Abstract:Morita therapy is known as a psychotherapy grounded in the culture of Japan, particularly its Buddhist culture. Its popularity in Japan and other East Asian countries is cited as an example of the relevance and importance of culture and religion in psychotherapy. To complement such interpretations, this study adopts a socio-historical approach to examine the role and significance of work in Morita's theory and practice within the broader work environment and culture of the 1920s and 1930s in Japan. Morita conceptualized shinkeishitsu as a personality disease and a social illness caused by an alienating work environment. He proposed a remedy that emphasized the subjective emotional experience of work. To his primarily middle-class clients and readers, Morita's reconciliation between the self and society and that between autonomy and compliance was persuasive and useful, providing a philosophy whereby they could integrate into the work environment without loss of self-worth. The socio-historical character of Morita therapy is vital to understanding its power and appeal during Morita's time. Moreover, it sheds light on the complex interrelationships between work, mental health, and society.

Mitigate web phishing using site signatures

Abstract:Phishing is now a serious threat to the security of Internet users' confidential information. Basically, an attacker (phisher) tricks people into divulging sensitive information by sending fake messages to a large number of users at random. Unsuspecting users who follow the instruction in the messages are directed to well-built spoofed web pages and asked to provide sensitive information, which the phisher then steals. Statistics published by the anti-phishing working group (APWG) show that, at the end of Q2 in 2008, the number of malicious web pages designed to steal users' confidential information had increased by 258% over the same period in 2007. Therefore, protecting users from phishing attacks is extremely important. Existing anti-phishing solutions detect mimicked phishing pages by either text-based features or visual similarities of web pages. The former one can be bypassed using image based phishing attacks while the latter one may suffer from great variants of phishing pages. In this paper, we propose a novel technique that identify the real domain name of a visiting web page based on signatures created for web sites. Site signatures, including distinctive texts and images, can be systematically generated by analyzing common parts from pages of a web site. On matching a signature, the domain name of the visiting URL is checked first and then redirected if the domain name is unmatched. The result shows the proposed method achieves a high accuracy and low error rates.
Morita therapy for schizophrenia: An updated meta-analysis

Abstract:Morita therapy was developed for common mental problems, and our aim was to evaluate the clinical effect of Morita therapy on schizophrenia. The literature was searched in 10 databases, namely, PubMed, Chinese National Knowledge Infrastructure (CNKI), Sinomed, Wanfang, Cochrane Library, UpToDate, Web of Science, Medline, PsycINFO and Embase, from inception to September 4, 2019. Random-effects models were used. For continuous results, the standardized mean differences (SMDs) and 95% confidence intervals (CIs) were calculated to synthesize the effects. Thirty studies were included, with a total of 2651 patients with schizophrenia. Compared to pharmacotherapy alone and standard care alone, Morita therapy plus pharmacotherapy and Morita therapy plus standard care both had significant effects on mental state (pooled effect size = -1.09, 95% CI: -0.35, -0.83), social functioning (pooled effect size = -0.61, 95% CI: -2.30, -0.92) and behavior (pooled effect size = 1.13, 95% CI: 0.75, 1.51). Significant heterogeneity between studies was found for mental state (I2 = 89%, p < 0.05) and social functioning (I2 = 95%, p < 0.05), but no heterogeneity was found for behavior (I2 = 0%, p = 0.84). Morita therapy has positive effects on mental state and social functioning among patients with schizophrenia, but it leads to some problems with behavior among these patients. Most included studies have unclear bias, and the forest plots show high heterogeneity among the results. Thus, Morita therapy cannot be implemented in clinical practice as a feasible strategy, the conclusion has yet to be confirmed, and new trials and future studies are desired.

