Abstract:Nowadays, robots are increasingly used in various surgery applications. Meanwhile, in many of these applications, the operation tools of surgical robots need to be in contact with human soft tissues. Furthermore, this tool-soft tissue interaction brings great challenges to robot control and system performance due to the nonlinearity, viscoelasticity, and uncertainties of the soft environment. To address these challenges and achieve the desired interaction behavior, a learning-based force controller for a surgical robot, which consists of a feedforward plus feedback controller, a radial basis function neural network-based controller, and an adaptive proportional-integral-type sliding mode control-based compensator, is presented in this letter. To display the stability of the proposed controller, the control system of the robot is analyzed through the Lyapunov method. Finally, several experiments are carried out in the robot prototype and the results illustrate that good tracking performance and guaranteed robustness can be obtained by the proposed controller.
Towards Robotic Eye Surgery: Marker-free, Online Hand-eye Calibration using Optical Coherence Tomography Images

Abstract:Ophthalmic microsurgery is known to be a challenging operation, which requires very precise and dexterous manipulation. Image guided robot-assisted surgery (RAS) is a promising solution that brings significant improvements in outcomes and reduces the physical limitations of human surgeons. However, this technology must be further developed before it can be routinely used in clinics. One of the problems is the lack of proper calibration between the robotic manipulator and appropriate imaging device. In this work, we developed a flexible framework for hand-eye calibration of an ophthalmic robot with a microscope-integrated Optical Coherence Tomography (MIOCT) without any markers. The proposed method consists of three main steps: a) we estimate the OCT calibration parameters; b) with micro-scale displacements controlled by the robot, we detect and segment the needle tip in 3D-OCT volume; c) we find the transformation between the coordinate system of the OCT camera and the coordinate system of the robot. We verified the capability of our framework in ex-vivo pig eye experiments and compared the results with a reference method (marker-based). In all experiments, our method showed a small difference from the marker based method, with a mean calibration error of 9.2 $\mu$m and 7.0 $\mu$m, respectively. Additionally, the noise test shows the robustness of the proposed method.

Abstract:Robot-assist laparoscopic surgery has been widely acknowledged in the minimally invasive surgery for its capability of operating within limited space and its robustness. Particularly, the vision-guided camera-holder system is suggested worldwide for it could track the instruments autonomously without additional equipment and its low cost, in which computer vision contributes a lot to the recognition and localization of the surgical instruments. This paper proposes vision-based real-time tracking of three basic surgical instruments in laparoscopic surgery. Two algorithms in computer vision are compared for instruments detection - Template Matching and Deep Learning, and the real-time tracking is completed based on the detection. Optimizations are made for both of algorithms. For Template Matching, three methods to enhance efficiency are applied. For Deep Learning, different loss functions and Attention Mechanisms are introduced and compared. Models are evaluated by the metrics of Precision, Recall and Detecting Time. The results reveal that despite the high precision of 0.83, the detection time of Template Matching reaches above 5000 ms, which is too slow to meet the real-time requirements in the surgery, while YOLOv5 models with CIoU and SENet performs best overall with the precision of 0.98 and the detection time of around 20 ms.

Abstract:In laparoscopic surgeries, intelligent-assisted endoscope-holding robots (AEHRs) with traditional servo control have limited performance due to the lack of endoscopic attitude control, hand–eye coordination, and image definition. This article proposes an endoscope field-of-view (FOV) autonomous tracking method for robot-assisted surgery (RAS) considering pose (i.e., position and attitude) control, hand–eye coordination, and image definition, and it can realize that the AEHR system can safely, stably, and quickly track the movement of surgical instruments. First, the general framework of the intelligent tracking system with robot-assisted endoscope holding is constructed according to the clinical needs and shortcomings of traditional research. Second, the kinematic model and remote center of motion (RCM) constraint equations of AEHRs are established. Then, surgical instrument tips are segmented and localized using the deep learning method (DLM), and further, a surgical FOV intelligent adjustment control model with multiple indexes is developed. Finally, an experimental system based on the hybrid of AEHRs and speech interaction is built. The simulation and experimental results demonstrate that the proposed method can achieve fast, accurate, and effective instrument tips tracking (ITT), reduce hand–eye incoordination, and improve the safety and stability of FOV intelligent adjustment.

