Abstract:Background Advanced ankylosing spondylitis is often associated with thoracolumbar kyphosis, resulting in an abnormal spinopelvic balance and pelvic morphology. Different osteotomy techniques have been used to correct AS deformities, unfortunnaly, not all AS patients can gain spinal sagittal balance and good horizontal vision after osteotomy. Materials and Methods Fourteen consecutive AS patients with severe thoracolumbar kyphosis who were treated with two-level PSO were studied retrospectively. All were male with a mean age of 34.9 ± 9.6 years. The followup ranged from 1-5 years. Preoperative computer simulations using the Surgimap Spinal software were performed for all patients, and the osteotomy level and angle determined from the computer simulation were used surgically. Spinal sagittal parameters were measured preoperatively, after the computer simulation, and postoperatively and included thoracic kyphosis (TK), lumbar lordosis (LL), sagittal vertical axis (SVA), pelvic incidence, pelvic tilt (PT), and sacral slope (SS). The level of correlation between the computer simulation and postoperative parameters was evaluated, and the differences between preoperative and postoperative parameters were compared. The visual analog scale (VAS) for back pain and clinical outcome was also assessed. Results Six cases underwent PSO at L1 and L3, five cases at L2 and T12, and three cases at L3 and T12. TK was corrected from 57.8 ± 15.2° preoperatively to 45.3 ± 7.7° postoperatively ( P < 0.05), LL from 9.3 ± 17.5° to -52.3 ± 3.9° ( P < 0.001), SVA from 154.5 ± 36.7 to 37.8 ± 8.4 mm ( P < 0.001), PT from 43.3 ± 6.1° to 18.0 ± 0.9° ( P < 0.001), and SS from 0.8 ± 7.0° to 26.5 ± 10.6° ( P < 0.001). The LL, VAS, and PT of the simulated two-level PSO were highly consistent with, or almost the same as, the postoperative parameters. The correlations between the computer simulations and postoperative parameters were significant. The VAS decreased significantly from 6.1 ± 1.9 to 2.0 ± 1.1 ( P < 0.001). In terms of clinical outcome, 10 cases were graded “excellent” and 4 cases were graded “good.” Conclusion Two-level PSO using a preoperative computer simulation is a feasible, safe, and effective technique for the treatment of severe thoracolumbar kyphosis in AS patients with normal cervical motion.

Abstract:Objective To evaluate the clinical outcome and health related quality of life in degenerative patients with ky?phoscoliosis who underwent S2AI placement and identify the potential risk factors of pelvic incidence variation postoperative and at last follow?up. Methods The present study reviewed degenerative patients with kyphoscoliosis who accepted surgery uti?lized S2AI screw between January 2015 and January 2019. 25 patients were included in our study, 4 males and 21 females, among these patients, the mean age were 58.84±6.03 years, range from 50-68 years. All patients were conducted long fusion distal to pelvis utilizing S2AI screws. According to the variation of pelvic incidence at last follow?up, we divided patients into two subgroups: 1) Group I: Patients' pelvic incidence increased more than 5 degree compared with post?operation. 2) Group S:Patients' pelvic incidence varied less than 5 degree compared with post?operation. 13 patient (2 male, 11 female; mean age:57.23±6.06 years) were categorized into group I, and 12 patients (2 male, 10 female; mean age: 60.58±5.73 years) were catego?rized into group S.Cobb′s angle,lumbar lordosis(LL),regional kyphosis(RK),sagittal vertical axis(SVA), pelvic incidence(PI), pelvic tilt(PT), sacral slop(SS) were recorded at pre?operation,post?operation and last follow up. The MOS item short from the health survey(SF?36) and Oswestry disability index (ODI) were also recorded at pre?operation and last follow up. Results There was no statistical difference in gender, age, preoperative Cobb angle, SVA between two groups. In group I, pelvic inci?dence were decreased postoperatively and increased at the last follow?up (56.92°±14.47° vs 42.69°±14.23° vs 51.62°±14.53°, P＜0.05). In group S, pelvic incidence were decreased postoperatively ( 46.21°± 12.26° vs 37.08°± 12.99°, P＜0.05) and re?mained stable at the last follow up ( 37.08°±12.99° vs 37.17°±13.34°, P＞0.05). Cobb angle (46.08°±20.52° vs 19.96°±12.64° vs 20.28°± 12.01°), RK (32.88°± 15.28° vs-12.16°± 17.75° vs-11.64°± 17.29°), SVA (94.61 ± 92.59 mm vs 25.49 ± 23.89 mm) were improved significantly after surgery and remained stable at the last follow?up. HRQoL score was improved significantly in all patients at the last follow?up compared with preoperative (SF?36 PCS: 36.88±10.89 vs 67.91±10.46,P＜0.05; SF?36 MCS:33.48±10.96 vs 60.19±8.10, P＜0.05; ODI: 46.00%±13.54% vs 23.40%±8.86%, P＜0.05). Conclusion Pelvic fixation utiliz?ing S2AI screw can provide substantial correction and patients'HRQoL can be improved significantly after surgery. Meanwhile, PI changed at the last follow?up in partial patient. The variation of PI may be associated with the preoperative sagittal malalignment.

