTS: A total of 43 patients (36 males and 7 females) with a mean age of 34.2 years (range, 18- 59 years) were identified. There were 17 patients accompanied with OT concordance and 26 patients with OT discordance preoperatively. The cervical ROM was significantly lower (24.0 degrees vs 56.1 degrees, p<.001) and SVA COG-C7 was significantly larger (71.7 mm vs 7.4 mm, p<.001) in patients with OT discordance. Furthermore, the PT was larger (41.0 degrees vs 33.5 degrees, p=.010) in patients with OT discordance. After surgery, the whole cohort showed an improvement in LL (-8.6 degrees vs -52.8 degrees, p<.001). Moreover, the CBVA (25.4 degrees vs 1.3 degrees, p<.001) and SVA COG-C7 (46.2 mm vs 21.6 mm, p<.001) were significantly decreased following lumbar PSO. There were 13 patients accompanied with OT discordance postoperatively, and the cervical ROM was still lower (22.5 degrees vs 42.8 degrees, p=.024) in these patients. Postoperative PT was larger (26.5 degrees vs 20.1 degrees, p=.033) in the patients with OT discordance. At the latest follow-up, there were 17 patients accompanied with OT discordance. In these 17 patients, the cervical ROM was significantly lower (21.0 degrees vs 47.0 degrees, p=.001) and PT was significantly higher (26.2 degrees vs 19.2 degrees, p=.012). The ODI and VAS-back scores demonstrated no significant differences between the two groups preoperatively, 2 years postoperatively, or at the latest follow-up. CONCLUSIONS: OT discordance in AS-related thoracolumbar kyphosis could be caused by the reduced cervical ROM. To maintain global spinal balance, the pelvis rotated further backward in response to the larger SVA COG-C7. Moreover, the larger SVA COG-C7 could be decreased after the lumbar PSO. Although there were radiographic differences between the patients with OT concordance and with OT discordance, there was no difference in clinical outcomes, and that a larger sample size and longer follow-up are needed. Abstract:Objective: To evaluate the effectiveness of Scoliosis Research Society (SRS)-Schwab grade Ⅳ osteotomy in the treatment of post-traumatic thoracolumbar kyphosis (PTK). Methods: From October 2012 to January 2015, a total of 31 patients [12 males, 19 females, mean age (43±10) years] with symptomatic PTK undergoing SRS-Schwab grade Ⅳ osteotomy in Nanjing Drum Tower Hospital were retrospectively reviewed.Radiographic changes were evaluated with preoperative, postoperative and follow-up X-ray.The measurements included thoracic kyphosis (TK), lumbar lordosis (LL), thoracolumbar kyphosis (TLK), focal kyphosis (FK), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS) and sagittal vertical axis (SVA). Clinical outcome was assessed using the visual analogue scale (VAS), the Oswestry disability index (ODI) and Frankel neurological grade before surgery and during follow-up.The preoperative, postoperative and follow-up data were compared with paired t test. Results: All patients were followed-up for 28±5 months (24-38 months). The TK and LL improved significantly postoperatively from 15.6°±7.8° and 67.3°±12.2° to 28.1°±6.2° and 48.1°±9.3°(t=-6.985, 6.968, both P<0.05), and maintained at 27.3°±6.9° and 49.4°±7.9° at last follow-up, respectively.The TLK and FK improved significantly postoperatively from 29.4°±12.2° and 36.4°±9.5° to 7.7°±5.3° and 5.6°±4.2°(t=9.083, 16.510, both P<0.05), with a correction loss of 2.4°±2.0° and 2.3°±2.2° at final follow-up, respectively.No significant difference was observed in the PI, PT, SS and SVA among preoperative, post-operative and the last follow-up data.In addition, the VAS and ODI improved significantly at the final follow-up.Four cases with Frankel D experienced complete neurological recovery at the final follow-up.Radiographic evaluation showed solid bony fusion.No instrument-related complication was observed during the follow up. Conclusions: The SRS-Schwab grade Ⅳ osteotomy brings satisfactory sagittal alignment and good clinical outcomes in patients with PTK.