Abstract:Purpose: The knowledge of laparoscope vision can greatly improve the surgical operation room (OR) efficiency. For the vision-based computer-assisted surgery, the hand-eye calibration establishes the coordinate relationship between laparoscope and robot slave arm. While significant advances have been made for hand-eye calibration in recent years, efficient algorithm for minimally invasive surgical robot is still a major challenge. Removing the external calibration object in abdominal environment to estimate the hand-eye transformation is still a critical problem. Methods: We propose a novel hand-eye calibration algorithm to tackle the problem which relies purely on surgical instrument already in the operating scenario for robot-assisted minimally invasive surgery (RMIS). Our model is formed by the geometry information of the surgical instrument and the remote center-of-motion (RCM) constraint. We also enhance the algorithm with stereo laparoscope model. Results: Promising validation of synthetic simulation and experimental surgical robot system have been conducted to evaluate the proposed method. We report results that the proposed method can exhibit the hand-eye calibration without calibration object. Conclusion: Vision-based hand-eye calibration is developed. We demonstrate the feasibility to perform hand-eye calibration by taking advantage of the components of surgical robot system, leading to the efficiency of surgical OR.
Bimanual Manipulation of Steady Hand Eye Robots with Adaptive Sclera Force Control: Cooperative vs. Teleoperation Strategies

Abstract:Performing retinal vein cannulation (RVC) as a potential treatment for retinal vein occlusion (RVO) without the assistance of a surgical robotic system is very challenging to do safely. The main limitation is the physiological hand tremor of surgeons. Robot-assisted eye surgery technology may resolve the problems of hand tremors and fatigue and improve the safety and precision of RVC. The Steady-Hand Eye Robot (SHER) is an admittance-based robotic system that can filter out hand tremors and enables ophthalmologists to manipulate a surgical instrument inside the eye cooperatively. However, the admittance-based cooperative control mode does not safely minimize the contact force between the surgical instrument and the sclera to prevent tissue damage. Additionally, features like haptic feedback or hand motion scaling, which can improve the safety and precision of surgery, require a teleoperation control framework. This work presents a bimanual adaptive teleoperation (BMAT) control framework using SHER 2.0 and SHER 2.1 robotic systems. We integrate them with an adaptive force control (AFC) algorithm to automatically minimize the tool-sclera interaction force. The scleral forces are measured using two fiber Bragg grating (FBG)-based force-sensing tools. We compare the performance of the BMAT mode with a bimanual adaptive cooperative (BMAC) mode in a vessel-following experiment under a surgical microscope. Experimental results demonstrate the effectiveness of the proposed BMAT control framework in performing a safe bimanual telemanipulation of the eye without over-stretching it, even in the absence of registration between the two robots.

Abstract:A surgeon's physiological hand tremor can significantly impact the outcome of delicate and precise retinal surgery, such as retinal vein cannulation (RVC) and epiretinal membrane peeling. Robot-assisted eye surgery technology provides ophthalmologists with advanced capabilities such as hand tremor cancellation, hand motion scaling, and safety constraints that enable them to perform these otherwise challenging and high-risk surgeries with high precision and safety. Steady-Hand Eye Robot (SHER) with cooperative control mode can filter out surgeon's hand tremor, yet another important safety feature, that is, minimizing the contact force between the surgical instrument and sclera surface for avoiding tissue damage cannot be met in this control mode. Also, other capabilities, such as hand motion scaling and haptic feedback, require a teleoperation control framework. In this work, for the first time, we implemented a teleoperation control mode incorporated with an adaptive sclera force control algorithm using a PHANTOM Omni haptic device and a force-sensing surgical instrument equipped with Fiber Bragg Grating (FBG) sensors attached to the SHER 2.1 end-effector. This adaptive sclera force control algorithm allows the robot to dynamically minimize the tool-sclera contact force. Moreover, for the first time, we compared the performance of the proposed adaptive teleoperation mode with the cooperative mode by conducting a vessel-following experiment inside an eye phantom under a microscope.

A Force-Feedback Methodology for Teleoperated Suturing Task in Robotic-Assisted Minimally Invasive Surgery

Abstract:With robotic-assisted minimally invasive surgery (RAMIS), patients and surgeons benefit from a reduced incision size and dexterous instruments. However, current robotic surgery platforms lack haptic feedback, which is an essential element of safe operation. Moreover, teleportation control challenges make complex surgical tasks like suturing more time-consuming than those that use manual tools. This paper presents a new force-sensing instrument that semi-automates the suturing task and facilitates teleoperated robotic manipulation. In order to generate the ideal needle insertion trajectory and pass the needle through its curvature, the end-effector mechanism has a rotating degree of freedom. Impedance control was used to provide sensory information about needle-tissue interaction forces to the operator using an indirect force estimation approach based on data-based models. The operator's motion commands were then regulated using a hyperplanar virtual fixture (VF) designed to maintain the desired distance between the end-effector and tissue surface while avoiding unwanted contact. To construct the geometry of the VF, an optoelectronic sensor-based approach was developed. Based on the experimental investigation of the hyperplane VF methodology, improved needle-tissue interaction force, manipulation accuracy, and task completion times were demonstrated. Finally, experimental validation of the trained force estimation models and the perceived interaction forces by the user was conducted using online data, demonstrating the potential of the developed approach in improving task performance.
A "eye-in-body" integrated surgery robot system for stereotactic surgery