Abstract:Objectives: To analyze the clinical characteristics and surgical results of posttraumatic kyphosis of the thoracolumbar segment. Methods: Thirty-four patients with posttraumatic kyphosis of the thoracolumbar segment underwent posterior osteotomy, kyphosis correction, and fixation at our hospital. The kyphosis apex was from T10 to L2. There were 15 male and 19 female patients. The mean age was 48.9 years. Full-spine standing radiographs were obtained before surgery, after surgery, and at follow-up. The sagittal spine balance status was evaluated, and the kyphotic angle, the lumbar lordotic angle, and the thoracic kyphotic angle were measured. The Frankel grading system for neurological functions and the Visual Analogue Scale for back pain were evaluated before surgery and at follow-up. The Oswestry Disability Index was applied for assessment of life quality and surgical results of patients. The Patient Satisfied Index (PSI) was also used for determination of surgical results. Patients were grouped according to the extent of kyphotic angle and the surgical technique. The difference in kyphosis correction and back pain between groups was observed before and after surgery. Relief from neurological symptoms was also observed. Results: The average kyphotic angle was 48.5 degrees, the average lumbar lordotic angle was 57.9 degrees, and the average thoracic kyphotic angle was 11.4 degrees before surgery. The sagittal spine balance was well maintained in 14 cases, and negative imbalance and positive imbalance were seen in 13 cases and 7 cases, respectively. There was significant difference in back pain between patients in the abnormal and normal groups of the lumbar lordotic angle. The average follow-up time after surgery was 32.1 months. The average kyphotic angle correction rate was 90.4% after surgery and 87.2% at follow-up. The average Oswestry Disability Index was 50.4% before surgery, which improved to 29.4% at follow-up, with an improvement rate of 41.5%. The Patient Satisfied Index result showed a total satisfactory rate of 94.1%. There were 16 cases of neurological deficit before surgery; 8 of them achieved various degrees of improvement after surgery. Conclusions: Surgical correction of posttraumatic kyphosis of the thoracolumbar segment can show good radiologic and clinical results with the appropriate procedure according to the extent of the kyphosis angle. Some patients with neurological deficit are still worthy of surgical treatment.
the eye of the beholder: A Qualitative Study of Mutual Obligations and Areas of Ambiguity in the Hospital-Physician Relationship

Abstract:Hospitals and physicians have been working together for years in providing specialized health services. However, Hospital-Physician Relationships are considered lukewarm at best. We build on psychological contract theory to develop an in-depth understanding of how physicians (N=15) and hospital executives (N=15) in Belgium experience and interpret obligations in their working relationship. Our analysis yielded a rich understanding of mutual obligations and areas of ambiguity. Two major themes emerged from the analysis. A distinction should be made between administrative obligations (adequate support and responsive decision making) and professional obligations (clinical excellence and physician autonomy). Two areas of ambiguity could be identified reflecting both dimensions. An economic trade-off exists in the day-to-day interaction and therefore views on how the way care should be organized differ. In addition, the extent to which medical decisions should take into account the corresponding impact on hospital finance varies.

Abstract:Objective:To evaluate the effectiveness of SRS-Schwab grade Ⅳ osteotomy combined with satellite rod for thoracolumbar old osteoporotic fracture with severe kyphosis. Methods:Between April 2013 and August 2016, 20 cases of thoracolumbar old osteoporotic fracture with severe kyphosis were treated with SRS-Schwab grade Ⅳ osteotomy combined with satellite rod. All the patients were females, aged 49-71 years (mean, 54.8 years). The disease duration was 6-28 months with an average of 14 months. The T value of bone density was -4.4 to -1.8 (mean, -2.8). The preoperative Cobb angle was (43.0±11.3)°. The vertebral compression fracture segment was T 12 in 9 cases, L 1 in 8 cases, and L 2 in 3 cases. Preoperative spinal cord function was evaluated by Frankel classification; there were 5 cases of grade D and 15 cases of grade E. The operation time, intraoperative blood loss, and perioperative complication were recorded. The Cobb angle for kyphosis and sagittal vertical axis (SVA) were recorded beforeoperation, at 3 months after operation, and at last follow-up. Oswestry disability index (ODI) was used to evaluate the effectiveness before operation and at last follow-up, and the evaluation indicators included pain degree, daily life self-care ability, extracting, walking, sitting, standing, sleeping, social activities, and traveling. Results:The operation time was 180-314 minutes (mean, 226 minutes). The intraoperative blood loss was 390-1 800 mL (mean, 750 mL). All the incisions healed by first intension without incision infection. Twenty patients were followed up 24-52 months, with an average of 30.9 months. During the follow-up period, no significant complication such as correction loss, nail breakage, rod breakage, pseudoarthrosis formation, or proximal and distal junctional kyphosis occurred. All patients were able to walk upright after operation, and the pain relieved significantly at 6 months after operation. Bone fusion achieved at 12 months after operation. The Frankel grade of nerve function improved from grade D to grade E at last follow-up in 5 patients with nerve damage before operation. At last follow-up, the indicator scores of ODI significantly improved when compared with preoperative values ( P<0.05). Cobb angle significantly improved at 3 months after operation and at last follow-up ( P<0.05) when compared with preoperative one, but there was no significant difference in the Cobb angles between 3 months after operation and last follow-up ( P>0.05). There was no significant difference in SVA between pre- and post-operation ( P>0.05). Conclusion:SRS-Schwab grade Ⅳ osteotomy combined with satellite rod for thoracolumbar old osteoporotic fracture with severe kyphosis is effective in achieving satisfactory clinical outcomes, as well as maintaining correction of kyphosis.
[Surgical Correction of Post-Traumatic Kyphosis of Thoracolumbar Spine].