Abstract:Objective To investigate the correlations of quality of life and sagittal parameters in ankylosing spondylitis (AS) patients with thoracolumbar kyphosis.Methods From June 2011 to August 2013,64 AS patients were reviewed,including 30 outpatients and 34 inpatients underwent pedicle subtraction osteotomy (PSO).Spino-pelvic parameters were measured on standing lateral full-spine radiographs,including cervical 7 tilt (C7T),sagittal vertical axis (SVA),global kyphosis (GK),thoracic kyphosis (TK),lumbar lordosis (LL),pelvic incidence (PI),sacral slope (SS),and pelvic tilt (PT).Simultaneously,the scores of Bath ankylosing spondylitis disease activity index (BASDAI),Bath ankylosing spondylitis functional index (BASFI),SF-36 questionnaire and the Oswestry disability index (ODI) were recorded.The subjects were divided in to mild kyphosis group (GK＜60°,n=29) and severe kyphosis group (GK≥60°,n=35).The correlations were be analyzed between the quality of life and the sagittal parameters in AS patients.Multiple regression analysis was used to investigate the sagittal parameters which impaired the quality of life in AS patients.Results The scores of physical role,general health,social function,and emotional role were lower in severe kyphosis group when compared with mild kyphosis group.C7T,TK,GK,SVA,PT,LL,and SS were significant different in the two groups.Lower C7T and LL leaded to the score of physical role decreased; high GK resulted in the low score of emotional role; large SVA diminished the score of social function of AS patients.For AS patients underwent PSO with a mean follow-up of 16 months (range,6-36 months),all the parameters were significantly improved,except for PI and TK,and the scores of general health,social functioning,and role emotional were significant increase at the last follow-up.Conclusion The quality of life in physical role,general health,social function and emotional role were significantly decreased in severe kyphosis group,and C7T,LL,GK,and SVA were the most important factors impaired the quality of life in AS patients.The quality of life can be improved by correcting the sagittal parameters in AS patients with thoracolumbar kyphosis.

Abstract:OBJECTIVE:To evaluate the clinical outcomes of posterior surgical corrective methods for ankylosing spondylitic kyphosis.METHODS:From June 2003 to June 2008, 21 cases of ankylosing spondylitic kyphosis received posterior surgical correction. There were 17 male and 4 female, and the average age was 39.5 years (range, 20 to 57 years). The total spine X-ray and CT were used to evaluate sagittal balance and thoracolumbar spine kyphosis angle, and chin brow-vertical angle was obtained from clinical lateral photograph. The surgical goal was to correct sagittal imbalance and chin brow-vertical angle. The simulated osteotomy was performed in computer before surgery to determine the correction methods. The surgical methods included: 16 cases of monosegmental closing osteotomy correction, 3 cases of anterior opening-posterior closing osteotomy correction, and 2 cases of combined pedicle subtraction osteotomy in thoracolumbar spine and Smith-Peterson osteotomy in lumbar spine. All patients were followed up after surgery, and the improvement of sagittal imbalance, chin brow-vertical angle and thoracolumbar spine kyphosis angle were assessed. The symptoms relief and satisfied rate were also evaluated.RESULTS:The average operation time was 4.4 hours, and the average blood loss was 1770 ml. Before surgery, the average thoracolumbar kyphosis angle was 62.1°, the average anterior shift of C(7) plumb line was 172.9 mm, and the average chin brow-vertical angle was 34.9°. The average follow-up was 28.8 months after surgery. The average correction rate of thoracolumbar kyphosis angle was 60%, the average improvement rate of anterior shift of C(7) plumb line was 64%, and the average correction rate of chin brow-vertical angle was 98%. The improvement rate of back pain was 64% during follow-up. The total surgical satisfactory rate was 95%.CONCLUSION:Based on the simulated osteotomy in computer before surgery, according to the characteristics of ankylosing spondylitic kyphosis, different posterior osteotomy and correction methods can achieve good results.
Correlations of spinopelvic parameters with quality of life of patients with adult degenerative scoliosis after posterior correction