Searching for Meaning with Victor Frankl and Walker Percy

Abstract:Patients present to physicians searching for more than scientific names to call their maladies. They rather enter examination rooms with value-laden narratives of illness, suffering, hopes, and worries. One potentially helpful paradigm, inspired in part by existentialism, is to see patients on a search for meaning. This perspective is particularly important in the seemingly meaningless ruins of modernity. Here, we will summarize Victor Frankl's account of logotherapy found in his much-circulated book Man's Search for Meaning and assess the limitations imposed by his religious agnosticism. At best he can offer patients a finite, impersonal meaning this side of the grave. Following Kierkegaard's depiction of the religious sphere of existence, American novelist Walker Percy will be shown to supplement logotherapy with a theological mooring. The spiritual crisis of the modern world is treatable only by Christian faith supplying ultimate meaning. Taken together, Frankl and Percy show how Catholic physicians can be guides in their patients' personal searches for meaning. This paradigm may prove chiefly beneficial in goals of care conversations, encountering "aesthetic" patients living only for pleasure, and engaging patients amidst tragedy-ridden circumstances. Although only Christian faith will ultimately satisfy the search for meaning, we first of all need encouragement to take responsibility for seeking meaning, and confidence that even the most hopeless situation can become meaningful. Summary: Victor Frankl's Man's Search for Meaning can enlighten clinical encounters for physicians to see patients on a search for meaning, particularly amidst suffering and tragedy in a post-modern world lacking transcendence. As shown in Walker Percy's literature, however, ultimate meaning can only be found in Christian faith where the Word became flesh and continues to dwell among us.

Abstract:Abstract Meaning in life is an important topic for professional training and social work education. This article aims to illustrate how Viktor Frankl’s logotherapy could be applied as the framework in teaching meaning in life to social work students and helping professionals. It critically examines the experience of teaching meaning in life and integrating it with various training and education components, such as human behaviours, end-of-life and bereavement care, and self-care of helping professionals. Reflections on these teaching experiences suggest that the significance of teaching meaning in life is recognised by helping professionals and students, especially its dual influence on both personal and professional aspects. Lack of systematic and continuous training and education of meaning in life may be a hindrance for more in-depth understanding and learning. More case examples and discussion may help advance helping professionals’ practice to facilitate service users’ meaning searching. Reflecting on meaning in life may also facilitate helping professionals’ coping with existential challenges in work, but such self-care training should be offered regularly to provide continuous support. This article suggests that teaching meaning in life may have great implications for the psychological health of helping professionals and the service users who receive their professional services.

Abstract:Background Meaning in life is a benchmark indicator of flourishing that can likely mitigate the severity of depression symptoms among persons seeking mental healthcare. However, patients contending with serious mental health difficulties often experience a painful void or absence of ultimate meaning in their lives that might hinder recovery. This two‐wave longitudinal study examined temporal associations between perceived presence of meaning in life, struggles with ultimate meaning, flourishing, and depression symptoms among adults in a spiritually integrated inpatient treatment program. Methods Of the 242 patients assessed at intake, 90% (N = 218; 40% Cisgender male; 57% Cisgender female; 3.0% nonbinary) completed validated measures of these meaning‐related factors and mental health outcomes at discharge. Results Cross‐sectional analyses revealed perceptions of meaning in life and ultimate meaning struggles were inversely linked with one another along with being associated with indices of positive and negative mental health in varying ways at the start and end of treatment. Drawing upon a two‐wave cross‐lagged panel design, longitudinal structural equation modeling analyses supported a Primary Meaning Model whereby having a subjective sense of meaning in life at intake was prospectively linked with lower levels of ultimate meaning struggles and greater flourishing at discharge. However, baseline levels of mental health outcomes were not predictive of the meaning‐related factors in this sample. Conclusion Findings highlight the utility of assessing patients' perceived meaning in life and ultimate meaning struggles in spiritually integrated programs and for clinicians to be prepared to possibly address these meaning‐related concerns in the treatment process.