Abstract:OBJECTIVE To determine the effectiveness of surgical correction for post-traumatic kyphosis of thoracolumbar spine. METHODS From 1996 to 2003, 33 consecutive patients with post-traumatic kyphosis of thoracolumbar spine were corrected surgically. The mean age was 40.3 years (range, 13 - 65 years). The mean time between the initial injury and surgical correction was 36.0 months (range, 6 - 220 months). The kyphotic deformity averaged 40.8 degrees (range, 20 degrees - 82 degrees ). All the patients had neurological deficits. Twelve patients had obvious back pain. Seven patients lost sphincter function completely and nineteen patients lost the function partly. Twenty-three patients had ever undergone laminectomy and/or instrumentation. The treatment procedure consisted of anterior release and posterior spinal osteotomy with instrumentation (15 patients), posterior closing wedge osteotomy with instrumentation (12 patients), anterior release and instrumentation (6 patients). RESULTS Kyphosis was corrected from an average of 40.8 degrees to an average of 5.7 degrees, the corrective rate was 86.0% (40.8 degrees - 5.7 degrees /40.8 degrees). There were no severe complications. The average follow-up period was 24.6 months (range, 6 - 84 months). There was no loss of correction at follow-up. Ten of these patients showed an improvement in neural function by one or two levels according to the classification. Sphincter function recovered partly in ten patients. Back pain was relieved significantly in all of twelve patients with back pain preoperatively. Bony fusion was achieved in thirty-two patients. One patient had nonunion and achieved bony fusion after revision. CONCLUSION Posterior closing wedge osteotomy was suitable to kyphosis less than 40 degrees. Anterior release and posterior spinal osteotomy was effective, especially to the patients with severe kyphosis deformity or with operation history. Patients with incomplete neurological deficits and/or severe back pain could get benefit from osteotomy of spine, even if their medical history was long.
Impact of Cervical Range of Motion on the Global Spinal Alignment in Ankylosing Spondylitis Patients with Thoracolumbar Kyphosis Following Pedicle Subtraction Osteotomy.

Abstract:BACKGROUND CONTEXT: The head's center of gravity (COG) plumb line (PL) and C7 PL could be simultaneously positioned over the pelvis in adult spinal deformity with normal cervical mobility. However, the position of the head in relation to the global spinal alignment has yet to be investigated in ankylosing spondylitis (AS) patients with thoracolumbar kyphosis. PURPOSE: The objective of this study was to analyze the position of head in relation to the global spinal alignment in AS-related thoracolumbar kyphosis. STUDY DESIGN/SETTING: Retrospective single-center study. PATIENT SAMPLE: AS patients who underwent lumbar pedicle subtraction osteotomy for thoracolumbar kyphosis from January 2010 to August 2016 were reviewed. Only patients with a visible ear canal on the preoperative, immediate postoperative, and final follow-up radiographs were included. OUTCOME MEASURES: The chin-brow angle, cervical range of motion (ROM), lumbar lordosis (LL), thoracolumbar angle, thoracic kyphosis (TK), L1 pelvic angle, pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), TK+LL+PI, PI-LL, maximal kyphosis (MK), deformity angular rate, T1 pelvic angle, T1 tilt, spinosacral angle, sagittal vertical axis (SVA) COG-C7, SVA COG-femoral head (FH), and SVA C7-S were evaluated. Data regarding the health-related quality of life, including the Oswestry disability index (ODI) and visual analogue scale (VAS)-back score, were also collected preoperatively, 2 years postoperatively, and at the latest follow-up. METHODS: The cohort was divided into patients with occiput-trunk (OT) concordance or with OT discordance according to the SVA COG-C7 <= 30 mm or >30 mm, respectively. There was no funding in this study and there are no conflict of interest-associated biases. RESULTS: A total of 43 patients (36 males and 7 females) with a mean age of 34.2 years (range, 18- 59 years) were identified. There were 17 patients accompanied with OT concordance and 26 patients with OT discordance preoperatively. The cervical ROM was significantly lower (24.0 degrees vs 56.1 degrees, p<.001) and SVA COG-C7 was significantly larger (71.7 mm vs 7.4 mm, p<.001) in patients with OT discordance. Furthermore, the PT was larger (41.0 degrees vs 33.5 degrees, p=.010) in patients with OT discordance. After surgery, the whole cohort showed an improvement in LL (-8.6 degrees vs -52.8 degrees, p<.001). Moreover, the CBVA (25.4 degrees vs 1.3 degrees, p<.001) and SVA COG-C7 (46.2 mm vs 21.6 mm, p<.001) were significantly decreased following lumbar PSO. There were 13 patients accompanied with OT discordance postoperatively, and the cervical ROM was still lower (22.5 degrees vs 42.8 degrees, p=.024) in these patients. Postoperative PT was larger (26.5 degrees vs 20.1 degrees, p=.033) in the patients with OT discordance. At the latest follow-up, there were 17 patients accompanied with OT discordance. In these 17 patients, the cervical ROM was significantly lower (21.0 degrees vs 47.0 degrees, p=.001) and PT was significantly higher (26.2 degrees vs 19.2 degrees, p=.012). The ODI and VAS-back scores demonstrated no significant differences between the two groups preoperatively, 2 years postoperatively, or at the latest follow-up. CONCLUSIONS: OT discordance in AS-related thoracolumbar kyphosis could be caused by the reduced cervical ROM. To maintain global spinal balance, the pelvis rotated further backward in response to the larger SVA COG-C7. Moreover, the larger SVA COG-C7 could be decreased after the lumbar PSO. Although there were radiographic differences between the patients with OT concordance and with OT discordance, there was no difference in clinical outcomes, and that a larger sample size and longer follow-up are needed. Abstract:Objective: To evaluate the effectiveness of Scoliosis Research Society (SRS)-Schwab grade Ⅳ osteotomy in the treatment of post-traumatic thoracolumbar kyphosis (PTK). Methods: From October 2012 to January 2015, a total of 31 patients [12 males, 19 females, mean age (43±10) years] with symptomatic PTK undergoing SRS-Schwab grade Ⅳ osteotomy in Nanjing Drum Tower Hospital were retrospectively reviewed.Radiographic changes were evaluated with preoperative, postoperative and follow-up X-ray.The measurements included thoracic kyphosis (TK), lumbar lordosis (LL), thoracolumbar kyphosis (TLK), focal kyphosis (FK), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS) and sagittal vertical axis (SVA). Clinical outcome was assessed using the visual analogue scale (VAS), the Oswestry disability index (ODI) and Frankel neurological grade before surgery and during follow-up.The preoperative, postoperative and follow-up data were compared with paired t test. Results: All patients were followed-up for 28±5 months (24-38 months). The TK and LL improved significantly postoperatively from 15.6°±7.8° and 67.3°±12.2° to 28.1°±6.2° and 48.1°±9.3°(t=-6.985, 6.968, both P<0.05), and maintained at 27.3°±6.9° and 49.4°±7.9° at last follow-up, respectively.The TLK and FK improved significantly postoperatively from 29.4°±12.2° and 36.4°±9.5° to 7.7°±5.3° and 5.6°±4.2°(t=9.083, 16.510, both P<0.05), with a correction loss of 2.4°±2.0° and 2.3°±2.2° at final follow-up, respectively.No significant difference was observed in the PI, PT, SS and SVA among preoperative, post-operative and the last follow-up data.In addition, the VAS and ODI improved significantly at the final follow-up.Four cases with Frankel D experienced complete neurological recovery at the final follow-up.Radiographic evaluation showed solid bony fusion.No instrument-related complication was observed during the follow up. Conclusions: The SRS-Schwab grade Ⅳ osteotomy brings satisfactory sagittal alignment and good clinical outcomes in patients with PTK.
Clinical and Radiographic Evaluation of Posterior Surgical Correction for the Treatment of Moderate to Severe Post-Tuberculosis Kyphosis in 36 Cases with a Minimum 2-Year Follow-Up.

