Abstract:A 43-year-old woman was scheduled to undergo cardiac pacemaker implantation because of sick sinus syndrome. Following percutaneous access of the left subclavian vein, the guide wire traveled straight across to the right subclavian vein, and could not be turned acutely downward into the left persistent superior vena cava despite multiple attempts with lead reshaping. Injection of contrast medium showed the persistent left superior vena cava and absent right superior vena cava (Figure 1). A right subclavian approach was used to accomplish a bipolar lead with active fixation (Medtronic 5076-58, Minneapolis, MN) implantation in the right atrium, via the coronary sinus (Figure 2). The RV lead could not enter the right ventricle. A singlechamber pacing system (AAI) was selected, due to the normal atrioventricular conduction. Abstract:Background:Cardiac resynchronization therapy (CRT) has been shown to benefit patients with heart failure and left bundle branch block (LBBB). However, CRT implantation is challenging when the superior venous access is not feasible. Case summary:A 50-year-old man with a history of dilated cardiomyopathy and complete LBBB was referred to our hospital for CRT management. Angiography showed that the left and right brachiocephalic veins were occluded. Cardiac resynchronization therapy was finally implanted via the iliac vein. Follow-up echocardiography showed improved cardiac function, and the pacing system was functioning properly. Discussion:The iliac vein access is feasible for CRT implantation with good stability, which can be a viable alternative to avoid unnecessary risk associated with thoracotomy and epicardial lead placement.
The Feasibility and Safety of Endomyocardial Biopsy by Lumenless Pacing Lead-Sheath Directed Method During the Cardiac Implantable Electronic Device Implantation

Abstract:Background Patients with cardiovascular implantable electronic devices (CIEDs) indication are complicated with special cardiomyopathy or other unspecified cardiac abnormalities and may need endomyocardial biopsy (EMB). However, EMB by a bioptome is usually avoided to reduce the risk of lead displacement in the CIED peri-procedural period. Objective We aimed to assess the safety and feasibility of a novel approach for transvenous right ventricular (RV) EMB using the lead-sheath method (L-S-M) during CIED implantation and compared it to the traditional bioptome method (T-B-M). Methods Consecutive eighty patients referred for EMB were enrolled. In the L-S-M group, a sheath with a lumenless pacing lead was positioned toward the middle to apical interventricular septum under fluoroscopy (n=60). The CIED implantation was performed through the same venous access site. In the T-B-M group, a bioptome was used (n=20). The clinical characteristics, procedural details, success rate and complications were evaluated. Results 380 RV EMBs procedures were performed with comparable 4.1±0.8 in the L-S-M group and 3.8±0.8 samples/patient in the T-B-M group. In the L-S-M group, seven (11.7%) patients experienced minor complications with 3 transient right bundle branch block, 2 transient atrioventricular block (AVB) and 2 regional minor device pocket hematomas. In the T-B-M group, three (15%) experienced one temporary pacing for transient AVB, one chest discomfort and one regional hematoma. No cardiac tamponade was detected. Conclusions RV EMB by the innovative L-S-M method is technically feasible, safe and can yield valuable and early diagnostic insights for patients who are candidates for CIEDs.

Abstract:This single-center study delves into the novel use of the internal jugular vein (IJ) as an alternative access pathway for the implantation of the CardioMEMS device, a wireless pulmonary artery pressure sensor. This approach is compared to the conventional femoral vein access in terms of procedural times, contrast volume, and patient outcomes. The study spans five years, from 2018 to 2023, evaluating the efficacy and safety of IJ access for CardioMEMS implantation. The analysis revealed a tendency towards shorter procedural times and reduced contrast volume with IJ access, although statistical significance was not reached due to the small sample size. Notably, IJ access provided a quicker needle-to-door time, suggesting benefits such as improved patient throughput and potentially shorter hospital stays, which are pivotal in a healthcare landscape that prioritizes efficiency and cost-effectiveness. In conclusion, this investigation offers initial evidence that IJ vein access is a feasible and potentially superior alternative for CardioMEMS device implantation. While the findings are preliminary due to the study's limited scale, they pave the way for future larger-scale, multicenter studies. Such research could confirm the benefits of IJ access and possibly influence clinical practices, enhancing the management of heart failure patients.