Abstract:Purpose: Current stereotactic surgical robots system relies on cumbersome operations such as calibration, tracking and registration to establish the accurate intraoperative coordinate transformation chain, which makes the system not easy to use. To overcome this problem, a novel stereotactic surgical robot system has been proposed and validated. Methods: First, a hand-eye integrated scheme is proposed to avoid the intraoperative calibration between robot arm and motion tracking system. Second, a special reference-tool-based patient registration and tracking method is developed to avoid intraoperative registration. Third, a model-free visual servo method is used to reduce the accuracy requirement of hand-eye relationship and robot kinematic model. Finally, a prototype of the system is constructed and performance tests and a pedicle screw drilling experiment are performed. Results: The results show that the proposed system has acceptable accuracy. The target positioning error in the plane was - 0.68 ± 0.52 mm and 0.06 ± 0.41 mm. The orientation error was 0.43 ± 0.25°. The pedicle screw drilling experiment shows that the system can complete accurate stereotactic surgery. Conclusions: The stereotactic surgical robot system described in this paper can perform stereotactic surgery without the intraoperative hand-eye calibration and nor manual registration and can achieve an acceptable position and orientation accuracy while tolerating the errors in the hand-eye coordinate transformation error and the robot kinematics model error.
Adaptive Control Improves Sclera Force Safety in Robot-Assisted Eye Surgery: A Clinical Study

Abstract:The integration of robotics into retinal microsurgery leads to a reduction in surgeon perception of tool-to-tissue interaction forces. This blunting of human tactile sensory input, which is due to the inflexible mass and large inertia of the robotic arm as compared to the milli-Newton scale of the interaction forces and fragile tissues during ophthalmic surgery, identifies a potential iatrogenic risk during robotic eye surgery. In this paper, we aim to evaluate two variants of an adaptive force control scheme implemented on the Steady-Hand Eye Robot (SHER) that are intended to mitigate the risk of unsafe scleral forces. The present study enrolled ten retina fellows and ophthalmology residents into a simulated procedure, which simply asked the trainees to follow retinal vessels in a model retina surgery environment. For this purpose, we have developed a force-sensing (equipped with Fiber Bragg Grating (FBG)) instrument to attach to the robot. A piezo-actuated linear stage for creating random lateral motions to the eyeball phantom has been provided to simulate disturbances during surgery. The SHER and all of its dependencies were set up in an operating room in the Wilmer Eye Institute at the Johns Hopkins Hospital. The clinicians conducted robot-assisted experiments with the adaptive controls incorporated as well as freehand manipulations. The results indicate that the Adaptive Norm Control (ANC) method, is able to maintain scleral forces at predetermined safe levels better than even freehand manipulations. Novice clinicians in robot training however, subjectively preferred freehand maneuvers over robotic manipulations. Clinician preferences once highly skilled with the robot is not assessed in this study.
Optimization-based Concurrent Control of a High Dexterity Robot for Vitreoretinal Surgery

Abstract:Vitreoretinal surgery requires dexterity and force sensitivity from the clinician. A system to cooperatively control an integrated surgical robot for high dexterity manipulation within the eye's vitreous space was developed and validated in simulation. The system is composed of a 2 degrees of freedom (DoF) snake-like continuum manipulator that is attached to the end-effector of a 5-DoF rigid robot arm. It is capable of receiving position and orientation commands from a 5-DoF input device in real-time, as well as following pre-planned trajectories. The manipulator is moved to each target pose in real-time, using an optimization method to calculate the inverse kinematics solution. Constraints on the position and orientation ensure the target pose does not harm the patient within the vitreous space, enabling the robot to safely assist the clinician with vitreoretinal surgery when operating in real-time. The simulation demonstrates the system's feasibility and benefits over the existing non-dexterous system.
Enhancing Precision in Tactile Internet-Enabled Remote Robotic Surgery: Kalman Filter Approach

Abstract:Accurately estimating the position of a patient's side robotic arm in real time in a remote surgery task is a significant challenge, particularly in Tactile Internet (TI) environments. This paper presents a Kalman Filter (KF) based computationally efficient position estimation method. The study also assume no prior knowledge of the dynamic system model of the robotic arm system. Instead, The JIGSAW dataset, which is a comprehensive collection of robotic surgical data, and the Master Tool Manipulator's (MTM) input are utilized to learn the system model using System Identification (SI) toolkit available in Matlab. We further investigate the effectiveness of KF to determine the position of the Patient Side Manipulator (PSM) under simulated network conditions that include delays, jitter, and packet loss. These conditions reflect the typical challenges encountered in real-world Tactile Internet applications. The results of the study highlight KF's resilience and effectiveness in achieving accurate state estimation despite network-induced uncertainties with over 90\% estimation accuracy.