Abstract:OBJECT:The object of this study was to compare the clinical and radiographic outcomes of 36 patients with posttuberculosis kyphosis who underwent one of two types of osteotomy.METHODS:Each patient underwent single-stage correction via a posterior surgical approach. A modified pedicle subtraction osteotomy (mPSO) was performed when the kyphotic deformity was less than 70° (7 cases), whereas a posterior vertebral column resection (VCR) was performed when the kyphotic deformity exceeded 70° (29 cases). Full-length standing radiographs were obtained before surgery and at follow-up visits. These images were used to measure the kyphosis angle; sagittal alignment of the lumbar, thoracic, and cervical regions; and sagittal balance of the spine. Back pain was rated using the visual analog scale (VAS), and neurological function was classified based on the American Spinal Injury Association (ASIA) grading system. Each patient's overall satisfaction with surgical treatment was measured with the Patient Satisfaction Index. For purposes of comparison, patients were studied in 2 groups based on the region of their kyphotic apex. Half of the cohort had apical kyphosis in the lower thoracic spine or thoracolumbar junction (TL group). Using both radiographic and clinical assessments, the authors compared this group with the other half of the patients who had apical kyphosis in the upper to mid thoracic spine (MT group).RESULTS:The cohort included 15 males and 21 females, with an average age of 34 years at the time of surgery. The minimum follow-up was 24 months, and the mean follow-up was 31 months. Following surgery, kyphosis across the treated segments was reduced by an average of 60°. Lumbar lordosis also improved by an average of 24°, and thoracic kyphosis improved by an average of 20°. Both back pain and neurological function improved after surgical treatment. There was a 67% improvement in VAS scores, and 13 of the 36 patients had improvement in their ASIA grade. The 2 surgical procedures used for deformity correction (mPSO and VCR) demonstrated comparable radiographic and clinical results. Note, however, that differences were found in both radiographic and clinical outcomes in comparing patients who had lower thoracic or thoracolumbar (TL group) versus upper to midthoracic (MT group) apical kyphosis.CONCLUSIONS:Posterior tubercular kyphosis can be effectively improved through corrective surgery, and deformity correction can be accompanied by improvement in clinical symptoms. When appropriately selected, both the mPSO and the VCR can be expected to yield satisfactory reduction of post-tuberculosis kyphotic deformities. Differences in radiographic and clinical outcomes should be anticipated, however, when treating such deformities in different regions of the spine.
Pedicle subtraction osteotomy and disc resection with cage placement in post-traumatic thoracolumbar kyphosis, a retrospective study