Abstract:Isolated noncompaction of the ventricular myocardium is an unclassified cardiomyopathy and is thought to be due to arrest of myocardial morphogenesis. Fetal arrhythmias may occur in approximately half of the patients and account for the death in this disorder. We describe a patient with isolated noncompaction of the right ventricular myocardium in whom implantation of biventricular pacemaker was thought to be effective to prevent the risk of sudden cardiac death and complications.

Abstract:Abstract Background Cardiac interventions may be challenging in patients with congenital cardiac abnormalities. This case reports cardiac resynchronisation therapy pacemaker (CRT-P) implantation, and single catheter ablation of atrioventricular (AV) node with remote magnetic navigation (RMN) via peripheral vascular access in a patient with Kartagener ́s syndrome and permanent atrial fibrillation (AF). Case summary A 74-year-old male with situs inversus presented for treatment of permanent AF and severe heart failure. In echocardiography left ventricular ejection fraction was 30% and there was severe dyskinesia due to a left bundle branch block. After successful CRT-P implantation, we performed atrioventricular node (AVN) ablation because biventricular pacing (BiV) was < 75% despite maximal rate control medication. The ablation catheter was inserted from the right basilic vein, and no other catheters were used. Despite peripheral vascular access manipulation of the ablation catheter with RMN was easy and the ablation was successful. After the ablation, BiV pacing instantly increased to 100%, and left ventricular function and symptomatic status improved gradually. Conclusions CRT-P implantation and RMN-guided single catheter ablation of the AVN in a patient with dextrocardia via peripheral vascular access was effective and safe. The use of RMN and peripheral vascular access may offer important advantages also in other patient groups.

Abstract:This paper reviews and updates the single moving dipole (SMD) and two moving dipole (TMD) inverse electrocardiographic and electroencephalographic solutions. These inverse solutions are particularly appropriate when the electrical activity of the heart or brain may be represented by one or two well-localized foci. They attempt to match the measured body surface or evoked scalp potentials to potentials generated on the surface of a model of the intervening volume conductor by one or two moving current dipoles, respectively. The two alternative methods of solution are discussed initially. The first is a direct least-squares error match of measured and model-generated surface potentials, the second an indirect solution based on the least-squares error match of the potentials due to equivalent multipole series representations of the real and model sources, respectively. Next, brief reviews of moving dipole inverse solutions in the EEG and ECG fields are presented. Simulation studies, as well as experimental and clinical studies in animals and humans, are described. The Discussion section summarizes the optimum solution approaches that should be used in clinical EEG and ECG studies in man. It also cautions against the temptation to translate the numerical adequacy of inverse SMD and TMD solutions into physiological validity, without independent knowledge as to the nature of the underlying sources.
A Next Step in Minimizing the Invasiveness of Transcatheter Aortic Valve Replacement

Abstract:Background The femoral vein is commonly used as a pacemaker access site during transcatheter aortic valve replacement (TAVR). Using an upper arm vein as an alternative access site potentially causes fewer bleeding complications and shorter time to mobilization. We aimed to assess the safety and efficacy of an upper arm vein as a temporary pacemaker access site during TAVR. Methods We evaluated all patients undergoing TAVR in our center between January 2020 and January 2023. Upper arm, femoral, and jugular vein pacemaker access was used in 255 (45.8%), 191 (34.3%), and 111 (19.9%) patients, respectively. Clinical outcomes were analyzed according to pacemaker access in the overall population and in a propensity-matched population involving 165 upper arm and 165 femoral vein patients. Primary endpoint was Bleeding Academic Research Consortium (BARC) type 2, 3, or 5 pacemaker access site-related bleeding. Results In the overall population, primary endpoint was lowest for upper arm, followed by femoral and jugular vein access (2.4% vs. 5.8% vs. 10.8%, p = 0.003). Time to mobilization was significantly longer (p < 0.001) in the jugular cohort compared with the other cohorts. In the propensity-matched cohort, primary endpoint showed a trend toward lower occurrence in the upper arm compared with the femoral cohort (2.4% vs. 6.1%, p = 0.10). Time to mobilization was significantly shorter (480 vs. 1140 min, p < 0.001) in the upper arm cohort, with a comparable skin-to-skin time (83 vs. 85 min, p = 0.75). Cross-over from upper arm pacemaker access was required in 17 patients (6.3% of attempted cases via an upper arm vein). Conclusions Using an upper arm vein as a temporary pacemaker access site is safe and feasible. Its use might be associated with fewer bleeding complications and shorter time to mobilization compared with the femoral vein.