Abstract:Introduction Irvin D. Yalom defines existential psychotherapy as a dynamic therapeutic approach that focuses on concerns rooted in existence with the four ultimate concerns being death, isolation, meaning in life, and freedom. Patients in advanced stages of cancer often experience elevated levels of psychological distress, encompassing conditions such as depression, anxiety, and a sense of spiritual hopelessness. Recently, interest in spiritual well-being has prompted a new wave of interventions that directly target this population, namely logotherapy and other existential interventions based on existential principles. Objectives In this review, the primary focus was to comprehend the current evidence on the application of existential psychotherapy for individuals coping with advanced cancer and give an overview of the therapy approaches used. Methods Narrative review of scientific literature using Pubmed search engine. Results Terao and Satoh identified nine types of existential psychotherapies which were investigated using randomized controlled trials for patients with advanced cancer or in terminal care: Meaning-Centered Group Psychotherapy (MCGP), Individual Meaning-Centered Psychotherapy (IMCP), Meaning-Making intervention (MMi), Meaning of Life Intervention, Managing Cancer and Living Meaningfully (CALM), Hope Intervention, Cognitive and Existential Intervention, Dignity Therapy, and Life-Review Interviews. All deal with the issues pointed by Yalom. Existential or spiritual well-being improvements were validated in MCGP, IMCP, Meaning of Life intervention, and Life-Review intervention. Conclusions Current evidence is still based on a very limited number of studies. Abstract:The aim of this integrative literature review was to identify high quality empirical research and theoretical literature on the sources of meaning in life and people's beliefs regarding the meaning of life. This will inform current mental health clinical practice and research by providing a synthesis of empirical and theoretical literature. Failure to address meaninglessness or the existential crisis can lead to psychopathologies such as depression, anxiety, addiction, aggression, hopelessness, apathy, lower levels of well-being, physical illness, and suicide. Integrative literature reviews incorporate empirical research and theoretical literature. The inclusion criteria were primary research and theoretical papers and books by prominent theorists. Thirty-nine items underwent the critical appraisal process. Thirty-two papers or books were included. Overwhelmingly the results revealed that relationships, particularly relationships with family, are cited as the most important source of meaning in people's lives in all cultures and age groups. There was no consensus identified to answer the meaning of life question. These results and future research will allow mental health clinicians to help patients deepen their understanding of themselves, identify where they find meaning and understand their beliefs about meaning of life, contributing to a reduction in symptomatology and meaninglessness, and an increase in happiness, life satisfaction, positive affect, better coping, psychosocial health and well-being, and more meaningfulness in life.

Abstract:Current approaches to the treatment of non-affective psychosis include elements of mentalization-based treatment and the potential in enhancing mentalizing capacity in this patient group has been widely emphasized. This article presents the "psychotic identity dilemma", a concept by Stavros Mentzos, and a therapeutic approach considering this concept as a valuable complementary addition to these treatments. The idea of a dilemma between closeness and distance, which in itself cannot be represented mentally at first, helps to respond to specific needs of patients with psychotic disorders by placing the treatment focus on fundamental interpersonal processes. Following this train of thought, this article attempts to shed light on the importance of the "real relationship" between therapist and patient as well as the exploration of the "here and now", especially at the beginning of psychotherapeutic treatment. Two treatment modes are suggested, one characterized by the experience of interpersonality within the therapeutic relationship and a second one characterized by the reflection of interpersonal phenomena. These modes are framed by Stavros Mentzos' concept of an identity dilemma. We describe how mentalizing first needs to be addressed implicitly in a tolerable, exemplary relationship in which closeness and distance are regulated based on the therapist's countertransference, then explicitly. A series of interventions are described, which allow for moments of shared attention, promote intentionality and contingency and, later in the course of therapy, help to integrate experiences into narratives.