Abstract:BackgroundIt is estimated that upwards of 50,000 individuals suffer traumatic fracture of the spine each year, and the instability of the fractured vertebra and/or the local deformity results in pain and, if kyphosis increases, neurological impairment can occur. There is a significant controversy over the ideal management. The purpose of the study is to present clinical and radiographic results of pedicle subtraction osteotomy and disc resection with cage placement in correcting post-traumatic thoracolumbar kyphosis.MethodsFrom May 2010 to May 2013, 46 consecutive patients experiencing post-traumatic thoracolumbar kyphosis underwent the technique of one-stage pedicle subtraction osteotomy and disc resection with cage placement and long-segment fixation. Pelvic incidence (PI), pelvic tilt (PT), sagittal vertical axis (SVA), and sagittal Cobb angle were measured to evaluate the sagittal balance. Oswestry disability index (ODI), visual analog scale (VAS), and general complications were recorded.ResultsThe average surgical time was 260 min (240–320 min). The mean intraoperative blood loss was 643 ml (400–1200 ml). The maximum correction angle was 58° with an average of 47°, and the SVA improved from +10.7 ± 3.5 cm (+7.2 to + 17.1 cm) to +4.1 ± 2.7 cm (+3.2 to + 7.6 cm) at final follow-up (p < 0.01). PT reduced from preoperative 27.2 ± 5.3° to postoperative 15.2 ± 4.7° (p < 0.01). The VAS changed from preoperative 7.8 ± 1.6 (5.0–9.0) to 3.2 ± 1.8 (2.0–5.0) (p < 0.01). Clinical symptoms and neurological function were significantly improved at the final follow-up. All patients completed follow-up of 41 months on average.ConclusionsPedicle subtraction osteotomy and disc resection with cage placement and long-segment fixation are effective and safe methods to treat thoracolumbar post-traumatic kyphosis.

Abstract:Objective:To evaluate the outcome of combined anterior and posterior approaches for angular kyphosis of thoracolumbar spine.Method:From 1998 to 2003,18 patients with kyphosis of thoracolumbar spine were corrected surgically.The mean age was 34.0 years(range,13~56 years).There were 14 patients with neurological deficits.Seven patients had obvious back pain.Eight patients were with sphincter dysfunction.Eleven patients had ever undergone laminectomy and/or instrumentation.The treatment procedure was combined anterior and posterior approaches.Result:Kyphosis was corrected from an average of 54.8°(range,30°~138°) to an average of 14.7°(-5°~75°),the corrective rate was 70.6%(41.7%~112.5%).Two patients had nonunion of anterior graft and required revision surgery,and achieved bony fusion afterward.The average follow-up period was 24.8 months(range,7~37 months).There were no severe complications.There was no missing of correction at the follow-up.Five of these patients showed an improvement in neural function.Sphincter function recovered partly in three patients.Back pain was relieved significantly in all seven patients.Bony fusion was achieved in all patients.Conclusion:Osteo-relaxation and internal-fixation through combined anterior and posterior approaches could correct kyphosis of thoracolumbar spine effectively,especially in patients with severe kyphosis deformity or with previous operation.
[Clinical Outcomes of Surgical Correction for Ankylosing Spondylitic Kyphosis].

Abstract:OBJECTIVE:To evaluate the clinical outcomes of posterior surgical corrective methods for ankylosing spondylitic kyphosis.METHODS:From June 2003 to June 2008, 21 cases of ankylosing spondylitic kyphosis received posterior surgical correction. There were 17 male and 4 female, and the average age was 39.5 years (range, 20 to 57 years). The total spine X-ray and CT were used to evaluate sagittal balance and thoracolumbar spine kyphosis angle, and chin brow-vertical angle was obtained from clinical lateral photograph. The surgical goal was to correct sagittal imbalance and chin brow-vertical angle. The simulated osteotomy was performed in computer before surgery to determine the correction methods. The surgical methods included: 16 cases of monosegmental closing osteotomy correction, 3 cases of anterior opening-posterior closing osteotomy correction, and 2 cases of combined pedicle subtraction osteotomy in thoracolumbar spine and Smith-Peterson osteotomy in lumbar spine. All patients were followed up after surgery, and the improvement of sagittal imbalance, chin brow-vertical angle and thoracolumbar spine kyphosis angle were assessed. The symptoms relief and satisfied rate were also evaluated.RESULTS:The average operation time was 4.4 hours, and the average blood loss was 1770 ml. Before surgery, the average thoracolumbar kyphosis angle was 62.1°, the average anterior shift of C(7) plumb line was 172.9 mm, and the average chin brow-vertical angle was 34.9°. The average follow-up was 28.8 months after surgery. The average correction rate of thoracolumbar kyphosis angle was 60%, the average improvement rate of anterior shift of C(7) plumb line was 64%, and the average correction rate of chin brow-vertical angle was 98%. The improvement rate of back pain was 64% during follow-up. The total surgical satisfactory rate was 95%.CONCLUSION:Based on the simulated osteotomy in computer before surgery, according to the characteristics of ankylosing spondylitic kyphosis, different posterior osteotomy and correction methods can achieve good results.