Abstract:Objectives: This study aimed to investigate the correlations of spinopelvic parameters with the quality of life of patients with adult degenerative scoliosis (ADS) after posterior correction, and their relationships with efficacy Patients and methods: Ninety patients (40 females, 50 males; mean age: 62.4±3.3 years; range, 47 to 73 years) with adult spinal deformity treated from March 2016 to May 2020 were retrospectively enrolled. The Scoliosis Research Society (SRS)-22 questionnaire was filled in by the patients, and the Oswestry disability index (ODI) and Visual Analog Scale (VAS) for back and lower limb pain were assessed. All the patients underwent posterior correction. Spearman's analysis was conducted for the correlations of the spinopelvic sagittal parameters with quality of life. The risk factors for efficacy were identified using the multivariate logistic regression model to construct a nomogram model for efficacy and risk prediction. Results: After the operation, significant reductions were detected in the sagittal vertical axis (SVA), pelvic tilt (PT), T1 pelvic angle (TPA), pelvic incidence minus lumbar lordosis (PI-LL), and the ODI score (p<0.05). The SVA and LL were significantly negatively correlated with all subitems on the SRS-22 questionnaire but positively correlated with VAS scores for back pain (p<0.05). Thoracic kyphosis was significantly positively correlated with self-image and mental status on the SRS-22 questionnaire (p<0.05), while TPA was negatively correlated with pain and self-image (p<0.05). The PI-LL was significantly negatively correlated with pain (p<0.05). Conclusion: The SVA, LL, PT, and PI-LL were independent predictors of improvement in ODI after operation for ADS. The postoperative changes in spinopelvic parameters affected the clinical outcomes in patients with ADS.
A Retrospective Study to Compare the Efficacy of Preoperative Halo-Gravity Traction and Postoperative Halo-Femoral Traction after Posterior Spinal Release in Corrective Surgery for Severe Kyphoscoliosis.

Abstract:BACKGROUND This retrospective clinical study aimed to compare the efficacy of preoperative halo-gravity traction with postoperative halo-femoral traction after posterior spinal release in corrective surgery for patients with severe kyphoscoliosis. MATERIAL AND METHODS A retrospective clinical study included patients who underwent elective corrective surgery for severe kyphoscoliosis (N=60) between 2013 and 2015. Two patient groups were compared, the postoperative halo-femoral traction after posterior spinal release (R-HF) group (N=30) and the preoperative halo-gravity traction (HGT) group (N=30). Demographic and clinicopathological data included age, gender, Cobb angle, degree of spinal curvature, history of osteotomy, and etiological factors. Patients in the two study groups were matched. Postoperative surgical outcome was evaluated by the radiographic coronal Cobb angle, global kyphosis, coronal balance, and the sagittal vertical axis (SVA). Clinical outcome was assessed using the Scoliosis Research Society Outcomes Questionnaire (SRS-22). RESULTS The preoperative Cobb angle was similar between the R+HF group and the HGT group (123.5±12.7° vs. 123.1±14.1°; P=0.909). Following postoperative traction, a significantly higher correction rate was found in the R+HF group than the HGT group (31.8±7.8% vs. 19.3±12.9%; P=0.001). The postoperative correction rate in the R+HF group was significantly higher than the HGT group (44.7±7.8% vs. 39.0±12.8%; P=0.042). In both study groups, the postoperative SRS-22 scores were significantly improved with no statistical difference between the two groups, and no neurological complications occurred. CONCLUSIONS Patients with severe kyphoscoliosis who underwent postoperative halo-femoral traction after posterior spinal release achieved satisfactory radiographic correction.

Abstract:Objective:To evaluate the effectiveness of SRS-Schwab grade Ⅳ osteotomy combined with satellite rod for thoracolumbar old osteoporotic fracture with severe kyphosis. Methods:Between April 2013 and August 2016, 20 cases of thoracolumbar old osteoporotic fracture with severe kyphosis were treated with SRS-Schwab grade Ⅳ osteotomy combined with satellite rod. All the patients were females, aged 49-71 years (mean, 54.8 years). The disease duration was 6-28 months with an average of 14 months. The T value of bone density was -4.4 to -1.8 (mean, -2.8). The preoperative Cobb angle was (43.0±11.3)°. The vertebral compression fracture segment was T 12 in 9 cases, L 1 in 8 cases, and L 2 in 3 cases. Preoperative spinal cord function was evaluated by Frankel classification; there were 5 cases of grade D and 15 cases of grade E. The operation time, intraoperative blood loss, and perioperative complication were recorded. The Cobb angle for kyphosis and sagittal vertical axis (SVA) were recorded beforeoperation, at 3 months after operation, and at last follow-up. Oswestry disability index (ODI) was used to evaluate the effectiveness before operation and at last follow-up, and the evaluation indicators included pain degree, daily life self-care ability, extracting, walking, sitting, standing, sleeping, social activities, and traveling. Results:The operation time was 180-314 minutes (mean, 226 minutes). The intraoperative blood loss was 390-1 800 mL (mean, 750 mL). All the incisions healed by first intension without incision infection. Twenty patients were followed up 24-52 months, with an average of 30.9 months. During the follow-up period, no significant complication such as correction loss, nail breakage, rod breakage, pseudoarthrosis formation, or proximal and distal junctional kyphosis occurred. All patients were able to walk upright after operation, and the pain relieved significantly at 6 months after operation. Bone fusion achieved at 12 months after operation. The Frankel grade of nerve function improved from grade D to grade E at last follow-up in 5 patients with nerve damage before operation. At last follow-up, the indicator scores of ODI significantly improved when compared with preoperative values ( P<0.05). Cobb angle significantly improved at 3 months after operation and at last follow-up ( P<0.05) when compared with preoperative one, but there was no significant difference in the Cobb angles between 3 months after operation and last follow-up ( P>0.05). There was no significant difference in SVA between pre- and post-operation ( P>0.05). Conclusion:SRS-Schwab grade Ⅳ osteotomy combined with satellite rod for thoracolumbar old osteoporotic fracture with severe kyphosis is effective in achieving satisfactory clinical outcomes, as well as maintaining correction of kyphosis.
the eye of the beholder: A Qualitative Study of Mutual Obligations and Areas of Ambiguity in the Hospital-Physician Relationship