Abstract:Abstract Background The Persistent Left (PL) Superior Vena Cava (SVC) is a congenital malformation of the thoracic venous return that has an incidence of 0.2-3% in the general population and an incidence of about 0.5% in patients undergoing cardiac implantable electronic device. In this case, the placement of the leads, especially for the Right Ventricle (RV) and Left Ventricle (LV) through one of the branches could be challenging. Methods A 66-year-old man with hypertension, diabetes and dyslipidemia, in optimal pharmacologic therapy for congestive heart failure (NYHA II), hospitalized for recurrent syncopal episodes, with extreme sinus bradycardia, first-degree AV block and Left Bundle Branch Block, The patient had a LV hypertrophy and mild global systolic function (LV ejection fraction of 40%) for these we opted for a pacemaker CRT device implantation. Results Isolation of left cephalic vein was used as venous access for right atrium (RA) and RV leads while Seldinger technique via subclavian vein was used as venous access for LV lead. The guidewire through the subclavian vein pointed out the presence of PLSVC. Left subclavian venogram confirmed PLSVC and revealed a bridging innominate vein. To pass through the innominate vein and reach the SVC, a 0.035-inch hydrophilic guidewire was used. A 6Fr-sheath was advanced through the hydrophilic guidewire, subsequently a second metallic guidewire was advanced through the same introducer by removing the dilator. The RV and RA leads were placed respectively on low right interventricular septum and RA appendage passing through bridging innominate vein. Electrical parameters were normal. For the placement of the LV lead a 9.5Fr sheath introducer was advanced over the guidewire through the PLSVC. A venogram showed a lateral and a postero-lateral branch of the coronary sinus with acute angulation. With a 6Fr quadripolar Josephson catheter the postero-lateral branch was entered, eventually a 9Fr peelable outer guide catheter was advanced through the electrophysiology catheter becoming selective with the interested branch. A quadripolar S shaped lead was used, it was advanced into the targeted vein through a 0,014fr hydrophilic guidewire. Adequate electrical parameters and stability were achieved. Conclusion Lead implantation in patient with PLSVC is challenging. Placing the RA and RV leads through SVC ensure a better stability along time. The cannulation of CS is easily achieved by approach via PLSVC while selective vein cannulation can be difficult due to the modified acute angulation.

Abstract:Leadless pacemakers offer the opportunity to avoid transvenous hardware among patients with tricuspid valve prostheses. We present the first case of a helix-based fixation leadless pacemaker implanted through valve-in-valve tricuspid prostheses in a 43-year-old female with extensive prior cardiac history. At the time of presentation, epicardial pacing was no longer a viable option in the setting of pacemaker dependence. Placement of a helix-fixed, leadless right ventricular pacemaker was performed as a bridge to dual-chamber leadless pacing. This was safely and effectively performed and highlighted favorable procedural characteristics that included RV cavity dimensions and prosthesis type.