Abstract:BACKGROUND CONTEXT: Radiographic realignment objectives for the surgical correction of adult spinal deformity (ASD) have been well-described. However, the optimal sagittal spinopelvic alignment after corrective osteotomy for thoracolumbar kyphosis secondary to ankylosing spondylitis (AS) is still unknown so far. PURPOSE: To comprehensively investigate the relationship between radiographic parameters and clinical outcomes measured by Oswestry Disability Index (ODI) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) in thoracolumbar kyphosis patients caused by AS following one-level pedicle subtraction osteotomy (PSO) and to determine the optimal sagittal alignment at a minimum of 2-year follow-up. STUDY DESIGN: A retrospective review of consecutive AS patients from one institution (2006-2016). PATIENT SAMPLE: One hundred AS-related thoracolumbar kyphosis patients who underwent one-level PSO with a minimum of 2-year follow-up (range, 2-10 years). OUTCOME MEASURES: Spinopelvic parameters at final follow-up (>= 2 years) included thoracic kyphosis, lumbar lordosis, osteotomized vertebra angle, pelvic tilt (PT), sacral slope (SS), pelvic incidence, sagittal vertical axis, spinosacral angle (SSA), T1 pelvic angle (TPA), spinopelvic angle (SPA), and global kyphosis. Clinical outcomes at final follow-up (>= 2 years) was evaluated by ODI and BASDAI, and ODI<20 was defined as good clinical outcome. METHODS: Pre- and postoperative radiographic and clinical outcomes were compared by paired samples t test. The correlation between the postoperative radiographic parameters and clinical outcomes was investigated by Pearson correlation analysis. Linear regression analysis was performed to construct predictive models of the clinically relevant radiographic parameters based on ODI scores and to determine the realignment goals. Multiple stepwise regression analysis was applied to figure out the major radiographic contributors of ODI score at the final follow-up. RESULTS: In total, 100 AS patients (92 males and 8 females) with a mean age of 34.7 +/- 9.5 years (range, 17-63 years) were followed up for 38.6 +/- 17.5 months (range, 24-120 months). At the final follow-up, PT and TPA were positively correlated with both ODI and BASDAI score (p<.05). Although SS, SSA, and SPA were negatively associated with the score of ODI (p<.05), and BASDAI was negatively related to SPA (p<.05). Predicting regression models of the clinically-relevant radiographic parameters were built based on the ODI score at the last follow-up. CONCLUSIONS: Based on the regression models, the optimal sagittal alignment of AS patients satisfying good clinical outcome (ODI<20) at a minimum of 2-year follow-up was: PT<24 degrees, SSA>108 degrees, TPA<22 degrees, and SPA>152 degrees. Realizing the aforementioned realignment goals may contribute to satisfied clinical outcome for AS patients with thoracolumbar kyphosis undergoing one-level PSO. (C) 2019 Elsevier Inc. Two-Level Osteotomy for the Corrective Surgery of Severe Kyphosis from Ankylosing Spondylitis A Retrospective Series

Abstract:STUDY DESIGN:Retrospective study.OBJECTIVE:To describe the treatment results of patients with severe ankylosing spondylitis (AS) kyphosis who underwent two-level osteotomy and correction surgery.SUMMARY OF BACKGROUND DATA:The best solution for the fixed kyphotic deformity of AS is corrective osteotomy. Many osteotomy options are available: pedicle subtraction osteotomy (PSO), Smith-Peterson osteotomy (SPO), and vertical column resection (VCR). These procedures all provide multiplanar deformity correction. Nevertheless, when the AS deformity is severe, an additional osteotomy site to get more correction, achieve more ideal, and smooth curvature of the spine than single osteotomy.METHODS:From May, 2008 to August, 2016, 19 patients of severe AS kyphosis underwent two-level spinal osteotomy and correction surgery. The patients had an average kyphosis angle greater than 90°. The radiological features and clinical evaluation, including Oswestry Disability Index (ODI), visual analog scale (VAS), and Scoliosis Research Society (SRS)-22 components were assessed before surgery and at follow-up. The patients underwent either one-level PSO combined with one-level SPO (n = 9) or two-level PSO (n = 10).RESULTS:The height was increased after surgery in all patients (P < 0.05). The median follow-up was 24 months. The kyphosis angle improved from 92.0 ± 16.6° to 30.0 ± 17.2°. The chin-brow vertical angle improved from 37.6 ± 19.2° to -0.6 ± 2.5°. The sacral slope improved from 3.9 ± 11.8° to 21.7 ± 7.4°. Sagittal imbalance improved from 241.4 ± 115.3 mm to 74.6 ± 48.5 mm (P < 0.05). Lumbar lordosis improved from -3.9 ± 20.8° to 29.4 ± 14.1° (all P < 0.05). There were significant improvements in the ODI, VAS, and all components of SRS-22 (all P < 0.05). All patients with pseudarthrosis (n = 5) underwent PSO + SPO and achieved satisfactory results. Six complications were observed perioperatively, but without permanent sequelae.CONCLUSION:Two-level osteotomy and correction procedure can achieve satisfactory results in severe AS kyphosis. PSO + SPO could be a good option for patients with pseudarthrosis because of relatively easier and faster operation.LEVEL OF EVIDENCE:4.
Efficacy of posterior staged correction in the treatment of severe kyphoscoliosis