Abstract:Hospitals and physicians have been working together for years in providing specialized health services. However, Hospital-Physician Relationships are considered lukewarm at best. We build on psychological contract theory to develop an in-depth understanding of how physicians (N=15) and hospital executives (N=15) in Belgium experience and interpret obligations in their working relationship. Our analysis yielded a rich understanding of mutual obligations and areas of ambiguity. Two major themes emerged from the analysis. A distinction should be made between administrative obligations (adequate support and responsive decision making) and professional obligations (clinical excellence and physician autonomy). Two areas of ambiguity could be identified reflecting both dimensions. An economic trade-off exists in the day-to-day interaction and therefore views on how the way care should be organized differ. In addition, the extent to which medical decisions should take into account the corresponding impact on hospital finance varies.

Abstract:Objective: To evaluate the efficacy and safety of Smith-Petersen osteotomy (SPO) assisted by releasing disk space from posterior approach for thoracolumbar kyphosis. Methods: A review was conducted on 8 patients (3 males and 5 females) with thoracolumbar kyphosis were treated with SPO assisted by releasing disk space from posterior approach at Department of Orthopaedics, Peking University Third Hospital from June 2016 to September 2017. The age was 56.5 years (range:18-71 years). There were 3 cases of Scheuermanns kyphosis, 2 cases of degenerative kyphosis, 1 case of proximal junctional kyphosis (PJK) after lumbar surgery, and 2 cases of kyphosis after thoracolumbar laminectomy. The paired t test was used for statistical analysis in thoracolumbar kyphosis angle, osteotomy segment kyphosis angle, sagittal vertical value (SVA), visual analogue score (VAS), Oswestry dysfunction index (ODI) before and after surgery. Statistical difference was confirmed with P<0.05. Results: Osteotomy level included 2 cases in T(11-12), 3 cases in T(12)-L(1), 3 cases in L(1-2.) The average operation time was 339 min (range: 247-416 min), bleeding volume was 1 275 ml (range: 500-2 500 ml). The mean follow-up time was 16.5 months (range: 12-24 months). The average thoracolumbar kyphosis angle was 59.9° (range: 40°-73°) pre-operation, 9.5°(range:-5.1°-20°) post-operation and 13.5°(range:-1.3°-28°) at the latest follow-up. It made an average correction with 46.4°and corrective rate with 78.0%. The osteotomy segment kyphosis angle was 37.9° (range: 26°-46°) pre-operation, -1.3° (range:-11°-13°) post-operation making an 39.2° open-up angle, and 2.0° (range:-13.5°-13°) at the latest follow-up. Lumbar lordosis was 47.5° (range: 2°-76°) pre-operation, 41.2°(range:15°-62°) post-operation and 36.9°(range:15°-58°) at the latest follow-up. SVA was 54 mm(range:-34 mm-149 mm) pre-operation and 39 mm(range:-3 mm-119 mm) at the latest follow-up. VAS score of low back pain was 6.3(range:0-9) pre-operation and 3.0(range:0-6) at the latest follow-up. ODI score was 21.9(range: 0-42) pre-operation and 11.0(range: 0-26) at latest follow-up. Comparing to pre-operation value, there were statistical difference in the thoracolumbar kyphosis angle(t= 8.547, P=0.000), osteotomy segment kyphosis angle(t=9.739, P=0.000), VAS(t=3.077, P=0.018), ODI(t=5.800, P=0.001) at the latest follow-up. There was no neuropathic complication in all patients. Cerebrospinal fluid leakage occurred in 2 cases with spinal surgery history, and recovered after symptomatic treatment. Conclusions: SPO assisted by releasing disk space from posterior approach could safely achieve effective correction of rigid thoracolumbar kyphosis deformity within 40°.
Correction of Thoracolumbar Kyphosis Through Accurate Pedicle Subtraction Osteotomy Using Osteotomes