Abstract:Lichtenberg's chapter on "Empathy, Motivational Systems, and a Theory of Cure" was a brilliant effort to integrate his new paradigm of multimotivational systems with Kohut's empathic approach to psychotherapy. Lichtenberg shows how the clinical dialogue with patients can be monitored by using empathic perception to track shifts in motivational systems. At any given moment, a particular motivational system may become dominant. Successfully identifying the dominant motivation through empathic perception creates in patients a feeling of being understood without judgment. This ambience of safety allows for a shift that activates an exploratory motivation with patients and a spirit of inquiry. An observational platform is created where experience from the past with attachment figures and interaction in the here and now in therapy can be explored from new perspectives. Recent research in psychotherapy that uses monitoring the clinical dialogue with patients by tracking shifts in motivational systems supports the view of its positive effects. Lichtenberg's "theory of cure" is based on the premise that an empathic perception that monitors shifts in motivational systems will mobilize intrinsic self-righting capacities in patients. I note that there is controversy about what mutative factors in psychotherapy are most important: building an ambience of safety and trust or creating new insights into personal and internal conflicts through the use of language and symbolic representations. Both factors are crucial for positive changes in psychotherapy, and their importance will vary from patient to patient and from one patient–therapist relation to another. I note that Wilma Bucci's multiple code theory that sees experience coded at subsymbolic and verbal symbolic levels supports this view. I close by proposing that we can build on Lichtenberg's multimotivational system by adding a trans-survival need to give meaning to our lives.

Creating critical palliative hypnotic adjustments: temporality, hope, and meaning

Abstract:When cure is not possible, suffering often takes form as pain and distressing symptoms, death anxiety, existential distress, and meaninglessness. This paper describes important elements connecting palliative care principles with hypnotic approaches designed to provide support, palliate symptoms, foster hope, and address existential and spiritual distress. We offer a developmental process for and examples of hypnotic suggestions customized to simultaneously ameliorate physical symptoms and address profound distress arising from physical, social, psychological, existential, and spiritual challenges commonly encountered in terminal illness. This process necessarily requires use of the patient's vernacular to hypnotically deepen inwardly focused attention in order to explore and access internal resources, reframe negative automatic thoughts, and create positive meanings for experiences that disinvite suffering. Effective delivery utilizes cognitive tools such as clinical and scientific principles, artistic forms such as poetry and haiku, and a thorough assessment of needs. This approach strategically addresses an overarching dimension of temporality through suggestions that sequentially address multiple sources of suffering that are layered throughout the various dimensions of self. This requires focus and presence in the present moment; it ultimately fosters a therapeutic relationship that can safely hold past painful experience as helpful new meanings emerge that build resiliency for that experience. This work benefits from inwardly focused concentration and a holding environment to identify and access helpful inner resources, which include an increasingly malleable relationship with temporal memories.

Abstract:Expatriates experiencing emotional distress and a call for globally oriented psychotherapy receive an increased focus in the research agendas. That one may better understand how expatriates may be helped in times of distress, the insight in their actual psychotherapy experience may serve as a valuable avenue. The aim of this qualitative study was to illuminate the lived experience of psychotherapy and the meaning that expatriates attributed to these experiences within their expatriate context. Semi-structured interviews were utilized for the data collection and Interpretative Phenomenological Analysis (IPA) was employed for data analysis. The following themes emerged from the expatriates’ narratives about their psychotherapy experience: “The recognition of the expatriate complexity,” “Personal growth vs Dependency,” “Endurance vs Change,” “The globally minded therapist,” and “Language makes or breaks.” The overall common psychotherapy experience was expatriates’ considerable need to get the recognition of their expatriate complexity in a global context. Findings are discussed in relation to the existing expatriate and multicultural counselling literature taking into account the importance of cultural aspects in mental health treatment. The current study presents a unique and important contribution in the field of expatriate mental health as it highlights the psychotherapy experience that can be valuable for professionals in various settings offering psychological support.

Abstract:The interviews of 10 female counseling psychology doctoral students regarding their views about meaning in life (MIL) were analyzed using consensual qualitative research. The participants defined MIL as goals or purposes, were actively involved in searching for meaning, and believed that MIL had been stimulated by life-changing experiences and culture. On a personal level, they gained meaning from relationships, personal growth, and religion/spirituality. On a professional level, they gained meaning from providing therapy, conducting research, and teaching. As therapists, participants approached MIL indirectly by asking about client goals/motivations or by focusing on other clinical problems that if resolved would enhance MIL. Implications for doctoral training and for psychotherapy are offered.