Abstract:Objective:To analyze the efficacy of posterior staged correction in the treatment of severe kyphoscoliosis.Methods:Retrospective analysis was conducted on 61 patients with severe kyphoscoliosis who underwent one-stage posterior Ponte osteotomy followed by Halo-femoral traction and two-stage posterior correction in Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School between January 2010 and January 2020. There were 23 males and 38 females with an average age of 22.0(18.0, 25.5) years. The etiologies were idiopathic in 26 cases, congenital in 17 cases, neuromuscular in 16 cases and Marfan syndrome with kyphoscoliosis in 2 cases. The curves were thoracic in 49 cases, thoracolumbar in 3 cases and double major in 9 cases. The apical vertebrae were T 5 level in 1 case, T 7 level in 2 cases, T 8 level in 9 cases, T 9 level in 15 cases, T 10 level in 23 cases, T 11 level in 8 cases, T 12 level in 1 case, and L 1 level in 2 cases. The flexibility of main curve was 13.5%±8.6%. The Cobb angle of main curve, global kyphosis (GK), coronal trunk shift (CTS), sagittal vertical axis (SVA), thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS) were assessed at pre-operation, post-traction, post-operation and the last follow-up. The quality of life was evaluated using the MOS item short form health survey (SF)-36 questionnaire, and the complications during peri-operation and long-term follow-up were recorded. Results:All 61 patients were followed up for 25.0 (24.0, 26.5) months. The Cobb angle of main curve and GK were 121.4°±13.9° and 86.8°±20.0° at pre-operation, 94.1°±18.7° and 66.9°±15.3° at post-traction, 78.5°±20.3° and 54.7°±13.6° at post-operation and 79.5°±20.1° and 53.2°±11.3° at the last follow-up, respectively. The differences were statistically significant ( F=210.54, P<0.001; F=93.74, P<0.001). There was no significant difference between the last follow-up and post-operation ( P>0.05). There was no significant correction loss of SVA, TK, LL, PI, PT or SS at the last follow-up when compared with those at post-operation ( P>0.05). The CTS was 17.1±9.8 mm at pre-operation, 17.5±11.4 mm at post-operation, 11.1 (5.9, 23.3) mm at the last follow-up and the difference was statistically significant (χ 2=6.70, P=0.035). The difference between the last follow-up and post-operation was statistically significant ( P=0.032). The scores of physical functioning 80.0 (75.0, 85.0), general health 82.0 (69.5, 87.0) and social functioning 75.0 (62.5, 75.0) in SF-36 at the last follow-up were significantly improved compared with those at pre-operation ( Z=-2.11, P=0.035; Z=-3.64, P<0.001; Z=-2.07, P=0.039). During the traction process, the complications included pin track infection in 1 case, deep vein thrombosis of the lower extremity in 2, misplacement of pedicle screws in 3, coronal decompensation at immediate post-operation in 2, sagittal decompensation at immediate post-operation in 1, and 1 patient had broken rod at 3 years follow-up, respectively. Conclusion:The posterior staged correction could provide satisfying radiographic and clinical outcomes in patients with severe kyphoscoliosis, which could be well maintained during 2 years follow-up. Therefore, the posterior staged correction is a safe and effective treatment for severe kyphoscoliosis.

Abstract:OBJECTIVE:To observe the results of posterior osteotomy and correction in the surgical treatment of old tuberculous kyphosis.METHODS:From June 2004 to December 2008, 31 cases of old tuberculous kyphosis with posterior osteotomy and correction technique were treated. There were 12 cases of male and 19 cases of female. The average age was 33.4 years. Pedicle subtraction osteotomy or vertebral column resection were applied in surgery. The kyphosis angle, lumbar lordosis angle and sagittal balance condition of the spine were measured before and after surgery, as well as follow-up. The Frankel grading system for neurological function of lower extremities, the Oswestry disability index (ODI) for life quality, and patient satisfactory index (PSI) for satisfaction of surgery were applied before surgery and at follow-up.RESULTS:The average kyphosis angle was 94° ± 27°, the average lumbar lordosis angle was 71° ± 20°, and the average sagittal C(7) plumb line was (-15 ± 44) mm away from the balance region before surgery. The average kyphosis angle decreased to 26° ± 11° in one week after surgery, with an improvement rate of 71.4%. The average follow-up time was 22.5 months. The average kyphosis angle was 28° ± 12° at the final follow-up, with an improvement rate of 70.0%. The average lumbar lordosis angle was 46° ± 11°, with an improvement rate of 35.1%. The postoperative kyphosis angle and lumbar lordosis angle were significantly different with that of pre-operation (for kyphosis angle: t = 16.3, P < 0.05; for lumbar lordosis angle: t = 8.1, P < 0.05). The average sagittal C(7) plumb line was (-4 ± 22) mm away from the balance region at the final follow-up, with an improvement rate of 73.4%. The Frankel grading were E in 13 cases, D in 13 cases, and C in 5 cases before surgery, and were E in 20 cases, D in 8 cases, and C in 3 cases at the final follow-up. The average ODI was 13 ± 12 before surgery, and was 7 ± 8 at the final follow-up, with an improvement rate of 45.2%. The PSI results showed a satisfied rate of 90.3%.CONCLUSION:Good results can be achieved by applying proper posterior osteotomy and correction technique according to the severity of old tuberculous kyphosis.
SRS-Schwab Grade 4 Osteotomy for Congenital Thoracolumbar Kyphosis: a Minimum of 2 Years Follow-Up Study.

Abstract:BACKGROUND CONTEXT: Several osteotomy techniques including pedicle subtraction osteotomy and vertebral column resection have been employed in the correction of congenital kyphosis (CK) and satisfying outcomes have been demonstrated. However. the Scoliosis Research Society (SRS)Schwab Grade 4 osteotomy, defined as resection of posterior elements, partial vertebral body, and superior adjacent disc, is rarely reported in the treatment of CK. PURPOSE: The present study aimed to evaluate the efficiency and safety of SRS-Schwab Grade 4 osteotomy in patients with CK, and to propose its optimal indication. STUDY DESIGN: This is a retrospective analysis of clinical and radiographic outcomes of patients with CK undergoing SRS-Schwab Grade 4 osteotomy. PATIENTS SAMPLE: Patients with thoracolumbar CK undergoing SRS-Schwab Grade 4 osteotomy from January 2010 to May 2015 followed up for at least 2 years were retrospectively reviewed. OUTCOME MEASURES: The thoracic kyphosis, lumbar lordosis, segmental kyphosis (SK). sagittal vertical axis (SVA), pelvic incidence, pelvic tilt, and sacral slope were measured on lateral spinal x-rays. Patients were required to fulfill the SRS-22 questionnaire at preoperation and the last follow-up. MATERIALS AND METHODS: The sagittal spinal-pelvic parameters were assessed at preoperation, postoperation, and last follow-up. The comparison between preoperation and postoperation was performed by paired samples t test. RESULTS: A total of 38 patients with CK (17 male and 21 female) with an average age of 16.5 +/- 9.9 years were included. The mean operating time was 242.7 +/- 88.1 minutes and blood loss was 634.5 +/- 177.8 mL. The mean follow-up was 38.8 +/- 203 months. The SK was 49.5 +/- 11.7 degrees at preoperation, 6.8 +/- 7.4 degrees at postoperation (p<.001), and 8.0 +/- 8.1 degrees at the last follow-up. No significant correction loss during follow-up was found in SK (p=.125). The SVA was improved from -36.0 +/- 18.3 mm at preoperation to 3.7 +/- 17.8 mm at postoperation (p<001), and the correction was well maintained during follow-up (p=.113). Compared with preoperation. the mean postoperative scores of each domains of SRS-22 questionnaire improved at different levels. Intraoperative complications included one case with incidental dural tear and another with transient root injury. Proximal junctional kyphosis occurred in three patients at 6 months after operation and remained stable during the follow-up. At the last follow-up, all patients were identified to have achieved solid bony fusion. CONCLUSIONS: The SRS-Schwab Grade 4 osteotomy, if selected appropriately, could provide satisfying correction of congenital kyphosis. [Posterior spinal transpedicular wedge osteotomy for kyphosis due to delayed osteoporotic vertebral fracture in elderly]