Abstract:Objective To evaluate clinical effect of accurate pedicle subtraction osteotomy (PSO) using osteotomes in the treatment of thoracolumbar kyphosis (TLK).Methods From June 2007 to October 2010,18 patients with TLK underwent accurate PSO using osteotomes under X-ray fluoroscopy,including 13 males and 5 females,with an average age of 48.6 years.The primary causes of TLK included old fracture (11cases),chronic tuberculosis (4 cases) and hemivertebra (3 cases).Deformity apex occurred at T12 (5 cases),L1 (9 cases),and L2 (4 cases).Radiological assessment for sagittal balance was performed by measuring Cobb angle.The Frankel grade,visual analogue scale (VAS) and Oswestry disability index (ODI) were used to evaluate pre-and post-operative neurological status,back pain and function.Results The mean operative time,mean blood loss and mean postoperative drainage volume were 247.0±29.3 minutes,708.5±34.5 ml and 337.3±74.6 ml,respectively.All patients were followed up for 1 to 4.5 years (average,2.8 years).Solid fusion was achieved in all patients.Cobb angle was corrected from preoperative 42.3°±5.7° to 2.2°±1.9° three months postoperatively and 2.7°±2.1 ° at final follow-up.VAS and ODI scores decreased from preoperative 8.5±1.0 and 72.8％±8.3％ to 2.1±0.7 and 21.6％±9.2％ three months postoperatively,and 1.9±0.6 and 19.3％±8.6％ at final follow-up,respectively.With regard to Frankel grade,a 1-grade and 2-grade improvement was observed in 7 cases and 2 cases 3 months postoperatively,respectively.At final follow-up,a 1-grade and 2-grade improvement was observed in 5 cases and 4 cases,respectively.Two patients had transient neurological symptoms postoperatively,which recovered after drug treatment for 2 weeks.No other complications occurred.Conclusion It is safe and effective to correct TLK through accurate PSO using osteotomes,which has some advantages,such as less blood loss,higher fusion rate and fewer complications.
Combined anterior and posterior approaches for correction of kyphosis of thoracolumbar spine

Abstract:Objective:To evaluate the outcome of combined anterior and posterior approaches for angular kyphosis of thoracolumbar spine.Method:From 1998 to 2003,18 patients with kyphosis of thoracolumbar spine were corrected surgically.The mean age was 34.0 years(range,13~56 years).There were 14 patients with neurological deficits.Seven patients had obvious back pain.Eight patients were with sphincter dysfunction.Eleven patients had ever undergone laminectomy and/or instrumentation.The treatment procedure was combined anterior and posterior approaches.Result:Kyphosis was corrected from an average of 54.8°(range,30°~138°) to an average of 14.7°(-5°~75°),the corrective rate was 70.6%(41.7%~112.5%).Two patients had nonunion of anterior graft and required revision surgery,and achieved bony fusion afterward.The average follow-up period was 24.8 months(range,7~37 months).There were no severe complications.There was no missing of correction at the follow-up.Five of these patients showed an improvement in neural function.Sphincter function recovered partly in three patients.Back pain was relieved significantly in all seven patients.Bony fusion was achieved in all patients.Conclusion:Osteo-relaxation and internal-fixation through combined anterior and posterior approaches could correct kyphosis of thoracolumbar spine effectively,especially in patients with severe kyphosis deformity or with previous operation.