Safety and Efficiency of Cephalic Vein Puncture by Modified Seldinger Technique Compared to Subclavian Vein Puncture for Cardiac Implantable Electronic Devices

Abstract:Cephalic vein puncture has been described as novel access for CIED implantation, promising high success rates and simplified handling. Implantation of at least one lead via CVP was successful in 90% of the patients. Total implantation and fluoroscopy times did not differ between cephalic and subclavian implantations. Pneumothorax only occurred after subclavian puncture. Introduction The establishment of venous access is one of the driving factors for complications during implantation of pacemakers and defibrillators (cardiac implantable electronic devices [CIED]). Recently, a novel approach of accessing the cephalic vein for CIED by cephalic vein puncture (CVP) using a modified Seldinger technique has been described, promising high success rates and simplified handling with steeper learning curves. In this single‐center registry, we analyzed the safety and efficiency of CVP to SVP access after defining CVP as the primary access route in our center. Methods A total of 229 consecutive patients receiving a CIED were included in the registry. Sixty‐one patients were implanted by primary or bail‐out SVP; 168 patients received primary cephalic preparation and CVP was performed when possible, using a hydrophilic transradial sheath. Results Implantation of at least one lead via CVP was successful in 151 of 168 patients (90%), and implantation of all leads was possible in 122 of 168 patients (72.6%). Total implantation times and fluoroscopy times and doses did not differ between CVP and SVP implantations. Pneumothorax occurred in 0/122 patients implanted via CVP alone, but 8/107 (7.5%) patients received at least one lead via SVP. Conclusion Our data confirms high success rates of the CVP for CIED implantation. Moreover, this method can be used without significantly prolonging the total procedure time or applying fluoroscopy dose compared to the highly efficient SVP while showing lower overall complication rates.

Abstract:We reviewed the different approaches used for central vein access during insertion of cardiac implantable electronic devices. The benefits and hazards of each approach (cephalic vein cutdown, axillary vein cannulation using venography and ultrasound) are discussed. Each approach has its advantages and hazards that need to be considered for the individual patient and balanced against the skills of the operator. The benefits of ultrasound guided venous access in reducing radiation exposure to the patient and implanter, avoiding the need for angiographic contrast and in minimizing the risk of pneumothorax and inadvertent arterial puncture are highlighted. Trainees should be taught each approach to deal with patient variability. Ultrasound guidance should be considered as a mainstream option for most patients.

Abstract:This study compared the diagnosis and treatment of white American psychiatric patients to those of black, Mexican, Vietnamese, and Filipino American psychiatric patients (N = 293) in four public mental health agencies. Aspects of diagnosis examined were primary psychiatric diagnosis, somatic complaints, and social, legal, and economic problems. Aspects of treatment studied were treatment modality, duration of treatment, frequency of treatment, number of visits, and therapists' disciplines. Although important differences among groups emerged, the pronounced differences in diagnosis and treatment reported in the literature between whites and ethnic minorities were not supported. This might be partially attributed to the similarity of economic class among these samples.

Abstract:To date, several techniques for transseptal left ventricular (LV) endocardial pacing for cardiac resynchronization therapy (CRT) have been proposed in patients for whom routine transvenous LV pacing is infeasible. These techniques are of varying difficulty and complexity, and some requires devices not easily accessible. Abstract:BACKGROUND: Atrial fibrillation is the most common sustained cardiac arrhythmia in adults and it is associated with a high burden of mortality and morbidity worldwide. Catheter ablation is increasingly used to improve symptoms and prognosis in selected patients. Lower limb venous access with subsequent transseptal approach to the left atrium is the standard procedure for atrial fibrillation catheter ablation.CASE PRESENTATION: We report an unusual case of complex venous anomaly with a left-sided inferior vena cava with hemiazygos continuation to a persistent left superior vena cava draining in an enlarged coronary sinus in a patient with persistent atrial fibrillation scheduled for transcatheter ablation.DISCUSSION: Lower limb venous anomalies may limit a standard transseptal approach to the left atrium thus precluding an effective catheter ablation procedure for atrial fibrillation. Alternative interventions, such as unconventional percutaneous access, thoracoscopic approach and "ablate and pace" procedures, may be necessary in patients with symptomatic atrial fibrillation and complex venous anomalies.