Abstract:Objective: To evaluate the clinical effects of posterior spinal transpedicular wedge osteotomy for kyphosis due to delayed osteoporotic vertebral fracture in elderly. Methods: From July 2009 to February 2014,26 patients with kyphosis caused by delayed osteoporotic vertebral fracture were treated with transpedicular wedge osteotomy. There were 10 males and 16 females,aged from 55 to 75 years old with an average of 67 years. There were 1 osteotomy in thoracic vertebra,21 osteotomies in thoracolumbar vertebrae and 4 in lumbar vertebrae. Total 29 vertebrae were involved, 23 cases with single vertebral fracture and 3 cases with double vertebral fractures. Preoperative Cobb angles were 32°~51° with the mean of (42.00 ± 4.75) ° and VAS scores were 6 to 9 points with an average of (8.40 ± 0.75) points. According to the Frankel grade of spinal cord function, 4 cases were grade D and 22 cases were grade E. Intraoperative bleeding, operation time and perioperative complications were recorded, and improvements of Cobb angle were evaluated by X-rays. VAS score and Frankel grade were respectively used to evaluate the pain and nerve function. Results: The average operation time were 155 min (ranged, 120 to 175) and the mean intraoperative bleeding were 1 100 ml (ranged,800 to 1 500). Postoperative at 2 days, Cobb angle and VAS score were (9.60 ± 2.50) ° and (4.00 ± 1.00) points, respectively, ranged from 5° to 15° and 1 to 5 points. VAS score and Cobb angle improved obviously compared with preoperative (P < 0.05), and the improvement rate of Cobb angle was 76%. Frankel grade of 1 case changed from grade E to C, and the others did not become worse. The follow-up period ranged from 3 to 24 months with an average of 16.4 months. At the final follow-up, Cobb angles and VAS score were (11.00 ± 3.50)° and (4.40 ± 1.25) points, respectively, ranged from 5° to 19° and 1 to 6 points. The patient whose Frankel grade E changed to C at 2 days after surgery and changed to grade D at the latest follow-up. Vertebral body fracture below the fusion level happened in 1 case at 3 months after surgery, vertebral body fracture above the fusion level happened in 1 case at 5 months after surgery, and their chest pain symptoms were relieved after symptomatic treatment and anti osteoporosis treatment. All osteotomy levels obtained fusion which confirmed by X-ray and no internal fixation loosening and breakage were found. Conclusion: The clinical effect of posterior transpedicular wedge osteotomy for kyphosis due to delayed osteoporotic vertebral fracture was satisfactory, but manipulation during the operation should be cautious and prevent adjacent vertebral body fracture should be pay attention to prevent.
A Retrospective Study to Compare the Efficacy of Preoperative Halo-Gravity Traction and Postoperative Halo-Femoral Traction after Posterior Spinal Release in Corrective Surgery for Severe Kyphoscoliosis.

Abstract:BACKGROUND This retrospective clinical study aimed to compare the efficacy of preoperative halo-gravity traction with postoperative halo-femoral traction after posterior spinal release in corrective surgery for patients with severe kyphoscoliosis. MATERIAL AND METHODS A retrospective clinical study included patients who underwent elective corrective surgery for severe kyphoscoliosis (N=60) between 2013 and 2015. Two patient groups were compared, the postoperative halo-femoral traction after posterior spinal release (R-HF) group (N=30) and the preoperative halo-gravity traction (HGT) group (N=30). Demographic and clinicopathological data included age, gender, Cobb angle, degree of spinal curvature, history of osteotomy, and etiological factors. Patients in the two study groups were matched. Postoperative surgical outcome was evaluated by the radiographic coronal Cobb angle, global kyphosis, coronal balance, and the sagittal vertical axis (SVA). Clinical outcome was assessed using the Scoliosis Research Society Outcomes Questionnaire (SRS-22). RESULTS The preoperative Cobb angle was similar between the R+HF group and the HGT group (123.5±12.7° vs. 123.1±14.1°; P=0.909). Following postoperative traction, a significantly higher correction rate was found in the R+HF group than the HGT group (31.8±7.8% vs. 19.3±12.9%; P=0.001). The postoperative correction rate in the R+HF group was significantly higher than the HGT group (44.7±7.8% vs. 39.0±12.8%; P=0.042). In both study groups, the postoperative SRS-22 scores were significantly improved with no statistical difference between the two groups, and no neurological complications occurred.