Abstract:Study Design. A retrospective radiographical study. Objective. To identify the radiographical predictors for sagittal imbalance in patients with thoracolumbar kyphosis secondary to ankylosing spondylitis (AS) after 1-level lumbar pedicle subtraction osteotomy (PSO). Summary of Background Data. Few studies had correlated the preoperative sagittal parameters with postoperative sagittal alignments to determine the radiographical predictors for postoperative sagittal imbalance in patients with AS after 1-level lumbar PSO. Methods. Thirty-six patients with thoracolumbar kyphosis secondary to AS who underwent 1-level lumbar PSO were recruited with a minimal follow-up of 24 months (mean = 27.4 mo; range, 24-53 mo). Correlation analysis and subsequent stepwise multiple regression analysis were used to evaluate the correlations between preoperative parameters, including global kyphosis, local kyphosis, thoracic kyphosis, thoracolumbar Cobb angle, lumbar lordosis, pelvic incidence (PI), pelvic tilt, sacral slope, and sagittal vertical axis (SVA), as well as SVA at the last follow-up. All these patients were further divided into 2 groups according to the PI value (group A: PI >50 degrees; group B: PI <= 50 degrees). The correction outcomes were compared between these 2 groups. Results. The preoperative SVA was not significantly different between group A and group B (157.6 mm vs. 124.5 mm; P > 0.05), and both groups had similar magnitudes of kyphosis corrections at the last follow-up (global kyphosis: 42.9 degrees vs. 46.1 degrees; local kyphosis: 42.7 degrees vs. 40.5 degrees; lumbar lordosis: 35.7 degrees vs. 43.0 degrees). However, group A patients had significantly larger SVA at the last follow-up (73.2 mm vs. 28.7 mm; P < 0.05) and a higher incidence of postoperative sagittal imbalance (77.8% vs. 25.9%; P < 0.05) than those in group B. The stepwise multiple regression analysis demonstrated that both preoperative SVA and PI were significant independent predictors of postoperative sagittal alignments, which explained 52.0% and 9.7% of the variability of SVA at the last follow-up, respectively. Conclusion. Patients with AS with either larger preoperative SVA or larger PI are more likely to experience failed sagittal realignments after 1-level lumbar PSO. For these patients, additional osteotomies may be recommended for satisfactory correction outcomes.
Analysis of the Surgical Outcome of Asymmetrical Pedicle Subtraction Osteotomy in Ankylosing Spondylitis Patients with Thoracolumbar Kyphoscoliotic Deformity

Abstract:Objective To investigate the influence of asymmetrical pedicle subtraction osteotomy (APSO) on the reconstruction of coronal and sagittal balance in ankylosing spondylitis (AS) patients with thoracolumbar kyphoscoliotic deformity.Methods Between October 2005 and June 2012,sixteen AS patients (13 males and 3 females) with a mean age of 35.4 years (range,22-48 years) with thoracolumbar kyphoscoliotic deformity undergoing APSO were included in this study.Preoperative,postoperative and last follow-up full-length antero-posterior and lateral spine radiographs were available.Coronal and sagittal parameters were measured,including Cobb angle,central sacral vertical line (CSVL),global kyphosis (GK),sagittal vertical axis (SVA),thoracic kyphosis (TK),lumbar lordosis (LL),pelvic tilt (PT),sacral slope (SS),and pelvic incidence (PI).SF-36 questionnaire was used to evaluate the quality of life of AS patients.The preoperative and postoperative data were compared by paired sample t test.Results The average time of follow-up was 36 months (range,24-63 months).The mean Cobb angle was improved from 25.8°to 7.6°,and the correction rate was 70.5％.The CSVL was corrected from 5.6 cm to 1.8 cm.The mean GK was corrected from 76.8° to 25.6°,and the correction rate was 66.7％.The SVA was restored from 15.1 cm to 3.8 cm.In addition,LL,PT,and SS were improved from-0.4°,33.6°,and 10.3° to 44.1°,22.6°,and 20.9°,respectively.In terms of Cobb angle,CSVL,GK,SVA,LL,PT,and SS,no significant differences were observed.The scores of bodily pain,general health,social and emotional functioning were significantly increased at the last follow-up.Conclusion AS patients with thoracolumbar kyphoscoliotic deformity have both sagittal and coronal imbalance with impairment in quality of life.APSO can achieve successful realignment of biplanar balance by correcting thoracolumbar kyphosis and scoliosis simultaneously,and improve the quality of life in AS patients with kyphoscoliotic deformity.