Abstract:Robotic ophthalmic surgery is an emerging technology to facilitate high-precision interventions such as retina penetration in subretinal injection and removal of floating tissues in retinal detachment depending on the input imaging modalities such as microscopy and intraoperative OCT (iOCT). Although iOCT is explored to locate the needle tip within its range-limited ROI, it is still difficult to coordinate iOCT's motion with the needle, especially at the initial target-approaching stage. Meanwhile, due to 2D perspective projection and thus the loss of depth information, current image-based methods cannot effectively estimate the needle tip's trajectory towards both retinal and floating targets. To address this limitation, we propose to use the shadow positions of the target and the instrument tip to estimate their relative depth position and accordingly optimize the instrument tip's insertion trajectory until the tip approaches targets within iOCT's scanning area. Our method succeeds target approaching on a retina model, and achieves an average depth error of 0.0127 mm and 0.3473 mm for floating and retinal targets respectively in the surgical simulator without damaging the retina.

Real-to-Sim Registration of Deformable Soft Tissue with Position-Based Dynamics for Surgical Robot Autonomy

Abstract:Autonomy in robotic surgery is very challenging in unstructured environments, especially when interacting with deformable soft tissues. The main difficulty is to generate model-based control methods that account for deformation dynamics during tissue manipulation. Previous works in vision-based perception can capture the geometric changes within the scene, however, model-based controllers integrated with dynamic properties, a more accurate and safe approach, has not been studied before. Considering the mechanic coupling between the robot and the environment, it is crucial to develop a registered, simulated dynamical model. In this work, we propose an online, continuous, real-to-sim registration method to bridge 3D visual perception with position-based dynamics (PBD) modeling of tissues. The PBD method is employed to simulate soft tissue dynamics as well as rigid tool interactions for model-based control. Meanwhile, a vision-based strategy is used to generate 3D reconstructed point cloud surfaces based on real-world manipulation, so as to register and update the simulation. To verify this real-to-sim approach, tissue experiments have been conducted on the da Vinci Research Kit. Our real-to-sim approach successfully reduces registration error online, which is especially important for safety during autonomous control. Moreover, it achieves higher accuracy in occluded areas than fusion-based reconstruction.

A Parallel Robot With Remote Centre-of-Motion for Eye Surgery: Design, Kinematics, Prototype, and Experiments

Abstract:Background: Millions of patients suffering from eye disease cannot receive proper treatment due to the lack of qualified surgeons. Medical robots have the potential to solve this problem and have attracted significant attention in the research community. Method: This paper proposes a novel parallel robot with a remote centre of motion for minimally invasive eye surgery. Kinematics models, singularity and workspace analyses, and dimension optimisation are conducted. A prototype was developed, and experiments were conducted to test its mobility, accuracy, precision and stiffness. Results: The prototype robot can successfully perform the required motions, and has a precision ranging from 7 ± 2 μm to 30 ± 8 μm, accuracy from 21 ± 10 μm to 568 ± 374 μm, and stiffness ranging from 1.22 ± 0.39 N/mm to 10.53 ± 5.18 N/mm. Conclusion: The prototype robot has a great potential for performing the minimally invasive surgery. Its stiffness meets the design requirement, but its accuracy and precision need to be further improved.

Abstract:High precision is required for ophthalmic robotic systems. This paper presents the kinematic calibration for the delta robot which is part of the next generation of Steady-Hand Eye Robot (SHER). A linear error model is derived based on geometric error parameters. Two experiments with different ranges of workspace are conducted with laser sensors measuring displacement. The error parameters are identified and applied in the kinematics to compensate for modeling error. To achieve better accuracy, Bernstein polynomials are adopted to fit the error residuals after compensation. After the kinematic calibration process, the error residuals of the delta robot are reduced to satisfy the clinical requirements.

During such procedures, the sclera can potentially be injured from extreme tool-to-sclera contact force caused by surgeon's unintentional misoperations.We present an active interventional robotic system to prevent such iatrogenic accidents by enabling the robotic system to actively counteract the surgeon's possible unsafe operations in advance of their occurrence. Relying on a novel force sensing tool to measure and collect scleral forces, we construct a recurrent neural network with long short-term memory unit to oversee surgeon's operation and predict possible unsafe scleral forces up to the next 200 ms. We then apply a linear admittance control to actuate the robot to reduce the undesired scleral force. The system is implemented using an existing "steady hand" eye robot platform. The proposed method is evaluated on an artificial eye phantom by performing a "vessel following" mock retinal surgery operation.Empirical validation over multiple trials indicates that the proposed active interventional robotic system could help to reduce the number of unsafe manipulation events.We develop an active interventional robotic system to actively prevent surgeon's unsafe operations in retinal surgery. The result of the evaluation experiments shows that the proposed system can improve the surgeon's performance.</p>
